Psychosocial Status and Self-Perception in Patients with Cleft Lip and/or Palate.

To describe how the psychosocial status of patients with cleft lip and/or palate (CL/P) relates to patient-reported outcomes (PROs). Cross-sectional retrospective chart review. Tertiary care pediatric hospital. Patients aged 8 to 29 years attending cleft team evaluations during a 1-year period. CLEFT-Q. Patients (N = 158) with isolated or syndromic CL/P and mean age 13.4 ± 3.0 years were included. Fifteen (9%) patients had siblings who also had CL/P. Of 104 patients who met with the team psychologist, psychosocial concerns were identified in 49 (47%) patients, including 25 (24%) with Attention-Deficit/Hyperactivity Disorder or behavior concerns, 28 (27%) with anxiety, and 14 (13%) with depression or mood concerns. Younger age and having siblings with cleft were associated with better PROs, while psychosocial concerns were associated with worse PROs on Speech, Psychosocial, and Face Appearance scales. Patient perception of cleft outcomes is linked to psychosocial factors.

The features of clinical and anamnestic indicators, cognitive function, quality of life, and psychological status in patients with various stages of chronic lower extremity ischemia

Objective. To estimate the frequency of risk factors and the features of the psychosocial status in patients with atherosclerosis of the lower extremity arteries. Subjects and methods. The investigation prospectively included 198 patients with atherosclerosis of the lower extremity arteries. The patients’ mean age was 67.9±8.0 years. Data copying, questioning, instrumental and analytical methods were used to study the patients’ clinical and anamnestic indicators. The survey was conducted using the SF-36 questionnaire and the SpielbergerKhanin test, Mini-Mental State Examination (MMSE), and Morisky-Green scales. Depending on the stage of ischemia, according to the Pokrovsky-Fontaine classification, the investigators formed two investigation groups: 1) 127 patients with Stage 0–IIA ischemia; 2) 71 patients with Stage IIB-IV ischemia. Results. There was a male preponderance in both groups: 106 men in Group 1 (p=0.03) and 52 in Group 2 (p=0.026). The number of smokers was statistically significantly higher in Group 2 than in Group 1 (37.0 and 49.0%, respectively; p=0.05). No statistically significant differences in diabetes were found between the groups. Systolic blood pressure was also higher in Group 2 than in Group 1 (136.3±15.3 and 130.7±12.9 mm Hg, respectively; p=0.0015). Analysis of physical (p = 0.00019) and psychosocial (p=0.0415) states revealed lower rates in Group 2. Conclusion. The findings suggest that Group 2 patients had more severe clinical, physical, and psychosocial conditions, which indicates the need to attract a psychologist for patients with more severe ischemia, as well as that to more carefully monitor hemodynamic parameters and to correct modifiable risk factors in both groups.

Psychosocial well-being and mental adjustment of patients with stage III melanoma in the re-entry phase: a scoping review

Abstract Background: The number of patients with stage III melanoma is rising because of increasing incidence and improved survival thanks to newly available adjuvant treatments. Patients may face specific challenges in the re-entry phase, the first 18 months after treatment, because of a high risk of recurrence and the intensity and long-term side effects of adjuvant treatment. The aims of this scoping review included summarizing the current state of evidence concerning patient psychosocial well-being and mental adjustment to the re-entry phase, as well as identifying knowledge gaps. Methods: A systematic literature search was conducted in PubMed and CINAHL using the PRISMA-ScR checklist. Results: Two qualitative and seven quantitative studies were included. Some studies found no evidence of diminished social and emotional functioning compared with the general population, while others reported emotional distress and impairment in psychological health. Regarding existential health, frequently expressed concerns encompassed fear of recurrence, hopelessness, and uncertainty about the future. Patients typically used behavioral coping, cognitive coping, and meaning-making to manage challenges in the re-entry phase. Only one study considered the effects of adjuvant therapy use on the re-entry phase. Conclusions: Limited evidence is available on the psychosocial status of patients with stage III melanoma in the re-entry phase, a phase in which patients face existential concerns. It may be beneficial to provide patients with support regarding existential concerns and meaning-making, for example by developing a practical tool to help set meaningful goals for the re-entry phase. A better understanding of the psychosocial well-being of these patients, the impact of adjuvant therapy, and the relationship between emotional distress, existential concerns, and meaning-making will require further research.

