THE FIGHT AGAINST BREAST CANCER

This article reports the experience of a 60-year-old woman, living in a low-income community, who faced a battle with breast cancer. Initially, the disease was neglected and treated with home remedies, but when she sought medical help, she was diagnosed with advanced breast cancer. Despite medical recommendations, the patient chose not to undergo conventional treatment, following only palliative care. Family support played a crucial role in her decisions, influencing her choice to refuse surgery. However, the patient eventually withdrew from medical care, highlighting the need for a holistic approach and ongoing support for patients in similar situations. This report highlights the importance of raising awareness about breast cancer, early detection and adequate medical monitoring, as well as the need for emotional and psychological support for patients and their families throughout the treatment process.

DEVELOPMENT AND IMPLEMENTATION OF THE INFORM GUIDELINES FOR THE TREATMENT AND MANAGEMENT OF PROSTHETIC HIP INFECTION

Hip prosthetic joint infection (PJI) is a debilitating complication following joint replacement surgery, with significant impact on patients and healthcare systems. The INFection ORthopaedic Management: Evidence into Practice (INFORM:EP) study, builds upon the 6-year INFORM programme by developing evidence-based guidelines for the identification and management of hip PJI.A panel of 21 expert stakeholders collaborated to develop best practice guidelines based on evidence from INFORM \[1\]. An expert consensus process was used to refine guidelines using RAND/UCLA criteria. The guidelines were then implemented over a 12-month period through a Learning Collaborative of 24 healthcare professionals from 12 orthopaedic centres in England. Qualitative interviews were conducted with 17 members of the collaborative and findings used to inform the development of an implementation support toolkit. Patient and public involvement contextualised the implementation of the guidelines. The study is registered with the ISCRTN (34710385).The INFORM guidelines, structured around the stages of PJI management, were largely supported by surgeons, although barriers included limited awareness among non-surgical team members, lack of job planning for multidisciplinary teams, and challenges in ensuring timely referrals from primary care. Psychological support for patients was identified as a critical gap. Advanced Nurse Practitioners and multidisciplinary team (MDT) coordinators were seen as potential bridges to address these knowledge gaps. The guidelines were also viewed as a useful tool for service development.This study presents the first evidence-based guidelines for hip PJI management, offering a comprehensive approach to prevention, treatment, and postoperative care. Effective implementation is crucial, involving wider dissemination amongst primary and community care, as well as non-specialist treatment centres. Further resources are needed to ensure job planning for MDTs and psychological support for patients. Overall, this study lays the foundation for improved PJI management, benefiting patients and healthcare systems.

Abstract CT293: eCAN Project : The EU Joint Action on strengthening eHealth including telemedicine and telemonitoring for cancer prevention and care

Abstract The integration of telemedicine tools such as teleconsultation and telemonitoring in cancer patients care may allow to improve person-centered care and patients’ empowerment. The eCAN JA (https://ecanja.eu/) explores the role of telemedicine tools in clinical trials focusing on tele-rehabilitation and tele-psychological support in different populations of cancer patients in 10 European countries.The main aim is to assess the effect of teleconsultation and telemonitoring program focused on rehabilitation and psychological support for cancer patients on patient reported outcomes measures (PROMs) in three different pilot randomized controlled trials. MethodsThe pilot trials have been launched in September 2023 in 10 European countries with a multicentric, prospective, randomized, open label design in patients affected by Breast Cancer (BC) (Pilot 1a), Head and Neck (H&N) (Pilot 1b) and advanced cancer (Pilot 2). According to the sample size calculation, the aim is to enroll a minimum of 354 patients, in 18 European cancer centers. Patients are randomly assigned either to the intervention or control group using a 1:1 ratioThe main objectives of the pilots are: 1-To assess the effect of teleconsultation program and telemonitoring focused on rehabilitation for patients with BC (1a) and H&N (1b) cancer on the PROMs (quality of life (QoL) and pain) compared to usual care.2-To assess the effect of teleconsultation program and telemonitoring focused on psychological support for patients with advanced cancer (pilot 2) on the PROMs (QoL and distress thermometer) compared to usual care. Patients in the intervention group receive weekly teleconsultation during 8 weeks through the secure Edumeet platform. In pilot 1, tele-rehabilitation training is performed by a remote physiotherapist and consist of a series of revalidation exercises. In pilot 2, tele-psychological support is performed by a remote psychologist and consist of techniques for managing emotions and stress.PROMs data will be collected longitudinally during the intervention with dedicated digital application (eCAN app) and distant Telemonitoring with eCAN dashboard. Patients in the control group receive usual (in person) care. Results64/354 patients have been enrolled at present. An interim analysis is planned for the end of January 2024 in order to assess feasibility, patients compliance and dropout rate.. ConclusionThe integration of telemedicine tools such as teleconsultation and telemonitoring may allow to improve person-centered care and patients’ empowerment. Management of pain in cancer patients— lessons from practices during the COVID-19: a qualitative study of cancer care providers’ perspectives

