Decoding hand kinematics from population responses in sensorimotor cortex during grasping

Objective. The hand—a complex effector comprising dozens of degrees of freedom of movement—endows us with the ability to flexibly, precisely, and effortlessly interact with objects. The neural signals associated with dexterous hand movements in primary motor cortex (M1) and somatosensory cortex (SC) have received comparatively less attention than have those associated with proximal upper limb control. Approach. To fill this gap, we trained two monkeys to grasp objects varying in size and shape while tracking their hand postures and recording single-unit activity from M1 and SC. We then decoded their hand kinematics across tens of joints from population activity in these areas. Main results. We found that we could accurately decode kinematics with a small number of neural signals and that different cortical fields carry different amounts of information about hand kinematics. In particular, neural signals in rostral M1 led to better performance than did signals in caudal M1, whereas Brodmann’s area 3a outperformed areas 1 and 2 in SC. Moreover, decoding performance was higher for joint angles than joint angular velocities, in contrast to what has been found with proximal limb decoders. Significance. We conclude that cortical signals can be used for dexterous hand control in brain machine interface applications and that postural representations in SC may be exploited via intracortical stimulation to close the sensorimotor loop.

Changes of functional response in sensorimotor cortex of preterm and full-term infants during the first year: An fNIRS study

BackgroundMotor impairments are frequently associated with preterm birth and interfere in acquisition of essential skills to global development. Using Near Infrared Spectroscopy (NIRS), the study of neural correlates of motor development in early stages of life are feasible in an ecological assessment. AimsTo evaluate changes in cortical activity in response to a sensorimotor stimulation in preterm and full-term infants at 6 and 12 months of age. Study designA longitudinal study was conducted with 22 infants (12 preterm and 10 full-term). Hemodynamic activity during sensorimotor task (8 blocks of 8 s of vibration applied to infant's right hand) was measured by Functional Near Infrared Spectroscopy (fNIRS). The optical probe consisted of 84 channels positioned according to the international 10–20 system coordinates, covering the frontal (38 channels), parietal (16 channels), temporal (22 channels) and occipital (8 channels) lobes of both hemispheres. ResultsPreterm and full-term infants exhibited differences of location of the activation as well on the hemodynamic response in both the evaluated age groups. ConclusionsGroup differences in activation of sensorimotor cortex observed in this study demonstrate the potential of fNIRS application for preterm evaluation of motor development in children. Overall, the present work contributes to our understanding of cortical activation of cerebral motor skills spanning early ages in preterm-born children.

Visual responsiveness in sensorimotor cortex is increased following amputation and reduced after mirror therapy.

Phantom limb pain (PLP) following amputation, which is experienced by the vast majority of amputees, has been reported to be relieved with daily sessions of mirror therapy. During each session, a mirror is used to view the reflected image of the intact limb moving, providing visual feedback consistent with the movement of the missing/phantom limb. To investigate potential neural correlates of the treatment effect, we measured brain responses in volunteers with unilateral leg amputation using functional magnetic resonance imaging (fMRI) during a four-week course of mirror therapy. Mirror therapy commenced immediately following baseline scans, which were repeated after approximately two and four week intervals. We focused on responses in the region of sensorimotor cortex corresponding to primary somatosensory and motor representations of the missing leg. At baseline, prior to starting therapy, we found a strong and unexpected response in sensorimotor cortex of amputees to visually presented images of limbs. This response was stronger for images of feet compared to hands and there was no such response in matched controls. Further, this response to visually presented limbs was no longer present at the end of the four week mirror therapy treatment, when perceived phantom limb pain was also reduced. A similar pattern of results was also observed in extrastriate and parietal regions typically responsive to viewing hand actions, but not in regions corresponding to secondary somatosensory cortex. Finally, there was a significant correlation between initial visual responsiveness in sensorimotor cortex and reduction in PLP suggesting a potential marker for predicting efficacy of mirror therapy. Thus, enhanced visual responsiveness in sensorimotor cortex is associated with PLP and modulated over the course of mirror therapy.

