Towards accurate screening and prevention for PTSD (2-ASAP): protocol of a longitudinal prospective cohort study

BackgroundEffective preventive interventions for PTSD rely on early identification of individuals at risk for developing PTSD. To establish early post-trauma who are at risk, there is a need for accurate prognostic risk screening instruments for PTSD that can be widely implemented in recently trauma-exposed adults. Achieving such accuracy and generalizability requires external validation of machine learning classification models. The current 2-ASAP cohort study will perform external validation on both full and minimal feature sets of supervised machine learning classification models assessing individual risk to follow an adverse PTSD symptom trajectory over the course of 1 year. We will derive these models from the TraumaTIPS cohort, separately for men and women.MethodThe 2-ASAP longitudinal cohort will include N = 863 adults (N = 436 females, N = 427 males) who were recently exposed to acute civilian trauma. We will include civilian victims of accidents, crime and calamities at Victim Support Netherlands; and who were presented for medical evaluation of (suspected) traumatic injuries by emergency transportation to the emergency department. The baseline assessment within 2 months post-trauma will include self-report questionnaires on demographic, medical and traumatic event characteristics; potential risk and protective factors for PTSD; PTSD symptom severity and other adverse outcomes; and current best-practice PTSD screening instruments. Participants will be followed at 3, 6, 9, and 12 months post-trauma, assessing PTSD symptom severity and other adverse outcomes via self-report questionnaires.DiscussionThe ultimate goal of our study is to improve accurate screening and prevention for PTSD in recently trauma-exposed civilians. To enable future large-scale implementation, we will use self-report data to inform the prognostic models; and we will derive a minimal feature set of the classification models. This can be transformed into a short online screening instrument that is user-friendly for recently trauma-exposed adults to fill in. The eventual short online screening instrument will classify early post-trauma which adults are at risk for developing PTSD. Those at risk can be targeted and may subsequently benefit from preventive interventions, aiming to reduce PTSD and relatedly improve psychological, functional and economic outcomes.

Development and usability testing of diabetes risk calculator (diacal): a health education application

Despite the growing popularity of the mobile health application, the application that addressed to calculated diabetes risk is still limited. DiaCal was developed to prevent type 2 diabetes mellitus by screening and early detection approach. This study aimed to develop and examine the usability of DiaCal smartphone application-based education. This study was conducted with a cross-sectional approach. The framework of this application is based on the American Diabetes Association diabetes risk screening instrument. The development of the DiaCal was divided into three phases: preparation, design, and piloting. System Usability Testing (SUS) instruments used to examine the user-level acceptance of this application. DiaCal app developed in android platform core modules: a) data entry, b) conversion and calculation, c) output of the risk assessment, d) education. Twenty respondents were recruited in this study to evaluate DiaCal through SUS instrument. The average adjective range score is 85.25 which indicates that the DiaCal application is in the “excellent” category and the grade level scale is in “A+”. This study showed a significant usability and acceptability of DiaCal in terms of effectiveness, efficiency, and satisfaction.

Predicting Outcomes in Frail Older Community-Dwellers in Western Australia: Results from the Community Assessment of Risk Screening and Treatment Strategies (CARTS) Programme.

Understanding risk factors for frailty, functional decline and incidence of adverse healthcare outcomes amongst community-dwelling older adults is important to plan population-level health and social care services. We examined variables associated with one-year risk of institutionalisation, hospitalisation and death among patients assessed in their own home by a community-based Aged Care Assessment Team (ACAT) in Western Australia. Frailty and risk were measured using the Clinical Frailty Scale (CFS) and Risk Instrument for Screening in the Community (RISC), respectively. Predictive accuracy was measured from the area under the curve (AUC). Data from 417 patients, median 82 ± 10 years, were included. At 12-month follow-up, 22.5% (n = 94) were institutionalised, 44.6% (n = 186) were hospitalised at least once and 9.8% (n = 41) had died. Frailty was common, median CFS score 6/9 ± 1, and significantly associated with institutionalisation (p = 0.001), hospitalisation (p = 0.007) and death (p < 0.001). Impaired activities of daily living (ADL) measured on the RISC had moderate correlation with admission to long-term care (r = 0.51) and significantly predicted institutionalisation (p < 0.001) and death (p = 0.01). The RISC had the highest accuracy for institutionalisation (AUC 0.76). The CFS and RISC had fair to good accuracy for mortality (AUC of 0.69 and 0.74, respectively), but neither accurately predicted hospitalisation. Home assessment of community-dwelling older patients by an ACAT in Western Australia revealed high levels of frailty, ADL impairment and incident adverse outcomes, suggesting that anticipatory care planning is imperative for these patients.

