Evaluation of Awareness Toward Various Treatment Options and Maintenance Practices among Removable Partial and Complete Denture Patients in Western Maharashtra – A Questionnaire-Based Study

ABSTRACT Introduction: Researchers must concentrate their focus on whether patients understand various treatment options available among removable complete and partial denture patients and whether they are aware of the maintenance protocol of the prosthesis Materials and Methods: All the patients who completed removable partial or complete denture treatment in a period of 6 months were included in the study. Collected data were statistically analyzed using Chi-square test at a significance level of P < 0.05. Results: Awareness needs to be increased regarding the treatment options available for removable complete and partial denture treatment causes because many subjects in this study were only aware of the removable partial denture and complete denture treatment for missing teeth and not the other options, especially among individuals in the lower socioeconomic group Conclusion: The findings indicate that awareness needs to be increased regarding the functions of prostheses such as esthetics and phonetics because many subjects in this study were only aware of the masticatory function performed by prosthesis.

Confidence of clinical year dental students in performing removable denture treatment and their perception of the quality of prosthodontic studies

Objective: To find out the distribution of clinical year students’ confidence to perform prosthodontic treatments and perception towards the quality of prosthodontic studies they have received as a factor that can affect confidence. Material and Methods: Data was collected using online questionnaires and filled out by clinical year students who met the inclusion criteria. Further, the data was grouped according to the year of entrance to the clinical year program in the Faculty of Dentistry Universitas Indonesia. Data was then processed using SPSS version 22 for univariate analysis. Results: This study included 80 respondents. The highest level of confidence were seen when performing removable partial denture treatment (8.29 ± 0.76), try-in stage (wax form) procedure of a removable partial denture (9.00 ± 1.41), and primary impression taking for a complete denture procedure (9.00 ± 1.15). The lowest confidence was seen when performing complete denture treatment (7.08 ± 1,26) and surveying cast for a removable partial denture (6.00 ± 1.41). More than 95% of respondents showed a positive perception towards the quality of prosthodontic studies they received. Conclusion: Most respondents have a positive perception towards the quality of prosthodontic learning at Faculty of Dentistry Universitas Indonesia.

Impact of Removable Partial Dentures on Masticatory Performance in Partial Edentulism: A Systematic Review.

To evaluate the impact of removable partial dentures (RPDs) on masticatory performance (MP) of partially edentulous subjects. Electronic database and hand searches were conducted to explore clinical trials reporting the evidence of the effect of metal- and acrylic-based RPDs on mastication. The validated objective and subjective measures of assessment of mastication were included. The article selection, data extraction, and assessment of risk of bias were conducted by two reviewers independently. Out of 11,200 studies, 1 randomized controlled trial and 23 clinical trials met the inclusion criteria. Positive impact on different parameters of masticatory function after RPD treatment was reported in 14 studies. Subjective evaluation of chewing ability (CA) displayed an improvement in 83.3% of studies, while improvement was evidenced only in 69.5% of studies upon objective evaluation. Most studies reported data on free-end saddles. Of the 13 studies associated with bilateral free-end saddle dentures, 9 studies showed a positive effect on masticatory efficiency. Various factors were found to have an impact on the MP of RPDs. Subjective evaluation displays higher positive impact of RPDs in MP than objective evaluation. Assessment of patients' perspectives is important in clinical decision-making. Multiple factors affect the MP with RPD treatment.

Clinical evaluation of removable partial denture treatment in the Lubuk Minturun, Padang City

Dental and oral health problems are one of the health problems that require special attention in Indonesia. The most common dental health problem in Indonesia is cavities or dental caries. The incidence of caries in Indonesia is very high, reaching 88.8% of the total population of Indonesia. This high number is influenced by several factors, one of which is daily intake of high-sugar foods, bad habits of the community that can affect dental and oral health, and low public awareness of dental and oral health. Low public awareness of oral health has led to low public interest in caring for their teeth. This causes the teeth to no longer be treated, resulting in tooth loss. Tooth loss can affect a person's speech, mastication, and aesthetic functions. Based on the conditions experienced by the Indonesian people, rehabilitative measures are needed, namely in the form of denture treatment in cases of tooth loss. The purpose of this community service is to provide services to the people of Indonesia through the act of making free dentures which are expected to overcome complaints of tooth loss experienced by the surrounding community and an evaluation of the treatment of the programs that have been implemented. The community service method is to carry out rehabilitative activities by making removable partial dentures free of charge. The results of the community service by dentures treatments could improve dental and oral health of the subjects seen 100% satisfaction of post-treatment evaluations.

