Changes in Cerebral Hemodynamics During Systemic Pulmonary Shunt and Pulmonary Artery Banding in Infants with Congenital Heart Disease.

Palliative surgery is often performed in the treatment of congenital heart disease. Two representative palliative procedures are the systemic pulmonary shunt and pulmonary artery banding. Dramatic changes in cerebral hemodynamics may occur in these operations due to changes in the pulmonary-to-systemic blood flow ratio and systemic oxygenation. However, there seem to be almost no studies evaluating them. Accordingly, we evaluated cerebral perfusion by transcranial Doppler ultrasonography and cerebral oxygenation by near infrared spectroscopy during these procedures. In the post hoc analysis of a previous prospective observational study, cerebral blood flow velocities of the middle cerebral artery measured by transcranial Doppler were compared between the start and end of surgery as were the pulsatility index and resistance index. The cerebral oxygenation values were also compared between the start and end of surgery. Twenty-two infants with systemic pulmonary shunt and 20 infants with pulmonary artery banding were evaluated. There were no significant differences of the flow velocities between the start and end of surgery in either procedure. The pulsatility index significantly increased after pulmonary artery banding, which may compete with the increase in cerebral perfusion due to the increase in systemic blood flow. The cerebral oxygenation decreased in both procedures, possibly due to an increase in body temperature. Arterial oxygen saturation was almost the same before and after both procedures. Contrary to our expectation, the changes in cerebral hemodynamics in the palliative operations were small if the management of physiological indices such as arterial oxygen saturation was properly performed during the procedures.

Pulmonary Hypoplasia Resulting from Pulmonary Artery Banding in Infancy: A Neonatal Rat Model Study.

The aim of this study was to establish a neonatal rat model of decreased pulmonary blood flow (PBF) for studying pulmonary pathophysiological changes in newborn lung development with reduced PBF. Horizontal thoracotomy surgery with banding of the main pulmonary artery (PA) was performed on 30 rats in the PA banding (PAB) group and without banding on another 30 rats in the sham group within 6 h after birth. The body growth and mortality were recorded. Constriction of PA was checked by echocardiography on postnatal day 7 (P7). Lung morphology was assessed with computed tomography scanning and three-dimensional reconstruction. Histological differences of two groups were evaluated using hematoxylin and eosin (H&E) staining, Masson's trichrome staining, TdT-mediated dUTP nick-end labeling assay, and CD31 labeling with microscopic examination. PA ultrasound confirmed the establishment of constriction on P7. Relative to the sham group, the neonates' physical growth, survival fraction, and lung geometry volume were decreased in the PAB group over time (p < 0.05). Histologic appearance with reduced PBF characterized a markedly simplified alveolarization with noted lower radial alveolar count and alveolar septal thickness in the PAB group (p < 0.0001), pulmonary arteries with thinner/uneven membranous layers and smaller lumina. The deficient alveolar capillary bed, enhanced pulmonary collagen deposition, and increased apoptotic alveolar epithelium were significant in the PAB group compared to the sham group (p < 0.0001). A neonatal rat PAB model demonstrated that PBF reduction during early infancy impairs alveolarization and pulmonary microvasculature.

Pulmonary artery banding in infants and young children with left ventricular dilated cardiomyopathy: A novel therapeutic strategy before heart transplantation

Dilated cardiomyopathy (DCM) in childhood has a considerable morbidity and mortality and high incidence of heart transplantation. Pulmonary artery banding (PAB) has been proposed in patients with corrected transposition of the great arteries to retrain the sub-pulmonic left ventricle (LV) and to improve a failing sub-aortic right ventricle. We evaluated the short-term and medium-term effects of PAB in young patients with LVDCM. A retrospective single-center observational study was performed to evaluate the possible benefits of a dilatable surgical PAB in infants and young children with LVDCM. Reported are 12 patients (10 infants, 2 toddlers) with LVDCM referred for heart transplant who received a surgical PAB. There were no hospital deaths. Clinical functional status improved in all patients. The pressure gradient across the PAB increased within 20 days from 28 ± 7 to 43 ± 15 mm Hg. The LV ejection fraction increased from 14.5% ± 5% pre-PAB to 27% ± 13% at hospital discharge and to 47% ± 10% at 3 to 6 months. The LV end-diastolic diameter (z-score) decreased (p > 0.001) from 46 ± 6.1 (+7.0 ± 1.3) to 35 ± 15 mm (+3.0 ± 1.3) after 3 to 6 months and to 34 ± 15 mm (+1.3 ± 1.14) after a median age of 2 years (maximum 6.6 years), respectively. Plasma B-type natriuretic peptide levels decreased from 3431 ± 2610 to 288 ± 321 pg/ml at discharge and to 102 ± 96 pg/ml 22 months later. Eight children were subsequently de-banded by transcatheter technique and 6 of them are currently at Ross Heart Failure Classification for Children class I. Two patients, both with non-compaction DCM, deteriorated at 5 and 6 months after PAB debanding and finally died. In young children with LVDCM and still-preserved right ventricular function, PAB led to an improvement of LV and mitral valve function by ventricular interaction.

