Quality of Life in Pediatric Iritable Bowel Syndrome among a Sample of Egyptian Children at Ain Shams University Hospitals

Abstract Background Iritable bowel syndrome (IBS) is one of the pediatric functional gastrointestinal disorder with upper and lower gastrointestinal symptoms.IBS afects the child’s daily activities and quality of life (QoL) negatively, and can often persist into adulthood. Objectives To measure the dimensions of QoL of children with IBS among a sample of Egyptian pediatric patients at Ain Shams University Hospitals. Methods A cross-sectional study was conducted on 80 children aged 5-15 years old, who were diagnosed as IBS, according to Rome IV criteria Pediatric quality of life Inventory Generic Core (PedsQL generic core scale was used to measure QoL of the patients,where 100 is the maximum quality score. Results A total of 80 children participated in the study ; 48 (60%) males and 32 (40%) females with mean age 10.6 (SD2.7) years. Physical functioningscorewascalculatedtobe96.04(SD5.5), socialfunctioning score was 99.37 (SD3.95), emotional functioning score was 90.37 (SD8.85), and school functioning score was 73.25 (SD27.34).The school functioning scale was higher among male participants (P-value0.31), also the emotional scale was higher among male participants with no statistical significant diference (P-value0.70). Conclusion IBS afects the school functioning dimension of OoL among the pediatric age group rather than other dimensions.

Pediatric skeletal injuries in emergency department; an alarm of child maltreatment in Egypt: A cross-sectional study

BackgroundChild physical abuse, a type of child maltreatment (CM), poses a significant global public health concern. Nonaccidental fractures and soft tissue injuries, which encompass any action that directly or indirectly harms a child, are the primary indicators of physical abuse in children. ObjectiveThis study aimed to investigate the prevalence, risk factors, and outcomes of accidental and nonaccidental skeletal fractures in a sample of Egyptian children. MethodsA cross-sectional analytical study was conducted between March 2022 and August 2022. A total of 156 children who presented with skeletal injuries and attended Mobarak Central Hospital were enrolled. Patients were subjected to full history taking, complete examination, and investigations. A structured questionnaire was administered to all the legal guardians. ResultsPhysical abuse was reported in 22.4 % of cases, while medical neglect was reported in 19.9 % of cases. The incidence of physical abuse was notably higher among children whose fathers were smokers and/or drug addicts (p ≤ 0.05). The most common form of physical abuse was hitting (94.3 %). Among skeletal injuries, fractures were predominant (94.3 %), primarily closed fractures associated with contusions. Fractures of the upper limb accounted for the highest incidence (94.3 %) of skeletal injuries, and 60 % of physically abused children experienced moderate-severity injuries. ConclusionThe most common fracture observed in abused children is the upper limb fracture, typically involving a single bone. Clinicians should be more vigilant in suspecting abuse, even in cases where there is an isolated fracture, and advocate for the development of parental training programs.

Effect of the Low Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols (FODMAP) Diet on Control of Pediatric Irritable Bowel Syndrome and Quality of Life Among a Sample of Egyptian Children: A Randomized Controlled Clinical Trial.

Irritable bowel syndrome (IBS) is a pediatric pain-dominant functional gastrointestinal disorder that has a negative impact on all children's dimensions of quality of life. A dietary approach that focuses on limiting food elements with high fermentable oligosaccharides, disaccharides, monosaccharides, and polyols(FODMAP) can be used to decrease symptoms of IBS. This study aims to evaluate the effect of low FODMAP dietary intervention on health-related quality of life among a sample of Egyptian children. Eighty-four children aged 5-15 years old were randomly assigned to two groups, 42 patients in the low FODMAP diet group and 42 patients in the standard diet group. They received the diet for six weeks and were followed up weekly using a visual analog scale (VAS) for pain severity assessment, the Pediatric Quality of Life (PedsQL) InventoryGastrointestinal (GI) Symptoms Module Scale, and the PedsQL Inventory Generic Core Scale to assess the physical and psychosocial functioning of the patients. The VAS score decreased more in the low FODMAP group, which caused a significant difference between the two groups (p<0.001). The PedsQL Inventory GI Symptoms Modulescore increased more among the low FODMAPgroup, and this caused a significant difference between the two groups (p<0.001). PedsQL Inventory Generic Core score increased more among the low FODMAPs group, and this caused a significant difference between the two groups (p<0.001). Low FODMAP dietary intervention in pediatrics for six weeks decreased abdominal pain severity, improved gastrointestinal symptoms, and improved the health-related quality of life of the affected children.

