Managing Postoperative Complications in a Patient with Excessive Ankle Stiffness: A Case Study Addressing Balance, Muscle Strength, and Quality of Life

Introduction: Postoperative complications decrease muscle strength and balance. Thus, managing the complications is crucial for optimizing patient outcomes and promoting speedy recovery. This case study explores the use of Instrument-Assisted Soft Tissue Mobilization (IASTM) and electrotherapy in managing postoperative complications in a patient with excessive ankle stiffness, addressing balance and muscle strength, as they are essential components of functional mobility in patients with ankle stiffness. Case Presentation: A 41-year-old female athlete with Achilles tendon contracture participated in this study. A 12-week IASTM and electrotherapy was implemented. Isokinetic muscle strength, balance, and quality of life were investigated. Results: The results showed significant improvement in the stability index, path length and area, dorsiflexion/plantarflexion peak torque, range of motion (ROM), total work, and reduced passive stiffness. The results of all domains of quality of life (SF-36), Foot and Ankle Ability Measure (FAAM), , Achilles Tendon Rupture Score (ATRS), Visual Analog Scale (VAS), and Foot and Ankle Outcome Score (FAOS) improved. The intervention was successful in improving balance, muscle strength, sagittal plane ROM, and quality of life and reduced pain during balance in the patient. Conclusions These findings suggest that the intervention may be an effective treatment option for individuals with excessive ankle stiffness and related balance and strength deficits.

CAN KINETICS AND KINEMATICS OF SINGLE LEG FORWARD AND CROSSOVER TRIPLE HOP TESTS DETERMINE RECREATIONAL MALE ATHLETES' RETURN TO SPORTS AFTER ACL RECONSTRUCTION?

Hop tests are used to determine return to sports after ACL reconstruction. They mostly measure distance and symmetry but do not assess kinematics and kinetics. Recently, biomechanical evaluations have been incorporated into these functional jump tests for the better assessment of return to sport. We assessed the sagittal plane range of motion (ROM) of the knee, the deviation axis of rotation (DAOR), and the vertical ground reaction force (vGRF) normalized to body weight in nine healthy participants during the single leg (SLH) and crossover hop tests (COHT).Participants' leg lengths were measured. Jumping distances were marked in the test area as being 4/5 of the leg length. Four sensors were placed on the thighs, the legs and the feet. These body parts were handled as a single rigid body. Eight 480 Hz cameras were used to capture the movements of these rigid bodies. vGRF at landing were measured using a force plate (Bertec, Inc, USA). The ROM of the knee joint and the DAOR were obtained from kinematic data.Participants' joint kinematics metrics were similar in within-subjects statistical tests for SLH and COHT. We therefore asked whether the repeated vGRF normalized to body weight will be similar in both legs during these jumps. Joint kinematics metrics however were different in between subjects indicating the existence of a personalized jumping strategy. These hop tests can be recorded at the beginning of the training season for each individual, which can establish a comparative evaluation database for prospective lower extremity injury recovery and return to sport after ACL injury.

Reliability and measurement properties of upper cervical flexion-extension range of motion testing in people with cervicogenic headache and asymptomatic controls

ABSTRACT Objective The objective was to determine reliability of sagittal plane range of motion (ROM) assessment in a healthy population and in people with cervicogenic headache (CGH). Methods Upper cervical flexion/extension ROM was measured using an iPhone magnetometer sensor and retraction/protraction ROM measured by linear displacement. Two independent raters evaluated these movements in 33 subjects with CGH and 33 age and gender matched asymptomatic healthy controls on two occasions. Measurement procedures were standardized; and the order of testing randomized. Reliability, standard error of measurement (SEM) and minimum detectable change (MDC) were calculated. Results Subjects comprised 30 females and 36 males. The iPhone method demonstrated high reliability (ICC >0.82) with SEM values ranging from 0.62 to 1.0 and MDC values ranging from 1.70 to 4.81 respectively. Evaluation of linear displacement demonstrated moderate reliability (ICC >64). with SEM values ranging from 0.38 to 1.91 and MDC values ranging from 1.05 to 1.63 respectively. Compared to healthy controls, upper cervical flexion, retraction and protraction ROM was significantly less in the CGH group with mean difference of 6.50°, 1.52 cm and 2.34 cm respectively. Discussion Upper cervical spine sagittal plane ROM can be measured with moderate to high reliability and was found to be more restricted in people with CGH.

