Anaplastic lymphoma kinase-positive pulmonary inflammatory myofibroblastic tumour: a case report

BackgroundPulmonary inflammatory myofibroblastic tumour (IMT) is a rare condition that usually presents in young individuals and is associated with anaplastic lymphoma kinase (ALK)-translocation.Case presentationWe report a case of an 18-year-old Caucasian man with ALK-translocated pulmonary IMT treated with multimodality therapy. The patient presented with breathlessness and was found to have a collapsed left lung. Further investigations revealed an ALK-translocated pulmonary IMT. This is usually treated with an ALK-inhibitor but patient declined after discussing potential side-effects and had repeated rigid bronchoscopic interventions for local disease control. Due to persistent local recurrence, patient received radical external beam radiotherapy (EBRT) with pulse steroids, and one year later started on Ibuprofen, a non-steroidal anti-inflammatory agent (NSAID). Following multimodality treatment, he developed a complete response. He remains treatment-free for the past seven years. Eleven years on from his diagnosis, he remains in remission with a ECOG performance status of zero.ConclusionsAchieving long-term local control in pulmonary IMT can be challenging. Multimodality treatment is sometimes needed but the overall outlook remains good.

A Rare Case of Incidentally Diagnosed Pulmonary Inflammatory Myofibroblastic Tumour with Dramatic Response to Crizotinib in a Postpartum Woman.

Inflammatory myofibroblastic tumours are rare neoplasms which most commonly affect children and young adults. With an intermediate malignant potential, they are typically detected in the abdomen, lung, mediastinum, head and neck, gastrointestinal tract, and genitourinary tract. We describe the case of a 33-year-old postpartum woman incidentally diagnosed with a pulmonary inflammatory myofibroblastic tumour following complaints of poorly controlled hypertension a week after caesarean section. She was ALK-negative and received an ALK inhibitor with complete resolution of the lesion. A ROS1-TFG fusion confirmed the diagnosis of an inflammatory myofibroblastic tumour after CT-guided fine needle aspiration. This case highlights an uncommon presentation posing a diagnostic and therapeutic challenge and the potential treatment option of crizotinib. Inflammatory myofibroblastic tumour (IMT) is a rarely reported neoplasm arising in the abdominal soft tissues, the lung, mediastinum, head, neck, gastrointestinal tract, and genitourinary tract, with intermediate malignant potential.IMT is definitively diagnosed only after histological examination following surgical biopsy, based on immunohistochemical markers and the molecular characteristics of the tumour, but small biopsies may have a role in a large lesion.IMT did not cause any complications during gestation.Therapeutic approaches include surgical resection and chemotherapy, including with crizotinib, an ALK tyrosine kinase inhibitor.

Treatment, pathological characteristics, and prognosis of pulmonary inflammatory myofibroblastic tumor–a retrospective study of 8 cases

ObjectiveInflammatory myofibroblastic tumor (IMT) is a rare disease. We reviewed data from eight patients diagnosed with pulmonary IMT (PIMT) at our hospital with the aim of summarizing and analyzing the characteristics of PIMT to improve our understanding of the disease.MethodsFrom January 2012 to December 2019, eight patients underwent surgical intervention for PIMT at The First Affiliated Hospital of Bengbu Medical College. Resected tumors were subjected to pathological and immunohistochemical analyses. The follow-up duration for all patients ranged from 2 years and 3 months to 9 years and 9 months (median: 6 years and 9 months).ResultsThe male:female ratio was 5:3, and the mean age was 48.50 years (21–74 years). Two patients (25%) with lung disease discovered via chest computed tomography during physical examinations had not experienced any symptoms. Six patients (75%) presented at the hospital because of cough, expectoration, blood in sputum, and chest tightness. Lesions from all eight patients were surgically removed, and PIMT was confirmed based on pathological examinations and immunohistochemical results. No patient received additional treatment after discharge. All cases have been followed up to the time of writing, without any tumor recurrence or distant metastasis.ConclusionThe age of onset of PIMT is usually over 40 years, and its clinical symptoms are easily confused with those of lung cancer. PIMT can only be diagnosed by histopathology and immunohistochemistry. Complete surgical resection is the preferred treatment, as patients undergoing surgery require no additional treatment, such as chemotherapy, and the survival rate is good.

