Venous Thromboembolism in Peritoneal Mesothelioma: Uncovering the Hidden Risk.

Venous thromboembolism (VTE) is a common complication in patients with abdominal malignancies. Despite known associations between pleural mesothelioma and increased VTE risk, the characteristics of VTE in patients with peritoneal mesothelioma (PeM) remain undescribed. Patients treated for PeM were retrospectively identified from our institutional database. The frequency of VTE was assessed and logistic regression modeling was employed to assess VTE risk factors. The association between VTE and overall survival was also ascertained. Recommended thromboprophylaxis for patients who underwent surgery at our institution comprised a single preoperative dose of prophylactic anticoagulation, followed by daily dosing for fourweeks postoperatively. Among 120 PeM patients, 26 (21.7%) experienced VTE, including 19/91 (20.9%) surgical patients, 4/23 (17.4%) patients who received systemic therapy, and 3/6 (50%) patients who underwent observation (p=0.21). Most events were symptomatic (n=16, 62%) and were attributable to pulmonary emboli (n=16, 62%). The 90-day postoperative VTE rate was 4.4% (4/91), including 1 of 60 patients who underwent index surgical intervention at our institution and 3 patients with surgery elsewhere. A low serum albumin concentration was associated with VTE in non-surgical patients (odds ratio 0.12, confidence interval [CI] 0.02-0.72; p=0.03). No significant difference in overall survival was observed between patients with and without VTE (median 46.0 months [CI 24.9-67.0] vs. 55.0 months [CI 27.5-82.5]; hazard ratio 0.98 [CI 0.54-1.81], p=0.98). A high risk of VTE was observed in PeM patients, warranting suspicion throughout the disease trajectory. Postoperative VTE rates were within acceptable limits with 4-week thromboprophylaxis.

Surgical Thromboprophylaxis in Patients With Head and Neck Cancer: An Economic Model.

To quantify postoperative venous thromboembolism (VTE) incidence in head and neck cancer (HNC) patients, and assess the economic implications of chemoprophylaxis. Retrospective cost-effective analysis. Fifty-threehealth care organizations. The TriNetX Research Network was queried to identify the 1-month VTE rate in HNC patients undergoing neck dissection from 2012 to 2022. A literature search provided additional postsurgical VTE rates in HNC patients. Costs of prophylactic heparin and enoxaparin were obtained from a drug wholesaler, and VTE-associated medical costs were sourced from the literature. A break-even analysis determined the absolute risk reduction (ARR) in the VTE rate necessary for a medication to break-even on cost. In TriNetX, 8193 HNC surgical patients underwent neck dissection, and an additional 1640 patients underwent neck dissection plus free flap reconstruction without chemoprophylaxis. Respective 1-month VTE rates were 1.3% (n = 103) and 2.5% (n = 41). Four additional studies of 1546 postoperative HNC patients not prescribed chemoprophylaxis reported a mean VTE rate of 3.8% (n = 59), ranging from 1.9% to 13.0%. At $8.40 per week, heparin resulted in cost savings if it decreased the VTE rate by an ARR of at least 0.05%, while enoxaparin, at $23.66 per week, needed to achieve a 0.14% ARR. Considering potential added costs from bleeding complications, heparin, and enoxaparin remained cost-effective if chemoprophylaxis did not increase bleeding complications by an absolute risk of more than 2.86% and 2.79%, respectively. Postoperative VTE rates varied in HNC patients. Despite this, achievable ARRs suggested the potential cost-effectiveness of routine chemoprophylaxis with heparin and enoxaparin.

The impact of bDMARDs on postoperative complications in patients with rheumatoid arthritis: A systematic review and meta-analysis.

The influence of biological disease-modifying antirheumatic drugs (bDMARDs) on postoperative surgical site infection (SSI) and venous thromboembolism (VTE) in patients with rheumatoid arthritis (RA) has not yet been clarified. A systematic literature search was performed using PubMed, Web of ScienceTM, Scopus, and The Cochrane Library databases to identify eligible studies published up to August 2023. All studies comparing postoperative SSI or VTE rates in RA patients with or without bDMARD treatment were included. The protocol for this study was registered in PROSPERO (CRD42021246264) and is available on the University of York website. Overall, 20 studies with 71,885 RA patients and 6 studies with 7918 RA patients were included for postoperative SSI and VTE comparisons, respectively. Patients treated with bDMARDs had significantly higher rates of postoperative SSI than those without treatment (odds ratio 1.50, 95% confidence interval 1.23-1.83, P < .0001). However, these significant differences disappeared in the analysis restricted to 9 studies involving non-tumor necrosis factor α inhibitors. The use of bDMARDs seemed to increase the rate of postoperative VTE (odds ratio 2.20, 95% confidence interval 1.30-3.72, P = .003). A subgroup analysis showed that postoperative osseous complications were significantly less frequent in RA patients with bDMARD treatment than in those without treatment. RA patients treated with bDMARDs had an increased risk of not only postoperative SSI but also VTE. While bDMARD usage merits appropriate attention, there might be positive aspects as well. Further data will be needed to confirm the postoperative risks of bDMARD usage in RA patients.

