Immediate rehabilitation of severely atrophic maxillae using conventional and extended length subcrestal angulated (ELSA) implants: A retrospective analysis of 187 implants in 33 patients with up to three years of function.

The purpose of this study was to evaluate and determine clinical outcomes and survival rates of straight and extended length subcrestal angulated (ELSA) implants (20-24mm lengths) placed for immediate (up to 72h post-operative) or delayed rehabilitation (4 months post-operative) of severely atrophic maxillae. Prosthetic treatment consisted of interim and definitive implant-supported fixed implant-supported prostheses with up to 3 years of function. A retrospective analysis was conducted of 38 patients (five dropped out; final total was 33), treated between 2017 and 2019 in a private practice. Extended length subcrestal angulated (ELSA) implants and conventional endosseous straight implants (Southern Implants, Irene, South Africa) with (fixed) full arch prostheses were used to restore patients with edentulous maxillae immediately (within 72hs) or delayed (4 months). ELSA implants have subcrestal angulations of 24° and 36° of the restorative platforms, external hexagon crestal anti-rotation abutment connections, and lengths between 18 and 26mm. Implant loading was determined by implant insertion torque values (as determined on the surgical units); 120Ncm was the threshold level for immediate loading. Clinical and radiographic examinations were done that recorded the clinical outcomes of implants and prostheses. SPSS was used to process the data. Thirty-three patients and 187 implants were included with follow-up periods of at least 12 months (range 12-36 months). The mean age of the study population was 62.6 ± 8.4 years old (at the time of implant placement). Thirty-three patients (86.8%) were followed for 12 months; 13 patients (39.4%) were followed for 24-35 months; 9 patients (27.3%) were followed for 36 months. In total, 13 implants in six patients failed secondary to sinus infections. Mean bone levels (MBLs) were respectively: 0.88±2.12mm at loading, -1.53±2.03mm at 12 months, -2.26±1.45mm at 24 months, and -2.54±1.46mm at 36 months. The ELSA implants showed significantly lower (p = 0.014) MBL at 36 months than did the conventional implants. One hundred thirty-seven implants were placed and loaded within 72h; 50 implants were placed and loaded 4 months post placement. The combined implant survival rates were 93.0% at 12 months, 91.1% at 24 months, and 100% at 36 months. The ELSA implants showed significantly lower (p = 0.014) MBLs at 36 months when compared to the conventional implants. The results of this retrospective clinical chart review indicated that the use of ELSA implants placed into anterior maxillae and nasal crests with accentuated distal tilts (>30°) and simultaneous sinus augmentation provided favorable outcomes for prosthetic rehabilitation in patients with severe atrophic maxillae. ELSA implants placed with simultaneous sinus augmentation are an alternative option to zygomatic implants (ZI) when immediate loading is prescribed. Sinus infections were thought to be the proximate causes of all implant failures. Further long-term clinical studies are warranted with larger patient populations.

CAD - CAM in Pediatric Dentistry: A brief insight

On introduction of computer aided technology/computer aided manufacturing in pediatric dentistry, treating children has become effortless, mainly children with special health-care needs. This technology aims at the fabrication of restorations with higher strength and better aesthetics by an effortless, quicker and more precise operational process. This technology has proved to be a boon in various fields in pediatric dentistry. The latest trend of minimally invasive dentistry, relies on selective caries removal and restoring it with material which is highly adhesive and aesthetic. CAD-CAM makes use of an optical impression of the required area and occlusion which is taken using hassle-free intra-oral scanners. Machinable high strength composite/ceramic blocks are used which can be polymerized under pressure and temperature. The CAD-CAM technology has its advantage of needing less chair side time, brisk production of restorations with increased precision, eliminating the need for classic methods of impression, high strength and aesthetic restorations and prostheses. CAD-CAM technology can be used for smile designing in patients with developmental defects like Amylogenesis Imperfect, Molar-Incisor Hypo mineralization, etc., prosthetic rehabilitation in patients’ requiring stainless steel crowns and zirconia crowns, restorative procedures like inlays and onlays, naso-alveolar molding etc. Thus, it opens up a sea of possibilities for providing a high quality of treatment for the child with minimal hassle and maximal benefits to the child. Masticatory Function in Stage IV Periodontitis Patients Treated with Fixed Prosthetic Rehabilitations: A Case Series.