Sensory processing profile among a sample of Egyptian children with different types of delayed language development: correlations of different variables

BackgroundResearch on sensory processing, particularly its association with language disorders, has been increasing in recent years, aiming to understand the correlation between language and sensory processing. This study aimed to determine sensory profiles among children aged 36 and 120 months with language disorders (autism spectrum disorders (ASD), attention deficit hyperactivity disorders (ADHD), specific language impairment (SLI), and intellectual disability (ID)) and correlate their language ages with their sensory profile responses.ResultsThe study involved 120 children with language disorders (ASD, ADHD, SLI, and ID), and 30 typically developing children. All children were subjected to assessment by the Sensory Profile and the Preschool Language Scale Fourth Edition (PLS-4) Arabic edition. Based on their sensory profile, children with language disorders are distinguished from typically developing peers. The former has a higher prevalence of sensory modulation disorders as well as atypical emotional and behavioral responses to sensory inputs compared to typically developing children. Children with ASD have the highest atypical sensory responses, followed by those with SLI, ADHD, and ID. A negative correlation was found between some sensory profile scores and receptive language scores. Expressive language was related to oral processing in some subtypes of language disorders.ConclusionsThe study indicates that detailed sensory processing assessment in children reveals different faces for different types of language disorders.

Detection of CYBA Gene Expression by Reverse Transcription PCR as a Diagnostic Tool for Autosomal Recessive Chronic Granulomatous Disease in a Sample of Egyptian Children

Abstract Chronic granulomatous disease (CGD) is an inherited primary immunodeficiency disease caused by functional impairment of the nicotinamide dinucleotide phosphate (NADPH) oxidase complex in neutrophilic granulocytes and monocytes, characterized by recurrent and severe infections, dysregulated inflammation, and autoimmunity. Approximately two-thirds of patients with CGD have defects in Cytochrome Bβ (CYBB), the X-linked gene encoding glycoprotein 91 phagocytic oxidase (gp91phox). Rarer autosomal recessive (AR) forms of CGD are caused by autosomal recessive mutations in CYBA genes encoding protein 22 phagocytic oxidase (p22phox). However, the incidence of autosomal recessive CGD can be higher than X-linked CGD in countries with high rates of consanguineous marriage. In the present study we aimed to diagnose the commonest AR-type of CGD by detection of CYBA gene expression using real time RT-PCR as a cheaper diagnostic method for CGD subtypes categorization among immunodeficient children. This case-control study was conducted on 15 children provisionally diagnosed as CGD patients with DHR stimulation index using phorbol myristate acetate (PMA) &amp;lt; 30% (Group I). The study also included 12 mothers and 8 fathers of the studied patients to detect the genetic mutations in carriers, if any (Group IIa and IIb respectively), and 14 apparently healthy children as a control group (Group III). We found that cases with a fold change of CYBA gene expression less than 0.6 were considered under-expressed. At this cut-off value, from our molecularly studied subjects 1 case and 2 mothers showed under-expression of CYBA gene. In conclusion, we could establish the diagnosis of AR-form of CGD, derives from defects in the CYBA gene, which encodes p22phox of the NADPH oxidase using real time RT-PCR, without the need to use complex and expensive methodologies such as northern blot, slot blot, or genomic DNA sequencing.