Venous Thromboembolism Chemoprophylaxis Adherence Rates After Major Cancer Surgery

Venous thromboembolism (VTE) represents a major source of preventable morbidity and mortality and is a leading cause of death in the US after cancer surgery. Previous research demonstrated variability in VTE chemoprophylaxis prescribing, although it is unknown how these rates compare with performance in the Veterans Health Administration (VHA). To determine VTE rates after cancer surgery, as well as rates of inpatient and outpatient (posthospital discharge) chemoprophylaxis adherence within the VHA. This retrospective cohort study within 101 hospitals of the VHA health system included patients aged 41 years or older without preexisting bleeding disorders or anticoagulation usage who underwent surgical treatment for cancer with general surgery, thoracic surgery, or urology between January 1, 2015, and December 31, 2022. The VHA Corporate Data Warehouse, Pharmacy Benefits Management database, and the Veterans Affairs Surgical Quality Improvement Program database were used to identify eligible patients. Data analysis was conducted between January 2022 and July 2023. Inpatient surgery for cancer with general surgery, thoracic surgery, or urology. Rates of postoperative VTE events within 30 days of surgery and VTE chemoprophylaxis adherence were determined. Multivariable Poisson regression was used to determine incidence-rate ratios of inpatient and postdischarge chemoprophylaxis adherence by surgical specialty. Overall, 30 039 veterans (median [IQR] age, 67 [62-71] years; 29 386 men [97.8%]; 7771 African American or Black patients [25.9%]) who underwent surgery for cancer and were at highest risk for VTE were included. The overall postoperative VTE rate was 1.3% (385 patients) with 199 patients (0.7%) receiving a diagnosis during inpatient hospitalization and 186 patients (0.6%) receiving a diagnosis postdischarge. Inpatient chemoprophylaxis was ordered for 24 139 patients (80.4%). Inpatient chemoprophylaxis ordering rates were highest for patients who underwent procedures with general surgery (10 102 of 10 301 patients [98.1%]) and lowest for patients who underwent procedures with urology (11 471 of 17 089 patients [67.1%]). Overall, 3142 patients (10.5%) received postdischarge chemoprophylaxis, with notable variation by specialty. These findings indicate the overall VTE rate after cancer surgery within the VHA is low, VHA inpatient chemoprophylaxis rates are high, and postdischarge VTE chemoprophylaxis prescribing is similar to that of non-VHA health systems. Specialty and procedure variation exists for chemoprophylaxis and may be justified given the low risks of overall and postdischarge VTE.

Factors related to morbidity and mortality of meningiomas resection‑associated venous thromboembolism (Review).

Patients undergoing intracranial meningioma removal have been reported to have an increased risk of venous thromboembolism (VTE). The present study aimed to study meningioma operations and ascertain rates of postoperative VTE more closely and to find out the associated parameters with VTE-related morbidity and mortality in meningioma patients following resection. This meta-analysis included articles involving meningiomas surgery and postoperative VTE [thromboembolic complications: deep venous thrombosis (DVT) and pulmonary embolism (PE)] published in full-text form between January 1980 and January 2021). Collected variables included: First author name, study period covered, publication year, total number of patients and age, number of males, surgical duration, body mass index (BMI), tumor location, proliferation marker for human tumor cells Ki-67 and VTE-related morbidity and mortality. After the initial search and applying all exclusion and inclusion criteria, five articles were left in the final article pool. The total number of patients was 6,505 who underwent surgery for meningiomas and 299 (4.5%) revealed postoperative VTE. The final results showed no potentially significant difference between the total sample and the postoperative VTE group in tumor location and proliferation marker Ki-67 for human cells. By contrast, the results of the analysis for surgical duration and BMI showed a statistically significant difference. Patients who had experienced open surgery for meningiomas were associated with postoperative VTE. Furthermore, surgical duration and BMI were statistically significant VTE-related parameters in patients who underwent meningioma surgery, showing an association with VTE-related morbidity and mortality.

Risk of Postoperative Venous Thromboembolism After Benign Colorectal Surgery: Systematic Review and Meta-analysis.

