Outcomes of Non operative Treatment of Isolated, Acute Posterior Cruciate Ligament (PCL) Injury

1.1. Objectives: The aim of this study is to evaluate the results of non-operative treatment of acute, isolated PCL injury. 1.2. Study Design: Prospective Observational Study. 1.3. Setting: Orthopedics and spine unit, Hayatabad Medical Complex, Peshawar. 1.4. Method: 63 patients who had post traumatic PCL injury were recruited for our study). There were 23 partial PCL injuries and 40 total PCL injuries among the 63 knees. These 63 knees were subjected to a thorough subjective, objective, functional, and radiological examination. 1.5. Results: There was no discomfort in 38 knees (60%), mild pain in 14 knees (22%), and significant pain on exercise in 6 (9.5%) knees at the most recent follow-up examination (10 percent). In 54 knees (93%), there was no swelling, mild, intermittent swelling in three knees (5%), and moderate swelling on exertion in one knee (2 percent). Fifty-two patients (91%) experienced no giving way, whereas five patients (9%) complained of giving way on occasion, especially while walking downstairs. 1.6. Conclusion: We currently believe that patients with acute, isolated, complete PCL tears and up to 10 mm of posterior tibial translation (anterior border of tibial plateau flush with anterior surface of femoral condyles with knee in 90-degree flexion) can be treated non operatively and that the majority of patients can achieve a satisfactory functional result.

Comparative study of conservative versus operative management of closed displaced intraarticular fracture of calcaneum

Intraarticular fractures of calcaneum are frequently complex injuries, requiring experience in the diagnosis and management, for favourable results. The historical aspects of calcaneal fracture treatment are difficult to appraise, secondary to varied forms of treatments, classifications, numerous retrospective studies and mixture of diagnoses and results. Significant controversy remains over the results of non-operative treatment. Due to the lack of standardization, accurate comparisons cannot be made between reported surgical and conservative treatment modalities to precisely determine the long term and short-term functional results between each method. Here is a cohort prospective study to compare patients of closed displaced intra-articular fracture of calcaneum. 30 patients were taken for this study out of which fifteen patients were treated operatively and fifteen patients were treated non-operatively. Patients were followed up to an average of 1 year. The functional outcome was measured by AOFAS hind foot score. It was observed that under the operative group 73.3% of the patients had excellent results, 13.3% had good result and 6.7% each had fair and poor results. Similarly, under the conservative group 53.3% of the patients had fair results, 26.7% had good result, 20% had poor result and none of the patients had excellent results. Distal biceps tendon rupture – Is surgical reconstruction really always necessary?

Summary Background Rupture oft the distal biceps tendon is a rare injury which leads to a considerable loss of supination and flexion strength. A deficit of 40% of flexion and more than 50% of supination strength is reported in the literature. Therefore, surgical reconstruction of the distal tendon is the method of choice. However, some patients opt for non-operative management and a small number of patients are not fit for surgery. The purpose of this retrospective study was to assess the results of non-operative treatment using objective muscle strength testing and evaluation of the subjective satisfaction. Patients and method Between 1999 and 2010, 206 patients had suffered from a distal biceps tendon rupture after trauma. Clinical examination, sonography and MRI confirmed the diagnosis. Most of these patients (85,4%, N - 176) underwent surgical intervention by refixation. 30 patients were treated non-operatively. All these patients were treated with a short-term cast-fixation followed by physiotherapy. The screened patients underwent clinical assessment using a modified Mayo Clinic Performance Index. The supination and flexion strength was measured with an isometric strength test, taking muscle dominance into account. Visual Analogue Scale (VAS) was used for the evaluation of the subjective satisfaction. Results Of the 30 patients who were treated non-operatively 24 (80%) underwent follow-up examination. The mean time between trauma and follow-up was 80 months (18-138 months). A deficit of ROM was only found in one patient. The loss of supination strength was 26,6%, compared to the other arm. Loss of flexion strength was 16,1% -18,1% depending on whether the forearm was in pronation, neutral position or supination. The overall subjective satisfaction for non-operative management was 8,9 for the VAS (10 max). The modified Mayo elbow performing score was 81,05 (90 max). Conclusion In the literature, a significant loss of strength is reported in case of distal biceps tendon rupture. However the evidence for this is sparse. We evaluated more than 20 patients and we found that the loss of supination and flexion strength was not as severe as reported. A high patient satisfaction was found in this study as well. Taking further into consideration the loss of strength after surgical repair with the risk of nerve lesions, non-operative treatment seems to be an acceptable alternative option.