The present study assessed the impact of a fixed prosthetic rehabilitation on masticatory function in patients diagnosed with stage IV periodontitis. Eligible participants were adults in need of complex rehabilitation due to masticatory dysfunction. Masticatory function was evaluated using the two-colored chewing gum mixing ability test (VOH) at the diagnostic phase (T0), 1 week after delivery of the prosthetic prototype (T1), and 1 week after delivery of the final prosthetic solution (T2). Ten subjects were treated with a fixed prosthesis following periodontal and implant surgery using an individualized, fully digital workflow. Full-mouth plaque and bleeding scores, pocket depth, and clinical attachment level improved significantly. VOH was 0.472 ± 0.168 at T0, 0.358 ± 0.166 at T1, and 0.250 ± 0.123 at T2. A significant improvement in VOH was observed from T0 to T1 (difference: -0.114; 95% CI: -0.199 to -0.029; P = .014) and from T1 to T2 (difference: -0.108; 95% CI: -0.200 to -0.015; P = .027). From T0 to T2, VOH increased by 44.3%. Self-perceived assessment of masticatory function also improved from T0 to T2 (P = .002). The fixed prosthetic rehabilitation in patients with stage IV periodontitis allowed for a significant improvement in objective and subjective measurements of masticatory function.

OPPORTUNITIES FOR TREATMENT WITH A TEMPORARY OBTURATOR MADE OF THERMOPLASTIC FOIL

Prosthetic treatment with a temporary obturator is an intermediate stage in the overall prosthetic rehabilitation of patients with maxillary resection. According to prevailing opinions, treatment with a temporary obturator should start around 2 weeks to 2-3 months after resection and to continue until the final completion of the healing processes in the defect. Due to the dynamic changes in the defect, this treatment stage is associated with many difficulties, requiring frequent correction of the obturator and up to 12-14 clinical visits on average. The aim of the research is to track the opportunity for fabrication of a temporary obturator of thermoplastic foil with vacuum forming apparatus and its efficiency for restoring speech and fluid intake in patients with maxillary resection. For the aim of the study we conducted a treatment of a 45-year-old patient operated a week ago for maxillary carcinoma. As a result, the left alveolar bone and a part of the hard palate with a diameter of 10 mm were resected, with teeth 13 to 23, 16 and 17 preserved. The defect was padded with gauze and an impression with an irreversible hydrocolloid impression material was taken. In the laboratory a grade 4 plaster working model was cast on which the borders of the palatal plate were outlined. For the frontal teeth, these were the cervical areas of the teeth, and for the distal - their equators. In the area of the defect and the missing teeth the border reached the transitional fold, and distally - the soft palate. The plate was made from foil of 1 mm diameter which was drawn onto the model with a vacuum forming apparatus and was cut along the indicated borders. The palatal plate - obturator successfully closed the comunication to the nasal cavity and restored the opportunity for fluid intake, which was the main problem for the patient. The use of a foil with a thickness of 1 mm provided stability and resistance of the construction. As a result of this, the necessary hermetic seal of the defect was achieved and the speech of the patient was normalised. The described treatment methodology with a temporary obturator allows the successful restoration of speech and fluid intake during the healing processes in the defect. The main advantages of the method are the fast and easy fabrication, the opportunity for multiple corrections and the short period of adaptation.