The effect of hip abductor fatigue on knee kinematics and kinetics during normal gait.

ObjectiveTo investigate the effect of hip abductor fatigue on the kinematics and kinetics of the knee joint during walking in healthy people to provide a new approach for the prevention and treatment of knee-related injuries and diseases.MethodsTwenty healthy participants, ten females, and ten males, with a mean age of 25.10 ± 1.2 years, were recruited. Isometric muscle strength testing equipment was used to measure the changes in muscle strength before and after fatigue, and the surface electromyography (SEMG) data during fatigue were recorded synchronously. The Vicon system and an AMTI© force platform were used to record the kinematic parameters and ground reaction force (GRF) of twenty participants walking at a self-selected speed before and after fatigue. Visual 3D software was used to calculate the angles and torques of the hip and knee joints.ResultsAfter fatigue, the muscle strength, median frequency (MF) and mean frequency (MNF) of participants decreased significantly (P < 0.001). The sagittal plane range of motion (ROM) of the knee (P < 0.0001) and hip joint (P < 0.01) on the fatigue side was significantly smaller than before fatigue. After fatigue, the first and second peaks of the external knee adduction moment (EKAM) in participants were greater than before fatigue (P < 0.0001), and the peak values of the knee abduction moment were also higher than those before fatigue (P < 0.05). On the horizontal plane, there is also a larger peak of internal moment during walking after fatigue (P < 0.01).ConclusionHip abductor fatigue affects knee kinematics and kinetics during normal gait. Therefore, evaluating hip abductor strength and providing intensive training for patients with muscle weakness may be an important part of preventing knee-related injuries.

Comparing lab and field agility kinematics in young talented female football players: Implications for ACL injury prevention

ABSTRACT Modifiable (biomechanical and neuromuscular) anterior cruciate ligament (ACL) injury risk factors have been identified in laboratory settings. These risk factors were subsequently used in ACL injury prevention measures. Due to the lack of ecological validity, the use of on-field data in the ACL injury risk screening is increasingly advocated. Though, the kinematic differences between laboratory and on-field settings have never been investigated. The aim of the present study was to investigate the lower-limb kinematics of female footballers during agility movements performed both in laboratory and football field environments. Twenty-eight healthy young female talented football (soccer) players (14.9 ± 0.9 years) participated. Lower-limb joint kinematics was collected through wearable inertial sensors (Xsens Link) in three conditions: (1) laboratory setting during unanticipated sidestep cutting at 40-50°; on the football pitch (2) football-specific exercises (F-EX) and (3) football games (F-GAME). A hierarchical two-level random effect model in Statistical Parametric Mapping was used to compare joint kinematics among the conditions. Waveform consistency was investigated through Pearson’s correlation coefficient and standardized z-score vector. In-lab kinematics differed from the on-field ones, while the latter were similar in overall shape and peaks. Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found for on-field kinematics (p < 0.044). The largest differences were found during landing and weight acceptance. The biomechanical differences between lab and field settings suggest the application of context-related adaptations in female footballers and have implications in ACL injury prevention strategies. Highlights Talented youth female football players showed kinematical differences between the lab condition and the on-field ones, thus adopting a context-related motor strategy. Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found on the field. Such differences pertain to the ACL injury mechanism and prevention strategies. Preventative training should support the adoption of non-linear motor learning to stimulate greater self-organization and adaptability. It is recommended to test football players in an ecological environment to improve subsequent primary ACL injury prevention programmes.

The effect of a frontal plane gait perturbation bout on knee biomechanics and muscle activation in older adults and individuals with knee osteoarthritis