Pediatric psychoSocial Risk Index: Factor Analysis and Item Reduction Study.

The Pediatric psychoSocial Risk Index (PSRI) is psychosocial risk screening instrument for health practitioners. The objective of this study was to confirm validity evidence of a truncated version of PSRI. PSRI was completed initially by 100 parents of children aged 0 to 18 years admitted to a tertiary hospital; 50 parents repeated the PSRI 3 days later. Analysis includes principal component analysis (PCA) to include the least number of items that explain the most variance in a shortened version of PSRI as well as confirming test-retest reliability and internal consistency of the shortened instrument. PSRI originally had 86 items, 85 close-ended items were analyzed. Three items were excluded because of missing test-retest data. Item reduction resulted in truncation of 16 items; 66 items remained. A Kaiser-Mayer-Orkin test of sampling adequacy resulted in reduction of 14 items; 52 items remained. Initial PCA led to reduction of 26 items. The PCA was rerun on remaining items, resulting in reduction of 6 further items; 18 items remained. Two items with >10% missingness were removed leaving 16 items in the final PSRI. Test-retest reliability was 0.98 and mean within-person across-item reliability was 0.95. Cronbach α was 0.9. Remaining items represented 9 social risk themes: food insecurity, medical complexity, home environment, behavioral issues, financial insecurity, parenting confidence, parental mental health, social support, and unmet medical needs. PSRI was reduced from 86 to 16 items with high internal consistency and reliability. PSRI demonstrates adequate validity supporting practitioners to screen families about their psychosocial risk.

Interrater reliability of the violence risk assessment checklist for youth: a case vignette study

BackgroundFacilities providing health- and social services for youth are commonly faced with the need for assessment and management of violent behavior. These providers often experience shortage of resources, compromising the feasibility of conducting comprehensive violence risk assessments. The Violence Risk Assessment Checklist for Youth aged 12–18 (V-RISK-Y) is a 12-item violence risk screening instrument developed to rapidly identify youth at high risk for violent behavior in situations requiring expedient evaluation of violence risk. The V-RISK-Y instrument was piloted in acute psychiatric units for youth, yielding positive results of predictive validity. The aim of the present study was to assess the interrater reliability of V-RISK-Y in child and adolescent psychiatric units and acute child protective services institutions.MethodsA case vignette study design was utilized to assess interrater reliability of V-RISK-Y. Staff at youth facilities (N = 163) in Norway and Sweden scored V-RISK-Y for three vignettes, and interrater reliability was assessed with the intraclass correlation coefficient (ICC).ResultsResults indicate good interrater reliability for the sum score and Low-Moderate-High risk level appraisal across staff from the different facilities and professions. For single items, interrater reliability ranged from poor to excellent.ConclusionsThis study is an important step in establishing the psychometric properties of V-RISK-Y. Findings support the structured professional judgment tradition the instrument is based on, with high agreement on the overall risk assessment. This study had a case vignette design, and the next step is to assess the reliability and validity of V-RISK-Y in naturalistic settings.

Pediatric Dysphagia Risk Screening Instrument (PDRSI) in Children With Cerebral Palsy.

This study aimed to evaluate whether the Pediatric Dysphagia Risk Screening Instrument (PDRSI) was a suitable test for children with cerebral palsy (CP) and assess the instrument's Turkish validity and reliability. One-hundred twenty-six children with CP participated in this study. "Cronbach's alpha (ɑ)," "Cronbach's ɑ when one item is deleted," "inter-item correlation," and "corrected item-to-total correlation" were used to assess internal consistency. In addition, inter-rater agreement tests (Cohen's kappa coefficient) were conducted for reliability. Construct validity was used to assess the validity. Moreover, flexible fiberoptic endoscopic evaluation of the swallowing method was used to describe the receiver operating characteristic curve analysis and calculate the sensitivity and specificity of T-PDRSI. It was found that the PDRSI had adequate validity and reliability. The PDRSI can be used in children with CP as a valid and reliable instrument with high sensitivity and specificity.