Nonoperative Management of Retrocalcaneal Pain with AFO and Stretching Regimen

Retrocalcaneal heel pain is caused by a spectrum of etiologies all resulting in the same symptom of pain at the tendon-Achilles insertion. Several studies have reported the outcomes of operative treatment, but none have reported the outcomes or success rates of nonoperative treatment. We describe a detailed treatment algorithm and report the clinical outcomes. One hundred thirty-five patients were prescribed a treatment regimen consisting of an AFO and stretching program and were enrolled in our prospective study. One hundred three patients completed pre- and post-treatment Foot Function Indices and were included in the analysis. The effect on FFI from clinical and radiographic factors was examined. Of the study population, 76% had a BMI greater than 25, 80% were older than 50 years, and 75% had an exostosis on radiographs. The mean pre-treatment FFI was 48.4 and the mean post-treatment FFI was 18.6 indicating a statistically significant improvement in function of 29.8. Neither BMI nor age had a significant effect on the magnitude of improvement; though, smokers had significantly less improvement. FFI improvement in patients with an exostosis were less than those without an exostosis. Patients with an exostosis less than 1 cm had less improvement than those with an exostosis of 1 cm or more. Patients with Types I and III exostoses had significantly less improvement in FFI compared to Types II and IV. Twelve of the 103 (11.6%) were not pleased with the results of nonoperative treatment and elected to have a procedure performed. Our study is the first to report the outcome of non operatively treated retrocalcaneal heel pain and to classify retrocalcaneal exostoses. Using our treatment algorithm, we had an 88% success rate in alleviating symptoms and avoiding surgery. Our data suggests that the use of an AFO and stretching regimen may benefit patients suffering from retrocalcaneal heel pain.

Nonoperative Treatment of Superior Labrum Anterior Posterior Tears

Background: Although there are multiple reports documenting successful outcomes with operative treatment of superior labrum anterior posterior (SLAP) tears, there are few reports on the results of nonoperative treatment. Hypothesis: Nonoperative treatment of SLAP tears will result in improved outcomes over pretreatment values using validated, patient-derived outcome instruments. Study Design: Case series; Level of evidence, 4. Methods: A total of 371 patients with a diagnosis of labral tear at our institution were mailed a questionnaire that included the following validated, patient-derived outcome assessment instruments: Short Form 36 (SF-36), European Quality of Life measure (EuroQol), visual analog pain scale (VAS), American Shoulder and Elbow Surgeons (ASES) score, and simple shoulder test (SST). Sixty-six surveys did not reach the patients because of incorrect addresses, and 50 surveys were returned, for a 16.4% (50 of 305) response rate. Of the patients with a clinically documented SLAP lesion (positive O’Brien test, pain at the bicipital groove, and positive magnetic resonance imaging) and sufficient follow-up data (minimum 1 year), 39 patients who met the criteria returned the survey and 19 had nonoperative treatment. Twenty patients (51%) from the overall surveyed group were considered nonoperative treatment failures and had arthroscopic surgical reconstruction. Nonoperative treatment consisted of nonsteroidal anti-inflammatory drugs and a physical therapy protocol focused on scapular stabilization exercises and posterior capsular stretching. Statistical analysis was performed using the paired t test; values of P < .05 were considered significant. Results: At an average follow-up of 3.1 years, function improved significantly (ASES function 30.8 to 45.0 [P < .001]; ASES total 58.5 to 84.7 [P = .001], SST 8.3 to 11.0 [P = .02]) in those patients with successful nonoperative treatment. Quality of life also improved after treatment (EuroQol 0.76 to 0.89, P = .009). Pain relief was significant, as VAS pain scores decreased from 4.5 to 2.1 (P = .043). All patients with successful nonoperative treatment returned to sports. Seventy-one percent of all athletes were able to return to preparticipation levels, but only 66% of overhead athletes returned to their sport at the same or higher level. Conclusion: Using validated, patient-derived outcome instruments, the present study shows that successful nonoperative treatment of superior labral tears results in improved pain relief and functional outcomes compared with pretreatment assessments. Although 20 patients (51%) in this group elected surgery and may be considered nonoperative treatment failures, those patients with successful nonoperative treatment had significant improvements in pain, function, and quality of life. Return to sports was comparable with patients with successful surgical treatment, although return to overhead sports at the same level was difficult to achieve (66%). Based on these findings, a trial of nonoperative treatment may be considered in patients with the diagnosis of isolated superior labral tear. In overhead athletes and in those patients where pain relief and functional improvement is not achieved, surgical treatment should be considered.

