Variability of Image Features Computed from Conventional and Respiratory-Gated PET/CT Images of Lung Cancer

Radiomics is being explored for potential applications in radiation therapy. How various imaging protocols affect quantitative image features is currently a highly active area of research. To assess the variability of image features derived from conventional [three-dimensional (3D)] and respiratory-gated (RG) positron emission tomography (PET)/computed tomography (CT) images of lung cancer patients, image features were computed from 23 lung cancer patients. Both protocols for each patient were acquired during the same imaging session. PET tumor volumes were segmented using an adaptive technique which accounted for background. CT tumor volumes were delineated with a commercial segmentation tool. Using RG PET images, the tumor center of mass motion, length, and rotation were calculated. Fifty-six image features were extracted from all images consisting of shape descriptors, first-order features, and second-order texture features. Overall, 26.6% and 26.2% of total features demonstrated less than 5% difference between 3D and RG protocols for CT and PET, respectively. Between 10 RG phases in PET, 53.4% of features demonstrated percent differences less than 5%. The features with least variability for PET were sphericity, spherical disproportion, entropy (first and second order), sum entropy, information measure of correlation 2, Short Run Emphasis (SRE), Long Run Emphasis (LRE), and Run Percentage (RPC); and those for CT were minimum intensity, mean intensity, Root Mean Square (RMS), Short Run Emphasis (SRE), and RPC. Quantitative analysis using a 3D acquisition versus RG acquisition (to reduce the effects of motion) provided notably different image feature values. This study suggests that the variability between 3D and RG features is mainly due to the impact of respiratory motion.

Super-Resolution in Respiratory Synchronized Positron Emission Tomography

Respiratory motion is a major source of reduced quality in positron emission tomography (PET). In order to minimize its effects, the use of respiratory synchronized acquisitions, leading to gated frames, has been suggested. Such frames, however, are of low signal-to-noise ratio (SNR) as they contain reduced statistics. Super-resolution (SR) techniques make use of the motion in a sequence of images in order to improve their quality. They aim at enhancing a low-resolution image belonging to a sequence of images representing different views of the same scene. In this work, a maximum a posteriori (MAP) super-resolution algorithm has been implemented and applied to respiratory gated PET images for motion compensation. An edge preserving Huber regularization term was used to ensure convergence. Motion fields were recovered using a B-spline based elastic registration algorithm. The performance of the SR algorithm was evaluated through the use of both simulated and clinical datasets by assessing image SNR, as well as the contrast, position and extent of the different lesions. Results were compared to summing the registered synchronized frames on both simulated and clinical datasets. The super-resolution image had higher SNR (by a factor of over 4 on average) and lesion contrast (by a factor of 2) than the single respiratory synchronized frame using the same reconstruction matrix size. In comparison to the motion corrected or the motion free images a similar SNR was obtained, while improvements of up to 20% in the recovered lesion size and contrast were measured. Finally, the recovered lesion locations on the SR images were systematically closer to the true simulated lesion positions. These observations concerning the SNR, lesion contrast and size were confirmed on two clinical datasets included in the study. In conclusion, the use of SR techniques applied to respiratory motion synchronized images lead to motion compensation combined with improved image SNR and contrast, without any increase in the overall acquisition times.

Automatic tumour volume delineation in respiratory‐gated PET images

Positron emission tomography (PET) is a state-of-the-art functional imaging technique used in the accurate detection of cancer. The main problem with the tumours present in the lungs is that they are non-stationary during each respiratory cycle. Tumours in the lungs can get displaced up to 2.5 cm during respiration. Accurate detection of the tumour enables avoiding the addition of extra margin around the tumour that is usually used during radiotherapy treatment planning. This paper presents a novel method to detect and track tumour in respiratory-gated PET images. The approach followed to achieve this task is to automatically delineate the tumour from the first frame using support vector machines. The resulting volume and position information from the first frame is used in tracking its motion in the subsequent frames with the help of level set (LS) deformable model. An excellent accuracy of 97% is obtained using wavelets and support vector machines. The volume calculated as a result of the machine learning (ML) stage is used as a constraint for deformable models and the tumour is tracked in the remaining seven phases of the respiratory cycle. As a result, the complete information about tumour movement during each respiratory cycle is available in relatively short time. The combination of the LS and ML approach accurately delineated the tumour volume from all frames, thereby providing a scope of using PET images towards planning an accurate and effective radiotherapy treatment for lung cancer.

Motion Correction and Attenuation Correction for Respiratory Gated PET Images

Positron emission tomography (PET) is a molecular imaging technique which provides important functional information about the human body. However, thoracic PET images are often substantially degraded by respiratory motion, which adversely impacts on subsequent diagnosis. In this paper, a motion correction and attenuation correction method is proposed to correct for motion in respiratory gated PET images and to yield an accurate distribution of the radioactivity concentration. Experimental results show that this method can effectively correct for motion and improve PET image quality. The method is able to provide improved diagnostic information without increasing the acquisition time or the radiation burden.