Prosthetic rehabilitation in patients with jaw reconstruction by fibula free flap: A systematic review

This systematic review aimed to evaluate the dental prosthetic rehabilitation (DPR) in patients after jaw reconstruction with fibula free flap. Four databases were searched from January 2000 to January 2023. Of the 2507 studies identified, 36 observational studies were included. Cancer was the most common surgical indications for jawbone resection with 58.3 % of cases followed by benign tumours which representing 24 %. The DPR rate was estimated at 51.6 % across the studies (ranging from 38 % to 55 % depending on the benign or malignant nature of the tumors). Implant-supported prostheses represented 58.9 % of cases of which 66.9 % were fixed and 33.1 % were implant-stabilized overdentures. Virtual surgical planning (VSP) was used in 20 % of studies and aimed to improve the position of the grafted fibula, quality, and aesthetics of DPR and to decrease ischemia and the operating time. One in two authors performed DPR 12 months after jaw reconstruction. If implant survival rate reached 93 % in non-irradiated fibula, it fell to 38 %, 55 %, and 77 % if implantation occurred in the 12, 17, and 24 months after radiotherapy, respectively. Various parameters should be better investigated in further studies including the typology of the prostheses (implant-supported vs removable), the use of VSP, and the optimal time for DPR taking into account the characteristics of the tumor, the size of bone defect, and the need for external irradiation therapy

MAXILLOFACIAL REHABILITATION IN PATIENT WITH NEOPLASMS USING ZYGOMATIC IMPLANT-SUPPORTED PROSTHESES

Objective: The aim of this study was to report the clinical results of the use оf the zygomatic implants for prosthetic rehabilitation in patients with maxillectomy due to upper jaw tumors. Materials and Methods: The study included 12 patients who underwent prosthetic rehabilitation using a zygomatic implant after maxillectomy for the treatment of upper jaw tumors in the period from 2021 to 2023. There were 8 male patients and 4 female patients with an average age of 53.2 years old. The type of tumors was determined by CT, MRT methods and histopathological examination. The distribution of lesions was the following: benign 4 and malignant 8,7 patients have undergone radiation therapy before or after implant placement. After the tumor was removed, immediate surgical obdurators were placed. Main prosthetic rehabilitation performed 6-12 months after tumor removal, but before that, a temporary obturator had made and used. 6-12 months after tumor resection, 1-4 zygomatic implants were inserted into the zygomatic bone unilaterally or bilaterally. A total of 36 zygomatic implants were installed, 2 of which were unsuccessful and were removed in 1 patient. The implant systems used Zygoma TiUnite (Nobel Biocare, Zurich, Switzerland) and multi-unit abutments (MUA). The implants were placed using the surgical guide, which was planned and prepared digitally. After which they loaded immediately using prefabricated complete dentures. They verified, adapted and screwed by titanium bases onto the zygomatic implants. The palatal part of these prostheses is a separate unit, which attach to the main part of the prostheses with magnets. By detaching the palatal part of the prostheses, through the created opening, doctor or patient can keep track of the upper tissues, hygiene and antiseptic cleansing (irrigation, rinsing). Evaluation of functional efficiency of the treatment was assessed before and after prosthetic rehabilitation with zygomatic implants using the Oral Health Impact Profile (OHIP-14) and the masticatory functional index. QOL was evaluated using the University of Washington Quality of Life version 4 (UW-QOLv4) questionnaire. Results։ No postsurgical complications were seen, and the patients were discharged from the hospital after 7-10 days. The patients were able to return to a normal diet (hard) after just 7 days following surgery, with no further complaints regarding function or pain, apart from the residual swelling caused by the intervention. Patients' health-related quality of life (HR-QOL) before treatment was ≥48.3%. HR-QOL and overall quality of life after rehabilitation increased to 76.8%, and assessed as good. The improvement of chewing function was also the most important goal for the patients (pre- 26,4% and post- 67,3%), masticatory function scores increased after prosthetic treatment with implants. Conclusions: The use of prostheses fixed on zygomatic implants for patients with maxillary defects is an effective method of prosthodontic rehabilitation in complex clinical cases after maxillectomy

Development and Comparison of Conventional and 3D-Printed Laboratory Models of Maxillary Defects.