The adequate treatment of antero-inferior glenoid rim fractures is a controversial issue. Marginal knowledge exists about the results of non-operative treatment. Therefore, the aim of this study was to evaluate the results of conservative treatment of acute fragment type lesions (type Ib) of the antero-inferior glenoid.A total of 10 patients (5 female and 5 male, mean age 56.5 years) were included in this retrospective study. A complete clinical examination of the shoulders and four functional scores, the Constant Score (CS), Rowe Score (RS), Western Ontario Shoulder Instability Index (WOSI) and Subjective Shoulder Value (SSV) as well as a radiologic evaluation using true a/p and axillary radiographs were performed.After a mean follow-up of 26.4 months no clinical signs of subjective instability or redislocation were found. The median CS reached 80.5 points (range 52-88) versus 84 points (range 73-90) on the healthy contralateral side (p >0.05). The RS, WOSI and SSV achieved 95 points (range 70-100), 86.7% (range 51-99.6%) and 85 % (range 50-100%), respectively. The radiologic evaluation showed a consolidated fragment in all patients. The anterior subchondral sclerosis zone was reformed completely in seven patients. The mean intra-articular step-off was 2 mm (range 0-6 mm). Osteoarthritis was seen in two cases (one grade I and one grade II according to Samilson and Prieto) none of which was symptomatic and the one grade II was present at the time of trauma.The conservative treatment of acute fragment type lesions (type Ib) of the antero-inferior glenoid can lead to excellent and satisfactory clinical results and therefore represents a suitable treatment alternative in the middle aged patient population. Nevertheless, future studies have to explicitly define the exact indications for a conservative versus surgical treatment approach.

Treatment of osteochondral lesions of the talus: a systematic review

The aim of this study was to summarize all eligible studies to compare the effectiveness of treatment strategies for osteochondral defects (OCD) of the talus. Electronic databases from January 1966 to December 2006 were systematically screened. The proportion of the patient population treated successfully was noted, and percentages were calculated. For each treatment strategy, study size weighted success rates were calculated. Fifty-two studies described the results of 65 treatment groups of treatment strategies for OCD of the talus. One randomized clinical trial was identified. Seven studies described the results of non-operative treatment, 4 of excision, 13 of excision and curettage, 18 of excision, curettage and bone marrow stimulation (BMS), 4 of an autogenous bone graft, 2 of transmalleolar drilling (TMD), 9 of osteochondral transplantation (OATS), 4 of autologous chondrocyte implantation (ACI), 3 of retrograde drilling and 1 of fixation. OATS, BMS and ACI scored success rates of 87, 85 and 76%, respectively. Retrograde drilling and fixation scored 88 and 89%, respectively. Together with the newer techniques OATS and ACI, BMS was identified as an effective treatment strategy for OCD of the talus. Because of the relatively high cost of ACI and the knee morbidity seen in OATS, we conclude that BMS is the treatment of choice for primary osteochondral talar lesions. However, due to great diversity in the articles and variability in treatment results, no definitive conclusions can be drawn. Further sufficiently powered, randomized clinical trials with uniform methodology and validated outcome measures should be initiated to compare the outcome of surgical strategies for OCD of the talus.