Clinical analysis of primary lung tumors in 56 children

Objective: To investigate the clinical manifestation, pathological type, treatment and prognosis of primary lung tumors in children. Methods: We collected and retrospectively analyzed the clinical manifestation, pathological type, therapeutic method and prognosis of 56 primary lung tumors patients who diagnosed from 2009 to 2019 in Guangzhou Women and Children Medical Center. Results: There were 56 patients identified as the primary lung tumors, including pleuropulmonary blastoma (PPB, n=28), pulmonary inflammatory myofibroblastic tumor(IMT, n=20), mucoepidermoid carcinoma(n=6), infantile hemangioma (n=1), pulmonary sclerosing hermangioma(n=1). Respiratory symptoms were the most manifestation at the time of diagnosis including 26 patients with cough, 3 with hemoptysis, and 17 with dyspnea. Others included 15 with fever, 3 with chest pain, and 2 with epigastiric pain. The primary tumor of 18 cases were located in the lower lobe of left lung, 11 cases in the lower lobe of right lung, 10 cases in the upper lobe of left lung, 7 cases in the upper lobe of right lung, 6 cases in the middle lobe of right lung, and 4 cases in pulmonary hilum. Among the 56 patients, 41 patients underwent thoracotomy, 13 thoracoscopy, and 2 fiberoptic bronchoscopy. Five patients with type Ⅰ PPB were still alive at the end of follow-up without chemotherapy. Among 5 patients with type Ⅱ PPB, 2 patients without chemotherapy died after recurrence, 3 patients suffered postoperative chemotherapy were still alive at the end of follow-up. All of the 18 patients with type Ⅲ PPB underwent postoperative chemotherapy with IVADo regimen. Recurrence occurred in 6 cases, distant metastasis occurred in 3 cases, and cancer-related deaths occurred in 8 cases. For 20 patients with IMT, recurrence occurred in 5 of 13 patients experienced wedge resection, 1 of 6 patients experienced lobectomy and 1 of 6 underwent fiberoptic bronchoscopy, respectively. For 6 mucoepidermoid carcinoma patients, lobectomy was carried on 5 patients, wedge resection on 1 patient, all of them were still alive at the end of follow-up. One hermangioma patient underwent fiberoptic bronchoscopy and other 1 sclerosing hermangioma patient underwent wedge resection, both of them were still alive at the end of follow-up. Conclusions: The clinical manifestations of the primary lung tumors in children are nonspecific. Complete resection and achieving negative marginattribute to the excellent outcome. Adjunctive treatment such as chemotherapy is necessary for patients with type Ⅱ and type Ⅲ PPB.

Surgical treatment outcomes of pulmonary inflammatory myofibroblastic tumors.

BACKGROUND:Pulmonary inflammatory myofibroblastic tumor (PIMT) is an extremely rare disease. The aim of this study was to share the surgical outcomes of these tumors.METHODS:Patients who were operated for pulmonary myofibroblastic tumors between January 2005 and January 2021 were determined by retrospectively scanning patient files. Patients' demographic characteristics, tumor location, surgical techniques, and other parameters were obtained from the patient files. The KaplanMeier method was used for survival calculations, whereas the log-rank test was used for comparison of survival calculations.RESULTS:PIMTs were noted in 14 patients (0.12%) in a total of 11,108 thoracic procedures performed in our institution between January 2005 and January 2021. The mean age of the patients was 28.2 (range: 2–67) years. Of the patients, six were male and eight were female, with 50% (n = 7) aged under 18 years. A total of 17 surgical procedures were performed on 14 patients. One patient underwent pneumonectomy, two patients lobectomy, ten0 patients wedge resection, and one patient underwent debulking surgery. A total of 11 patients had complete surgery, whereas three patients had incomplete surgery. The 10-year overall survival was 84.6% and the 10-year disease-free survival (DFS) was 75.0%. Complete resection was found to be the only and significant factor that had an effect on survival (P = 0.004) and DFS (P = 0.012).CONCLUSION:PIMTs are extremely rare. Complete surgery should be considered an effective factor in survival and DFS.