Exergaming with a pediatric exoskeleton: Facilitating rehabilitation and research in children with cerebral palsy.

Effective rehabilitation of children with cerebral palsy (CP) requires intensive task-specific exercise but many in this population lack the motor capabilities to complete the desired training tasks. Providing robotic assistance is a potential solution yet the effects of this assistance are unclear. We combined a novel exoskeleton and exercise video game (exergame) to create a new rehabilitation paradigm for children with CP. We incorporated high density electroencephalography (EEG) to assess cortical activity. Movement to targets in the game was controlled by knee extension while standing. The distance between targets was the same with and without the exoskeleton to isolate the effect of robotic assistance. Our results show that children with CP maintain or increase knee extensor muscle activity during knee extension in the presence of synergistic robotic assistance. Our EEG findings also demonstrate that participants remained engaged in the exercise with robotic assistance. Interestingly we observed a developmental trajectory of sensorimotor mu rhythm in children with CP similar, though delayed, to those reported in typically developing children. While not the goal here, the exoskeleton significantly increased knee extension in 3/6 participants during use. Future work will focus on utilizing the exoskeleton to enhance volitional knee extension capability and in combination with EMG and EEG to study sensorimotor cortex response to progressive exercise in children with CP.

Functional Laterality of Task-Evoked Activation in Sensorimotor Cortex of Preterm Infants: An Optimized 3 T fMRI Study Employing a Customized Neonatal Head Coil.

BackgroundFunctional magnetic resonance imaging (fMRI) in neonates has been introduced as a non-invasive method for studying sensorimotor processing in the developing brain. However, previous neonatal studies have delivered conflicting results regarding localization, lateralization, and directionality of blood oxygenation level dependent (BOLD) responses in sensorimotor cortex (SMC). Amongst the confounding factors in interpreting neonatal fMRI studies include the use of standard adult MR-coils providing insufficient signal to noise, and liberal statistical thresholds, compromising clinical interpretation at the single subject level.Patients / methodsHere, we employed a custom-designed neonatal MR-coil adapted and optimized to the head size of a newborn in order to improve robustness, reliability and validity of neonatal sensorimotor fMRI.Thirteen preterm infants with a median gestational age of 26 weeks were scanned at term-corrected age using a prototype 8-channel neonatal head coil at 3T (Achieva, Philips, Best, NL). Sensorimotor stimulation was elicited by passive extension/flexion of the elbow at 1 Hz in a block design. Analysis of temporal signal to noise ratio (tSNR) was performed on the whole brain and the SMC, and was compared to data acquired with an ‘adult’ 8 channel head coil published previously. Task-evoked activation was determined by single-subject SPM8 analyses, thresholded at p < 0.05, whole-brain FWE-corrected.ResultsUsing a custom-designed neonatal MR-coil, we found significant positive BOLD responses in contralateral SMC after unilateral passive sensorimotor stimulation in all neonates (analyses restricted to artifact-free data sets = 8/13). Improved imaging characteristics of the neonatal MR-coil were evidenced by additional phantom and in vivo tSNR measurements: phantom studies revealed a 240% global increase in tSNR; in vivo studies revealed a 73% global and a 55% local (SMC) increase in tSNR, as compared to the ‘adult’ MR-coil.ConclusionsOur findings strengthen the importance of using optimized coil settings for neonatal fMRI, yielding robust and reproducible SMC activation at the single subject level. We conclude that functional lateralization of SMC activation, as found in children and adults, is already present in the newborn period.

Low-Frequency Intracortical Electrical Stimulation Decreases Sensorimotor Cortex Hyperexcitability in the Acute Phase of Ischemic Stroke.

