Multidisciplinary health teams.

The medical profession needs to learn team skills and review its managerial functions in anticipation of increasing involvement with other health professionals in a team setting. Educatinal techniques for acquiring these skills are already available or are being developed for other organizations. Review of the traditional medical hierarchy and its legal implications, architecture of health institutions, medical records systems, and the selection of medical students are other areas for specific attention.

BECAUSE there is only a nominal charge for drugs prescribed for National Health Service patients, neither the doctors nor the patients are particularly concerned about their true cost. Nevertheless, the annual cost to the Service is about £300 million, but because nearly all drugs are prescribed through the Service, the government has been able to negotiate competitive contracts and establish a system of voluntary price regulation with most of the pharmaceutical companies, so that drug prices in Britain are lower than in any other Western country. The circumstances that determine the choice of a career form a fascinat­ ing subject for speculation in the realm of human psychology. Few generals have duplicated George S. Patton's almost neonatally developed ambition to become a soldier (my father appointed him to West Point), and I have en­ countered only one physician or surgeon who cannot recall the time when he did not expect to be a doctor. During long service on committees charged with the selection of medical students, I have interviewed hundreds of young people who had decided to study medicine. After a coupl e of years of this duty I concluded that little was to be gained by asking the trite and obvious question: Why do you want to be a doctor? Many, including not a few who went on to successful careers in scientific medicine, were unable to give any very definite answer. I learned to regard this as a better omen than a flowery profession of a desire to help mankind. When I put the same question to a number of senior clinical colleagues I usually got an answer which indi­ cated that some rather trivial incident had directed their attention to the idea of studying medicine. Thereafter some ill-defined inner urge sufficed to po­ tentiate the original slightly supraliminal stimulus. That is surely an effective form of motivation. Joseph Erlanger (5) has told us that the idea of medicine as an occupa­ tion came to him from an older sister who, in my youth, had nicknamed me Doc because of my interest in lower forms of life. A noted clinical micro­ biologist told me that he went to medical school because of the impact of an incident related in a book for children he had read at the age of eight: the recovery of a child from a severe head injury without any specific treatment! Sometimes discouragement encountered in attempting another metier serves to lower the threshold for an excitatory suggestion that medicine may be a way out. Such was the case of Claude Bernard, whose literary activities dur­ ing his apprenticeship to an apothecary in Lyon caused his employer to re­ quest that the young man be restored to his parents. Orientations of social values in medical student assessment

A study of nine years of selection of medical students involved the analysis of data on predictive criteria and performance criteria. The predictive criteria consisted of traditional variables used by admissions committees, and a series of psychological tests, including an experimental test of social values orientation. Special attention was paid to variables discriminating students who succeeded in the school from those who had to repeat a year, and those who had to withdraw or were dropped because of physical and/or psychological reasons.

Attributes of Lifelong Competence in Medical Practice

Consideration of the characteristics associated with lifelong competence raises an ancient dispute about the relative influences of nature and nurture—of inheritance and environment. The process of selecting students for admission to medical school is concerned with nature. The medical curriculum and hospital training programs represent an extreme form of nurture or environment influence. What do we know about the relative contribution of these two factors to competence? Although the production of competent physicians is a major goal of the medical educational process, surprisingly few attempts have been made to determine how well this goal is achieved. The large volume of research on selection of medical students almost suggests that picking students who can complete the four-year curriculum is a goal in itself. What Should the Profession Do About the Incompetent Physician?

Ideally answer to the question, "What should the profession do about the incompetent physician?" is too simple to warrant discussion. It should be answered with one word, prevention. There should not be any incompetent physicians, for their very existence could be prevented by several methods. First, selection of medical students should be on such a scientific basis that the potentially incompetent physician could be eliminated by denying him admission. If, despite the refinements of the inexact science of modern psychology an incompetent student is admitted he can be promptly eliminated at the end of the first year and certainly before graduation. What? Have we detected an incompetent house officer? Improbable, but it may not be too late to persuade him to adopt another profession because he surely will be denied a license by a state board of medical examiners. And here my story should end. But wait! Let's be practical.