Operated pulmonary inflammatory myofibroblastic tumors: Our experience with 17 cases.

Background In this study, we aimed to evaluate the clinicopathological features of pulmonary inflammatory myofibroblastic tumor cases operated in our clinic. Methods A total of 17 inflammatory myofibroblastic tumor patients (5 males, 12 females; median age: 46 years) who were operated in our clinic between February 2000 and July 2019 were included. Data including sex, age, symptoms, accompanying diseases, tumor localization, tumor diameter, endobronchial extension, maximum standard uptake value of the tumors, surgery type, recurrence, and survival data were analyzed. Results Two patients were diagnosed preoperatively and two patients were diagnosed during surgery using frozen-section method before resection. Three (17.7%) patients underwent pneumonectomy, five (29.4%) patients lobectomy, three (17.7%) patients segmentectomy, five (29.4%) patients wedge resection, and one (5.8%) patient bronchial sleeve resection. All patients had complete resection with negative margins. None of them had lymph node metastasis. Median follow-up was 122 (range, 8 to 245 months) months. None of the patients received adjuvant therapy, there was no tumor recurrence or tumor-related death. Conclusion It is difficult to make a preoperative diagnosis of inflammatory myofibroblastic tumor patients. Systematic lymph node dissection is not required in diagnosed patients. Complete resection is the most important prognostic factor, and it is critical to achieve this with the smallest resection possible.

Pulmonary inflammatory myofibroblastic tumor in a breast cancer patient, a rare presentation

Inflammatory myofibroblastic tumor (IMT), a benign and rare neoplasm, of mesenchymal origin, pseudoarcomatous, is responsible for about 0.7% of all neoplastic presentations in the pulmonary region, being the most frequent cause of pulmonary nodules in pediatric patients, approximately 50% of the presentations. Thus, the present study aims to report a rare case of IMT in the lung site in a 31-year-old patient undergoing treatment for breast cancer, according to a literary review, relating the data according to the clinical presentation. Due to scarce documentation in the medical literature, no cases were found similar to the one described in relation to the reported oncological primary condition of the patient, making the present case a clinical challenge in modern medical practice.