Ischemic stroke causes a series of complex pathophysiological events in the brain. Electrical stimulation of the brain has been considered as a novel neuroprotection intervention to save the penumbra. However, the effect on the cells' responsiveness and their ability to survive has yet to be established. The objective of the present study was to investigate the effects of low-frequency intracortical electrical stimulation (lf-ICES) applied to the ischemia-affected sensorimotor cortex immediately following ischemic stroke. Twenty male Sprague-Dawley rats were instrumented with an intracortical microelectrode array (IC MEA) and a cuff-electrode around the sciatic nerve. Photothrombosis intervention was performed within the sensorimotor cortex and the electrophysiological changes were assessed by analysis of the neural responses to stimulation of the sciatic nerve. Neuroprotection intervention consisted of eight 23 min lf-ICES blocks applied to the IC MEA during the initial 4 h following photothrombosis. Our results revealed that the area and magnitude of the sensorimotor cortex response significantly increased if ischemic stroke was allowed to progress uninterrupted, whereas this was not observed for the group of rats subjected to lf-ICES. Our findings indicate that low-frequency electrical stimulation is able to minimize hyperexcitability and may therefore be a candidate as neuroprotection intervention in the future.

EPV 8. Thalamic and subthalamic deep brain stimulation evoke distinct and consistent patterns of cortical responses

Recent research revealed that cortical areas are substantially modulated by deep brain stimulation (DBS). To better understand the spatial and temporal pattern of cortical modulation, we recorded and localized responses evoked by low-frequency DBS using magnetoencephalography (MEG). We investigated nine patients suffering from Essential Tremor (ET) and 11 patients with Parkinson’s disease (PD) following DBS surgery. The DBS target area was the subthalamic nucleus (STN) in PD patients and the ventral intermediate nucleus of the thalamus (VIM) in ET patients. We applied unilateral, bipolar DBS with a frequency of 5 Hz at clinically effective amplitude, using an external stimulator and externalized leads. Dipole fitting was applied to MEG data to estimate the anatomical origin of evoked responses. Evoked responses were observed in 7 of 9 ET patients, and 7 of 11 PD patients. The temporal profile of responses was very consistent within groups. The response to VIM stimulation was best explained by a dipole which alternated between anterior and posterior orientation periodically (peaks around 13, 40, 77, and 116 ms). It localized to the central sulcus ipsilateral to DBS in all cases. Three patients showed an additional response in the contralateral cerebellum. STN DBS evoked responses around 4, 11 and 27 ms in sensorimotor channels, which were an order of magnitude smaller than those evoked by VIM DBS. Localization results were variable across subjects. Ipsilateral precentral gyrus was the most frequently estimated origin among a number of cortical and subcortical areas. Finally, we observed very short latency responses around 1 ms in individual ET and PD patients. We conclude that MEG enables localization of cortical responses evoked by DBS. Our results suggest that sensorimotor cortex is primarily modulated. The response in sensorimotor cortex is larger for VIM than for STN stimulation, which is characterized by a distinct spatio-temporal profile of responses. Deeper source estimates, lower sensor amplitudes and earlier peaks point towards stronger subcortical contributions compared to VIM DBS.

Beta oscillations reveal ethnicity ingroup bias in sensorimotor resonance to pain of others.

People evaluate members of their own social group more favorably and empathize more strongly with their ingroup members. Using electroencephalography (EEG), we explored whether resonant responses of sensorimotor cortex to the pain of others are modulated by the ethnicity of these others. White participants watched video clips of ethnic ingroup and outgroup hands, being either penetrated by a needle syringe or touched by a cotton swab, while EEG was recorded. Time-frequency analysis was applied to Laplacian-transformed signals from the sensors overlying sensorimotor cortex in order to assess event-related desynchronization and synchronization (ERD/ERS) of sensorimotor mu (7-12 Hz) and beta (13-30 Hz) rhythms. When watching needle injections, beta ERD was significantly stronger for ingroup compared with outgroup hands. This ethnicity bias was restricted to painful actions, as beta ERD for ingroup and outgroup hands neither differed when observing no-pain videos, nor during presentation of the hands without any treatment. Such vicarious sensorimotor activation could play a role in social interaction by enhancing the understanding of the feelings and reactions of others and hence facilitating behavioral coordination among group members.