Venous thromboembolism is a well-established preventable complication after colectomy. Specific guidance on venous thromboembolism prevention after colectomy for benign disease is limited. This meta-analysis aimed to quantify the venous thromboembolism risk after benign colorectal resection and determine its variability. Following Preferred Reporting Items for Systematic Review and Meta-Analysis and Meta-analysis of Observational Studies in Epidemiology Guidelines (PROSPERO: CRD42021265438), Embase, MEDLINE, and 4 other registered medical literature databases were searched from the database inception to June 21, 2021. Inclusion criteria: randomized controlled trials and large population-based database cohort studies reporting 30-day and 90-day venous thromboembolism rates after benign colorectal resection in patients aged ≥18 years. Exclusion criteria: patients undergoing colorectal cancer or completely endoscopic surgery. Thirty- and 90-day venous thromboembolism incidence rates per 1000 person-years after benign colorectal surgery. Seventeen studies were eligible for meta-analysis reporting on 250,170 patients. Pooled 30-day and 90-day venous thromboembolism incidence rates after benign colorectal resection were 284 (95% CI, 224-360) and 84 (95% CI, 33-218) per 1000 person-years. Stratified by admission type, 30-day venous thromboembolism incidence rates per 1000 person-years were 532 (95% CI, 447-664) for emergency resections and 213 (95% CI, 100-453) for elective colorectal resections. Thirty-day venous thromboembolism incidence rates per 1000 person-years after colectomy were 485 (95% CI, 411-573) for patients with ulcerative colitis, 228 (95% CI, 181-288) for patients with Crohn's disease, and 208 (95% CI, 152-288) for patients with diverticulitis. High degree of heterogeneity was observed within most meta-analyses attributable to large cohorts minimizing within-study variance. Venous thromboembolism rates remain high up to 90 days after colectomy and vary by indication for surgery. Emergency resections compared to elective benign resections have higher rates of postoperative venous thromboembolism. Further studies reporting venous thromboembolism rates by type of benign disease need to stratify rates by admission type to more accurately define venous thromboembolism risk after colectomy. Postoperative Venous Thromboembolism Following Hysterectomy in the Department of Defense.

Hysterectomy is the most common major gynecologic procedure performed in the USA. Surgical complications, such as venous thromboembolism (VTE), are known risks that can be mitigated by preoperative risk stratification and perioperative prophylaxis. Based on recent data, the current post-hysterectomy VTE rate is found to be 0.5%. Postoperative VTE significantly impacts health care costs and patients' quality of life. Additionally, for active duty personnel, it can negatively impact military readiness. We hypothesize that the incidence of post-hysterectomy VTE rates will be lower within the military beneficiary population because of the benefits of universal health care coverage. The Military Health System (MHS) Data Repository and Management Analysis and Reporting Tool was used to conduct a retrospective cohort study of postoperative VTE rates within 60 days of surgery among women who underwent a hysterectomy at a military treatment facility between October 1, 2013, and July 7, 2020. Patient demographics, Caprini risk assessment, preoperative VTE prophylaxis, and surgical details were obtained by chart review. Statistical analysis was performed using the chi-squared test and Student t-test. Among the 23,391 women who underwent a hysterectomy at a military treatment facility from October 2013 to July 2020, 79 (0.34%) women were diagnosed with VTE within 60 days of their surgery. This post-hysterectomy VTE incidence rate (0.34%) is significantly lower than the current national rate (0.5%, P < .0015). There were no significant differences in postoperative VTE rates with regard to race/ethnicity, active duty status, branch of service, or military rank. Most women with post-hysterectomy VTE had a moderate-to-high (4.29 ± 1.5) preoperative Caprini risk score; however, only 25% received preoperative VTE chemoprophylaxis. MHS beneficiaries (active duty personnel, dependents, and retirees) have full medical coverage with little to no personal financial burden for their health care. We hypothesized a lower VTE rate in the Department of Defense because of universal access to care and a presumed younger and healthier population. The postoperative VTE incidence was significantly lower in the military beneficiary population (0.34%) compared to the reported national incidence (0.5%). Additionally, despite all VTE cases having moderate-to-high preoperative Caprini risk scores, the majority (75%) received only sequential compression devices for preoperative VTE prophylaxis. Although post-hysterectomy VTE rates are low within the Department of Defense, additional prospective studies are needed to determine if stricter adherence to preoperative chemoprophylaxis can further reduce post-hysterectomy VTE rates within the MHS.

Venous Thromboembolism Rates After Free Flap Reconstruction of the Head and Neck Region.