BackgroundWalking can be a challenging task for individuals with knee osteoarthritis and many older adults. The purpose was to determine the immediate effect of a frontal plane perturbation bout during walking on knee biomechanics and muscle activation patterns in these groups. Methods44 asymptomatic older adults and 32 individuals with knee osteoarthritis were recruited. Sagittal and frontal plane knee biomechanics and muscle activation levels were calculated and recorded during treadmill walking. After a baseline collection at 6-min, a random series of unexpected medial/lateral walkway surface perturbations were delivered over approximately 24 min. Data was collected immediately after the perturbations. Discrete measures extracted from biomechanical waveforms, and principal component analysis to analyze muscle activation were utilized to determine time effects and interactions using analysis of variance models (alpha = 0.05). FindingsAfter the perturbation bout, sagittal plane range of motion was significantly increased in the osteoarthritis group (Effect Size = 0.24) and in both groups peak knee adduction moment (Effect Size = 0.10) and difference between peak flexion and extension moments (Effect Size = 0.16) were significantly increased. Muscle amplitudes in both groups were significantly reduced (PP1-scores) after perturbation bout, whereas significant time-based gait cycle activation pattern alterations identified by PP2- and PP3-scores were related to group and muscle assignment. InterpretationPerturbations were tolerated by all participants, resulting in significant alterations to biomechanical outcomes and muscle activation levels and patterns. Demand on the knee joint was not increased after perturbations. Gait perturbation training in these groups may be feasible using a frontal plane perturbation bout.

Foot and Ankle Motion after Tibiotalocalcaneal Arthrodesis: Comparison with Tibiotalar Arthrodesis Using a Multi-Segment Foot Model.

Tibiotalocalcaneal arthrodesis is an established surgical procedure for treating patients with end-stage ankle joint arthritis and subtalar joint arthritis. Although it greatly relives pain, a major drawback is loss of range of motion. Although it is known to restrict an additional subtalar joint compared to tibiotalar arthrodesis, there is a lack of gait analysis studies comparing the two methods. This study aimed to evaluate the differences in kinematics of the foot and ankle joints between tibiotalar and tibiotalocalcaneal arthrodesis. We also compared preoperative and postoperative statuses for each surgical method. The study included 12 and 9 patients who underwent tibiotalar and tibiotalocalcaneal arthrodesis, respectively, and 40 healthy participants were included in the control group. The DuPont foot model was used to analyze intersegmental foot and ankle kinematics during gait. Compared to controls, both tibiotalar and tibiotalocalcaneal arthrodesis resulted in slow gait speed with reduced stride length, increased step width, and decreased range of sagittal plane motion. Both fusion methods showed similar range of motion in all segments and planes following surgery. Coronal positions showed more supination of the forefoot and pronation of the hindfoot segment after each operation, particularly tibiotalocalcaneal arthrodesis. Gait after tibiotalocalcaneal arthrodesis did not significantly differ from that after tibiotalar arthrodesis, but there was a tendency of more pronation in the hindfoot segment. Both fusion methods limited foot and ankle motion in similar ways. Comparing tibiotalar and tibiotalocalcaneal arthrodesis suggests that additionally fusing the subtalar joint does not cause greater movement restriction in patients. Objectively comparing tibiotalar and tibiotalocalcaneal arthrodesis will facilitate further understanding of the effect of tibiotalocalcaneal arthrodesis on movement and the value of subtalar joint motion for improved preoperative counselling.

Are external and internal measures of knee joint loading closely related in knee OA subjects?

The elderly are susceptible to many disorders that alter the gait pattern and could lead to falls and reduction of mobility. One of the most applied therapeutical approaches to correct altered gait patterns is the insertion of insoles. Principal Component Analysis (PCA) is a powerful method used to reduce redundant information and it allows the comparison of the complete waveform. The purpose of this study was to verify the influence of wedges on lower limbs’ net joint moment and range of motion (ROM) during the gait of healthy elderly participants using PCA. In addition, discrete values of lower limbs’ peak net moment and ROM were also evaluated. 20 subjects walked with no wedges (control condition) and wearing six different wedges. The variables analyzed were the Principal Components from joint net moments and ROM in the sagittal plane in the ankle and knee and joint net moments in frontal plane in the knee. The discrete variables were peak joint net moments and ROM in sagittal plane in knee and ankle. The results showed the influence of the wedges to be clearer by analyzing through PCA methods than to use discrete parameters of gait curves, where the differences between conditions could be hidden.

