Enhancing online dose distribution reconstruction in radiotherapy through sparse sampled rotating fiber arrays and deep learning

A optical fiber array dosimeter based on radiation-induced luminescence can be used in two-dimensional dose distribution measurements. However, using rotating fiber arrays introduces challenges in terms of online monitoring and variations in detector performance due to the long data acquisition time. This study aims to propose and demonstrate a novel method for online fast reconstruction of two-dimensional dose distributions in radiation therapy using projection data obtained from sparsely sampled rotating fiber arrays. The projection data were processed in both the projection and image domains to achieve restoration and noise reduction by deep learning techniques. The performance of the proposed method was evaluated using simulation data obtained by Monte Carlo toolkit Geant4 and experimental data obtained from a medical accelerator. Results showed that the method can successfully reconstruct dose distribution through sparse sampling and reduce the measurement time to one-sixth of the original. The peak signal-to-noise ratio, root mean square error, and structural similarity index were 23.72, 2.68, and 0.95, respectively. Additionally, the 3%/3 mm gamma pass rate reached 98.4% under specific irradiation conditions with a dose of at least 10% of the maximum dose. The integration of sparse sampling techniques, rotating fiber arrays, and deep learning techniques offers new possibilities for the fast and efficient reconstruction of two-dimensional dose distributions, ensuring high-quality radiotherapy outcomes.

MiniSCIDOM: a scintillator-based tomograph for volumetric dose reconstruction of single laser-driven proton bunches

Abstract Laser plasma accelerators (LPAs) enable the generation of intense and short proton bunches on a micrometre scale, thus offering new experimental capabilities to research fields such as ultra-high dose rate radiobiology or material analysis. Being spectrally broadband, laser-accelerated proton bunches allow for tailored volumetric dose deposition in a sample via single bunches to excite or probe specific sample properties. The rising number of such experiments indicates a need for diagnostics providing spatially resolved characterization of dose distributions with volumes of approximately 1 cm ${}^3$ for single proton bunches to allow for fast online feedback. Here we present the scintillator-based miniSCIDOM detector for online single-bunch tomographic reconstruction of dose distributions in volumes of up to approximately 1 cm ${}^3$ . The detector achieves a spatial resolution below 500 $\unicode{x3bc}$ m and a sensitivity of 100 mGy. The detector performance is tested at a proton therapy cyclotron and an LPA proton source. The experiments’ primary focus is the characterization of the scintillator’s ionization quenching behaviour.

A tomographic reconstruction algorithm for cross-sectional imaging of IMRT beams from six projections

Objective. Patient-specific Quality Assurance (QA) measurements are of key importance in radiotherapy for safe and efficient treatment delivery and allow early detection of clinically relevant errors. Such QA processes remain challenging to implement for complex Intensity Modulated Radiation Therapy (IMRT) radiotherapy fields delivered using a multileaf collimator (MLC) which often feature small open segments and raise QA issues similar to those encountered in small field dosimetry. Recently, detectors based on long scintillating fibers have been proposed to measure a few parallel projections of the irradiation field with good performance for small field dosimetry. The purpose of this work is to develop and validate a novel approach to reconstruct MLC-shaped small irradiation fields from six projections. Approach. The proposed field reconstruction method uses a limited number of geometric parameters to model the irradiation field. These parameters are iteratively estimated with a steepest descent algorithm. The reconstruction method was first validated on simulated data. Real data were measured with a water-equivalent slab phantom equipped with a detector made of 6 scintillating-fiber ribbons placed at 1 m from the source. A radiochromic film was used to acquire a reference measurement of a first dose distribution in the slab phantom at the same source-to-detector distance and the treatment planning system (TPS) provided the reference for another dose distribution. In addition, simulated errors introduced on the delivered dose, field location and field shape were used to evaluate the ability of the proposed method to efficiently identify a deviation between the planned and delivered treatments. Main results. For a first small IMRT segment, 3%/3 mm, 2%/2 mm and 2%/1 mm gamma analysis conducted between the reconstructed dose distribution and the dose measured with radiochromic film exhibited pass rates of 100%, 99.9% and 95.7%, respectively. For a second and smaller IMRT segment, the same gamma analysis performed between the reconstructed dose distribution and the reference provided by the TPS showed pass rates of 100%, 99.4% and 92.6% for the 3%/3 mm, 2%/2 mm and 2%/1 mm gamma criteria, respectively. Gamma analysis of the simulated treatment delivery errors showed the ability of the reconstruction algorithm to detect a 3% deviation between the planned and delivered doses, as well as shifts lower than 7 mm and 3 mm when considering an individual leaf and a whole field shift, respectively. Significance. The proposed method allows accurate tomographic reconstruction of IMRT segments by processing projections measured with six scintillating-fiber ribbons and is suitable for water-equivalent real-time small IMRT segments QA.

