P203 PLASMA TRANSTHYRETIN FORMS IN PATIENTS WITH WILD–TYPE TRANSTHYRETIN CARDIAC AMYLOIDOSIS: INFLUENCED OF TAFAMIDIS TREATMENT

Abstract Introduction Transthyretin (TTR) is a plasma protein that transports thyroxin and retinol complexed to retinol–binding protein (RBP). TTR misfolding and aggregation can lead to the extracellular deposition of amyloid in the myocardium, causing TTR cardiac amyloidosis (ATTR–CA). Aim of this study is to develop a native electrophoretic method to characterize circulating TTR forms in plasma samples of ATTR–CA patients, before and after administration of tafamidis, a TTR stabilizer. Material & Methods Plasma samples from patients affected by wild–type ATTR amyloidosis (ATTRwt, n=10) were collected before (T0) and during tafamidis treatment at 14, 30 and 90 days. Plasma samples of sex– and age–matched healthy controls (n=6) were also collected. All samples were separated on a native 4–20% Tris–Gly polyacrylamide gel. Western blot analysis was performed with anti–TTR or anti–RBP antibodies. Proteins were detected by Clarity ECL substrate. ImageLab software was used for image acquisition and densitometric analysis of the blots. Total TTR (mg/dl) and RBP (mg/dl) were quantified by nephelometry. Results In both groups, the most represented forms were: TTR dimers or trimers (37–50kDa), TTR tetramers complexed with RBP protein in 1:1 (75kDa,) or 1:2 ratio (100kDa), and high molecular weight aggregates (>150kDa). RBP protein was detectable in association with TTR tetramers and some of the TTR aggregates (250kDa, 480kDa). Based on TTR electrophoretic pattern at T0, patients could be divided in 2 groups: ATTR–1, characterised by the presence of TTR tetramers (55kDa), and ATTR–2, characterised by its absence. The ATTR–1 group showed also higher levels of total TTR in comparison with ATTR–2 (mean±SD 25.5±3.8, 16.6±4.2 respectively, p=0.004), while plasma RBP levels were similar (5.1±0.7, 5.1±1.4 respectively). The electrophoretic patterns of ATTR–1 at T0, as well as TTR and RBP levels were comparable to those of controls (TTR 22.6±3.8, RBP 4.4±0.9). Tafamidis treatment was associated with a progressive increase of total TTR levels both in ATTR–1 and –2 (T90 31.4±5.8, 28.7±6.8) but the 2 groups continued to be distinguishable by the electrophoretic pattern. Conclusions The native electrophoresis allowed us to detect diverse circulating TTR forms and to classify patients in 2 groups according to their electrophoretic pattern. TTR quali/quantitative evaluation by electrophoresis could represent a valid tool to assess the individual pharmacological response to tafamidis.

Abstract Introduction Transthyretin (TTR) is a homotetrameric 55-KDa plasma protein that transports thyroxin and retinol complexed to retinol-binding protein (RBP). TTR misfolding and aggregation can lead to the extracellular deposition of amyloid (ATTR) representing one of the most frequent forms of amyloidosis in elderly. Aim of this study is to develop a native electrophoretic method to characterize circulating TTR in plasma samples of ATTR patients before and during treatment with tafamidis, a TTR stabilizer. Methods Plasma from ATTR patients (n=6), collected before (T0) and during tafamidis treatment, and plasma of healthy controls (n=6) were obtained from Fondazione Toscana G. Monasterio (Pisa, Italy). Plasma samples were separated on a native 4–20% Tris-Gly polyacrylamide gel. Western blot analysis was performed with anti-TTR (DAKO) or anti-RBP (Siemens Healthineer) antibodies. Proteins were detected by Clarity ECL substrate (BioRad). Results Circulating forms of TTR were qualitatively similar between ATTRwt patients at T0 and controls. In both groups, the most represented forms were: TTR dimers or trimers (∼37-50 kDa), TTR tetramers complexed with RBP protein in 1:1 ratio (∼80 kDa,) or 1:2 ratio (∼100 kDa), and high molecular weight (MW) aggregates (>150 kDa). Neither TTR monomers nor the tetramers were visible. RBP protein was detectable in association with TTR tetramers and some of the higher MW fractions (∼150 kDa, >250 kDa). Following tafamidis treatment, all ATTRwt patients displayed a progressive increase of the intensity of the band corresponding to TTR-RBP complexes, in agreement with the drug stabilizing action on TTR tetramers. Interestingly dimers and trimers, detectable at T0, were progressively lost during tafamidis treatment. Conclusions The native electrophoretic allowed us to detect several circulating TTR fractions. Data suggest a similar pattern of circulating TTR both in patients and healthy control: TTR tetramer exists only complexed with RBP and in equilibrium with low and high MW forms. Furthermore, the method allowed to appreciate the stabilizing effect of tafamidis on circulating TTR tetramers complexed with RBP. The study of circulating TTR fractions can expand our knowledge on mechanisms triggering its destabilization even when not mutated.

