Self-perceived muscular strength, physical activity, and Mediterranean Diet: Impact on health-related quality of life in older adults

The global rise in life expectancy poses challenges for individuals in maintaining independence, health, and quality of life. Health-related quality of life (HRQoL), a key indicator encompassing physical and mental well-being, is crucial for population health monitoring. Regular physical activity (PA), especially muscle-strengthening routines and adherence to the Mediterranean Diet (MD), positively influence HRQoL. Understanding the link between muscular strength and HRQoL is essential for effective physical activity interventions. This study explores the association between self-perceived muscular strength (MS), HRQoL, MD, and sports habits in older adults.The study followed a cross-sectional, descriptive and comparative design, with a sample of 433 physically active Spanish subjects aged 41 to 80. Stratified by MS levels, the high-level group (n= 210; 74.76% men; 57.00 years old (SD: 8.28), and the low level (n= 223; 51.50%; 71.30% men; 55.64 years (SD: 8.56). Self-reported data on MS, HRQoL, MD and PA were collected using the Physical Self-Perception Profile (PSPP 30), Health-Related Quality of Life (SF-36), Mediterranean Diet Adherence Screener (MEDAS) and Rapid Assessment of Physical Activity Questionnaire (RAPA-Q) questionnaires, respectively. An ad-hoc questionnaire captured sports habits and sociodemographic data.The higher MS group exhibited superior physical function (p=0.002, d=0.16), physical role (p=0.023, d=0.22), general health (p<0.001, d=0.61), vitality (p<0.001, d=0.52), and mental health (p<0.001, d=0.29), along with stronger adherence to the Mediterranean Diet (p<0.001, d=0.31). Logistic regression models indicated that a high MS level is associated with engagement in maintenance sports (ORcr = 1.71, 95% CI 1.17-2.50, p=0.005), practicing 3 or more sports (ORadj = 1.78, 95% CI 0.95–3.36, p = 0.028), leading a very active lifestyle (ORadj = 1.96, 95% CI 1.32–2.94, p < 0.001), and adherence to MD (ORadj = 1.64, 95% CI 1.10–2.45, p < 0.016).Engaging in diverse sports, giving priority to maintenance activities and including daily muscle-strengthening routines beyond WHO guidelines, correlates with higher self-perceived strength in older adults. This, in turn, aligns with an elevated quality of life encompassing both physical and mental health, as well as increased adherence to the Mediterranean Diet. Consideration of these factors is crucial when prescribing physical activity for this population and designing institutional health programs.

Influence of Alpine Skiing on Health-Related Quality of Life and Physical Self-Concept in Physically Active Adults over 55 Years of Age.

Background: Older adults have the highest rates of a sedentary lifestyle. Alpine skiing could be considered a suitable activity to encourage continued sports practice and improve the health of this population in winter. The aim of this study was to analyse the relationship between the practice of alpine skiing and health-related quality of life (HRQoL), physical self-concept (PSC), and sport motivation. Methods: The study design was cross-sectional and descriptive, involving 280 Spanish adults aged over 55 years and physically active (75.35% skiers). To assess physical activity, PSC, HRQoL, and sport motivation, we used the Rapid Assessment of Physical Activity Questionnaire (RAPA-Q), the Physical Self-Perception Profile (PSPP 30), the Health-Related Quality of Life (SF-36), and the Sport Motivation Scale (SMS) questionnaires, respectively. Results: In the skier’s group, higher values were found for PSC (p < 0.001; d = 0.64), the physical component of HRQoL (p < 0.001, d = 0.48), physical function (p < 0.001, d = 61), and intrinsic motivation (p < 0.001; d = 0.85). The practice of alpine skiing was associated with higher levels of the physical health component (ORadj = 2.13, 95% CI 1.18–3.95, p = 0.013), PSC (ORadj = 2.92, 95% CI 1.58–5.52, p < 0.001), and intrinsic motivation (ORadj = 2.24, 95% CI 1.22–4.23, p = 0.010). Conclusions: The practice of alpine skiing is positively associated with higher values of HRQoL, PSC, and intrinsic motivation, and based on the above, it seems that alpine skiing can contribute to healthy ageing and improve the quality of life of older adults.