Treatment planning of carbon ion radiotherapy for prostate cancer based on cellular experiments with PC3 human prostate cancer cells

[Purpose] Treatment plans for carbon ion radiotherapy (CIRT) in Japan are designed to uniformly deliver the prescribed clinical dose based on the radiosensitivity of human salivary gland (HSG) cells to the planning target volume (PTV). However, sensitivity to carbon beams varies between cell lines, that is, it should be checked that the clinical dose distribution based on the cell radiosensitivity of the treatment site is uniform within the PTV.[Methods] We modeled the linear energy transfer (LET) dependence of the linear-quadratic (LQ) coefficients specific to prostate cancer, which accounts for the majority of CIRT. This was achieved by irradiating prostate cancer cells (PC3) with X-rays from a 4 MV-Linac and carbon beams with different LETs of 11.1–214.3 keV/μm. By using the radiosensitivity of PC3 cells derived from cellular experiments, we reconstructed prostate-cancer-specific clinical dose distributions on patient computed tomography (CT).[Results] The LQ coefficient, α, of PC3 cells was larger than that of HSG cells at low (<50 keV/μm) LET and smaller at high (>50 keV/μm) LET, which was validated by cellular experiments performed on rectangular SOBPs. The reconstructed dose distribution on patient CT was sloped when 1 fraction incident from the one side of the patient was considered, but remained uniform from the sum of 12 fractions of the left–right opposing beams (as is used in clinical practice).[Conclusion] Our study reveals the inhomogeneity of clinical doses in single-field plans calculated using the PC3 radiosensitivity data. However, this inhomogeneity is compensated by using the combination of left–right opposing beams.

CyberKnife Xsight versus fiducial-based target-tracking: a novel 3D dosimetric comparison in a dynamic phantom

BackgroundThe CyberKnife Xsight lung-tracking system (XLTS) provides an alternative to fiducial-based target-tracking systems (FTTS) for non-small-cell lung cancer (NSCLC) patients without invasive fiducial insertion procedures. This study provides a method for 3D independent dosimetric verification of the accuracy of the FTTS compared to the XLTS without relying on log-files generated by the CyberKnife system.MethodsA respiratory motion trace was taken from a 4D-CT of a real lung cancer patient and applied to a modified QUASAR™ respiratory motion phantom. A novel approach to 3D dosimetry was developed using Gafchromic EBT3 film, allowing the 3D dose distribution delivered to the moving phantom to be reconstructed. Treatments were planned using the recommended margins for one and three fiducial markers and XLTS 2-view, 1-view and 0-view target-tracking modalities. The dose delivery accuracy was analysed by comparing the reconstructed dose distributions to the planned dose distributions using gamma index analysis.ResultsFor the 3%/2 mm gamma criterion, gamma passing rates up to 99.37% were observed for the static deliveries. The 3-fiducial and 1-fiducial-based deliveries exhibited passing rates of 93.74% and 97.82%, respectively, in the absence of target rotation. When target rotation was considered, the passing rate for 1-fiducial tracking degraded to 91.24%. The passing rates observed for XLTS 2-view, 1-view and 0-view target-tracking were 92.78%, 96.22% and 76.08%, respectively.ConclusionsExcept for the XLTS 0-view, the dosimetric accuracy of the XLTS was comparable to the FTTS under equivalent treatment conditions. This study gives us further confidence in the CyberKnife XLTS and FTTS systems.

A novel QA phantom based on scintillating fiber ribbons with implementation of 2D dose tomography for small-field radiotherapy.