Burden of seizures and comorbidities in patients with epilepsy: a survey based on the tertiary hospital-based Epilepsy Syndrome Registry in Japan.

To examine the current medical and psychosocial status of patients with epilepsy, aiming to facilitate appropriate application of the Intractable/Rare Diseases Act of Japan. By analysing the cross-sectional data of patients registered in the tertiary hospital-based Epilepsy Syndrome Registry of Japan, we investigated the proportion of patients who met the severity criteria as defined by the Act (seizure frequency of at least once a month, or presence of intellectual/neurological/psychiatric symptoms, or both) and whether there are candidate syndrome/diseases to be added to the existing list in the Act. In total, 2,209 patients were registered. After excluding self-limited/idiopathic epilepsies, 1,851 of 2,110 patients (87.7%) met the severity criteria. The patients were classified into eight main epilepsy syndromes (594 patients), 20 groups based on aetiology (1,078 patients), and three groups without known aetiology (427 patients). Most of the groups classified by syndrome or aetiology had high proportions of patients satisfying the severity criteria (>90%), but some groups had relatively low proportions (<80%) resulting from favourable outcome of surgical therapy. Several small groups with known syndrome/aetiology await detailed analysis based on a sufficiently large enough number of patients registered, some of whom may potentially be added to the list of the Act. The registry provides data to examine the usefulness of the severity criteria and list of diseases that are operationally defined by the Act. Most epilepsy patients with various syndromes/diseases and aetiology groups are covered by the Act but some are not, and the list of designated syndromes/diseases should be complemented by further amendments, as suggested by future research.

Analysis of Real-World Implementation of the Biopsychosocial Approach to Healthcare: Evidence From a Combination of Qualitative and Quantitative Methods.

Aims: The modern medical model has been transformed into a biopsychosocial model. The integration of the biopsychosocial approach in healthcare can help improve the effectiveness of diagnosis and treatment. This study explored the actual application of the biopsychosocial approach in healthcare and provides a basis for targeted interventions to promote the biopsychosocial approach in healthcare.Methods: Study 1 involved one-on-one interviews with 30 medical staff and focus group interviews with 16 recent patients. Study 2 was a cross-sectional survey of 13,105 medical staff in Hangzhou, China that analyzed the status quo implementation of the biopsychosocial approach in healthcare.Results: Study 1 found that medical staff did not welcome patients to report information unrelated to their disease, hoping patients did not express their emotions. In the treatment process, patients believed that medical staff refused to attend to or did not encourage reporting of any information other than the disease, and that patients should have reasonable expectations for medical staff. Study 2 found that medical staff had a 37.5% probability of actively paying attention to the patient's psychosocial status. Female medical staff (38.5%) were actively concerned about the patient's psychosocial status significantly more than male medical staff (34.2%) (P < 0.01). The medical staff in the psychiatric department (58.4%) paid more active attention to the patient's psychosocial status than staff in the non-psychiatric departments (37.2%). Gender, department, hospital level, and professional title were the factors associated with the medical staff's attention to the patient's psychosocial status (P < 0.05). The influence of age on the probability of medical staff actively paying attention to the psychosocial status of patients increased with the number of years of employment. Participants that were 31–40 years old, had an intermediate professional title, and 11–15 years of employment were the least likely to actively pay attention to patients' psychosocial status.Conclusion: Although the biopsychosocial approach has been popularized for many years, it has not been widely used in medical care. Medical staff pay more attention to patients' physical symptoms and less attention to patients' psychosocial status. It is recommended that training will be provided to medical personnel on implementing a biopsychosocial approach with particular attention to the sociodemographic characteristics of medical personnel. Additionally, we propose helping patients set reasonable expectations, and formulating guidelines for implementing the biopsychosocial approach.