Risk Factor Assessment for Survival of Removable Partial Dentures and Their Abutment Teeth: A Retrospective Analysis.

To investigate the risk factors related to failure of removable partial dentures (RPDs) and to compare the survival of RPDs when abutment teeth have good vs reduced bone support. In this longitudinal patient record-based cohort study with a mean follow-up of 44.9 months, Cox regression models were used to evaluate the associations between clinical covariates and abutment tooth failure, as well as prosthesis failure. A total of 142 patients and 172 RPDs were included. Of the 172 prostheses, 64 (nearly 40%) failed after a mean time of about 4 years. Loss of abutment teeth was the main reason leading to RPD failure, followed closely by poor fit and adaptation. No factor was shown to be statistically significant at the prosthesis level. Women (Hazard Ratio [HR] = 0.542), endodontic treatment (HR = 3.460), presence of post and core (HR = 0.302), presence of a prosthetic crown (HR = 3.403), and abutment tooth type (in relation to incisor: canine HR = 0.196, premolar HR = 0.449) were the risk factors statistically significantly associated with the loss of abutment teeth. The pre-prosthesis amount of bone support of the teeth did not affect their prognosis as abutments for RPDs. RPD treatment modality presented a high failure rate after a mean follow-up of 4 years. Vital abutment teeth had a better survival rate than the ones treated endodontically (whether restored with or without a post and core); nevertheless, the amount of bone support did not affect their survival.

Association between gonial angle on orthopantomogram and reline frequency in removable partial denture wearers.

Removable dentures are a reasonable option for prosthetic treatment. However, continuous residual ridge resorption and reline procedures are inevitable owing to the strong pressure exerted on the mucosa or inappropriate pressure distribution. This study aimed to elucidate the association between the gonial angle on orthopantomogram (GAO) with occlusal force and reline frequency in removable partial denture wearers. Participants were patients who had previously received removable partial denture treatment for a free-end defect. Age, sex, number of remaining teeth, remaining opposing teeth, and occlusal support were investigated. GAO was measured using panoramic radiographs. The mean relining interval was calculated as the mean number of days between denture insertion and the first relining or subsequent relining. The association between the mean relining interval and each factor was investigated. Sixty-five subjects (33 females) were analyzed. The median value of the mean relining interval was 533.3 days, and the median GAO was 123°. There was a significant positive correlation between the mean relining interval and GAO (rs = 0.335). The number of remaining opposing teeth and occlusal support exhibited weak negative correlations with mean relining interval (rs = -0.187 and -0.214, respectively). Multivariate analysis using a generalized linear model showed that GAO was a significant explanatory variable for the mean relining interval. Within the limitations of this study, GAO was found GAO was found to contribute to the increased frequency of relining due to ridge resorption in patients with a small GAO.

A Prospective Comparative Study of Mastication Predominance and Masticatory Performance in Kennedy Class I Patients.

Mastication predominance in Kennedy class I (KC I) patients has not been well defined. This study aimed to investigate mastication predominance and masticatory performance in KC I patients, including the significance of remaining posterior teeth and removable partial-denture (RPD) treatment. KC I patients who had differences in the number of posterior teeth between left and right sides (D+) and KC I patients who had no differences (D−) were enrolled. Healthy dentate (HD) subjects were also registered as a positive control. Mastication predominance, defined by mastication predominance index (MPI; range 0–100%) calculated from electromyogram activities during voluntary chewing, and masticatory performance were evaluated at pre- and post-RPD treatment. Pre-MPI in KC I D+ was significantly higher than in HD. RPD treatment could significantly improve MPI and masticatory performance in both KC I groups. However, there were significant differences in masticatory performance between each KC I group and HD, regardless of RPD treatment. It was considered that the mastication predominance in KC I patients was affected by the difference in the number of remaining posterior teeth. RPD treatment could improve mastication predominance and masticatory performance in KC I patients, although the latter was not similar to HD group.