INFLAMMATORY MYOFIBROBLASTIC TUMOR OF THE MEDIASTINUM: A RARE CASE REPORT

TOPIC: Disorders of the Mediastinum TYPE: Medical Student/Resident Case Reports INTRODUCTION: Inflammatory fibroblastic tumors (IMT) are rare. It is most common in children and adolescent. The most common sites are lung, liver, spleen, head and neck and very rarely mediastinum. We present a patient who was diagnosed with rare IMT of the mediastinum (IMTM). CASE PRESENTATION: A white female in her late twenties with history of substance abuse presented with progressive dysphagia, odynophagia and weight loss since last 1 month with exertional shortness of breath. Physical examination showed cachectic female with tenderness to palpation of the neck and intravenous track marks on both upper extremities with normal saturation on room air. Computed tomography (CT) of the neck revealed 3 x 3.5 cm right paratracheal mass and therefore she underwent CT of the chest which revealed 10 x 9.4 cm mass in the right para-hilar area invading the mediastinum resulting in obstruction of the esophagus and right main bronchus along with compression of inferior aspect of the trachea. There was also an encasement of right pulmonary artery and right side of the atrium. Bronchoscopy with biopsy was performed, which remained non-diagnostic. Patient underwent endoscopic bronchial ultrasound with biopsy but again remained to be non-diagnostic. Our patient was considered as a high risk for a thoracoscopy procedure under general anesthesia for tumor resection due to her airway and vascular compromise along with risk of aspiration due to esophageal obstruction. Therefore, she underwent with CT guided biopsy of the mediastinal mass which revealed inflammatory cells with atypical appearing spindle cells confirming the diagnosis of IMTM with TFG-ROS1 gene fusion. She was discharged home with gastrostomy tube for nutrition and was started on crizotinib by oncologist. After five months, her tumor size shrink to 7.5 x 7.2 cm. She is currently tolerating soft diet and continued to be on crizotinib neo-adjunctively with plan for surgical resection in the future. DISCUSSION: IMTM are discovered on imaging when patient presents with symptoms including dyspnea, cough, thoracic discomfort and dysphagia. Establishing diagnosis of IMT on needle biopsy is difficult due to small tissue sample. Only 6.3% of IMT cases are diagnosed on analysis of biopsy specimen alone. Definitive diagnosis requires histopathologic and immunohistochemical study from a resected tumor. IMT most commonly have ALK gene rearrangements. Recently, kinase fusions including ROS1 and PDGFRβ are also identified. Surgical resection is considered as a definitive treatment, but, crizotinib, a new chemotherapeutic agent is being used recently, with improvement in the tumor burden which are not amenable to resection. CONCLUSIONS: In conclusion, we are reporting a rare IMT occurring in mediastinum which rarely presents with dysphagia and need to be included as differential diagnosis in young patients presenting with obstructive dysphagia. A case of pulmonary inflammatory myofibroblastic tumor treated with bronchoscopic therapy plus lobectomy

BackgroundInflammatory myofibroblastic tumor (IMT) is a rare tumor with malignant potential. We presented a case of a young adult who was diagnosed with IMT and treated with loop electrocautery therapy to relieve airway obstruction, followed by lobectomy to complete resection. Recent studies have supported the use of such interventional resection methods.Case presentationA non-smoking 30-year-old woman presented with a 1-month history of progressive dyspnea and productive cough. The Chest X-ray showed a homogenous opacity invading the entire left hemithorax, and the mediastinum content was attracted to the left side. In an effort to avoid pneumonectomy and afford rapid palliation of dyspnea, loop electrocautery was selected as the most appropriate therapy. The left upper lobectomy by thoracoscopy was performed instead of left upper lobe sleeve resection in order to better prevent the recurrence of lung atelectasis. After 6 years of follow-up, no evidence of recurrence has been found till now.ConclusionInterventional bronchoscopy coupled with surgical resection serves not only as a palliative management to bronchial obstruction but also a way to avoid pneumonectomy.

P38.16 Expression of ROS1 Gene Rearrangement in Pulmonary Inflammatory Myofibroblastic Tumor With Cerebral Metastasis: Case Report