Complete atrioventricular septal defect: Outcome of pulmonary artery banding improved by adjustable device

We sought to evaluate pulmonary artery banding in infants with complete atrioventricular septal defects. From 2000 to 2009, 20 infants with complete atrioventricular septal defects underwent pulmonary artery banding because of unsuitable anatomy (unbalanced ventricles, associated lesions, or both) or clinical condition (infection, chronic lung disease, or noncardiac malformation). Patients were divided into 2 groups: the conventional PAB group (n = 13 [65%]; mean age, 74 ± 56 days [range, 6-187 days]; mean weight, 3.3 ± 1.1 kg [range, 2.1-5.8 kg]) and the FloWatch-PAB group (n = 7 [35%]; mean age, 111 ± 40 days [range, 81-187 days]; mean weight, 4.3 ± 1.2 kg [range, 3.2-6.1 kg]). There was no statistical difference in age or weight. Preoperative mechanical ventilation was required in 3 (23%) of 13 infants in the conventional PAB group and 5 (71%) of 7 infants in the FloWatch-PAB group (P < .05). Ten (77%) of 13 infants in the conventional PAB group died versus 0 (0%) of 7 infants in the FloWatch-PAB group (P < .001). Sternal closure was delayed in 6 (46%) of 13 infants in the conventional PAB group and 0 (0%) of 7 infants in the FloWatch-PAB group (P < .05). The mean duration of mechanical ventilation, intensive care unit stay, and hospital stay was significantly longer (P < .05) in the conventional PAB group than in the FloWatch-PAB group (21 ± 17 days [range, 4-61 days] vs 3 ± 2 days [range, 1-8 days], 22 ± 18 days [range, 5-61 days] vs 7 ± 6 days [range, 2-21 days], and 54 ± 12 days [range, 40-71 days] vs 29 ± 25 days [range, 9-81 days], respectively). Left atrioventricular valve regurgitation increased (mild to moderate) in 2 infants in the conventional PAB group and decreased (severe to moderate) in 2 infants in the FloWatch-PAB group. Six of 10 survivors (1 in the conventional PAB group and 5 in the FloWatch-PAB group) underwent pulmonary artery debanding and repair after a median interval of 125 days (range, 34-871 days); 4 of 10 are awaiting repair. In selected patients with complete atrioventricular septal defects, pulmonary artery banding followed by late repair is a viable alternative strategy. In our study the FloWatch-PAB device resulted in improved survival and made later repair possible in a better clinical state.

A Blue Baby Grows Up

This report describes the history of a 41-year-old female patient born with atresia of the right atrioventricular valve, hypoplasia of the right ventricle, an atrial septal defect, and malposed great arteries. The patient underwent surgical pulmonary artery banding in infancy but did not undergo corrective surgery due to the development of pulmonary arterial hypertension. This report touches on several important clinical issues that may arise when caring for the adult with complex palliated cyanotic congenital heart disease, including the development and treatment of pulmonary arterial hypertension.

Dilatable pulmonary artery banding in infants with low birth weight or complex congenital heart disease allows avoidance or postponement of subsequent surgery☆

This study evaluated the efficiency and feasibility of dilatable bands in selected patients. Two types of dilatable handmade bands were retrospectively evaluated and divided into two groups: main pulmonary artery bands and bilateral branch pulmonary artery bands (hybrid stage I palliation). Stepwise balloon angioplasty (BA) was performed, increasing the diameter either to completely dilate with total release of the band, or in others, to partially dilate the bands in order to improve flow and/or saturation. Balloon angioplasty was performed in 20 patients (median birth weight 2.9 kg, range: 1.3-4.5 kg). Main pulmonary artery: Partial dilation: Six patients: Large ventricular septal defects (VSDs) and complex lesions requiring additional surgery. Progressive dilation allowed postponement of surgery in four children and allowed percutaneous VSD closure in one. Complete dilation: Eight patients: Spontaneous restriction of VSDs occurred in six patients; the bands were subsequently percutaneously completely released after a median of 39 weeks (7-91 weeks). The median gradient decreased from 90 to 38 mmHg (p<0.0001). Bilateral branch pulmonary artery: An average 8.5% increase in saturations was achieved in five patients, and in one patient, a hybrid procedure with borderline left ventricle, complete dilation allowed successful percutaneous biventricular repair. Dilation of both main and bilateral branch pulmonary artery bands is possible, effective and safe. Dilatable main pulmonary artery bands allow for progressive dilation with postponement of surgery or complete release of the bands. Bilateral dilatable branch pulmonary bands offer palliative benefit, especially in hybrid cases where pulmonary blood flow may be limited by the bands before the ideal conditions for a stage II procedure exist.