Trunk, pelvis, and knee kinematics during running in females with and without patellofemoral pain

Females are two times more likely to develop patellofemoral pain (PFP) than males. Abnormal trunk and pelvis kinematics are thought to contribute to the pathomechanics of this condition. However, there is a scarcity of evidence investigating proximal segments kinematics in females with PFP. The purpose of this study was to investigate whether females with PFP demonstrate altered trunk, pelvis, and knee joint kinematics compared with healthy controls during running. Thirty-four females (17 PFP, 17 controls) underwent a 3-dimensional motion analysis during treadmill running at preferred and fixed speeds, each trial for 30 s. Variables of interest included magnitudes of peak angles for trunk (forward flexion, ipsilateral trunk lean), pelvis (anterior tilt, contralateral drop), knee (flexion, valgus, internal rotation), range of motion (RoM) of trunk and pelvis in sagittal and frontal planes and RoM of knee joint in the three cardinal planes of motion. Kinematic data were compared between groups using mixed model repeated measure analysis of variance with the trial as the repeated measure. The PFP group displayed significantly less pelvis frontal plane RoM, greater knee frontal plane RoM, and less knee sagittal plane RoM during running compared with controls, irrespective of running trial. No differences were found in peak kinematic variables between PFP and healthy groups. These results may suggest a rigid stabilization strategy at the pelvis, which the body has adapted to prevent further frontal plane knee malalignment. Less knee sagittal plane RoM may be indicative of another protective strategy in the PFP group to avoid patellofemoral joint reaction force. Clinical assessments and rehabilitative treatments may benefit from considering a global program with focus on pelvis kinematics in addition to the knee joint in females with PFP.

P5. Fixation stability of four different cervical disc prostheses endplate configurations

<h3>BACKGROUND CONTEXT</h3> Motion preservation devices have become increasingly popular in the treatment of spinal pathologies that require biomechanical support following removal of the degenerated tissue. As these devices continue to be developed to treat multiple adjacent diseased spinal levels, like the use of multiple disc prostheses or placement of a disc prosthesis adjacent to a fused level, in-depth biomechanical data that supports these established surgical methods are needed. <h3>PURPOSE</h3> To assess immediate stability of four cervical disc prostheses under low endurance multidirectional cyclic loading conditions in a cadaveric cervical spine model. <h3>STUDY DESIGN/SETTING</h3> Each prosthesis was implanted in a cervical functional spinal unit (FSU) and cyclically loaded 2000 times under physiological ranges of combined flexion/extension coupled with axial rotation at a rate of 2.5deg/s. <h3>PATIENT SAMPLE</h3> There were 16 (C4-C5) and 16 (C6-C7) FSUs dissected from eight male and eight female cadaveric cervical spines with an average age of 55.9 +/- 9.7 years. Three FSUs were not usable due to poor tissue quality which reduced the sample size to 29 FSUs. <h3>OUTCOME MEASURES</h3> A one-way repeated measures ANOVA with a Student Newman-Keuls test was used to compare differences in the combined motion data. Anteroposterior endplate migration of both the upper and lower implant components was measured after 20, 250, 500, 1,000, 1,500 and 2,000 cycles. Tests were stopped if implant component migration exceeded 3mm or endplate fracture was observed, and migrations less than 0.3mm were defined as clinically stable. <h3>METHODS</h3> Four cervical disc prostheses that had a similar ball and socket type articulation mechanism but varying endplate fixation features were tested: 1. A clinically used and FDA-approved single central keel (SCK). 2. A clinically used (outside the U.S.) low profile teeth (LPT) with domed endplate shape to allow a better fit in case of concave vertebral endplate. 3. A clinically used (outside the U.S.) tripod keel (TK) and 4. Small single keel (SSK). A minimum sample size of six implants per prosthesis type were used. Three additional TK disc prostheses and two additional LPT disc prostheses were implanted. Specimens were cyclically tested 2,000 times at a rate of 2.5deg/s to an end limit of 1.5Nm in flexion or extension coupled with 1.5Nm of right and left axial rotation resulting in a total applied moment of 2.25Nm. A one-way repeated measures ANOVA with a Student Newman-Keuls test was used to compare differences in the combined motion data. Anteroposterior endplate migration of both the upper and lower implant components was measured after 20, 250, 500, 1,000, 1,500, and 2,000 cycles. Tests were stopped if implant component migration exceeded 3mm or endplate fracture was observed, and migrations less than 0.3mm were defined as clinically stable. <h3>RESULTS</h3> No statistical difference was found between the combined (flexion plus extension) sagittal plane range of motion or the combined (left plus right) axial motion. All but one of the specimen migration values were below 0.3mm. Upper prosthesis component migration ranged between 0.02mm and 0.08mm and between 0.01mm and 0.04mm for the lower components. No significant differences occurred between the upper components or the lower prosthesis components of any implant type. <h3>CONCLUSIONS</h3> Three different low profile endplate fixation designs for a cervical disc were found to provide comparable immediate holding strength to an existing FDA-approved central-based keel design. <h3>FDA DEVICE/DRUG STATUS</h3> Prodisc C (Approved for this indication), Prodisc C Vivo (Investigational/Not approved), Prodisc C Nova (Investigational/Not approved), Prodisc C SK (Investigational/Not approved).