RBP and Alb in neonatal hydronephrosis and its association with IL-12 levels in pregnant women before delivery.

Expression levels of retinol-binding protein (RBP) and albumin (Alb) in the kidney of patients with neonatal hydronephrosis were detected to explore whether RBP and Alb can be used as diagnostic indicators for neonatal hydronephrosis. Blood and urine samples from neonates with hydronephrosis in the obstetrics and gynaecology department of Hongqi Hospital Affiliated to Mudanjiang Medical University from January 2016 to January 2018, and healthy newborns were collected. RBP and Alb levels in neonates, and the expression of IL-12 in serum of the mothers were detected by ELISA. To establish the rat model, 30 male Sprague Dawley (SD) rats were randomly divided into two groups: sham operation group (n=15) and unilateral ureteral obstruction (UUO) (n=15). Expression of RBP and Alb protein was detected by western blot analysis. Another 30 female SD rats were randomly divided into the Sham and IL-12 knockdown groups. The Sham group was given NC treatment, while IL-12 knockdown group was given treatment with IL-12 knockdown adenovirus through tail vein injection. Results of ELISA showed that serum RBP and renal urinary Alb levels were higher in neonates with hydronephrosis compared with the control group (P=0.009 or P=0.013). Compared with the control group, levels of IL-12 were significantly lower in mother of neonatal hydronephrosis group (P=0.001). After the IL-12 knockdown treatment in pregnant rats, levels of RBP and Alb hydronephrosis-related indicators were significantly increased. The incidence of hydronephrosis in rats delivered via IL-12 knockdown pregnant rats was significantly higher than that of normal pregnant rats (P=0.001). The combination of RBP and Alb in the diagnosis of neonatal hydronephrosis has important clinical significance, and neonatal hydronephrosis is closely related to the level of IL-12 in mothers before birth. Our findings provide new ideas and theoretical basis for the diagnosis and prediction of neonatal hydronephrosis.

Proxy markers of serum retinol concentration, used alone and in combination, to assess population vitamin A status in Kenyan children: a cross-sectional study.

BackgroundSerum retinol concentration determined by high-performance liquid chromatography (HPLC) is recommended by the World Health Organization to assess population vitamin A status. This assay is expensive, technically demanding and rarely available in developing countries. Our objective was a) to assess the diagnostic performance of proxy markers in detecting vitamin A deficiency and b) to derive decision rules based on these markers to estimate vitamin A deficiency prevalence.MethodsA survey was conducted in 15 rural primary schools in Eastern Province, Kenya, with 375 children aged 6 to 12 years (25 randomly selected per school). Serum retinol concentration <0.70 μmol/L by HPLC was used to define vitamin A deficiency. Proxy markers for vitamin A deficiency were serum concentrations of retinol binding protein (RBP), transthyretin, retinol measured by fluorometry and RBP:transthyretin molar ratio.ResultsThe prevalence of vitamin A deficiency (HPLC) was 18%. Transthyretin and RBP showed the best diagnostic performance individually, with area-under-the-curve (AUC) values of 0.96 and 0.93. When combined, and with C-reactive protein added, the AUC increased to 0.98. A simple decision rule {(−15.277 × [RBP, μmol/L] - 7.013 × [Transthyretin, μmol/L] + 0.367 × [C-reactive protein, mg/L] + 24.714) > 0.496} yielded prevalence estimates of vitamin A deficiency that is unbiased by diagnostic error.ConclusionsThe combination of transthyretin, RBP and C-reactive protein concentrations could eventually replace retinol concentration by HPLC in resource-poor settings as the preferred method to assess the population burden of vitamin A deficiency.Electronic supplementary materialThe online version of this article (doi:10.1186/s12916-014-0256-5) contains supplementary material, which is available to authorized users.

