Analysing the reasons why patients with complete missing teeth seek re-prosthodontics

Basic construction material of such dental constructions is not without well-known disadvantages. Identification of the main reasons for the replacement of complete removable acrylic plate dentures will make it possible to find the most relevant area for development in the field of improvement of this type of prosthetics.AIMS: To assess the reasons for patients' requests for replacement of complete removable acrylic dentures.METHODS: In the period from 2021 to 2022, a comprehensive dental examination of patients with complete absence of teeth who sought dental prosthetic care to replace previously used full removable plate prostheses was carried out, the reasons for the need for a new dental structure of this type and the frequency of their occurrence in the near and distant future were determined. Statistical differences between the indicators were assessed using the nonparametric method of analysis - the χ2 test. In order to assess the risk of a particular cause of referral, relative risk was calculated at 95% confidence interval.RESULTS: The main reason for the patients with complete absence of teeth to replace the operated full removable plate prosthesis in the period up to 3 years is one or another mechanical breakage. After three years of use of this type of dental structure, risk of mechanical failure and functional deficiencies statistically significantly increases by 1.8 and 1.6 times respectively. When prostheses are used for more than 6 years, referrals due to various functional deficiencies begin to dominate the percentage with 58.3%.CONCLUSIONS: The increasing risk of functional defects in the form of impaired fixation and stabilisation makes it necessary to develop a set of measures to prevent the development of atrophic processes in the alveolar bone tissue of the upper and lower jaw alveolar processes, as well as dystrophic changes in the mucous membrane of the denture bed.

Candida-related lesions dalam rongga mulut: diagnosis dan strategi penatalaksanaanya

Background: Candida–related lesions can present as denture sore mouth intraorally and cheilitis angular extra orally. Some predisposing factors include ill-fitting dentures, malnutrition, and poor oral hygiene. Objective: To determine the proper diagnosis and management of sore mouth dentures and cheilitis angular in patients with ill-fitting dentures and bad oral hygiene. Case report: A 59-year-old male patient presented with a burning sensation on the hard palate and sores at the corners of the lips, which were aggravated by the meal and opening of the mouth. No history of systemic diseases was found. Extra-orally erythema fissure lesions were found surrounded by multiple white areas at the corners of the lips bilaterally and were painful. Intra-oral examination showed erythema macular lesions that extend to the gingiva and palate mucosa that contact the denture base. Nystatin oral suspension combined with fluconazole, and multivitamins as supportive therapy. Educational suggestions such as the replacement of dentures and improve oral hygiene conditions were given. Conclusion: Denture sore mouth and cheilitis angular in this case are caused by Candida albicans related to the insufficient denture and poor oral hygiene. Management for this case by providing causative treatment with single or combine antifungal drugs along with proper communication, instructions, and education regarding the diseases.

Evolution of Chewing Force in Geriatric Edentulous Patients

Abstract Objective The aim of this study was to assess the evolution of chewing force in edentulous geriatric patients rehabilitated with complete dentures. Chewing function is compromised in individuals who lose all their teeth, as well as in those with complete dentures. The maintenance and recovery of the chewing function in these patients require replacement of the lost natural teeth. Materials and Methods In this study, the chewing force was evaluated by electromyographic analysis of the temporalis and masseter muscle endings in 120 patients with old and new bimaxillary complete dentures. Results Replacement of complete dentures was shown to improve stability, retention, occlusion, and support of the prosthesis. All jaw movements were evaluated using electromyography to assess the evolution of chewing force with and without occlusal adjustment, identify premature contact points, identify occlusal interference, respect the balanced occlusal scheme, and assess the increase in muscle strength. This study found that muscle strength in patients with prosthesis without occlusal adjustment was 527.1 N but was higher for prostheses with occlusal adjustment at 614.7 N, and this strength of 614.7 N was maintained after 1 month of control. Conclusion The chewing force evolved in patients with complete dentures, and an increase was observed when occlusal adjustment was performed.

