Fifteen-Year Outcomes After Valve-Sparing Aortic Root Remodelling or Exclusion of the Non-Coronary Sinus: When to Preserve the Sinuses of Valsalva?

Isolated exclusion of the non-coronary sinus (NCS) is an attractive strategy in valve-sparing aortic root surgery, which avoids the mobilisation and re-implantation of coronary ostia. However, the long-term durability of aortic valve repair and the fate of remnant sinuses of Valsalva remain unclear. From January 2006 to December 2013, 29 patients underwent replacement of the ascending aorta extending to the NCS (group NCS) and 56 patients underwent a modified Yacoub procedure (group MY) in our centre by a single surgeon. Significant difference of preoperative parameters was observed between two groups in the presence of bicuspid aortic valve (41.4% vs 12.5%, p=0.002) and the diameter of the sinus of Valsalva (47.3±4.7 mm vs 51.5±4.9 mm, p=0.01). The group NCS, as compared to the group MY, was associated with significantly shorter cardiopulmonary bypass time (106.6±40.5 min vs 138.4±37.5 min, p=0.001) and aortic cross clamping time (69.0±21.8 min vs 105.4±27.8 min, p<0.01). The mean follow-up was 11.5±2.8 years. No surgical re-intervention was performed for aortopathies of the aortic root; the neo-sinus were not dilated in either groups (38.2±4.2 mm vs 34.0±4.0 mm, p<0.01). The 10-year freedom from aortic valve-related re-operation was estimated to be 96.6±3.4% and 94.5±3.1% (p=0.58), and the cumulative 10-year survival rates were 95.2±4.6% and 85.6±4.7% (p=0.61) in the group NCS and the group MY, respectively. Aortic valve-sparing isolated NCS replacement can be safely performed in selected patients; its early outcomes, overall survival and long-term freedom from aortic valve-related or aortopathy-related re-intervention were comparable to those obtained with the Yacoub procedure.

Simple completion Bentall procedure of preserving aortic bioprosthesis after aortic valve replacement.

The “completion Bentall” is a redo procedure that consists in replacing the aortic root with reimplantation of coronary ostia in the case of a previously implanted normally functioning aortic prosthesis.1 Bioprosthesis-preserving completion Bentall operation in particular is an extremely rare situation.2-5 We describe a simple and feasible technique of completion Bentall procedure preserving aortic bioprosthesis after aortic valve replacement.

Long-Term Outcomes of Surgical Treatment for Acute Type-A Aortic Dissection with Coronary Artery Involvement.

The surgical strategies for acute type-A aortic dissection (aTAAD) with coronary artery involvement have been controversial, and its prognosis remains unclear. Thus, in this study, we aim to determine the characteristics, surgical strategies, and prognosis of patients with coronary artery involvement due to aTAAD.Retrospective analysis of 65 consecutive aTAAD patients with coronary artery involvement between September 2005 and January 2012 was performed. The patients were divided into two groups: those treated with aTAAD repair and coronary ostia reimplantation (Neri type-A, group A, n = 37) and those with aTAAD repair and coronary artery bypass grafting (Neri type B and C, group B, n = 28).Overall in-hospital mortality was determined to be 8.1% for group A and 21.4% for group B (P = 0.124). No significant difference was determined between groups A and B in cardiopulmonary bypass time, cross-clamp time, cerebral perfusion time, and hospitalization time. Intensive care unit (ICU) stay was 5.8 ± 7.4 days for group A, whereas it was 12.4 ± 10.6 days for group B (P = 0.009). The morbidity of postoperative temporary and permanent neurological dysfunction was similar between the two groups, while renal and respiratory dysfunction were 8.1% versus 25.0% and 16.2% versus 39.3%, respectively (P = 0.062, P = 0.036). Average follow-up time was 112.0 ± 44.8 months, and survival curves have not shown statistical significance between two groups (P = 0.386).Coronary artery dissection with Neri type B and C in acute TAAD has been associated with higher early death, but comparable long-term survival after discharge. However, combined immediate coronary artery bypass grafting and aortic repair remains a safe, effective, and acceptable approach to these challenging group of patients.