Alterations in retinoid status after long-term exposure to PBDEs in zebrafish (Danio rerio)

This study examined the disruptive effect of exposure to polybrominated diphenyl ethers (PBDEs) on retinoid content in zebrafish (Danio rerio). Adult zebrafish were exposed to an environmentally relevant concentration (0.45μg/L) and a higher concentration (9.6μg/L) of DE-71 for 60 days. Retinoid content and gene transcription levels were examined in female zebrafish. PBDE exposure caused a significant decrease of retinyl ester content in the intestine and a downregulation of intestinal cellular retinol binding protein gene transcription (CRBP1a). In the liver, retinyl ester content was significantly decreased, while retinol content was increased. An upregulation of liver CRBP2a and retinol binding protein (RBP) gene transcription and an increased level of RBP protein were observed. In the eyes, both the retinal and retinyl ester content were increased and CRBP1a gene transcription was upregulated. However, the gene encoding for retinal dehydrogenase (RALDH2), responsible for retinoic acid synthesis, was downregulated in the eyes. CYP26a, the gene responsible for retinoic acid degradation, was upregulated, which indicated an increased level of retinoic acid. In the ovaries, the increased deposition of retinoids was also observed, while gene transcription levels of both CRBPs (CRBP1a and CRBP1b) were upregulated. An increased deposition of retinal was measured in the eggs. Overall, this study demonstrated that long-term exposure of zebrafish to environmentally relevant concentrations of DE-71 disrupted the transport, storage and metabolism of retinoid in various tissues. This study also indicated that retinoid levels in zebrafish are sensitive to PBDE exposure and highlighted the importance of liver storage, which appears to support important functions in reproduction and vision.

Kidney function and retinol status in type 2 diabetes mellitus patients

Kidneys play an important role in retinol turnover. We postulated that retinol homeostasis is disturbed in diabetic nephropathy. The aim of this research was to study the effect of kidney impairment on urinary excretion and on serum concentrations of retinol in type 2 diabetes mellitus patients. For this purpose, 41 type 2 diabetes patients and 9 sex -and age-matched healthy subjects were enrolled. Serum and urinary retinol and retinol-binding protein (RBP) were assessed by high-pressure liquid chromatography and enzyme-linked immunosorbent assay, respectively. The study showed that 17 out of 41 diabetic patients (41.5%) and none of the controls excreted retinol in urine (P<0.02). Retinol excretion in the urine in these patients was 1.5-fold more prevalent than hypercreatininemia. Urinary retinol significantly correlated with clinically diagnosed nephropathy (P=0.02). All but one of the patients with hypercreatininemia excreted retinol in the urine. Serum retinol and RBP in patients with hypercreatininemia were higher than in controls (P<0.002). Values of urinary retinol, unlike urinary RBP, albumin and total protein, did not overlap between patients and controls. Our results indicate that (i) urinary retinol is a specific sign of tubular damage in type 2 diabetic patients and (ii) urinary retinol enables a more clear-cut identification of proximal tubule dysfunction in type 2 diabetes patients than urinary RBP or albumin.

