POST-CHOLECYSTECTOMY SYNDROME: REASONS AND TACTICS OF SURGICAL TREATMENT

There were examined some experiments in the meanings in the area of postcholecystectomy syndrome. There were decided that some part of them is faced with mistakes of operating or iatrogenic injuries, others - with not full examination of patients or mistaken meaning of information. The main of diagnostics of biliary pathology after cholecystectomy is hepatic sonography, endoscopic retrograde cholangiopancreatography (ERCP), dynamic radionuclide hepatobiliary imaging. In the process during the examinations of papillary stenosis or choledocholithiasis were made endoscopic intervention. Biliary-enteric anastomosis was made to the patients with long papillary stenosis. The conservative cure was made by diagnostics of dynamic radionuclide hepatobiliary imaging of dysfunction Oddi's sphincter.

State of hepatobiliary system at endocrinous pathology

The aim of research is to estimate clinicopathologic aspects of hepatobiliary system abnormalities at leading endocrinous diseases among children and adolescents. There were examined 140 children and adolescents with thyroid gland pathology and 125 children with diabetes mellitus of 1 type. Comparison group consists of 30 children. There was estimated hepatobiliary system condition using fractional duodenal intubation, bile biochemistry, US of liver and gall-bladder, dynamic radionuclide hepatobiliary imaging. It was determined that changes from side of hepatobiliary system depend mostly from functional characteristic of thyroid gland than from duration of disease at pathology of thyroid gland. Changes in biochemical composition of bile at thyreopathy were conditioned by abnormality of biliary excretion motor activity. Clinic-functional changes from hepatobiliary system occurred in 50% of cases and fatty hepatosis was mostly frequent diagnosed at SD of the 1 st type. There were suggested complex of diagnostic examination in order to define pathology of hepatobiliary system at endocrinopathy.

Spontaneous Perforation of the Gallbladder During Infancy Diagnosed on Hepatobiliary Imaging

Spontaneous gallbladder perforation is a very rare condition in infants. Most gallbladder perforations occur in adults, frequently due to trauma. Ultrasound, peritoneal lavage, and retrograde cholangiography may give indirect evidence of the possibility of this condition. There is no definitive diagnostic modality that can definitely determine the diagnosis in such cases. A case of gallbladder perforation with localized biliary ascites detected on radionuclide hepatobiliary imaging is presented.

The use of radionuclide imaging in the evaluation of suspected biliary damage during laparoscopic cholecystectomy

Laparoscopic cholecystectomy offers a reliable and effective alternative to the standard operative cholecystectomy with reduced morbidity and patient recovery time. We report three cases in which radionuclide hepatobiliary imaging was utilized to evaluate the integrity of the extrahepatic biliary ducts following suspected biliary damage during the procedure, documenting bile leakage in two of the patients.

Comparative Imaging of Gallbladder Cancer

We reviewed various imaging approaches in 22 patients with gallbladder cancer. Nineteen had had ultrasonography and nine computed tomography performed. A gallbladder mass or diffuse wall thickening was seen by ultrasonography in 42% and computed tomography in 33% of patients. A significant number of patients had no gallbladder wall abnormality detected by ultrasonography (37%) or computed tomography (56%). Performing both ultrasonography and computed tomography improved the diagnostic rate; in this subgroup the detection rate was 51%. Cholelithiasis, dilated biliary ducts, the liver metastases were associated findings. Percutaneous cholangiography in jaundiced patients revealed the level of bile duct occlusion and often suggested the diagnosis. Radionuclide hepatobiliary imaging simply revealed non-visualization of the gallbladder.

Evaluation of intrahepatic cholestasis with radionuclide hepatobiliary imaging.

Since hepatobiliary imaging with 99mTc=labeled iminodiacetic acid derivatives (HIDA) allows relatively separate, sequential measurement of hepatocyte clearance, parenchymal transit, and biliary excretion, these agents should be useful in differentiating intrahepatic cholestasis (IC) from other hepatobiliary disorders. We studied 18 patients with clinical evidence of IC in whom the parenchymal transit time was increased disproportionately to any decrease in hepatocyte clearance. In a second group of 14 patients with hepatocyte disease but without clinical evidence of IC, the average parenchymal transit time was increased less in relation to the average decrease in hepatocyte clearance than in the IC group. In 15 patients with extrahepatic biliary obstruction, the average hepatocyte clearance was disproportionately increased, as in IC, but large-duct obstruction was identified by scintigrams, ultrasound, or computed tomography. These preliminary results suggest that IC can be diagnosed with hepatobiliary imaging.

A comparison of radionuclide hepatobiliary imaging and real-time ultrasound for the detection of acute cholecystitis.

