Endodontic therapy in teeth with calcified canals

Treatment of obliterated canals as a result of calcifications is a challenge even for experienced endodontists. For that reason, the aim of this study was to present some cases from practice where, even in spite of almost complete canals obliteration, success in endodontic therapy has been achieved. In the cases presented, endodontic therapy was indicated due to symptoms, periapical lesions, or the need for prosthetic treatment with post placement. By working carefully and using EDTA, we achieve successful endodontic treatment and preserve tooth functionality.

Oral Health Status of Healthcare Workers in Ilembula/Tanzania during the COVID-19 Condition.

The challenge of reduced dental treatment and education infrastructure in the Tanzanian highlands affects the oral health situation of both the general population and local healthcare workers. The aim of this study was to investigate the oral health status of healthcare workers at Ilembula Lutheran Hospital (ILH), Tanzania, during the COVID-19 pandemic. In total, 134 healthcare workers (62 women, 72 men; mean age 36.48 ± 9.56 years, range 19-59 years; median age 35.00 years) participated in this cross-sectional study, conducted from 12 February to 27 February. A dental examiner trained in oral health screening performed the oral health data collection. Data collection was performed by probability sampling using the Ilembula Data Collection Form-Oral Health (IDCF-Oral Health) questionnaire distributed in paper form. Ethical approval was obtained from the National Institute for Medical Research/Tanzania. The decayed, missing, and filled teeth (DMF/T) index proposed by the World Health Organization (WHO) was used with the associated caries measurement method and the simplified oral hygiene index (OHI-S). Details regarding edentulism, nutritional habits, and socio-economic factors were collected. Statistical analysis was performed using linear regression (α = 0.05). The average DMF-T index was 3.33 ± 0.82, with age, gender, meal frequency, and soft drink consumption significantly influencing the index. No evidence of dental plaque was detected in 43.3% of the participants. Of the participants, 32.8% required prosthetic treatment (Kennedy Class III), while 16.4% needed it for acute malocclusions. Oral hygiene products were used in 97% of cases. A total of 35.8% of the participants had an OHI-S score of up to 1.0, with (p < 0.001) age and (p < 0.001) sex having a significant influence on the index. The current oral health situation of healthcare workers at ILH shows a moderate need for restorative and prosthetic treatment in rural Tanzania. Despite the COVID-19 pandemic, there was no change in the need for dental treatment, which may be explained by the generally restricted access to dental healthcare in the investigated region. The development of an interdisciplinary oral health prophylaxis system could help to reduce the need for future treatments.

Prosthetic status and Treatment need in Indian population:A Systematic Review

Introduction To examine the prevalence of partial or complete edentulousness in different geographic areas of the Indian population. Materials and Methods An electronic search in PubMed/Medline, Cochrane Library, ScienceDirect, Wiley Online Library, and Google Scholar was conducted to find relevant papers that had been written in English exclusively and published before May 2022. Based on the PICO, the primary research question for this study was ‘What are the prosthetic status and treatment needs of the adult population in India?’ By evaluating the title, abstract, and full text of the articles, if they met the inclusion criteria, the relevance of the articles was verified. The quality of the included studies was assessed using the Joanna Briggs Institute (JBI) tool for analytical cross-sectional studies. Result The database search yielded 278 items, of which 123 were discarded due to being irrelevant, duplicates, or lacking data. The remaining 155 full-text articles were assessed for eligibility 20 articles were yielded by manual search and 9 articles were added through a search of a reference list of relevant articles. A total of 151 full-text papers were evaluated for eligibility, with 5 articles being excluded. The current systematic review includes a total of 35 studies. Conclusion The current systematic review states the prevalence of complete edentulousness is comparatively less than partial edentulism, ranging between 0.8% to 14%, except for the tribal population where it is around 42-43%. On the other hand, figures depicting partial edentulism are between 15 to 98% distributed in the different geographical regions. The prosthetic status reported in the studies involved ranges between 3% to 100% with a mean of approximately 35%. Prosthetic treatment need reported overall was between 14% to 100%, in which requirement was found to be more in the maxilla in maximum of the studies included while only a few studies predicted more treatment need in the mandible. The requirement and correlation with gender cannot be established as the figures vary in all the studies included. This research establishes a baseline reference and highlights the lack of attention given to oral health. Key Words: Geographic areas, Population, Prosthetic treatment

Importance of Metalloproteinase 8 (MMP-8) in the Diagnosis of Periodontitis.