Venous thromboembolism (VTE) is a major concern for the postoperative hospitalized patient, especially after long and complex procedures. Cancer itself also contributes to the hypercoagulable state, further complicating the management of patients. Despite prophylaxis, breakthrough events can occur. We aimed to assess our institutional VTE and bleeding rates after free flap reconstruction of the head and neck (H&N) region and the factors associated with VTE events. A retrospective review of the patients who underwent H&N free flap reconstruction at an academic center from 2012 to 2021 was performed from a prospectively maintained database. Data regarding patient demographics, medical history, surgical details, and overall outcomes were collected. Outcomes studied included postoperative 30-day VTE rates and major bleeding events. Patients who had a VTE event were compared with the rest of the cohort to identify factors associated with VTE. Free flap reconstruction of the H&N region was performed in 949 patients. Reconstruction after cancer extirpation for squamous cell carcinoma was the most common etiology (79%). The most common flap was thigh based (50%), followed by the fibula (29%). The most common postoperative VTE chemoprophylaxis regimen was enoxaparin 30 mg twice daily (83%). The VTE and bleeding rates over the 10-year period were 4.6% (n = 44) and 8.7% (n = 83), respectively. Body mass index (28.7 ± 5.8 vs 26.2 ± 6.6, P = 0.013) and pulmonary comorbidities were found to be significantly higher in patients who had a VTE event (43% vs 27%, P = 0.017). Patients with a VTE event had a prolonged hospital stay of 8 more days (19.2 ± 17.4 vs 11 ± 7, P = 0.003) and a higher incidence of bleeding events (27% vs 8%, P < 0.001). Postoperative VTE is a significant complication associated with increased length of hospitalization in patients undergoing free flap reconstruction of the H&N region. Institutional measures should be implemented on an individualized basis based on patient comorbidities to improve the postoperative VTE rates, while balancing the bleeding events.

Chemoprophylaxis Timing Is Not Associated With Postoperative Bleeding After Spinal Trauma Surgery

BackgroundLittle is known regarding appropriate timing for chemical venous thromboembolism (VTE) prophylaxis initiation in operative traumatic spinal injuries. We hypothesized that the incidence of post-operative bleeding leading to neurological decline or re-operation would not increase in patients who received early VTE prophylaxis (≤2 days post-surgery) as compared to those who received late VTE prophylaxis (≥ 3 days post-surgery). MethodsThis is a retrospective cohort study. Spine trauma patients who underwent spinal surgery, defined as anterior cervical discectomy and fusion, posterior cervical spinal fusion, anterior or posterior thoracic/lumbar spinal fusion, or vertebral percutaneous fixation from July 2015 to July 2020 were included. Demographics, pre-injury anti-thrombotics, operative characteristics, pre- and post-operative VTE prophylaxis, and post-operative complications, including spinal bleeding, and VTE were collected. Univariate analysis was performed, comparing baseline characteristics, VTE prophylaxis timing, and complications between the early and late groups. ResultsTwo-hundred-eighty-two patients were included; 189 were in the early group (1.7 ± 0.5 days), and 93 were in the late (4.4 ± 2.1 days) group. The late group received enoxaparin more often than patients in the early group (41.9 % vs. 19 %, p < 0.001). Baseline characteristics, hospital course, and surgical management were similar between the groups. The rate of post-operative complications, including hematoma and VTE was similar between the groups. None of the patients in the early group had post-operative bleeding. ConclusionIn this retrospective cohort study, VTE prophylaxis timing was not associated with clinically significant post-operative spinal bleeding and VTE in trauma patients.

Venous Thromboembolism Prophylaxis Adherence Rate in the Veterans Health Administration

INTRODUCTION: Venous thromboembolism (VTE) represents a major source of preventable morbidity and mortality after cancer operation in the US. Previous research demonstrated variability in both inpatient and post-discharge VTE chemoprophylaxis prescribing, though it is unknown how these rates compare with performance in the Veterans Health Administration (VHA). The objective of this study is to determine VTE rate after major cancer operation as well as rate of chemoprophylaxis adherence within the VHA. METHODS: The Veterans Affairs Surgical Quality Improvement Program database (VASQIP) was used to identify patients without pre-existing bleeding disorder or anticoagulation usage who underwent a major abdominal or thoracic operation for cancer between January 1, 2015 and December 31, 2018. Rates of VTE events and VTE chemoprophylaxis adherence were determined. Multivariable logistic regression was used to determine odds of inpatient and outpatient chemoprophylaxis adherence. RESULTS: Overall, 13,377 patients met criteria for inclusion. All patients had a Caprini VTE Risk Index score greater than 5. The overall postoperative VTE rate was 1.7% (n = 227) with 49.4% occurring during inpatient hospitalization and 50.6% occurring while outpatient. Inpatient chemoprophylaxis was ordered for 80.3% of patients, yet only 8.6% of patients received outpatient chemoprophylaxis, with notable variation by specialty (Table). CONCLUSION: The overall VTE rate after major cancer operation within the VA is low. Inpatient chemoprophylaxis administration rate is high. Post-discharge VTE chemoprophylaxis prescribing is low, yet similar to other large, non-VA health systems. Specialty variation exists for chemoprophylaxis, and tailoring use based on risk may be justified. Does Synchronous Liver Intervention Increase Risk of Venous Thromboembolism in Patients with Metastatic Small-Bowel and Pancreatic Neuroendocrine Tumors?