BackgroundThe ongoing COVID-19 pandemic has impacted health systems globally and affected managing many chronic conditions, including cancer. This study aimed to explore the perceptions of multi-disciplinary cancer care providers on how cancer pain management was affected by the COVID-19 pandemic.MethodsParticipants were eligible if they were cancer care providers of any specialty and discipline from two tertiary hospitals in Australia. Data were collected using semi-structured interviews to explore cancer care providers’ perspectives on cancer pain management within COVID-19. Thematic analysis of interview transcripts used an integrated approach that started with inductive coding before coding deductively against a behaviour framework called the COM-B Model, which proposes that ‘capability’, ‘motivation’ and ‘opportunity’ are requisites for any behaviour.ResultsTwenty-three providers participated. Five themes were developed and interpreted from the analysis of data, namely: “Telehealth enables remote access to cancer pain management but also created a digital divide”, “Access to cancer pain management in the community is compromised due to the pandemic”, “COVID-19 negatively impacts hospital resource allocation”, “Patients were required to trade off cancer pain management against other health priorities” and “Hospital restrictions result in decreased social and psychological support for patients with cancer pain”.ConclusionsThe landscape of cancer pain management in the Australian health system underwent substantial shifts during the COVID-19 pandemic, with lasting impacts. Cancer care providers perceived the pandemic to have significant adverse effects on pain management across multiple levels, with repercussions for patients experiencing cancer-related pain. A more adaptive health system model needs to be established in the future to accommodate vulnerable cancer patients.

Giant Congenital Melanocytic Naevi in Newborns: A Case Series

Giant Congenital Melanocytic Naevi (GCMN) is a very rare condition that mostly occurs due to an oncogenic mutation involving the NRAS (neuroblastoma RAS viral oncogene homologue) gene. GCMNs can be clinically diagnosed due to their typical features, but histopathology confirms the diagnosis. GCMN has a high propensity for transforming into malignant melanoma and leptomeningeal melanocytosis, leading to neurological deficits such as epilepsy and neurofibromatosis. As it is associated with Central Nervous System (CNS) melanosis, Magnetic Resonance Imaging (MRI) of the brain and spine is crucial. Here, three cases are reported of the babies born with GCMN from August 2022 to July 2023 in a tertiary care hospital among a total of 11,915 live births, where the incidence is much higher than found in the literature. Present series describes three cases with GCMN, including clinical features, MRI findings, short-term outcomes, risk factors, and modalities of the management. All three cases showed extensive blackish pigmented patches involving most of the body surface and MRI evidence of Neurocutaneous Melanosis (NCM). The third baby succumbed to death due to fulminant sepsis on day 2 of life. Management of GCMN requires a multimodal approach, including medical, surgical, chemotherapeutic, palliative, and psychological support for patients as well as parents, and follow-up is necessary for the early detection of malignant transformation. Detailed knowledge of this very rare condition may enable us to develop newer treatment modalities to achieve better outcomes in the near future.