Primary pulmonary inflammatory myofibroblastic tumor (IMT) is a rare benign neoplasm; the tumor has been characterized by the application of immunohistochemical techniques, molecular biology and cytogenetics, which are very precious for the diagnosis. The purpose of this case was to determine whether immunohistochemistry for ROS1 could predict ROS1 rearrangement in inflammatory myofibroblastic tumor. A 50-year-old Moroccan man with history of mitral stenosis and mechanical valve replacement; presented to our hospital with a complaint of fatigue; headaches and lower back pain for one month. Contrast-enhanced computed tomography of his chest revealed a heterogenous well-defined mass lesion with central irregular calcification with no mediastinal lymphadenopathy or satellite nodules; this mass involving the left seventh rib and vertebral body and left atrial cavity. CT brain showed four solid enhancing irregular shaped mass lesions with hypodense areas consistent with metastasis CT-guided biopsy of the chest lesion showed fibrosis with dystrophic calcification; and non-specific chronic inflammation. He underwent radical excision of the right frontal lesion, and histopathology was distinctive fascicular growth of spindle cells with with a prominent inflammatory infiltrate, showed weak, diffuse and dot-like cytoplasmic staining with anaplastic lymphoma kinase (ALK)-negative immunohistochemistry and Fluorescence in situ hybridization (FISH) analysis was performed on interphase nuclei on 5 micron thin sections of formalin-fixed paraffin-embedded tissue that was found to be positive for TFG-ROS1 fusion. Because surgical excision was not possible crizotinib therapy (250 mg, bid) was chosen for the existence of ROS1 fusion gene with brain radiotherapy boost to metastasis. Treatment with glucocorticoids (methylprednisolone 1 mg/kg) and radiotherapy (5 days/week for 3 weeks at 3 Gy/fraction, 45 Gy/15 days) was started for cerebral metastasis . After taking crizotinib for 3 months, the strong changes in the size of the pulmonary mass was showed at chest chest computed tomography with decreased infiltration around the adjacent structures. Inflammatory pseudotumour of the lung with brain metastasis is very uncommon; we must continued therapy with crizotinib and long-term follow-up is necessary, for detection of recurrence, metastasis and to understand the natural course, especially in patients with myofibroblastic differentiation ROS1 positive Crizotinib is effective for pulmonary IMT, and should be taken into consideration for the treatment of inoperable pulmonary IMT patients who lack ALK rearrangement.

P89.10 Genomic Alteration Features and Targeted Therapy Opportunities of Primary Thoracic Sarcoma Patients

Primary sarcomas of the thorax are rare, which may arise in the lung, mediastinum, pleura and chest wall. Studies of comprehensive molecular characteristics and targeted therapy opportunity in thoracic sarcoma patients are lacking. Formalin fixed, paraffin embedded (FFPE) tumor tissues and matched blood samples of 30 thoracic sarcoma patients were collected for 450 cancer gene targeted next-generation sequencing (NGS) assay in a College of American Pathologists (CAP) accredited and Clinical Laboratory Improvement Amendments (CLIA) certified laboratory. Genomic alterations and tumor mutational burden (TMB) values were assessed by next-generation sequencing assay with a mean coverage of 1000X, including single base substitution, short and long insertion/deletions, copy number variations, gene fusions and rearrangements. In total, tumor samples from 30 thoracic sarcoma patients were recruited, including 16 males and 14 females with a median age of 39 (range 11-67). This cohort of patient was comprised of seven undifferentiated pleomorphic sarcoma (UPS), four spindle cell sarcoma, three inflammatory myofibroblastic tumor (IMTs), three Ewing sarcoma (ES), three myxofibrosarcoma, two pulmonary artery sarcoma, two rhabdomyosarcoma, two fibrosarcoma, two angiosarcoma, one leiomyosarcoma and one epithelioid sarcoma. The most frequently mutated genes were TP53 (33%), ATRX (27%), CDKN2A (17%), NF1 (17%), PDGFRA (17%), CCND3 (13%) and CDKN2B (13%). Cases with TP53 alterations had lower age than wild type TP53 (35 vs. 62 years, respectively, P=0.0035). The median TMB of total patients was 1.5 muts/Mb. Twelve (40%) patients harbored at least one actionable genomic alteration, including ALK, PDGFB, ROS1 rearrangement, VEGFA, PDGFRA, KDR,KIT, FGFR1, FGFR3, CDK4 amplification and CDKN2A,CDKN2B deletion. Here, we identified two patients with pulmonary IMT harboring TFG-ROS1 fusion, who achieved continuous remission following treatment with crizotinib (Figure 1). Our study revealed that approximately 40% of patients with thoracic sarcoma harbored at least one druggable genomic alteration. A broader molecular testing, such as NGS, is necessary to explore the comprehensive genetic characteristics and potential drug targets for patients with thoracic sarcoma.