Periodontitis is a complex condition. Left untreated, it leads to tooth loss and the need for prosthetic treatment. The incidence of periodontitis is steadily increasing, so new methods are being sought to aid in the diagnosis of the disease. Among the methods postulated is the determination of concentrations of bioactive compounds which include extracellular matrix metalloproteinases (MMPs). These enzymes are present in various structural elements of the stomatognathic system. The most promising enzyme of this group appears to be metalloproteinase 8 (MMP-8). MMP-8 assays are performed in gingival fluid or saliva, and MMP-8 levels have been shown to be higher in patients with periodontitis compared to healthy subjects and correlated with some clinical parameters of the condition and the severity of the disease. In addition, the preliminary usefulness of this enzyme in evaluating the effectiveness of periodontal treatment and doxycycline therapy has been demonstrated. Determination of the active form of MMP-8 (aMMP-8) in oral rinse fluid using off-the-shelf assays shows the highest potential. Despite reports about aMMP-8 and promising data on the role of MMP-8 in periodontal diagnosis, a clear determination of the usefulness of this enzyme requires further research.

Comprehensive Geriatric Assessment and Quality of Life Aspects in Patients with Recurrent/Metastatic Head and Neck Squamous Cell Carcinoma (HNSCC).

To define frailty in older cancer patients, the aim of this study was to assess the geriatric status and quality of life (QoL) aspects in patients suffering from recurrent/metastatic head and neck squamous cell carcinoma (r/m HNSCC) under palliative treatment. A comprehensive geriatric assessment (CGA) was performed on 21 r/m HNSCC patients at two defined assessments, and the QoL aspects and the impact of descriptive data were evaluated. The Kolmogorov-Smirnov test, Spearman's rho correlation, and two-way mixed ANOVA were used for statistical analysis. All patients were found to be "frail". Pain, fatigue, and the burden of illness were the highest-rated symptoms. Oral function and orofacial appearance were highly impaired. A significant impact of descriptive data on the CGA and QoL results was found (all p ≤ 0.05). Thus, the CGA results revealed high frailty, severe comorbidities, and high impairments in QoL aspects. The CGA and QoL results were negatively affected by the primary HNSCC treatment approach, the need for prosthetic treatment, and worse oral functional capacity. Therefore, frailty in r/m HNSCC patients seems to be multidimensional. The evaluation of the CGA and QoL aspects in r/m HNSCC patients can be recommended to detect special needs, organize aftercare, and improve the support for frail and vulnerable cancer patients to create a multidisciplinary treatment approach.

Evaluation of Mobile and Community Dental Service Use among People Experiencing Homelessness

The aim of the study was to evaluate the patterns of mobile and community dental service use among people experiencing homelessness in Bratislava, Slovakia. Data from medical records of homeless people who visited the mobile and community dental clinic from November 2012 to July 2018 have been retrospectively reviewed. The experience of providing dental services has been reported from the perspective of the dental care provider. Descriptive statistics were used for data analysis. In total, 319 patients (75.5% men) attended the clinic. Extraction of a tooth was the main dental treatment indicated in more than 80% (n = 276) of patients. A total of 363 teeth were extracted with an average number of 1.6 extracted teeth per patient. The main indications for extraction were dental caries and its sequelae (83.7%) and periodontal disease (15.2%). The patients with the need for prosthetic and conservative treatment were referred to the clinics at University Hospital; however, only 19 patients received the treatment. The barriers to accessing dental care were cost, logistic problems and individual psychological factors. We found that people experiencing homelessness in Slovakia have high needs and demands of emergency dental care and many systemic and individual barriers prevent them from accessing care. Community-based dental services are important for improving access to dental care and reducing oral health inequities.

