Perceptions and Satisfaction of the Relatives of Intensive Care Unit Patients Regarding the Role of Nurses

Purpose: This study was carried out to determine theperceptions and satisfaction of the relatives ofintensive care unit patients regarding the role of nurses.Materials and Methods: The sample of the studyconsisted of 154 patients’ relatives in the intensive careunits of a University hospital between July andOctober 2019. The Family's Perception of the Role ofNurses scale and Intensive Care Unit SatisfactionScale was used for data collection.Results: The mean total expectation score of thepatients' relatives was 3.760.80 and the mean totalscore of nursing practices was 3.660.80. Theintensive care satisfaction score of patient relativeswas determined as 68,1220,83. There was a significant positive correlation between nursingpractices and care satisfaction, decision-makingsatisfaction, information satisfaction, and ICUsatisfaction.Conclusions: Patient relatives' behavioral roleexpectations from nurses and perceived role enactmentscore were above the average and there was astatistically significant difference between them. Theintensive care satisfaction levels of the patient relativeswere above average and there was a significant,positive, and high-level relationship between theperception of nursing practices and intensive caresatisfaction.

Prediction of post-traumatic stress disorder in family members of ICU patients: a machine learning approach.

Post-traumatic stress disorder (PTSD) can affect family members of patients admitted to the intensive care unit (ICU). Easily accessible patient's and relative's information may help develop accurate risk stratification tools to direct relatives at higher risk of PTSD toward appropriate management. PTSD was measured 90days after ICU discharge using validated instruments (Impact of Event Scale and Impact of Event Scale-Revised) in 2374 family members. Various supervised machine learning approaches were used to predict PTSD in family members and evaluated on an independent held-out test dataset. To better understand variables' contributions to PTSD predicted probability, we used machine learning interpretability methods on the best predictive algorithm. Non-linear ensemble learning tree-based methods showed better predictive performances (Random Forest-area under curve, AUC = 0.73 [0.68-0.77] and XGBoost-AUC = 0.73 [0.69-0.78]) than regularized linear models, kernel-based models, or deep learning models. In the best performing algorithm, most important features that positively contributed to PTSD's predicted probability were all non-modifiable factors, namely, lower patient's age, longer duration of ICU stay, relative's female sex, lower relative's age, relative being a spouse/child, and patient's death in ICU. A sensitivity analysis in bereaved relatives did not alter the algorithm's predictive performance. We propose a machine learning-based approach to predict PTSD in relatives of ICU patients at an individual level. In this model, PTSD is mostly influenced by non-modifiable factors.

Predicting the Negative Emotional Symptoms in Relatives of Patients Residing in Intensive Care Unit


 
 
 Abstract
 Balanced emotions are essential for wellbeing. Having a relative admitted to the intensive care unit (ICU) associated with negative emotional symptoms, and the severity of such symptoms is associated with patients' health status, patients and relatives' characteristics as well. Purposes: To assess the level of depression, anxiety and stress, as well as to identify the predictors of negative emotional symptoms among relatives of Jordanian ICU patients. Methods: Descriptive correlation design was used, and 140 first degree relatives were recruited through convenience sampling. Depression, Anxiety and Stress Scale was used to collect data about relatives’ negative emotional symptoms. Standard multiple regression was conducted to determine the extent to which variance in depression, anxiety and stress could be explained by patients' health status, patients' sociodemographics, and relatives' sociodemographics. Results: Approximately, 56%, 70%, and 76% of relatives are suffering from depression, anxiety and stress respectively. Relatives have been found suffering from mild depression plus moderate anxiety and stress. Patients' Glasgow Coma Scores is the most significant predictor for relatives' stress level. Patients' age and insurance are predictors of relatives' depression, anxiety and stress. Whereas, relatives' gender and age; are the most significant predictors for their negative emotional symptoms. Conclusions: Interestingly, variances in relatives' negative emotional symptoms were explained by patients' sociodemographics and relatives' sociodemographics, more than patients' health status. Therefore, patients' and relatives' sociodemographics should be considered in dealing with relatives' psychological status.This will help nurses to minimize the devastating impact of the negative emotional symptoms on relatives of ICU patients. Factors Affecting Anxiety and Depression Symptoms in Relatives of Intensive Care Unit Patients.

To determine the incidences of anxiety and depression in relatives of patients admitted to an intensive care unit (ICU) and to investigate the relationships between psychological symptoms and demographic features of the patients and their relatives. Relatives of 78 ICU patients were enrolled in the study. Sociodemographic features of patients and their relatives were recorded. The Turkish version of the Hospital Anxiety and Depression Scale was used to assess anxiety and depression. Twenty-eight (35.9%) cases with anxiety and 56 (71.8%) cases with depression were identified. The mean anxiety and depression scores were 9.49 ± 4.183 and 9.40 ± 4.286, respectively. Anxiety (P = .028) and concomitant anxiety with depression (P = .035) were more frequent among family members of young patients. The relationship to the patient, especially being a spouse, was significantly associated with symptoms (anxiety, P = .009; depression, P = .019; and both, P = .005). Spouses and family members of relatively young patients had higher rates of anxiety and depression. In contrast to the literature, depression was more common than anxiety among the relatives of ICU patients. Further research is needed on the impact of cultural and regional differences on anxiety and depression rates in family members of ICU patients.

The intensive care unit (ICU) is an environment in which the technique is more aspects of care. In this perspective, it is important to use communication in nursing, awakening a bond of trust between family and professional. Given this reality aimed to analyze the communication between the nursing staff and the relatives of ICU patients. The descriptive is qualitative approach in the ICU of a teaching hospital. Participants were thirty families of ICU patients. Data were collected through semi-structured interviews in September 2011 and analyzed using content analysis. It was found that family members do not receive nursing guidelines on the environment and health of the patient, and also does not provide time to interact during visits. Thereby the emerging need to encourage the training of these professionals to the conscious use of communication.

Comparison of anxiety, depression, and post-traumatic stress symptoms in relatives of ICU patients in an American and an Indian public hospital

Context:An intensive care unit (ICU) admission of a patient causes considerable stress among relatives. Whether this impact differs among populations with differing sociocultural factors is unknown.Aims:The aim was to compare the psychological impact of an ICU admission on relatives of patients in an American and Indian public hospital.Settings and Design:A cross-sectional study was carried out in ICUs of two tertiary care hospitals, one each in major metropolitan cities in the USA and India.Materials and Methods:A total of 90 relatives visiting patients were verbally administered a questionnaire between 48 hours and 72 hours of ICU admission that included the Hospital Anxiety and Depression Scale (HADS), Beck Depression Inventory-II (BDI-II) and Impact of Events Scale-Revised (IES-R) for post-traumatic stress response.Statistical Analysis:Statistical analysis was done using the Mann-Whitney and chi-square tests.Results:Relatives in the Indian ICU had more anxiety symptoms (median HADS-A score 11 [inter-quartile range 9-13] vs. 4 [1.5-6] in the American cohort; P<0.0001), more depression symptoms (BDI-II score 14 [8.5-19] vs. 6 [1.5-10.5], P<0.0001) but a comparable post-traumatic stress response (IES-R score>30). 55% of all relatives had an incongruous perception regarding “change in the patient's condition” compared to the objective change in severity of illness. “Change in worry” was incongruous compared to the perception of improvement of the patient's condition in 78% of relatives.Conclusions:Relatives of patients in the Indian ICU had greater anxiety and depression symptoms compared to those in the American cohort, and had significant differences in factors that may be associated with this psychological impact. Both groups showed substantial discordance between the perceived and objective change in severity of illness.