INTRODUCTION: Previous literature suggests a higher incidence of venous thromboembolism (VTE) in patients with pancreatic neuroendocrine tumor (PNET) but no other gastrointestinal NET. In this study, we analyze the incidence of VTE in patients with metastatic small-bowel neuroendocrine tumors (SBNETs) and PNET undergoing synchronous liver procedures to determine whether these patients require extended VTE prophylaxis. METHODS: The American College of Surgeons NSQIP database was queried from 2008 to 2019 for patients undergoing operation for SBNET, PNET, benign small bowel, and pancreatic pathology. Patients with SBNET and PNET were stratified by additional operative procedures for liver metastasis (LM). The rate of 30-day postoperative VTE (pulmonary embolism [PE] and/or deep vein thrombosis [DVT]) was compared. Pearson’s chi-square and ANOVA tests were used for analysis. RESULTS: A total of 89,054 patients met inclusion criteria (63,396 benign small bowel, 3,073 SBNET, including 918 with LM, 18,724 benign pancreas, 3,861 PNET including 949 with LM). There was no statistically significant difference in the rate of VTE in patients with small-bowel disease with or without LM. Patients in the PNET with LM group had a significantly higher rate of PE but not DVT than those undergoing pancreatic operation for benign pathology (2.4% vs 1.1%; p < 0.001). Total operation time (odds ratio [OR] 1.002; p = 0.002), length of hospital stay (OR 1.027; p < 0.001), and preoperative dialysis (OR 3.779; p = 0.004) were independently associated with VTE. However, synchronous liver intervention was not an independent factor associated with VTE or PE in patients with PNET (Table 1). Venous Thromboembolism in Foot and Ankle Surgery: How Common is It?

Category: Other; Ankle; Ankle Arthritis; Bunion; Sports Introduction/Purpose: Venous thromboembolism (VTE) is a feared complication following orthopaedic surgery. The limited research exploring the rate of VTE following different types of foot and ankle surgery makes it difficult to establish clear guidelines for prescribing VTE prophylaxis. Therefore, the purpose of this study was to determine the rate of VTE in patients undergoing various types of foot and ankle procedures. Methods: A large online retrospective database that utilizes ICD and CTP codes was queried to determine the rate of VTE that occurred within 1-month of surgery in common foot and ankle procedures. Surgeries investigated included: Arthrodesis, Total Ankle Arthroplasty, Bunionectomy, Hammer Toe Correction, Watson-Jones, and Achilles repair. Only patients that did not receive postoperative anticoagulation or antiplatelet agents were included in the analysis. Demographic data was also collected. Results: From 2011 to 2022, 27,872 patients underwent Arthrodesis, 1,513 had a Total Ankle Arthroplasty, 31,808 had a Bunionectomy, 26,828 had Hammer Toe correction, 7,545 had a Watson-Jones procedure, and 8,770 had an Achilles repair. Of these procedures, Achilles repair had the highest rate of postoperative VTE (1.32%), followed by Watson-Jones (1.21%), Arthrodesis (0.89%), Ankle Arthroplasty (0.86%), Hammer Toe (0.76%) and Bunionectomy (0.65%). The overall rate of VTE in all procedures was found to be 0.84% (880/104,336). Conclusion: Venous thromboembolism is a serious complication following foot and ankle surgery. The unknown incidence of VTE in common foot and ankle surgeries may contribute to the lack of clear guidelines for prescribing VTE prophylaxis. This study helps to define rates of VTE in patients not receiving prophylaxis that underwent Arthrodesis, Ankle Arthroplasty, Bunionectomy, Hammer Toe, Watson-Jones, and Achilles repair. More studies are needed to help establish clear guidelines for foot and ankle surgeons to follow.

P144. The effect of operative duration on postoperative VTE rates in spinal surgery