ENDOCROWN RESTORATION OF THE ENDODONTICALLY TREATED TEETH BY USING CAD/CAM: CASE SERIES

Aim: As a result of large coronal destruction in endodontically treated teeth, significant losses occur in the dentin tissue around the pulp and the need for prosthetic treatment occurs. In this case series, endocrown applications produced in a single session with CAD/CAM system are presented as an alternative to crown restoration traditionally applied to premolar and molar teeth with excessive crown destruction.&#x0D; &#x0D; Case Series: In first case, 38-years-old male patient number 25, 46-years-old female patient number 36 in our second case, 21-years-old female patient number 46 in our third case, 26-years-old male patient number 46 in our fourth case, and in the last case 52-years-old male patient tooth number 15 has been applied to root canal treatment. As a result of the clinical and radiological examinations of the patients, it was observed that there were no systemic diseases. Endocrown restoration was considered appropriate and recommended to patients with less dentin tissue. Firstly, teeth are prepared for endocrown restoration. Afterwards, the impressions were digitalized by scanning the jaws with an intraoral optical scanner (Cerec Omnicam, Dentsply Sirona, USA). The restorations were designed with the help of the CEREC 4.3 software (Dentsply Sirona, USA) and the milling process was carried out using the feldspathic ceramic block (Vita Mark II, Vita Zahnfabrik, Germany) in the same session by CEREC inLab MC XL (Dentsply Sirona, USA). Later, the restorations were glazed and cemented with dual polymerized resin cement (RelyX Ultimate, 3M ESPE, USA).&#x0D; &#x0D; Conclusion: Endocrown restorations are a minimally invasive, conservative treatment approach that provides mechanical adhesion to the pulp chamber and cavity walls with adhesive resin cement and allows the preservation of the remaining tooth structure. For this reason, it is a treatment option that can be preferred instead of post-core and crown application in teeth with root canal treatment.

Tooth loss, Prosthodontic Treatment need and Association Factors in A sample of Adults attending College of Dentistry,Al Mustansiriya University.

The aim of this study was to evaluate the level of tooth loss, the prosthetic treatment need and associated factors in a sample of the over 20-years old patients attending college of dentistry,AL Mustansiriya University. A sample of 584 individuals (306males and 278 females) was selected from prosthodontics department. The data were collected by means of an oral examination and interviewer questionnaire. The present study showed that the level of tooth loss more in male than female and increasing with age. A number of factors are involved in the decision to extract teeth, which are mainly dental caries and periodontal disease. Prevalence of denture wear is low and high need for prosthetic treatment especially in those with lower levels of education and lower income.Knowledge of factors that influence tooth loss and wearing dentures is important for the effective planning and provision of oral health services, particularly in prioritising prosthodontic care.

To Evaluate the Prevalence of Edentulousness, the Present Prosthetic Status, and the Need for Prosthetic Treatment in the Population of Garhwa, Jharkhand.

Aim:This study aimed to evaluate the prevalence of edentulousness, the present prosthetic status, and the need for prosthetic treatment in the population of Garhwa, Jharkhand.Materials and Methods:A descriptive cross-sectional study was conducted among the population of Garhwa, Jharkhand. The sample size included 460 subjects belonging to an age group of 25 years and above. Detailed case history and thorough dental examination was done and all the findings were recorded and compiled. Each subject was interrogated with the help of a predesigned closed ended questionnaire.Results:A total of six patients were completely edentulous, while 55.04% of males and 42.80% of females were partially edentulous. All patients who were completely edentulous were denture wearer, while among partially edentulous patients, 6.31% of males and 3.15% of females had RPD and 13.68% and 6.31% of males and females, respectively, had FPD as prosthesis. Prosthetic status of females was better than males In a comparison of prosthetic status between male and female, out of 460 subjects, 195 subjects opted for prosthesis, in which 117 were male and 78 were female.Conclusion:Community-based oral health education programs should be conducted to improve patients' oral health knowledge. Educating patients regarding the limitations of prosthesis as mechanical substitutes for natural teeth must be a continuing process from the initial patient contact until adjustments are completed.

Determinants of dental prosthetic treatment need: A birth cohort study.