The objective of this study was to validate the Multiprofessional Screening Instrument for Broncho-aspiration Risk in Hospital Environment, which is aimed at the elderly population, based on response processes. Judges applied the instrument to different patients and randomly selected. After the application, the judges were interviewed so that it was possible to verify their impression regarding the relevance of the items about their interpretation of the written content, as well as grammatical and semantic issues. Suggestions for adding alternative questions and answers were considered, as well as proposals for adapting the questions that made up the instrument. Non-verbal reactions, such as facial expressions that suggested doubts or hesitations, by the judges concerning the instrument were also analyzed. The agreement of the judges concerning each item of the device was calculated by the Content Validity Index (CVI) and by the Intraclass Correlation Coefficient (ICC), and their results showed a high level of agreement. Through the suggestions of the judges, a new version of the Multi-professional Screening Instrument for the Risk of Broncho-aspiration in a Hospital Environment in the Elderly was elaborated. The results obtained showed that the validity of the Multi-professional Instrument for Screening the Risk of Broncho-aspiration in the Hospital Environment with the elderly population, based on the response processes, was achieved and makes it a promising device to assist professionals in hospital care for the elderly.

Validation of the Paykel Suicide Scale and the Plutchik Suicide Risk Scale in Spanish Women during the Perinatal Period

Introduction. No specific instruments are available to detect the risk of suicide in women during the perinatal period. Suicide in perinatal women is little studied despite being one of the main causes of maternal mortality. Its prevalence has increased by 100% in the United States in a decade. Moreover, it has consequences for the mother and the newborn. Objective. To validate the Paykel Suicide Scale and the Plutchik Suicide Risk Scale in Spanish women during the perinatal period. Materials and Methods. Cross‐sectional study with Spanish women who were pregnant or who had given birth less than 18 months ago. Information was collected on sociodemographic variables, obstetric variables, and the newborn. A questionnaire that included both suicide scales and the Edinburgh Postnatal Depression Scale (EDPS) was used. An exploratory factor analysis (EFA), convergent and criterion validation, as well as internal consistency analysis, were performed. Results. In total, 237 women participated. The EFA determined that in the Plutchik Suicide Risk Scale 4 components explained 54.8% of the variance, while in the Paykel Scale, a single component explained 53.0% of the variance. In the convergent validity, the risk of suicide or suicidal ideation was related to the Plutchik Suicide Risk Scale and the Paykel Suicide Scale, respectively, with the variables: perception of low social support, risk of intimate partner violence, level of anxiety, history of mental pathology, and having experienced a stressful event in the last year, among others (p &lt; 0.05). The area under the receiver operating characteristic curve for the Plutchik Suicide Risk Scale scores was 0.88 (95% CI: 0.82–0.93), and for the Paykel Scale, it was 0.90 (95% CI: 0.85–0.95). The value of Cronbach’s alpha (α) was placed for the Plutchik Suicide Risk Scale at 0.806 and for the Paykel Suicide Scale at 0.766. Conclusion. Both scales presented adequate psychometric characteristics to be used as a screening instrument for suicide risk in Spanish women in the perinatal period.

IS THERE AGREEMENT BETWEEN THE GLIM AND THE NRS CRITERIA IN THE NUTRITIONAL DIAGNOSIS OF HOSPITALIZED SURGICAL PATIENTS? PILOT STUDY.