Recording accurate impressions from maxillary defects is a critical and challenging stage in the prosthetic rehabilitation of patients following maxillectomy surgery. The aim of this study was to develop and optimize conventional and 3D-printed laboratory models of maxillary defects and to compare conventional and digital impression techniques using these models. Six different types of maxillary defect models were fabricated. A central palatal defect model was used to compare conventional silicon impressions with digital intra-oral scanning in terms of dimensional accuracy and total time taken to record the defect and produce a laboratory analogue. Digital workflow produced different results than the conventional technique in terms of defect size measurements which were statistically significant (p < 0.05). The time taken to record the arch and the defect using an intra-oral scanner was significantly less compared with the traditional impression method. However, there was no statistically significant difference between the two techniques in terms of the total time taken to fabricate a maxillary central defect model (p > 0.05). The laboratory models of different maxillary defects developed in this study have the potential to be used to compare conventional and digital workflow in prosthetic treatment procedures.

Clinical outcomes of implant-supported prosthetic rehabilitation of severely atrophic maxilla: A systematic review.

The purpose of this systematic review is to evaluate the clinical outcomes for the various methods of rehabilitation of a severely atrophic maxilla with the help of implant-supported fixed prosthesis. The relevant publications published between 2013 and 2022 and written only in English were identified using an electronic search. The primary research question for this study was developed based on the PICO framework, which stands for population, intervention, control, and outcomes which was "What are the clinical outcomes of implant-supported prosthetic rehabilitation in patients with severely atrophic maxilla?" The relevancy of the articles was confirmed by examining their titles, abstracts, and complete texts to determine whether they satisfied the requirements for inclusion. Utilizing specialized study design-related bias assessment forms, the risk of bias was evaluated. The database search resulted in 1568 results; however, 1529 of them were eliminated because of insufficient, duplicate, or missing data. Additionally, manual searching yielded 11 articles. After 50 full-text papers were assessed for eligibility, 17 articles were eliminated. Thus, 33 studies in total are included in the current systematic review. Risk of bias analysis and GRADE evidence analysis were performed. Data were found to be heterogeneous and thus meta-analysis could not be done and narrative synthesis is presented. The patient's condition and the clinician's expertise play a role in taking the decision on choice of technique for the fixed implant-supported rehabilitation of the severely atrophic maxilla. A high success and survival rate is produced by the majority of fixed implant-assisted prostheses despite the biologic and prosthetic problems. A single approach cannot be recommended as the gold standard. The choice is dependent on the patient's biological factors as well as the clinician's expertise. The included studies were assessed using GRADE criteria. The quality of evidence is low-medium. Therefore, to better comprehend the clinical effectiveness of the treatment alternatives, more well-designed randomized controlled trials with longer follow-up period are required.

Use of Sectional Tray Design in Recording Edentulous Impressions for Microstomia Patients: A Case Report

Prosthetic rehabilitation of patients with microstomia presents a major challenge to dentist as limited oral opening makes access to the oral cavity for any dental procedure difficult. The primary hurdle during the fabrication of dentures in such patient is making an impression. This article describes the treatment of a patient with microstomia where maxillary and mandibular custom sectional impression trays were used for making definitive impressions.

Prosthetic Rehabilitation of Patients with Maxillary Defects Post Mucormycosis: Case Series

Mucormycosis has emerged as an epidemic within a pandemic of COVID 19 in India. It has emerged especially during second wave of COVID 19. Four major factors which are critical for the treatment of mucormycosis are rapidity of diagnosis, reversal of underlying predisposing factors (if possible), appropriate surgical debridement of infected tissue and appropriate antifungal therapy. Patient with maxillary resection present a challenging situation for the prosthodontist. Prosthetic rehabilitation is often difficult to achieve in such cases due to absence of many teeth and lack of favorable tissue undercuts. Obturator prosthesis should recreate the partition between oral and nasal cavities to restore oral function and esthetics. This case series presented the prosthodontic management of patients with hemimaxillectomy and premaxillary resection secondary to mucormycosis using maxillary interim obturator.