The relative value of hepatobiliary scintigraphy vs. real-time ultrasound for the identification of acute cholecystitis was evaluated. No significant difference in sensitivity (97% vs. 97%) existed between the two modalities. Scintigraphy demonstrated better specificity (93% vs. 64%) and predictive value (77% vs. 40%). Although real-time ultrasound provided improved sensitivity over static gray-scale imaging for identification of gallbladder disease, hepatobiliary scintigraphy remains the procedure of choice for acute cholecystitis detection.

Diagnosis of biliary atresia with radionuclide hepatobiliary imaging.

Sixteen patients with biliary atresia and 11 patients with neonatal hepatitis were studied preoperatively with either Tc-99m-diethyl-IDA or TC-99m-diisopropyl-IDA. Two parameters were evaluated: hepatocyte clearance and time to appearance of radioactivity in the intestine. Two observers, using a visual grading system of 1 to 4, gave the 16 patients with biliary atresia a hepatocyte clearance grade of 1.7 +/- 0.6 (mean +/- SD); intestinal radioactivity was not seen through 24 hours. The hepatocyte clearance grade of the 11 patients with neonatal hepatitis was 2.1 +/- 0.9 (mean +/- SD) (p greater than 0.05); intestinal radioactivity was seen in nine of 11 patients (p less than 0.001). Using both parameters, 91% of the patients were classified correctly, 4% were misclassified, and 6% were classified as indeterminate; sensitivity and specificity for biliary atresia were 97% and 82%, respectively. Radionuclide imaging with the newer technetium-99m-labeled hepatobiliary radiopharmaceuticals appears promising for the noninvasive diagnosis of biliary atresia.

Diagnosis of acute pancreatitis using the amylase-creatinine clearance ratio and radionuclide hepatobiliary and pancreas imaging.

AbstractAmylase‐creatinine clearance ratio and radionuclide hepatobiliary and pancreas imaging were used together as noninvasive screening procedures in 24 patients suspected of having acute pancreatitis. An increased amylase‐creatinine clearance ratio, normal filling of the gallbladder demonstrated by the radionuclide study, and decreased uptake of the radionuclide in the pancreas scan were helpful in reaching the diagnosis of acute pancreatitis.

HIDA scanning and ultrasonography in the expeditious diagnosis of acute cholecystitis.

Radionuclide hepatobiliary imaging (HIDA scanning), a pathophysiologic modality, has become the diagnostic method of choice for suspected acute cholecystitis. The diagnosis is made quickly with this technic, which is simple, safe, and accurate. It is in widespread use and will soon be available for small community hospitals as well as large metropolitan and university centers. Ultrasound, though an excellent anatomic modality for diagnosing cholelithiasis, does not demonstrate the status of the cystic duct, information essential to the diagnosis of acute cholecystitis.

Radionuclide hepatobiliary imaging in the detection of traumatic biliary tract disease in children.

99m Technetium IDA (99m Tc-IDA) hepatobiliary imaging was used as a diagnostic procedure in children with suspected biliary trauma. A total of eleven 99mTc-IDA examinations were performed in eight children and correlated with the standard radiographs, 99mTc sulfur colloid liver scans, ultrasonographic studies and the clinical course. This technique is recommended in children who are clinically stable and in whom trauma to the biliary system is suspected. The 99mTc-IDA images accurately indicate parenchymal and bile duct injury.

Radionuclide hepatobiliary imaging in congenital biliary tract ectasia (Caroli disease).

We report five cases of Caroli disease in whom the diagnosis was established with99mTc-IDA imaging. This technique provided both functional and morphologic information.99mTc-sulfur colloid imaging detected liver abnormality in 4/4 cases but was non-specific. Ultrasonography (3/3) and computed tomography (2/2) readily showed dilated bile ducts but lacked the functional information of99mTc-IDA imaging.

Rapid Evaluation of Acute Abdominal Pain by Hepatobiliary Scanning

One hundred eight-six patients with suspected acute cholecystitis (AC) underwent radionuclide hepatobiliary imaging with technetium Tc 99m iprofenin to assess the ability of this tracer to detect AC. After intravenous injection of 5 to 10 mCi of this agent, 500,000 count anterior images were obtained at ten-minute intervals for 60 minutes. An abnormal hepatobiliary scan (HBS) diagnostic of AC was defined as one in which the common bile duct, but not the gallbladder, visualized within one hour of tracer administration. In this series, the sensitivity and specificity for HBS in the detection of AC were 97% and 87%, respectively. The hepatobiliary scan should be the procedure of choice for the rapid detection of AC. It is simple, rapid, and safe to perform, enabling a diagnosis to be established within one hour.
