Objective: Urinary tract infection is the second most frequent infectious pathology in childhood. This study seeks to establish the clinical and microbiological profile of urinary tract infections in pediatric patients at the Hospital Departamental Universitario del Quindío San Juan de Dios.
 Materials and methods: Retrospective descriptive observational study. By reviewing the medical records of patients between 0 and 16 years old with a diagnosis of urinary tract infection. Clinical, paraclinical and microbiological variables were obtained. An exploratory analysis was initially performed, followed by a multivariate analysis.
 Results: Data from 200 patients were included, predominantly female and infants patients. The main symptom at the time of consultation was fever. The main pathogen causing the infection was E. coli. The most used antibiotic was Amikacin (60.7%). ESBL was the most commonly found resistance profile (42.5%). 11 patients required antibiotic therapy guided by antibiogram, Amikacin being the most used (41.7%).
 Conclusions: Escherichia coli is the most isolated microorganism, presenting a low profile of bacterial resistance, which allows a better orientation of empirical antibiotic therapy. Amikacin was the most widely used antibiotic, showing very low resistance rates. Longer-duration regimens showed similar characteristics to shorter-duration regimens, which could support shorter regimens, but higher studies are required to prove this.

Epidemiology and Antibiotic Resistance of Enterobacteriaceae Isolated in Urinary Tract Infections at Oued Eddahab Military Hospital Agadir

Objective: Determine the current bacteriological profile of urinary tract infections and the antibiotic resistance of the main enterobacteria isolated in urine. Methods: Retrospective study over a period of 18 months from January 1, 2022 to June 30,2023 including all urinary cytobacteriological examinations of patients hospitalized in the various departments of the Oued Eddahab Military Hospital in Agadir, as well as outpatients. Results: Out of 4483 urine samples, 487 met the criteria for UTI (10.86% of total samples). The microorganisms were predominantly Enterobacteriaceae (421/487 86.44%). Of the infections, 75% originated from outpatients. The sex Ratio F/M was 1.35. E.Coli dominated the epidemiological profile for both inpatients and outpatients (63% vs. 77%). There was a high frequency of resistance to the main families of antibiotics, while quinolones, fosfomycin, nitrofurans, and aminoglycosides retained good activity. Resistance to third-generation cephalosporins through the production of extended-spectrum betalactamase was present in 09% of Enterobacteria. Finally, no resistance to imipenem and colistin was detected.

Bacterial profile of urinary tract infection among pregnant women: a systematic review

Urinary tract infection (UTI) is the infection and colonization of microorganisms in the urinary tract. Women carry a higher risk of being affected by UTI compared to males, especially during pregnancy due to physiological and hormonal changes. Appropriate and adequate management of UTI reduces the incidence of further maternal and fetal complications. Nonetheless, there is a growing concern regarding resistance to antibiotics among the bacteria causing UTIs in pregnant women. To identify the bacterial profile of UTIs during gestation by exploring the previous research conducted on this subject. A systematic online search was performed on scientific databases to obtain articles related to the current subject using search terms, including bacteria, profile, causative agent, sensitivity, resistance, symptomatic, pregnancy, gestation, and pregnant women. The articles of relevance were considered if they were original articles, written in the English language, available for full-text, and reporting bacterial profile of UTI during pregnancy. A total of eight articles were eligible and met the inclusion criteria. The studies were published between 2020 and 2023 and included a total number of 3,702 pregnant females from five different countries. The bacterial profile of UTI among pregnant women including symptomatic UTI, and resistance and sensitivity of bacteria to antibiotics varied between different regions. However, the causative agents of UTI displayed less variation as Escherichia coli and K.pneomniae were the major causative agents of UTI during pregnancy.