Psychosocial status of patients with unilateral and bilateral microtia before auricular reconstruction surgery

ObjectiveMicrotia can profoundly influence health-related quality of life. The aim of this study was to investigate the psychosocial status of children and adolescents with unilateral and bilateral microtia before reconstruction surgery. MethodsEighty-six patients with microtia from Peking Union Medical College Hospital (54 with unilateral microtia and 32 with bilateral microtia) responded to structured interviews before reconstruction surgery. The 3 clinically validated questionnaires were the Piers-Harris Children's Self-concept Scale (PHCSS), the Social Anxiety Scale for Children (SASC), and the Children's Loneliness Scale (CLS). Scores from the PHCSS and SASC were compared with Chinese norms. The total scores of the unilateral and bilateral groups were compared for all three questionnaires. ResultsMales with unilateral and bilateral microtia had significantly different PHSCC scores than norms (both P < 0.05), and male patients overall had a total score that was 10.61 ± 2.20 points lower than female patients overall. Patients who were 7–9 years-old in the unilateral group and 10 to 13 years-old in the bilateral group had lower total PHSCC scores than norms of the same age (both P < 0.05). There was no significant difference between patients and norms in the 14-16 years-old group. The SASC scores of patients were significantly higher than the norms (P < 0.05). The CLS score was significantly lower in the unilateral group than in the bilateral group (50.17 ± 14.63 vs. 61.38 ± 13.24, P < 0.05), but these two groups did not significantly differ in PHCSS and SASC scores. ConclusionCompared to Chinese norms, children with unilateral and bilateral microtia had lower self-concept, especially males and those who were 7–13 years-old. Patients with microtia also had increased social anxiety and loneliness. Patients with bilateral microtia were more likely to report loneliness than those with unilateral microtia.

Mutual Relationship Between Sleep Disorders, Quality of Life and Psychosocial Aspects in Patients With Psoriasis.

Psoriasis is a chronic, autoimmune skin disease affecting about 2–4% of the worldwide population. It is now perceived as a systemic disease because of the complex pathogenesis and multiple comorbidities. It leads to decreased quality of life and productivity of patients. Nowadays, sleep disorders are investigated as well in relation to psoriasis as another possible comorbidity. This review focuses on possible negative effects of sleep deprivation, decreased quality of life, and psychosocial status in patients with psoriasis and highlights their mutual, complex relationship of divergent consequences. The relationship between sleep disorders and psychosocial status in patients with psoriasis is bidirectional and resembles a vicious circle, one abnormality triggering the other. Sleep disorders additionally increase the risk of metabolic and psychiatric diseases in psoriatic patients who are already at increased risk of developing such disorders. There should be measures taken to screen patients with psoriasis for sleep disorders in order to diagnose early and treat.

Physical disabilities and psychosocial status of the patients attending in a selected specialized hospital

This study was conducted at CRP Saver Dhaka, with a view to described the various factors that affect the physical disabilities and psychosocial status of the disabled patients, from January to December, 2015. The sample size was 150 and the data were collected by using purposive sampling technique with face-to-face interview. The study revealed that, counterpart (42%). Rural area was found to have highest (56.7%) of Physical disabilities than urban areas (43.3%). Majority of respondents were come from middle and low income group. Neuromuscular disability found higher numbered (67%) than that of musculo skeletal disability (33%). Regarding reasons of disability stroke (25.3%) was in highest position followed by fall from height (21.39%), RTA (16%) and occupational (5.30%) respectively. Physical disability was one of the major causes of psychological problem. Physical disability hamper the social status, (10.7%) of the respondents were lost their jobs due to disabilities. Study found the significance difference between the physical disabilities and interruption of community involvement (χ2 =34.162, p=&lt;0.05). Studies regarding psychosocial aspect among the disabled are rare. This study might be improving the understanding of psychosocial problem of the physical disabled and thereby strengthening the design of more effective identification, prevention and intervention strategies.&#x0D; Asian J. Subjective Assessment of Satisfaction Degree and Psychosocial Status of Patients with Highly Atrophic Mandible Treated by Implant-retained Over-denture