To develop a novel instrument for real-time quality assurance (QA) procedures in radiotherapy. The system implements a scintillation-based phantom and associated signal acquisition and processing modules and aims to monitor two-dimensional (2D) dose distributions of small fields. For the proposed phantom, we have designed and realized a prototype implementing six high-resolution tissue-equivalent scintillating fiber ribbons stacked with in-plane 30° rotated orientations from each other. Each ribbon output is coupled to a silicon photodiode linear array (with an element pitch of 400μm) to detect scintillating signal, which represents the projected irradiation profile perpendicular to the ribbon's orientation. For the system providing six acquired projected dose profiles at different orientations, we have developed a two-step signal processing method to perform 2D dose reconstruction. The first step is to determine irradiation field geometry parameters using a tomographic geometry approach, and the second one is to perform specific penumbra estimation. The QA system prototype has been tested on a Novalis TrueBeam STX with a 6-MV photon beam for small elliptic fields defined by 5- and 10-mm cone collimators and for 10×10- and 20×10-mm2 rectangular fields defined by the micro-multileaf collimator. Gamma index analysis using EBT3 films as reference has been carried out with tight 2%-dose-difference (DD)/700-μm-distance-to-agreement (DTA) as well as 1%-DD/1-mm-DTA criteria for evaluating the system performances. The testing also includes an evaluation of the proposed two-step field reconstruction method in comparison with two conventional methods: filtered back projection (FBP) and simultaneous iterative reconstruction technique (SIRT). The reconstructed 2D dose distributions have gamma index pass rates higher than 95% for all the tested configurations as compared with EBT3 film measurements with both 2%-DD/700-μm-DTA and 1%-DD/1-mm criteria. 2D global gamma analysis shows that the two-step and FBP radiation field reconstruction methods systematically outperform the SIRT approach. Moreover, higher gamma index success rates are obtained with the two-step method than with FBP in the case of the fields defined with the stereotactic cones. The proposed small-field QA system makes a use of six water-equivalent scintillating detectors (fiber ribbons) to acquire dose distribution. The developed two-step signal processing method performs tomographic 2D dose reconstruction. A system prototype has been built and tested using hospital facilities with small rectangular and elliptic fields. Testing results show 2D reconstructed dose distributions with high accuracy and resolution. Such a system could potentially be an alternative approach to film dosimetry for small-field QA, which is still widely used as reference in clinical practice.

Clinical rationale for in vivo portal dosimetry in magnetic resonance guided online adaptive radiotherapy.

In magnetic resonance guided online adaptive radiotherapy, the patient model used for plan adaptation and dose calculation is created online under stringent time constraints. This study investigated the ability of in vivo portal dosimetry to detect deviations between the online patient model used for plan adaptation and the actual anatomy of the patient during delivery. Portal images acquired during treatment were used to reconstruct the delivered dose corresponding to online adapted plans of 42 prostate and 20 rectal cancer patients. The reconstructed dose distributions were compared with the dose distributions calculated online by the treatment planning system by γ-analysis and by the difference in median dose to the high-dose volume. Out of 245 prostate and 145 rectal cancer adapted plans, deviations were detected in 5 prostate and in 17 rectal adapted plans corresponding to 3 prostate and 6 rectal patients, respectively. For all but one of the alerts, deviations were explained due to discrepancies observed between the patient model used for plan adaptation and online magnetic resonance images. A single workflow incident in which the supporting arm of the anterior receive coil was accidentally moved in the treatment field was also detected. There is need for independent end-to-end checks in magnetic resonance guided online adaptive workflows including the verification of the online patient model. In vivo portal dosimetry can be used for such purpose as it can detect both patient related deviations and workflow incidents.

Experimental validation of 4D log file-based proton dose reconstruction for interplay assessment considering amplitude-sorted 4DCTs.

