Relationship between interprofessional collaboration and psychological distress experienced by healthcare professionals during COVID-19: a monocentric cross-sectional study.

Since the onset of the COVID-19 pandemic, global healthcare systems have faced unprecedented challenges, leading to significant psychological distress among healthcare professionals. Recognizing the importance of enhanced interprofessional collaboration in alleviating this burden, as emphasized by the World Health Organization in 2020, we investigated whether such collaboration could mitigate staff psychological distress during crises. To our knowledge, no study has yet explored the role of interprofessional collaboration as a resilience factor in crises. For this monocentric cross-sectional study at a German university hospital, we examined the relationship between the quality of interprofessional collaboration and the psychological distress of healthcare professionals during the initial pandemic wave. We employed validated mental health instruments, such as the GAD-7 and PHQ-2, to assess anxiety and depressive symptoms. Additionally, custom-designed questionnaires evaluated "Pandemic-Associated Burden and Anxiety (PAB; PAA)" and interprofessional crisis management experiences. A novel "Interprofessional collaboration and communication (IPC)" assessment tool was developed based on international competency frameworks, demonstrating strong reliability. The study involved 299 healthcare professionals (78.6% in direct contact with COVID-19 patients). Moderate levels of PAB/PAA were reported. However, a significant proportion experienced clinically relevant anxiety, as indicated by GAD-7. Negative IPC perceptions correlated with higher levels of psychological distress. Linear regression analysis showed associations between interprofessional collaboration and anxious and depressive symptoms, and pandemic-related burden. Our findings highlight the vital role of enhanced interprofessional collaboration in strengthening the psychological well-being of healthcare professionals during crises. The study underscores the need to foster a collaborative environment and integrate interprofessional education for resilience.

A Quantitative Study of the Relationship Between Interprofessional Collaboration and Job Satisfaction Among Child Welfare Caseworkers

There is a pressing need to improve job satisfaction among child welfare caseworkers given historically high turnover rates that adversely affect family outcomes. Guided by the job demands-resources model, this study examined whether caseworkers’ job satisfaction was associated with their quality of interprofessional collaboration (IPC). Linear regression models were estimated using cross-sectional data provided by the National Child Welfare Workforce Institute. The sample included caseworkers ( N = 1,489) in two states and one urban county in the United States. Results indicated that caseworker job satisfaction increased as their quality of IPC with service providers and court professionals improved. These findings suggest that agency leaders should foster and support IPC as a means of enhancing caseworker job satisfaction.

Perceptions of Interprofessional Collaboration for Children with Multiple and Complex Needs: Development and Psychometric Evaluation of a New Scale

ABSTRACT The quality of interprofessional collaboration is a key determinant of the success and effectiveness of integrative care for children with multiple and complex needs. This study reports on the development and first psychometric evaluation of a questionnaire that assesses professionals’ perceptions of interprofessional collaboration for child welfare-involved children with severe emotional and behavioural problems. We created the scale deductively using collaboration theory and inductively based on a large qualitative study examining interprofessional collaboration between residential care and mental health services in six European countries. Psychometric properties of the Perceptions of Interprofessional Scale for Children with Multiple and Complex Needs (PICOCC) were explored using exploratory and confirmatory factor analysis with two German samples of professionals working in child welfare and child and adolescent psychiatry (N = 360). Internal consistency was calculated using Cronbach’s alpha, known-groups validity was assessed, and convergent validity was established through examining the relationship with a widely used scale, the Perceptions of Interprofessional Collaboration Model - Questionnaire (PINCOM-Q). The 13-item scale had a one-factor solution, showed good internal consistency, and demonstrated known-groups validity. Strong correlations were reported for the PINCOM-Q entire scale and subscales coping, social support, communication, and organisational aims. These results support the theoretical conceptualisation of the construct underlying the scale and underline the potential of the scale in assessing and evaluating interprofessional collaboration in this field.