There is no consensus on which nutritional diagnosis methods are most relevant in the hospital clinical practice. This study investigated the agreement between the global leadership initiative on malnutrition (GLIM) criterion and the nutritional risk screening (NRS) instrument for the nutritional diagnosis of in-patients. Cross-sectional study with 95 hospitalized surgical patients. Clinical data, nutritional risk using the NRS and malnutrition using the GLIM criteria were evaluated. The data were analyzed using the chi-square, Mann-Whitney, McNemar and Kappa coefficient tests. There was good agreement between the two methods (Kappa=0.6067). Patients who were malnourished according to the GLIM or at nutritional risk by NRS were older (P=0.0461 by GLIM and P=0.0200 by NRS) and had a higher diagnosis rate of neoplasms (38.5%, P=0.0006 by GLIM and 32.7%, P=0.0030 by NRS). The GLIM criterion identified a lower percentage of patients with malnutrition (41.05%) in relation to the NRS regarding patients with nutritional risk (54.7%). The GLIM criteria and the NRS instrument are concordant methods for diagnosing malnutrition and nutritional risk in hospitalized surgical patients respectively.

DERSR-B. A Brief Scale for Detection of Emotional Dysregulation Risk

Emotional dysregulation (ED) is related to problems in understanding, perceiving, and regulating emotions. The aim is to find the psychometric properties of an instrument that measures ED and classifies the high/low ED group membership with the least possible error. For statistical purposes (factor analysis), two independent samples of males and females (n1 = 476) and (n2 = 562) were obtained, with ages sample 1 (15 -19 years; M= 15.8; SD=0.71) and sample 2 (15-19 years; M=15.6; SD= 0.69). Three factors were formed by sex, males with 14 items and females with 13 items, each loading on a single factor (total α=0.71 - 0.78 ɷ =0.67- 0.79 females; α= 0.70 - 0.79 ɷ=0.73 - 0.75 males) and good fit indices. In sum, a validated cut version instrument (DERSR-B), a risk screening instrument, was obtained.

Assessment of incidence of cerebral vascular diseases and prediction of stroke risk in chronic obstructive pulmonary disease patients using multimodal biomarkers.

Early assessment of cerebrovascular disease in chronic obstructive pulmonary disease (COPD) patients is an important issue for a favorable influence on the quality of life. This cross-sectional case-control study was conducted on 38 eligible COPD patients (mean age 55.5 ± 11.5, 25 males, and 13 females) and 26 age-/sex-matched healthy controls. All participants were subjected to stroke risk screening instruments that included the Stroke Riskometer™, the Framingham 10-Year Risk Score, the stroke risk screening tool (the Department of Disease Control of Thailand), the My Risk Stroke Calculator, and Q Stroke. Radiologically, diffusion tensor imaging (DTI) and echo-gradient MRI (T2 star) T2 star imaging were done. Color-coded duplex sonography was done. Laboratory investigations included C-reactive protein (CRP), serum amyloid A, plasma fibrinogen level, serum IL6, 8-Isoprostane, vWF and urinary albumin creatinine ratio. Stroke risk screening instruments revealed a significant increase in COPD patients. DTI showed a significant bilateral reduction in fractional isotropy and a significant bilateral increase in mean diffusivity of white matter through many areas in COPD patients. Patients also had a significant increase of intima-media thickness, presence of atherosclerotic focal thicknesses or plaques on duplex sonography. There was a significant elevation of CRP, serum amyloid A, plasma fibrinogen level, serum IL6, 8-isoprostane, von Willebrand factor (vWF), and urinary albumin creatinine ratio in COPD patients. COPD patients had an increased risk for stroke that could be assessed on stroke risk screening instruments, DTI, T2 star, duplex sonography, and laboratory investigation and could be correlated with the severity of the disease.

Validation of an electronic frailty index with electronic health records: eFRAGICAP index