Banding of the pulmonary artery (PAB) in preparation for arterial switch operation (ASO) in patients with transposition of the great arteries (TGA) represents a unique model of acute left ventricular pressure overload in humans. To establish the rate, magnitude, and determinants of left ventricular hypertrophy and the acute effect on ventricular function, serial bidimensional echocardiographic evaluations were performed on 26 patients with TGA after PAB. Mass, volume, and ejection fraction of the left ventricle were measured. Cardiac catheterization data before PAB and again before ASO were reviewed. The mean interval between the PAB and ASO was 9 +/- 4 days. The left ventricular to right ventricular pressure ratio before PAB was 0.5 and increased to 1.0 before ASO. The mean percentage increase in left ventricular mass from PAB to ASO was 96%, 95% of which was achieved in the first 7 days. The average rate of left ventricular hypertrophy for the entire period was 0.06 g/h and was 0.19 g/h during the interval from PAB to attainment of maximum left ventricular mass. The most rapid rate of hypertrophy was seen by day 2, with an exponential fall in the growth rate thereafter approaching zero by day 7. Ejection fraction was significantly reduced at 12 hours after PAB, but mean values returned to pre-PAB levels by 3.5 days after banding. The absolute rate of left ventricular hypertrophy correlated directly with body surface area but not to other hemodynamic variables. Doubling of left ventricular mass can be achieved in 1 week after PAB. Function falls acutely due to afterload excess and/or depressed contractility but recovers rapidly as compensatory hypertrophy occurs.

The relevance of intraoperative pressure and oxygen saturation monitoring during pulmonary artery banding in infancy

Different methods have been used to define an effective and tolerable degree of pulmonary artery banding (PAB) intraoperatively. As the hemodynamic response determines the results, a reduction of risk could be expected by evaluating several selected parameters and adjusting the band accordingly. Between 1981 and 1986, PAB was performed in 12 infants, three with complex cardiac lesions and nine with complete atrioventricular canal defects. Pressures in the distal pulmonary artery, in a systemic artery or the aorta, and in a central vein were monitored, and oxygen saturation measurements in both great arteries were taken before, during, and after the procedure. Adequate, complicated, intolerable, and contraindicated constriction were recognized and shown on characteristic pressure traces and oxygen saturation shifts. The measurements provided an indirect but reliable indication of the change in intracardiac shunting, and predicted whether the banding would be hemodynamically tolerated. Postoperatively, ten of the infants improved significantly, especially the seven infants previously in a low cardiac output state. One infant died on the day of surgery from acute right heart failure (8% hospital mortality). The results suggest that hemodynamically controlled PAB is an effective palliative treatment with a lower risk than had been previously reported.

Surgical Management of Univentricular Heart with Subaortic Obstruction

In children with a univentricular heart and a rudimentary subaortic chamber, surgical relief of subaortic obstruction caused by a restrictive bulboventricular foramen is associated with high morbidity and mortality. A 6-year-old child with a univentricular heart of the left ventricular type, a rudimentary subaortic chamber, and atresia of the left-sided atrioventricular valve had pulmonary artery banding in infancy. Severe subaortic obstruction subsequently developed. At operation, the pulmonary artery was transected and the stump was anastomosed directly to the posterior aspect of the ascending aorta, diverting left ventricular blood into the aorta through the pulmonary valve. The distal pulmonary artery was anastomosed side-to-side to the ascending aorta to provide pulmonary blood flow. Cardiac catheterization fifteen months after the operation demonstrated an excellent hemodynamic result. When the pulmonary artery is adequate in size, a bypass operation by way of an anastomosis between the ascending aorta and the pulmonary artery is a relatively safe and effective means of relieving the ventricular outflow obstruction caused by a restrictive bulboventricular foramen.