The unpredictable interplay between dynamic proton therapy delivery and target motion in the thorax can lead to severe dose distortions. A fraction-wise four-dimensional (4D) dose reconstruction workflow allows for the assessment of the applied dose after patient treatment while considering the actual beam delivery sequence extracted from machine log files, the recorded breathing pattern and the geometric information from a 4D computed tomography scan (4DCT). Such an algorithm capable of accounting for amplitude-sorted 4DCTs was implemented and its accuracy as well as its sensitivity to input parameter variations was experimentally evaluated. An anthropomorphic thorax phantom with a movable insert containing a target surrogate and a radiochromic film was irradiated with a monoenergetic field for various 1D target motion forms (sin, sin4 ) and peak-to-peak amplitudes (5/10/15/20/30mm). The measured characteristic film dose distributions were compared to the respective sections in the 4D reconstructed doses using a 2D γ-analysis (3mm, 3%); γ-pass rates were derived for different dose grid resolutions (1mm/3mm) and deformable image registrations (DIR, automatic/manual) applied during the 4D dose reconstruction process. In an additional analysis, the sensitivity of reconstructed dose distributions against potential asynchronous timing of the motion and machine log files was investigated for both a monoenergetic field and more realistic 4D robustly optimized fields by artificially introduced offsets of ±1/5/25/50/250ms. The resulting dose distributions with asynchronized log files were compared to those with synchronized log files by means of a 3D γ-analysis (1mm, 1%) and the evaluation of absolute dose differences. The induced characteristic interplay patterns on the films were well reproduced by the 4D dose reconstruction with 2D γ-pass rates ≥95% for almost all cases with motion magnitudes ≤15mm. In general, the 2D γ-pass rates showed a significant decrease for larger motion amplitudes and increase when using a finer dose grid resolution but were not affected by the choice of motion form (sin, sin4 ). There was also a trend, though not statistically significant, toward the manually defined DIR for better quality of the reconstructed dose distributions in the area imaged by the film. The 4D dose reconstruction results for the monoenergetic as well as the 4D robustly optimized fields were robust against small asynchronies between motion and machine log files of up to 5ms, which is in the order of potential network latencies. We have implemented a 4D log file-based proton dose reconstruction that accounts for amplitude-sorted 4DCTs. Its accuracy was proven to be clinically acceptable for target motion magnitudes of up to 15mm. Particular attention should be paid to the synchronization of the log file generating systems as the reconstructed dose distribution may vary with log file asynchronies larger than those caused by realistic network delays.

Dictionary-based software for proton dose reconstruction and submilimetric range verification

Objective. This paper presents a new method for fast reconstruction (compatible with in-beam use) of deposited dose during proton therapy using data acquired from a PET scanner. The most innovative feature of this novel method is the production of noiseless reconstructed dose distributions from which proton range can be derived with high precision. Approach. A new MLEM & simulated annealing (MSA) algorithm, developed especially in this work, reconstructs the deposited dose distribution from a realistic pre-calculated activity-dose dictionary. This dictionary contains the contribution of each beam in the plan to the 3D activity and dose maps, as calculated by a Monte Carlo simulation. The MSA algorithm, using a priori information of the treatment plan, seeks for the linear combination of activities of the precomputed beams that best fits the observed PET data, obtaining at the same time the deposited dose. Main results. the method has been tested using simulated data to determine its performance under 4 different test cases: (1) dependency of range detection accuracy with delivered dose, (2) in-beam versus offline verification, (3) ability to detect anatomical changes and (4) reconstruction of a realistic spread-out Bragg peak. The results show the ability of the method to accurately reconstruct doses from PET data corresponding to 1 Gy irradiations, both in intra-fraction and inter-fraction verification scenarios. For this dose level (1 Gy) the method was able to spot range variations as small as 0.6 mm. Significance. out method is able to reconstruct dose maps with remarkable accuracy from clinically relevant dose levels down to 1 Gy. Furthermore, due to the noiseless nature of reconstructed dose maps, an accuracy better than one millimeter was obtained in proton range estimates. These features make of this method a realistic option for range verification in proton therapy.

Preliminary tests of dosimetric quality and projected therapeutic outcomes of multi-phase 4D radiotherapy with proton and carbon ion beams

Objective. The purpose of this study was to perform preliminary pre-clinical tests to compare the dosimetric quality of two approaches to treating moving tumors with ion beams: synchronously delivering the beam with the motion of a moving planning target volume (PTV) using the recently developed multi-phase 4D dose delivery (MP4D) approach, and asynchronously delivering the ion beam to a motion-encompassing internal tumor volume (ITV) combined with rescanning. Approach. We created 4D optimized treatment plans with proton and carbon ion beams for two patients who had previously received treatment for non-small cell lung cancer. For each patient, we created several treatment plans, using approaches with and without motion mitigation: MP4D, ITV with rescanning, static deliveries to a stationary PTV, and deliveries to a moving tumor without motion compensation. Two sets of plans were optimized with margins or robust uncertainty scenarios. Each treatment plan was delivered using a recently-developed motion-synchronized dose delivery system (M-DDS); dose distributions in water were compared to measurements using gamma index analysis to confirm the accuracy of the calculations. Reconstructed dose distributions on the patient CT were analyzed to assess the dosimetric quality of the deliveries (conformity, uniformity, tumor coverage, and extent of hotspots). Main results. Gamma index analysis pass rates confirmed the accuracy of dose calculations. Dose coverage was >95% for all static and MP4D treatments. The best conformity and the lowest lung doses were achieved with MP4D deliveries. Robust optimization led to higher lung doses compared to conventional optimization for ITV deliveries, but not for MP4D deliveries. Significance. We compared dosimetric quality for two approaches to treating moving tumors with ion beams. Our findings suggest that the MP4D approach, using an M-DDS, provides conformal motion mitigation, with full target coverage and lower OAR doses.