Background. The problem of preventing complications of complex dental treatment is one of the main problems in modern dentistry, the solution of which is impossible without a systematic approach, including, among other things, the relationship of these complications with the general somatic status of a particular patient. The aim of the study. Identification of the features and terms of use of dentures in patients with reduced thyroid function. Methodology. The data of 304 patients who applied to a dental clinic for re-prosthetics of the dentition were studied. The distribution of patients into groups was carried out in accordance with the type of denture and the presence or absence of thyroid diseases established by the endocrinologist, leading to hypothyroidism. Standard clinical (survey, examination, etc.) and paraclinical (radiological, functional) methods of examination were used for such diagnoses. Edema of the oral mucosa was determined by palpation, and their frequency and severity were recorded. Results. Statistically significant differences in the reasons for replacement of dentures between patients of different groups were revealed. The difference in the terms of use of dentures in patients of different groups was determined. Clinical differences in the state of the tissues of the prosthetic bed and the supporting teeth of the bridge prostheses in patients, depending on the presence of hypofunction of the thyroid gland, were revealed. Conclusions. The presence of hypothyroidism in the anamnesis of patients worsens the long-term prognosis of dental orthopedic treatment. The dispensary observation of patients with hypothyroidism should differ from the observation of healthy individuals, in the direction of reducing the inter-examination intervals in order to identify early complications of dental treatment.

The Role of New Removable Complete Dentures in Stimulated Salivary Flow and Taste Perception.

To evaluate the effect of replacement of inadequate complete dentures on salivary flow and taste perception in geriatric patients. Thirty-three patients, 13 males and 20 females, with a mean age of 64.4 years were submitted to stimulated and unstimulated salivary flow rate and salivary pH measurements, and sense of taste evaluation. Tests were performed 3 months before complete denture substitution and 3 weeks after denture insertion. The mean for unstimulated saliva (USS) was 2.1 ml before and 2.7 ml after replacement (p = 0.003). The mean volume of stimulated saliva was 6.3 ml before and 8.2 ml after replacement (p = 0.004). The pH mean of USS was 7.8 ± 0.44 before and 8.02 ± 0.41 after replacement (p = 0.005). No statistically significant difference was determined in the sense of taste before and 3 weeks after complete denture replacement. The replacement of inadequate complete dentures increases saliva flow; however, it does not improve taste perception.

Patient satisfaction and oral health‐related quality of life after treatment with traditional and simplified protocols for complete denture construction

The objective of this randomised controlled clinical trial study was to compare the effectiveness of a traditional and a simplified protocol for construction of conventional CD. The replacement of conventional dentures can result in potential functional and aesthetic benefits to the patient. Previous studies suggest that simplified procedures for complete dentures (CD) construction achieve results similar to the traditional methods. Fifty patients were randomly divided into two equal groups, traditional protocol (T group) and simplified protocol (S group). Treatment outcomes were assessed before the insertion of the new dentures and 30 days and 6 months after the last adjustment. It included measurements of quality of life related to oral conditions measured by the Brazilian version of OHIP-Edentulous scale and patients' satisfaction with the upper and lower dentures using a visual analogue scale (VAS), which combines the patient's perception in relation to overall satisfaction with the comfort, stability, ability to chew, ability to talk and aesthetics. The results showed significant reduction in negative impacts of oral conditions on quality of life and improved satisfaction with the upper and lower dentures (p < 0.001) at the 30-day and 6-month follow-up. Quality of life improvement occurred in all dimensions of the OHIP-Edentulous (p < 0.001). Despite the significant reduction in quality of life impacts and significant increase in patients' satisfaction with the dentures, there were no differences between the traditional and simplified protocols (p > 0.05). It was concluded that the simplified protocol results in patent's perception of treatment outcomes similar to the traditional protocol.