Increase in Sensorimotor Cortex Response to Somatosensory Stimulation Over Subacute Poststroke Period Correlates With Motor Recovery in Hemiparetic Patients

Background. Somatosensory input to the motor cortex may play a critical role in motor relearning after hemiparetic stroke. Objective. The authors tested the hypothesis that motor recovery after hemiparetic stroke relates to changes in responsiveness of the sensorimotor cortex (SMC) to somatosensory input. Methods. A total of 10 hemiparetic stroke patients underwent serial functional magnetic resonance imaging (fMRI) during tactile stimulation and testing of sensorimotor function over 1 year—at early subacute, late subacute, and chronic poststroke time points. Results. Over the subacute poststroke period, increased responsiveness of the ipsilesional SMC to tactile stimulation of a stroke-affected digit correlated strongly with concurrent gains in motor function. Increased responsiveness of the ipsilesional and contralesional SMC over the subacute period also correlated strongly with motor recovery experienced over the first year poststroke. Conclusions. These findings suggest that increased responsiveness of the SMC to somatosensory stimulation over the subacute poststroke period may contribute to motor recovery.

Spontaneous blood oxygenation level‐dependent fMRI signal is modulated by behavioral state and correlates with evoked response in sensorimotor cortex: A 7.0‐T fMRI study

Synchrony measurements of spontaneous low-frequency blood oxygenation level-dependent (BOLD) fluctuations are increasingly being used to investigate spatial regions of functional connectivity. Although information regarding BOLD-BOLD synchrony between different regions is frequently reported, the relationship between spontaneous activity and behavioral state, and the association of spontaneous signal synchrony and evoked response, are less well characterized. The purpose of this study is to exploit the higher signal-to-noise ratio and in turn available spatial resolution at 7.0 T to understand the relationship between synchrony, measured as Pearson's R value, and amplitude, measured as signal standard deviation over time, in sensorimotor cortex for four separate behavioral states: eyes closed resting (EC), eyes open fixation (EO), EO with constant right hand fist clench (EO-F) and EO with 6 s off/6 s on (0.083 Hz) right hand finger tapping (EO-T). BOLD (TE/TR = 25/3,000 ms; 100 time points) scans were performed in healthy volunteers (7.0 T; 4 M/3 F; right-handed) at high spatial resolution = 1.6 × 1.6 × 1.6 mm3 . Results (z > 5; P < 0.05; low-pass filtering <0.067 Hz) reveal that synchrony is highest in the EC state (R = 0.35 ± 0.07) and reduces for EO (R = 0.26 ± 0.07), EO-F (R = 0.23 ± 0.07; P < 0.05), and EO-T (R = 0.12 ± 0.04; P < 0.05) conditions. Amplitude was highest in the EC condition and only reduced significantly (P < 0.05) for the EO-T condition. Synchrony within sensorimotor cortex correlated with evoked finger-tapping response magnitude (R = 0.81; P = 0.03), suggesting that spontaneous signal synchrony may be a predictor of evoked BOLD response magnitude and may account for intersubject variability in sensorimotor cortex.

Longitudinal monitoring of motor neuron circuitry in FALS rats using in-vivo phMRI.

Amyotrophic lateral sclerosis (ALS) presents challenges for diagnosis and objective monitoring of disease progression. We show, using pharmacologic MRI, that alterations in motor circuitry can be characterized using a passive stimulus in a rat model of familial ALS as a function of symptom progression. Presymptomatic familial ALS rats had a pattern of activation to amphetamine that was statistically indistinguishable from the wild-type controls. In contrast, symptomatic rats showed significantly decreased response in sensorimotor cortex and increased response in M2 motor cortex, caudate/putamen, and thalamus. These results are similar to findings in humans of altered response to motor tasks in ALS. It may be plausible to use a passive amphetamine challenge as a biomarker to assess progression of the disease and efficacy of potential treatments.