When puberty strikes: Longitudinal changes in cutting kinematics in 172 high-school female athletes

ObjectivesYoung female athletes involved in high-speed cutting and change-of-direction sports are particularly susceptible to anterior cruciate ligament (ACL) injuries. Little is known if maturational changes in cutting technique contribute to the increased injury risk. Our objective was to examine longitudinal changes in cutting kinematics in female athletes as they matured through puberty. DesignLongitudinal cohort study MethodsHigh school female athletes (n=172) performed an unanticipated 45° cutting task in the biomechanics laboratory across two or more pubertal stages. Pubertal stages were classified using a modified Pubertal Maturational Observational Scale (pre-, mid-, or post-pubertal stages). Generalized linear mixed models were used to compare kinematics across pubertal stages. ResultsAs girls matured through puberty, they displayed a decrease in sagittal-plane hip (1.8-2.6°, p<0.03) and knee range-of-motion (ROM) (2.7-2.9°, p≤0.01), and decreased peak hip (2.9-3.2°, p≤0.02) and knee flexion angles (2.7-2.9°, p≤0.01), which is indicative of greater quadriceps dominance. Peak knee abduction angles also increased as girls progressed through puberty (0.9-1.4°, p≤0.02), suggesting greater ligament dominance. In terms of trunk dominance, there were mixed findings with a decrease in trunk frontal- (2.5-5.7°, p≤0.03) and sagittal-plane ROM (2.0°, p≤0.01), but an increase in trunk transverse-plane ROM (2.8-3.6°, p≤0.02) observed as girls mature. Other significant changes in cutting technique were decreased peak trunk flexion (3.8-7.8°, p≤0.01), and decreased hip flexion (2.9-3.3°, p≤0.02) and knee flexion angles (2.0-3.0°, p≤0.03) at initial contact, suggesting a more upright and stiffer cutting posture. ConclusionsAs girls mature through puberty, there is a change in cutting strategy characterized by greater quadriceps and ligament dominance.

DOES BRAIN ACTIVATION DURING FUNCTIONAL MOVEMENT TASKS DIFFERENTIATE BETWEEN GOOD AND BAD MOVERS? AN INTEGRATED NEUROIMAGING ASSESSMENT OF MOTOR CONTROL IN YOUNG ATHLETES

Background:Aberrant frontal and sagittal plane knee motor control biomechanics contribute to increased anterior cruciate ligament (ACL) injury risk. Emergent data further indicates alterations in brain function may underlie ACL injury high risk biomechanics and primary injury. However, technical limitations have limited our ability to assess direct linkages between maladaptive biomechanics and brain function.Hypothesis/Purpose:(1) Increased frontal plane knee range of motion would associate with altered brain activity in regions important for sensorimotor control and (2) increased sagittal plane knee motor control timing error would associate with altered activity in sensorimotor control brain regions.Methods:Eighteen female high-school basketball and volleyball players (14.7 ± 1.4 years, 169.5 ± 7 cm, 65.8 ± 20.5 kg) underwent brain functional magnetic resonance imaging (fMRI) while performing a bilateral, combined hip, knee, and ankle flexion/extension movements against resistance (i.e., leg press) Figure 1(a). The participants completed this task to a reference beat of 1.2 Hz during four movement blocks of 30 seconds each interleaved in between 5 rest blocks of 30 seconds each. Concurrent frontal and sagittal plane range of motion (ROM) kinematics were measured using an MRI-compatible single camera motion capture system.Results:Increased frontal plane ROM was associated with increased brain activity in one cluster extending over the occipital fusiform gyrus and lingual gyrus (p = .003, z > 3.1). Increased sagittal plane motor control timing error was associated with increased brain activity in multiple clusters extending over the occipital cortex (lingual gyrus), frontal cortex, and anterior cingulate cortex (p < .001, z > 3.1); see Figure 1 (b).Conclusion:The associations of increased knee frontal plane ROM and sagittal plane timing error with increased activity in regions that integrate visuospatial information may be indicative of an increased propensity for knee injury biomechanics that are, in part, driven by reduced spatial awareness and an inability to adequately control knee abduction motion. Increased activation in these regions during movement tasks may underlie an impaired ability to control movements (i.e., less neural efficiency), leading to compromised knee positions during more complex sports scenarios. Shorter work boot shaft height improves ankle range of motion and decreases the oxygen cost of work