ObjectiveTo create an electronic frailty index (eFRAGICAP) using electronic health records (EHR) in Catalunya (Spain) and assess its predictive validity with a two-year follow-up of the outcomes: homecare need, institutionalization and mortality in the elderly. Additionally, to assess its concurrent validity compared to other standardized measures: the Clinical Frailty Scale (CFS) and the Risk Instrument for Screening in the Community (RISC).MethodsThe eFRAGICAP was based on the electronic frailty index (eFI) developed in United Kingdom, and includes 36 deficits identified through clinical diagnoses, prescriptions, physical examinations, and questionnaires registered in the EHR of primary health care centres (PHC). All subjects > 65 assigned to a PHC in Barcelona on 1st January, 2016 were included. Subjects were classified according to their eFRAGICAP index as: fit, mild, moderate or severe frailty. Predictive validity was assessed comparing results with the following outcomes: institutionalization, homecare need, and mortality at 24 months. Concurrent validation of the eFRAGICAP was performed with a sample of subjects (n = 333) drawn from the global cohort and the CFS and RISC. Discrimination and calibration measures for the outcomes of institutionalization, homecare need, and mortality and frailty scales were calculated.Results253,684 subjects had their eFRAGICAP index calculated. Mean age was 76.3 years (59.5% women). Of these, 41.1% were classified as fit, and 32.2% as presenting mild, 18.7% moderate, and 7.9% severe frailty. The mean age of the subjects included in the validation subsample (n = 333) was 79.9 years (57.7% women). Of these, 12.6% were classified as fit, and 31.5% presented mild, 39.6% moderate, and 16.2% severe frailty. Regarding the outcome analyses, the eFRAGICAP was good in the detection of subjects who were institutionalized, required homecare assistance, or died at 24 months (c-statistic of 0.841, 0.853, and 0.803, respectively). eFRAGICAP was also good in the detection of frail subjects compared to the CFS (AUC 0.821) and the RISC (AUC 0.848).ConclusionThe eFRAGICAP has a good discriminative capacity to identify frail subjects compared to other frailty scales and predictive outcomes.

124. Validation of the Ask Suicide-Screening Questions (ASQ) as a Suicide Screening Tool for Adolescents in the Outpatient Population of a Tertiary Hospital

Suicide is the second leading cause of death among people 15-29 years of age according to the 2014 Global Report on Preventing Suicide by the WHO. A short screening tool that is translated and validated is needed for early screening of adolescents at risk of suicide. This will be the first study to translate the ASQ or the Ask Suicide-Screening Questions, a 4-item suicide risk screening instrument, into Filipino and then validate it among Filipino adolescents. It will also look into the validity of the English version of the ASQ among Filipino adolescents.

Promoting Success and Persistence in Pandemic Times: An Experience With First-Year Students.

The transition and adaptation of students to higher education (HE) involve a wide range of challenges that justify some institutional practices promoting skills that enable students to increase their autonomy and to face the difficulties experienced. The requirements for this adaptation were particularly aggravated by the containment and sanitary conditions associated with coronavirus disease 2019 (COVID-19). With the aim of promoting academic success and preventing dropout in the first year, a support program was implemented for students enrolled in two courses in the area of education at a public university in northern Portugal during the first semester of 2020/2021. Three sessions of 50/60 min were implemented, namely, the first session focused on the verbalization of the demands, challenges, and difficulties of the transition, and the second and third sessions focused on the difficulties of academic adaptation and academic performance. Data from a dropout risk screening instrument and from the activities performed during sessions were analyzed. The main results point to student satisfaction with the content and the activities of the sessions and their usefulness. Students report not only high satisfaction levels with HE attendance, but also some emotional exhaustion due to academic activities. The continuity of the program is recommended with some improvements in its planning to ensure a more definitive version of the program in the next two years.

Understanding the factors which affect students' school achievement is an important knowledge source for strategic planning and encouraging changes in education. In this paper, we focused on examining the factors related to personal characteristics. The aim of this research was to determine the relation between school failure and achievements in certain functional abilities, such as: academic skills (reading, writing), attention, communication (receptive and expressive speech), class participation, and behavior. The sample included 195 younger school-age children of both genders. Students' functional abilities were assessed by the S.I.F.T.E.R. scale (Screening Instrument for Targeting Educational Risk). The results showed that school achievement correlated with all assessed functional abilities. The coefficient of multiple determination showed that 48% of individual differences in children's school achievement can be explained by individual differences in the given model of functional abilities. It should also be emphasized that only two functional abilities, attention and communication, were singled out as statistically significant particular predictors.

