Blood-conserving and therapeutic efficacy of intravenous tranexamic acid at different time points after primary total knee arthroplasty with tourniquet application: a randomised controlled trial

BackgroundThe use of a tourniquet in combination with tranexamic acid (TXA) not only ensures clear vision, reduces intraoperative blood loss and shortens operative time but also improves cement-bone inter-digitation in total knee arthroplasty (TKA). However, there is no proof whether the blood flow blocking effect of tourniquet affects the antifibrinolytic effect of TXA, and the optimal timing of TXA administration is still unclear. Therefore, this study aims to investigate the effect of the first dose of TXA administered intravenously before tourniquet compression and release in TKA on perioperative blood loss and therapeutic efficacy in patients.MethodsIn this double-blind trial, 90 patients undergoing primary TKA were randomised into 2 groups: Group A, patients received intravenous TXA 10 min before tourniquet compression (20 mg/kg) and 3, 6 and 24 h later (10 mg/kg), and Group B, patients were treated the same as those in Group A but received intravenous TXA before tourniquet release. The primary outcomes were changes in blood loss, haemoglobin and haematocrit. Secondary outcomes included operation and tourniquet times, blood transfusion rate, subcutaneous petechiae and circumferential changes in the operated limb, visual analogue scale (VAS) score, hospital for special surgery (HSS) score, length of stay (LOS) postoperatively, complications and patient satisfaction.ResultsNo statistically significant difference was found between the 2 groups with regard to age, sex, weight, body mass index (BMI), Kellgren-Lawrence class, preoperative blood volume, preoperative laboratory values, operation and tourniquet times, transfusion rate, knee circumference, preoperative HSS, or VAS score (P:n.s.). There was no significant difference in intraoperative blood loss (IBL) (52.7 ml vs. 63.4 ml, P = 0.07), hidden blood loss (HBL) (91.4 ml vs. 119.9, P = 0.4) or total blood loss (TBL) (144.1 ml vs. 183.3 ml, P = 0.72) between Groups A and B. Haemoglobin, haematocrit and red blood cell count (RBC) dropped to a low point on postoperative day 3 and then rebounded, returning to normal levels on day 21, and the trend of change between the 2 groups was not statistically significant (P:n.s.). There was no significant difference in subcutaneous ecchymosis incidence, knee swelling rate, HSS score, VAS score, LOS postoperatively, complication rate or patient satisfaction (P:n.s.).ConclusionTXA was administered intravenously prior to tourniquet compression could effectively reduce blood loss in patients who had undergone total knee arthroplasty. However, there was no significant difference in knee swelling rate, subcutaneous bruising and petechiae incidence, knee function, complication rate or satisfaction between patients who TXA was given intravenously before tourniquet compression and release in primary TKA.

Extra-articular biceps tendon supplement release in total knee arthroplasty for valgus deformity combined with severe flexion contracture.

To investigate the efficacy of extra-articular biceps tendon supplement (EBS) release in the treatment of valgus knee with severe flexion contracture. We retrospectively studied 27 patients who received EBS release for valgus knee combined with severe flexion contracture from 2018 to 2021. The full-length radiograph of the lower limbs in a standing position was taken before and after the operation, and the tibiofemoral angles before and after the operation were measured on the full-length radiograph of the lower limbs. Knee joint motion measurement and KSS scoring were performed before and after the operation (3 months, 6 months, and 1 year), and the obtained data were compared before and after the operation to evaluate clinical results. Compared with preoperation, postoperative HKA (p < 0.01), ROM at 3 months (p < 0.01), 6 months (p < 0.01), and 1 year (p < 0.01) were significantly improved, and KSS scores at 3 months (p < 0.01), 6 months (p < 0.01), and 1 year (p < 0.01) were significantly improved. Our EBS release has good clinical results for the treatment of valgus knee with severe flexion contracture.