Selection patterns of dietary foods in edentulous participants rehabilitated with maxillary complete dentures opposed by mandibular implant-supported prostheses: A multicenter longitudinal assessment

Documentation of long-term changes in food intake is lacking for those treated with a maxillary complete denture opposed by a mandibular, screw-retained, implant-supported fixed prosthesis. The purpose of this study was to evaluate the selection patterns of dietary foods over 5 years for edentulous participants treated in a multicenter prospective clinical trial that compared cast alloy versus laser-welded titanium frameworks of an implant-supported prostheses opposed by maxillary complete dentures to rehabilitate edentulous participants. The study assessed data from a multicenter prospective clinical trial that followed edentulous participants from an initial baseline of wearing existing complete dentures to implant placement, restoration with a mandibular implant-supported prosthesis opposed by a maxillary complete denture, to follow-up assessment of these treatments over 5 years. The 32 participants in the cohort were treated at 5 of 9 participating centers. The data collected included 2 dietary forms, a standardized 4-day food diary form, and a dietary habits questionnaire. Each participant completed forms before entering into treatment (control) and at the 1- and 5-year follow-up assessment after being treated with a maxillary complete denture opposing a mandibular complete-arch fixed implant-supported prosthesis. Descriptive statistics were calculated for each measure at each assessment point. Regression analysis and the Sign test were used to calculate change in the participants' nutritional status (α=.05). Among the findings, it was noted that difficulty in masticating hard, raw, and fibrous foods decreased and intake of vegetable portions increased significantly from 2.5 to 3.3 servings. Participant comfort in eating in public places and their enjoyment of eating were significantly improved from 50% of participants being uncomfortable with their prior complete denture treatment to only 4% after 5 years. Within the limitations of this study, it was concluded that vegetable intake and ability to masticate raw, hard, and fibrous food for these participants improved when they received a mandibular implant-supported prosthesis opposed by a maxillary complete denture. According to the analysis and findings, the overall eating experience was more pleasurable, and eating in public was more comfortable after replacement of complete dentures with a mandibular implant-supported prosthesis.

Mandibular implant supported complete dentures improved quality of life

Randomised controlled trial. Patients from hospital waiting lists who had been edentulous for at least two years and required replacement of conventional dentures were recruited. Baseline quality of life and satisfaction measures were taken and all patients were provided with new conventional complete dentures (CCDs) that they wore for three months, at which point they were reassessed using the same measures. Patients were then randomly assigned either to continue with CCDs (control) or to have implant-retained overdentures (IODs) made (test group). The control group was assessed after a further three months (six months after receiving CCDs). The test group was assessed three months after receiving IODs. The outcome measures used were the Oral Health Impact Profile-49 (OHIP-49) Denture Satisfaction Questionnaire (DSQ) and the Schedule for the Evaluation of Individual Quality of Life (SEIQoL). Three months after receiving CCDs patients reported significant improvements in satisfaction and quality of life (P < 0.05). However no further improvements were seen in the control group at six months for any measure. Following placement of IODs the test groups had significant additional improvements in the functional limitation, physical pain, psychological discomfort, physical disability, social disability, psychological disability and handicap scales of the OHIP and on 10 of the 11 scales of the Denture Satisfaction Questionnaire (P < 0.05, ANOVA). The findings show that, controlling for expectancy bias and variability in baseline levels, IODs significantly increase patient satisfaction, dental function and quality of life over and above those achieved with good quality CCDs.

