PETROLEUM GEOLOGY: TWO DECADES OF CHANGE

SOME TWENTY YEARS AGO, Dr E.N. Tiratsoo (whom I had known since the mid‐1930s) visited me at the Royal School of Mines, and told me of his plans to set up a new “journal of petroleum geology”.The Journal has now reached its twentieth volume.Thinking especially about the United Kingdom, the past twenty years have also seen the publication of the proceedings of four major conferences held in London, on the geological results of North Sea oil exploration, as well as other similarly‐based books from Norwegian sources. Somewhat earlier, the European Branch of the Organic Geochemistry Group of the Geochemical Society was formed. A European geophysics‐based society has also appeared, and a vigorous petroleum group was formed in the Geological Society in 1980. There has been a proliferation in literature (both books and journals) dealing with the geology of oil and gas. It is against this background that the Journal of Petroleum Geology has succeeded.

Recovery of horses from dysautonomia (grass sickness).

The outcome for 35 horses with chronic dysautonomia which were kept in the hospital at the Royal (Dick) School of Veterinary Studies and subsequently returned to their owners is recorded. They constituted 42.7 per cent of the 82 chronic cases seen between 1991 and 1994; the other 47 horses were euthanased while in hospital. Of the 35 animals returned to their owners four died and 27 were available for follow up; of these 27, 12 were working competitively and six were being trained for future competitive work. It takes at least a year before it is clear whether a horse can compete successfully again but all the surviving animals were capable of being ridden. Some of the horses suffered excessive sweating, had difficulty in swallowing some foodstuffs, or had coat changes for long periods after returning to a normal weight.

Hospital costs of treating haemophilic patients infected with HIV.

To calculate the costs of treating HIV-infected haemophilic patients. Two-year retrospective study of hospital-based resource use and costs, from April 1991 to March 1993. Haemophilia Centre and Haemostasis Unit, Royal Free Hospital and School of Medicine, London, UK. Sixty patients infected with HIV between October 1979 and July 1985. During the 2-year period a total of 1668 hospital visits were made by patients. The mean number of episodes per patient-year (PY) was 0.6 inpatient admissions, 11.5 outpatient visits and 1.8 day cases. The mean cost per PY was 32,528 pounds, with the majority of this spent on clotting factor concentrate products and haemophilia inpatient admissions (81%). A mean cost for HIV-related treatment of 6050 pounds was estimated. The additional cost incurred in switching this group of haemophilic patients from intermediate-purity factor concentrate to high-purity products was 8614 pounds per PY. When clotting factor concentrate and expenditure on haemophilia-related inpatient admissions were excluded, the mean cost of treating HIV infection per PY was 6065 pounds, varying with CD4+ count (< or = 50 x 10(6)/l, 13,093 pounds; 51-200 x 10(6)/l, 6521 pounds; 201-500 x 10(6)/l, 2848 pounds; > 501 x 10(6)/l, 1497 pounds). CD4+ count may be used as a marker of costs of HIV infection. The HIV-related cost estimates can be used for the planning of current and future hospital-based care in the National Health Service in the United Kingdom. The switch from intermediate-purity factor concentrate to high-purity products has increased the mean HIV-related cost per PY of treating haemophilic patients infected with HIV.
