Treatment of Children Trichobezoar a Retrospective Study of 11 Cases

The goal of this study to summarize the clinical features, treatment and prognosis of children trichobezoar, and to guide the clinical diagnosis and treatment. The clinical manifestations, auxiliary examination results, diagnosis and treatment process and family relationship of 11 cases of children with trichobezoar in our hospital were analyzed retrospectively. 11 cases were female, 4 cases were divorced single parent family, and 2 case was left behind child. Six patients were admitted to hospital with sudden exacerbation of chronic abdominal pain, and four of them had recurrent vomiting. Five patients were admitted to hospital with acute abdominal pain, and 3 of them had vomiting symptoms; The weight of 1 case was lower than -2 SD, 4 cases were between -2 SD ~ -1 SD. 6 cases had palpable left upper abdominal mass with mild tenderness, 1 case only had left upper abdominal tenderness, 4 cases had no positive abdominal signs; The results of color Doppler ultrasonography in 8 children indicated the strong echo mass in the stomach, and 3 of them showed that the hyperechoic group extended to duodenum through pylorus. 7 cases had a tail end extending into the small intestine after removing the gastrolith during the operation. Four cases were found with jejunal perforation, 2 cases were treated with intestinal resection and anastomosis, and 2 cases were treated with intestinal repair. All the children were improved and discharged after operation. For single parent families or left behind children with a history of chronic abdominal pain, vomiting and trichophilia, gastric hair stones should be highly suspected. For large hair stones in the stomach, they should be removed surgically. During the operation, the small intestine should be explored routinely to prevent the missed diagnosis of small intestinal perforation. Psychological guidance should be given to the children and their parents to prevent recurrence.

Association of serum proprotein convertase subtilisin/kexin type 9 with early atherosclerosis in newly diagnosed type 2 diabetes mellitus

Background and AimProprotein convertase subtilisin/kexin type 9 (PCSK9) is rapidly gaining attention as a potential risk of developing atherosclerosis due to its crucial role in the regulation of low-density lipoprotein cholesterol (LDL-C) metabolism. The present study investigated the relationship between serum PCSK9 levels and early atherosclerosis as assessed by carotid intimal-medial thickness (CIMT) and brachial-ankle pulse wave velocity (ba-PWV) in newly diagnosed type 2 diabetes mellitus (T2DM). Methods and ResultsA total of 100 newly diagnosed T2DM were enrolled and further divided into the thickened CIMT group (n = 41) and the non-thickened CIMT group (n = 59) according to the results of color Doppler ultrasonography. Serum PCSK9 levels, CIMT, ba-PWV, and metabolic parameters were measured.Patients in the thickened CIMT group had higher serum PCSK9 levels than patients in the non-thickened CIMT group (all P < 0.05). CIMT and ba-PWV were both positively correlated to serum PCSK9 levels, while serum PCSK9 levels were positively correlated to white blood cell count, neutrophil, lymphocyte, and high-sensitivity C-reactive protein (P < 0.05). Multiple linear regression indicated that serum PCSK9 level was an independent predictor of CIMT (β = 0.637, P < 0.001) and ba-PWV (β = 0.600, P < 0.001). Binary logistic regression analysis showed that serum PCSK9 levels were independent risk factors of thickened CIMT [OR = 1.120, 95%CI (1.041–1.204), P = 0.002]. ConclusionSerum PCSK9 levels are significantly correlated with CIMT and ba-PWV, independent of CAD risk factors. Therefore, serum PCSK9 level may have the potential to serve as a prescriptive biomarker for early arteriosclerosis in newly diagnosed T2DM.