Characteristics of Autism Spectrum Disorder among a Sample of Egyptian Children

Background Autism spectrum disorder (ASD) is a biologically based neurodevelopmental disorder characterized by impairments in two major domains; deficits in social communication and social interaction and restricted repetitive patterns of behavior, interests and activities. Aim of the Work Understanding and correlating the socioeconomic status (SES), clinical characteristics and the severity of ASD. Patients and Methods 100 autistic patients were assessed using DSM-V criteria, intelligence quotient (IQ) assessment using Stanford Binnet intelligence scale, assessment of severity of autistic symptoms using childhood autism rating scale (CARS), and psychometric evaluation for comorbid psychiatric conditions by using Pediatric Symptoms Checklist (PSCL), Full clinical examination, neurological examination, Electroencephalography (EEG) for some patient &amp; socioeconomic status (SES) assessment were also done. Results Severity of ASD was correlated with each of motor &amp; social delay (p = 0.007) (p = 0.000) respectively, impairment in Sphincter control (p = 0.012), imitation impairment (p = 0.010) and stereotypic behavior (p = 0.015. Our patients had comorbidities in the form of; gastrointestinal tract (GIT) symptoms, eating disorders and EEG findings (p = 0.004) (p = 0.005) (p = 0.038) respectively. Our patients mainly belonged to middle SES (57%) and severity of ASD was correlated with family possessions domain of SES (p = 0.001). Children who belonged to high SES were more likely to be diagnosed earlier (p = 0.016) with better IQ (p = 0.003) than those from lower SES. Conclusion It is important for developing countries to have their own research and to increase public awareness in all socioeconomic strata and establish integrated services for autism.

Psychometric properties and cross-cultural comparison of the Arabic version of the Child Behavior Checklist (CBCL), Youth Self Report (YSR), and Teacher’s Report Form (TRF) in a sample of Egyptian children

BackgroundThe Achenbach System of Empirically Based Assessment (ASEBA) forms are among the most studied instruments for assessing behavioral, emotional, social, and thought problems in children and adolescents worldwide. Although ASEBA instruments have been translated into Arabic, fewer studies have investigated their psychometric properties and norms in Arabic speaking societies than in other societies.MethodsRevisions were made to the Modern Standard Arabic (MSA) translations of the Child Behavior Checklist for Ages 6–18 (CBCL/6–18), the Teacher’s Report Form (TRF), and the Youth Self-Report (YSR). Parents of 6–18-year-olds who came to the general pediatric clinic in Tanta University Hospital during a 2-year period for routine check-ups were invited to fill out the CBCL/6–18 (N = 595), while 11–18-year-olds were invited to fill out the YSR (N = 409). TRFs were filled out by teachers (N = 329).ResultsConfirmatory factor analyses supported the previously reported eight-factor syndrome structure of the forms with good psychometric properties and moderate cross-informant correlations. The mean CBCL/6–18 and YSR Total Problem scores qualified for the previously established ASEBA Multicultural Norm Group 2, while the mean TRF Total Problem score qualified for group 3.ConclusionsThe good psychometric properties and the identification of Multicultural Norm Groups for scores obtained with the Arabic translations of ASEBA forms in Egyptian society support use of the ASEBA for assessment and outcome evaluations of behavioral, emotional, social, and thought problems among Egyptian youth.

Application of a rehabilitation program for executive functions in a sample of Egyptian children with learning disorder

