Risk Factors of Catheter-Related Infection in Unplanned Extubation of Totally Implantable Venous-Accessportsin Tumor Patients.

Background Totally implantable intravenous ports (TIVAPs) are mostly used for long-term intravenous infusion therapy in cancer patients and can be left in the body for long periods of time for easy management, making them a simple and safe infusion device. Although the risks associated with long-term retention of fully implantable IV ports are less than those associated with other intravenous catheters, various complications may still occur at the time of implantation or during long-term use. Purpose To provide a scientific basis for clinical reduction of implantable intravenous port-associated infection complications by studying the risk factors for catheter-associated infection complications in patients applying implantable intravenous ports. Methods A retrospective study was conducted on oncology patients treated with TIVAP at our hospital between January 2017 and November 2021, with a review of patients who were unplanned for extubation. Their demographic data, underlying disease status, and surgery-related data were counted to summarize and analyze the complications and related influencing factors of implantation and postimplantation. Results A total of 70 individuals with a mean age of 56.49 ± 12.19 years were included in the study. Among them, 39 were male and 64 had the highest percentage of epithelial tumors, followed by tumors of the lymphopoiesis system and mesenchymal tumors with 4 and 2 cases, respectively. Forty-eight of these patients did not have their ports removed as planned due to the occurrence of catheter-related hematogenous infections. In univariate analysis, BMI and neutropenia were risk factors for catheter-associated infections. In the multivariate analysis, BMI (OR = 1.38, 95% CI: 1.07–1.78, p=0.013) was an independent risk factor for catheter-associated infections. Conclusions The overall complication rate of fully implanted intravenous ports was high, but most complications improved with symptomatic management, and no deaths due to port complications were identified. Infection was the most common complication, with catheter-associated bloodstream infection being the most common cause of unplanned port extraction. Patients with a higher BMI were at high risk of developing implantable IV port-associated infections, which may be an independent risk factor for implantable IV port-associated infections.

Safety and Effectiveness of Palliative Tunneled Peritoneal Drainage Catheters in the Management of Refractory Malignant and Non-malignant Ascites.

To determine the safety and effectiveness of tunneled peritoneal catheters in the management of refractory malignant and non-malignant ascites. An IRB-approved retrospective review was undertaken of patients who underwent ultrasound and fluoroscopy-guided tunneled peritoneal catheter placement for management of refractory malignant or non-malignant ascites between January 1, 2009, and March 14, 2014. A total of 137 patients (76M/61F, mean age 62.9years) underwent tunneled peritoneal catheter placement for refractory malignant (N=119; 86.9%) or non-malignant (N=18; 13.1%) ascites. Technical success was 100% with no immediate complications. Nineteen patients (13.9%) experienced a total of 11 minor and 12 major complications. Nine patients developed a catheter-associated infection. The remaining complications included leakage at the dermatotomy site (N=8), catheter dislodgement (N=2), obstruction (N=2), and groin pain (N=2). Patients who developed a catheter-associated infection had a significantly longer catheter dwell time compared to those who did not develop an infection (median, 96.5 vs. 20days; p<0.01). Nine patients (6.6%) were lost to follow-up. Of the remaining 128 patients, 125 died and the majority had a catheter in place (90.4%) at the time of death. There was one catheter-associated death (bacterial peritonitis; 0.8%). The median time from catheter placement to death was significantly shorter in patients with malignant versus non-malignant ascites (18.5 vs. 85days; p<0.0001). Tunneled peritoneal drainage catheters are effective and relatively safe in the management of malignant and non-malignant ascites. Longer catheter dwell time may be a risk factor for catheter-associated infection, particularly in patients with a longer anticipated survival in the palliative setting.

Diabetes: a risk factor for catheter-associated infections.

The incidence of infectious complications associated with continuous regional anesthesia techniques is a matter of concern. Our objective was to determine whether patients suffering from diabetes are at an increased risk of catheter-related infectious complications. The German Network for Regional Anaesthesia database was analyzed between 2007 and 2012. After proof of plausibility, data of 36,881 patients undergoing continuous regional anesthesia were grouped in I: no diabetes (n = 32,891) and II: any diabetes (n = 3990). The analysis focused on catheter-related infections after strict definition. Differences among the groups were tested with t and χ tests. Odds ratios were calculated with logistic regression and adjusted for potential confounders. Patients with a diagnosis of diabetes had an increased incidence of catheter-related infections (no diabetes 3.0% vs any diabetes 4.2%; P < 0.001). Among all patients, diabetes remained an independent risk factor for infections for all sites after the adjustment for potential confounders (odds ratio [OR] = 1.26; 95% confidence interval [95% CI], 1.02-1.55; P = 0.036). The risk of infection was significantly increased in peripheral catheters only in the lower limb (adjusted OR = 2.42; 95% CI, 1.05-5.57; P = 0.039). If neuraxial catheters were used, the risk was significantly increased only in lumbar epidural (adjusted OR = 2.09; 95% CI, 1.18-3.73; P = 0.012) for diabetic patients compared with nondiabetic patients. The presence of diabetes is associated with an increased risk for catheter-related infections in lower limb and lumbar epidural. Specific care should be taken to avoid and detect infections in this population.