Results of non-operative treatment following hip fracture compared to surgical intervention

We followed all consecutive hip fracture patients admitted between 2004 and 2006, identified cases in which the intention was to treat non-operative and compared their functional outcome and mortality with a similar cohort treated surgically over the same period. We recorded length of hospital stay, place of discharge, pre and post-fracture mobility and residence, 30 days and 1 year mortality, re-admission due to same fracture and delayed surgery. The group treated surgically was recruited and matched for age, gender, pre and post-fracture mobility, mental confusion and independence. 25 patients were treated non-operative. 22 patients treated surgically over the same time period matched the patient characteristics of the non-operative arm. The mean hospital stay was 13 days in both groups. There were 4 extra-capsular fractures (3 displaced) and 21 intra-capsular fractures (5 displaced) in the non-operative arm and 11 extra-capsular fractures and 9 intra-capsular fractures in the surgically treated arm. 4 patients from the non-operative treatment group underwent late surgery because of persisting hip pain 20 days–2 months after the index event (2 cannulated screws, 1 hemiarthroplasty, 1 total hip arthroplasty). 11 patients in the surgical treatment arm underwent dynamic screw fixation, 1 had cannulated screw, 1 had total hip replacement and 7 had hemiarthroplasty. 14 of the non-operative treated patients were mobile independently or with aid before fracture but only 9 patients retained their pre-fracture mobility following treatment, compared to 16 patients pre-fracture and 11 patients post-fracture after surgery. 16 patients treated non-operative were living independently prior to injury but only 7 went back to their own residence. Of the operatively treated patients 14 patients were living independently and 10 patients went back to their previous residence. 1 month and 1 year mortality in the non-operative treated group was 4/21 and 7/21 respectively compared to 1/20 and 5/20 in the operative fixation group. There was no statistically significant difference in mobility, residence or mortality between the two groups (Fisher exact test, p > 0.05). Non-operative management after hip fracture is suitable for medically unfit patients and does not result in statistically significant difference in functional outcome or mortality compared to patients treated surgically.

BACKGROUND AND PATIENTS: The outcome of the impacted varus proximal humeral fracture has not been described. We present the results of a prospective analysis of 99 such fractures. This fracture was relatively common in the elderly population and non-operative treatment sometimes resulted in increased varus angulation. Decreased shoulder function was associated with increasing age but not with increasing varus angulation. After nonoperative management the outcome was good, regardless of the degree of varus, 1 year after fracture. Age was the main predictor of outcome. Physiotherapy did not improve outcome.

Impacted valgus fractures (B1.1) of the proximal humerus. The results of non-operative treatment.

We present a retrospective study of 125 patients with an impacted valgus fracture (B1.1) of the proximal humerus. This fracture rarely occurs in young patients and is much more common in elderly fit subjects. All patients were documented prospectively and followed for one year. None was treated surgically. At one year, 80.6% of the patients had a good or excellent result, the quality of which depended on the age of the patient and the degree of displacement of the fracture. Mean outcome scores based on these two parameters are presented. A comparison with data from other studies suggests that operative fixation of these fractures is not necessary.

The effect of placing a tensioned graft across open growth plates. A gross and histologic analysis.

Midsubstance tears of the anterior cruciate ligament in skeletally immature patients are increasingly common and are a challenging problem. The results of nonoperative treatment are no better in children than they are in adults. Physeal-sparing reconstructive procedures have yielded poor results. Reconstructive procedures that are utilized in adults violate the physis, potentially resulting in growth abnormalities. The objective of this study was to provide a model for reconstruction of the anterior cruciate ligament in skeletally immature patients by evaluating the effects of a tensioned connective-tissue graft placed across the canine physis. Twelve ten-week-old beagles underwent reconstruction of the anterior cruciate ligament consisting of placement of fascia lata autograft through drill-holes across the femoral and tibial physes, tensioning of the graft to 80 N, and fixing it with screws and washers. The contralateral limb served as a control. One dog was eliminated from the study secondary to a postoperative infection. Four months postoperatively, the dogs were killed and were inspected grossly, radiographically, and histologically for any evidence of growth disturbance. Significant valgus deformity of the distal part of the femur (p < 0.001) and significant varus deformity of the proximal part of the tibia (p = 0.03) developed in the treated limbs. Neither radiographic nor histologic examination demonstrated any evidence of physeal bar formation. Significant growth disturbances occur with excessively tensioned transphyseal reconstruction of the anterior cruciate ligament in the canine model. These growth disturbances occur without radiographic or histologic evidence of physeal bar formation.