Serial changes in urinary cadmium concentrations and degree of renal tubular injury after soil replacement in cadmium-polluted rice paddies

Targeting persons requiring observation in the Cd-polluted Kakehashi River basin a serial observation study was conducted. Namely we followed the serial changes in 50 subjects who ingested household rice for 10 years after replacement of Cd-polluted soil in rice paddies. The serial changes in urinary substance levels in individuals were determined adjusting for the potential effect of age using a general linear mixed model. Cd excretions decreased with increasing number of years elapsed, with the partial regression coefficients of the number of years elapsed statistically significant in the women. The ratio of the Cd excretion theoretical values at the completion of soil replacement and 10 years later was 0.60 and the reduction rate was calculated as 5.0% per year in women. However, it was surmised that in practice a decrease to the level of inhabitants of non-polluted districts would not be achievable. The indices of renal tubular injury (β 2-microglobulin, retinol binding protein (RBP), total protein, amino-N and glucose) with the exception of amino-N in men showed increased excretion in both sexes with increasing number of years elapsed with statistically significant differences in RBP and total protein in both sexes and glucose in men. In this study using a general linear mixed model, which is an appropriate statistical method to perform a follow-up study, Cd concentrations in rice and urine were found to decrease after Cd exposure was reduced, but the degree of renal tubular injury was not found to improve, leading to the conclusion that the renal tubular injury induced by environmental Cd exposure is irreversible.

Retinol binding protein expression is induced in HepG2 cells by zinc deficiency

Zinc (Zn) deficiency is often associated with low plasma vitamin A (retinol) concentrations. It has been suggested that the reduction in plasma retinol is secondary to reduced liver retinol binding protein (RBP) synthesis. In the present study, RBP expression was determined in HepG2 cells cultured in either Zn adequate media or chelated media containing varying concentrations of Zn. Levels of RBP mRNA increased in a time- and Zn concentration-dependent manner such that 0.5 μM Zn-treated cells exhibited a >7.5-fold increase while cells treated with 15 μM Zn were increased 2.9-fold at 72 h compared to controls. RBP protein also progressively increased by 72 h to levels >8-fold and 3-fold higher than controls, in 0.5 μM and 15 μM Zn-treated cells, respectively. The increase in RBP occurred without any change in DNA concentration between groups through 72 h. The Zn deficiency-induced elevations in RBP transcript levels could be reversed within 24–48 h of repletion in Zn adequate media. Thus, the reductions in plasma retinol observed in Zn deficiency are in part a direct consequence of the deficiency.

Induction of Mouse Retinol Binding Protein Gene Expression by Cyclic AMP in Hepa 1–6 Cells

Retinol binding protein (RBP) is the primary circulating transport molecule for retinol, facilitating its transport to target tissues and influencing target cell uptake. Specific signals and molecular mechanisms that regulate RBP gene expression are poorly understood. Using the mouse hepatoma cell line (Hepa 1–6), we examined the role of cAMP in the molecular regulation of RBP. Dibutyryl cAMP (dbcAMP) or the adenylate cyclase activator, forskolin, increased RBP mRNA levels >6-fold at 24 h. Increases in RBP mRNA were dose dependent over the range of 10 μM–1 mM for dbcAMP and 0.5–10 μM for forskolin. 8-Bromo cAMP, a nonhydrolyzable analog, over the range of 0.01–0.5 mM, increased RBP mRNA levels 9.2-fold at 24 h. Induction of RBP transcripts by analogs also resulted in a comparable increase in intracellular RBP protein. Cycloheximide (10 μg/ml) did not prevent cAMP-mediated induction of RBP mRNA, indicating thatde novoprotein synthesis is not required for cAMP-mediated induction of RBP transcription.These studies demonstrate that cAMP, or agents which elevate intracellular cAMP, increase RBP transcript levels. The time course and extent of RBP mRNA induction and the resultant increase in RBP protein support the concept that cAMP regulation of RBP gene expression may be physiologically relevent. Given the ubiquitous nature of cAMP as a second messenger, and the several mechanisms by which cAMP regulates gene expression, studies are in progress to define molecular mechanisms by which cAMP regulates RBP gene expression.