To estimate the incidence of the need for a dental prosthesis during early adulthood and its association with socioeconomic and oral disorder trajectories. In 1982, in the city of Pelotas, all live births (n=5914) and their mothers were assessed. A representative sample of this cohort study was examined for oral health conditions at 15, 24 and 31years (Oral Health Study-OHS). The need for a dental prosthesis was assessed at 24 and 31years of age, and variables of interest (gender, skin colour, socioeconomic status (SES), use of dental services, caries and periodontal disease) were collected from different waves of this cohort. Longitudinal association between outcome and variables of interest was assessed using multilevel mixed models. A total of 539 individuals were assessed in 2013 (60.7% response rate). The need for a dental prosthesis was 28.9% at 24years. (95% CI 24.9-33.2) and 49.0% at 31years (95% CI 44.7-53.3). Risk of presenting with a need for a dental prosthesis was higher in users of public services. Downwardly mobile and lower SES trajectory groups presented a higher risk of the need for a dental prosthesis. High-risk caries trajectory group showed a higher risk of presenting with the need for a dental prosthesis from 24 to 31years. Our findings demonstrate that the need for dental prosthesis from 24 to 31years old was determined by trajectories of exposure during the life cycle, reinforcing that tackling socioeconomic inequalities at any stage of life can have an effect on an individual's oral health.

Dentistry 4.0 Concept in the Design and Manufacturing of Prosthetic Dental Restorations

The paper is a comprehensive but compact review of the literature on the state of illnesses of the human stomatognathic system, related consequences in the form of dental deficiencies, and the resulting need for prosthetic treatment. Types of prosthetic restorations, including implants, as well as new classes of implantable devices called implant-scaffolds with a porous part integrated with a solid core, as well as biological engineering materials with the use of living cells, have been characterized. A review of works on current trends in the technical development of dental prosthetics aiding, called Dentistry 4.0, analogous to the concept of the highest stage of Industry 4.0 of the industrial revolution, has been presented. Authors’ own augmented holistic model of Industry 4.0 has been developed and presented. The studies on the significance of cone-beam computed tomography (CBCT) in planning prosthetic treatment, as well as in the design and manufacture of prosthetic restorations, have been described. The presented and fully digital approach is a radical turnaround in both clinical procedures and the technologies of implant preparation using computer-aided design and manufacturing methods (CAD/CAM) and additive manufacturing (AM) technologies, including selective laser sintering (SLS). The authors’ research illustrates the practical application of the Dentistry 4.0 approach for several types of prosthetic restorations. The development process of the modern approach is being observed all over the world. The use of the principles of the augmented holistic model of Industry 4.0 in advanced dental engineering indicates a change in the traditional relationship between a dentist and a dental engineer. The overall conclusion demonstrates that it is inevitable and extremely beneficial to implement the idea of Dentistry 4.0 following the assumptions of the authors’ own, holistic Industry 4.0 model.

Needs for gerodontological treatment in the elderly living in Lower Silesia.

Many factors influence decisions regarding gerodontological treatment. Apart from the clinical condition of a patient, there is a complex of crucial socioeconomic factors, comorbidities, place of residence, and psychological aspects. Therefore, gerodontological treatment plans are significantly diversified. One of the goals of our cross-sectional epidemiological study of Lower Silesia seniors aged 65-74 was to identify all their needs related to gerodontological treatment. From the randomly selected sample group of 1,600 people, 387 inhabitants of Wrocław and Oława reported to take part in the study. The anamnestic study identified demographic and socioeconomic determinants, coexistence of general diseases and behavioral variables related to oral health behaviors. In the clinical study, the decayed-missing-filled (DMF) index, the community periodontal index (CPI), oral hygiene indices, clinical pathological lesions in oral mucosa, and the index of prosthetic reconstruction of missing teeth according to the World Health Organization (WHO) were determined. Criteria for the need for prosthetic treatment, dental caries treatment, periodontal disease treatment, improvement of oral hygiene, and treatment of mucosal diseases, including potentially pre-cancerous and cancerous disorders, were defined. As many as 95.6% of all respondents required at least 1 form of dental treatment. The most common need (75%) was prosthetic treatment of missing teeth either in the maxilla or in the mandible. Forty-nine per cent of the respondents needed treatment of caries in the clinical crown or root of at least 1 tooth. Further, these needs were related to the following factors: treatment of oral mucosal diseases (35.4%), professional periodontal treatment (35%), improvement of very bad oral hygiene (29.2%), and oncological treatment of pre-cancerous and cancerous lesions in the oral cavity (9.6%). The needs for gerodontological treatment found in the regional study of young Lower Silesian seniors are very high and cannot be met by services provided and funded by the state.

Prosthetic treatment need and associated life course determinants in partially edentulous adults of age 18-35 years in Udupi taluk: A cross-sectional study.