Prosthodontic management of monostotic fibrous dysplasia of the maxilla – A case report

Prosthetic rehabilitation of patients with acquired defects of the maxilla presents a challenging task for a prosthodontist. These goals are achieved by early diagnosis, definitive interdisciplinary management, and restoration of the defect with a definitive prosthesis. This case report describes the complete prosthodontic rehabilitation of a rare case of monostotic fibrous dysplasia of the right maxilla in a middle-aged woman.

Clinical and microbiological aspects of the auricle prosthetic reconstruction

Relevance. Plastic and reconstructive surgery of the missing outer ear is not always successful and may cause complications. Prosthetic reconstruction using silicone ectoprostheses (epitheses) is an alternative solution to this problem. Purpose – the study aimed to verify clinical and microbiological protocol of prosthetic rehabilitation of patients with a missing auricle using temporary silicone epitheses during implant osseointegration placed for the final magnet-bar retention system and assessment of the prosthetic reconstruction effe ctiveness.Materials and methods. The study compared and analyzed the quality of life and microbial colonization of the skin biotope in the main group patients – with temporary silicone auricle epithesis during extraoral implant osseointegration and in the control group without such prostheses. Quality of life was assessed before and after treatment using the automated questionnaires (WHOQOL-BREF and QL PAER) developed by us for mobile devices. We determined the quantitative and qualitative composition of the skin microbiota behind the ear in all patients with fixed silicone samples for seven days.Results. According to all domains of the "WHOQOL-BREF" and "QL PAER" questionnaires, an integral index of QLI (p &lt; 0.05) was reliably higher after prosthesis fixation in the main group. Skin biotope colonization by strains of resident microorganisms in the region of Technovent silicone epithesis increased if B-460 glue was applied, and the water-soluble G609 glue did not have a similar effect.Conclusion. A digital protocol for the manufacture of a temporary silicone epithesis of the auricle for the period of extraoral implant osseointegration, with the fixation on a water-soluble glue, improves the quality of life of patients with anotia and minimizes microbial colonization of the biotope of t he skin behind the ear.

Prosthetic Rehabilitation of Patient with Total Maxillectomy with Hollow Bulb Closed Obturator and Cast Partial Denture: A Clinical Report

Total maxillectomy is the complete resection of one of the two maxillae or resection to the midline. Surgical intervention creates communication between the oral cavity, nasal cavity and maxillary sinus. Postsurgical maxillary defects predispose the patient to hypernasal speech, leakage of food bolus and liquids into the nasal cavity, impaired mastication and various degrees of cosmetic deformities. Prosthodontic rehabilitation with obturator prosthesis restores the missing structures and acts as a barrier between the communications among the various cavities. Hollow bulb obturator with the maximum coverage of the defect aids in retention, stability, support with improved speech resonance and reduced weight on the unsupported side. This clinical report presents the prosthetic management of a patient having total maxillectomy on left side with definitive hollow bulb closed obturator and cast partial denture on the maxilla.

Window denture: An unconventional approach for the bulky edentulous ridges

Prosthetic rehabilitation in patients having prominent labial ridge with conventional complete denture is an uphill task. Severe labial ridge undercuts are commonly seen incompletely edentulous maxilla than in mandible. This may be attributed to the fact related to the proclined maxillary anteriors, indicated for extraction. Treatment approaches generally involve either a surgical or a non-surgical option. The non-surgical or the prosthetic option generally involves the fabrication of conventional complete denture, which may further compromise the esthetics due to excessive thickness of the labial flange. To challenge such situations, an unconventional approach is needed for fabricating a complete denture. This case report delineates a simple, economical, conservative and non-surgical treatment approach for fabrication of denture in a patient with excessive bulky maxillary ridge, to pronounce the final facial aesthetics of the patient.

Significance of site-specific radiation dose and technique for success of implant-based prosthetic rehabilitation in irradiated head and neck cancer patients-A cohort study.