Severe neutropenia at time of port insertion is not a risk factor for catheter‐associated infections in children with acute lymphoblastic leukemia

The objective of this study was to determine whether severe neutropenia on the day of port-a-catheter (PORT) insertion was a risk factor for catheter-associated infection (CAI) in children with acute lymphoblastic leukemia (ALL). This was a retrospective study of children with ALL who had a PORT insertion between January 2005 and August 2008. Early (≤ 30 days) and late (>30 days) postprocedure complications were reviewed. The length of follow-up ranged between 7 months and 42 months. In total, 192 PORTs were inserted in 179 children. There were 43 CAIs (22%), and the infection rate was 0.35 per 1000 catheter-days. The CAI rate (15%) in children who had severe neutropenia on the day of the procedure did not differ statistically from the CAI rate (24%) in children who did not have severe neutropenia (P = .137). Conversely, patients with severe neutropenia who had a CAI were more likely to have their PORT removed (P = .019). The most common organisms to cause catheter removal were coagulase-negative Staphylococcus and Staphylococcus aureus. Patients with high-risk ALL had a statistically significant higher incidence of late CAI than patients with standard-risk ALL (P = .012). Age (P = .272), positive blood culture preprocedure (P = 1.0), and dexamethasone use (P = .201) were not risk factors for CAI. Patients who had an early CAI did not have a greater chance of having a late CAI. The catheter infection-free survival rate at 1 year was 88.6%. The current results indicated that severe neutropenia on the day of PORT insertion does not increase the risk of CAI in children with ALL.

To determine if severe neutropenia (neutrophil count < 500 cells/mm3) on the day of port-a-catheter (PORT) insertion was a risk factor for catheter-associated infection (CAI) in children with Acute Lymphoblastic Leukemia (ALL). We also evaluated the incidence of CAI and wound dehiscence and the role of other potential risk factors for CAI.

Port-A-Catheter Infection Rate and Associated Risk Factors in Children Diagnosed with Acute Lymphoblastic Leukemia.

Abstract 987Poster Board I-9 Background:Port-a-catheters are commonly used as vascular access device in children with Acute Lymphoblastic Leukemia (ALL) requiring long-term chemotherapy. Literature suggests that surgical procedures should not be performed in patients with very low neutrophil counts. Neutropenia, however, is very common in patients with ALL putting them at increased risk for infection and impaired wound healing. Objectives:1) To determine if severe neutropenia, defined as neutrophil count < 500/mm3, on the day of port-a-catheter insertion is a risk factor for catheter-associated infection in children with ALL; 2) To evaluate the incidence of catheter-associated infection and wound dehiscence; 3) To assess other potential risk factors for infection, such as ALL risk category, dexamethasone use during induction therapy and nutritional status at the time of port-a-catheter insertion. Methods:This was a retrospective study conducted in children newly diagnosed with ALL (January 2005 to August 2008) who had a port-a-catheter inserted to assess catheter-associated infections and dehiscence. Demographic data, clinical characteristics, port-a-catheter insertion data and complications post-procedure were reviewed. The post-procedure complications were classified as early (≤ 30 days) or late (> 30 days). The end of catheter follow-up was March, 2009. The nutritional status was evaluated using albumin and total protein levels as well as the BMI-for-age z-score or weight-for-age z-score. Statistical analysis included descriptive and inferential statistics (Chi-squared test, Wilcoxon rank sum test and Kaplan-Meier survival curve). Results:192 port-a-catheters were inserted in 179 patients with ALL in this 3.5 year time period. Most patients were started on chemotherapy 3 days prior to catheter insertion. A total of 43 catheter-associated infections (22.39%) were diagnosed and the infection rate was 0.35/1000 catheter-days. Children with severe neutropenia on the day of insertion (n=99) had a catheter-associated infection rate of 15.15%; whereas, non-severe neutropenic (≥ 500 cells/mm3) children (n=93) had a rate of 24.73%. This difference was not statistically significant (p=0.137). Out of 192 port-a-catheters, 12 (6.25%) had to be removed due to infection. The most common organisms to cause catheter removal were Coagulase Negative Staphylococcus and Staphylococcus aureus. Patients with high risk precursor B and T cell ALL had a statistically significant higher incidence of late catheter-associated infections (p=0.024) than standard risk ALL. Gender (p=0.863), use of dexamethasone during induction (p=0.201), low BMI-for-age z-score or weight-for-age z-score (p=0.659), low albumin (p=0.530), low total protein (p=0.759) and fever pre-procedure (p=0.339) were not risk factors for infection. Patients who had an early catheter-associated infection did not have a greater chance of having a late infection (p=0.813). Out of 9 wound dehiscences (4.68%), 5 presented also with a local infection. The catheter infection-free survival rate at 1 year was 88.6%, at 2 years was 86.7% and at 3 years was 83.9% (see Graph). Conclusion:This study shows that severe neutropenia on the day of port-a-catheter insertion does not increase the incidence of catheter-associated infection in children with ALL.