Aim:The aims of this study were to assess the prevalence of use and need for dental prostheses and to associate need with the life course determinants in young adults of age 18–35 years in Udupi taluk, Karnataka, India.Materials and Methods:It was a cross-sectional survey conducted on a total of 580 individuals those attended dental screening camps organized at random locations. A self-administered questionnaire was administered to participants to assess their life course determinants such as socioeconomic, behavioral, and psychological circumstances followed by an oral examination to assess their prosthetic status (WHO, 1997). Bivariate analysis followed by multivariate logistic regression analysis was carried out to determine the adjusted odds ratios (ORs) with 95% confidence interval (CI) for independent variables and the outcome.Results:The need and use of dental prostheses was observed in 38% and 2.2% of young adults, respectively. Adjusted multivariable analysis revealed that life course determinants such as parental rearing style (OR = 7.66 [95% CI: 3.88–15.14]) and interaction between expenditure at 10 years of age and economic hardships at 10 years of age (OR = 9.63 [95% CI: 3.12–29.72] and OR = 6.43 [95% CI: 1.89–21.88]) were significantly associated with the need for prostheses.Conclusion:The need for dental prostheses in the young adults can be related to socioeconomic and psychosocial circumstances during childhood, and thus the concept of life course approach has been highlighted.

The impact of a preventive and curative oral healthcare program on the prevalence and incidence of oral health problems in nursing home residents.

AimsTo assess the impact of an oral healthcare program in nursing homes on the initial treatment backlog and residents’ oral health stability.Materials and methodsThe study is a longitudinal cohort study in nursing home residents in Flanders, Belgium, to evaluate the oral healthcare programme Gerodent. The program consisted of: (1) the introduction of an oral healthcare team, (2) oral health education, (3) the implementation of oral health guidelines and protocols, and (4) regular visits of a mobile dental team. Data were extracted from the oral health records of 381 residents from 21 nursing homes who received treatments from the mobile dental team between October 2010 and March 2014 (mean follow-up period of 22.5 months). Oral health and treatment need between baseline and follow-up were compared.ResultsThe mean age at baseline was 82.4 years and the mean number of consultations per resident was 3.61 during the follow-up period. The proportion of residents with an oral treatment need was reduced from 65.9% to 31.3%. Among residents with natural teeth, there was significantly lower prevalence of caries (from 70.5% at baseline to 36.5% at follow-up; p<0.001), residual roots (from 54.2% to 25.1%; p<0.001), and need for fillings (from 31.9% to 17.1%; p<0.001) or extractions (from 64.3% to 31.6%; p<0.001). In the group with partial or full dentures (n = 223), 38.1% needed a repair, rebasing or renewal of their existing dentures at baseline and the respective figure at follow-up was 9.0% (p<0.001). In terms of oral health stability, 53% of the residents had no incident restorative and prosthetic treatment need throughout the follow-up period. A lower number of natural teeth at baseline (p<0.001) and a shorter follow-up period (p = 0.002) were associated with higher chances of oral health stability.ConclusionThe oral healthcare program Gerodent significantly reduced the treatment backlog and contributed to a considerable proportion of residents being stable in terms of oral health without any incident treatment needs.

ÍNDICE DE CPOD Y NECESIDAD DE TRATAMIENTO PROTÉSICO EN ADULTOS MAYORES DE LA PARROQUIA CHIQUINTAD

OBJETIVO: Determinar la frecuencia de la necesidad de tratamiento protésico y la frecuencia del índice CPOD en lospacientes adultos mayores de la parroquia de Chiquintad, Cuenca-Ecuador 2016. MATERIALES Y MÉTODOS: Se realizóun estudio descriptivo retrospectivo, la muestra estuvo formada por 400 adultos mayores de los cuales quedaron excluidos23 personas que no aceptaron el estudio teniendo una población de 377 personas adultas mayores con un rango de edadde 65 años a 106 años, quienes fueron evaluados clínicamente, mediante un cuestionario de la Organización Mundial de laSalud. RESULTADOS: Se determinó que la población evaluada es edéntula total, con un total de 302 pacientes (80,10 %) yedéntula parcial con un total de 75 pacientes (19,89 %). La frecuencia de la necesidad de tratamiento protésico, es alta en elsexo femenino con un 75 %, al igual que el sexo masculino con un 65% representando una necesidad de prótesis total. Lafrecuencia del índice CPOD es de 13,87 en personas edéntulas parciales. CONCLUSIONES: La población evaluada fue de377 adultos mayores, los cuales presentan edentulismo total en un 80,10%y parcial en un 19,89 %, la necesidad de tratamientoprotésico es necesaria en 302 personas de la población adulta mayor, las 75 personas restantes no lo requieren. Mediante elíndice de diagnóstico de piezas cariadas, perdidas y obturadas (CPOD), se encontró una mayor frecuencia de piezas pérdidascon 10,69 tanto en hombres como en mujeres.