Effect of Structured Briefing Prior to Patient Arrival on Interprofessional Communication and Collaboration in the Trauma Team.

PurposeTreating a multiple trauma patient is multidisciplinary team work. The performance of the trauma team is crucial to treating the patients safely and effectively. The fundamentals of the performance are the quality of interprofessional collaboration and the communication inside the team and its efficacy for patients’ safety and team’s well being. This is a prospective interventional study. The aim of this paper was to assess interprofessional collaboration and perceived efficacy inside the trauma team and to evaluate the effects of the implementation of a trauma team briefing tool before the arrival of the patient on perceived teamwork performance.Participants and MethodsThe study took place in the emergency department. Participants were members of the trauma team (emergency physicians and nurses). Two validated scales were selected that address interprofessional collaboration and team perceived efficacy: the TEAM survey (revised version) and the Mayo High Performance Teamwork Scale. A detailed and structured team briefing was used. The trauma team filled in the two scales (46 participants). Prior to every multiple trauma patient arrival, the briefing was then implemented for 3 months. At the end of the third month, the two scales were re-administered and the results analysed (31 participants). The main outcome was the variation of proportion of desirable answers. We considered significant only clearly separated confidence intervals (95% CI).ResultsAll items in the questionnaires had better responses in the second round. In 16 items, the differences found were statistically significant with a 95% confidence interval and p<0.05. The perceived communication and collaboration by healthcare professionals of the trauma team improved with the introduction of the tool.ConclusionA team briefing prior to the arrival of a multiple trauma patient enhances providers’ self perception of interprofessional collaboration in the management of multiple trauma patients.

Ethical climate and intention to leave among critical care clinicians: an observational study in 68 intensive care units across Europe and the United States

PurposeApart from organizational issues, quality of inter-professional collaboration during ethical decision-making may affect the intention to leave one’s job. To determine whether ethical climate is associated with the intention to leave after adjustment for country, ICU and clinicians characteristics.MethodsPerceptions of the ethical climate among clinicians working in 68 adult ICUs in 12 European countries and the US were measured using a self-assessment questionnaire, together with job characteristics and intent to leave as a sub-analysis of the Dispropricus study. The validated ethical decision-making climate questionnaire included seven factors: not avoiding decision-making at end-of-life (EOL), mutual respect within the interdisciplinary team, open interdisciplinary reflection, ethical awareness, self-reflective physician leadership, active decision-making at end-of-life by physicians, and involvement of nurses in EOL. Hierarchical mixed effect models were used to assess associations between these factors, and the intent to leave in clinicians within ICUs, within the different countries.ResultsOf 3610 nurses and 1137 physicians providing ICU bedside care, 63.1% and 62.9% participated, respectively. Of 2992 participating clinicians, 782 (26.1%) had intent to leave, of which 27% nurses, 24% junior and 22.7% senior physicians. After adjustment for country, ICU and clinicians characteristics, mutual respect OR 0.77 (95% CI 0.66- 0.90), open interdisciplinary reflection (OR 0.73 [95% CI 0.62–0.86]) and not avoiding EOL decisions (OR 0.87 [95% CI 0.77–0.98]) were all associated with a lower intent to leave.ConclusionThis is the first large multicenter study showing an independent association between clinicians’ intent to leave and the quality of the ethical climate in the ICU. Interventions to reduce intent to leave may be most effective when they focus on improving mutual respect, interdisciplinary reflection and active decision-making at EOL.