Association between self‐assessment of complete dentures and oral health‐related quality of life

The purpose of this study was to determine which factors related to patient self-assessment of dentures are associated with changes in oral health-related quality of life (OHRQoL) among edentulous patients after replacement of complete dentures, and to determine whether masticatory performance as determined using an objective method affects the changes in OHRQoL among edentulous patients. As a preliminary study, the existing questionnaire regarding self-assessment of dentures consisting of 39 question items, measured with a 100-mm visual analogue scale, was analysed by factor analysis. Then a questionnaire, composed of 22 question items, was developed containing six subscales of 'function', 'lower denture', 'upper denture', 'expectation', 'aesthetic and speech' and 'importance'. Final participants in the present study comprised 93 edentulous patients requiring new conventional complete dentures (44 men, 49 women; mean age, 75·0 years). These patients were asked to complete the Japanese version of the Oral Health Impact Profile (OHIP)-EDENT, comprising 19 question items for assessment of OHRQoL in edentulous patients, along with the developed questionnaire regarding self-assessment of dentures. Moreover, masticatory performance was measured using a colour-changeable chewing gum. The questionnaire and measurement were completed twice; before and after replacement of complete dentures. Stepwise multiple regression analysis identified 'lower denture' and 'aesthetic and speech' as significant independent variables besides OHIP-EDENT scores before replacement. These results suggest that sufficient retention of lower dentures and appropriate appearance may lead to improved OHRQoL in edentulous patients.

Oral health status of patients after replacement of removable partial dentures after three years

Objective(s): To assess the effect of removable partial dentures on the abutment teeth in comparison with that of theremaining dentition, and to investigate the effect of regular check up on periodontal health for patients using thistype of prosthesis.Methodology: One hundred patients wearing removable partial dentures for at least three years included in thestudy. Teeth used as direct or in direct retainers for the removable partial denture were considered as the studygroup, while the remaining dentition in the same jaw was considered as the control group. The following periodontalparameters were registered for each tooth, plaque and gingival indices, clinical attachment level and tooth mobility.Patients were divided into two groups as regular and irregular attendants.Results: Among the 100 patient, 30 were regular attendants. A significant difference in clinical attachment level andplaque accumulation was detected between study and control group of teeth. Rapid denture fabrication for institutionalized patients

Purpose: Edentulism is a common condition among the elderly population. However, it is difficult for elderly subjects who are institutionalized, hospitalized, or bedridden to visit dental offices for the repair or replacement of broken dentures. This paper describes a fast technique with minimum clinical sessions to fabricate a new maxillary denture. Case description: An institutionalized 78-year-old man with an extremely damage broken maxillary denture complained about repairing his denture. With the impossibility of repairing, the technique described was proposed so as to offer a treatment that would maximize the comfort of the patient. The maxilla-mandibular relationship was done at the first clinical session, after fabricating the maxillary resin baseplate in mouth, by fixing the old complete mandibular denture with the maxillary wax plane. At the second clinical session, the esthetics and function of artificial teeth was checked and final impression was made to the denture installation at next session. Conclusion: The reduced number of clinical sessions is a convenient option for clinicians to provide a more comfortable treatment for institutionalized or bedridden patients. This technique is also particularly useful when a replacement denture is urgently required, allowing its installation within few hours.

Increase of salivary flow rate along with improved occlusal force after the replacement of complete dentures

We hypothesized that improvement of occlusal force would cause an increase in salivary flow rate (SFR) even in elderly complete denture wearers. The purpose of this study was to examine the change of SFR after the replacement of complete dentures. Subjects were 22 edentulous outpatients of the Osaka University Dental Clinic. Unstimulated whole saliva was collected for 5 minutes. Stimulated whole saliva was collected for 2 minutes. The maximal occlusal force was measured with pressure-sensitive sheets. All measurements were done before and after replacement of the dentures. The replacement of complete dentures improved maximal occlusal force and increased their stimulated and unstimulated SFR. Stimulated SFR was positively correlated with unstimulated SFR both before and after the replacement. Maximal occlusal force, stimulated SFR, and unstimulated SFR after the replacement were significantly correlated to the measurements before the replacement. These results suggest that proper prosthetic treatment can increase not only maximal occlusal force but also salivary flow rate.

