Development and Validation of a Questionnaire on Radiation Protection in Dentistry

The use of ionizing radiation is legally regulated by health authorities due to the risk of harmful effects on patients and occupationally exposed personnel. In dentistry, the possible risk is carcinogenesis, being essential to know and apply radiological protection, in order to reduce this risk. There are few investigations on the level of knowledge of dental students in radiation protection, and none carried out in Central America. Questionnaires are a tool to determine this information; however, during their development, validation is essential, allowing the instrument to be used at different times or populations. The aim of this study was to develop and validate a questionnaire to assess the knowledge in radiation protection of undergraduate and postgraduate students in dentistry. A questionnaire on radiological protection was built based on literature and national legislation. For its validation, the pre-test was carried out in two stages: content review through the judgment of 6 experts, and application to a small sample of students. Once modified according to the pre-test, the questionnaire was applied to 62 students - who did not participate in the pre-test -. Content validity was assessed using Aiken's V coefficient in the first stage of the pre-test. Reproducibility was determined using the intraclass correlation coefficient (ICC), and reliability using Cronbach's alpha. An Aiken V coefficient of 0.95 was obtained, an ICC between 0.697 and 0.729 and a Cronbach's alpha of 0.727. The questionnaire developed is a valid instrument to evaluate the knowledge in radiological protection of undergraduate and postgraduate students in dentistry. This instrument will strengthen and improve the training of dental undergraduate and postgraduate students in radiological protection.

Thyroid Collars in Dental Radiology: 2021 Update.

Background: In 2013, the American Thyroid Association (ATA) issued a "Policy Statement on Thyroid Shielding During Diagnostic Medical and Dental Radiology." The recently updated National Council on Radiation Protection and Measurement Radiation Protection in Dentistry and Oral and Maxillofacial Imaging (NCRP Report No. 177) prompts this review of progress related to patient thyroid shielding since the ATA statement was published. Summary: Relevant publications appearing since the ATA statement were identified by querying PubMed for "thyroid and dental and (collar or shielding)" and substituting specific dental radiographic procedures in the search. The search was expanded by reviewing the cited papers in the PubMed-retrieved papers and by use of the Web of Science to retrieve papers citing the PubMed retrieved publications. Although many quantitative studies have appeared reflective of current dental radiographic instrumentation and practice, much more can be done to foster minimizing radiation to the thyroid. Conclusions: We list seven areas that should be pursued. Among them are harmonizing guidelines for the use of thyroid collars based on the recent studies and a comprehensive survey of current dental radiological practice patterns.

Radiation protection in dentistry - Do we practice what we learn?

Aim: Easy availability, overuse, and lack of reinforcement of the radiation hazards facts have unknowingly resulted in overlooking of as low as reasonably achievable (ALARA) among many dentists. The current survey was designed to assess the awareness, concern, and practice of radiation protection in general dental practice. Materials and Methods: The survey involved 156 dentists in West Bengal, India. A cross-sectional self-administered questionnaire consisting of a total of 28 items was employed. The obtained data were compiled systemically and significance of difference was calculated by Chi-squared test using Statistical Package for Social Sciences 17. Results: Bisecting angle technique and use of E-speed films with manual processing were the most common. The majority of the participants had no idea about the type of cone used (37.2%), tube current (37.8%), kVp 56.4% (88). Exposure time was equally variable with maximum variability in the case of digital radiographs. Use of lead barriers and aprons were poor. Conclusion: The knowledge and practice of radiation protection are not satisfactory. Repeated reinforcement and training and the most importantly change in attitude to follow ALARA is required.

The use of dental radiographs: Update and recommendations

The National Council on Radiation Protection & Measurements updated its recommendations on radiation protection in dentistry in 2003, the Centers for Disease Control and Prevention published its Guidelines for Infection Control in Dental Health-Care Settings in 2003, and the U.S. Food and Drug Administration updated its selection criteria for dental radiographs in 2004. This report summarizes the recommendations presented in these documents and addresses additional topics such as patient selection criteria, film selection for conventional radiographs, collimation, beam filtration, patient protective equipment, film holders, operator protection, film exposure and processing, infection control, quality assurance, image viewing, direct digital radiography and continuing education of dental health care workers who expose radiographs. This report discusses implementation of proper radiographic practices. In addition to these guidelines, dentists should be aware of, and comply with, applicable federal and state regulations. Dentists should weigh the benefits of dental radiographs against the consequences of increasing a patient's exposure to radiation and implement appropriate radiation control procedures.

Any discussion of possible changes in radiation protection in the dentist's surgery must of necessity be tentative until the NRPB reports to the Health and Safety Executive the results of its present survey into current practice. Nonetheless, there are a number of points in Mr. Gifford's review of the WHO Manual on Radiation Protection in Dentistry (January, 1978, p. 57) that deserve further comment. Mr. Gifford distinguishes three situations in dental radiography which he regards as the most hazardous. The first relates to the risk to those members of the dentist's staff, other than those directly involved in radiography (that is the dentist himself and his surgery assistant), from scattered radiation. It is difficult to visualize a situation in which the layout of the surgery puts those outside at greater risk than those within when a standard dental X-ray set is used.

Radiation protection in dentistry for patients, operators and office personnel

To minimize roentgen-ray dosage in oral roentgenography, the operator must ob­ serve three cardinal rules: (1) limitation of tissue area exposed to the primary roentgen-ray beam to the minimum coverage consistent with diagnostic re­ quirements; (2) selective utilization of x-ray wavelengths, and (3) exposure to the minimum amounts of radiation neces­ sary to obtain adequate roentgenograms. Of the 13 practical recommendations for reducing radiation dosage, by far the most effective is the use of the fastest available film.