Less soft tissue release in total knee arthroplasty for anteromedial compared to posteromedial knee osteoarthritis

In total knee arthroplasty (TKA), the aim of achieving a mechanically straight leg axis as well as symmetrical and equally wide gaps has become established as the gold standard in terms of surgical technique. In contrast to TKA unicompartmental knee arthroplasty (UKA) is performed in anteromedial osteoarthritis (AMOA) and does not normally require releases. This raises the hypothesis whether the type of osteoarthritis (AMOA vs. posteromedial osteoarthritis (PMOA)) determines the requirement for soft tissue releases in TKA.In this retrospective study, 114 patients with medial osteoarthritis of the knee who had been treated with a navigated total knee replacement were consecutively included. On the basis of the preoperative lateral radiographs, the patients were divided into two groups: AMOA and PMOA. The incidence and the extent of releases performed were recorded using the navigation records.Patient-specific data (gender, age) did not differ between the groups (NS). Knees with AMOA presented an overall varus alignment of 5.3 ± 3.5°, knees with PMOA 8.0 ± 4.0° (p < 0.001). 30 cases (44%) had to be released in the AMOA group, compared with 33 cases (72%) in the PMOA group (p = 0.004). In the case of medial release, the extension gap increased 3.3 ± 2.4 mm in the AMOA compared to 5.3 ± 3.7 mm in the PMOA group (p = 0.006). The medial flexion gap was released 2.2 ± 2.6 mm in the AMOA and 2.9 ± 3.0 mm in the PMOA group (p = 0.008).To achieve a neutral mechanical alignment, a release has to be performed due to asymmetry of the extension gap more often if PMOA is present than in AMOA. Surgeons should be prepared to perform more frequent and extensive medial releases in PMOA. Higher constrained implants should be available in case of unintended over release in PMOA.

Medial stabilizing technique preserves anatomical joint line and increases range of motion compared with the gap-balancing technique in navigated total knee arthroplasty

BackgroundMedial compartment stability is important in total knee arthroplasty. The medial stabilizing technique (MST) has been proposed to achieve medial stability without excessive medial soft tissue release in total knee arthroplasty. Herein, we compare the MST and the gap-balancing technique (GBT) in navigated total knee arthroplasty. MethodsWe retrospectively analyzed 70 patients with varus knee osteoarthritis who underwent primary total knee arthroplasty using the navigation system. They were divided into MST (n = 39) and GBT (n = 31) groups. We assessed intraoperative navigation data, radiographic data, and insert thickness. Preoperative and postoperative joint line changes were measured. We also assessed range of motion and clinical instability before and after total knee arthroplasty. These parameters were statistically compared between the groups. ResultsCompared with the GBT group, medial extension gaps were significantly smaller in the MST group (P = 0.008). The gap difference between medial and lateral extension was significantly greater in the MST group (P = 0.018). Other navigation data showed no significant differences. Insert thickness and joint line changes were significantly lower in the MST group (P = 0.001, P = 0.018, respectively). Postoperative range of motion was significantly greater in the MST group (P = 0.032). There was no objective or subjective knee instability in either group. ConclusionThe MST could avoid knee joint line changes and might increase postoperative range of motion. Although the MST permitted a discrepancy between medial and lateral gaps, no patients felt knee instability. The MST might improve the results of total knee arthroplasty.

A new improvement: subperiosteal cocktail application to effectively reduce pain and blood loss after total knee arthroplasty

BackgroundPain and blood loss after total knee arthroplasty (TKA) are unsolved clinical problems. Some studies reported that periarticular cocktail injection can effectively reduce pain and blood loss. However, there was no gold standard about the cocktail ingredient and injection location. More osteotomy and less soft tissue release in TKA with mild deformity; besides, plenty of nerves and blood vessels are contained in the periosteums and bone marrow. In this study, we aimed to detect the clinical results of subperiosteal cocktail application in TKA.MethodsTwo groups were included according to the different injection location in our study. In group 1, cocktails were injected into the muscles, tendons, suprapatellar bursa, and subpatellar bursa surrounding knee joint. In group 2, cocktail injection was performed under the periosteum of the distal femur and proximal tibia. Our primary outcomes were visual analogue scale (VAS) and hemoglobin (Hb), and the secondary outcomes were wound healing, infection, deep vein thrombosis (DVT), operation time, and hospitalization.ResultsAt the first operative day, the mean (standard deviation) VAS score in a state of static was lower in group 2 compared with group 1 (0.98 ± 0.27 in group 1 and 0.86 ± 0.60 in group 2, p < 0.05). In the state of flexion and extension, the mean (standard deviation) VAS was 1.61 ± 0.66 in group 1 and 1.10 ± 0.57 in group 2 (p < 0.05). The mean (standard deviation) blood loss was higher in group 1 than in group 2 at the first postoperative day (440.19 (167.68) ml in group 1 and 333.67 (205.99) ml in group 2, p < 0.05). At the third day after surgery, the mean (standard deviation) blood loss was 686.44 (140.29) ml in group 1 and 609.19 (260.30) ml in group 2, and there was significant difference between these two groups (p < 0.05).ConclusionsWe concluded that subperiosteal cocktail injection can significantly reduce pain and blood loss compared with periarticular cocktail injection after TKA.