<p><strong>Relevance.</strong> The category of patients with somatization disorders (SD) poses a serious problem for primary health care, where their prevalence reaches 40%. There is an opinion that in modern society there is a phenomenon of stigma, and a stigmatizing attitude towards the mentally ill persists. Currently, the phenomenon of self–stigmatization is being studied to a greater extent in severe mental disorders: the connection of self–stigmatization with social identity, a sense of self–worth, internal stability, and the ability to cope with stressful situations. The issue of developing destigmatized forms of care for patients with non–psychotic disorders, primarily psychotherapeutic offices in the primary health care network, remains relevant. <strong>The purpose</strong> of the study is to study differences in the severity of self–stigmatization and indicators of attitude towards the disease and treatment in patients with SD who sought help from a psychiatrist or psychotherapist and in women who avoided contacting specialized specialists. <strong>Methods.</strong> Questionnaire for assessing self–stigma I.I. Mikhailova, V.S. Yastrebova, a questionnaire to study attitudes towards the disease and treatment of B.D. Karvasarsky, correlation and regression analysis. The article presents <strong>the results</strong> of an empirical study of the relationships and parameters of motivation for treatment and the level of self–stigmatization in women suffering from somatization disorders with manifestations of «psychiatrist–avoiding behavior», on the basis of which the authors describe models of attitude towards treatment. <strong>Conclusions.</strong> Based on the identified relationships between the parameters of patients’ motivation to seek help from a psychiatrist and the level of self–stigmatization, it was possible to describe models of attitude towards treatment in patients with somatization disorders who have not sought specialized psychiatric help for a long time: «model of denial», «model of fixation», «model of ignoring» , confirmed by the presence of regressive connections and the «dubious benefit» model, justified by the presence of a positive linear correlation. The data obtained will form the basis for the development of a program of psychotherapeutic assistance and clinical and psychological support for patients with CP not only at the inpatient, but also at the outpatient stages of treatment.</p>

Psychological support of patients with motor disorders at the second stage of medical rehabilitation

The article presents the results of a literary review on the problem of the organization of differentiated psychological support at the second stage of medical rehabilitation of patients with the consequences of motor disorders that arose as a result of stroke or developed against the background of degenerative and dystrophic diseases of large joints and spine. The psychological consequences of motor disorders, patients’ characteristics, and possible approaches to psychocorrection are analyzed. It was revealed that regardless of the nosology of motor disorders, the targets of psychocorrection at the second stage of medical rehabilitation of patients are signs of anxiety and depression in the current emotional state, while the specific targets in the case of stroke are disorders of higher mental functions, and in the case of degenerative and dystrophic diseases of large joints and spine — subjective perception of pain syndrome. As the main guidelines for the systematic work of a psychologist as part of a multidisciplinary rehabilitation team at the second stage of medical rehabilitation of the studied contingent of patients, the following are indicated: determining the basic targets of psychocorrection, selecting relevant and nosology-sensitive motor disorders correction methods, and increasing the effectiveness of psychocorrection through the use of high-tech virtual and augmented reality tools.

Quality of life of lung cancer patients

Abstract Lung cancer is one of the most prevalent forms of cancer in patients in Ukraine. The objective of the study was to identify the main problems in the quality of life of patients with lung cancer in order to optimize medical care for this patient group. A survey of 411 patients of oncology institutions in nine regions of Ukraine was conducted in the period from November 2021 to February 2022. The sociological survey was performed using the EORTC QLQ-C30 and QLQ-LC29 questionnaires. As of the pre-war period, the quality of life of lung cancer patients in Ukraine amounted to 49.12 points on a 100-point scale. According to the QLQ-C30 symptom scale, the highest score among the complaints of Ukrainian patients is that of financial difficulties, the second place is occupied by fatigue. According to the QLQ-LC29 symptom scale, patients with lung cancer were most concerned with “Fear of progression”. Ukrainian patients were least concerned about diarrhea and hemoptysis. When providing medical care to cancer patients, components to ensure the proper quality of their lives and psychosocial care, including the standardization of psychological care and its introduction in all institutions that provide medical care to cancer patients; organization of social and psychological support for cancer patients and their families; as well as educational work with relatives, colleagues, medical professionals on awareness of the need for social and psychological support for patients must be taken into account.

