An exploration of targeted cryotherapy protocols, using the swellaway knee unit, on healthy male subjects

Purpose: With the development of the guidelines for acute injury management evolving from PRICE (Protection, Rest, Ice, Compression and Elevation) to POLICE (Protection, Optimal Loading, Ice, Compression, Elevation), the ability to facilitate optimal loading is key to promote early recovery with early activity. A 20-minute ice application has been reported to inhibit muscle strength and joint position sense. Therefore, the purpose of this study was to explore if targeted cryotherapy, over a smaller surface area using the Swellaway Knee Unit, could achieve the desired skin surface temperatures (Tsk), whilst minimising potential reductions in muscle strength or joint position sense post-intervention; hence facilitating early optimal loading.

Various therapies for the treatment of sprains have emerged as advances occur in biomedical engineering and photobiology. Therapy with coherent and non-coherent light is a treatment modality for various musculoskeletal injuries. The main certified phototherapy benefits are the reduction of nociceptive processes and the modulation of the inflammatory process, among others. The objective of this study was to analyse the changes caused by the use of light-emitting diodes (LED) (λ627 ± 10nm) with an energy density of 10J/cm2 in 40 subjects divided into two groups (20 placebo and 20 LED). All of the volunteers had acute ankle sprains by inversion of grade II treated with the PRICE (protection, rest, ice, compression and elevation) technique and were treated for 6days with LED therapy and LED therapy turned off (placebo). Pain assessment was performed on the 1st, 3rd and 6th days using the visual analogue scale (VAS) of pain, the McGill Pain Questionnaire and volumetry. The group treated with LED showed statistically decreased pain compared to the placebo group in both the VAS (85.79 vs 55.73%) and McGill questionnaire (83.33 vs 52.52%). The reduction of oedema in the LED group on the 3rd and 6th days after therapy was statistically superior to that in the placebo (p < 0.0001). Based on the results of this study, treatment with LED, using the tested dose, is effective for pain and oedema in the initial phase of ankle sprains.

ⓒAcute sprain of the ankle requires comprehensive history taking and physical examination in diagnosing the type of severity and deciding on the plan of treatment. Literature supports functional treatment as the treatment of choice for grade I and II injuries. During the acute phase, the goal of treatment focuses on controlling pain and swelling. PRICE (protection, rest, ice, compression, and elevation) is a well-established protocol at this phase. There is some evidence that application of ice and use of nonsteroidal anti-inflammatory drugs improves healing and speeds recovery. The symptoms of Osgood Schlatter Disease mimics different condition and thus it has to be diagnosed rationally. Clinical features include mild to severe and intermittent or continuous pain, tenderness, swelling and limp while walking. Radiological features include enlarged and fragmented tibial tubercle. Treatment of Protection, Rest, Ice, Compression and Elevation (PRICE) protocol is generally given in the initial stages followed by the complete rehabilitation of athlete into sports.Methods:&nbsp;I present the case of a patient, fifteen year old boy presenting the symptoms of pain, tenderness, swelling and limp while walking. He was referred by Orthopedician and he was on diclofenac medications. His detailed evaluation was carried out and was diagnosed on the basis of history, clinical findings and radiographic investigations as Osgood Schlatter Disease.Treatment:&nbsp;A three phase rehabilitation treatment plan was developed which was typically aimed at return of athlete as early as he can to the sports circuit. In Acute phase, Ice and rest was given; In Recovery phase, strengthening of surrounding musculature was given; In Maintenance phase, changes were made to athlete’s playing style with the help of Coach.Conclusion:&nbsp;Initial assessment, PRICE Protocol and making changes in playing style of the athlete helps in treating Osgood Schlatter Disease. The causative factor has to be looked upon while treating this condition.

A Novel Cryotherapy Compression Wrap in the Management of Acute Ankle Sprains: Potential Use for Special Operators on the Battlefield

Musculoskeletal injuries related to training and operational missions frequently affect military personnel. A common treatment for these injuries is the PRICE (protection, rest, ice, compression, and elevation) method, which is time consuming and impractical in the field. Therefore, the primary objective of this study was to determine the effectiveness of the cryotherapy wrap compared to a traditional treatment in the management of acute ankle sprains.A randomized controlled clinical trial was conducted in a university research laboratory with 13 subjects (9 males and 4 females) with the following physical characteristics: age (yr) 20.6 ? 2.2, height (cm) 177.0 ? 14.3, weight (kg) 76.6 ? 20.6, and body mass index (kg/m2) 24.1 ? 3.7. Participants were instructed to perform PRICE with a traditional ice pack and compression wrap (control group) or with an Arctic Ease? cryotherapy wrap (test group) for 48 hours following enrollment in the study. The Numeric Pain Scale, Foot and Ankle Ability Measure, and ankle/foot volumetric measurement were performed at initial presentation and 24-hour, 48-hour, and 7-day follow-up intervals.While the comparison of the Numeric Pain Scale scores, Foot and Ankle Ability Measure scores, and volumetric changes between groups revealed no statistically significant differences (p > 0.01), there was an 86% compliance rate for subjects in the cryotherapy wrap group compared to a 17% compliance rate of subjects in the control group.The cryotherapy wraps performed comparably to ice therapy and therefore may be especially applicable to military personnel required to operate in austere and hostile environments where traditional therapies are unrealistic. Although this pilot study did not demonstrate that the cryotherapy wraps produce statistically superior results, trends emerged in the data suggesting that subject compliance rate may be improved by using an alternative form of cryotherapy compression, which could lead to better management of pain, edema, and functional recovery. Future research should include a larger sample size to verify this claim.