Preoperative Radiological Parameters Predicting the Need for Lateral Retinacular Release in Total Knee Arthroplasty

BackgroundProper patellar tracking is essential for well-functioning total knee arthroplasty (TKA). Besides implanting components in the correct position and rotation, balancing parapatellar soft tissues is also important in aiding normal patellar tracking. Patellar maltracking during TKA can be improved by lateral retinacular release (LRR). MethodsWe studied the incidence of LRR in consecutive primary TKA with nonresurfaced patella and posterior-stabilized implant design. We analyzed data from 250 consecutive primary TKAs (212 patients) from January 2016 to May 2016. We evaluated the preoperative radiological parameters like patellar tilt, patellar shift, patellar morphology, Insall-Salvati ratio, femoro-tibial angle, distal femoral valgus angle, and proximal tibia varus angle which predict the need for LRR during TKA. We used multivariate regression analysis to find the association of individual radiological parameters and the LRR. ResultsThe need for LRR is significantly associated with preoperative radiological parameters like patellar shift and patellar tilt (P < .001). Compared to the nonreleased group, the adjusted odds of LRR were greater for morphological parameters like Wiberg type 3 patella (odds ratio [OR] 17.45, 95% confidence interval [CI] 7.21-42.20), lateral facet thinning (OR 4.38, 95% CI 2.37-8.07), lateral patellofemoral arthritis (OR 14.36, 95% CI 6.82-30.23), and coronal valgus deformity (OR 4.95, 95% CI 1.60-10.68). ConclusionPreoperative assessment of these radiological parameters in the axial view implies a high chance of tight lateral retinacular structures. This helps in identifying patients who have a higher likelihood for patellar maltracking during TKA. Appropriate LRR helps to provide better patellar tracking post TKA.

Weakening of the knee ligament complex due to sequential medial release in total knee arthroplasty.

The purpose of this study was to investigate the effects of sequential medial release on the stiffness and collateral stability of the ligament complex of the knee. Irrespective of the implantation technique used, varus deformity frequently requires release of the capsular ligament complex. Yet, no data are available on how stiffness and stability of the knee ligament complex are weakened by such release. After total knee arthroplasty, ten healthy Thiel-fixed knee joints were subjected to sequential medial release consisting of six release steps. After each step, stiffness and stability were determined at 0°, 30°, 60°, and 90°. Sequential medial release increasingly weakened the ligament complex. In extension, release of the anteromedial tibial sleeve 4cm below the joint line already weakened the ligament complex by approximately 13%. Release 6cm below the joint line reduced stiffness and stability by 15-20% over the entire range of motion. After detachment of the medial collateral ligament, stability was only about 60% of its initial value. Our study showed for the first time the association between medial release and stiffness and stability of the knee ligament complex. To maintain stability, vigorous detachment of the knee ligament complex should be avoided. Release of the anteromedial tibial sleeve already initiates loss of stability. The main stabiliser is the medial ligament, which should never be completely detached. IV.

Preoperative Varus-Valgus Stress Angle Difference Is Valuable for Predicting the Extent of Medial Release in Varus Deformity during Total Knee Arthroplasty.