Introduction Anxiety from surgery is something that is often experienced by someone, here the role of the operating room nurse is how to reduce the anxiety level of preoperative patients, including by applying therapeutic communication to preoperative patients. The purpose of this study was to determine the relationship between nurse therapeutic communication and preoperative patient anxiety.
 Methods The research design uses the Correlational Research Method with a Cross Sectional approach. The population of this study are patients who will undergo surgery. The research sample used Accidental Sampling and obtained a sample of 58 patients from a population of 86 patients. Data collection used the HARS questionnaire. Statistical Test Spearman rank statistical test with α (0.05) SPSS 20.
 Results The results of this study showed that most of the 42 respondents (72.42%) had good therapeutic communication criteria and almost all 47 respondents (81.03%) had moderate anxiety criteria. The results of the Spearman rank statistical test analysis obtained p value = 0.008, which means there is a relationship between nurse therapeutic communication and preoperative patient anxiety.
 Discussion Communication is an action that is the application of the nurse's duties as a counselor and educator. Therapeutic communication in nursing needs to be developed to provide education and psychological support for patients who will undergo an action that the patient has never undergone, such as surgery.

Information needs of informal caregivers in caring and rehabilitation for dioxin victims in Vietnam

BackgroundHealth outcomes among Agent Orange/dioxin (dioxin) victims are significant due to many individuals requiring daily assistance, informal care, and rehabilitation support. This study aimed to identify the information needs of informal caregivers of dioxin victims in Vietnam.MethodsA cross-sectional study was conducted in Quynh Phu district, Thai Binh province – an area with a large number of dioxin victims, from June 2019 to June 2020. Quantitative data were collected from 124 caregivers of victims via structured interviews. Qualitative data were collected using semi-structured interview guides with in-depth interviews (IDI) (n = 36) and two focus group discussions (FGD) (n = 12).ResultsThe results demonstrated that all caregivers of dioxin victims were family members, predominantly older (71.8%), 61.5 years old on average, living on low incomes (87.9%), and were farmers (80.7%). Almost all participants (96.8%) reported having information needs, particularly concerning dioxin’s harms, nutrition, dioxin-related policies and rehabilitation, and psychological support for patients. Caregivers reported that they would like to receive information via health staff counselling (85.0%), television (75.0%), and community loudspeaker (65.8%). Notably, the majority of caregivers reported the need for information regarding psychological support (70.0%). These findings are consistent with qualitative data, which identify an urgent need to provide information, especially through health staff and digital resources.ConclusionMany families with dioxin victims lived with little support and information, highlighting their high demand for information about care and rehabilitation. Thus, the healthcare system should promote information support, policy, and psychological support for caregivers and victims. An online support system for caregivers and victims is also recommended.

Whether acute or chronic, pain leads to a reorganization of the subject's psychic functioning, and thus to a different interpretation of his or her own painful experience. To transform the experience, pain must be addressed in all its biopsychosocial components, and through the use of various therapeutic devices, with the hope of overcoming it.

Long term efficacy of psilocybin in patients with cancer and major depressive disorder (MDD).

12021 Background: Up to 25% of people living with cancer have depression. Existing psychological interventions have limited efficacy in treating depression. Psilocybin, a 5-hydroytrptamintergic psychedelic, coupled with group therapy is safe and effective in ameliorating symptoms of depression (Agrawal et al, ASCO 2022). Response to psilocybin treatment was observed in 83.3% of patients at 8 weeks, and 50% of patients showed full remission of depressive symptoms. This study evaluates the efficacy of psilocybin-assisted group therapy in relieving depression in cancer patients over 18 months. Methods: Adults with curable or metastatic cancer and MDD (according to DSM-5) underwent psychological assessments at baseline. They were treated with single-dose psilocybin 25 mg administered simultaneously in group cohorts of 3-4, supported with group and individual psychological therapy, and evaluated at 8 weeks and 18 months. Primary objectives were long-term effectiveness on depression and anxiety severity (as measured by change in Montgomery Asberg Depression Rating Scale [MADRS] and Hamilton Depression Rating Scale [HAM-A] scores by independent rater assessments) as well as clinical response (≥50% decrease in MADRS total score from baseline to 18 months) and remission (defined as patients with a MADRS total score ≤10 at 18 months). Results: Of 30 patients enrolled in the parent study, 2 died and 4 were lost to follow-up. Clinical response was demonstrated in 18/28 patients (64.2%) from baseline to 18 months follow-up (see table); 16/28 patients (57.1%) demonstrated remission of depression at 18 months. In 24 evaluable patients, continued reduction in mean depression severity scores was also demonstrated from baseline to week 8 (MADRS by 20.6 points [95% CI 23.2, 18.0, p < 0.001] and HAMA-A by 16.7 points [95% CI 19.7, 13.7, p < 0.001]) and from baseline to 18 months (MADRS by 16.7 points [95% CI 20.4, 12.9, p < 0.001] and HAMA-A by 14.4 points [95% CI 17.3, 11.4, p < 0.001]). Conclusions: Long-term follow-up suggests continued robust clinical response and remission from depression with a single dose of psilocybin and simultaneous psychological support in patients with cancer and MDD. These findings indicate psilocybin may play a role in treatment of depression in cancer patients, and further investigations are ongoing. [Table: see text]