Inflammatory myofibroblastic tumor of the lung: histopathology and imaging findings

Inflammatory myofibroblastic tumor is a neoplasia of unknown clinical etiology that clinically and radiologically tends to mimic a malignant neoplasm. The tumor is histologically composed of a mixture of inflammatory cells, myofibroblastic spindle cells and plasma cells. It can occur in any part of the body. Although pulmonary inflammatory myofibroblastic tumor is the most common primary lung mass especially in childhood, this entity is usually not considered in differential diagnosis of lung nodules or masses. In this article, we aimed to disscus the pathology and imaging findings of the pulmonary inflammatory myofibroblastic tumor and increase the familiarity of radiologists and clinicians to this entity.

Clinical characteristics and prognosis of pulmonary inflammatory myofibroblastic tumor: An over 10-year retrospective analysis.

ImportancePulmonary inflammatory myofibroblastic tumors (PIMTs) are primary lung tumors in children. Misdiagnosis easily occurs because of the nonspecific clinical manifestations, laboratory examination results, and imaging findings in affected patients.ObjectiveTo summarize the clinical characteristics, diagnosis, and prognosis of children with PIMTs.MethodsThis retrospective analysis included 23 children with PIMTs who received treatment in our hospital from January 2008 to January 2019. The clinical manifestations, laboratory examination results, and computed tomography findings were retrospectively analyzed.ResultsThe population included 13 boys and 10 girls, with a median age at onset of 78 months (range, 10–126 months). Fourteen patients had PIMT lesions in the right lung and nine patients had PIMT lesions in the left lung. The surgical procedures included pulmonary wedge resection, pulmonary lobectomy, and total pneumonectomy. The median operation time was 115 min (range, 45–235 min); the median intraoperative blood loss volume was 30 mL (range, 3–500 mL). During the operation, one patient each had pulmonary hemorrhage, vena cava hemorrhage, and thoracic duct injury. Postoperative complications included pulmonary embolism in one patient and tumor recurrence in two patients; neither of these complications recurred after reoperation. The median follow‐up period was 49 months (range, 2–127 months).InterpretationChildren with PIMTs exhibited good prognoses and primarily underwent surgical resection as treatment. Complete preoperative evaluation should be performed in affected patients.

MWA is a curative alternative approach used to destroy malignant tumor cells with minimal normal tissue damage. This instrument uses high frequency microwave energy waves that produce tissue-heating effects to destruct solid tumors. The purpose of this case report is to illustrate how MWA effectively managed four various cases of lung cancer.

Inflammatory myofibroblastic tumor of the lung: presentations of an uncommon finding

Inflammatory myofibroblastic tumor (IMT) is a rare occurrence most commonly affecting the pediatric population. It is considered a diagnostic challenge because of nonspecific clinical symptoms and radiologic presentations. IMT is a soft-tissue tumor that can occur throughout the body but is most commonly found in the lung. This article discusses the case of a 10-year-old girl who presented to our hospital with abnormal chest radiograph. Imaging revealed a heterogenous mass in the superior aspect of the left lower lobe. It was found incidentally when the patient presented to the emergency department. Surgical resection proved a diagnostic and therapeutic measure confirming diagnosis of IMT. The tumor has potential to cause illness either by its rare malignant potential or by mass effect. Because lesions are often asymptomatic with a predilection for pediatric patients, cases describing appropriate management are essential. IMT is a rare tumor of borderline malignant potential. IMT of the lung may invade hilar structures, the mediastinum, and the pleura. Lesions range in diameter from 1.5 to 14 cm. Etiology of the tumor is unclear but may include immune response to infection, trauma, or surgery. Affected patients may present with cough and shortness of breath but are often asymptomatic. Histology is unique but not specific. Immunohistochemistry can differentiate the tumor, by staining for overexpression of the anaplastic lymphoma kinase oncogene. In conclusion, surgical resection is the preferred diagnostic and therapeutic regimen for IMT. The majority of IMTs follow a benign course, but exceptions qualify as low-grade malignancies.