A new masticatory performance scale to integrate food biting, comminution and mixing ability in removable partial denture wearers.

The existing masticatory performance (MP) tests cannot comprehensively evaluate objective masticatory function. We aimed to examine the construct validity and responsiveness and determine the clinically minimum important difference (MID) and related factors of a newly developed MP scale to integrate food biting, comminution and mixing ability in patients with removable partial dentures (RPDs). Masticatory performance tests using gummy jelly, peanuts and colour-changeable gum were performed before and 3months after RPD treatment in 248 partially dentate patients. A 10-point ordinal subscale was created for each MP test based on the distributions of MP values, and the three subscales were integrated into a single scale (iMP scale). We analysed the correlations between MP values and iMP scores and determined the effect size for RPD treatments and the MID on the basis of patient perception of chewing ability. iMP scores exhibited moderate correlations with the three MP values (|ρ|=0.73-0.79, p<.001) and with the change scores (|ρ| =0.61-0.64, p<.001). The mean iMP score significantly increased after RPD treatment (p<.001), and the effect size was 0.35. Of the 248 patients, 55 (22%) reported a slight improvement in chewing ability with a mean iMP score change of 2.1 points. Sex and the number of missing teeth were significantly associated with iMP scores (p<.05). The iMP scale comprehensively evaluated objective masticatory function, including food biting, comminution and mixing ability in RPD wearers, and an increase of two points on the iMP scale was considered to be the MID in RPD treatments.

Satisfaction and Complaints of Patients with Removable Partial Dentures: In Sample at Baghdad City, Iraq

Objective: This study examined patients’ complain regarding pain on eating, esthetic and speech problemsand gag reflex with various types of removable partial dentures (RPDs).Method: The study sample included 60 RPD wearers (36 females and 24 males) aged between (25-60)years old. Sex, age, occupation, any experience with previous RPDs, and the complaint with RPDs wererecorded. Data were analyzed using Chi-square test was applied for comparison among different variablesof oral complaints related to removable prosthesis; an independent a sample t-test was utilized. A P-value &lt;0.01 was the criterion for rejection of the invalid hypothesis.Results: the results show 60 patients, 16 (47.05%) were satisfied on acrylic and14 (77.77%) were satisfiedon flexible. And 6 (75%) were satisfied on metallic. The mean level of satisfaction among the three typesof RPDs was significantly different (P &lt; 0.01). In cases satisfaction between flexible and acrylic RPDswas significantly different. But, between either metal or acrylic, or metal and flexible RPDs no significantdifferences (P &lt; 0.01) were recognized.Conclusions: Within the limitations of this study, approximately half of cases that tooka removable partialdentures treatment were satisfied with it, showing that the quality of RPDs(in Iraq/Baghdad city) wassuitable for patients. Pain in eatingis the most common complaint that shows that dental treatments withRPDs should be provided with care when patients have pain in eating.

Responsiveness of methods to evaluate objective masticatory function in removable partial denture treatments.

Purpose The aim of this study was to examine the responsiveness of chewing tests after removable partial denture (RPD) treatment in this prospective cohort study.Methods Pre- and post-RPD treatment, objective masticatory function was evaluated in 248 patients via chewing tests involving peanuts, a gummy jelly, and a color-changeable gum. A statistical comparison was performed between the pre- and post-treatment scores, and the standardized response mean (SRM) was determined as the treatment effect size (ES).Results After RPD treatment, the median particle size in the sieving method using peanuts significantly decreased, and the glucose concentration in the gummy jelly method significantly increased (P < 0.001). The ESs of the sieving and gummy jelly methods were larger (|SRM| = 0.3-0.4) than that of the gum method (|SRM| = 0.1). In the subgroup analysis, the sieving and gummy jelly methods resulted in a broader effective range (|SRM| ≥ 0.3) than the gum method. In patients without pre-treatment dentures, the ES was medium in the sieving method (|SRM| = 0.6) and weak in the gummy jelly and gum methods (|SRM| = 0.3-0.4). The pre-treatment level of masticatory function was significantly correlated with ES (ρ = 0.93, P < 0.001).Conclusions These results suggest that the sieving and gummy jelly methods are more responsive than the gum method for RPD treatment and that the pre-treatment scores affect the responsiveness.