This study aimed at assessing patient’s satisfaction with new optimized conventional complete upper andlower dentures as compared to their previous problematic conventional dentures. A random sample ofpatients was selected (group 1). Each patient was provided with new optimized conventional upper and lowerdentures (group 2), then a subset who were not satisfied with the optimized lower prostheses were providedwith two implant-retained lower over-dentures (group 3). Self-assessment questionnaires were voluntarilyfilled by the participants. Male-female differences were assessed using the two independent samples t-test.Inter-group differences were evaluated using the two dependent samples t-test. Significant differences werefound between sexes in group 2, but not in group 3, in the direction of females being less satisfied than malesespecially with the lower denture, and between the groups in a number of variables in the direction of moresatisfaction with the lower dentures among group 3 as compared to group 2. Meanwhile, group 2 and 3 werefound to be equally satisfied with the upper denture.

Burden of Itch in Patients with Basal Cell Carcinoma.

The incidence of basal cell carcinoma has increased in recent years. The aim of this study was to analyse the influence of itch on quality of life and psychosocial status of patients with basal cell carcinoma. A total of 180 patients with basal cell carcinoma were included in the study. Itch intensity was assessed with Numerical Rating Scale and 4-Item Itch Questionnaire (4IIQ). Quality of life in basal cell carcinoma was assessed with the 36-item Short-Form Health Survey (SF-36), Dermatology Life Quality Index (DLQI), 6 Item Stigmatization Scale (6-ISS) and Beck Depression Inventory (BDI). Mean DLQI in the study group was 2.0 ± 2.0, while mean SF-36 was 71.38 ± 15.77 points. The intensity of itch assessed with 4IIQ correlated negatively with SF-36 (R = –0.48, p = 0.003). The mean ± SD BDI score was 6.16 ± 5.72 points and mean 6-ISS score was 0.66 ± 1.24. In conclusion, presence and intensity of itch may debilitate quality of life among patients with basal cell carcinoma.

Associations Between Altered Cerebral Activity Patterns and Psychosocial Disorders in Patients With Psychogenic Erectile Dysfunction: A Mediation Analysis of fMRI.

Previous studies had illustrated the significant neural pathological changes in patients with psychogenic erectile dysfunction (pED), while few works focused on the neural underpinning of the psychosocial status in patients with pED. This study aimed to investigate the associations among the altered cerebral activity patterns, impaired erectile function, and the disrupted psychosocial status in patients with pED. Thirty-two patients with pED and 28 healthy controls (HCs) were included. The amplitude of low-frequency fluctuations (ALFF), region-of-interest-based functional connectivity (FC), as well as Pearson correlation analyses and mediation analyses between neuroimaging outcomes and clinical outcomes were performed. Compared to HCs, patients with pED manifested lower erectile function, disrupted psychosocial status, as well as decreased ALFF in the left dorsolateral prefrontal cortex (dlPFC) and reduced FC between the left dlPFC and left angular gyrus, and left posterior cingulate cortex (PCC) and precuneus, which belonged to the default mode network (DMN). Moreover, both the ALFF of the left dlPFC and FC between the left dlPFC and left PCC and precuneus were significantly correlated with the sexual function and psychosocial status in patients with pED. The disrupted psychosocial status mediated the influence of atypical FC between dlPFC and DMN on decreased erectile function. This study widened our understanding of the important role of psychosocial disorders in pathological neural changes in patients with pED.

Implications of Bed Rest for Patients with Acute Deep Vein Thrombosis: A Qualitative Study.

Background/ObjectiveThe recommendation of bed rest for deep vein thrombosis (DVT) patients has changed during the last 20 years, and it has become a concern for researchers. The existing researches on potentially harmful treatment of bed rest for DVT patients focus only on physiological outcomes. This qualitative study explored the implications of bed rest from the perspective of patients with acute DVT. Understanding these implications will provide more evidence on whether bed rest should be used as a medical treatment of acute DVT.Patients and MethodsFor data collection, a descriptive qualitative design utilizing semi-structured, in-depth, face-to-face interviews with nine patients with acute DVT was conducted. In order to find the themes and subthemes emerging from the interviews for data analysis, the Colaizzi method, which was suggested by phenomenological methodology, was used.ResultsThe four major themes found were physical effects, psychological effects, social effects, and post-trauma growth. These themes illustrated the bed rest experiences of patients and it has a negative impact on the quality of life (QOL) amidst acute DVT.ConclusionBed rest for patients with acute DVT is a physically, emotionally, and socially distressing phenomenon that simultaneously affects QOL and induces post-traumatic growth. We believe that bed rest is not beneficial to the physical and mental health of patients with acute DVT. This study adds to the available evidence on the harmful effect of bed rest as a treatment from the perspective of patients with acute DVT. Further quantitative studies should compare the quality of life and psychosocial status of patients with and without bed rest amidst acute DVT.