Urinary tract infections (UTIs) are common pediatric infections and contribute to high morbidity and mortality. At present, the antimicrobial resistance emergency has quadrupled worldwide and poses a serious threat to the treatment of patients. However, there have been few studies on UTIs in children in Ethiopia, particularly in the east. This study aimed to assess the bacterial profile of urinary tract infections, their susceptibility to antimicrobial agents, and associated factors in under-five children at Hiwot Fana Specialized University Hospital, eastern Ethiopia. We conducted hospital-based quantitative study on 332 consecutively selected under-five children from March 20 to June 10, 2021. Parents and guardians were interviewed to collect data using a structured questionnaire. Random urine samples were collected aseptically, and standard microbiological techniques were used to identify the bacteria and test for susceptibility to various antibiotics. Data were entered into Epi Info version 7 and exported to Statistical Package for the Social Sciences (SPSS) version 25 for analysis. Data were analyzed using descriptive analysis, bivariate, and multivariable logistic regression analysis. The crude odds ratio (COR) and adjusted odds ratio (AOR) with their respective 95% confidence intervals (CI) were used to determine the significance of the predictors. A p-value at a 95% confidence interval of less than 0.05 was considered statistically significant. The overall prevalence of bacterial urinary tract infections was 80 (24.1%) 95% CI:19.40-29.00%). Most of the bacterial isolates 55 (68.75%) were gram-negative bacteria, predominantly E. coli 23 (28.75%) and K. pneumoniae 10 (12.50%). Being a rural resident (AOR: 4.10, 95%CI: 1.45 11.54), uncircumcised male (AOR: 3.52, 95%CI: 1.33, 9.39), previous history of antibiotic usage (AOR: 7.32, 95%CI: 2.11, 25.37), indwelling catheterization (AOR: 10.35, 95%CI: 3.74, 28.63), previous history of urinary tract infections (AOR: 5.64, 95% CI: 1.36, 23.38), and urinary frequency (AOR: 5.56, 95%CI: 2.03, 15.25) had higher odds of culture positive result. The majority of the isolates have shown high levels of antibiotic resistance. Meropenem, ciprofloxacin, and amoxicillin-clavulanic acid were effective against gram-negative uropathogens, whereas rifampin and ciprofloxacin were the most sensitive drugs for gram-positive isolates. From the tested bacterial isolates, 53/86 (61.6%), 11/86 (11.6%), and 2/86 (2.3%) were found to have multidrug resistance (MDR), extreme drug resistance (XDR), and pan drug resistance (PDR), respectively. About one-fourth of the children were culture-positive for many types of bacterial uropathogens; this is higher compared with most of the previous studies in Africa. Rural dwellers, uncircumcised males, indwelling catheterization, a history of antibiotic use and urinary tract infection, and frequent urination all had a higher risk of bacterial infections. Many isolates were resistant to multiple drugs, primarily beta-lactams. Urinary tract infections as well as the growth and spread of resistant bacterial pathogens should be monitor regularly.

Epidemiological, Clinical, Bacteriological Profile of Urinary Tract Infection in the Internal Medicine Unit of the Fousseyni Daou Hospital in Kayes

Introduction: Urinary tract infection is one of the most frequent public health problems in the world and concerns all ages. Objective: To describe the epidemiological, clinical and biological profile of urinary tract infection in the internal medicine unit of the Fousseyni Daou hospital in Kayes. Methodology: It was a descriptive and cross-sectional study with retrospective data collection which took place from January 1 to December 31, 2020 at the Internal Medicine Unit of the Fousseyni Daou Hospital in Kayes. It covered all outpatients and hospitalized patients presenting with a urinary tract infection during the study period. Results: We identified 79 cases of urinary tract infection out of 145 requests for ECBU, that is a positivity rate of 53.10% and out of a total of 1883 admissions that is a hospital prevalence of 4.19%. The age groups of 36 to 45 years and 46 to 55 years were more represented in 28 cases (35.44%) and 25 cases (31.64%) with an average age of 45 years ± 10. The female sex was predominant, 42 cases (53.16%) with a sex ratio of 0.88. Housewives were more represented 28 cases (34.44%). The majority of patients resided in rural areas 49 cases (62.03%). The history was dominated by the following pathologies: hypertension 48.10%, diabetes 36.70% and gastropathy 30.37%. The main clinical manifestations were: Urinary burning 67 cases (84.81%), Dysuria 65 cases (82.27%), Pelvic pain 59 cases (74.68%), Fever 57 cases (72.15%). The epidemiological profile of the isolated strains was dominated by enterobacteriaceae, followed by gram-positive cocci and gram-negative bacilli. The main bacteria responsible for urinary tract infections in order of frequency: Escherichia coli (45.56%), Klebsiella pneumoniae (11.39%), Pseudomonas aeruginosa (8.86%), Staphylococcus aureus (7.59%), Streptococcus sp (6.33%), Enterobacter cloacae (3.79%), Acinetobacter sp (3.79%). The study of the susceptibility and resistance profiles of the isolated strains showed significant sensitivity of enterobacteriaceae to colistin and resistance to amoxicillin and to the amoxicillin + clavulanic acid combination. Conclusion: Urinary tract infections are a major public health problem since they are responsible for significant morbidity and mortality and a high cost of care. Knowledge of the epidemiological-clinical and bacteriological profile of these infections is essential for efficient management.

