The effect of tracheotomy timing on short- term prognosis in hypertensive intracerebral haemorrhage patients after craniectomy: a retrospective analysis of early vs late intervention

Objective: The present study aimed to compare the clinical outcomes and short-term prognosis in patients with hypertensive intracerebral haemorrhage (HICH) with basal ganglia haematoma volume ≥ 60 mL who underwent early tracheotomy versus late tracheotomy post-craniectomy. Methods: A retrospective analysis was conducted based on the data of 102 patients with basal ganglia haematoma volume ≥ 60 mL after craniectomy between 2016 and 2021. Patients were divided into two groups: early tracheotomy and late tracheotomy. This study evaluated the effect of early tracheotomy on prognosis within 90 days and the impact of tracheotomy timing on overall survival in patients with HICH. Results: Patients in the early tracheotomy group showed a significant reduction in duration of ventilation and intracranial pressure (ICP) at 24 hours (post-tracheotomy) compared to those in the late tracheotomy group. Multivariable logistic regression indicated that late tracheotomy after craniectomy, old age, Glasgow Coma Scale (GCS) ≤ 6, large haematoma volume, and pneumonia after tracheotomy were risk factors for poor prognosis within 90 days in patients with HICH undergoing tracheotomy postoperatively. In terms of the impact of tracheotomy timing on overall survival in patients with HICH, 46 patients died within a follow-up period of 90 days; 19 in the early tracheotomy group (overall survival rate 62.7%), and 27 in the late tracheotomy group (overall survival of 47.1%). Conclusions: Early tracheotomy significantly improved the short-term prognosis of patients with HICH, with a higher overall survival rate compared to late tracheotomy within 90 days of illness.

Minimally invasive surfactant therapy versus intubation for surfactant delivery in preterm infant with RDS: evaluation of safety and efficacy

Background Minimally invasive surfactant therapy (MIST) is a promising mode of administration that offers the potential to limit barotrauma and prevent lung injury in preterm infants with respiratory distress syndrome (RDS). Objective This study assessed the effects of the implementation of MIST on safety and efficacy in infants who met criteria for surfactant administration and were treated by MIST as compared with a historical control group treated with surfactant via an endotracheal tube during mechanical ventilation. Methods This retrospective study included infants born between 2012 and 2017 who met the following inclusion criteria: gestational age 23–36 + 6 weeks, a diagnosis of RDS requiring at least 30% oxygen with or without nasal continuous positive airway pressure (nCPAP). MIST was introduced in 2014 and a comparison was made between the study group who received MIST and the control group who met similar criteria and received surfactant via an endotracheal tube during mechanical ventilation Results No significant differences were found between the groups in baseline and demographic data. Severity of initial disease, assessed by the CRIB II score, was similar in the two groups (control 4.6 ± 2.8, MIST 4.4 ± 2.4, p=.995). The requirement for oxygen during the first 3 d of life was significantly lower (area under the curve [AUC]: p=.001) in the MIST group as assessed by the AUC. Likewise, the mean days of oxygen requirement were significantly lower in the MIST group (Control: 10.3 d, MIST: 5.9 d, p=.04). Only six infants in the MIST group (13%) subsequently required intubation for mechanical ventilation, only one of whom adjacent to the procedure. A modest reduction in duration of ventilation was also noted. Duration of admission was 32 ± 23 d in the control group and 26 ± 21 d in the MIST group, p=.061. No significant differences were found between the groups in the incidence of major morbidities or mortality. No major adverse events related to the procedure were observed. Conclusions Transition to MIST was associated with significantly reduced need for oxygen, mechanical ventilation and surfactant, and a borderline shortened NICU admission.

Intravenous fish oil lipid emulsions in critically ill patients: an updated systematic review and meta-analysis

IntroductionIntravenous fish oil (FO) lipid emulsions (LEs) are rich in ω-3 polyunsaturated fatty acids, which exhibit anti-inflammatory and immunomodulatory effects. We previously demonstrated that FO-containing LEs may be able to decrease mortality and ventilation days in patients who are critically ill. Since 2014, several additional randomized controlled trials (RCTs) of FO-containing LEs have been published. Therefore, the purpose of this systematic review was to update our previous systematic review with the aim of elucidating the efficacy of FO-containing LEs on clinical outcomes of patients who are critically ill.MethodsWe searched electronic databases from 1980 to 2014. We included four new RCTs conducted in critically ill adult patients in which researchers evaluated FO-containing LEs in parenterally or enterally fed patients.ResultsA total of 10 RCTs (n = 733) met inclusion criteria. The mean methodological score was 8 (range, 3 to 12). No effect on overall mortality was found. When we aggregated the results of five RCTs in which infections were reported, we found that FO-containing LEs significantly reduced infections (risk ratio (RR) = 0.64; 95% confidence interval (CI), 0.44 to 0.92; P = 0.02; heterogeneity I2 = 0%). Subgroup analysis demonstrated that predominantly enteral nutrition–based trials showed a tendency toward a reduction in mortality (RR = 0.69; 95% CI, 0.40 to 1.18; P =0.18; heterogeneity I2 =35%). High-quality trials showed a significant reduction in hospital length of stay (LOS) (weighted mean difference = −7.42; 95% CI, −11.89 to −2.94; P = 0.001), whereas low-quality trials had no effect (P = 0.45). The results of the test for subgroup differences in hospital LOS was significant (P = 0.001).ConclusionFO-containing LEs may be associated with a reduction in infections and also could be associated with a reduction in duration of ventilation and hospital LOS. Further large-scale RCTs are warranted and should be aimed at consolidating potential positive treatment effects.