PurposeThe purpose of this study was to compare the predictive value of the valgus stress angle (SA), varus SA and varus-valgus SA difference (VVD) in predicting the degree of medial release in varus deformity during total knee arthroplasty (TKA).Materials and MethodsOne hundred eight TKAs (78 patients), which were performed in primary osteoarthritis with varus knee deformity, were retrospectively classified into three groups according to the degree of medial release (group A, mild release; group B, moderate release; and group C, severe release). Medical charts were reviewed, and long weight bearing standing anteroposterior radiographs and varus-valgus stress radiographs were evaluated.ResultsThe valgus SA was significantly different between group A and B and between group A and C. However, it was not significantly different between group B and C. The varus SA was significantly different between group B and C and between group A and C. However, it was not significantly different between group A and B. The VVD was significantly different in all intergroup comparisons.ConclusionsEvaluation of the VVD is more valuable in predicting the degree of medial release in TKA performed in varus knee deformity than evaluation of the simple valgus and varus SA.

Introduction This article is devoted to the problem of total knee arthroplasty in patients with severe valgus deformity (Krackow type III). Methods We analyzed mid-term results of 12 total knee arthroplasties combined with sliding osteotomy of the lateral femoral condyle and distalization of the lateral collateral ligament. Treatment results were measured with KSS and WOMAC scores. Results We had excellent and good results in 11 cases but one was complicated with periprosthetic fracture of the femur. Discussion Up-to-date, there are various techniques of soft-tissue release in total knee arthroplasty proposed. These techniques differ in the point of release and the power of effect. However, an adequate balance is not always achievable in severe valgus deformity. Therefore, implants providing enhanced frontal stabilization are required (CCK/VVC).

Timing of tourniquet release in total knee arthroplasty: A meta-analysis.

Background:For total knee arthroplasty (TKA), the tourniquet is routinely employed for better visualization, less blood loss, and easier cementation. However, the time to release tourniquet remains controversial. Therefore, we performed current meta-analysis to assess whether releasing tourniquet before wound closure is more effective in reducing blood loss than releasing tourniquet after wound closure in TKA without an increased risk of complications.Methods:To conduct this meta-analysis, we searched Medline, Embase, Web of science, and the Cochrane library up to November 2016, for randomized controlled trials comparing tourniquet releasing before and after wound closure in TKA. A meta-analysis was performed following the guidelines of the Cochrane Reviewer's Handbook and the PRISMA statement. Methodological quality of the trials was assessed using the Cochrane risk assessment scale. The data of the included studies were analyzed using Stata 12.0.Results:Sixteen trials involving 1010 patients were identified in current meta-analysis. Our meta-analysis demonstrated that there were no significant differences in the 2 groups in terms of calculated blood loss (weighted mean difference [WMD] = 160.65, 95% confidence interval [CI]: −0.2 to 321.49, P = .05), postoperative blood loss (WMD = −45.41, 95% CI: −120.11 to 29.29, P = .233),postoperative hemoglobin decline (WMD = 0.16, 95% CI: −2.5 to 2.82, P = .905), transfusion volume (WMD = 79.19, 95% CI: −5.05 to 163.44, P = .065),transfusion rates (relative risk [RR] = 1.19, 95% CI: 0.95–1.50, P = .134), major complications (RR = 0.51, 95% CI: 0.15–1.73, P = .278), and deep vein thrombosis (RR = 0.44, 95% CI: 0.14–1.37, P = .157).Compared with the group of releasing tourniquet after wound closure, the group of releasing tourniquet before wound closure had a higher volume of total blood loss (WMD = 130.96, 95% CI: 58.83–203.09, P = .000) and a longer operation time (WMD = 6.56, 95% CI: 3.12–10.01, P = .000). However, releasing tourniquet before wound closure could reduce minor complications (RR = 0.53, 95% CI: 0.34–0.82, P = .004).Conclusions:On the basis of current meta-analysis, the method of releasing tourniquet before wound closure could increase total blood loss and operation time; nevertheless, the risk of complications decreased. Thus, if patients are in severe anemia condition, the tourniquet perhaps should be released after wound closure to decrease blood loss. In contrary, releasing tourniquet before wound closure to decrease the risk of complications would be a better choice.

What Preoperative Radiographic Parameters Are Associated With Increased Medial Release in Total Knee Arthroplasty?