The Current and Future Treatment of Edentulism

The purpose of this review and summary is to focus the clinician's attention on existing potential limitations regarding the management of edentulism. The current published data and opinions concerning the need for treating edentulism, the quality of dentures, related morbidity, and alternative or related therapeutics (e.g., dental implants) suggest there are opportunities for improvement in the treatment of the edentulous population. This may be achieved by adopting a broader therapeutic strategy focused not solely on technical aspects of an oral prosthesis. Instead, a wider array of clinical features of the edentulous patient should be addressed. A contemporary strategy may include concerns for prevention of tooth loss, evaluation of residual alveolar ridge resorption, and related issues of denture function, continual evaluation of oral mucosal health, compassionate management of maladaptive patients, a rationale for timely replacement of dentures, and continued development of dental implant therapies. The importance of therapeutic technical quality can be underscored, but should not overwhelm the broader concerns for assuring the overall health and well-being of the edentulous population.

Maximum bite force after the replacement of complete dentures

In complete denture wearers the maximum bite force (MBF) is known to be considerably lower than in dentate people. Low MBF might therefore be an indication of poor denture fit but there is limited evidence on this. Therefore, the aim of the present study was to investigate whether MBF can be improved by the replacement of complete dentures for elderly people. Nine edentulous volunteers, average age 74·2 ± 5·5 years and average denture experience 19·4 ± 19·5 years (1–50 years), had replacement dentures made. Functional impressions were taken after border moulding using zinc oxide eugenol paste. After a rehearsal session, MBF was recorded with the old dentures, and with the new dentures immediately at insertion, at 3, 8 days, 2–3 weeks, 1, 2, 3 and 6–10 months post-insertion (p.i.). The MBF was recorded with the central bearing point method using a full-bridge strain gauge with a confirmed linearity from 1 to 1000 N and an accuracy of ±1 N. Data were analysed off-line using the mean of two peak readings per patient per session. The results indicate that MBF tended to be impaired when replacement dentures were first fitted (n.s.). However, this trend reversed during the first month p.i. for patients with a ‘moderate’ lower ridge resorption of Atwood (1963) grade 3 or 4 (n=5). Patients with more severe lower ridge resorption (Atwood grade 5 or 6; n=4) showed a significantly lower MBF over the entire observation period (P=0·05) and took longer to regain bite strength. Only patients with moderate bone resorption exceeded their pre-insertion level of MBF within the observation period of 6–10 months p.i. In contrast to one report of immediate improvement of MBF at insertion of a new or relined denture (Leyka et al., 2000), the present study suggests that, at least for elderly patients with severe bone resorption, delayed improvement of MBF should be expected.

A study on evaluation methods of masticatory ability for complete denture wearers--correlation between evaluation using a questionnaire and masticatory performance

The purpose of this study was to investigate the relation between evaluation by a questionnaire on masticatory aspects of 20 foods, and masticatory performance obtained using the sieving method, previously used in many studies. After replacement of upper and lower complete dentures made newly, 28 edentulous patients voluntarily participated in this study. The basal seat area of each patient was scored and divided into two groups, and masticatory ability was evaluated by two methods: using a questionnaire and the sieving method. The following conclusions were drawn from the data: 1. Evaluation of masticatory ability by both methods used in this study were not associated with gender, age, and evaluation of the basal seat area. 2. Evaluation using a questionnaire and masticatory performance showed significant correlation, and Spearman's correlation coefficient was 0.628 (p < 0.001).

A randomized clinical trial comparing the efficacy of mandibular implant-supported overdentures and conventional dentures in diabetic patients. Part IV: Comparisons of dietary intake