Work boots featuring design elements implemented to protect the foot and lower limb are common in many industries. However, boot design can have negative unintended consequences such as increases in work related fatigue and injury risk. This study compared joint kinematics and the physiological cost of two commercially available steel-toed work boots with different shaft designs worn throughout a simulated work task. A boot with a lower-cut and scalloped shaft allowed greater sagittal plane range of motion at the ankle joint compared to a boot with a higher-cut straight shaft, particularly in the late stance phase of the gait cycle. This was coupled with a decreased physiological cost of treadmill walking (2.6 and 3.1% improvement in oxygen consumption and walking economy, respectively), likely caused by a more efficient gait pattern. A lower-cut and scalloped shaft could lead to decreases in work-related fatigue and the subsequent risk of injuries in workers. Practitioner summary: Gait kinematics and the physiological cost of work boots with different shaft heights has not previously been investigated. A randomised cross-over study design found that a boot with a lower shaft height and scalloped collar improved ankle range of motion and reduced the physiological cost experienced by the wearers. Gender difference of hip-ankle compensations following a novel trip perturbation in young adults

BackgroundWomen have an increased likelihood of sustaining a fall-related injury compared to men; however, little is known about fall prevention strategies between genders. The purpose of this study was to compare the gender differences in lower limb reactions and three-dimensional recovery patterns following a treadmill-induced trip perturbation. MethodsSeventy-six participants who are right limb dominant enrolled in the study, which included 41 females (26.15 [9.92] years old) and 35 males (27.11 [9.15] years old). The outcome measures included a three-dimensional (3D) range of motion (ROM) analysis on the bilateral hip, knee, and ankle joints following the trip perturbation at a 0.89 m/s velocity for 0.12 m. This induced trip caused subjects to walk forward for a 0.26 s duration. FindingsThe female group demonstrated significantly increased frontal plane ROM in the right hip (t = 2.71, p = 0.01) and left ankle (t = 2.16, p = 0.03) as well as increased sagittal plane ROM in the right (t = 2.07, p = 0.04) and left (t = 2.36, p = 0.02) ankles. There was a significant gender interaction on 3D body region (F = 6.84, p = 0.01) following the perturbation. InterpretationThere was a 3D gender difference on the lower limbs for balance control. The female group demonstrated increased sagittal motion in both ankles following a trip perturbation. In addition, their ROM increased on the dominant hip and non-dominant ankle in the frontal plane, which was compensated by step width for standing stability. Clinicians might want to consider the implications of gender differences on lower limb reaction patterns to help patients avoid potential injuries/falls.

Acquired multi-segment foot kinematics in haemophilic children, adolescents and young adults with or without haemophilic ankle arthropathy.

The ankle joint remains vulnerable in children with haemophilia and is the primary joint affected. The purpose of this study was to dynamically characterize the segmental foot and ankle kinematics of male children, adolescents and young adults with or without ankle arthropathy. The barefoot multi-segment foot kinematics of 70 ankles from 35 haemophilia subjects between 6 and 20years old were captured with the Rizzoli Multi-Segment Foot Model. Joint damage of the tibiotalar and subtalar joints was scored using the IPSG-MRI score. The feet of patients with or without evidence of ankle arthropathy were compared with those of matched typically developing boys via a nonpaired comparison. The differences between the affected and nonaffected sides of patients with unilateral ankle arthropathy were assessed using a paired comparison. Subjects without arthropathy demonstrated a nonsignificant trend towards a higher frontal plane range of motion (RoM) at the midfoot upon loading response and a lower sagittal plane RoM at the midfoot during midstance. No differences were observed between the affected side group and their matched control group. The affected side of unilaterally affected subjects exhibited a nonsignificant tendency towards a higher frontal plane RoM at the ankle joint upon loading response and terminal stance compared to the healthy side. Most patients maintained physiological rocker function of the ankle and had no (mal)adaptive motion patterns in the more distal joints of the foot. Therefore, established structural lesions may remain subclinical with respect to moderate functional activities like walking.