Optimal ventilation strategy

Premature delivery, is always associated to the failure of respiratory transition and a delayed achievement of an adequate functional residual capacity. For this reason preterm babies (expecially the Extremely Low for Gestational Age -ELGA- infants) frequently need a respiratory support. Non-invasive ventilation (NIV) is widespread used in the management of respiratory distress even in ELGA infants without increasing of neonatal mortality or neurological impairment. The most recent meta-analysis and reviews on NIV approach demonstrated that NIV is a valid alternative to mechanical ventilation (MV) in the management of respiratory failure and resulted in significant reductions in the incidence of Bronchopulmonary dysplasia (BPD) amongst surviving infants [1,2]. Nevertheless, even if non-invasive respiratory support seems to be very efficacious in preterm babies, sometimes ELGA infants require MV. Pressure-limited ventilation is yet the standard mode in neonatal intensive care unit, because the attempts to use traditional volume-controlled ventilation it has been often impractical in very small preterm infants. Moreover pressure-limited ventilation continues to be the primary mode of ventilation in neonates because of its relative simplicity and ability to ventilate effectively despite large ETT leak. The major disadvantage of pressure-limited ventilation is that the tidal volume (VT) varies with changes in lung compliance (e.g after surfactant therapy). The consequences of such rapid improvements in compliance are inadvertent hyperventilation and lung injury from excessively large VT (volutrauma); inadvertent hyperventilation may induce hypocapnia, with high risk of cerebral damage. In literature there are extensive evidence that excessive volume, rather than pressure, is the key determinant of ventilator-induced lung injury (VILI). Inadequate (too small) VT also causes significant problems (atelectrauma): in particular inefficient gas exchange due to increased dead-space to VT ratio. Therefore in the last years volume-targeted ventilation has become an important standard of care in neonatal and pediatric respiratory support too. There are many volume-targeted ventilation modes to be used in neonatal period, but Volume Guarantee (VG) ventilation, it is the most extensively studied volume-targeted ventilation mode in the neonate with respiratory distress. The Cochrane review on volume-targeted ventilation resulted in significant reductions in duration of ventilation, rates of pneumothorax, significant reduction in intraventricular haemorrhage (IVH) and a reduction in the incidence of BPD amongst surviving infants, of borderline statistical significance [3]. The VG ventilation plus adequate PEEP and an open lung strategy [4], seems to be an adequate alternative to High frequency Oscillatory Ventilation (HFOV) in mechanically ventilated infants for RDS.

Gadolinium containing contrast agents for pulmonary ventilation magnetic resonance imaging: preliminary results.

Five gadolinium containing contrast media (gadopentetate dimeglumine [Magnevist; Berlex Imaging, Montville, NJ, USA], gadobutrol [Gadovist; Schering, Järfälla, Sweden], gadoteridol [ProHance; Bracco-Byk Gulden, Konstanz, Germany], gadobenate dimeglumine [MultiHance; Bracco-Byk Gulden, Konstanz, Germany], and gadopentetate dimeglumine added with mannitol and a surface active detergent) were evaluated for their efficacy in magnetic resonance depiction of lung ventilation. All contrast agent aerosols were generated by a jet nebulizer. Twelve intubated domestic pigs were mechanically ventilated with the respective aerosolized contrast agent and studied on a 1.5 T MR imager. T1-weighted TSE images using respiratory gating were obtained before and after a 10-minute ventilation period. Pulmonary signal intensity (SI) and signal-to-noise (SNR) changes were measured for both lungs. Mean SI increases ranged between 13.5% and 45.8% (right lung) and 14% and 39.8% (left lung). SNR changes ranged from +14.7% to +46.8% and from +13.1% to +40.5% for the right and left lung, respectively. The highest SI and SNR increases were observed in the gadoteridol group. The use of gadolinium for MR ventilation imaging is primarily hindered by its viscosity properties and thus, its capability of aerosolization. Of the five agents tested, the medium with the lowest viscosity at room temperature (gadoteridol) showed the most promising enhancement results. The results reaffirm the potential of gadolinium-based contrast agents as a pulmonary imaging alternative. With a reduction of ventilation duration down to ten minutes, the method appears tolerable in a clinical setting.