Cross-cultural adaptation and validation of the rapid assessment of physical activity questionnaire (RAPA) in Hungarian elderly over 50 years

BackgroundReliable and valid instruments are needed to estimate physical activity levels. The aim was to culturally adapt the “Rapid Assessment of Physical Activity” (RAPA) into Hungarian and to investigate the validity and reliability of this adapted version in the elderly over 50 years.MethodsIn our cross-sectional study 222 subjects were recruited in Hungary between December 2020 and January 2021(age 61.1 ± 7.9 years, 28% male). Criterion validity of RAPA and International Physical Activity Questionnaire (IPAQ)—Hungarian long version was tested by Spearman’s rank correlation. The examination of repeatability was based on a group of 32 people, and on the one-week test–retest reliability approach, and in addition to this during the statistical analysis intra-class correlation coefficient was calculated. To examine the sensitivity and specificity of the RAPA, negative and positive physical activity values were calculated from the results of the long version of the IPAQ and the RAPA. We tested 4 hypotheses (3 validity, 1 reliability). We considered acceptable validity and reliability if > 75% of hypotheses were confirmed. Results: All of the hypotheses (100%) were confirmed. Based on results of the validity testing of the newly adapted questionnaire was showed a moderate correlation between the examined measurement tools (R = 0.542, p < 0.001). The test–retest results of the questionnaire (N = 32, R = 0.988, p < 0.001) showed strong association.ConclusionRAPA showed fair to moderate validity and strong test–retest reliability similar to other studies. Based on our study’s results the RAPA is a valid and reliable questionnaire to measure the elderly Hungarian population’s physical activity.

Validation of the rapid assessment of physical activity questionnaire in older patients with chronic obstructive pulmonary disease

Purpose: There are not many questionnaires that have proven validity and reliability in evaluating the level of physical activity in older patients with chronic obstructive pulmonary disease (COPD). In clinical and scientific studies, it is important to quickly determine the physical activity level of patients in terms of time management. The aim of the study was to examine the validity of the rapid assessment of physical activity (RAPA) questionnaire in older patients with COPD. &#x0D; &#x0D; Methods: A total of 105 older patients with COPD surveyed the short form of the International Physical Activity Questionnaire (IPAQ-SF) and after 2 weeks RAPA questionnaire as well. The IPAQ-SF was accepted as the gold standard for physical activity assessment in COPD. Furthermore, patients were assessed for a 6-min walking test (6MWT) and perceived dyspnea (with modified medical research dyspnea scale (mMRC)). Correlation coefficients (r) was used for the relationship between RAPA and IPAQ-SF.&#x0D; &#x0D; Results: There were significant positive correlations between RAPA and IPAQ-SF total score (r = 0.968, p&lt; 0.001), 6MWT (r = 0.626, p&lt; 0.001), in addition to significant negative correlation between the RAPA and mMRC scores (r = -0.454, p&lt; 0.001). The RAPA showed good sensitivity (87.5%) and a specificity (100%) value. Positive and negative predictive values of the RAPA were 100% and 96.4% respectively. &#x0D; &#x0D; Conclusion: This study showed the RAPA was an easy-to-use, valid, and reliable questionnaire to measure physical activity levels in older patients with COPD.

Telerehabilitation for Lung Transplant Candidates and Recipients During the COVID-19 Pandemic: Program Evaluation.