A Report on the Education, Employment and Marital Status of Thalassemia Patients from a Tertiary Care Center in the Middle East

Very few reports in the literature have focused on the psychosocial status of patients with thalassemia. The aim of this study was to report on the education, employment, and marital status of thalassemia patients in Lebanon and potential influencing factors. A total of 228 patients from the Chronic Care Center, Hazmieh, Lebanon, were incorporated for the data analysis. Demographic, social, and clinical variables were collected. Statistical analysis was performed using the Pearson χ2 test, Fisher Exact test, and binary logistic regression. In this sample, 54.4% were employed, and 45.6% not employed. Of those employed, 65.3% were male, 62.9% single or divorced, 77.4% splenectomized. University level was reached by 26.3% subjects, 7.9% reached high school level, and 32.5% have a level less than high school. Multivariate analysis revealed higher education was most likely attained by males [odds ratio (OR) = 2.23, 95% confidence interval (95% CI): 0.23–0.86] and those with no heart disease and no joint disease (OR = 27.5, 95% CI: 2.80–270 and OR = 3.40, 95% CI: 0.90–12.7, respectively). For employment, a lower average ferritin was associated with current employment. Neither the type of thalassemia nor transfusion status or type of chelation therapy corresponded with higher education or employment status. In conclusion, this is one of the few studies in the literature to look at education, employment, and marital status of thalassemia patients. Such information is essential to develop effective psychosocial support plans for our thalassemia patients.

P0033COMPARISON OF ANXIETY, DEPRESSION AND QUALITY OF LIFE IN PATIENTS WITH AUTOSOMAL DOMINANT POLYCYSTIC KIDNEY DISEASE: A CROSS-SECTIONAL, RANDOMIZED CONTROLLED PILOT STUDY

Abstract Background and Aims Patients with autosomal dominant polycystic kidney (ADPKD), in addition to uremic signs and symptoms, the need for renal replacement therapy and the risk of transferring the disease to their children put them under physical and mental stress. This may affect the psychosocial status of patients. In this study, we compared diabetic nephropathy (DN) patients who were expected to progress rapidly with early-stage ADPKD patients to reveal anxiety, depression and quality of life. Method In our cross-sectional, randomized controlled and single-center study, 55 adult patients with early stage ADPKD with eGFR ≥ 60 ml/min/1.73 m2 and 50 patients with DN as control group were included. Both groups were applied to state trait anxiety inventory (STAI), patient health questionnaire 9 (PHQ-9), quality of life short form 36 and Beck depression inventory in order to evaluate their depression, anxiety and quality of life. Depression, anxiety and quality of life questionnaires’ results, demographic and clinical characteristics, laboratory findings of the two groups were compared. Results Comparison of patients’ and control groups demographic and clinical characteristics, and laboratory data were shown in Table 1. According to state trait anxiety inventory; moderate and high levels of anxiety were present in 94% patients with ADPKD and in 66% patients with DN (p&amp;lt;0.001). According to Beck depression inventory; depression is detected in 58.2% patients with ADPKD and in 26% patients with DN (p=0.001). When the PHQ-9 results were compared; depression is prevalent in 55.6% patients with ADPKD and in 34.7% patients with DN (p=0.037). Although ADPKD patients were at an early stage, we found anxiety and depression more frequent and severe than DN patients. While physical life quality was similar in both group; mental quality of life was lower in ADPKD patients according to quality of life short form 36 (p= 0.32 and p= 0.03 respectively) (Table 2). Independent variables affecting Beck depression inventory score; being a woman, smoking and being diagnosed with ADPKD; independent variables affecting PHQ9 score; being female and the excess of the number of medical tablets used daily; independent variables affecting personal trait anxiety score; being a woman, smoking, being diagnosed with ADPKDD, and presence of hypertension. Conclusion In ADPKD patients, anxiety, depression and decrease in quality of life can be seen more frequently than other uremic patients even in the early period when renal functions are preserved. This group of patients should be evaluated in this respect from the early stages of the disease.

