Profiles of childhood adversities in Inuit from Nunavik: description and associations with indicators of socioeconomic characteristics, support, and community involvement

ObjectivesDistress and associated health problems reported by Nunavik Inuit emanate from heterogeneous roots, including adverse childhood experiences. This study aims to (1) identify distinct childhood adversity profiles and (2) examine associations between these profiles and sex, socioeconomic characteristics, social support, and community involvement among Nunavimmiut.MethodsIn a sample of 1109 adult Nunavimmiut, sex, socioeconomic characteristics, support, community involvement, residential school attendance, and 10 forms of adverse childhood experiences (ACEs) were documented using questionnaires. Latent class analyses and weighted comparisons were performed for three subgroups: 18–49 years; 50 years and above with experience of residential school; and 50 years and above without experience of residential school. The analysis design, the manuscript drafts, and the key findings were discussed and co-interpreted with the collaboration of community representatives, taking into consideration Inuit culture and needs.ResultsA total of 77.6% of Nunavimmiut reported having experienced at least one form of childhood adversity. Three ACE profiles were identified among the 18–49-year-olds: low ACEs (43.0%), household stressors (30.7%), and multiple ACEs (26.3%). Two profiles characterized ACEs experienced among the 50-year-olds and over with and without history of residential schooling: low ACEs (80.1% and 77.2%, respectively) and multiple ACEs (19.9% and 22.8%, respectively). Among the group of 18–49-year-olds, as compared to the low ACE profile, the profile with household stressors included proportionally more women (odds ratio [OR] = 1.5) and was associated with lower involvement in volunteering and community activities (mean score reduced by 0.29 standard deviation [SD]) and lower family cohesion (SD = − 0.11), while the multiple ACE profile was related to a lower rate of employment (OR = 0.62), lower family cohesion (SD = − 0.28), and lower satisfaction with ability to practice traditional activities (SD = − 0.26).ConclusionChildhood adversities among Nunavimmiut do not occur in isolation and experiencing multiple forms of childhood adversities predicts lower socioeconomic status, support, and community involvement in adulthood. Implications for the planning of health and community services in Nunavik are discussed.

Intergenerational residential school attendance and increased substance use among First Nation adults living off-reserve: An analysis of the aboriginal peoples survey 2017.

The Truth and Reconciliation Commission of Canada (TRCC) published 94 Calls to Action in 2015 to address long-term, intergenerational effects of the residential school system, highlighting the pervasive impact of colonialism on the wellbeing of Indigenous peoples in Canada. Indeed, research with Indigenous populations in Canada has captured that prior experiences of residential schools contributes to the intergenerational transmission of mental and physical health disparities. Despite these studies, further research is needed that contextualizes the influence of residential schools within broader frameworks that consider Indigenous social determinants of health in Canada. As such, the purpose of the present study was to examine patterns of substance use and mental and physical health among individuals with a history of residential school attendance (RSA) and individuals reporting parent or two-generation (parent and grandparent) RSA. Data from the Aboriginal Peoples Survey (2017), involving 10,030 First Nations individuals living off reserve, were analyzed. Self-reported mental and physical health scores were significantly lower among those had attended residential schools, whose parents attended residential schools, and whose grandparents attended residential schools, when compared to those who did not. Further, family RSA was associated with increased substance use among participants, though the findings were variable based on sex and specific substance analyzed. Meanwhile, individual and family RSA was not associated with increased likelihood of a mental health diagnosis. These findings provide additional support for how both parental and two-generation family histories of RSA are associated with individual physical and mental health outcomes. Further, these findings articulate the need for the TRCC's Calls to Action to be actually implemented, including community-based approaches that harness the strength of Indigenous people and communities who aim to close the gap in these health disparities for their children and families.