Dosimetric Validation of a System to Treat Moving Tumors Using Scanned Ion Beams That Are Synchronized With Anatomical Motion.

PurposeThe purpose of this study was to validate the dosimetric performance of scanned ion beam deliveries with motion-synchronization to heterogenous targets.MethodsA 4D library of treatment plans, comprised of up to 10 3D sub-plans, was created with robust and conventional 4D optimization methods. Each sub-plan corresponded to one phase of periodic target motion. The plan libraries were delivered to a test phantom, comprising plastic slabs, dosimeters, and heterogenous phantoms. This phantom emulated range changes that occur when treating moving tumors. Similar treatment plans, but without motion synchronization, were also delivered to a test phantom with a stationary target and to a moving target; these were used to assess how the target motion degrades the quality of dose distributions and the extent to which motion synchronization can improve dosimetric quality. The accuracy of calculated dose distributions was verified by comparison with corresponding measurements. Comparisons utilized the gamma index analysis method. Plan quality was assessed based on conformity, dose coverage, overdose, and homogeneity values, each extracted from calculated dose distributions.ResultsHigh pass rates for the gamma index analysis confirmed that the methods used to calculate and reconstruct dose distributions were sufficiently accurate for the purposes of this study. Calculated and reconstructed dose distributions revealed that the motion-synchronized and static deliveries exhibited similar quality in terms of dose coverage, overdose, and homogeneity for all deliveries considered. Motion-synchronization substantially improved conformity in deliveries with moving targets. Importantly, measurements at multiple locations within the target also confirmed that the motion-synchronized delivery system satisfactorily compensated for changes in beam range caused by the phantom motion. Specifically, the overall planning and delivery approach achieved the desired dose distribution by avoiding range undershoots and overshoots caused by tumor motion.ConclusionsWe validated a dose delivery system that synchronizes the movement of the ion beam to that of a moving target in a test phantom. Measured and calculated dose distributions revealed that this system satisfactorily compensated for target motion in the presence of beam range changes due to target motion. The implication of this finding is that the prototype system is suitable for additional preclinical research studies, such as irregular anatomic motion.

Verification of the delivered patient radiation dose for non-coplanar beam therapy.

PurposeThere is an increased interest in using non‐coplanar beams for radiotherapy, including SBRT and SRS. This approach can significantly reduce doses to organs‐at‐risk, however, it requires stringent quality assurance, especially when a dynamic treatment couch is used. In this work, new functionality that allows using non‐coplanar beam arrangements in addition to conventional coplanar beams was added and validated to the previously developed in vivo dose verification system.MethodsThe existing program code was modified to manage the additional treatment couch parameters: angle and positions. Ten non‐coplanar test plans that use a static couch were created in the treatment planning system. Also, two plans that use a dynamic treatment couch were created and delivered using Varian Developer mode, since the treatment planning system does not support a dynamic couch. All non‐coplanar test trajectories were delivered on a simple geometric phantom, using an Edge linear accelerator (Varian Medical Systems) with the megavoltage imager deployed and acquiring megavoltage transmission images that were used to calculate the delivered 3D dose distributions in the phantom with the updated dose calculation algorithm. The reconstructed dose distributions were compared using the 3D chi‐comparison test with 2%/2mm tolerances to the corresponding reference dose distributions obtained from the treatment planning system.ResultsThe chi‐comparison test resulted in at least a 97.0% pass rate over the entire 3D volume for all tested trajectories. For static gantry, static couch non‐coplanar fields, and non‐coplanar arcs using dynamic couch the pass rates observed were at least 98%, while for the static couch, non‐transverse coplanar arc fields, pass rates were at least 97%.ConclusionsA model‐based 3D dose calculation algorithm has been extended and validated for a variety of non‐coplanar beam trajectories of different complexities. This system can potentially be applied for quality assurance of treatment delivery systems that use complex, non‐coplanar beam arrangements.