<h3>BACKGROUND CONTEXT</h3> The relationship between operative duration and its effect on postoperative venous thromboembolism (VTE) remains unclear in spine surgery. Previous studies have been limited in cohort size or have lacked adequate variability of surgical duration to determine the extent of effect surgery length has on VTE rates. Information regarding patient demographics and the effect of surgical durations on VTE are critical to establish appropriate guidelines for determining which patients could benefit from postoperative chemoprophylaxis. <h3>PURPOSE</h3> To determine the effect of operative duration on postoperative VTE rates in patients undergoing elective thoracolumbar spinal procedures. <h3>STUDY DESIGN/SETTING</h3> Retrospective cohort study. <h3>PATIENT SAMPLE</h3> Patients >18 years of age who underwent thoracolumbar spinal fusion between 2007-2021 were retrospectively identified. A 10:1 propensity match incorporating patient demographics and surgical characteristics was conducted to match and compare patients for postoperative VTE events. <h3>OUTCOME MEASURES</h3> Postoperative VTE events, cut-to-close duration, prior history of clots, Oswestry Disability Index (ODI), Mental (MCS-12) and Physical (PCS-12) Component Summary of the Short-Form 12, visual analog scale (VAS) Back, VAS Leg. <h3>METHODS</h3> A structured query language (SQL) search was used to identify patients who underwent a thoracolumbar surgery and had a postoperative VTE. Only patients with deep VTEs were included. Demographics, surgical characteristics and patient reported outcome measures (PROMs) were collected from the electronic medical record. Patients were categorized into a VTE and non-VTE group. Categorical comparisons were analyzed using Pearson chi-square analysis or Fisher's Exact test. Continuous variables were analyzed using independent samples t-test or Mann-Whitney U test. A binary logistic regression was then performed to determine if cut-to-close duration was a predictor of increased postoperative VTE events—controlling for demographics and a history of clots. A receiver operating characteristic (ROC) curve using Youden's index was created to determine a "cutoff" time for predicting increased likelihood of a VTE based on surgical duration. <h3>RESULTS</h3> A total of 1,456 patients (122 VTE, 1,334 non-VTE) were included. The VTE group was younger (61.5 vs 65.5, p<0.001), predominantly male (60.8% vs 44.5%, p=0.001), experienced a previous VTE (15.6% vs 2.92%, p<0.001) and had higher cut-to-close time (304 mins vs 209 mins, p<0.001). However, there was no difference in CCI between groups (1.08 vs 0.66, p=0.119). When evaluating PROMs, the VTE group had worse 1-year postoperative minus preoperative (∆) ODI (-11.79 vs -21.99, p=0.039), ∆PCS-12 (4.30 vs 8.52, p=0.045), and ∆VAS Leg (-1.41 vs -4.08, p=0.001). Operative duration was an independent predictor of increased postoperative VTE rates on bivariate regression analysis (β=0.01, p<0.001). When evaluating the ROC, a cutoff time of 245 mins was identified (AUC [95% CI] = 0.672 [0.615 – 0.728]) as an optimal predictor of increased risk for a VTE. <h3>CONCLUSIONS</h3> A history of a prior VTE was associated with greater VTE rates during elective thoracolumbar surgery. Patients with a VTE had worse 1-year ∆ODI, ∆PCS-12, and ∆VAS Leg indicating that patients have worse overall improvement after VTE development, even after the VTE has been treated. Additionally, a higher operative duration is an independent risk factor for postoperative VTE. <h3>FDA DEVICE/DRUG STATUS</h3> This abstract does not discuss or include any applicable devices or drugs.

Comorbid conditions increase the incidence of venous thromboembolism after colorectal surgery

Colorectal surgery (CRS) patients are at high risk for venous thromboembolism (VTE). The Caprini model elucidates that comorbidities compound to increase VTE risk, but this association in CRS patients remains undefined. We hypothesize that the compounding presence of comorbidities in patients undergoing colorectal resection (CRR) or small bowel resection (SBR) is associated with greater postoperative VTE risk. This study was a single-institution, retrospective review of patients undergoing CRR and/or SBR from July 2011 to July 2020. Patient demographics, baseline laboratory values, medical comorbidities, and 30-day outcomes were recorded. Incidence of 30-day preoperative or postoperative VTE, including deep venous thrombosis (DVT), pulmonary embolism (PE), portal vein thrombosis (PVT), or inferior vena cava thrombosis (IVCT), were determined. All patients received multimodal in-hospital VTE prophylaxis. After July 2017, patients at high risk for VTE received post-discharge extended chemoprophylaxis. A total of 4354 patients were identified; 129 patients had a postoperative VTE. There was a high incidence of pre-existing comorbidities. In multivariate analysis, acute kidney injury ( p = 0.002) and preoperative 30-day VTE ( p <0.001), American Society of Anesthesiologists classification IV ( p = 0.015) and V ( p = 0.041), and inherited hypercoagulability states ( p = 0.029) were associated with postoperative VTEs. In a logistic regression model, multiple comorbidities ( p <0.001) and preoperative VTE ( p <0.001) were predictive of postoperative VTE. Open SBR ( p = 0.004) and open ( p = 0.001) and laparoscopic ( p <0.001) colectomy were associated with postoperative VTE. After July 2017, there were lower rates of postoperative VTE ( p = 0.041). Multimorbidity is linearly associated with increased rates of postoperative VTE after CRS surgery. This high-risk population could benefit from novel in-hospital and post-hospital VTE prophylaxis measures.

