Issues and Challenges in Psychiatric Consultation-Liaison nurse (PCLn)

Mental health problems are not currently given the same importance as physical health. Nurses, particularly those working in non-psychiatric settings, report that they do not feel adequately prepared to meet the mental health needs of patients. The psychiatric consultation-liaison nursing role has arisen in part, as a response to these difficulties and aims to facilitate access to mental health nursing expertise for general hospital patients and staff.

An examination of the services provided by Psychiatric Consultation Liaison Nurses in a general hospital

The political, professional and economic context in which mental health care is delivered has witnessed significant changes in recent times. The movement of psychiatric patients from institutions to community settings has seen the emphasis of the loci of care delivery shifting with increasing numbers presenting to general hospitals. The speciality of the Psychiatric Consultation Liaison Nurse (PCLN) has emerged as a bridge between mental health and general hospital services. A descriptive, non-experimental research approach was employed to establish patient profiles and to provide an overview of the depth of service provision. This design was selected as the most appropriate because of the limited information on the PCLN in an Irish setting and secondly, as the study was limited to one rural geographical location. A questionnaire was utilized to collate the details of patients who had received a mental health assessment. The findings presented here are the profiles of patients assessed by the PCLN during the 3-month study period. The results add further credence to the existing evaluative studies in presenting the characteristics of patients within the Irish context and further contribute to the unique body of evidence that defines the mental health nurse in an advancing role.

Effects of a nursing intervention on quality of life outcomes in post-surgical women with gynecological cancers.

Women with gynecological cancers have reported poor health-related quality of life (QOL), with complex physical and psychological needs post-surgery and during chemotherapy treatment. There are no studies reporting interventions addressing these needs post-hospital discharge in this population. Patients were randomized into two groups. The intervention group received 6 months of specialized care by an Advanced Practice Nurse (APN); in addition, women with high distress were evaluated and monitored by a psychiatric consultation-liaison nurse (PCLN). The attention control group was assisted with symptom management by a research assistant. The effects of the 6-month intervention were evaluated using self-report questionnaires at baseline (24-48 h after surgery), 1, 3, and 6 months post- surgery. QOL assessments included the Center for Epidemiological Studies-Depression Scale , the ambiguity subscale of the Mishel Uncertainty in Illness Scale , the Symptom Distress Scale, and the Short-Form Health Survey (SF-12). The sample for the longitudinal analysis included 123 who completed QOL outcome measures across three occasions post-surgery. The APN intervention resulted in significantly less uncertainty than the attention control intervention 6 months after surgery. When the sub-group who received the APN plus PCLN intervention was compared with the total attention control group, the sub-group had significantly less uncertainty, less symptom distress, and better SF-12 mental and physical QOL over time. Nurse tailored interventions that target both physical and psychological aspects of QOL in women recovering from cancer surgery and undergoing chemotherapy produce stronger outcomes than interventions that target solely one QOL aspect.

Evaluation of outcomes in Psychiatric Consultation-Liaison Nursing practice

This report describes and evaluates the current status of outcome analysis in Psychiatric Consultation-Liaison Nursing (PCLN) and offers suggestions for future development. The status of outcome evaluation generally in psychiatric nursing is described with attention given to the scope of practice of PCLN and outcomes used by psychiatric consultation-liaison nurses (PCLNs) to evaluate their interventions. An evaluation framework based on Donabedian’s paradigm of structure, process, and outcome is presented and its applicability shown to PCLN. This framework is further explicated with regard to a review of published studies of PCLN within the outcome domains of cost reduction, satisfaction with services, changes in clinical status, and perceptions of work environment. Finally, recommendations for further development of outcomes in PCLN are offered, based on the strengths and limitations of the extant literature.

EVALUATING THE ROLE OF A PSYCHIATRIC CONSULTATION-LIAISON NURSE IN THE AUSTRALIAN GENERAL HOSPITAL

The importance of Psychiatric Consultation-Liaison Nursing (PCLN) in improving health outcomes for patients experiencing mental health problems has received some attention in the nursing literature. However, little effort has been made to evaluate the impact of this role. The study presented in this paper was conducted in order to assist in redressing this paucity. Focus groups were conducted with nurses ( n = 17) employed at a large general hospital in Melbourne, Australia. The nurse participants were asked to discuss their views about utilizing the services of the specialist PCLN employed in this hospital. The responses of participants were overwhelmingly positive. The four main themes to emerge from the study were: helping staff, making contact, implementing strategies, and utilizing attributes. These findings are presented and discussed as a contribution to and extension of the existing literature.

The role of the psychiatric consultation-liaison nurse in the improved care of patients experiencing mental health problems receiving care within a general hospital environment

The Psychiatric Consultation-Liaison Nurse (PCLN) has an important role in supporting nurses and other health care professionals in caring for patients experiencing mental health problems in a non-psychiatric environment. The limited available research findings suggest the PCLN role to be effective in terms of staff satisfaction, and some suggestion of cost savings is apparent. This paper refers to a survey of the attitudes of health professionals (n-113) to the services provided by the PCLN in a large metropolitan hospital in Melbourne, Australia. The results indicate a high level of satisfaction with the services of the PCLN. Enhancement of the participants’ professional role, the professional role of nursing and improving health outcomes was amongst the perceived benefits of the PCLN role.

An overview of the role and functions of a psychiatric consultation liaison nurse: an Australian perspective.

The role of the psychiatric consultation liaison nurse (PCLN) has increased substantially in popularity over the last few years. Despite the growth of this position, a paucity of literature regarding the role, functions and effectiveness of psychiatric consultation liaison nursing continues to exist. The current study was undertaken as part of the Victorian Nurse Practitioner Project. A significant aspect of this study concerned collection of data on the activities of the PCLN. This approach enabled an extensive and detailed profile of the PCLN to be formulated. The findings indicate that the PCLN provided a service to nursing, medicine and allied health in relation to patients experiencing mental health problems in the general hospital setting. Patients referred to the PCLN presented varied clinical features in terms of medical, surgical and mental health disorders. The PCLN performed a range of interventions. The results of this study make a significant contribution to address the current paucity of literature.

The psychiatric consultation-liaison nurse: towards articulating a model for practice.

Psychiatric consultation-liaison nursing (PCLN) has experienced considerable growth in the past decade in Australia. In spite of this growth, a model for PCLN practice has not been adequately developed and articulated. The aim of this paper is to begin the process of articulating a model for PCLN practice in order to redress the paucity of literature in this area. The paper includes a brief history of PCLN; articulation of the components of the PCLN role, namely consultation, liaison and culture brokering; the impact of PCLN on quality improvement; and the importance of research articulation for the development of the PCLN role.