BackgroundPreoperative varus deformity of the knee is a common malalignment in patients undergoing primary total knee arthroplasty (TKA). We are unaware of any studies that have correlated how various preoperative radiographic parameters can predict the amount of medial releases performed to achieve optimal coronal alignment and ligamentous balance. MethodsA retrospective review was performed on 67 patients who required at least a medial tibial reduction osteotomy (MTRO) during primary TKA to achieve coronal balance. This patient population was matched 1:1 to another cohort of TKA patients by age, gender, and body mass index who did not require an MTRO. A radiographic evaluation was used to compare the 2 cohorts. ResultsPreoperatively, the MTRO cohort was noted to have significantly increased varus tibiofemoral (86.12° vs 93.43°), tibial articular surface (85.79° vs 87.54°), and medial tibial articular surface angles (75.22° vs 85.34°) compared to the control cohort. The MTRO cohort had 3.13 mm of medial tibial offset and 9.06 mm of lateral joint space opening and the control cohort had 0.09 mm and 4.07 mm, respectively. The medial tibial articular surface angle and lateral joint space widening were statistically associated with the MTRO cohort. The final tibiofemoral angle in the MTRO cohort was 92.43° and was 93.40° in the control cohort. ConclusionThe MTRO cohort was noted to have several preoperative radiographic parameters that were significantly different than the control cohort. However, the medial tibial articular surface angle and lateral joint space widening were the only radiographic parameters that were statistically associated with requiring an MTRO.

Reduction Osteotomy vs Pie-Crust Technique as Possible Alternatives for Medial Release in Total Knee Arthroplasty and Compared in a Prospective Randomized Controlled Trial

BackgroundTo compare the gap change between the pie-crust technique and reduction osteotomy to determine their effects on flexion and extension gaps and their success rates in achieving ligament balancing during total knee arthroplasty. MethodsIn a prospective randomized controlled trial, 106 total knee arthroplasties were allocated to each group with 53 cases. If there was a narrow medial gap with an imbalance of ≥3 mm after the initial limited medial release, either reduction osteotomy or pie-crust technique was performed. The changes of extension and flexion medial gaps along with the success rate of mediolateral balancing were compared. ResultsThere was a significant difference in the change of medial gap in knee extension with mean changes of 3.5 ± 0.5 mm and 2.3 ± 0.8 mm in the reduction osteotomy and pie-crust groups, respectively (P < .001). For flexion gap, greater change was found in the pie-crust group compared with the reduction osteotomy group; the mean medial gap changes in knee flexion were 1.1 ± 0.5 mm and 2.3 ± 1.2 mm in the reduction osteotomy and pie-crust groups, respectively. The success rates were 90.6% and 67.9% in reduction osteotomy and pie-crust groups, respectively (P = .007). ConclusionAs an alternative medial release method, reduction osteotomy was more effective in extension gap balancing, and pie-crust technique was more effective in flexion gap balancing. The overall success rate of mediolateral ligament balancing was higher in the reduction osteotomy group than in the pie-crust group.

Varus-valgus stress radiograph as a predictor for extensive medial release in total knee arthroplasty.

The purpose of this study was to identify the risk factors for predicting the reduction osteotomy as extensive medial release during total knee arthroplasty (TKA) using multivariate logistic regression. A total of 404 TKAs were enrolled and sorted into two groups according to the extent of medial release and then analysed for the statistical significance of various risk factors including age, gender, body mass index (BMI), pre-operative knee mechanical axis angle (KMAA), mechanical varus stress angle (MVrSA), mechanical valgus stress angle (MVgSA), and sum of the mechanical varus and valgus stress angles (SMVVA) with use of multivariate logistic regression analysis. SMVVA to a more varus direction was found to be a significant risk factor for the reduction osteotomy (p < 0.0001, adjusted odds ratio (OR) = 2.705 with 95% CI 2.126-3.443). KMAA and MVgSA to a more varus direction were also significant risk factors (p = 0.010, adjusted OR = 1.189 with 95% CI 1.041-1.357, and p = 0.005, adjusted OR = 1.401 with 95% CI 1.109-1.767). The other variables were not significant risk factors. The overall results suggest that careful attention should be given to the need for extensive medial release and failure of the conventional soft tissue release technique during TKA in patients with a greater varus angle in the pre-operative SMVVA, MVgSA, and KMAA, especially with a greater varus SMVVA, which was the strongest predictor of reduction osteotomy. Timing of Tourniquet Release in Total Knee Arthroplasty.