Status i potrzeby leczenia protetycznego braków zębowych u osób w wieku 65–74 lat w województwie zachodniopomorskim w zależności od źródła finansowania

Wstęp: Według prognoz demograficznych liczba osób w wieku 65 lat i powyżej w skali globalnej osiągnie w 2030 r. miliard. Wiedza na temat epidemiologii statusu protetycznego i potrzeb leczenia protetycznego w populacji osób starszych jest niezbędna do ustalenia czynników ryzyka utraty zębów, a także przygotowania kompleksowego leczenia stomatologicznego oraz stworzenia odpowiednich programów profilaktycznych dla tej grupy badanych.Celem pracy było porównanie statusu protetycznego i potrzeb leczenia protetycznego braków zębowych u osób starszych w zależności od źródła finansowania ich leczenia stomatologicznego.Materiały i metody: Badanie przeprowadzono na grupie 294 osób (163 kobiety i 131 mężczyzn) w wieku 65–74 lat, mieszkających na terenie województwa zachodniopomorskiego. W badaniu klinicznym oceniono liczbę zachowanych zębów, średnią liczbę PUW i jej składowe, liczbę zachowanych stref podparcia napodstawie zmodyfikowanej klasyfikacji wg Eichnera, status protetyczny oraz potrzeby leczenia protetycznego braków zębowych wg klasyfikacji WHO (Genewa, 1997). Uzyskane dane porównano ze źródłem finansowania leczenia stomatologicznego badanych.Wyniki: Wśród badanych korzystających wyłącznie z usług stomatologicznych w ramach Narodowego Funduszu Zdrowia (NFZ), w porównaniu do osób korzystających tylko z usług stomatologicznych gabinetów prywatnych, stwierdzono: wyższy odsetek osób bezzębnych (27,4% vs 2,06%), niższy odsetek osób z zachowanym funkcjonalnym uzębieniem (15,07% vs 49,48%), wyższą średnią liczbę zębów usuniętych (18,14% vs 9,73%), wyższy odsetek osób nieposiadających stref podparcia (45,21% vs 10,31%), niższy odsetek osób użytkujących uzupełnienia stałe w szczęce (5,48% vs 29,9%) i w żuchwie (1,37% vs 17,53%), wyższy odsetek osób użytkujących protezę całkowitą w szczęce (34,25% vs 4,12%) i w żuchwie (23,29% vs 3,09%), wyższy odsetek osób wymagających uzupełnienia protetycznego bezzębia w szczęce (23,29% vs 2,06%) i w żuchwie (20,55% vs 3,09%).Wniosek: W badaniu stwierdzono istotnie gorszy stan zdrowia jamy ustnej u pacjentów deklarujących korzystanie wyłącznie z usług stomatologicznych wramach świadczeń gwarantowanych NFZ.Zwięzła wersja rozprawy doktorskiej przyjętej przez Radę Wydziału Lekarsko-Stomatologicznego Pomorskiego Uniwersytetu Medycznego w Szczecinie. Immediate loading of four interforaminal implants supporting a locator-retained mandibular overdenture in the elderly. Results of a 3-year randomized, controlled, prospective clinical study.