Validation of dose distribution for liver tumors treated with real-time-image gated spot-scanning proton therapy by log data based dose reconstruction.

In spot scanning proton therapy (SSPT), the spot position relative to the target may fluctuate through tumor motion even when gating the radiation by utilizing a fiducial marker. We have established a procedure that evaluates the delivered dose distribution by utilizing log data on tumor motion and spot information. The purpose of this study is to show the reliability of the dose distributions for liver tumors treated with real-time-image gated SSPT (RGPT). In the evaluation procedure, the delivered spot information and the marker position are synchronized on the basis of log data on the timing of the spot irradiation and fluoroscopic X-ray irradiation. Then a treatment planning system reconstructs the delivered dose distribution. Dose distributions accumulated for all fractions were reconstructed for eight liver cases. The log data were acquired in all 168 fractions for all eight cases. The evaluation was performed for the values of maximum dose, minimum dose, D99, and D5–D95 for the clinical target volumes (CTVs) and mean liver dose (MLD) scaled by the prescribed dose. These dosimetric parameters were statistically compared between the planned dose distribution and the reconstructed dose distribution. The mean difference of the maximum dose was 1.3% (95% confidence interval [CI]: 0.6%—2.1%). Regarding the minimum dose, the mean difference was 0.1% (95% CI: −0.5%—0.7%). The mean differences of D99, D5–D95 and MLD were below 1%. The reliability of dose distributions for liver tumors treated with RGPT-SSPT was shown by the evaluation of the accumulated dose distributions.

Automatic dosimetric verification of online adapted plans on the Unity MR-Linac using 3D EPID dosimetry

Background and purposeThe Unity MR-Linac is equipped with an EPID, the images from which contain information about the dose delivered to the patient. The purpose of this study was to introduce a framework for the automatic dosimetric verification of online adapted plans using 3D EPID dosimetry and to present the obtained dosimetric results. Materials and methodsThe framework was active during the delivery of 1207 online adapted plans corresponding to 127 clinical IMRT treatments (74 prostate, 19 rectum, 19 liver and 15 lymph node oligometastases). EPID reconstructed dose distributions in the patient geometry were calculated automatically and then compared to the dose distributions calculated online by the treatment planning system (TPS). The comparison was performed by γ-analysis (3% global/2mm/10% threshold) and by the difference in median dose to the high-dose volume (ΔHDVD50). 85% for γ-pass rate and 5% for ΔHDVD50 were used as tolerance limit values. Results93% of the online plans were verified automatically by the framework. Missing EPID data was the reason for automation failure. 91% of the verified plans were within tolerance. ConclusionAutomatic dosimetric verification of online adapted plans on the Unity MR-Linac is feasible using in vivo 3D EPID dosimetry. Almost all online adapted plans were approved automatically by the framework. This newly developed framework is a major step forward towards the clinical implementation of a permanent safety net for the entire online adaptive workflow.

Reconstruction of dose distributions for fine carbon-ion beams using iterative approximation toward carbon-knife

For the practical application of carbon-knife with fine carbon-ion beams, the quantification of the dose distribution is essential and requires a high spatial resolution. We propose a novel method to quantify dose distributions with a spatial resolution smaller than the dosimeter size. The proposed method innovates the iterative reconstruction technique. Using a diode dosimeter with a sensitive area of 1 mm2, two-dimensional dose-area-product (DAP) distributions were measured at a 0.1 mm step at the surface and near the Bragg peak depths for fine carbon-ion beams of ∼1 mm size at the full width at half maximum (FWHM). Then, the dose distributions were reconstructed with a spatial resolution of 0.1 × 0.1 mm2 from the measured DAP distributions. However, an unnaturally high noise was observed in the reconstructed dose distributions, which were considered to originate from the measurement reproducibility errors of the DAP distributions estimated to be 2.5%–3%. Therefore, a low-pass filtering process was implemented to reduce the errors on the reconstructed dose distributions. The optimum cut-off frequencies of the low-pass filter were estimated depending on the amplitude of the induced noise. Using the filtering process with the obtained optimum cut-off frequency, the dose distribution was quantified with an average error of approximately 3% or less with respect to the peak value, when the actual measurement had an error of 3%. In the reconstructed dose rate distributions, a steep penumbra P80-20 ∼ 0.2 mm was observed at the surface, and a dose rate at the center axis of ∼90 Gy s−1 and a beam size of ∼1.1 mm at FWHM near the Bragg peak were obtained. The proposed method is expected to be useful for the measurement-based determination of microbeam models for commissioning and dose distribution calculations toward carbon-knife applications.