BackgroundThe COVID-19 pandemic resulted in a rapid shift from center-based rehabilitation to telerehabilitation for chronic respiratory disease and lung transplantation due to infection control precautions. Clinical experience with this delivery model on a large scale has not been described.ObjectiveThe aim of this study is to describe usage and satisfaction of providers and lung transplant (LTx) candidates and recipients and functional outcomes following the broad implementation of telerehabilitation with remote patient monitoring during the first wave of the COVID-19 pandemic.MethodsThis study was a program evaluation of providers, LTx candidates, and early LTx recipients who used a web-based, remote monitoring app for at least four weeks between March 16 and September 1, 2020, to participate in telerehabilitation. Within-subjects analysis was performed for physical activity, Self-efficacy For Exercise (SEE) scale score, aerobic and resistance exercise volumes, 6-minute walk test results, and Short Physical Performance Battery (SPPB) results.ResultsIn total, 78 LTx candidates and 33 recipients were included (57 [51%] males, mean age 58 [SD 12] years, 58 [52%] with interstitial lung disease, 34 [31%] with chronic obstructive pulmonary disease). A total of 50 (64%) LTx candidates and 17 (51%) LTx recipients entered ≥10 prescribed exercise sessions into the app during the study time frame. In addition, 35/42 (83%) candidates agreed the app helped prepare them for surgery and 18/21 (85%) recipients found the app helpful in their self-recovery. The strongest barrier perceived by physiotherapists delivering the telerehabilitation was patient access to home exercise and monitoring equipment. Between the time of app registration and ≥4 weeks on the waiting list, 26 LTx candidates used a treadmill, with sessions increasing in mean duration (from 16 to 22 minutes, P=.002) but not speed (from 1.7 to 1.75 mph, P=.31). Quadriceps weight (pounds) for leg extension did not change (median 3.5, IQR 2.4-5 versus median 4.3, IQR 3-5; P=.08; n=37). On the Rapid Assessment of Physical Activity questionnaire (RAPA), 57% of LTx candidates scored as active, which improved to 87% (P=.02; n=23). There was a decrease in pretransplant 6-minute walk distance (6MWD) from 346 (SD 84) meters to 307 (SD 85) meters (P=.002; n=45) and no change in the SPPB result (12 [IQR 9.5-12] versus 12 [IQR 10-12]; P=.90; n=42). A total of 9 LTx recipients used a treadmill that increased in speed (from 1.9 to 2.7 mph; P=.003) between hospital discharge and three months posttransplant. Quadriceps weight increased (3 [IQR 0-3] pounds versus 5 [IQR 3.8-6.5] pounds; P<.001; n=15). At three months posttransplant, 76% of LTx recipients scored as active (n=17), with a high total SEE score of 74 (SD 11; n=12). In addition, three months posttransplant, 6MWD was 62% (SD 18%) predicted (n=8).ConclusionsWe were able to provide telerehabilitation despite challenges around exercise equipment. This early experience will inform the development of a robust and equitable telerehabilitation model beyond the COVID-19 pandemic.

Background: Sedentary behaviour contributes to non-communicable diseases, which account for almost 71% of world deaths. Of these, cardiovascular disease is one of the largest causes of preventable death. It is not yet fully understood what level of sedentary behaviour is safe. People with an intellectual disability have poorer health than the general population with higher rates of multi-morbidity, obesity and inactivity. There is a paucity of evidence on whether this poorer health is due to sedentary behaviour or physical inactivity. This systematic review will investigate the sedentary behaviour levels of adults with an intellectual disability. Method: The PRISMA-P framework will be applied to achieve high-quality articles. An extensive search will be conducted in Medline, Embase, psycINFO and Cinahl and grey literature sources. All articles will be independently reviewed by two reviewers and a third to resolve disputes. Initially, the articles will be reviewed by title and abstract and then the full article will be reviewed using stringent inclusion criteria. All article data will be summarised in a standardised tabular format. The National Institute of Health's quality assessment tool will be used to assess article quality. GRADE will be used to assess the quality of the evidence. The primary outcome of interest is the prevalence of sedentary behaviour levels for people with an intellectual disability. The definition of sedentary behaviour to be used for the purposes of this study is: 'low physical activity as identified by metabolic equivalent (MET) or step levels or as measured by the Rapid Assessment of Physical activity questionnaire (RAPA) or the International Physical Activity questionnaire (IPAQ) or sitting for more than 3 hours per day'. Conclusion: This systematic review will provide a critical insight into the prevalence of sedentary behaviour in adults with an intellectual disability.

Symptoms of depression are associated with physical inactivity but not modified by gender or the presence of a cardiovascular disease; a cross-sectional study

BackgroundDepressive symptomatology may act as a barrier to enhance physical activity. This phenomenon is predominantly found in patients with an established cardiovascular disease (CVD) and in female patients. This cross-sectional study investigated (1) the association between symptoms of depression and physical inactivity, and (2) whether this association is different between primary and secondary prevention patients, and between men and women.MethodsThe study design concerns a secondary analysis of baseline data from a randomized clinical trial, including primary and secondary prevention patients (n = 2184; mean age 71.6 ± 8.94), from 34 general practitioner panels. The Rapid Assessment of Physical Activity questionnaire (RAPA) was used to measure patient reported activity levels. Symptoms of depression were determined using the Patient Health Questionnaire (PHQ-9). Multilevel linear regression analysis was used to explore the association between symptoms of depression and physical activity while adjusting for confounders. Gender and whether or not having a CVD were considered as potential effect modifiers for the association between symptoms of depression and inactivity.ResultsSymptoms of depression were associated with lower levels of physical activity. This association was neither different for men and women nor for primary and secondary prevention patients.ConclusionIn primary care patients’ symptoms of depression were associated with physical inactivity. This association was not modified by gender or the presence of a CVD. Future research should focus on lifestyle interventions aiming at the increase of physical activity levels, while emphasizing on improving symptoms of depression in men, women, and patients both with and without a history of CVD.
