Hepatitis E Virus (HEV) in Heavy Pigs in Slaughterhouses of Northern Italy: Investigation of Seroprevalence, Viraemia, and Faecal Shedding.

Hepatitis E virus (HEV) is considered an emerging threat in Europe, owing to the increased number of human cases and the widespread presence of the virus in pigs at farms. Most cases in industrialized countries are caused by the zoonotic HEV-3 genotype. The main transmission route of HEV-3 in Europe is foodborne, through consumption of raw or undercooked liver pork and wild boar meat. Pigs become susceptible to HEV infection after the loss of maternal immunity, and the majority of adult pigs test positive for IgG anti-HEV antibodies. Nonetheless, HEV-infected pigs in terms of liver, faeces, and rarely blood are identified at slaughterhouses. The present study aimed to investigate the prevalence of HEV-positive batches of Italian heavy pigs at slaughterhouses, assessing the presence of animals still shedding HEV upon their arrival at the slaughterhouse by sampling faeces collected from the floor of the trucks used for their transport. The occurrence of viraemic animals and the seroprevalence of anti-HEV antibodies were also assessed. The results obtained indicated the presence of anti-HEV IgM (1.9%), and a high seroprevalence of anti-HEV total antibodies (IgG, IgM, IgA; 89.2%, n = 260). HEV RNA was not detected in either plasma or faecal samples. Nevertheless, seropositive animals were identified in all eight batches investigated, confirming the widespread exposure of pigs to HEV at both individual and farm levels. Future studies are needed to assess the factors associated with the risk of HEV presence on farms, with the aim to prevent virus introduction and spread within farms, thereby eliminating the risk at slaughterhouse.

Prevalence of Toxoplasma gondii, Hepatitis E Virus, and Salmonella Antibodies in Meat Juice Samples from Pigs at Slaughter in Switzerland

Toxoplasma gondii, hepatitis E virus (HEV), and Salmonella are zoonotic foodborne pathogens that may be transmitted to humans through the consumption of raw or undercooked pork. The aim of this study was to determine the seroprevalence of anti-T. gondii, anti-HEV, and anti-Salmonella antibodies from healthy pigs at slaughter in Switzerland. From August to September 2020, the diaphragm muscle of Swiss fattening pigs was collected in three Swiss abattoirs from 188 farms. Two randomly chosen pig carcasses per farm were selected. On the basis of the slaughter data, we noted the production system and the canton of origin, comparing indoor (n = 120) and free-range (n = 68) farming and regional allocation. The meat juice of these samples was analyzed for pathogen-specific antibodies by using commercial enzyme-linked immunosorbent assay kits. The seroprevalences were 1.3% for T. gondii, 71.8% for the HEV, and 5.3% for Salmonella, respectively. Comparing the origins, the results of many cantons were not meaningful due to the low number of samples. No regional accumulations were found for T. gondii and HEV. The results showed that 2.1% of the farms had least one T. gondii-seropositive animal, 80.3% had at least one HEV-seropositive animal, and 8.5% had at least one Salmonella-seropositive animal, respectively. The seropositivity of T. gondii was higher in free-range pigs than in indoor pigs, whereas anti-Salmonella antibodies were more common in pigs from indoor farming than in outdoor pigs. The seroprevalence of anti-HEV antibodies was similar in free-range and indoor farming pigs. Compared with studies from 2012, the seroprevalence of T. gondii has decreased, whereas the seroprevalence of the HEV has increased and is highly prevalent among fattening pigs in Switzerland. The low seroprevalence of Salmonella has remained stable in recent years.