Retrospective dose reconstruction of prostate stereotactic body radiotherapy using cone-beam CT and a log file during VMAT delivery with flattening-filter-free mode.

This study aimed to reconstruct the dose distribution of single fraction of stereotactic body radiotherapy for patients with prostate cancer using cone-beam computed tomography (CBCT) and a log file during volumetric-modulated arc therapy (VMAT) delivery with flattening-filter-free (FFF) mode. Twenty patients with clinically localized prostate cancer were treated with FFF-VMAT, and projection images for in-treatment CBCT (iCBCT) imaging were concomitantly acquired with a log file. A D95 dose of 36.25Gy in five fractions was prescribed to each planning target volume (PTV) on each treatment planning CT (pCT). Deformed pCT (dCT) was obtained from the iCBCT using a hybrid deformable image registration algorithm. Dose distributions on the dCT were calculated using Pinnacle3 v9.10 by converting the log file data to Pinnacle3 data format using an in-house software. Dose warping was performed by referring to deformation vector fields calculated from pCT and dCT. Reconstructed dose distribution was compared with that of the original plan. Dose differences between the original and reconstructed dose distributions were within 3% at the isocenter and observed in PTV and organ-at-risk (OAR) regions. Differences in OAR regions were relatively larger than those in the PTV, presumably because OARs were more deformed than the PTV. Therefore, our method can be used successfully to reconstruct the dose distributions of one fraction using iCBCT and a log file during FFF-VMAT delivery.

X-ray-induced acoustic computed tomography for guiding prone stereotactic partial breast irradiation: a simulation study.

The aim of this study is to investigate the feasibility of x-ray-induced acoustic computed tomography (XACT) as an image guidance tool for tracking x-ray beam location and monitoring radiation dose delivered to the patient during stereotactic partial breast irradiation (SPBI). An in-house simulation workflow was developed to assess the ability of XACT to act as an in vivo dosimetry tool for SPBI. To evaluate this simulation workflow, a three-dimensional (3D) digital breast phantom was created by a series of two-dimensional (2D) breast CT slices from a patient. Three different tissue types (skin, adipose tissue, and glandular tissue) were segmented and the postlumpectomy seroma was simulated inside the digital breast phantom. A treatment plan was made with three beam angles to deliver radiation dose to the seroma in breast to simulate SPBI. The three beam angles for 2D simulations were 17°, 90° and 159° (couch angles were 0 degrees) while the angles were 90 degrees (couch angles were 0°, 27°, 90°) in 3D simulation. A multi-step simulation platform capable of modelling XACT was developed. First, the dose distribution was converted to an initial pressure distribution. The propagation of this pressure disturbance in the form of induced acoustic waves was then modeled using the k-wave MATLAB toolbox. The waves were then detected by a hemispherical-shaped ultrasound transducer array (6320 transducer locations distributed on the surface of the breast). Finally, the time-varying pressure signals detected at each transducer location were used to reconstruct an image of the initial pressure distribution using a 3D time-reversal reconstruction algorithm. Finally, the reconstructed XACT images of the radiation beams were overlaid onto the structure breast CT. It was found that XACT was able to reconstruct the dose distribution of SPBI in 3D. In the reconstructed 3D volumetric dose distribution, the average doses in the GTV (Gross Target Volume) and PTV (Planning Target Volume) were 86.15% and 80.89%, respectively. When compared to the treatment plan, the XACT reconstructed dose distribution in the GTV and PTV had a RMSE (root mean square error) of 2.408 % and 2.299 % over all pixels. The 3D breast XACT imaging reconstruction with time-reversal reconstruction algorithm can be finished within several minutes. This work explores the feasibility of using the novel imaging modality of XACT as an in vivo dosimeter for SPBI radiotherapy. It shows that XACT imaging can provide the x-ray beam location and dose information in deep tissue during the treatment in real time in 3D. This study lays the groundwork for a variety of future studies related to the use of XACT as a dosimeter at different cancer sites.