Ultrasound Characteristics of the Nutcracker Syndrome

The article presents the results of color Doppler ultrasonography of the left renal vein and gonadal vein with the determination of the peak systolic velocity of blood flow and abnormal blood reflux in patients with left-sided varicocele.&#x0D; The objective of the research was to compare preoperative ultrasound characteristics of the left renal vein, left gonadal vein, peak systolic velocity of blood flow and the presence of abnormal blood reflux, the formation of ultrasound criteria for the selection of patients for surgical correction of phlebohypertension.&#x0D; Results. Ultrasound signs of aorta mesenteric compression were absent in 24 (24.5%) patients; the signs of aorta mesenteric compression without critical left renal vein stenosis were observed in 64 (65.3%) patients; critical stenosis of the left renal vein was diagnosed in 10 (10.2 %) patients. The patients with critical stenosis of the left renal vein underwent left renal vein transposition.&#x0D; Conclusions. Ivanissevich surgery with prognostically low risk of relapse is recommended for the patients with left-sided varicocele without any signs of aorta mesenteric compression. Patients with the signs of aorta mesenteric compression require clear determination of the degree of left renal vein stenosis. We consider the transposition of the left renal vein to be indicated in case of critical stenosis when the correlation of the diameters of the distal and proximal segments of the left renal vein is ≥ 3 and the ratio of peak systolic velocities in the proximal and distal segments is ≥ 6. Thus, the diagnosis of phleborenohypertension in the patients with varicocele by means of color Doppler ultrasonography with the determination of blood flow velocity indicators in the left renal vein circulation and the determination of the critical stenosis of the left renal vein is decisive in choosing the surgical method of treatment. This provides an opportunity to evaluate the cause of varicocele occurrence and choose the optimal method of surgical treatment.&#x0D;

Incidence of Deep Vein Thrombosis After Spinal Cord Injury: A Prospective Study in 37 Consecutive Patients With Traumatic or Nontraumatic Spinal Cord Injury Treated by Mechanical Prophylaxis

Patients with spinal cord injury (SCI) are at particular risk for deep vein thrombosis (DVT) during their hospital course. In most researches on the prevention of thromboembolic events after SCI, the cause of SCI was usually limited to traumatic origin, and pharmaco-prophylaxis was usually started immediately after SCI irrespective of the presence of DVT. For this reason, it is difficult to determine the exact incidence of DVT after SCI from all possible causes in the absence of anticoagulation treatment. We sought to determine the incidence of DVT and the effect of mechanical treatments without chemical prophylaxis. From November 2009 to October 2010, 37 consecutive patients were admitted to our institute for SCI regardless of causes. Patient data including age, sex, types of injury in motor completeness, causes of SCI, and results of color Doppler ultrasonography (DUS) were collected. Routine mechanical prophylaxis for DVT was performed in all patients; pharmacologic prophylaxis was not used to avoid the potential consequences that may have confounded their benefits. All patients were routinely checked for DVT of lower limbs. Examinations were usually performed within 1 week of injury and repeated fortnightly until any medications for DVT were started in cases of a positive DUS result. In total, 16 of the 37 (43%) patients with acute SCI routinely given mechanical prophylaxis without anticoagulation were found to have DVT in the lower extremities by color DUS. Ten patients showed new thrombosis by DUS within 7 days after injury, three patients after 2 weeks to 3 weeks, and three patients at more than 1 month after injury. The majority of DVT occurred in the distal leg vein (81.2%, soleal vein). The incidence of DVT in patients with traumatic SCI was not different from that of patients with nontraumatic SCI in this study (p>0.05). Age, sex, type of motor impairment, and cause of SCI were not found to be significantly related to the occurrence of DVT. The incidence of DVT in patients with SCI routinely given mechanical prophylaxis without anticoagulation was higher when compared with those reported in the setting of routine pharmaco-prophylaxis. Anticoagulation should not be excluded from initial DVT prophylaxis measures in the SCI patients unless there is any ongoing bleeding or severe coagulopathy. Further studies will be necessary to get a more precise data and to understand the clinical relevance of these results.