BackgroundIn recent years, significant progress has been made on ways to improve executive function (EF) skills for school readiness involving direct EF training and classroom educational programs. Due to the absence of a well-structured Arabic program for EF training in children, the rationale of this study is to implement a comprehensive, evidence-based intervention program to help Egyptian children with learning disorders to overcome their EF impairment. It uses the multimodality approach to help meet the needs of students with a variety of learning styles. The aim of this study is to adapt the combined form of the “Executive Functions Training-Elementary” and the “Promoting Executive Function In The Classroom” programs and its application to test its effectiveness in the rehabilitation of Egyptian learning-disordered children.ResultsThe results showed significant improvement in the Arabic version of Barkley Deficits in Executive Functioning Scale, Children and Adolescents long-form (BDEFS-CA) scores for executive functions (EF) evaluation (executive function summery score from 228.63 to 213.77 with p-value < 0.001), and it also showed significant improvement in the Arabic dyslexia assessment test (ADAT) scores (from 1.89 to 1.33 with p-value < 0.001) for dyslexia evaluation.ConclusionThe study concluded that designing an Arabic rehabilitation program specific for executive function difficulties was effective for improving both EF deficits and dyslexia, but there is a need for further studies comparing this program to other methods of traditional interventions.Trial registrationThe study was retrospectively registered at www.clinicaltrials.gov NCT05476133, approved on 26 July 2022. Microbiota and Potential Asthma Markers: A Case Control Study in a Sample of Egyptian Children

Background: It is well known that allergy development is linked to alteration in microbiome and cytokines levels. colonization of children gut by wide array of microbes and bacteria is associated with mucosal and systemic immune responses as well as allergy development. Investigating the role of gut microbiota and serum cytokines clarifies the pathophysiology of the disease and enhance development of management plan. This study aimed to assess fecal microbiota in asthmatic children and correlate it with serum CXCL8 and p38 MAPK as potential asthma severity markers. Results: This case control study enrolled 56 asthmatic children aged 2-8 years, and 20 non asthmatic children of matched sex and age group as a control. Fresh stool samples were obtained from enrolled children for analysis of gut microbiota through DNA extraction and Real time PCR, using species-specific primers, serum CXCL8 and P38MAPK levels were estimated by ELISA. Higher level of Bifidobacterium and lower level of Lactobacillus was reported in asthmatic compared to non-asthmatic children, current results showed significant difference between asthmatic and non-asthmatic subgroups regarding CXCL 8 serum level. Study reported significant negative correlation between presence of asthma and serum markers CXCL8 and p38MAPK while significant positive correlation between presence of asthma and Bifidobacterium Log. Conclusions: the association between CXCL8 level, p38 MAPK and microbiome suggesting a link between gut bacteria and inflammatory status. Moreover, elevated CXCL8 , and p38 MAPK level increase symptoms severity. The alteration of microbiome level associated with elevated markers level suggesting the protective role of gut microbiome in asthma control.

Cytokines’ Expression in Children with Viral Pneumonia: A prospective Study in a Sample of Egyptian Children

Background: Community acquired pneumonia still a prominent reason of mortality and morbidity in developing countries which can caused by many pathogens with predominant of viral etiologies in children. Studying of cytokines response in viral pneumonia is useful to improve management and outcome. Aim: This study aimed to compare the level of cytokines (IL5, IL6, IL8, IL1B and IL10) in children diagnosed with viral and non-viral pneumonia, correlate with the causative virus and the clinical picture. Methods: An observational, prospective study included 101 children with pneumonia. Serum was analyzed different cytokines (IL10, IL1B, IL5, IL8, and IL6) by ELISA. Result: No significant difference was reported between cytokines level in children with viral pneumonia and non-viral pneumonia in our study. A significant difference was found regarding IL-6 concentration between patients with and without Human Metapneumovirus and Para 3 infections was reported. Conclusion: Cytokines level in pneumonia is affected by many factors as the causative organism, nutritional status, age, severity, and duration of infection. Additionally, recent research has disclosed that interleukin responses are considerably altered in numerous disease states. A large-scale study with measurement of cytokines in subsequent days is recommended.