Psychosocial status and sleep quality of the patients with idiopathic intracranial hypertension

Aim: To investigate the level of depression and anxiety measures, sleep quality indexes and to measure the quality of life in patients with idiopathic intracranial hypertension (IIH).Material and Methods: Using a case-control cross-sectional design, 17 patients with IIH were compared with 16 healthy controls of similar age and gender. Patients were assessed for aggression, depression, and anxiety using the Buss-Perry Aggression Questionnaire, the Beck Depression Inventory (BDI) and the Beck Anxiety Inventory (BAI). The quality of life levels was assessed by the 36-Item Short-Form Health Survey (SF-36). The sleep quality of the individuals was evaluated using the Pittsburgh Sleep Quality Index (PSQI), STOP questionnaire, Epworth sleepiness scale (ESS). Results: The scores of BDI and the BAI were higher in the patient group, however, there were no significant differences (p=0.36, p=0.14, respectively). The results of the questionnaires related to sleep quality including PSQI, STOP questionnaire and ESS were worse in the patient group; however, no statistical differences were found (p=0.65, p=0.14, p=0.33, respectively). All of the eight domains of the SF-36 were also compared between the groups which revealed that the scores of all of the subitems were lower in the patient group. However, the statistical difference was only found in the subitem of ‘energy/fatigue’ (p = 0.02). Conclusion: Our study results provide encouraging conclusions remarking the worse psychosocial status in patients with IIH. The relevance and clinical impact of psychological status and quality of life in patients with IIH need to be investigated in future large-scale studies. Besides, the low energy/fatigue measures in IIH patients and its temporal course after the initiation of IIH treatment may constitute crucial topics to be investigated in these studies.

The quality life in patients with chronic obstructive pulmonary disease combined with arterial hypertension

Purpose of the study. Was to investigate life quality in patients with comorbidly pathology of chronic obstructive pulmonary disease in combination with arterial hypertension.&#x0D; Materials and methods. 77 subjects with a diagnosis of chronic obstructive pulmonary disease stage I–III (according to the GOLD criteria) in combination with arterial hypertension I–III stage. The comparison groups included 30 patients with essential hypertension and 45 patients with chronic obstructive pulmonary disease. All groups were completely comparable by the length and disease severity. All patients underwent general clinical examination, blood pressure measurement, spirometry, electrocardiography, and echocardiography. Study of the life quality in patients was carried out by means of self-questioning using General official Russian-language analogue of the MOS SF-36 questionnaire.&#x0D; Results. It was determined, that life quality parameters of physical status and psychosocial status were reduced in all patients, but to a greater extent in patients with comorbidity than in patients of the comparison group. Disease duration influenced on life quality. In patients with newly diagnosed chronic obstructive pulmonary disease with a history of marked increase in blood pressure, changes in life quality were more expressed in comparison with the comparison group; at the same time there was tendency of it’s increasing over time. When assessing the relationship between quality of life and parameters of pulmonary ventilation, it was found that depending on the severity of respiratory function disturbances it is directly proportional to decreased quality of life, both physical and psychosocial status. Life quality of in patients with chronic obstructive pulmonary disease worsens due to addition and progression of essential hypertension.&#x0D; Conclusions. Сomorbidly pathology has a negative impact on physical and psychosocial status of patients, which is significantly reduces life quality and depends on the severity of pulmonary ventilation disturbances, «length of the disease» and hypertension degree.

Nurse-Led Collaborative Management Using Telemonitoring Improves Quality of Life and Prevention of Rehospitalization in Patients with Heart Failure.