Quality of life of patients with retinoblastoma and methods of their support and rehabilitation

The review article is devoted to evaluation of the quality of life of patients with retinoblastoma (RB), in particular children following enucleation of the eyeball. The main methods of cosmetic rehabilitation and psychological support of patients with RB are considered. The analysis of the literature suggests that it is important to pay attention not only to the physical condition, but also to the emotional well-being of patients with RB, and to develop specialized rehabilitation measures.

Nursing Intervention for Quality of Life in Patients with Ostomy: A Systematic Review.

Ostomy-related challenges in everyday life may negatively affect patients' Quality of Life (QL), even long after ostomy surgery. Nurses provide care interventions that may have a significant effect on the patients' QL. Therefore, the purpose of the present study was to determine evidence of effective nursing interventions on the QL of patients with an ostomy. A systematic review was conducted following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. English articles published from 2000 to 2021 were identified and retrieved from PubMed, Medscape, MedlinePlus, ScienceDirect, Web of Science, Scopus, Wiley Online Library, ProQuest, ClinicalKey, SAGE, IRCT, and Cochrane. We assessed all included studies (18 articles) using the JBI Critical Appraisal Checklist. Interventions reviewed in the study were categorized based on intervention, content of nursing intervention, training method, and target group of the nursing intervention. The nursing interventions involved natural treatments, self-care, and psychological support for patients. The training methods used in the studies included demonstration and lecture, mixed media presentation including PowerPoints, and Internet tools such as WeChat Applications. Some interventions were only presented to patients, caregivers, caregivers and patients together, or peer patients. According to the results of the present study, it is suggested that a regular care plan be used with the participation of families and patients from pre-operational to discharge along with nurses' follow-ups and home care to promote the QL. A combination of self-care and psychological education can promote these patients' QL.

Prevalence of anxiety and depression symptoms in a sample of outpatients with ATTR cardiac amyloidosis.

Patients with ATTR cardiac amyloidosis (ATTR-CA) face rare disease that could negatively influence psychological well-being with consequences on the course of the disease and quality of life. However, to date, no study analyzed the prevalence of anxiety and depression in patients with ATTR-CA and which clinical and sociodemographic characteristics are linked with these psychopathological conditions. A total of 109 consecutive patients (83% males) aged 62-90 years with ATTR-CA were recruited. In order to better understand the prevalence of anxiety and depression in ATTR-CA, a control group composed by 33 individuals equaling gender, education, and age were recruited. The level of anxiety and depression was measured using the Italian version of the Hospital Anxiety and Depression Scale (HADS). Sociodemographic and clinic characteristics were registered. Almost half of patients (49%) reported a clinical level of depression or anxiety, or both. ATTR-CA patients reported higher levels of anxiety and depression than control group. Results showed that older patients with ATTR-CA, especially females, with more advanced disease could be more at risk to develop an anxious disorder. Furthermore, being a woman, and presenting with a greater severity of symptoms, would appear to be a risk factor for developing a depressive disorder. Overall, these results highlighted the high presence of anxiety and depression in ATTR-CA patients, suggesting to physicians to pay attention to the psychological well-being of ATTR-CA patients. In fact, a psychological support for patients with high level of psychopathological disease could reduce disease burden and improve quality of life in ATTR-CA population.