Opinions on Youth Suicide Risk Screening from Individuals with Neurodevelopmental Disabilities and Their Therapists: A Pilot Study

Background: Individuals with neurodevelopmental disorders (NDD) are at increased risk for suicide, yet little work has been done to address the specific needs for this population. Specifically, there are no validated suicide risk screening instruments and processes for individuals with NDD; this study aimed to assess the opinions of individuals with NDD and their therapists on suicide risk screening in order to inform best practices for screening. Method: A pilot study was launched to qualitatively evaluate processes and instruments that may be used in future studies on suicide screening risk in NDDs. Participants and their therapists were surveyed after filling out suicide risk screening instruments and provided qualitative feedback on their opinions of screening for suicide risk. Results: Most participants (9/15) reported positive experiences of being screened for suicide risk. Additionally, almost all therapists (14/15) were in support of suicide risk screening. Several themes, such as interpersonal benefits, emerged as reasons for supporting screening. Conclusions: The findings from this pilot study provide initial qualitative evidence that many individuals with NDD and their therapists would be comfortable with and are in support of suicide risk screening for this population. Screening tools to guide clinicians on how to ask about suicide risk are needed and appear to be desired by clinicians on the frontlines of mental health treatment for people with NDD.

Improving Psychosocial Risk Assessment and Service Provision for Craniofacial Team Patients: A Quality Improvement Project.

To improve psychosocial risk assessment and service provision for children with craniofacial conditions presenting for annual interdisciplinary team visits. Institute for Healthcare quality improvement model. U.S. pediatric academic medical center. Caregivers of children ages 0-17 years with craniofacial conditions presenting for 1692 team visits between August 2017 and July 2019. Key drivers included: (1) standardizing pre-visit triage processes; (2) administering the Psychosocial Assessment Tool-Craniofacial Version (PAT-CV); (3) utilizing PAT-CV scores in real time to add patients to psychosocial provider schedules; and (4) family education. Interventions included improving patient screening, increasing PAT-CV completion rate, altering clinic flow, providing patient and parent education about psychosocial services, and altering team member roles to fully integrate PAT-CV administration and scoring in the clinic. The primary outcome was the percentage of patients identified for psychosocial consultations via nurse triage, PAT-CV score, family or provider request who completed consultations. The secondary outcome was the percentage of patients completing needed psychosocial consultations based on elevated PAT-CV scores. Use of the PAT-CV resulted in an increase in the percentage of patients with elevated psychosocial risk who received a psychosocial consultation from 86.7% to 93.4%. The percentage of children receiving psychosocial consultation at their annual team visit due to elevated PAT-CV scores increased from 72% to 90%. Integrating a validated psychosocial risk screening instrument can improve risk identification and psychosocial consultation completion. A combination of risk screening approaches may be indicated to identify patients in need of psychosocial services.

A Comparison of Suicide Attempt Histories of Pediatric and Adult Medical Inpatients and Implications for Screening

Objective A suicide attempt is the most potent predictor of future suicidal behavior, yet little is known about how to manage and respond to reports of attempt histories in hospitalized medical patients. This study aims to describe the prevalence and characteristics of pediatric and adult medical inpatients who report a past suicide attempt. Method Participants were medical inpatients, aged 10–93 years, enrolled in two suicide risk screening instrument validation studies. Participants completed the Ask Suicide-Screening Questions (ASQ) and the Patient Health Questionnaire (PHQ). Results A total of 1324 medical inpatients (624 pediatric, 700 adult) completed the ASQ, with 114 participants (8.6%) reporting a past suicide attempt (51 pediatric; 63 adults). Comparing youth to adults, there was no significant difference between attempt rates (χ 2=0.29, p = 0.59). Youth with a past attempt were significantly more likely to report past week suicidal ideation (OR = 28.22; 95% CI = 5.90, 135.06) and have a history of mental health care (OR = 9.11; 95% CI = 2.59–32.10), compared to those without a past attempt. Adults with a past attempt were significantly more likely to screen positive for depression, compared to those without attempt histories (OR = 5.00; 95% CI = 2.31–10.83). Conclusions Nearly 9% of hospitalized medical patients endorsed a past suicide attempt when screened. Since adolescence is a critical time for detecting suicide risk, screening that includes past suicidal behavior may be an important means to identify youth with recent suicidal thoughts. By assessing recency of suicide attempts in adults, medical settings may optimize the effectiveness of how positive suicide risk screens are managed. HIGHLIGHTS Roughly 9% of medical patients reported a past suicide attempt when screened. Adolescence is a critical time for detecting suicide risk and intervening. Assessing past suicide attempts in adults can help with managing positive screens.