Radiotherapy aggravates implant-based prosthetic rehabilitation in patients with head and neck cancer. To evaluate the impact of radiation dose at implant and parotid gland site for prosthetic rehabilitation. The retrospective study includes 121 irradiated head and neck cancer patients with 751 inserted implants. Radiation doses on implant bed and parotid gland site were recorded by 3-dimensional modulated radiation plans. Implant success was clinically and radiographically evaluated according to modified Albrektsson criteria and compared to treatment- and patient-specific data. Implant overall survival after 5 years was 92.4% with an implant success rate of 74.9%. Main reasons for implant failure were marginal bone resorption (20.9%), implant not in situ or unloaded (9.6%) and peri-implantitis (7.5%). A mean radiation dose of 62.6Gy was applied with a mean parotid dose of 35 Gy. Modulating radiation techniques went along with lower grades of xerostomia (p < 0.001). At implant site mean doses of 57.5, 42.0, and 32.3Gy were recorded for oral, oropharyngeal, and hypopharyngeal/laryngeal carcinoma, respectively. Implant success inversely correlated to radiation dose at implant site. Strong predictors for implant failure in uni- and multivariate analysis were implant-specific dose >50 Gy (HR 7.9), parotid dose >30 Gy (HR 2.3), bone (HR 14.5) and soft tissue (HR 4.5) transplants, bad oral hygiene (HR 3.8), nonmodulated radiation treatment planning (HR 14.5), and nontelescopic prosthetics (HR 5.2). Radiotherapy impedes implant success in a dose-dependent manner at implant site. Modern radiation techniques effectively reduce xerostomia favoring implant-based prosthetic rehabilitation. Implantation in bone grafts is more critical and telescopic-retained overdentures should be preferred.

Prosthetic Rehabilitation of Patient with Ocular Defects using Conventional Technique: A Systematic Review

This study's objective is to assess the current scientific evidence on the use of prosthetic rehabilitation of patients with ocular defects using the conventional technique. Electronic PubMed and Google Scholar searches were conducted to identify relevant articles from January 2015 until December 2019, focusing on rehabilitation of patients with ocular defects. There are 790 journals explained about this, with 80 articles met the inclusion criteria for the articles. They yielded 790 articles. A total of 80 studies were met inclusion criteria, with 315 patients as subjects. All of the studies were conducted in the past 5 years, and most conducted in Asia (75), followed by South America (3), Europe, and Africa (1). Most of the studies were using fabrication ocular prosthesis custom made, with 3 studies used stock ocular prosthesis and 16 studies used stock custom. There were 87,94% of studies used fabrication ocular prosthesis custom made. The aesthetic aspect of the custom-made ocular prosthesis was far better than the stock ocular prosthesis. It also a simple and cost-effective method and results in a more esthetically pleasing and accurate prosthetic outcome. Quality of life in irradiated patients with head and neck cancer: A preliminary study about the impact of prosthetic rehabilitation

BackgroundOral quality of life is of great importance in head and neck cancer, where each patient combines functional, social and esthetic needs. Our study aimed to evaluate the influence of prosthetic and/or maxillofacial rehabilitation on patients’ perceived oral quality of life. Material and MethodsThe General Oral Health Assessment Index (GOHAI) was used in 28 patients with head and neck cancer who had undergone radiotherapy, recruited at La Timone University Hospital, Marseille, France, and who required prosthetic rehabilitation. The questionnaire was completed at three timepoints in the study: before insertion of the prosthesis (T0), then one week (T1) and three months after insertion (T2). ResultsThe percentage of patients with poor quality of oral health decreased from 96.4% to 64.3% between T0 and T1. Between T0 and T1, the mean score of the psychosocial component of the GOHAI increased from 14.28 ± 4.51 to 20.14 ± 5.20 and the mean functional component score increased from 9.32 ± 3.86 to 12.07 ± 4.04. ConclusionsProsthetic rehabilitation appeared to have a positive impact on oral quality of life in our study subjects, particularly on social relations and self-esteem. Its influence on pain and discomfort remains to be clarified. This preliminary study gives a prospective view of the impact of prosthetic rehabilitation in patients with head and neck cancer who had undergone radiotherapy. Analysis of data yielded by cross-referencing of different questionnaires should make it possible to refine these results.