Expression of retinol binding protein and transthyretin during early embryogenesis

Previous studies have shown that anterior lateral plate endoderm from stage 6 chicken embryos is necessary and sufficient to enable precardiac mesoderm to complete its cardiogenic program in vitro, culminating in a rhythmically contractile multicellular vesicle (Sugi and Lough [1994] Dev. Dyn. 200:155–162). To identify cardiogenic factors, we have begun to characterize proteins that are secreted by endoderm cell explants. Fluorography of proteins from endoderm-conditioned medium revealed 1–2 dozen bands, the most prominent of which migrated at approximately 17 and 25 kD. The bulk of the 17-kD band, which migrates near FGFs and subunits of the transforming growth factor-β family, was identified by N-terminal sequencing as transthyretin (TTR). A component of the 25-kD band was identified by Western blotting as retinol binding protein (RBP). RT/PCR analysis revealed that mRNAs for both proteins are in the embryo as early as stage 3. In situ hybridization localized these mRNAs to the extraembryonic endoderm at stage 6, after which they were detected in endoderm overlying the embryo proper, including the developing heart. Later, RBP and TTR mRNA and protein were detected in cells associated with the developing heart. Western blotting of whole embryo proteins revealed the presence of RBP by stage 7, followed by sequential increases to stage 25; by contrast, content of RBP in isolated hearts peaked at stage 14, then declined. Immunohistochemistry revealed the presence of RBP protein in the extracellular matrix subjacent to lateral plate endoderm beginning at stage 8; upon formation of the definitive heart, intense staining was observed in the cardiac “jelly.” By contrast TTR was intracellular, first detected as subtle deposits in stage 6 embryonic endoderm, which by stage 8 were prominent in the dorsally invaginated endoderm subjacent to the precardiac splanchnic mesoderm. At stages 11–14, TTR was detected only in myocardial cells. Such localization of RBP and TTR may indicate a role in the transport and distribution of retinol and thyroid hormone, respectively, from yolk to embryo prior to establishment of the circulatory system, and is suggestive of a subsequent role in heart development. Retinoid binding proteins in mouse yolk sac and chorio-allantoic placentas.

In the adult, as well as in the embryo, a number of specific extra- and intracellular binding proteins such as the plasma retinol binding protein (RBP), the cellular retinol binding protein type I (CRBP I), and also the cellular receptors for RBP are thought to regulate transport and metabolism of retinol (vitamin A). Since the regulation of materno-fetal transport of vitamin A is not well understood, we examined the localization of these proteins during the development of the mouse chorio-allantoic and yolk sac placentas. The labyrinthine region of the chorio-allantoic placenta, where exchange of substances can occur between the maternal and fetal circulations, did not contain RBP (mRNA or protein) or antigen(s) similar to the bovine RBP-receptor p63, whereas the visceral endoderm of the yolk sac placenta, the second site for materno-fetal transport, did. Furthermore, only the endodermal cells of the visceral yolk sac appeared to strongly accumulate radiolabelled retinoids. The cellular retinol binding protein (CRBP I) was detected both in the trophoblast layer of the placental labyrinth closest to the fetal endothelium (layer III), and in the visceral endoderm of the yolk sac. Together, these findings suggest that the yolk sac placenta mediates retinol transfer to the embryo/fetus throughout the entire gestation. The chorio-allantoic placenta, on the other hand, does not appear to have this capacity, while the presence of CRBP I does suggest a retinol-metabolizing capability.

Retinol-Binding Protein in the Baboon (Papio Anubis) Uterus: Immunohistochemical Characterization and Gene Expression1