Microbiological Profile of Urinary Tract Infections in Males: A Cross-sectional Study

Introduction: Urinary Tract Infections (UTI) are uncommon among men. It is because the male urethra is longer and is away from the anal opening. Moreover, the perianal area is dry, and the prostatic secretion prevents from occurring of any infection. Hence, if men suffer from UTI, it is considered as a complicated infection. Complicated UTIs are difficult to treat and are more prone to treatment failures. Microorganisms causing these infections have a wide spectrum ranging from a variety of both Gram-negative and gram-positive organisms. Early diagnosis and treatment of UTIs, plays a major role in preventing the spread of infection to the upper urinary tract. Aim: To determine the microbiological profile and antibiotic sensitivity pattern from urine samples of male patients. Materials and Methods: The present cross-sectional study was done for a period of two years (1/3/2019-31/4/2021) in the Department of Microbiology, Malla Reddy Institute of Medical Sciences and Research, Jeedimetla, Hyderabad, Telangana, India. Urine samples from male inpatients and outpatients suffering from various clinical conditions received in the microbiology laboratory were processed and analysed. These samples were cultured by conventional semi-quantitative methods and the organisms were identified using standard biochemical reactions. Antibiotic sensitivity testing was done on Mueller Hinton agar by Kirby Bauer’s disk diffusion technique. Clinical history, associated conditions, and comorbidities were noted from the case sheets of all the culture-positive patients to identify the risk factors which made them prone to UTIs. The data was descriptive and percentages were calculated for all the numerical data obtained. Results: Out of 304 urine samples received, 104 (34%) were found to be positive for bacterial and fungal isolates. E.coli (44%) was the predominant organism, followed by Klebsiella spp (15%) among the Gram-negative bacteria isolated. Coagulase-negative Staphylococcus (5%) was the predominant organism among the gram-positive bacteria followed by Staphylococcus aureus (4%) and Enterococcus spp (4%). 84.8% of E.coli and 68% of Klebsiella spp were Extended Spectrum Beta Lactamase (ESBL) enzymes producing organisms. An analysis of the health conditions of the patients who were culture positive showed that 28% of patients had signs and symptoms suggestive of UTI. Diabetes (25%), Hypertension (20%), and Benign Prostatic Hypertrophy (BPH) (5%) were observed commonly in patients above 60 years. Conclusion: UTI in male patients are quite uncommon as in female patients. The microbiological profile and drug sensitivity pattern of these organisms causing the infection is similar to that in female patients suffering from UTI. But, as UTI in males is considered a complicated infection thorough investigation of these patients for various health conditions like diabetes mellitus, hypertension, and BPH has to be done, particularly in elderly males. This helps in initiating prompt treatment to prevent recurrent UTIs and drug resistance.

Clinical and Pathological Profile of Urinary Tract Infection in Children and Adolescents; A Hospital Based Study

Background: Urinary tract infections (UTIs) encompass a wide array of infections, accounting for a vast number of community as well as hospital-acquired infections. Aim of the Study: The aim was to analyse the clinical presentation of UTI in children and Adolescents between 6 year to 18 years of age and to analyse the causative microorganism and their drug susceptibility in urinary tract infection in children and Adolescents of the above age group. Methods: This retrospective was conducted at the Department of Pediatrics in Bangladesh Shishu Hospital, Dhaka, Bangladesh. A total of 51 patients were enrolled and analyzed in this study based on culture-positive urine isolates. The study duration was September 2021 to August 2022. The ethical committee of the institution approved the study protocol. Antimicrobial susceptibility was done for positive urine culture by the Kirby-Bauer disk diffusion method. Statistical analysis was done using Statistical package for social sciences (SPSS) software version 16. Result: This is a retrospective study; 98 children were enrolled and analyzed. The majority of the study population was aged 6-10 years (62.75%), 11(21.57%) patients were from the age group 11-15 years, and 8(15.69%) patients were from the age group 16-18 years. Conclusion: The most common clinical presentations of UTI in admitted patients are fever, dysuria and abdominal pain. Gram-negative bacilli were found to be responsible for UTI, and the most frequently isolated bacteria were E-coli.

