Endometriosis in an ectopic kidney: a rare case report and literature review

BackgroundEndometriosis mainly occurs in female pelvic organs. Endometriosis in the kidney is extremely rare.Case presentationWe herein describe a case of a 19-year-old girl with occasional mild abdominal pain associated with an ectopic left kidney. SPECT-CT showed no abnormal radioactive distribution in the left pelvis, suggesting loss of function of the ectopic kidney. Laparoscopic left ectopic kidney resection was subsequently performed. Histopathology revealed endometriosis of the ectopic left kidney.ConclusionsIn female patients with clinical manifestations of abdominal pain and gross hematuria, the possibility of renal endometriosis should be considered.

Renal Endometriosis Mimicking a Malignancy—a Rare Case of Reno-Mullerian Fusion?

Endometriosis is a common gynaecological condition characterised by ectopic endometrial tissue growth beyond the uterine cavity. Urinary tract endometriosis represents only 1.2% of all cases, with renal endometriosis accounting for less than 1% of urinary tract involvement. An asymptomatic, 49-year-old, perimenopausal Irish female was found to have an incidental mass at the upper pole of the right kidney on imaging studies. Histological analysis was recommended to outrule a renal malignancy and an open radical nephrectomy was performed following multidisciplinary input. Histological analysis surprisingly revealed the presence of endometriosis and endosalpingiosis, accompanied by a significant smooth muscle component, which mimicked a renal neoplasm. The aetiology of renal endometriosis remains unclear. However, given the lack of a history of endometriosis and the absence of other foci of disease in this patient, together with the smooth muscle predominant phenotype, this may represent a case of reno-mullerian fusion or endometrial displacement during gestational development.

Laparoscopic Resection of Renal Capsular Endometriosis in a Woman with Menstrual-Related Flank Pain: Case Report

BackgroundAlthough involvement of the urinary system is not uncommon, endometriosis in the kidneys is rare. To date, laparoscopic partial nephrectomy has been the preferred approach for managing renal endometriosis. Here, we report for the first time the results of laparoscopic removal of a renal capsular endometriosis in a malrotated kidney in an attempt to save the whole kidney parenchyma, in terms of feasibility and safety.Case presentationA 37-year-old female presented with periodic right flank pain associated with her menstrual cycle. On imaging, a malrotated right kidney and a hypodense irregular-shaped lesion measuring 30 * 20 * 15 mm were seen in the superior portion of the right perinephric space. Histologic evaluation of the ultrasound-guided biopsy was consistent with renal capsular endometriosis. The patient underwent laparoscopic surgery to remove the capsular mass while preserving the normal renal parenchyma. Pathological examination of the biopsy obtained during surgery was in favor of renal endometriosis. At 6-month follow-up, the patient’s pain had completely disappeared and no complications had occurred. In addition, imaging did not show any remarkable recurrence.ConclusionRenal endometriosis should be strongly considered as a differential diagnosis in female patients with a renal capsular mass and exacerbation of flank pain during menstruation. Based on our experience, with preoperative needle biopsy and clearing the pathology, laparoscopic removal of the mass in spite of renal anatomic abnormality is feasible and safe and thus could be considered as a possible treatment option.

Renal Endometriosis: An Unusual Presentation of a Common Disease

Renal endometriosis tends to be misdiagnosed as renal tumor: a rare case report.

Renal endometriosis is a rare disease for which the mechanisms of pathogenesis are still unclear. As such, early diagnosis and an appropriate treatment are often delayed because of the tendency to be misdiagnosed as a renal tumor. In October 2013 we performed a radical nephrectomy for a 37-year-old woman with renal endometriosis who was preoperatively misdiagnosed as having a right renal tumor. Avoiding the misdiagnosis of renal endometriosis requires a detailed case history, especially regarding whether the cyclicity of lumbodorsal pain and hematuria correlates with patients' menstrual cycles. Imaging examinations are commonly helpful for localization, whereas relieving symptoms with drugs to create a hypoestrogenic state is useful for clinical diagnosis. However, a final diagnosis for renal endometriosis still must depend on histopathologic examination.

Renal endometriosis mimicking an angiomyolipoma

A 42-year-old woman, with a history of dysmenorrhea associated with low-back soreness, presented with sudden onset of right flank pain, a palpable tender mass in the right upper quadrant of the abdomen, and hematuria. A contrast-enhanced computed tomography revealed a huge encapsulated right renal tumor with a minimal fat component, which was initially diagnosed as a renal angiomyolipoma (AML) on the basis of her history and imaging findings. She underwent a right nephrectomy after initially receiving conservative treatment. Results of a pathologic examination of the resected specimen, however, revealed renal endometriosis. After the operation, she received hormone therapy with danazol. During the 10 months after the operation, no untoward events developed. Herein, we report a rare case of renal endometriosis that initially mimicked an AML.

Renal endometriosis presenting with a giant subcapsular hematoma: case report

To describe a case of renal subcapsular hematoma due to renal endometriosis. Case report. Departments of Urology and Pathology, Baskent University Faculty of Medicine, Ankara, Turkey. A 46-year-old premenopausal woman was admitted with a left lumbar pain and mass. Ultrasonography and computerized tomography revealed a giant-sized renal subcapsular hematoma. Computerized tomography, percutaneous drainage catheter placement, surgical exploration, and excision of renal capsule. None. Histopathologic examination revealed endometriosis located beneath the fibrous renal capsule. Renal capsular endometriosis should be kept in mind among the causes of renal subcapsular hematoma.

Endométriose rénale. À propos d'un cas

The authors report a case of renal endometriosis. The preoperative diagnosis of this pathology is difficult because the clinical aspect and the radiological appearance are non specific. This fact limits the place of the medical treatment and of the conservative surgery nevertheless desirable in this young population.