Anterior elbow release for post-traumatic flexion contractures in patients 21 years or younger

An elbow contracture in a young person can be a devastating problem. Significant contractures will lead to functional loss of the extremity. Appropriately performed contracture release can have profound implications on the overall well-being of the patient. The purpose of this study was to report improvements in sagittal-plane range of motion and the complication rate following an anterior elbow release for flexion contractures in patients 21 years or younger. We performed a retrospectivereview of 27 patients with a median age of 16.8 years who were treated surgically for elbow flexion contracture with an anterior approach. Follow-up was possible in 18 of these patients at an average of 31 months. An anterior approach was performed in all 18 patients, with 4 patients undergoing an additional posterior incision to address posterior structures limiting extension. Elbow extension improved by an average of 35°, from -54° to -19°. The mean total arc of elbow motion improved by 37°, from 65° to 102°. Two complications occurred: traction-related neurapraxia of the lateral antebrachial cutaneous nerve and transient neurapraxia of the posterior interosseous nerve. Elbow contracture release through an anterior approach is an acceptable surgical option. Significant improvement is obtained with a low risk of complications.

Improved Clinical and Functional Outcomes in Crouch Gait Following Minimally Invasive Hamstring Lengthening and Serial Casting in Children With Cerebral Palsy.

Serial extension casting represents a novel solution for addressing residual knee flexion contractures following hamstring lengthening in children with cerebral palsy. The purpose of this study was to investigate postoperative changes in patients following hamstring lengthening with a serial casting protocol. Measures from preoperative and postoperative gait analyses were reviewed retrospectively for 19 patients with cerebral palsy who underwent hamstring lengthening followed by serial extension casting. Postoperative changes in clinical, functional, and kinematic parameters were assessed using paired parametric methods. Improvements were measured in popliteal angle, knee contracture, peak stance phase knee extension, sagittal plane range of motion of the knee during walking, Gait Deviation Index, and pediatric outcomes data collection instrument Global score. Nearly 80% of the cohort (15/19 patients) demonstrated a significant or moderate response to the intervention, whereas 20% demonstrated no improvement. Of note, significantly increased anterior pelvic tilt was also observed. Hamstring lengthening combined with a serial casting protocol was associated with significant postoperative improvements in a range of clinical (eg, knee contracture), functional (eg, pediatric outcomes data collection instrument Global), and kinematic (eg, knee extension in stance) parameters. Improvements following this minimally invasive surgery were comparable to outcomes from procedures with higher complication rates. The Influence Of Concussion History On Landing Biomechanics In Adolescent Athletes: A Pilot Investigation.

Sport-related concussion (SRC) is now classified as a major public health concern affecting millions of athletes each year. Recent evidence suggests that previously concussed athletes are at greater risk for lower extremity (LE) injury beyond the resolution of traditional SRC assessment batteries. The current literature has yet to elucidate the biomechanical movement patterns of sport-specific activities that may provide rationale for LE injury risk following SRC. PURPOSE: To examine landing biomechanics in adolescent athletes who report a SRC history. METHODS: Three athletes with a SRC history (age: 11.3 ± 3.5 yrs; previous SRCs: 1.3 ± 0.6) and two athletes without a SRC history (age: 13.0 ± 4.2 yrs) completed drop-landings from 30 cm and 60 cm. Three dimensional kinematic data were measured via a 10-camera motion analysis system at 100 Hz, while kinetic data were collected at 1000 Hz with two adjacent force platforms. The preferred limb to initiate the drop-landing was used for data analysis in comparing landing mechanics between groups. Five landing trials were collected to ensure landing performance stability. The independent variables were group (concussion, control) and landing height (30, 60 cm), while the dependent variables included maximum vertical ground reaction force (vGRF), loading rate, and knee joint kinematic parameters during the landing tasks. RESULTS: Athletes with a previous SRC demonstrated a 7.9% increase (4.22 BWs vs 3.89 BWs) in maximum vGRF and 14.7% increase in loading rate (109.2 BWs / sec vs 93.2 BWs / sec) from the 60 cm height compared to control athletes. Additionally, previously concussed athletes demonstrated an 8.8 degrees (-4.9° vs 3.9°) and 11.2 degrees (-8.1° vs 3.1°) increase in knee abduction angle from the 30 cm and 60 cm heights compared to control athletes. Knee sagittal plane range of motion was decreased by 56.2% (52.0° vs 81.2°) from the 30 cm height and 30.3% (62.3° vs 81.1°) from the 60 cm height in athletes with a previous SRC. CONCLUSIONS: Preliminary analysis highlights that previously concussed adolescent athletes demonstrate landing mechanics that may increase LE injury risk. Specifically, athletes with a SRC history demonstrate sagittal and frontal plane knee motion during landings that are associated with greater risk of an anterior cruciate ligament injury.

