The Role of Platelet-Rich Plasma Injection for Muscle Strains in Athletes.

Muscle tears/strains are among the most common musculoskeletal injuries, posing a serious challenge for sports medicine. Aiming to reduce the time to return to play and the rate of reinjuries, apart from the traditional conservative treatments and rehabilitation protocols, new and innovative therapeutic options have emerged, particularly platelet-rich plasma (PRP). This study aims to present the available evidence regarding PRP injection for the treatment of muscle strains in athletes. Two databases were searched for articles published between January 2012 and December 2022 in Portuguese or English. The query used for the PubMed database was ("Muscles/injuries"[Mesh]) AND ("Athletes"[Mesh] OR "Athletic Injuries"[Mesh]) AND "Platelet-Rich Plasma"[Mesh], while for the Web of Science database the search was performed for "Platelet-rich plasma" AND "Muscle injuries" AND ("Athletes" OR "Athletic injuries"). Eleven studies involving athletes diagnosed with muscle injuries who received treatment with PRP injection alone, or in combination with traditional conservative treatment, compared to a control group, were included. Four randomized controlled trials, four systematic reviews/meta-analyses, two retrospective studies, and one comparative study were included. Current evidence from the highest-quality studies does not support the hypothesis of reduction of time to return to play and the rate of reinjuries after PRP injection, even though some studies reported positive results. However, the available evidence suggests that PRP might have a beneficial effect on the pain perceived by athletes following an acute muscle strain. It is challenging to arrive at definitive conclusions and translate these findings into a clinical context for treating muscle strains in athletes. The existing trials present several inconsistencies and limitations, with a heterogeneous set of patients and injuries, as well as the use of different and inconsistent methods for preparing, administering, and measuring the effects of PRP. To achieve consistent outcomes, standardizing PRP administration procedures is essential.

The role of PRP injection in the management of plantar fasciitis: The analytical prospective study in 60 patients

Introduction: Plantar fasciitis is the most common overuse injury. Steroid injections are a popular short-term method of treating it. However lack of an inflammatory process histologically in plantar fasciitis questions its mode of action. Recently, promising results were reported with the use of Platelet-rich plasma (PRP) injections for treating muscle and tendon injuries and degeneration. The rationale lies in reversing the blood ratio by decreasing RBCs (less useful in the healing process) to 5%, and increasing platelets to 94% to stimulate recovery. We conducted a study with an aim to assess the efficacy of PRP injections for treating chronic plantar fasciitis. Materials and Methods: Study was conducted in a private medical college in Andhra Pradesh, including 44 male and 16 females, attending orthopaedics OPD from November 2016 to October 2018, suffering with chronic plantar fasciitis. From each patient 10 cc of whole blood was drawn and PRP was prepared by differential centrifugation and injected locally. They were reviewed at 4 weeks 8 weeks 3 months and 6 months. Injection was repeated if no significant improvement seen at 4th week. The result assessed using Base line Visual Analogue Score (VAS), Foot and Ankle Disability Index (FADI) score.Results: There was highly significant improvement in the VAS and FADI score on each successive visit, when compared with baseline scores.Conclusion: PRP injection has a beneficial effect in treatment of plantar fasciitis.

Background: platelet-rich plasma (PRP) is a human plasma product enriched by platelets, growth factors, and fibrinogen with high hemostatic and healing properties. Objectives: The aim of this study was to evaluate the effect of autologous PRP on wound healing in women undergoing modified radical mastectomy (MRM). Patients and Methods: In this randomized and controlled trial, 100 females were admitted to El Sayed Galal hospital, Egypt from October of 2017 to August of 2018 for MRM. The patients were randomly assigned to two groups. The intervention group received PRP after surgery, whereas the control group received the usual care. All patients were evaluated at baseline, five days, and 2 weeks after surgery. The primary endpoint used the REEDA scale for assessing the changes in wound healing. The secondary outcome measures used were the Vancouver scar scale (VSS). All scale scores were analyzed using a repeated measures test for the variance. Results: At the end of the study, the PRP group showed a greater reduction in the edema ecchymosed discharge approximation (REEDA) score compared to the control group (85.5% reduction in the PRP group; 72% in the control group) (P < 0.001). Furthermore, patients treated with PRP experienced a 93% reduction in the VAS score at the end of follow-up, but the control group only observed a 79% reduction (P < 0.001). Conclusions: It seems that applying PRP is an effective therapeutic approach for wound healing, and faster wound healing is expected due to the presence of more platelets and growth factors.