Atraumatic osteonecrosis of the knee.

The purposes of this study were to define the clinical, demographic, and radiographic patterns of atraumatic osteonecrosis of the distal part of the femur and the proximal part of the tibia at presentation and to report the outcome of treatment of this condition. Two hundred and forty-eight knees in 136 patients who were younger than the age of fifty-five years were treated at our institution between July 1, 1974, and September 15, 1998, for atraumatic osteonecrosis of the distal part of the femur or the proximal part of the tibia, or both. Demographic and radiographic features were characterized. The results of nonoperative treatment, core decompression, arthroscopic debridement, and total knee arthroplasty were evaluated. There were 106 female patients and thirty male patients, and their mean age was thirty-six years (range, fifteen to fifty-four years) at the time of diagnosis. One hundred and one patients (74 percent) had involvement of other large joints, with eighteen (13 percent) presenting initially with knee symptoms. One hundred and one patients (74 percent) had a disease that affected the immune system; sixty-seven of them had systemic lupus erythematosus. One hundred and twenty-three patients (90 percent) had a history of corticosteroid use. Technetium-99m bone-scanning missed lesions in sixteen (29 percent) of fifty-six knees. Eight (20 percent) of forty-one initially symptomatic knees treated nonoperatively had a successful clinical outcome (a Knee Society score of at least 80 points and no additional surgery) at a mean of eight years. The knees that remained severely symptomatic for three months were treated with either core decompression (ninety-one knees) or total knee arthroplasty (seven knees). Seventy-two (79 percent) of the ninety-one knees treated with core decompression had a good or excellent clinical outcome at a mean of seven years. Efforts to avoid total knee arthroplasty with repeat core decompression or arthroscopic debridement led to a successful outcome in fifteen (60 percent) of twenty-five knees. Thirty-four (71 percent) of forty-eight knees treated with total knee arthroplasty had a successful clinical outcome at a mean of nine years. Atraumatic osteonecrosis of the knee predominantly affects women, and in our study it was associated with corticosteroid use in 90 percent of the patients. Evaluation should include standard radiographic and magnetic resonance imaging of all symptomatic joints. Prognosis was negatively related to large juxta-articular lesions. Nonoperative treatment should be reserved for asymptomatic knees only. Core decompression was successful (a Knee Society score of at least 80 points and no additional surgery) in 79 percent of the knees in which the disease was in an early stage. Total knee arthroplasty was successful in only 71 percent of the knees.

Congenital muscular torticollis and sternomastoid tumor: Results of nonoperative treatment

Background: Congenital muscular torticollis (CMT) and sternocleidomastoid tumor of infancy remains to be one of the mysteries of pediatric surgery. Its cause is still debated and its management is controversial. Methods: Fifty-seven infants and children treated consecutively for sternomastoid tumors and CMT over a 5-year period (1992 to 1997) at the Pediatric Surgery Clinic of Social Security Council Ankara Children's Hospital are reviewed. The obstetric history was recorded in 48 patients. A lump in the neck, head tilt, plagiocephaly, and facial asymmetry were reasons for refferal correlating with the age of admission. Under 18 months of age passive and active stretching exercses (PSE and ASE) were initiated in all of the cases. Results: In 28 children under the age of 3 months the outcome was excellent with none needing surgery. Successively 25% of the 3- to 6-month-old infants, 70% of the 6- to 18-month-old children, and 100% of all the older children required surgery. Spontaneous normal vaginal delivery was recorded in 23 of 48 (48%) cases, whereas vaginal vacuum extraction in four cases, vaginal forceps in six, vaginal as breech in two, and caesarean section in 11 was noted. Conclusions: It was concluded that PSE and ASE are highly effective for the treatment of congenital muscular torticollis. The success rate of conservative treatment is primarily dependent on the patients' age at the initiation of exercises. The hypothesis of injury through the birth canal and intrauterine malposition are valid only in 12 (25%) and 13 (27%) of cases, respectively. Therefore, it is difficult to attribute as an etiologic factor.

