Pseudoaneurysm of profunda femoris artery: a rare complication after intramedullary fixation for an intertrochanteric femur fracture

Intertrochanteric fracture fixation with a trochanteric femoral nail rarely leads to any vascular or neurological complications. The aim of this case report is to identify a patient with post-operative Pseudoaneurysm of profunda femoris artery and how to manage it. We report a case of 79-year-old male who developed a Pseudoaneurysm of the profunda femoris artery 3 days after intramedullary femoral nailing for a intertrochanteric femur fracture. Percutaneous embolization was performed with subsequent resolution of the symptoms. Diagnosis of vascular complications after hip surgery may be very challenging because symptoms are often nonspecific. Despite their rarity, it is important to know this type of complications to address the diagnostic pathway in the right direction and to treat them promptly.

Unusual presentation of pseudoaneurysm with trochanteric fracture femur with associated long-term antiepileptic therapy.

Arterial injury following impalement due to a trochanteric hip fracture-fragment per se has been documented rarely. We report a case of pseudoaneurysm of profunda femoris artery at the first perforator branch in a 48-year-old male, with trochanteric hip fracture following a fall during an epileptic episode. Persistent recalcitrant pain, globular swelling in the groin, unexplained drop in the haemoglobin level, and color doppler ultrasonography findings were salient features to the diagnosis. Additionally, we collected all reported cases of pseudoaneurysm associated with hip fractures. We reviewed the literature regarding the incidence, treatment, and prognosis for the same. Acute vascular injury was probably caused by the spikes of fractured lesser trochanter which was found to be displaced superomedially. All trochanteric fractures especially those with displaced lesser trochanter fragment should be closely watched for the possibility of vascular injury. Early diagnosis and treatment in a staged manner can prevent the catastrophic vascular event and hence the limb.

A cost effective endovascular approach for management of post-catheterization profunda femoris artery pseudoaneurysm using thrombin

Post-catheterization PSA is one of the most commonly encountered vascular complications of cardiac and peripheral angiographic procedures. We report the case of patient who developed deep-seated profunda femoris artery pseudoaneurysm (PSA) following cardiac catheterization. Despite, repeated ultrasound guided compressions the PSA failed to close and instead produced local site pressure ulcers. The secondary infection followed which precluded use of percutaneous thrombin injection. The PSA was finally closed via a total endovascular technique combining intravascular thrombin injection and coil embolization, thus obviating the need for expensive measures like cover stents or invasive surgical repairs.

Profunda femoris artery pseudoaneurysm after surgery and trauma

Pseudoaneurysms of the profunda femoris artery have been reported following different types of trauma and from orthopedic procedures performed in the proximal femur. Two cases of profunda femoris artery pseudoaneurysm with two rare causes are presented. The first one is a core decompression of femoral head for osteonecrosis and the second one is a proximal femur fracture nailing. Awareness and careful follow-up are the key issues for the early diagnosis.

Profunda femoris artery pseudo aneurysm is a rare occurrence following fractures of the proximal femur or their surgical fixation. They usually present late because of the deep position of the artery. Patients present with a painful expansile mass in the thigh with or without progressive anaemia. Modalities such as ultrasound scanning may aid in diagnosis but CT arteriography is used mostly for accurate diagnosis and intervention. We report a case of delayed presentation of profunda femoris pseudo aneurysm following dynamic hip screw fixation for intertrochanteric femur fracture.

Pseudoaneurysm of the profunda femoris artery after external fixation for a comminuted femur fracture

Pseudoaneurysms of the profunda femoris artery are rare but recognized complication following percutaneous arterial interventions, penetrating traumas, bone fractures and surgical interventions. Most patients are asymptomatic initially and later present with a pulsatile, expanding mass with an audible bruit in the thigh. Angiography is important in the evaluation of collateral pathways before considering vascular reconstruction or embolization. We discuss a rare case of profunda femoris artery pseudoaneurysm after external fixators and the successful treatment by transarterial coil embolization.

Profunda Femoris Artery Pseudoaneurysm Presenting with Triad of Thigh Swelling, Bleeding and Anaemia

Introduction: We describe an alternative presentation of 2 cases of femur fractures with pseudoaneurysms of the profunda femoris artery. Clinical Picture: In both cases, there is a recurrent triad of: (1) thigh swelling, (2) bleeding from the fasciotomy wound, and (3) anaemia with a falling haemoglobin trend. Treatment: Surgical exploration is often not diagnostic or therapeutic. Angiography can accurately diagnose the presence of a pseudoaneurysm and intervention with coil embolisation is effective in arresting further bleeding. Outcome: Both cases show good outcome following coil embolisation. Conclusion: Recognition of this triad is necessary to ensure early accurate diagnosis so that proper treatment is rendered to prevent further recurrent bleeds.
