We appreciate Dr. Cortese's pertinent comments concerning our article [1,2]. In particular we agree that in our experience the transradial approach is preferred by patients. Indeed before the percutaneous coronary procedures we have also assessed in all patients the preferred vascular approach after explaining advantages and disadvantages of the two approaches. Sixty-three percent of patients indicate transradial approach as the preferred access, 30% had no opinion and only 7% preferred the transfemoral approach (unpublished data). Concerning themore intense antiplatelet therapy in the transradial approach patients, we recently confirmed these data in a multicenter study of patients with acute coronary syndromes [3]. This means that transradial approachmay allowamore aggressive antiplatelet therapy without increasing bleeding risk. However we do not know the level at which the risk of excess bleedings related to aggressive antiplatelet therapy outweighs the benefit of transradial approach in reducing these events.