Reliability and Validity of Arabic Translation of Child Sexual Abuse Questionnaire for Use in Children and Adolescents

ABSTRACT Child Sexual Abuse Questionnaire was developed to assess Child Sexual Victimization experiences, and the associated characteristics and circumstances. The aim of the study is to evaluate the reliability and validity of the Arabic Child Sexual Abuse Questionnaire (CSAQ) in a sample of Egyptian children. The original CSAQ-English was translated into Arabic based on the established guidelines. Then, a cross-sectional study was carried out on a group of 150 male and female Egyptian children aging 8–14 years to test the translated questionnaire for validity and reliability. The Statistical analysis was performed to internal consistency, Item-Total correlation, as well as Inter-Item correlation. Cronbach’s Alpha coefficient to assess internal consistency was found to be satisfactory, 0.88 and 0.89 for CSA without physical contact items and CSA with physical contact items, respectively. The item-total correlations were found to be within r = .234; P = .004 to r = .927; P = <0.001, while Inter-item correlations ranged from 0.125 to 0.928. The CSAQ-Arabic version is a reliable and valid tool that can be used by researchers in clinical practice to measure sexual victimization experiences in Arabic-speaking children based on the content and face validation together with a good internal reliability.

Influence of COVID-19 pandemic lockdown on a sample of Egyptian children with Down syndrome

BackgroundDown syndrome (DS) is characterized by variable degrees of intellectual disability (ID). The coronavirus disease-2019 (COVID-19) lockdown prevented children with DS from reaching their rehabilitation facilities. This could have led to deterioration of their abilities and mental health hazards. The aim of this cohort study was to investigate frequency of COVID-19, the influence of COVID-19 pandemic on health, and some abilities of children with DS, and to explore factors that could have governed receiving home-based training during the lockdown. A survey of 150 individuals with Down syndrome was answered by their caregivers. Additionally, 135 participants were subjected to assessment of cognitive, language, and motor abilities using Portage program. They were divided into 2 groups: group I who received online therapy sessions during the lockdown and group II who did not receive sessions. Logistic regression was used to determine the factors which influenced getting home-based training.ResultsThe percentage of COVID-19 cases was 3.3%. All evaluated abilities were reduced despite receiving online sessions particularly language performance (P < 0.001). Male gender, having severe ID and low parental education were among the factors which encouraged parents to get virtual training.ConclusionCOVID-19 pandemic had a negative impact on the abilities of DS children even those who got rehabilitation sessions. Their dependence on social interaction could have limited the benefit of virtual sessions. Factors that influence a parent’s decision to get home-based training should be monitored and targeted in order to overcome obstacles or concepts that may prevent families from enduring home-based intervention.

Urinary Phthalate Levels in Relation to Obesity among a Sample of Egyptian Children

BACKGROUND: Childhood obesity is considered a risk factor for chronic diseases later in life. Phthalates (phthalate acid esters), predominant constituents of plasticizers, are well-thought-out global environmental contaminants. AIM: This study aims to investigate the relationship between obesity and urinary phthalates in Egyptian children. MATERIALS AND METHODS: This cross-sectional study included 210 children; 71 children were obese. Age ranged between 8.8 and 16 years with a mean of 12.93 ± 1.29 years. Sociodemographic data were collected. Clinical examination included measuring body weight, height, waist and hip circumferences (WC and HC), and calculation of body mass index (BMI). The lipid profile was analyzed. Urine samples were tested for phthalates levels using high-performance liquid chromatography. RESULTS: Urinary phthalates metabolites mono benzyl (MBzP), monobutyl (MBP), monoethyl (MEP), and mono (2ethylhexyl) phthalate (MEHP) were detected in all urinary samples with varying levels. The median concentrations of MBzP, MEHP, MBP, and MEP were 1.4, 54.5, 29.9, and 490 (ng/ml), respectively. In obese children, urinary MBP, MEP, and MEHP demonstrated significantly higher mean levels than in non-obese children. Physical indicators of obesity as body weight, BMI, WC, and HC were significantly positively correlated with urinary levels of MEHP and MEP, while urinary MBzP demonstrated a significant positive association with serum triglycerides levels. CONCLUSION: The present study suggests an association between phthalates exposure and childhood and adolescent adiposity.