A surgical approach for S,L,L single ventricle incorporating total right atrium-pulmonary artery diversion

Patients with single ventricle and unfavorable anatomy for partitioning procedures have been considered inoperable. Such was the case in two patients with single ventricle and subaortic inverted (hypoplastic) infundibulum associated with subaortic obstruction and levo-transposition of the great vessels (S,L,L). Both had undergone pulmonary artery banding in infancy and presented 11 and 14 years later, cyanotic and severely incapacitated. Both patients were treated surgically by resection of the subaortic obstruction, patch closure of the right artrioventricular valve, closure of the proximal pulmonary artery, and diversion of systemic venous flow through a xenograft valved conduit from the right atrium to the distal pulmonary artery. Early postoperative complications included atrial dysrhythmias and right-sided congestion. The first patient, who had massive cardiomegaly and complete heart block preoperatively, developed congestive failure 20 months postoperatively. The second patient has an excellent functional result and is leading a normal life 1 years after operation. This procedure may be of value for patients with single ventricle in whom corrective ventricular septation is considered too risky or technically unfeasible.

Pulmonary artery banding in infants with complete atrioventricular canal

Management of symptomatic atrioventricular canal (AVC) in infancy may be difficult. Between July, 1969, and September, 1977, 31 infants with complete AVC presented in congestive heart failure (CHF) to the University of Minnesota Hospitals. Fifteen of these patients have responded to medical management and have been followed as outpatients. The other 16 patients remained in CHF. Six of them died of persistent heart failure within 4 months. The other 10 infants, aged 3 weeks to 1 year (mean 4 months), underwent pulmonary artery banding and seven survived operation. One of the survivors died with apparent pneumonia 1 month postoperatively. Each of the remaining six patients, who have been followed for 9 months to 9 years, had minimal mitral insufficiency and a large ventricular shunt. The three patients dying after banding had significant mitral insufficiency. We believe that pulmonary artery banding is an effective palliative procedure for infants with complete AVC and CHF who have large ventricular shunts and minimal mitral insufficiency.

Pulmonary Artery Banding in Infants: A Physiological Intraoperative Method of Determining the Effectiveness of the Procedure

A physiological method has been devised to evaluate the degree of intraoperative pulmonary artery constriction during operations for the palliation of infants with severe congestive heart failure. This approach depends on precisely determining the degree of shunting between the right and left sides of the heart by serial determinations of arterial oxygen saturation. Two patients are cited who illustrate the advantages of this technique.

Ventricular septal defect with increased pulmonary vascular resistance: Late results of surgical closure

The fate of 57 children who underwent surgical closure of ventricular septal defect in the presence of increased pulmonary vascular resistance was analyzed. Eighteen (32 percent) died at or immediately after operation. Seven late deaths, due to the Eisenmenger syndrome, occurred 1 to 7 years after operation. Follow-up studies, including cardiac catheterization, were performed in 25 survivors, 1 to 11 years (mean 5 years) after operation. In patients operated on after age 2 years, the degree of preoperative elevation of pulmonary vascular resistance largely determined the prognosis: The condition of patients with mild elevation (pulmonary vascular resistance less than one third the systemic level) returned to normal after operation, with one exception. Patients with more severe elevation (pulmonary vascular resistance more than one third the systemic level) most often showed progressive pulmonary vascular disease; the condition of a few remained unchanged and that of only one returned to normal after operation. Operative mortality rate was high in children with elevated pulmonary vascular resistance during the second year of life who were operated on before age 2 years, but pulmonary vascular resistance returned to normal in the three survivors. When pulmonary vascular resistance was increased in the first year of life, it generally decreased spontaneously (independently of operation); the early increase in these cases is believed to represent delayed maturation of the pulmonary vascular bed rather than pulmonary vascular disease. The overall results of surgery in ventricular septal defect associated with a high level of pulmonary vascular resistance are unfavorable and call for interruption of the natural history before significant pulmonary vascular obstructive changes have taken place; this can be achieved by surgical closure before age 2 years or pulmonary artery banding in infancy.

Pulmonary Artery Banding in Infants with Complex Congenital Heart Defects

Pulmonary artery banding was performed in 60 patients with complex congenital heart defects, other than isolated ventricular septal defect, for treatment of high-output cardiac failure with a large left-to-right shunt and pulmonary hypertension. The average age at banding was 3.3 months. Half of the patients also had ligation of a patent ductus arteriosus, resection of coarctation, or creation of an atrial septal defect at the time of banding. Forty-one of the group survived banding; there was a higher survival rate in the patients 3 months of age or older. Twelve patients had catheterization studies performed after banding that demonstrated reduction in pulmonary artery pressure, decrease in the left-to-right shunt, no increase in pulmonary vascular resistance, and significant reduction in left ventricular end-diastolic volume and mass. Eight patients subsequently underwent additional palliative procedures with 7 survivors. Nine patients have had corrective operations performed, and 7 have survived. Pulmonary artery banding is an effective procedure in the staged treatment of infants with complex congenital heart defects.