The effects of disease management using telemonitoring for patients with heart failure (HF) remain controversial. Hence, we embedded care coordination and enhanced collaborative self-management through interactive communication via a telemonitoring system (collaborative management; CM). This study evaluated whether CM improved psychosocial status and prevented rehospitalization in patients with HF in comparison with self-management education (SM), and usual care (UC).We randomly allocated 59 patients into 3 groups; UC (n = 19), SM (n = 20), and CM (n = 20). The UC group received one patient education session, and the SM and CM groups participated in disease management programs for 12 months. The CM group received telemonitoring concurrently. All groups were followed up for another 12 months. Data were collected at baseline and at 6, 12, 18, and 24 months.The primary endpoint was quality of life (QOL). Secondary endpoints included self-efficacy, self-care, and incidence of rehospitalization. The QOL score improved in CM compared to UC at 18 and 24 months (P < 0.05). There were no significant differences among the 3 groups in self-efficacy and self-care. However, compared within each group, only the CM had significant changes in self-efficacy and in self-care (P < 0.01). Rehospitalization rates were high in the UC (11/19; 57.9%) compared with the SM (5/20; 27.8%) and CM groups (4/20; 20.0%). The readmission-free survival rate differed significantly between the CM and UC groups (P = 0.020).We conclude that CM has the potential to improve psychosocial status in patients with HF and prevent rehospitalization due to HF.

Effect of tele-rehabilitation on glucose control, exercise capacity, physical fitness, muscle strength and psychosocial status in patients with type 2 diabetes: A double blind randomized controlled trial

AimTo determine the effect of a tele-rehabilitation (TR) program on glucose control, exercise capacity, physical fitness, muscle strength and psychosocial status in patients with type 2 diabetes mellitus (DM). MethodFifty type 2 DM participants were enrolled in the study and divided randomly into two groups; TR (n = 25, mean age: 52.82 ± 11.86) or control (n = 25, mean age: 53.04 ± 10.45) group. Participants in the TR group performed breathing and callisthenic exercises, three times a week, for 6 weeks, at home by internet based video conferences. Outcome measures including, HbA1c level, 6 min walk testing, physical fitness and muscle strength dynamometer measurement, Beck Depression Inventory were performed before and after the 6 weeks. ResultsHbA1c (p = 0.00), 6 min walking distance (p = 0.00), physical fitness subparameters; sit-up (p = 0.00), sit-and-reach (p = 0.04), back scratch (p = 0.00), lateral flexion right (p = 0.04), left (p = 0.00) and time up go tests (p = 0.00), muscles strength (p = 0.00); deltoideus-anterior, middle, quadriceps femoris and gluteus maximus, and depression levels (p = 0.00) changed significantly (p = 0.00) in TR groups. There were no significant improvements in control group (p > 0.05). ConclusionOur findings suggest that TR interventions found to be safe and effective, and may be an alternative treatment model for type 2 DM management. In addition to these health benefits, patients and rehabilitation team may save time, labor and treatment costs by using TR.

Meta-analysis of the influence of transitional care on quality of life of patients with chronic obstructive pulmonary disease

Objective To reconcile previous studies on the impact of transitional nursing intervention on quality of life in patients with chronic obstructive pulmonary disease (COPD) after discharge from the Chinese population. Methods PubMed, EMBASE, Cochrane Library, Clinical Trial, CBM, Joanna Briggs Institute Library, MEDLINE, Wanfang database and CNKI database, Vip database were searched by computer. The literatures about the influence of transitional nursing on the quality of life of COPD patients were searched until January 1, 2018. the data were analyzed by Rev Man 5.3 software. The results showed that P < 0.05 on both sides was statistically significant. Results Five clinical studies were included, and the data were combined according to the symptom, activity, social psychology and St.George′s Respiratory Questionnaire (SGRQ) total score in the SGRQ scale. Mean difference and its 95% confidence interval of symptoms, activities, social psychology and SGRQ score were -13.09 (-18.71, -7.48), 12.05 (-14.55, -9.45), -17.87 (-22.27, -13.46) and -15.42 (-19.74, -11.10) (Z=4.57-9.42, all P <0.05). Conclusions The transitional nursing has a significant positive effect on improving the quality of life among patients with COPD, which can effectively improve the psychosocial status of patients, and make the patients respond more actively to the problems in the process of treatment and rehabilitation.