EDUCATIONAL OBJECTIVES As a result of reading this article, physicians should be able to: 1. Describe the available tourniquet application strategies for patients treated with total knee arthroplasty (TKA). 2. Differentiate the potential strengths and limitations of tourniquet release before and after wound closure in TKA. 3. Cite the available evidence regarding tourniquet release before and after wound closure in TKA. 4. Recognize the optimal tourniquet application strategy in TKA based on the available evidence. The authors conducted a meta-analysis to determine the optimal tourniquet application strategy in total knee arthroplasty (TKA). Fourteen high-quality randomized, controlled trials were retrieved. The results showed tourniquet release before wound closure for hemostasis could significantly increase not only the total measured blood loss but also the calculated blood loss in patients treated with TKA, regardless of fixation type. With proper control of the amount of pressure (less than 293 mm Hg) and the duration of application (less than 150 minutes), tourniquet release after wound closure can reduce blood loss in patients treated with primary TKA without increasing the risk of complications. Partial Lateral Patellar Facetectomy as an Alternative to Lateral Release in Total Knee Arthroplasty (TKA)

This study presents the selective use of partial patellar lateral facetectomy for maltracking during primary TKA, as an alternative to lateral release. Twenty three partial facetectomies were performed out of 191 TKAs (12%). Balanced tracking was achieved in 22 knees. At follow-up 2 patients had persistent anterior knee pain. Mean Knee Society score (KSS) was 94 and mean functional KSS was 86. Mean patellar score was 28. Patellar tilt angles were within 2 degrees in all but one knee. Patellar translation was within 2 mm in all cases. No complications were recorded. A control group of 46 matched patients had similar functional and radiographic results. If tracking is not satisfactory at the conclusion of TKA, our method of choice would be partial lateral facetectomy.

A new patient-specific planning method based on joint contact force balance with soft tissue release in total knee arthroplasty

Although the conventional total knee arthoplasty (TKA) planning used 2D or 3D images of the patient’s lower extremity to determine bone alignment, it was shown that good bone alignment does not always guarantee successful results due to unbalanced joint contact forces and soft tissue tension. Thus, the joint contact force balance and soft tissue tension were pre-operatively calculated using finite element analysis (FEA). Although the results of FEA are accurate, the rigid body spring method (RBSM) has introduced for the joint contact force or pressure distribution due to the high costs for FEA. In this paper, an innovative method was described to quantify the pre- and post-operative knee contact force balance using various engineering techniques, including the free-form deformation (FFD) technique, and RBSM-based cartilage contact force and ligament tension estimation algorithm. Nine cases of TKA planning were performed with three different surgical options, including over-correction of the valgus ratio of the femur, tibial plateau cutting length, and MCL release to redistribute the medial and lateral contact force balance. This method is useful as a patient-specific preoperative planning tool in TKA surgery.

Less anterior knee pain with a routine lateral release in total knee arthroplasty without patellar resurfacing: a prospective, randomized study

Anterior knee pain is a major cause of complaint in total knee arthroplasty (TKA) without patellar resurfacing. The concept of improved patellar tracking and decreased retropatellar contact pressure for lateral retinacular release theoretically suggests that patients with lateral retinacular release in TKA would achieve a lower incidence of anterior knee pain when compared without lateral retinacular release. We sought to determine (1) whether those patients who received a routine lateral retinacular release in TKA would attain lower incidence of anterior knee pain as compared to patients who received TKA without lateral retinacular release and (2) whether lateral retinacular release would increase the lateral retinacular release-related complications. A total of 148 patients who underwent TKA with the use of the Gemini MK II mobile bearing were randomized to receive either routine lateral retinacular release (intervention group) or not (control group). Patients were assessed by the visual analogue scale for anterior knee pain, the Knee Society clinical scoring system of knee score and function score, and patellar score for clinical function. Patients' satisfaction and lateral retinacular release-related complications were also evaluated. The overall incidence of anterior knee pain in the intervention group at 18 months follow-up was 5.6%, while that of the control group was 20.6% (p = 0.009). No statistical difference was detected between the two groups in terms of lateral retinacular release-related complications (n.s.), patients' satisfaction (n.s.), knee score (n.s.), function score (n.s.), and patellar score (n.s.) at 18 months follow-up. The present study suggests that routine lateral retinacular release can reduce anterior knee pain and does not increase lateral retinacular release-related complications, in TKA with the use of the Gemini MK II mobile bearing without patellar resurfacing. Relationship between Tibial Bone Defect and Extent of Medial Release in Total Knee Arthroplasty