Total Correction of Ventricular Septal Defect in Infancy Using Extracorporeal Circulation: Surgical Considerations and Results of Operation

Between 1955 and 1970, 143 Mayo Clinic patients less than 2 years old (78 boys and 65 girls; ages 10 weeks to 24 months, median age 14 months) underwent total correction of a ventricular septal defect using extracorporeal circulation. The distribution of associated lesions was as follows: 66% had no other lesions, 11% had an atrial septal defect, 9% had pulmonary stenosis, 8% had a patent ductus arteriosus, and 6% had other lesions. Two patients were asymptomatic; clinical features in the remainder included congestive heart failure (76%), failure to thrive (55%), and repeated respiratory infections (32%). Operative mortality was inversely related to age and weight. Neither of the asymptomatic patients died. The overall mortality was 21% for the entire interval; for the most recent five-year period, the rate was 10% (22% for infants less than 12 months and 0% for those 12 to 24 months). These data compare favorably with our previous experience and that of others for palliative pulmonary artery banding in infancy followed by correction at a later age, and they support our current approach of closure of the ventricular septal defect using extracorporeal circulation in almost all infants who require surgical aid.

Physiologic correction of transposition of the great arteries. Indications for and results of operation in 32 patients.

The total management of a group of 32 patients with transposition of the great arteries (TGA) is reported. All but one had required creation of an atrial septal defect in infancy. Eighteen patients had no associated cardiac defects, and physiologic correction was performed between 8 months and 9½ years of age. The only death in this group occurred 9 months after operation. Eight of nine patients with TGA and an associated ventricular septal defect (VSD) had required pulmonary artery banding in infancy. Physiologic correction was performed between 2¼ and 6 years of age. Three of these patients died during the postoperative period, and one died 9 months after operation. Two patients had TGA and pulmonary stenosis (PS). Physiologic correction was performed at 2 1/3 and 3¾ years of age. Both patients have done well. Three patients had TGA, VSD, and PS. Total correction was performed between 2 1/3 and 3½ years of age. All three patients died in the postoperative period. The over-all operative mortality was 19%. Our present plan of management has evolved from the experience reported above. All infants with TGA have balloon atrioseptostomy. If no VSD is present, physiologic correction is performed between 1 and 2 years of age. Patients with TGA and a VSD are catheterized at 8 months of age, and pulmonary banding is performed if pulmonary artery hypertension is present. Physiologic correction is performed at about 4 years of age. Patients with TGA, VSD, and PS may require systemicpulmonary artery anastomosis. Correction at 5 years of age with the use of a homograft from the right ventricle to the pulmonary artery, as described by Rastelli and his associates, is recommended because of the poor results obtained with an intra-atrial baffle in this group.

Surgery for transposition of the great arteries without intact ventricular septum.

In transposition of the great arteries without intact ventricular septum, the ventricular configuration is one factor limiting total intracardiac repair. In six patients with uncorrectable ventricular configuration and pulmonic stenosis, a superior vena cava-pulmonary artery anastomosis was done without mortality. Pulmonary vascular disease, another limiting factor, was found in four patients who failed to survive an intraventricular repair combined with a Mustard atrial septoplasty. There was one survivor. Two 8-year-old children who had pulmonary artery banding in infancy survived total intracardiac repair combined with removal of the band. The data and experience lead to the opinion that an optimal pulmonary artery banding in infancy followed by a total repair in the latter part of the first decade warrants an adequate clinical trial.

The spectrum of pulmonary vascular disease in transposition of the great arteries.

Obstructive pulmonary vascular disease (PVD) was evaluated in lung sections taken at autopsy in 53 patients with complete transposition of the great arteries. Twentythree had intact ventricular septa and 30 had ventricular septal defects. Children of similar age and associated defects had widely differing degrees of PVD and presented a spectrum of disease. In a small number of patients, the presence of pulmonic stenosis (congenital or acquired by pulmonary artery banding in infancy), a small ventricular septal defect, or a large atrial septal defect was not protective against the development of severe PVD. The findings of this study demonstrate the need for thorough hemodynamic evaluation of patients with complete transposition of the great arteries prior to repair by present technics. In addition, operation should probably be performed as soon as technically feasible.