Spinal and Supraspinal Effects of Long-Term Stimulation of Sensorimotor Cortex in Rats

Sensorimotor cortex (SMC) modifies spinal cord reflex function throughout life and is essential for operant conditioning of the H-reflex. To further explore this long-term SMC influence over spinal cord function and its possible clinical uses, we assessed the effect of long-term SMC stimulation on the soleus H-reflex. In freely moving rats, the soleus H-reflex was measured 24 h/day for 12 wk. The soleus background EMG and M response associated with H-reflex elicitation were kept stable throughout. SMC stimulation was delivered in a 20-day-on/20-day-off/20-day-on protocol in which a train of biphasic 1-ms pulses at 25 Hz for 1 s was delivered every 10 s for the on-days. The SMC stimulus was automatically adjusted to maintain a constant descending volley. H-reflex size gradually increased during the 20 on-days, stayed high during the 20 off-days, and rose further during the next 20 on-days. In addition, the SMC stimulus needed to maintain a stable descending volley rose steadily over days. It fell during the 20 off-days and rose again when stimulation resumed. These results suggest that SMC stimulation, like H-reflex operant conditioning, induces activity-dependent plasticity in both the brain and the spinal cord and that the plasticity responsible for the H-reflex increase persists longer after the end of SMC stimulation than that underlying the change in the SMC response to stimulation.

Human brain structures related to plantar vibrotactile stimulation: A functional magnetic resonance imaging study

The purpose of this study was to investigate the sensorimotor cortex response to plantar vibrotactile stimulation using a newly developed MRI compatible vibration device. Ten healthy subjects (20–45 years) were investigated. Vibrotactile stimulation of the sole of the foot with a frequency of 50 Hz and a displacement of 1 mm was performed during fMRI (echo-planar imaging sequence at 1.5 T) using an MRI compatible moving magnet actuator that is able to produce vibration frequencies between 0 and 100 Hz and displacement amplitudes between 0 and 4 mm. The fMRI measurement during vibrotactile stimulation of the right foot revealed brain activation contralaterally within the primary sensorimotor cortex, bilaterally within the secondary somatosensory cortex, bilaterally within the superior temporal, inferior parietal, and posterior insular region, bilaterally within the anterior and posterior cingular gyrus, bilaterally within the thalamus and caudate nucleus, contralaterally within the lentiform nucleus, and bilaterally within the anterior and posterior cerebellar lobe. The advantages of the new MRI compatible vibration device include effective transmission of the stimulus and controlled vibration amplitudes, frequencies, and intensities. The results indicate that plantar vibration can be a suitable paradigm to observe activation within the sensorimotor network in fMRI. Furthermore, the method may be used to determine the optimal responsiveness of the individual sensorimotor network.

The temporal/spatial evolution of optical signals in human cortex.

Intraoperative measures of functioning brain are an important aspect to understanding normal and diseased cortical response. Previous studies, in animal models, have used optical reflectance maps to illustrate the location and timing of functional activity. We used optical reflectance mapping in patients undergoing parietal tumor resection to reveal the temporal/spatial evolution of perfusion and other related metabolic responses of sensorimotor cortex to peripheral somesthetic stimulation. The somatosensory cortex of seven anesthetized patients was mapped in response to transcutaneous electrical median and ulnar nerve stimulation using optical reflectance imaging. The time course and spatial extent of this response were measured and correlated with evoked potential maps collected during the same conditions. Observable signals first appeared within 1-2 sec, peaked at 3 sec, and disappeared by 9 sec. These signals colocalized with the largest evoked potentials in both the sensory and motor regions and demonstrated topological specificity with median and ulnar nerve stimulation. Maps of this temporal/spatial resolution illustrate the integrative and dynamic nature of the neuronal, vascular, and metabolic responses of human cortex. These data also provide insight to the mechanisms responsible for signals obtained using other brain imaging techniques such as PET and fMRI.