Parent and/or Grandparent Attendance at Residential School and Dimensions of Cultural Identity and Engagement: Associations with Mental Health and Substance Use among First Nations Adults Living off Reserve

Limited studies have assessed how parent and/or grandparent attendance at residential schools is associated with mental health and substance use among First Nations peoples living off reserve, while also considering how cultural dimensions relate to these outcomes. Analyses of the 2017 Aboriginal Peoples Survey revealed that the odds of self-reported diagnosed mood and anxiety disorders, past-year heavy drinking, and frequent marijuana use were significantly higher among First Nations adults living off reserve who had either a parent and/or grandparent who attended residential schools, even when controlling for covariates. In predicting diagnosed mood disorder, positive cultural identity affect and cultural engagement moderated the effect of parent residential school attendance while cultural exploration moderated the effect of two generations of attendance. Cultural exploration was a protective factor for grandparent residential school attendance in relation to past-year frequent marijuana use. Interventions that are trauma-informed and culturally-based should be considered for this population.

Predictors of Land-Based Activity Participation in a National Representative Sample of Indigenous Individuals Living Off-Reserve.

This study examined data from the 2017 Aboriginal Peoples Survey to consider predictors of land-based activity engagement. We hypothesized that higher self-reported mental and physical health scores, an increased sense of cultural belonging, living in a rural community, and no prior individual or family history of residential school attendance would predict a higher frequency of land-based activity engagement among First Nations individuals living off-reserve. Results from linear regression analyses suggested that an increased sense of cultural belonging, being male, and living in a rural community with a population of less than 1000 people were significant predictors of the frequency of land-based activity engagement. With these preliminary findings, further research can explore how physical and mental health outcomes influence the frequency of land-based activity engagement, in addition to how community-specific indicators may promote higher frequency of these activities, particularly among First Nations individuals living off-reserve.

Parent-Child Separations and Mental Health among First Nations and Métis Peoples in Canada: Links to Intergenerational Residential School Attendance.

First Nations children are over 17 times more likely to be removed from their families and placed in the child welfare system (CWS) than non-Indigenous children in Canada. The high rates of parent-child separation have been linked to discriminatory public services and the Indian Residential School (IRS) system, which instigated a multi-generational cycle of family disruption. However, limited empirical evidence exists linking the IRS to subsequent parent-child separations, the CWS, and mental health outcomes among First Nations, Inuit, and Métis populations in Canada. The current studies examine these relationships using a nationally representative sample of First Nations youth (ages 12–17 years) living in communities across Canada (Study 1), and among First Nations and Métis adults (ages 18+ years) in Canada (Study 2). Study 1 revealed that First Nations youth with a parent who attended IRS had increased odds of not living with either of their biological parents, and both IRS and not living with biological parents independently predicted greater psychological distress. Similarly, Study 2 revealed that First Nations and Métis adults with familial IRS history displayed greater odds of spending time in the CWS, and both IRS and CWS predicted elevated depressive symptoms. The increased distress and depressive symptoms associated with parent-child separations calls for First Nations-led interventions to address the inequities in the practices of removing Indigenous children and youth from their families.

The Valuation of Local School Quality under School Choice

School choice programs break the link between residential location and school attendance, and should weaken the capitalization of school quality into house prices. For the first time, I quantify the effect of one such program—charter school expansions—across several states using a dataset covering charter entries and house prices. I embed an event study of charter entry into a boundary discontinuity design and find that, on average, school choice decreases the valuation of traditional schools by four percentage points. Suggestive evidence shows school choice can lead to neighborhood change through resorting as school boundaries become less important. (JEL H75, I21, I24, I28, R23, R31)

The effect of parental residential school attendance and parental involvement on Indigenous youth’s participation in sport and physical activity during school