Tomographic-based 3D scintillation dosimetry using a three-view plenoptic imaging system.

To demonstrate the feasibility of a three-plenoptic camera projection, scintillation-based dosimetry system for measuring three-dimensional (3D) dose distributions of static photon radiation fields. Static x-ray photon beams were delivered to a cubic plastic scintillator volume embedded within acrylic blocks. For each beam, three orthogonal projections of the scintillating light emission were recorded using a multifocus plenoptic camera. Experimental 3D reconstructions of the light distribution were obtained using an iterative maximum likelihood-expectation maximization (ML-EM) algorithm. For this purpose, the elements of the system matrix representing the contribution of the scintillator volume voxels to the camera sensor pixels were calculated using optical design software. A reconstruction-specific correction was applied to light reconstructions to account for scintillating light imaged by the camera but not directly resulting from dose deposition. Cross beam profiles (CBPs) and percentage depth dose (PDD) curves were compared to treatment planning system data for square fields. Three-dimensional and 3D gamma analyses were performed for concave-shaped dose distributions and the Pearson correlation coefficient and reconstruction error were employed to assess the quality of the measured relative 3D dose distributions. A full and accurate model of the plenoptic camera-based scintillation dosimetry system was implemented using the light ray tracing capabilities of optical design software. With this model, light distributions were successfully reconstructed over a volume of 60×60×60mm at a resolution of 2mm. For relative 3D measurements of square radiation fields of , and compared with treatment planning system reference distributions, the maximum root-mean-square error of the CBPs evaluated at two different depths was of 3.2%, 1.2%, and 1.1%, respectively; as for the corresponding linearly fitted PDDs of the square fields, the slopes of the reconstructed dose distributions overestimated those of the reference distributions by at most 0.2%/cm. The 2D gamma passing rate with a criterion of 2%/2mm for the concave-shaped photon field was of 61.6%, 66.1%, and 76.4% using one, two, and three plenoptic projections; the respective success rates become 77.1%, 87.5%, and 94.9% using a criterion of 3%/3mm. The 3D correlation coefficient for the corresponding reconstructions was of 0.688, 0.905, and 0.976, respectively. Three-dimensional light distributions emitted from within a plastic scintillator volume were successfully recovered using optical design software to establish a complete tomographic model of a plenoptic camera-based prototype. The tomographic model can equivalently extend to dynamic dose delivery measurements, providing temporal resolution limited by the camera's exposure time. This feasibility study enables a simplified design-to-implementation process for volumetric scintillation dosimetry prototypes toward fully meeting the clinical needs of 3D dose measurements for static and dynamic delivery techniques.

Three-dimensional dose reconstruction-based pretreatment dosimetric verification in volumetric modulated arc therapy for prostate cancer.

PurposeWe performed three-dimensional (3D) dose reconstruction-based pretreatment verification to evaluate gamma analysis acceptance criteria in volumetric modulated arc therapy (VMAT) for prostate cancer.Materials and MethodsPretreatment verification for 28 VMAT plans for prostate cancer was performed using the COMPASS system with a dolphin detector. The 3D reconstructed dose distribution of the treatment planning system calculation (TC) was compared with that of COMPASS independent calculation (CC) and COMPASS reconstruction from the dolphin detector measurement (CR). Gamma results (gamma failure rate and average gamma value [GFR and γAvg]) and dose-volume histogram (DVH) deviations, 98%, 2% and mean dose-volume difference (DD98%, DD2% and DDmean), were evaluated. Gamma analyses were performed with two acceptance criteria, 2%/2 mm and 3%/3 mm.ResultsThe GFR in 2%/2 mm criteria were less than 8%, and those in 3%/3 mm criteria were less than 1% for all structures in comparisons between TC, CC, and CR. In the comparison between TC and CR, GFR and γAvg in 2%/2 mm criteria were significantly higher than those in 3%/3 mm criteria. The DVH deviations were within 2%, except for DDmean (%) for rectum and bladder.ConclusionsThe 3%/3 mm criteria were not strict enough to identify any discrepancies between planned and measured doses, and DVH deviations were less than 2% in most parameters. Therefore, gamma criteria of 2%/2 mm and DVH related parameters could be a useful tool for pretreatment verification for VMAT in prostate cancer.