Baseline assessment on the quality of interprofessional collaboration among Filipino Mental Health Professionals

Abstract Background Mental health care has recently been a priority health program in the Philippines as evident in the present lobbying of the “Mental Health Act of 2016“. In spite of these developments, the existence and quality of interprofessional collaboration among Filipino Mental Health Professionals remain unexamined. Purpose The aim of this paper is to assess the quality of IPC (IPCQ) and examine what variables affect it. Method Through a respondent-driven sampling, a survey research design was given to a group of Filipino Mental Health Professionals. The survey asked the demographic profile and the perceptions on the quality of IPC through the Collaborative Practice Assessment Tool-Revised (CPAT-R) which is a 21-item tool with 7-point Likert scale and five factors: 1) patient/community-centered care, 2) collaborative communication, 3) interprofessional conflict, 4) role clarification, and 5) environment. Results/discussion Findings revealed that 44 out of 51 (86.3%) participants experienced IPC in their practice; they generally assessed their teams with good quality (Mdn = 5.5). Despite having agreeable collaborative communication (Mdn = 6.3), teams have a subdued ability in interprofessional conflict (Mdn = 4.5). Conclusion: IPC is a concept that must be seen as not gender-biased, a strategy that should be implemented deliberately with long-term goals, and a competency that develops in a non-linear progression. Having a professional obligation to collaborate enhances agreeability towards IPC, while lack of role clarification within the team reduces it.

A nursing perspective of interprofessional work in critical care: Findings from a secondary analysis

PurposeThis article presents a secondary analysis of nurse interviews from a 2-year comparative ethnographic study exploring cultures of collaboration across intensive care units (ICU). Critically ill patients rely on their interprofessional health care team to communicate and problem-solve quickly to give patients the best outcome available. Critical care nurses function at the hub of patient care giving them a distinct perspective of how interprofessional interactions impact collaborative practice. Materials and methodsSecondary analysis of a subset of primary qualitative data is appropriate when analysis extends rather than exceeds the primary study aim. Primary ethnographic data included 178 semistructured interviews of ICU professionals from 8 medical-surgical ICUs in North America; purposeful maximum variation sampling was used to represent each profession accurately. Fifteen anonymized ICU nurse interview transcripts were coded iteratively to identify emerging themes impacting interprofessional collaborative practice. ResultsFindings suggest that quality of interprofessional collaboration is a product of a multitude of factors occurring at multiple levels within the organization. Managerial and organizational factors related to ICU nurse training and staffing may impede development of nurses' interprofessional skills. ConclusionDeliberative development of ICU nurses' interprofessional skills is essential if nursing is to move from primary coordinator to active collaborator in patient management.

The Influence of Authentic Leadership and Empowerment on New-Graduate Nurses’ Perceptions of Interprofessional Collaboration

The aim of this study was to examine new-graduate nurses' perceptions of the influence of authentic leadership and structural empowerment on the quality of interprofessional collaboration in healthcare work environments. Although the challenges associated with true interprofessional collaboration are well documented, new-graduate nurses may feel particularly challenged in becoming contributing members. Little research exists to inform nurse leaders' efforts to facilitate effective collaboration in acute care settings. A predictive nonexperimental design was used to test a model integrating authentic leadership and workplace empowerment as resources that support interprofessional collaboration. Multiple regression analysis revealed that 24% of the variance in perceived interprofessional collaboration was explained by unit-leader authentic leadership and structural empowerment (R = 0.24, F = 29.55, P = .001). Authentic leadership (β = .294) and structural empowerment (β = .288) were significant independent predictors. Results suggest that authentic leadership and structural empowerment may promote interprofessional collaborative practice in new nurses.

Interdisciplinary collaboration between primary care, social insurance and social services in the rehabilitation of people with musculoskeletal disorder: Effects on self-rated health and physical performance

Previous research shows there can be good results from co-financing between welfare sectors on the perceived quality of interprofessional collaboration. However, little is known about the impact on patient outcome of such schemes. This study aimed to assess whether co-financed teams with personnel from primary care, social insurance and social services have any effect on patients' health status. A comparative study of patients attending health care centres with and without a co-financed collaboration model was carried out. Although research has shown positive results from co-financed collaboration on staff and organization, we could not find that this new interdisciplinary team structure gave a better patient health outcome than conventional care.