Introduction: The objective of this study was to assess the effect of parental residential school attendance on youth's participation in sport and physical activity (S/PA) at school and to explore the potential role that parental education and parental involvement in school activities may play in mediating this relationship.
 Methods: This cross-sectional study used the 2012 Aboriginal Peoples Survey and included youth ages 12-17 (N=4,840). Structural equation modeling techniques were used for models with observed and latent variables. Sampling weights and bootstrap weights were used for all analyses.
 Results: The results indicate 62.87% of youth participated in S/PA; 27.54% at school; 31.52% outside of school, and 40.94% in school and outside school. Mothers who attended residential schools had lower levels of educational attainment. Maternal education had a positive effect on parental involvement in school activities in dual parent households and parental involvement had a positive effect on S/PA. The indirect effects of residential school attendance on parental involvement and participation in S/PA were significant only for youth living with both parents and only for the effect of maternal residential school attendance. Maternal residential school attendance had a negative effect on parental involvement and on participation in S/PA.
 Discussion: The findings from this study indicate that interpersonal factors and historical contexts shape Indigenous youth's participation in S/PA.

Factors Affecting Initiation and Duration of Breastfeeding Among Off-Reserve Indigenous Children in Canada

Indigenous children in Canada are less likely to be breastfed compared to non-Indigenous children; however, little information about rates and correlates of breastfeeding exist. We used a nationally representative survey to examine breastfeeding initiation (n = 9,330) and duration (n = 6,760) among First Nations, Métis, and Inuit children. In our sample, 72.5% of children had been breastfed, and 57.9% of these individuals were breastfed until six months. Factors associated with increased breastfeeding included mothers’ educational attainment, children’s weight at birth, mothers' residential school attendance, and region of residence. Having Indian Status and lower household income were associated with lower breastfeeding initiation and duration. Our findings suggest that targeted efforts to encourage and support breastfeeding among Indigenous women are needed. Additional research using contemporary data are required in Canada.

The biological impacts of Indigenous residential school attendance on the next generation.

We investigated the biological impacts of Indigenous residential school attendance on the adult children of survivors, operationalized through allostatic load (AL); and the extent to which intergenerational trauma, operationalized through adverse childhood experience (ACE) score, mediated this association. Data were collected in-person from a university-based sample of Indigenous adults (N = 90, mean age: 28 years) in a mid-sized city in western Canada between 2015 and 2016. Associations were analyzed in multinominal regression models, with terciled AL and ACE scores as outcomes. The cross-products of coefficients method was used to test mediation. Overall, 42.7% and 33.7% reported their mother and father had attended residential school; respectively. In an adjusted model, maternal, but not paternal, residential school attendance was a risk factor associated with a moderate increase in AL among her adult children. The strength of this association did not change when the analysis was limited to mothers who raised their children. Maternal and paternal residential school attendance were each associated with increased ACE score among adults raised by survivors. However, ACE score did not explain the association between maternal residential school attendance and offspring AL score in mediational analyses. The present findings suggest colonial residential school experiences may have become biologically embedded, passed to subsequent generations, and exhibited through the dysregulation of allostatic systems among the adult children of maternal residential school survivors. Maternal exposure to residential school influenced biological dysregulation among her adult children in ways that could not be further exacerbated by her children's exposure to ACEs. The Canadian Truth and Reconciliation Commission asked governments to acknowledge the impact of residential schools on the current state of Indigenous health. Our findings underline the importance of this call by demonstrating how the residential school experience may get under the skin to impact the health of the next generation.

Suicidal thoughts and attempts in First Nations communities: links to parental Indian residential school attendance across development.