Implant-retained overdentures have become a standard option for the prosthetic treatment of the edentulous mandible in the elderly. This prospective study aimed to compare immediate and conventional loading of four interforaminal implants supporting a Locator-retained mandibular overdenture in elderly patients regarding implant survival, implant stability, and implant-related complications. The study population comprised 20 completely edentulous patients (11 males) aged 60 years and older with severe mandible resorption. Each patient received four interforaminal implants (Neoss Ltd., Harrogate, UK). Following randomization, implants were loaded either immediately after dental implant surgery or 3 months after implant placement with the Locator-abutment system. At follow-up visits 3, 6 12, 24, and 36 months after loading, implant stability was evaluated with Periotest and Ostell. Twenty patients received 80 implants. In eight patients, 32 implants were loaded immediately. Two patients had to be switched from the immediate to the conventional loading group due to insufficient primary stability (≤30 Ncm). Implant survival was similar in both groups after 36 months. No implant was lost. Decreasing Periotest, and accordingly, increasing Ostell measurements indicated adequate osseointegration in both groups. The course of treatment was not significantly different in the two groups. There were comparable incidences of postoperative complaints like swelling, hematoma, or wound dehiscence, as well as need for prosthetic treatment due to abutment loosening or occlusal discrepancies. Incidence of pressure marks and number of patient visits were significantly higher in the conventional loading group. With sufficient primary stability, immediate loading of four interforaminal implants in the edentulous mandible might be the preferential choice in the elderly, reducing total treatment time, and number of patient visits.

Dental prosthetic status and treatment needs of adult population in Jizan, Saudi Arabia: A survey report.

Objectives:The study aimed to evaluate and compare the dental prosthetic status and treatment needs of adult population in Jizan, Saudi Arabia, in relation to the age.Materials and Methods:A total of 1779 people aged 35–74 years from 4 survey areas (Hay al matar, Mahata khams, Dara ut tawhid, and Suq ad dakhili) selected through convenient sampling, around Jizan University, were surveyed, using the WHO survey criteria, 1997.Statistical Analysis:Number and percentages were calculated, and univariate analysis was performed using Chi-square test at 5% level of significance.Results:Different forms of prosthesis were present among patients in the upper (19.9%) and lower (19%) arches, respectively. Prosthetic treatment need was recognized in subjects, 56.4% for the upper and 57.2% for the lower arches, respectively. The prosthetic status and treatment needs differed statistically with respect to age.Conclusion:More than half of the surveyed adult populations were in need of some or the other forms of prosthesis. This study provides data for an oral health-care provider program for Jizan.

Interdisciplinary index of prosthodontic/substitution orthodontic treatment need for patients with missing teeth.

The need for treatment in cases of missing teeth may result from aesthetic demands or functional impairment, although tooth loss itself does not necessarily constitute a need for prosthetic replacement. In selected cases, restorative treatment can be replaced by tooth autotransplantation or substitution orthodontic treatment. The authors have tried to make an index based not on missing particular teeth, but on the presence of spacing requiring restoration. An attempt has been made to categorize the restorative treatment need. Orthodontic treatment was considered, when it could completely eliminate the need for prosthetic treatment. The proposed classification could be used for assessing eligibility for public refund of restorative or substitution orthodontic treatment, as well as to motivate the patients to have restorations. It should be an individual approach-based decision, which treatment: orthodontic substitution tooth movement or prosthodontic is more cost-effective for the rest of the patient's life.

The type and duration of use of removable dentures in seniors over 65 years of age from Lublin in Poland

Abstract Introduction. Age-related tooth loss causes an increasing need for prosthetic treatment. Maintaining a healthy masticatory system, as well as proper speaking functions is one of the main goals for prosthetic rehabilitation of the elderly. Aim. The authors aimed at checking what types of moveable dentures people over 65, residents of nursing and family homes tend to wear and how long do they do it. Material and methods. Some 240 people over 65 were looked at - 117 were residents of nursing homes in Lublin, while another 123 lived in their own family home. The authors took the following factors into consideration: type of denture, and how long were the dentures used. Results. It appeared that movable dentures were used by some 57.26% of nursing home residents and 69.11% of people living in their family homes. Residents used mostly full denture, both in maxilla and mandible, respectively 71.05% and 67.14%. Similarly, seniors living at home had worn mainly full maxillary denture in 50.00% of cases, and full mandibular denture in 44.44% of cases. The mean time of using maxillary and mandibular dentures was longer in seniors residing in nursing homes (maxilla - 7.24 years, mandible - 7.48 years) than in persons living with their families (maxilla - 5.39 years, mandible - 4.63 years). Conclusion. The supply of dentures in both groups of seniors is unsatisfactory. Most of the examined seniors have used the dental prostheses for too long.