Responsiveness of methods to evaluate chewing ability after removable partial denture treatments.

The ability of an instrument to detect a clinically important change in patient-reported outcomes following prosthetic treatment is critical for its use in clinical practice and research settings. This study aimed to examine the responsiveness of instruments in evaluating patient perception of chewing ability after removable partial denture (RPD) treatment in a prospective cohort. A single 100-mm visual analogue scale (VAS), 20-item food intake questionnaire (FIQ) and 6-point chewing difficulty scale (CDS) were administered to 248 partially edentulous patients to evaluate chewing ability pre- and post-RPD treatment. We conducted a statistical comparison between the pre- and post-treatment scores and determined the effect size (r) of RPD treatment for the instruments. A significant improvement in chewing ability after RPD treatment was identified by VAS and CDS (P<.05), but not by FIQ (P=.16), which identified an improvement after controlling for sex, age, the number of missing teeth and presence/absence of existing removable dentures. The effect size was medium for VAS (r=.54), weak for CDS (r=.14-.17) and absent for FIQ (r=-.09). The increase in r was significantly associated with impaired chewing ability before treatment (ρ=-0.87, P<.001). These results suggest that the 100-mm VAS, followed by CDS, is the most responsive instrument to detect an improvement in chewing ability after RPD treatment. Pre-treatment impairment of chewing ability was associated with better responsiveness of the instruments.

Pre-treatment impairment of oral health-related quality of life is associated with variations in minimal clinically important differences among patients with removable partial denture treatment.

The minimal clinically important differences (MID) of the Oral Health-related Quality of Life (OHRQoL) are the benchmark to meaningfully interpret treatment outcomes. However, there is no available information regarding the factors impact the MID variations after removable partial denture (RPD) treatment. The purpose of this study was to determine the influence of the pre-treatment OHRQoL impairment on the MID in the oral health impact profile (OHIP) after RPD treatment. In this prospective study, OHIP and Self-reported Oral Health (SROH) questionnaires were administered to patients before and 3months after RPD treatment. The MID in each OHIP version (OHIP-49, OHIP-20, OHIP-14 and OHIP-5) was the mean score of patients who showed a minimum SROH improvement. Multiple linear regression analysis was conducted to determine the influence of the pre-treatment OHIP summary score on the MID score. Among 248 participants who completed both evaluations, 85 (34%) patients had minimum SROH improvements after RPD treatment. Multiple regression analysis indicated that each 10-point increase in the pre-treatment summary OHIP score was associated with an increase in the MID score of three points for OHIP-49 and four points for OHIP-20, OHIP-14 and OHIP-5 (P<.05), after controlling for age, sex and the number of missing teeth. Therefore, the MID increased as the pre-treatment OHRQoL was impaired. The MID was accounted for by approximately one-third of the pre-treatment summary score, independent of the OHIP version, in the interpretation of the effects of RPD treatment on patients' OHRQoL.

Influence of the Covering Area of Major Connectors of Mandibular Dentures on the Accuracy of Speech Production: A Pilot Study