The Influence of Ankle Braces on Functional Performance Tests and Ankle Joint Range of Motion.

The lateral ankle sprain is one of the most common lower-extremity injuries in sports. Previous research has found that some prophylactic ankle supports reduce the risk of recurrent ankle sprains and provide extra support to the joint. However, there is a continued concern that these supports may negatively influence performance. To determine if wearing an ankle brace influences athlete performance and ankle kinematics during functional performance tests. Repeated measures. University gymnasium. Male and female recreational or competitive athletes (n = 20). Participants performed 3 trials of a standing long jump, vertical jump, 40-yard sprint, and T-drill under each of the following 3 conditions: wearing traditional lace-up brace (brace 1), modified lace-up brace (brace 2), and no-brace. A 2-dimensional motion capture camera was used to measure ankle range of motion (ROM) in the sagittal plane during the vertical and standing long jumps and in the frontal plane during the cutting phase of the T-drill. Performance of each test and ankle ROM were compared between each of the braced conditions. Ankle braces did not influence performance in speed or agility functional performance tests (P > .05). Ankle braces negatively affected performance of the standing long jump (P = .01) and vertical jump (P = .01). There was no significant difference between brace or no-brace conditions in ankle inversion ROM during the T-drill (P > .05). Both brace conditions restricted ROM in the sagittal plane during the vertical and standing long jumps (P < .05). Braced conditions restricted sagittal plane ROM during the vertical jump and long jump. This decrease in ROM explains the decline in functional performance also seen during these tests.

Kinematic alignment in total knee arthroplasty better reproduces normal gait than mechanical alignment.

Kinematic alignment technique for TKA aims to restore the individual knee anatomy and ligament tension, to restore native knee kinematics. The aim of this study was to compare parameters of kinematics during gait (knee flexion-extension, adduction-abduction, internal-external tibial rotation and walking speed) of TKA patients operated by either kinematic alignment or mechanical alignment technique with a group of healthy controls. The hypothesis was that the kinematic parameters of kinematically aligned TKAs would more closely resemble that of healthy controls than mechanically aligned TKAs. This was a retrospective case-control study. Eighteen kinematically aligned TKAs were matched by gender, age, operating surgeon and prosthesis to 18 mechanically aligned TKAs. Post-operative 3D knee kinematics analysis, performed with an optoelectronic knee assessment device (KneeKG®), was compared between mechanical alignment TKA patients, kinematic alignment TKA patients and healthy controls. Radiographic measures and clinical scores were also compared between the two TKA groups. The kinematic alignment group showed no significant knee kinematic differences compared to healthy knees in sagittal plane range of motion, maximum flexion, abduction-adduction curves or knee external tibial rotation. Conversely, the mechanical alignment group displayed several significant knee kinematic differences to the healthy group: less sagittal plane range of motion (49.1° vs. 54.0°, p = 0.020), decreased maximum flexion (52.3° vs. 57.5°, p = 0.002), increased adduction angle (2.0-7.5° vs. - 2.8-3.0°, p < 0.05), and increased external tibial rotation (by a mean of 2.3 ± 0.7°, p < 0.001). The post-operative KOOS score was significantly higher in the kinematic alignment group compared to the mechanical alignment group (74.2 vs. 60.7, p = 0.034). The knee kinematics of patients with kinematically aligned TKAs more closely resembled that of normal healthy controls than that of patients with mechanically aligned TKAs. This may be the result of a better restoration of the individual's knee anatomy and ligament tension. A return to normal gait parameters post-TKA will lead to improved clinical outcomes and greater patient satisfaction. III.