PurposeTo understand the relationship between tibial bone defect and extent of medial release in total knee arthroplasty.Materials and MethodsWe evaluated 32 knees in 22 patients with variable degrees of tibial bone defect on medial plateau has undergone total knee arthroplasty. In this study, 31 cases had been diagnosed as degenerative osteoarthritis and 1 case was osteonecrosis. We excluded cases with infection, traumatic arthritis, or neuropathic joints. With regard to gender, 29 cases were female and 3 cases were male. The following relationships were analyzed: preoperative degrees of tibial bone defect and varus deformity; the femorotibial angle of both weight-bearing whole extremity radiogram, distractive stress radiogram, and the extent of medial release.ResultsAverage tibial bone defect was 9.8±4.1 mm. Average femorotibial angle on weight-bearing whole extremity radiograph was varus 10.0°±6.2°. Average femorotibial angle on distractive stress radiograph was varus 0.7°±4.6°. Statistically the extent of medial release showed no significant relationship with the degree of tibial bone defect and femoro-tibial angle of whole extremity radiogram. However, it revealed a statistically significant relationship with the femorotibial angle on distractive radiogram (r=0.465, p=0.007).ConclusionsPreoperative distractive stress radiograph might be a useful method to predict the extent of intraoperative medial release during total knee arthroplasty.

Combining different rotational alignment axes with navigation may reduce the need for lateral retinacular release in total knee arthroplasty

The aim of the study was to compare femoropatellar alignment and the incidence of lateral retinacular release (LRR) in total knee arthroplasty (TKA) in which the rotational alignment of the femoral component was determined using a combination of different rotational alignment axes and navigation or a single reference axis in the standard procedure. We assessed 66 patients undergoing TKA in whom the rotation of the femoral component was determined on the posterior condylar axis in standard procedures (group A) and 65 patients in whom it was determined by combining the posterior condylar axis, anteroposterior axis and epicondylar axis in navigated procedures (group B). The mean age was 68 and 69 years in groups A and B, respectively. Patellar tracking was assessed after deflation of the tourniquet and LRR performed in the presence of maltracking. Visual analogue scale (VAS), Knee Society Score (KSS), Lonner patellar score and patellar tilt were recorded. LRR was carried out in 18 cases (27 %) in group A and in four (6 %) in group B (p = 0.003). The KSS and VAS were improved significantly compared to the preoperative status, but with no significant differences between the two groups. The patellar score showed a greater improvement in the navigated compared to the standard group at the four week follow-up. Patella tilt improved significantly in both groups. The complication rate was similar in the two groups. Combining different rotational alignment axes with navigation significantly reduces patellar maltracking and the need for LRR compared to the standard procedure in which the posterior condylar axis is used as single anatomical reference.

Predictors and outcomes of lateral release in total knee arthroplasty: A cohort study of 1859 knees

The use of lateral retinacular release (LR) to improve patellar tracking during total knee arthroplasty (TKR) remains contentious. This cohort study explored which pre-operative factors predicted LR, and analysed the effect of LR on short term complications and long term outcomes. One thousand eight hundred fifty-nine TKR patients were followed up for a mean of 5.49years. One hundred fifty-four (8.3%) underwent LR. After regression analysis only, valgus deformity, year of surgery, maximal flexion and the operative surgeon predicted LR. LR patients stayed longer in hospital (10.8 vs 8.6days) and had higher transfusion rates (20.3% vs 10.1%) than non-LR patients, but patellar fracture and infection rates were the same. Range of motion was poorer in the LR patients, but AKS Knee and functional scores were unaffected long term.