Retinols and retinoic acid (vitamin A) are essential for embryonic development; they are transported in circulation bound to retinol-binding protein (RBP). RBP has been shown to be synthesized in extrahepatic sites, i.e., the progestational uterus and concepti of pig, sheep, and cows. This study was undertaken to determine whether RBP is also synthesized by the primate reproductive tract. Uterine tissues were obtained from mature female baboons during the follicular and luteal stages of the menstrual cycle, after steroid (estrogen +/- progesterone) treatment of ovariectomized baboons and between Days 18 and 32 of pregnancy. Portions of the uterine tissues were either fixed in Bouin's fixative and embedded in paraffin, subjected to explant culture, or immediately frozen in liquid nitrogen for RNA isolation. Our studies indicate that RBP is present in the uterine glandular epithelium of the baboon during the late luteal stages of the menstrual cycle. Steroid hormone treatment of ovariectomized female baboons confirmed that both transcription and translation of uterine RBP is under progestational control. When uterine and placental tissues from pregnant baboons were analyzed between Days 18 and 32 of pregnancy, it was apparent that immunoreactive RBP increased in the mid and basal glands initially, but by Day 32 all of the glands stained positively. In contrast, the mRNA levels peaked at Day 25, and RBP message was more highly expressed in the basalis region of the uterus compared to the functionalis. Immunoreactivity for RBP was higher at the implantation site compared to the nonimplantation site. Neither RBP protein nor message was detectable in the baboon conceptus/placenta.(ABSTRACT TRUNCATED AT 250 WORDS)

Retinol-Binding Protein: Immunolocalization of Protein and Abundance of Messenger Ribonucleic Acid in Conceptus and Maternal Tissues during Pregnancy in Pigs1

Retinol-binding protein (RBP) is a major secretory product of liver as well as uterine endometrium and periimplantation conceptuses of pigs. The present study examined distribution of RBP protein and abundance of RBP mRNA in pig maternal and conceptus tissues throughout pregnancy. A porcine liver RBP cDNA clone was isolated and used as probe in these studies. Northern blot analysis detected a 1.0-1.1-kb transcript in conceptus, trophoblast, yolk sac, placenta, endometrium, myometrium, oviduct, and numerous fetal tissues. Slot blot analyses of RBP mRNA abundance in endometrium, myometrium, conceptus, trophoblast, chorioallantoic placenta, and embryo/fetus indicated differential RBP gene expression in each tissue. Immunocytochemical localization studies indicated the presence of immunoreactive RBP in endometrial surface and glandular epithelium, circular and longitudinal muscle of myometrium, parenchymal cells of fetal liver, and proximal convoluted tubules of fetal kidney. Results suggest an integrated system of RBP gene expression and protein secretion that allows for transport of retinol from the uterine endometrium to the periimplantation conceptus during early pregnancy and to the fetal-placental unit throughout pregnancy in pigs.

Differentiation-dependent expression of retinoid-binding proteins in BFC-1 beta adipocytes.

Recently, we demonstrated that adipose tissue plays an important role in retinol storage and retinol-binding protein (RBP) synthesis. Our data suggested that RBP expression in adipose tissue is dependent on the state of adipocyte differentiation. To examine this possibility, we explored the differentiation-dependent expression of RBP using BFC-1 beta preadipocytes, which can be stimulated to undergo adipose differentiation. Total RNA was isolated from undifferentiated (preadipocytes) and differentiated (adipocytes) BFC-1 beta cells and analyzed by Northern blotting. RBP mRNA was not detected in the preadipocytes, but considerable RBP mRNA was present in differentiated BFC-1 beta cells. In BFC-1 beta cells, induced to differentiate with insulin and thyroid hormone, RBP mRNA was first detected after 4 days, reached a maximum level by day 10, and remained at this maximum level for at least 2 more days. Cellular retinol-binding protein was expressed at low levels in the BFC-1 beta preadipocytes and the level of expression increased for 6 days after induction to differentiate and slowly declined on later days. Neither the maximum level of RBP expression nor the day on which this level was reached was influenced by the level of retinol provided in the BFC-1 beta culture medium. BFC-1 beta cells secreted newly synthesized RBP into the culture medium at a rate of 43 +/- 14 ng RBP/24 h/10(6) adipocytes. When the BFC-1 beta adipocytes were provided 1.0 microM retinol in the medium, they accumulated the retinol and synthesized retinyl esters. These studies with BFC-1 beta cells confirm that RBP synthesis and secretion and retinol accumulation are intrinsic properties of differentiated adipocytes. Furthermore, they suggest that RBP and cellular retinol-binding protein gene expression are regulated as part of a package of genes which are modulated during adipocyte differentiation.