Serologic evidence of the circulation of the hepatitis E virus and the prevalence of antibodies against hepatitis A in an indigenous population in northern Argentina

In 2005 a universal vaccination program against hepatitis A was introduced in Argentina. Nevertheless, there are still some unvaccinated marginal population groups. There are no data about the seroprevalence of hepatitis E in the northern region of Argentina mainly because of lack of awareness of this emergent pathogen. We aimed to determine the seroprevalence of hepatitis A, and hepatitis E in an indigenous population in northern Argentina. One hundred and twenty six (126) donor serum samples collected near San Salvador de Jujuy were analyzed for anti-HAV IgG and HEV IgG and IgM, alkaline phosphatase and transaminase values. Volunteers were interviewed about their living conditions, animal farming, consumption of tap water or river water, and level of education. Seroprevalence of specific anti-HAV antibodies was high (80.2%, 95% confidence interval, 72.1–86.7%) in children under 5 years of age, indicating early infection in life. Seroprevalence of anti-HEV antibodies was 5.6% (95% CI: 2.3–11.2%), being slightly higher than the values found in healthy patients from other regions of the country. Although we could not characterize the genotype of the circulating HEV strain, the clear epidemiological difference between seroprevalence of HAV and HEV in a community with poor sanitary conditions suggest that the circulating HEV strains spread through a different transmission route than HAV. Furthermore a significant correlation between anti-HEV IgG and swine farming was found (p<0.05), which supports a zoonotic transmission path. We reassessed the epidemiological pattern of HAV infection and reported evidence of HEV infection for the first-time in a community belonging to the Guarani ethnic group, highlighting the need to include hepatitis E testing in routine diagnostics in the region.

Development of ELISA test kits for semi-quantitative detection of anti-HEV antibodies in pigs in the Republic of Belarus

Viral hepatitis E infection affects up to 80-100 % of domestic pigs worldwide and is characterized by high seroprevalence among domestic pigs in temperate climate countries. Epizootic monitoring of HEV infection is insufficient in the Republic of Belarus due to lack of the required number of available and inexpensive diagnostic ELISA kits with good sensitivity and specificity. In this regard, research on development of domestic ELISA kit for semi-quantitative detection of antibodies to hepatitis E virus in pigs with subsequent assessment of seroprevalence to HEV in pig population in the Republic of Belarus is relevant. The results of studies on development of enzyme-linked immunosorbent assay kit for semi-quantitative determination of class G immunoglobulins to HEV in pig blood serum using recombinant proteins, including immunodominant amino acid sequences corresponding to the ORF2 and ORF3 proteins of HEV genotype 3 are presented in the paper. The optimal concentration for sorption of ORF2 and ORF3 proteins has been determined, which is 2 μg/ml and 1 μg/ml, respectively. The diagnostic sensitivity of this test kit makes 94.8 %, and the diagnostic specificity makes 100 %. Coefficients of variation being the criterion for assessing the intra-serial and inter-serial reproducibility of this test kit, make 3.5 % and 12.4 %, respectively, which allows to obtain reproducible results and identify specific anti-HEV antibodies in all positive samples of pig blood serum. When studying 1235 pig sera samples from various pig farms of Brest, Vitebsk, Gomel, Grodno, Minsk and Mogilev regions, seroprevalence of anti-HEV antibodies has been determined in 168 or 13.6 % of animals. The described diagnostic method can be widely used in science and practice for the further study of seroprevalence of anti-HEV. Acknowledgments. Seroprevalence and risk factors of recent infection with hepatitis E virus during an acute outbreak in an urban setting in Chad, 2017

BackgroundFrom September 2016–April 2017, Am Timan, Chad, experienced a large HEV outbreak in an urban setting with a limited impact in terms of morbidity and mortality. To better understand HEV epidemiology in this context, we estimated the seroprevalence of anti-HEV antibodies (IgM and IgG) and assessed the risk factors for recent HEV infections (positive anti-HEV IgM) during this outbreak.MethodsA serological survey using simple random sampling was implemented in Am Timan at the tail-end of the outbreak (sample size aim = 384 household). Household members provided us with blood samples and household heads answered questions around water, sanitation and hygiene practices and animal ownership. Blood samples were tested for HEV IgG and IgM antibodies using Enzyme-Immune-Assay (EIA). We calculated weighted prevalence estimates and prevalence ratios (PRs) for possible risk factors for recent infection using multivariate Cox regression.ResultsWe included 241 households (1529 participants). IgM prevalence decreased with age: 12.6% (< 5 years) to 4.3% (> 15 years). IgG prevalence increased with age: 23.5% (< 5 years) to 75.9% (> 15 years). Risk factors for recent HEV infections included: sharing the sanitation facility with other HHs (PR 1.72; 95%CI: 1.08–2.73), not systematically using soap for HW (PR 1.85; 95%CI: 1.30–2.63) and having animals sleeping inside the compound (PR 1.69; 95%CI: 1.15–2.50).ConclusionsEvidence suggests that Am Timan was already highly endemic for HEV before the outbreak, potentially explaining the limited extent of the outbreak. Recent infection with HEV was linked to household level exposures. Future HEV outbreak response must include ensuring access to safe water, and reducing household level transmission through active hygiene and sanitation promotion activities.