418 Incidence of Venous Thromboembolism and Perioperative SARS-CoV-2 Infection: Multicentre Prospective International Cohort Study

BackgroundSARS-CoV-2 has been associated with an increased rate of venous thromboembolism (VTE) in critically ill patients. Since surgical patients are already at higher risk of VTE than general populations, this study aimed to determine if patients with perioperative or previous SARS-CoV-2 were at further risk of VTE.MethodInternational, multicentre, prospective cohort study of elective and emergency patients undergoing surgery during October 2020. Patient from all surgical specialties were included. The primary outcome measure was VTE (pulmonary embolism or deep vein thrombosis) within 30 days of surgery. SARS-CoV-2 diagnosis was defined as perioperative (7-days before to 30-days after surgery), recent (1–6 weeks before surgery), and previous (³7 weeks before surgery).ResultsThe postoperative VTE rate was 0.5% (666/123,591) in patients without SARS-CoV-2 diagnosis, 2.2% (50/2,317) in patients with perioperative SARS-CoV-2, 1.6% (15/953) in patients with recent SARS-CoV-2, and 1.0% (11/1,148) in patients with previous SARS-CoV-2. After adjustment for confounding factors, patients with perioperative (adjusted odds ratio 1.48, 95% confidence interval 1.08–2.03) and recent SARS-CoV-2 (OR 1.94, 1.15–3.29) remained at higher risk of VTE, with a borderline finding in previous SARS-CoV-2 (OR 1.65, 0.90–3.02). Overall, VTE was independently associated with 30-day mortality (OR 5.39, 4.33–6.70). In SARS-CoV-2 infected patients, mortality without VTE was 7.4% (319/4,342) and with VTE was 40.8% (31/76).ConclusionsPatients undergoing surgery with a perioperative or recent SARS-CoV-2 diagnosis are at increased risk of VTE compared to non-infected surgical patients.

Application of venous thromboembolism prophylaxis program in patients with colorectal cancer using the enhanced recovery after surgery protocol

IntroductionThe incidence of postoperative symptomatic venous thromboembolism (VTE) in western colorectal cancer is 1.1–2.5%. Anticoagulation and mechanical devices are recommended for moderate-to high-risk patients. Hospital stay and immobilization, as risk factors for VTE, are reduced by enhanced recovery after surgery (ERAS). This study aimed to evaluate short- and long-term outcomes for a VTE prophylaxis program after minimally invasive colorectal cancer surgery with ERAS protocol. In addition, predicting factors associated with VTE were investigated. Materials and methodsWe included 1043 patients diagnosed with colorectal cancer who required surgical treatment between January 2017 and December 2019 at a single institution. The patients enrolled followed the VTE prophylaxis program. ResultsFive (0.5%) patients developed symptomatic VTE, and the median follow-up period was 21 months. The Caprini score for all VTE patients was ≤8 points; thus, only mechanical prophylaxis was applied. The incidence rate of postoperative symptomatic VTE was only 0.5%. There was no association between variables considered as associated with VTE onset, such as age, perioperative complication, and length of postoperative day. TNM staging (OR 2.44, 95% CI 1.4–4.16, p = 0.001) and the Caprini score (OR 1.75, 95% CI 1.1–2.8, p = 0.001) were associated with VTE onset. ConclusionAlthough pharmacological prophylaxis was only performed for Caprini scores ≥9, the VTE incidence rate of patients with colorectal cancer undergoing VTE prophylaxis program was 0.6%; 0.7% is the incidence criterion of the moderate group recommended for pharmacological prophylaxis. Continuous follow-up is required for patients with advanced-stage colorectal cancer with high-risk Caprini scores.

A Propensity-Matched Analysis of the Postoperative Venous Thromboembolism Rate After Pancreatoduodenectomy Based on Operative Approach

BackgroundThe use of minimally invasive approaches for pancreatoduodenectomy has increased in recent years, but the risk of postoperative VTE is undefined. We aimed to compare venous thromboembolism (VTE) rates after open and minimally invasive pancreatoduodenectomy using an administrative dataset. MethodsPatients who underwent pancreatoduodenectomy within the National Surgical Quality Improvement Program targeted pancreatectomy database (2016–2018) were identified. VTE was compared between patients who underwent open or minimally invasive pancreatoduodenectomy directly and after propensity score matching 1:1 for demographics, comorbidities, and peri-/intra-operative factors. ResultsA total of 12,227 patients underwent pancreatoduodenectomy during the study period (open: n = 11,217; minimally invasive: n = 1010). Before matching, the VTE rate was higher among patients who underwent minimally invasive pancreatoduodenectomy (5.2% vs. 3.8%, p = 0.033), and minimally invasive resection was independently associated with VTE (OR = 1.46, 95%CI = 1.09–2.06). After matching, there were 916 patients per group without differences in demographics or comorbidities. Patients who underwent minimally invasive pancreatoduodenectomy had longer median operative times (422 vs. 348 min). The VTE rate remained higher following minimally invasive pancreatoduodenectomy after matching (5.1% vs. 2.9%, p = 0.018), mainly driven by a higher DVT rate (3.9% vs. 1.7%, p = 0.005). ConclusionsMinimally invasive pancreatoduodenectomy is associated with a higher postoperative VTE rate compared to open pancreatoduodenectomy.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study.