Retinoids and retinoid-binding protein expression in rat adipocytes.

Adipose tissue has been reported to contain relatively high levels of the specific mRNA for retinol-binding protein (RBP) (Makover A., Soprano, D.R., Wyatt, M. L., and Goodman, D.S. (1989) J. Lipid Res. 30, 171-180). Studies were conducted to explore retinoid and retinoid-binding protein storage and metabolism in adipose tissue. In these studies, we measured RBP and cellular retinol-binding protein (CRBP) mRNA levels and retinoid levels in 6 adipose depots in male rats. Total RNA was isolated from inguinal, dorsal, mesenteric, epididymal, perinephric, and brown adipose tissue, and average RBP and CRBP mRNA levels were determined by Northern blot analysis. The relative levels of RBP mRNA in these 6 anatomically different adipose depots averaged, respectively, 6.3, 6.7, 16, 34, 37, and 21% of the level in a rat liver RNA standard. Retinoid levels in the 6 depots were similar and averaged approximately 6-7 micrograms of retinol eq/g of adipose tissue. Since adipose tissue contains several cell types, the cellular localizations of RBP and CRBP expression and retinoid storage were examined. RNA was prepared from isolated rat adipocytes and stromal-vascular cells. Cellular levels of the mRNAs for RBP, CRBP, apolipoprotein E (apoE), lipoprotein lipase, adipocyte P2, and adipsin were measured by Northern blot analysis. RBP was expressed almost exclusively in the adipocytes and only weakly in the stromal-vascular cells. Both CRBP and apoE mRNA levels were relatively high in the stromal-vascular cell preparations and only very low mRNA levels were found in the adipocytes. Lipoprotein lipase, adipsin, and adipocyte P2 mRNAs were found in substantial levels in both the adipocytes and stromal-vascular cells, but with higher levels present in the adipocytes. Cultured adipocytes synthesized RBP protein and secreted it into the medium. Only adipocytes (not stromal-vascular cells) contained retinol, at levels between 0.65-0.8 micrograms of retinol eq/10(6) cells. These studies demonstrate that adipocytes store retinoid and synthesize and secrete RBP, and suggest that rat adipocytes may be dynamically involved in retinoid storage and metabolism.

Assessment of urinary retinol-binding protein as an index of proximal tubular injury.

The urinary excretion of retinol-binding protein (RBP), beta 2-microglobulin (beta 2-m), and beta-N-acetyl-D-glucosaminidase was monitored in patients with renal tubular damage secondary to multiple injuries, rhabdomyolysis, antibiotic treatment, or poisoning by various chemicals such as solvents, heavy metals, or pesticides. In almost all cases, RBP proved to be a more sensitive index of renal tubular damage than was beta-N-acetyl-D-glucosaminidase and, being more stable in acid urine, a more practical analyte to measure than was beta 2-m. We corroborated this finding by studying the relationships between these three analytes in more than 150 patients. On the average, an increase in the urinary excretion of beta-N-acetyl-D-glucosaminidase becomes detectable when urinary RBP already exceeds the normal value by 50- to 100-fold. In urines with pH greater than 6, RBP and beta 2-m concentrations are well correlated (r = 0.93, n = 150), beta 2-m tending to be more frequently positive (i.e., greater than 311 micrograms/L). But in urines with pH less than 6 (about 30-40% of the samples), the RBP/beta 2-m concentration ratio increases as pH decreases, up to 500 in some patients with massive tubular injury. Because the renal uptake of proteins involves a saturable process, the urinary excretion of RBP, like that of beta 2-m, specifically reflects the reabsorption capacity of proximal tubules only when the glomerular filtration rate is normal or slightly impaired (i.e., serum creatinine less than 20 mg/L). Under these conditions the determination of RBP protein in urine appears the most appropriate test when early detection of tubular injury is desirable.