Seroprevalence of hepatitis E virus infection among dogs in several developed cities in the Guangdong province of China

Hepatitis E virus (HEV), as a zoonotic disease virus, was seldom studied in dogs especially in stray dogs. As previously reported, dog might be an accidental host of HEV for human beings and some risk factors might play an important role in HEV transmission. Thus, we designed this study to evaluate the seroprevalence of HEV infection among dogs in several cities in Guangdong province of China. This surveillance may help us understand risk factors including location, gender, live type, and diet habit for HEV transmission. The overall seroprevalence of anti-HEV antibodies in dogs was 19.00%. Positive rate of anti-HEV antibodies in other food groups (21.13%) was higher than that in dog food groups (9.77%) (P < 0.05), which suggested that diet habit might be a vital element of infecting HEV for dogs and play an important role in living environment. However, the analysis indicated that no strong relationship was observed among different cities, gender groups, and live type. Our study demonstrated that HEV is prevalent in dogs in the Guangdong province of China. As diet habit might become a vital element of infecting HEV for dogs and play an important role in living environment, similar studies of dogs should be conducted in the future. J. Med. The Prevalence of Hepatitis E Virus Infections among Swine, Swine Farmers and the General Population in Guangdong Province, China

Hepatitis E virus (HEV) infection is widespread in China, but few studies have been carried out in Guangdong Province. This study aimed to characterize the prevalence of HEV infections among swine, swine farmers and the general population in Guangdong Province. We conducted an epidemiological study that included swine, swine farmers and health examination attendees in Guangdong from 2011 to 2013. The overall seroprevalence of anti-HEV antibodies in swine was 64.7%. The results revealed that growing pigs, sows and boars (OR ranges from 3.5 to 21.5) have a higher risk than nursery pigs. HEV RNA in swine bile showed that HEV is epidemic in swine in the Pearl River Delta, with the highest prevalence of 22.73% in Foshan. Some genomes of HEV strains from each district were sequenced. Phylogenetic analysis of partial open reading frame 2 (ORF2) shows that they belong to genotype IV and are most closely related to isolates from China. In total, 307 participants were enrolled in the study, including 114 swine farmers and 193 attendees from hospitals. IgG anti-HEV was detected in 48.25% of swine farmers and in 38.34% of the general population. Seroprevalence rates were almost stratified by age, with a higher positive rate for males compared to females across all age groups. Women on swine farms appeared to have a lower risk of infection compared to the general population, revealing that the risk factors for HEV infection are not unique. The results suggested that there were other risk factors for HEV infection. HEV infection is prevalent in Guangdong, but due to the small sample sizes, more investigations are needed to assess the potential impact of HEV infection, and many additional risk factors should be considered.

Seroprevalence of hepatitis E virus (HEV) antibody and the possible association with chronic liver disease: a case-control study in Albania

A case-control study involving 109 in-patients with chronic liver disease and 190 in-patients with no apparent liver disease was conducted to evaluate the seroprevalence of anti-HEV antibodies and the possible association with chronic liver disease. Among cases, the anti-HEV prevalence was 36.6% which increased significantly by age; among controls, the prevalence was 12.1% (P<0.05) and was similar among age groups <60 years. Among cases, aged >50 years (OR 4.0, 95% CI 1.4-11) and the presence of end stage liver disease (ESLD) (OR 4.3, 95% CI 1.4-12.8) were associated independently with anti-HEV positivity. The mean optical density, determined by anti-HEV immunoenzymatic test, was significantly higher among patients with ESLD, compared to the other patients. These results indicate that there is a high seroprevalence of anti-HEV in patients with chronic liver disease and a possible association between HEV infection and/or anti-HEV production and advanced stage chronic liver disease.