Background/Objectives: Speech impairment during the initial phase of removable partial denture (RPD) treatment can prevent patient adaptation to RPDs. This quasi-experimental study aimed to investigate the influence of the covering area of major connectors located in the mandibular anterior region on the accuracy of speech production. Methods: Fifteen adults (12 men and 3 women aged 26–40 years) with healthy dentition were recruited. Four target syllables were used for the assessments: [ʃi], [ʧi], [çi], and [ki]. There were four speaking conditions: without a connector (control), wearing lingual bar (5 mm width), wearing lingual plates (12 mm width), and wearing connectors covering the occlusal surface of lower dentition (16 mm width; CO). The accuracy of speech production was evaluated by mean appearance ratio of correct labels (MARC) and incorrect labels (MARIC) of the phonetic segments which were calculated with a speech evaluation system (VoiceAnalyzer). Results: The MARC in the 3 target syllables [ʃi], [ʧi], and [ki] significantly decreased with CO. For the target syllable [ʃi], the MARICs of [ʧi], [ʤi], and [çi] with CO were significantly lower than that in the control. For the target syllable [ʧi], the MARIC of [ki] with CO was significantly higher than that in the control. For the target syllable [çi], the MARIC of [gi] with CO was significantly higher than that in the control. Conclusion: RPDs with major connectors covering the lower dentition could disturb speech production during the initial phase of RPD treatment. Prosthodontic treatment with such RPD designs requires further consideration regarding the information given to patients before starting the treatment.

Minimal clinically important differences of oral health-related quality of life after removable partial denture treatments

ObjectivesThis study aimed to determine the minimal clinically important difference (MID) of the oral health-related quality of life (OHRQoL) in removable partial dentures (RPDs) wearers, using the oral health impact profile (OHIP). MethodsIn this study, RPD patients were asked to complete the OHIP and self-reported oral health (SROH) questionnaires before the treatment and 3 months after the RPD treatment. The Wilcoxon signed ranked test compared the pre- and post-treatment status and the Spearman correlation analysis evaluated the relationship between the pre- and post- treatment change scores of the OHIP and SROH. The median OHIP change score in a subgroup that showed minimum SROH improvement was determined as the MID. ResultsOf the 277 participants, 248 (89.5 %) completed the pre-and post- treatment evaluations. SROH and OHIP scores showed significant improvements after RPD treatment (P < 0.05). The OHIP change scores showed significant moderate correlations with the SROH change scores (P < 0.05). The MIDs for the summary scores of OHIP-49, 20, 14, and 5 versions were 14, 7, 3, and 2 points, respectively. An additional 2–3 points were assigned to the four dimensions (oral function, orofacial pain, orofacial appearance, and psychological impact). ConclusionsAn OHIP change of more than 14 points (OHIP-49), 7 points (OHIP-20), 3 points (OHIP-14), 2 points (OHIP-5) and 2–3 points (4 dimensions) was considered clinically meaningful in RPD treatments. Clinical SignificanceThe proposed MID scores could be used as a benchmark for clinical decision making, interpretation of RPD treatment effects on OHRQoL, and sample size calculations in clinical trials.

Frequency of complete and removable partial denture treatment in the primary health centres in three different regions of Kosovo from 2002 to 2013

IntroductionEdentulism and prosthodontic care are very common, especially in the elderly. The study investigated the treatment with complete dentures (CDs) and acrylic removable partial dentures (ARPDs) among people receiving new prosthodontic treatment in the Primary Health Centres of the three regions in Kosovo from 2002 to 2013.MethodsThe data on ARPDs delivery and CDs delivery was obtained from the archives of primary health centres from three Kosovo regions (Prizren, Peje, Ferizaj) from 2002 to 2013. The data was analysed concerning year of treatment, type of dentures, jaw, age, gender and urban or rural origin of the patients. The trend of treatment was determined, and the binomial logistic regression model was used for predicting odds of ARPD versus CD treatment by year of treatment and patient characteristics.ResultsFrom 2002 to 2013, 9,478 patients received 11,655 CDs and 4,401 ARPDs. Delivery of CDs significantly increased by 57.45 dentures per year (R2=0.609) and delivery of ARPDs by 30.39 dentures per year (R2=0.569). Each year the odds for ARPD versus CD increased by 4.2% (95% CI: 3.0%–5.4%). Younger patients have higher odds for ARPD rather than CD and odds for ARPD are decreasing as the age of patients rises. The gender, residence, and jaw all had a significant impact on prosthodontic treatment too.ConclusionsIn Primary Health Centres of Kosovo, there is a trend for higher frequencies of both dentures (more obvious for ARPD), and the frequency is highly dependent on the age of patients.