Operative Treatment of Acute Displaced Intra-Articular Calcaneus Fractures

A retrospective study of 18 operatively treated calcaneal fractures was conducted. The fractures were all type II and type III fractures according to the classification system of Crosby et al. The average follow-up was 32 months and consisted of subjective and objective criteria. The findings were compared with the results of nonoperative treatment reported by Crosby et al. The findings suggests that type II fractures have a similar outcome when either operative or nonoperative treatment is used. Type III fractures, however, fared considerably better with operative intervention when compared with those treated with nonoperative techniques.

Painful heel syndrome: results of nonoperative treatment.

One hundred five patients (70% female and 30% male; average age, 48 years) with 132 symptomatic heels were treated according to a standard nonoperative protocol and then reviewed at an average follow-up of 29 months. The treatment protocol consisted of nonsteroidal anti-inflammatory medications, relative rest, viscoelastic polymer heel cushions, Achilles tendon stretching exercises, and, occasionally, injections. Obesity, lifestyle (athletic versus sedentary), sex, and presence or size of heel spur did not influence the treatment outcome. Ninety-four patients (89.5%) had resolution of heel pain within 10.9 months. Six patients (5.7%) continued to have significant pain, but did not elect to have operative treatment, and five patients (4.8%) elected to have surgical intervention. Despite attention to the outcome of surgical treatment for heel pain in the current literature, initial treatment for heel pain is nonoperative. The treatment protocol used in this study was successful for 89.5% of the patients.

The Predictive Value of Noninvasive Testing in Intermittent Claudication

Noninvasive tests may be used to aid in deciding the treatment of patients with vascular disease by providing hemodynamic information. The authors stud ied whether the information on the severity of the disease and the location of the diseased segment had any predictive value in regard to the results of nonoper ative treatment of patients with intermittent claudication. Two hundred and one patients suffering from intermittent claudication and not recommended for operative treatment entered the study. They all had nonin vasive tests done, including segmental blood pressure measurements and femo ral velocity waveform analyses. After two years the mortality, surgical intervention rate, patient's subjective assessment, and changes in ankle-brachial index were recorded and analyzed. Twenty-nine patients had died and 12 patients had been operated on during the follow-up. The severity of the disease influenced the mortality; the lower the ankle-brachial index, the higher the mortality. The patients' subjective feel ings of change in symptoms were not affected by the severity of the disease at the start. The location of the disease, whether aortoiliac, femoropopliteal, or femorotibial, did not affect the progression of the disease, but it strongly influenced the patients' subjective assessment of the results of conservative treat ment. Patients with proximal disease were less satisfied with conservative treat ment and more often needed surgical intervention. At the two-year follow-up, 53% of the patients felt better, 35% felt unchanged, and 12% felt worse. Patients' assessments of disease progression are good.

Partial versus complete acute anterior cruciate ligament tears. The results of nonoperative treatment

A study was undertaken to determine whether a significantly different clinical outcome could be expected following nonoperative treatment of acute partial anterior cruciate ligament (ACL) tears from that of complete tears. A detailed follow-up of 107 patients with arthroscopically confirmed tears was obtained; 72 were complete tears and 35 partial. The overall results in those with partial tears were 23% excellent, 29% good, 17% fair, and 31% poor; with complete tears the results were 11% excellent, 20% good, 15% fair, and 54% poor. The patients with partial tears had a lower incidence of associated meniscal tears, needed fewer reconstructions and more of them returned to sport than those with complete tears.