Aortic Root and Ascending Aortic Aneurysm Related to One Case. Reimplantation of the Right Coronary Artery by 8 mm Dacron Tube (Cabrol Hemi Mustache) and Review of the Literature

The aortic aneurysm is the 13th leading cause of death in Western countries. The incidence of thoracic aortic aneurysms is estimated at 4.5 cases per 100,000. The diagnosis is often made on a chest x-ray or other imaging tests, such as an echocardiogram done for other heart diseases. Echocardiography is the first test to assess the diameter of the ascending aorta and its progression over time. Most patients are first assessed and followed up with spiral thoracic computed tomography with injection of contrast medium, supplemented by 3-dimensional reconstruction of the aneurysm in order to improve the accuracy of measurements, identification of its proximal part and distal. When dilation of the ascending aorta reaches the critical diameter of 50 mm, there is a risk of aortic dissection or rupture. Supravalvular aneurysms are treated by replacing the ectatic portion with a Dacron&#174 tube in the supracoronary position. Aortic root aneurysms, including coronary ostia, require tube replacement, reimplantation of coronary ostia, as well as surgery on the aortic valve. In this article, we report a case of aneurysm of the aortic root and the ascending aorta treated by aortic valve replacement and the ascending aorta associated with the Cabrol hemi-mustache technique and we review the literature.

Repair of a Syphilitic Aneurysm of the Noncoronary Sinus.

About one-tenth of patients with untreated chronic syphilis and tertiary syphilis develop structural complications involving the coronary ostia, ascending aorta, or aortic root. We describe a unique case of a large aortic root aneurysm of the noncoronary sinus with extrinsic compression of the right coronary artery, a complication of tertiary syphilis. Surgical intervention involved valve-sparing aortic root reconstruction with right coronary ostia reimplantation (hemi-Yacoub). The patient's postoperative course was uneventful; he is healthy approximately 2 years later.

RF05 AORTIC VALVE ENDOCARDITIS COMPLICATED BY AORTO-VENTRICULAR DISCONTINUITY

Abstract Background and Aim: Aortic valve endocarditis remains a life threatening condition especially in case of extension of infection to the surrounding tissue. Aorto-ventricular discontinuity complicates the surgical procedures with worse short- and long-term outcomes. We aim to review patients’ characteristics and surgical results of this dreadful infective disease. Methods: we reviewed prospectively collected data from our central database looking at patients operated for infective aortic valve/prosthesis endocarditis complicated by pseudoaneurysm. We retrieved information of 35 patients operated on during the period 2011-2017. Results: Table 1 summarises demographic and preoperative data. Patients were relatively young, mostly male and most of them had a prior cardiac operation. Redo Bentall operation was performed in 32 cases, it was associated with other procedures in 7 patients: CABG (n = 2), MV surgery (n = 3), MVS + CABG (n = 1) and PVR (n = 1). Three patients underwent AVR: isolated (n = 1), plus MVR (n = 1), associated with ascending aorta replacement (n = 1). Unplanned CABG was necessary in 2 cases: for failure in coronary ostia reimplantation (n = 1) and myocardial ischaemia during CPB weaning (n = 1). The postoperative course was complicated in 29 cases. Nineteen patients sustained LCOS hesitating in MOFS and death in 6 cases. Two patients had major neurologic deficit (permanent cerebral stroke and coma). AKI complicated the course in 15 cases, CVVHD was necessary in 5 patients. Pre-discharge mortality was 23%. Conclusions: patients with complicated aortic valve endocarditis presented generally in a poor preoperative status. Complex aortic root surgery is the only life-saving procedure but it is still associated with high mortality and morbidity rate.

The trap-door technique for coronary reimplantation in aortic root surgery.

Coronary ostia reimplantation is a crucial stage of every aortic root replacement procedure. The 'button technique' is currently the most used method and it has shown satisfactory outcomes over time. However, coronary reimplantation techniques still present drawbacks such as pseudoaneurysm formation, kinking, bleeding and coronary dehiscence, especially when coronaries are hard to mobilize. We adapted the trap-door technique, commonly used in the arterial switch operations, in order to apply it in older patients requiring an aortic root replacement procedure. The key passage of such a technique is the creation of an L-shaped trap-door flap in the conduit graft in order to produce a cone-like extension of the conduit from which the coronary ostium can arise. We applied this technique in 24 patients over 8 years and we did not experience any complication related to coronary ostia reimplantation. In conclusion, we think that this method could be considered as an alternative technique in order to avoid the use of an interposition vascular graft or a pericardial patch when the conventional reimplantation techniques are not feasible.