A Study On Clinical Profile of Urinary Tract Infection in Diabetes Mellitus in a Tertiary Care Hospital

Background: Diabetic Patients are at high risk of developing various infections especially if the diabetes is uncontrolled. Urinary tract infections (UTI) are one of the common infections in diabetics. Knowledge about the clinical and microbiological profile of UTI in diabetic patients will not only aid in early diagnosis but also helps in early initiation of appropriate empirical antibiotic therapy ensuring early recovery. Material & Methods: Prospective hospital based observational study. The study was conducted in the In-patient department, Department of General medicine at KMCT Medical college, Manassery, Kozhikode, Kerala, India. This study was conducted in 170 Diabetic patients above 18 years of age with symptoms of UTI with significant pyuria who were admitted under department of General Medicine, KMCT medical college, Kerala, who satisfied the inclusion criteria and gave consent for study. study consisted a total of 170subjects. Simple Random sampling method. Results: Fever was the most common clinical presentation followed by dysuria. E. coli was the most commonly isolated organism followed by Klebsiella. Most of the organisms were sensitive to Amikacin. Antibiotic resistance and Pyelonephritis were higher in patients with poor glycaemic control. The most common ultrasonographic abnormality was prostatomegaly. Conclusion: Patients with poor glycaemic control are at high risk of resistant UTI and Pyelonephritis. Good glycaemic control with routine monitoring of HbA1C is mandatory in diabetic patients to prevent UTI and to avoid its complications. E. coli was the most common organism isolated followed by Klebsiella. Most of the organisms were sensitive to Amikacin. Hence, Amikacin can be recommended as empirical antibiotic forDiabetic patients who are admitted with UTI.

Background: Urinary tract infection (UTI) is the most common bacterial infection which is seen in childhood population, which starts from the calyces of the kidney to the urethra. It is an important cause of morbidity and mortality in children mostly in the first 2 years of life. The Gram negative enteric bacilli, especially Escherichia coli and Klebsiella spp. the dominant pathogens. The objective of the study was to analyze the causative microorganism and their Antibiotic Resistance Pattern in UTI in childhood population between 0-12years of the age. Methods: This one year prospective and observational study was conducted in the Department of Microbiology; Rama Medical College Hospital & Research Centre Kanpur, Uttar Pradesh, India, on 100 pediatric patients (aged 0-12 years). All the pediatric age group (0-12 years) with suspected UTI and, whose urine culture showed one or two pathogen with colony counts greater than or equal to 105 colony forming units (CFU)/ml was included for the study. Clinical data was obtained from OPD and IPD Patients. Antimicrobial susceptibility was done for positive urine culture by Kirby-Bauer disk diffusion method. The data was expressed in terms of frequency and percentage.

Bacteriological Profile and Antibiotic Resistance Pattern in Children with Urinary Tract Infection in a Tertiary Care Hospital

Introduction: Urinary tract infection (UTI) is one of the most common bacterial infections seen in children. The knowledge of bacterial causes of UTIs and their antimicrobial resistance patterns may help clinicians in choosing the appropriate antimicrobials.
 Objectives: The present study was designed to find out the bacteriological profile of urinary tract infection and their antibiotic resistance pattern in children.
 Methodology: A cross sectional study was done in the department of Microbiology, Sir Salimullah Medical College from January to December 2019. Midstream urine samples were collected from clinically suspected cases of UTI in the age group of 0 to 18 years from various indoor and outdoor patients of Mitford hospital for this study.
 Results: Out of 2763 cases, 363 were culture positives, where females were 202(55.65%) with highest number of cases within the age group of 0 to 4 years (63.36%). Most common isolate identified was Escherisia coli, followed by Pseudomonas, Proteus, Morganella and only Gram positive bacteria was S. aureus. Organisms show highest resistance to Amoxiclav, Cefixime, Cotrimoxazole, Ciprofloxacin and highest sensitivity to Amikacin.
 Conclusion: This study revealed that UTI was more common in female & under five children. Escherichia coli were the commonest isolated organism and all isolates show high resistance to common antimicrobial agents.