Statement of problem. It is unclear whether the replacement of conventional mandibular dentures with implant-supported overdentures alters the diet and thus improves the nutritional intake of edentulous persons.Purpose. The purpose of this study was to compare the pretreatment and posttreatment diets of edentulous diabetic patients who received new dentures with either a conventional complete mandibular denture (CD) or a mandibular implant-supported overdenture (IOD).Material and methods. New dentures were made for 89 edentulous diabetic patients with acceptable metabolic control without insulin (NIT) or with insulin (IT). A randomized approach was used to assign 37 patients a mandibular CD and 52 patients a mandibular IOD supported by 2 cylindrical implants. Of the 89 patients, 58 submitted a dietary log for 7 consecutive days before treatment (PT) and 6 months after treatment completion (PTC). An average daily intake of 28 essential nutrients was determined for each patient at each time interval. Separate 2 × 2 × 2 repeated analysis of variance (ANOVA) tests were performed for each nutrient to compare the means of the 2 denture groups (CD and IOD), 2 diabetic groups (NIT and IT), and 2 time intervals (PT and PTC). The intakes were also compared with the recommended daily allowance (RDA).Results. ANOVAs for all 28 nutritional variables showed no main effect for either denture type or diabetic treatment. Time effects were seen for magnesium, potassium, copper, and monounsaturated fats. The PTC mean intake of the total sample (N = 58) decreased for all 3 minerals and increased for monounsaturated fats with study dentures. Post hoc tests showed the differences between PT and PTC means to be statistically significant for only magnesium (P=.043) and potassium (P=.015). The percentage of patients with PT intake 25% or more below the RDA ranged from 33% to 85% in the CD group and from 24% to 100% in the IOD group for the same 11 nutrients. PTC fiber intake deficiency was noted in almost all participants. Carbohydrate consumption was markedly lower than that recommended by the American Diabetic Association.Conclusion. As is often the case with elderly groups, this group of edentulous diabetic patients showed highly comprised nutritional intakes of fiber, vitamins, and minerals. The replacement of old dentures with new dentures that included either a mandibular CD or IOD did not alter patient diets such that the patients improved their nutritional intakes of essential micronutrients and macronutrients. Survey of prosthodontic impression procedures for complete dentures in general dental practice in the United Kingdom

Statement of problem. Anecdotal evidence suggests impression techniques used in general dental practice may vary from those taught at dental schools. There is little published information on this topic. Purpose. This study identifies the materials and methods used by general dental practitioners for recording impressions for the provision of replacement of complete dentures. Methods. A total of 905 questionnaires were sent to general dental practitioners in the Greater Manchester area to identify current clinical practices. Results. The results revealed that 88% of respondents use only irreversible hydrocolloid for primary impressions. If multiple responses are included, 99% of respondents mentioned irreversible hydrocolloids as an option for primary impressions. In response to the same question for secondary impressions, 94% of respondents mentioned irreversible hydrocolloids as an option. Other material mentioned as an option for secondary impressions included zinc oxide–eugenol (29%) and polyvinyl siloxane (13%). With regard to special trays, 75% of respondents routinely used laboratory constructed special trays to take definitive impressions. Questions on the requisite spacing, perforation, handle design of special trays, and on disinfection showed a diversity of opinion among practitioners Conclusions. Although this survey reflected a diverse range of clinical preferences, it is clear that irreversible hydrocolloid dominates the impression market for complete dentures. The use of special trays is normal practice for complete dentures in general dental practice in the United Kingdom. (J Prosthet Dent 1999;81:295-9.)

Treatment for low tolerance to dentures: Supplemental report

A program for the symptomatic and objective relief of sore mouth and low tolerance to dentures has been described. This program was successful in over 90 per cent of the patients observed. The following steps are followed: 1. Obtain a complete history, physical examination, and laboratory studies. 2. Initiate therapy with the combined liver fraction tablet. 3. After two to four weeks, give the patients who have achlorhydria the slow-release acid supplementation tablet with each meal, in addition to the liver fraction tablet. 4. Place emphasis on increased food intake as denture tolerance permits, with special emphasis upon protein needs. 5. Proceed with replacement of dentures as indicated. These patients are willing experimentors and the periodic omission of either or both of the supplements or the reduction of dietary foods may be expected. A return of symptoms will usually lead to continuation of the beneficial regimen.