Orthotopic Replacement of Aortic Heart Valves with Tissue-Engineered Grafts

Heart valve tissue engineering aims to create a graft with improved durability compared to routinely used valve substitutes. This study presents the function and morphological changes of a tissue-engineered aortic valve (TEV) compared to the cryopreserved valve (CPV), aortic valve (AV) allografts in an orthotopic position in sheep. Ovine AV conduits (n=5) were decellularized with detergents. Autologous endothelial cells (ECs) were seeded onto the valve surface and cultured under physiological conditions using a high pulsatile flow. Grafts were implanted as a root with reimplantation of coronary ostia in sheep. Crystalloid cardioplegia and isogenic blood transfusions from previous sacrificed sheep were used. Only antiplatelet aggregation therapy was used postoperatively. CPVs (n=4) served as controls. The grafts were investigated for function (echocardiography, magnetic resonance investigation), morpho/histological appearance, graft rejection, and calcification at 3 months. Decellularization led to cell-free scaffolds with preserved extracellular matrices, including the basement membrane. TEVs were covered with ECs expressing typical endothelial markers. Neither dilatation, stenosis, reductions of cusp mobility nor a significant transvalvular gradient, were observed in the TEV group. Explanted valves exhibited normal morphology without signs of inflammation. An endothelial monolayer covered cusps and the valve sinus. In the CPV group, sporadic, macroscopic, calcified degeneration with mild AV insufficiency was noted. Histology revealed signs of rejection and incipient calcification of the tissue. Tissue-engineered AV based on decellularized valve allografts satisfy short-term requirements of the systemic circulation in sheep. Although results of long-term experiments are pending, the lack of degenerative traits thus far, makes these grafts a promising alternative for future aortic heart valve surgery.

Left Main Coronary Stenosis as a Consequence of Bentall Operation: Percutaneous Treatment

A 65-year-old man suffering from ascending aorta aneurysm and atherosclerotic three vessel disease without left main involvement underwent aortic root replacement with coronary ostia reimplantation according to the modified Bentall technique and multiple coronary artery bypass grafts. Gelatin-resorcin-formaldehyde glue was used to reinforce the aortic coronary buttons and to facilitate hemostasis. Five months after surgery, the patient experienced rapidly worsening effort angina. Coronary angiography showed severe left main narrowing. The considerable amount of time that elapsed between surgery and the onset of symptoms implies that the problem was not related to an imperfect suture technique, but was most likely caused by an inflammatory and proliferative response to the glue that had been used. We performed elective percutaneous coronary intervention and stenting of the protected coronary vessel without technical difficulties and with a satisfactory final result. The patient is currently symptom-free after 1 year's follow-up.

Aneurysms of the ascending aorta and transverse arch: Surgical experience in 80 patients

Aneurysms of the ascending aorta and transverse arch constitute formidable surgical challenges. To assess the impact of surgical techniques on operative morbidity and mortality and late results, we reviewed 80 consecutive patients operated from 1976 through 1980. Average age was 52 years and 81% were male. The operative mortality was 17.5% (14 deaths). In patients with aneurysm of the ascending aorta, operative deaths were due to cardiac factors (three patients), neurologic factors (three patients), cardiac factors (two patients), and exsanguination (one patient) accounted for the six operative deaths in patients with transverse arch aneurysms. Two late neurologic deaths occurred in this group. The following conclusions were reached when the surgical techniques were reviewed: Annuloaortic ectasia is best treated by insertion of a conduit with reimplantation of coronary ostia. Dissections are optimally managed by Dacron graft insertion in the ascending aorta and valve replacement. Aortic valve resuspension was done in six patients, with three undergoing subsequent aortic valve replacement for insufficiency. Aneurysms of the transverse arch treated with profound hypothermia and circulatory arrest were associated with fewer neurologic complications, and the operations were more expeditiously completed. Eleven of 80 patients (14%) had or subsequently needed additional surgical procedures on the aortic valve (insufficiency) or the distal aorta.