Pharmacological characterisation of excitatory synaptic transmission in the cat red nucleus in vivo

Extracellular recordings were made from the magnocellular neurones of the red nucleus (mRN) in anaesthetised cats. A study was made of the effects of selective excitatory amino acid receptor antagonists on excitatory monosynaptic responses evoked from the sensorimotor cortex (SMC) and cerebellar interpositus nucleus (IPN). Iontophoretically applied CNQX and NBQX antagonised both SMC and IPN responses whereas, d-AP5 inhibited the SMC response but was ineffective to the IPN. At currents that selectively antagonised NMDA responses, CPPene had no effect on either SMC or IPN responses. 7-chlorokynurenate inhibited both SMC and IPN responses but required currents that antagonised both AMPA and NMDA responses and was therefore acting in a non-selective manner. Iontophoretically applied glycine was inhibitory to both agonist and synaptic responses, whilst d-serine potentiated NMDA responses but did not enhance monosynaptic responses of the SMC. However in the presence of either 7-chlorokynurenate or high currents of CNQX that reduced the SMC synaptic activation of the mRN neurones, d-serine attenuated the inhibitory action of these antagonists. It is concluded that monosynaptic responses from the SMC are mediated by both NMDA and non-NMDA receptors whereas the monosynaptic responses evoked from the IPN are mediated only by non-NMDA receptors. The lack of effect of CPPene is consistent with the postulate that two NMDA receptor subtypes are present on mRN neurones.

Neuronal responses in sensorimotor cortex to ramp displacements and maintained positions imposed on hindlimb of the unanesthetized monkey

1. Neuronal responses to passively imposed displacements of the foot were examined in area 3a, the precentral motor cortex (MI), and the first somatic sensory cortex (SI) of unanesthetized monkeys. The foot displacements, in the form of "ramps" and maintained displacements, were applied at several different velocities and achieved a number of maintained positions. The response in relation to the ramp was termed a "dynamic" response, while that in association with the maintained displacement was termed a "static" response. 2. Units in the rostral part of MI (MI/r) and in area 3a usually responded in relation both the ramps and to maintained displacements. The activity of the majority of area 3a and MI/r units was strongly modulated only during ramps of one direction. Such response properties contrast with those observed in the caudal part of MI (MI/c) and areas 3b and 1 of SI, where most units responded in relation to the ramp phase of the displacement in both of two ramp directions and not to the maintained displacements. 3. Virtually all units, including those in MI, displayed a dynamic response. However, the dynamic response amplitude of area 3a neurons greatly exceeded that of MI units for equal velocities of displacement. 4. For area 3a units, peak neuronal activity was well correlated with ramp velocity over the range examined. Such units were a minority in areas 3b and 1 of SI (excluding area 3a) and MI. 5. Most area 3a units and almost half of MI/r units displayed, in addition to a dynamic response, a static response that reflected foot position. MI units were more sensitive than area 3a units to equal degrees of maintained displacement. 6. These results demonstrate that in the unanesthetized, intact animal, responses of most neurons in area 3a and MI fall into two categories: a) mixed dynamic-static responses resembling those of muscle spindle afferents, and b) purely dynamic responses. These data are consistent with the hypothesis that afferent input concerning the velocity and position of a segment of a limb may have a role in cortically mediated regulation of movement and posture, and that both areas 3a and the MI cortex may be directly involved in this regulation. 7. The cytoarchitectonic characteristics of area 3a, as defined in single-unit studies, are discussed. It is argued that the salient characteristic of area 3a in the primate is a thinned internal granular layer (layer IV) that contrasts with the heavily granular area 3b and the agranular area 4.