SummarySARS‐CoV‐2 has been associated with an increased rate of venous thromboembolism in critically ill patients. Postoperative venous thromboembolism rate was 0.5% (666/123,591) in patients without SARS‐CoV‐2; 2.2% (50/2317) in patients with peri‐operative SARS‐CoV‐2; 1.6% (15/953) in patients with recent SARS‐CoV‐2; and 1.0% (11/1148) in patients with previous SARS‐CoV‐2. After adjustment for confounding factors, patients with peri‐operative (adjusted odds ratio 1.5 (95%CI 1.1–2.0)) and recent SARS‐CoV‐2 (1.9 (95%CI 1.2–3.3)) remained at higher risk of venous thromboembolism, with a borderline finding in previous SARS‐CoV‐2 (1.7 (95%CI 0.9–3.0)). Overall, venous thromboembolism was independently associated with 30‐day mortality (5.4 (95%CI 4.3–6.7)). In patients with SARS‐CoV‐2, mortality without venous thromboembolism was 7.4% (319/4342) and with venous thromboembolism was 40.8% (31/76). Patients undergoing surgery with peri‐operative or recent SARS‐CoV‐2 appear to be at increased risk of postoperative venous thromboembolism compared with patients with no history of SARS‐CoV‐2 infection. Optimal venous thromboembolism prophylaxis and treatment are unknown in this cohort of patients, and these data should be interpreted accordingly.

Incidence of Venous Thromboembolism and Safety of Perioperative Subcutaneous Heparin During Inflatable Penile Prosthesis Surgery

ObjectiveTo identify the incidence of venous thromboembolism (VTE) risk factors, postoperative VTE, and to assess the morbidity of perioperative pharmacologic VTE prophylaxis in men undergoing inflatable penile prosthesis (IPP) surgery. MethodsWe retrospectively reviewed 215 patients undergoing IPP surgery between July 2017 and June 2019. Univariate and multivariate statistical analyzes were performed to assess pre-operative Caprini risk score and compare post-operative day 0 scrotal drain output, scrotal hematoma formation, and VTE in men who received subcutaneous heparin (SqH) vs those who did not receive SqH. ResultsOf 215 IPP patients, 84% were classified as high or highest risk for VTE utilizing the Caprini risk score. A total of 119 (55%) received perioperative SqH with or without additional anti-thrombotics. Post-operative day 0 scrotal drain output was higher in those who received SqH compared to those who did not receive SqH, 99.9 mL vs 75.6 mL, respectively (P = .001). Minor scrotal hematomas occurred in similar rates in patients who received perioperative SqH vs those who did not, 3.8% vs 6.3%, respectively (P = .38). Similar results were found on subgroup analysis when eliminating patients who received SqH concurrently with other anti-thrombotics. The overall rate of postoperative VTE was 0.9%. No post-operative infections occurred. ConclusionPatients undergoing IPP surgery are at elevated risk for VTE. To our knowledge, this is the first study showing SqH use in the perioperative IPP surgery setting is safe when used in conjunction with a scrotal drain. Preoperative VTE risk stratification may be performed and can be used to guide clinical decision making regarding pharmacologic prophylaxis.

Racial differences in symptomatic postoperative venous thromboembolism rates after major oncologic resection

BackgroundRecent evidence suggests an increased incidence of venous thromboembolism among cancer patients of Black race. This study aimed to determine whether Black patients undergoing major oncologic resection experience increased rates of postoperative venous thromboembolism. MethodsA cohort study of patients who underwent major oncologic resection was performed using American College of Surgeons National Surgical Quality Improvement Program (2016−2018). Primary outcome was venous thromboembolism within 30 days of surgery. Multivariable logistic regression was performed to evaluate the independent association of race and venous thromboembolism. ResultsOf 91,707 patients, 67.7% were White, 9.5% Black, and 22.9% other race. Venous thromboembolism rates differed slightly by race: 2.2% among Whites, 2.4% Blacks, and 1.8% other (P = .002). Black patients were older, with higher rates of obesity, diabetes, and smoking. By multivariable logistic regression, risk of venous thromboembolism was lower among patients of other compared with White race (odds ratio 0.83; 95% confidence interval, 0.74–0.94). There was no difference in odds of venous thromboembolism among Black relative to White patients (odds ratio 1.08; 95% confidence interval, 0.93–1.26). When stratified by age, rates of venous thromboembolism were >50% higher among Black patients older than 75 years compared with White patients (odds ratio 1.54; 95% confidence interval, 1.17–2.03). ConclusionDespite evidence that Black patients with cancer experience higher rates of venous thromboembolism, they do not appear to have an increased risk in the postoperative period.