Color Doppler Ultrasonography in Occlusive Diseases of the Brachiocephalic and Proximal Subclavian Arteries

Objective. The purpose of this study was to investigate the capability of color Doppler ultrasonography (CDU) in showing the brachiocephalic and proximal subclavian arteries and to determine the accuracy of CDU for diagnosis of occlusive diseases of those arteries. Methods. Two groups of patients were examined. The first group was examined with CDU to determine whether the brachiocephalic and subclavian artery origins could be seen. The second group, including patients with occlusive arterial disease, was examined with CDU before digital subtraction angiography (DSA). Results of CDU and DSA were compared. Results. In the first group, the origins of 42 (84%) of 50 brachiocephalic arteries, 48 (96%) of 50 right subclavian arteries, and 25 (50%) of 50 left subclavian arteries could be displayed by CDU. In the second group, 8 (89%) of 9 lesions on the right and 23 (96%) of 24 lesions on the left were diagnosed correctly. Color Doppler ultrasonography had sensitivity, specificity, positive predictive value, negative predictive value, and accuracy values of 88%, 94%, 78%, 97%, and 94%, respectively, for detecting major stenosis and 100%, 98%, 94%, 100%, and 99% for detecting occlusion. Agreement between the CDU and DSA findings was substantial for stenosis (κ = 0.78) and almost perfect for occlusion (κ = 0.96). Conclusions. The percentage of proximal left subclavian arteries shown on CDU was considerably lower compared with the right subclavian and brachiocephalic arteries. However, there was no significant difference between the two sides in diagnosing occlusive arterial diseases. With CDU, occlusion can be diagnosed more accurately than stenosis. Role of contrast-enhanced 3D magnetic resonance portography in evaluating portal venous system compared with color Doppler ultrasonography

The purpose of this study was to evaluate the capability of contrast-enhanced three-dimensional (3D) MR portography in detecting abnormal findings associated with the portal venous system compared with the results of color Doppler ultrasonography (CDUS). MR portography findings were retrospectively compared with the results of CDUS examinations in 161 patients, who were suspected of having portal venous system abnormalities. Portal venous vessels were divided into main 5 groups including the main portal vein, its left and right intrahepatic branches, splenic vein and superior mesenteric vein. Imaging findings were classified as normal, occluded, or partially thrombosed. Results of clinical and imaging follow-up examinations including CDUS, MR portography or angiography, if available, were used as a proof of final diagnosis. The potential sites of varicose veins and collateral vessels were also examined by both imaging methods. Vascular abnormalities were identified in 79 of 161 patients. There was a statistically significant agreement between the results of MR portography and CDUS in evaluating portal venous system (kappa = 0.871, P < 0.05). The sensitivity of MR portography was slightly superior to CDUS in detecting partially thrombosis and occlusion in the main portal venous vessels. In addition, MR portograms were superior to CDUS in the management of patients with portal hypertension by identifying portosystemic collaterals more adequately, and clearly demonstrated portal venous vessels that cannot be visualized at CDUS. Results of present study indicates that contrast-enhanced 3D MR portography is well suited and superior to CDUS in the management of patients with portal hypertension.

Twenty-six cases have undergone CDU three times in weekly intervals. PSV and EDV measurements of cavernous arteries were obtained both proximally and distally. The results have shown that statistically there was no difference between each measurement obtained on either sides at three different tests. (ANOVA PSV F(2-50) = 0.63, EDV F(2-50) = 0.81). Four of the 26 cases had different PSV results and six cases had different EDV results in three CDU tests. There was a statistical difference in measurements between proximal and distal segments of the cavernous arteries. (t-test t = 2.53, P = 0.015) In conclusion, repetition of CDU would not be necessary in majority of the cases. However investigators should always be aware and cautious about falsely abnormal results of CDU. Proximal segments of the cavernous arteries should be the site to obtain true CDU measurements.

Color Doppler ultrasound of the genitourinary tract

This paper discusses the role of color Doppler ultrasonography (CDU) as it applies to the genitourinary tract. CDU is beneficial in evaluating both the native and allograft kidney in the diagnosis of renal arterial and venous stenosis, arteriovenous fistulas, pseudoaneurysms, arterial and venous thrombosis, and in the characterization and staging of renal masses. CDU has redefined the role of ultrasonography in the evaluation of the scrotum as it relates to testicular torsion, inflammatory disorders and varicoceles. Early results of CDU in prostate carcinoma and benign prostate hypertrophy have shown limited success. Identifying and assessing the neurovascular bundle may have an impact on the staging of prostate carcinoma. It is hoped that the future addition of ultrasound contrast agents will further establish the role of color Doppler in the diagnosis of a variety of genitourinary diseases and disorders.