The Indian residential school (IRS) system in Canada ran for over a century until the last school closed in 1996. Conditions in the IRSs resulted in generations of Indigenous children being exposed to chronic childhood adversity. The current investigation used data from the 2008-2010 First Nations Regional Health Survey to explore whether parental IRS attendance was associated with suicidal thoughts and attempts in childhood, adolescence and in adulthood among a representative sample of First Nations peoples living on-reserve across Canada. Analyses of the adult sample in Study 1 (unweighted n=7716; weighted n=186,830) revealed that having a parent who attended IRS was linked with increased risk for suicidal thoughts and attempts in adolescence and adulthood. Although females were negatively affected by having a parent who attended IRS, the link with suicidal ideation in adulthood was greater for males. Analyses of the youth sample in Study 2 (unweighted n=2883; weighted n=30,190) confirmed that parental IRS attendance was associated with an increased risk for suicidal ideation and attempts. In contrast to the adult sample, parental IRS attendance had a significantly greater relation with suicidal ideation among female youth. A significant interaction also emerged between parental IRS attendance and age in the youth sample, with the influence of parental attendance being particularly strong among youth ages 12-14, compared with those 15-17 years. These results underscore the need for culturally relevant early interventions for the large proportions of Indigenous children and youth intergenerationally affected by IRSs and other collective traumas.

Neighbors but Not Classmates: Neighborhood Disadvantage, Local Violent Crime, and the Heterogeneity of Educational Experiences in Chicago

Large urban school districts are increasingly offering their students options that break the link between residential location and school attendance. Individual decisions are likely to aggregate in different ways across communities, leaving students in some neighborhoods with more varied educational experiences. In this paper, I explore these patterns using a complete cohort of Chicago Public School high school freshmen in 2009 (N = 24,019) and explore the characteristics of different schools that students living in the same neighborhood attend. Students in poor and violent neighborhoods attend schools that are more heterogeneous in terms of type, achievement, safety, and student demographics. Students from these neighborhoods also scatter geographically to schools across the city, many traveling quite long distances. In contrast, students from safe and affluent neighborhoods attend many fewer schools, are less likely to make long distance trips, and are more likely to attend school with a large proportion of their neighbors.

Are Indigenous Determinants of Health Associated with Self-Reported Health Professional-Diagnosed Anxiety Disorders Among Canadian First Nations Adults?: Findings from the 2012 Aboriginal Peoples Survey.

We estimated the prevalence of self-reported health professional-diagnosed anxiety disorders among Canadian First Nations adults living off-reserve, and assessed the relationship between anxiety disorders and Indigenous determinants of health (Status Indian, residential school attendance, knowledge of Indigenous language, and participation in traditional activities) using the 2012 Aboriginal Peoples Survey. Multivariable logistic regression models were performed using bootstrap weights. The prevalence of anxiety disorders was 14.5% among off-reserve First Nations adults. There was an increased odds of anxiety disorders among those participating in traditional activities compared to their counterparts (aOR 1.46, 95% CI 1.12-1.90). No association was found between anxiety disorders and other Indigenous determinants of health. There is a high prevalence of self-reported anxiety among First Nations adults living off-reserve. However, further studies are warranted to identify and assess the role of Indigenous determinants of health for anxiety disorders and other prevalent mental health conditions in this population.

“I’m not really healed … I’m just bandaged up”: Perceptions of healing among former students of Indian residential schools

Attempts at resolution between former students of Indian residential schools and the non-Aboriginal Canadian population began with the signing of the Indian Residential Schools Settlement Agreement in 2006. The Settlement Agreement outlined provisions for the Truth and Reconciliation Commission to document the stories of former students and for the Resolution Health Support Program to offer emotional and cultural support to former students and their families. Although former students have catalogued their stories through the Truth and Reconciliation Commission process, experiences of healing from the events of Indian residential schools remain relatively unknown. The purpose of this qualitative study was to explore the perceptions of healing among former Indian residential school students. In partnership with an Aboriginal support agency in a small Saskatchewan city, we interviewed 10 Aboriginal people affected by residential schools. The focus of the interviews was to generate participants’ conceptions and experiences of healing regarding their residential school experiences. We found all participants continued to experience physical, mental, emotional, and/or spiritual impacts of residential school attendance. Disclosure of their experiences was an important turning point for some participants. Their efforts to move on varied from attempting to “forget” about their experience to reconnecting with their culture and/or following their spiritual, religious, or faith practices. Participants also noted the profound intergenerational effects of residential schools and the need for communities to promote healing. The findings will be used to guide an assessment of the healing needs among this population in Saskatchewan.