Preoperative anxiety and psychological support needs among patients with thyroid surgery in Vietnam

Objective: Psychological support for patients has been paid currently attention to and implemented in many medical facilities. The study aims to describe preoperative anxiety and the need for psychological support among patients with thyroid surgery and related factors. Subjects and method: A cross-sectional descriptive design was used with a convenience sample of 197 patients at the National Hospital of Endocrinology with thyroid surgery from April to June 2022. Results: The results indicated that 71.6% of patients had mild anxiety, 16.2% of patients had moderate anxiety, 11.7% of patients had no anxiety, and 0.5% of patients had severe anxiety. Common anxieties of patients undergoing thyroid surgery were postoperative complications (65%), postoperative pain (58.4%), unconsciousness after anesthesia, or surgery (46.7%), thyroid tissue remaining after surgery (44.2%), and death of anesthesia and surgery (41.1%). The average score of the need for social support was 24.02 (SD 6.01) of which family support was 12.88 (SD 3.93), and medical staff’s support was 11.14 (SD 2.73). 43.7% of patients had high support needs, 39.1% of patients had medium support needs, and 17.3% of patients had low support needs. Factors associated with patients’ anxiety included age, education level, marital status, waiting time for surgery, number of caregivers during the hospital stay, health insurance, cancer diagnosis, previous surgical history, chronic comorbidities, history of Covid19, number of Covid - 19 vaccine shots, and psychological support needs. Conclusion: The nurses should implement interventions to reduce anxiety before surgery for patients such as shortening the waiting time for surgery, regularly listening to the patient’s opinion, and consulting relevant information about the surgery.

A qualitative study about colorectal cancer patients and spousal caregivers’ experience and needs during COVID-19: implications for self-efficacy intervention

ObjectiveThis study aims to understand the experiences of colorectal cancer (CRC) patients and their spousal caregivers during the COVID-19 pandemic and to refine a self-efficacy (SE) intervention for these couples. MethodsA descriptive phenomenological approach was used in this study. Data were collected from 11 CRC couples. All interviews were recorded, transcribed, and analyzed using the Colaizzi strategy. ResultsThree themes and eight subthemes emerged: (1) Get and contribute support, (2) Life's challenges, and (3) Journey of reconstruction. The CRC couples encountered escalating challenges in coping with cancer during COVID-19. At the same time, they have received considerable support and developed confidence in rebuilding themselves in the process. Healthcare providers are advised to focus on giving appropriate support to CRC couples, so they can go further. ConclusionsThis study gave insights into healthcare providers on the experiences of CRC couples and the development of SE intervention program to support these couples: (1) initially providing caregiving training for spousal caregivers and psychological support for patients, (2) encouraging self-care for CRC couples in the middle stage, (3) guiding them to view life positively in the later stage, and (4) assessing their situation in time to identify their needs and to provide support. Healthcare providers are recommended to increase flexibility in the SE intervention program delivery format to reduce the impact of COVID-19 on CRC couples.

IntroductionContemporary in Ukraine the special priority has been the somatoform disorders increase. The most significant complications belong to the patient’s self-evaluation of the influence of the disease on their social functioning, influence essential part of the self-evaluation of the disease and the important point of therapeutic personality accomodate intervention.ObjectivesDevelop the stages of personalized models of psychotherapyMethodsOn the basis of the examined 270 somatoform disorders patients and 190 ischemic cardial disease patients we have elaborated a formal test that allows to evaluate quantitatively the influents of the disease on various spheres of patients´ social status.ResultsIt was absolutely unrespectable the common for ischemic cardial and somatoform disorders patients rise of significance of personal individual, common life factors in cases of aggravation of the main disease course. We created the personification accomodate psychotherapy system consist with CBT, suggestive and autosuggestive implementations. Elucidation of peculiarity of personal perception of the disease served as basis of elaboration of psychotherapy system, consulting, psychological support for patients with high-effectiveness 1,5 - 3 years catamnes in 85% patients.ConclusionsThe retrospective results showed the necessity the personification oriented models of psychotherapy, parted on stages. On the first stage - sedative-adapting the receptions of CBT and suggestive psychotherapy are used. There is group therapy on second stage.