Aboriginal Language and School Outcomes: Investigating the Associations for Young Adults

Being taught an Aboriginal language at school has generally been associated with positive school outcomes for Aboriginal children but not adults. This study attempted to understand this discordance by examining three possible explanations: (a) confounding variables, (b) a cohort effect, and (c) differences in the timing and duration of Aboriginal language instruction. Confounding variables (school attendance on reserve, parental education, and family residential school attendance) and duration of Aboriginal language instruction (six of more grades) were important contributors; whereas the presence of a cohort effect and the timing of Aboriginal language instruction were not found to be significant. Future studies of Aboriginal language instruction should consider family educational experiences, location of schooling, and the duration of Aboriginal language instruction.

The Intergenerational Effects of Residential Schools on Children’s Educational Experiences in Ontario and Canada’s Western Provinces

The intergenerational effects of Canada’s Indian Residential Schools have been widely discussed, but limited empirical work exists. I use the confidential wave of the 2001 Aboriginal Peoples Survey of Children and Youth (APSCY) to study the association between mothers’ residential school attendance and their children’s educational outcomes and experiences in Ontario, Manitoba, Saskatchewan, Alberta, and British Columbia. Holding a number of factors constant, I demonstrate that children whose mothers attended residential school are more likely to be suspended or expelled and have worse school experiences on average than children whose mothers did not. Children are also more likely to live off reserve and less likely to speak an Aboriginal language if their mothers attended a residential school. I also examine some contextual factors that may influence the relationship between mothers’ residential school attendance and their children’s educational outcomes. These findings suggest that dealing with the intergenerational legacy of residential schools is necessary for improving the educational outcomes of today’s Aboriginal youth.

Do discrimination, residential school attendance and cultural disruption add to individual-level diabetes risk among Aboriginal people in Canada?

BackgroundAboriginal peoples in Canada (First Nations, Metis and Inuit) are experiencing an epidemic of diabetes and its complications but little is known about the influence of factors attributed to colonization. The purpose of this study was to investigate the possible role of discrimination, residential school attendance and cultural disruption on diabetes occurrence among First Nations adults.MethodsThis 2012/13 cross sectional survey was conducted in two Saskatchewan First Nations communities comprising 580 households and 1570 adults. In addition to self-reported diabetes, interviewer-administered questionnaires collected information on possible diabetes determinants including widely recognized (e.g. age, sex, lifestyle, social determinants) and colonization-related factors. Clustering effect within households was adjusted using Generalized Estimating Equations.ResultsResponses were obtained from 874 (55.7 %) men and women aged 18 and older living in 406 (70.0 %) households. Diabetes prevalence was 15.8 % among women and 9.7 % among men. In the final models, increasing age and adiposity were significant risk factors for diabetes (e.g. OR 8.72 [95 % CI 4.62; 16.46] for those 50+, and OR 8.97 [95 % CI 3.58; 22.52] for BMI 30+) as was spending most time on-reserve. Residential school attendance and cultural disruption were not predictive of diabetes at an individual level but those experiencing the most discrimination had a lower prevalence of diabetes compared to those who experienced little discrimination (2.4 % versus 13.6 %; OR 0.11 [95 % CI 0.02; 0.50]). Those experiencing the most discrimination were significantly more likely to be married and to have higher incomes.ConclusionsKnown diabetes risk factors were important determinants of diabetes among First Nations people, but residential school attendance and cultural disruption were not predictive of diabetes on an individual level. In contrast, those experiencing the highest levels of discrimination had a low prevalence of diabetes. Although the reasons underlying this latter finding are unclear, it appears to relate to increased engagement with society off-reserve which may lead to an improvement in the social determinants of health. While this may have physical health benefits for First Nations people due to improved socio-economic status and other undefined influences, our findings suggest that this comes at a high emotional price.

Impact of residential schooling and of child abuse on substance use problem in Indigenous Peoples

Residential schools were the institutions, in operation from the 19th century to the late 20th century, which Indigenous children in Canada were forced to attend. The literature shows that many young people who attended these institutions were victims of neglect and abuse. Negative psychological effects resulting from child abuse have been amply documented. However, very few studies on this subject have been carried out among Canada's Indigenous Peoples. The objective of this study is to evaluate, for an Indigenous population in Quebec (Canada), the impact of residential schooling as well as self-reported experiences of sexual and physical abuse during childhood on the development of alcohol and drug use problems in adulthood. A total of 358 Indigenous participants were interviewed (164 men [45.8%] and 194 women [54.2%]). Alcoholism was evaluated using the Michigan Alcoholism Screening Test (MAST). Drug abuse was assessed with the Drug Abuse Screening Test-20 (DAST). Child abuse and residential schooling were assessed with dichotomous questions (yes/no). Among the participants, 28.5% (n=102) had attended residential schools, 35.2% (n=121) reported having experienced sexual abuse, and 34.1% (n=117) reported having experienced physical abuse before adulthood. Results of the exact logistic regression analyses indicated that residential school attendance was linked to alcohol problems, while child abuse was related to drug use problems. The results of this study highlight the importance of considering the consequences of historical traumas related to residential schools to better understand the current situation of Indigenous Peoples in Canada.

At the Edges of Embodiment: Determinants of Breastfeeding for First Nations Women

In Canada, First Nations women are far less likely to breastfeed than other women. First Nations people have been subjected to massive health and social disparities and are at the lowest end of the scale on every measure of well-being. The purpose of this study is to understand the experiences, strengths, and challenges of breastfeeding for First Nations women. Central to the current research is the notion of an embodiment within indigenous women's health and, more specifically, breastfeeding perspectives. Guided by an indigenous feminist standpoint, our research study evolved through honest discussions and is informed by relevant public health literature on breastfeeding. We collected quantitative data through a survey on demographics and feeding practices, and we conducted focus groups in three Canadian provinces (British Columbia, Manitoba, and Ontario) over a period of 1 year (2010) from 65 women in seven First Nation communities. Three overarching themes are discussed: social factors, including perceptions of self; breastfeeding environments; and intimacy, including the contribution of fathers. The main findings are that breastfeeding is conducive to bed sharing, whereas a history of residential school attendance, physical and psychological trauma, evacuations for childbirth, and teen pregnancy are obstacles to breastfeeding. Also, fathers play a pivotal role in a woman's decision to breastfeed. Findings from this study contribute to informing public health by reconsidering simplistic health promotion and public health policies and, instead, educating First Nations communities about the complexity of factors associated with multiple breastfeeding environments.

The Lifetime Effect of Residential School Attendance on Indigenous Health Status

I examined the health impact of lifetime Indian Residential school (IRS) attendance and the mediating influences of socioeconomic status and community adversity on health outcomes in a national sample of Aboriginal peoples in Canada. In an analysis of data on 13,881 Inuit, Métis, and off-reserve First Nations or North American Indian adults responding to the postcensus 2006 Aboriginal Peoples Survey administered October 2006 to March 2007, I tested the direct effect of IRS attendance on health and indirect effects through socioeconomic and community factors using logistic regression procedures. Negative health status was significantly more likely with IRS attendance than nonattendance. The direct effect of IRS attendance remained significant although it attenuated substantially when adjusting for demographic characteristics, socioeconomic status, and community-level adversities. Community adversity and socioeconomic factors, primarily income, employment status, and educational attainment mediated the effect of IRS on health. Residential school attendance is a significant health determinant in the Indigenous population and is adversely associated with subsequent health status both directly and through the effects of attendance on socioeconomic and community-level risks.