Bacteriological profile of urinary tract infections and antibiotic susceptibility of Escherichia coli in Algeria.

Background and Objectives:Urinary tract infections are one of the most common bacterial infections worldwide. The emergence of antibiotic-resistant bacterial strains is a serious problem and greatest challenge in public health care. The purpose of this study was to determine the prevalence of uropathogenic microorganisms and the antibiotic resistance pattern of Escherichia coli in Algeria.Materials and Methods:Urine samples were collected from 760 outpatients in the hospital of Tizi-Ouzou (Algeria). From the positive cultures, 120 strains of E. coli were isolated and tested for their susceptibility to antibiotics by disk diffusion method on Mueller Hinton agar medium.Results:Among the collected urine specimens, 270 (35.5%) yielded positive cultures for urinary tract infection. Females were more affected with a sex ratio F/M of 1.14. E. coli was the most prevalent isolated bacteria with a rate of 44.44%, followed by Klebsiella pneumoniae (12.21%), Pseudomonas aeruginosa (11.1%) and Proteus mirabilis (5.55%). Isolates of E. coli showed high level of resistance to cephalothin (85.83%), ticarcillin (82.5%), ampicillin (73.3%) and amoxicillin-clavulanic acid (58.33%). Imipenem was the most effective antimicrobial agent.Conclusion:These results highlight the inappropriate utilization of antibiotics and suggest the need to improve prescription practices in our country.

Bacteriological Profile of Urinary Tract Infections at the Avicenne Military Hospital in Marrakech

Urinary tract infection is the attack of a tissue of the urinary tract by one or more micro-organisms. It is a major public health problem. The objective of this work is to propose, based on updated data, the microbial ecology of urinary tract infection at the Avicenne Military Hospital in Marrakech, and to follow the antibiotic sensitivity profile of uropathogenic bacteria. This is a retrospective descriptive study over 4 years from January 2014 to December 2018, which was conducted on urinary cytobacteriological examinations from hospitalized patients and outpatients, treated at the microbiology laboratory of the Military Hospital Avicenne of Marrakech. Of the 17607 ECBUs that were examined, the diagnosis of urinary tract infection was retained in 2349 (13%). 75% of the ECBUs came from hospitalized patients and 25% from outpatients, with a sex ratio of 1.1. The analysis of the ECBU showed that Enterobacteriaceae constituted 80% of the positive urine isolates, with a predominance of Escherichia coli (62%), followed by Klebsiella pneumoniae (13%). The reading and interpretation of the antibiograms showed that Escherichia coli was resistant to aminopenicillin associated with clavulanic acid in 51% of the cases against 55% for Klebsiella pneumoniae. The resistance of enterobacteriaceae to fluoroquinolones is about 33% for Escherichia coli, 30% for Klebsiella spp, while for the Trimmethoprim+Sulfamethoxazole combination more than half of the strains were resistant. On the other hand, aminoglycosides still have a good activity profile on enterobacteriaceae. The prevalence of BMR is 6.5%, represented by enterobacteriaceae producing extended spectrum betalactamases (ESBL) isolated in 89% of cases, with a predominance of Escherichia coli (52%) followed by klebsiella pneumoniae (22%) and Enterobacter cloacae (16%). Then the ceftazidime-resistant Pseudomonas aeruginosa (PARC) occupies the 2nd place with 5%, and finally methicillin-resistant Staphylococcus aureus (MRSA) and imipenem-resistant Acinetobnacter baumanii (ABRI) which occupy the 3rd place with rates at 3%. These BMR constitute a worrying problem, hence the need for rigorous application of hygiene rules and rational prescription of antibiotics. The knowledge of bacteriological profiles and the use of targeted antibiotic susceptibility testing will allow a management adapted to each hospital context.

Clinical and bacteriological profile of urinary tract infection in children at a tertiary care hospital

Background: Urinary tract infection (UTI) is common in children and is an important cause of morbidity. UTI at young age can lead to renal injury and scarring, and ultimately lead to end stage renal disease in adulthood. Aim of the study The objectives of this study were to study the clinical and bacteriological profile of UTI in children.Methods: This prospective study was done in Sher-E-Bangla medical college hospital, Barishal from January 2018 to July 2019. A urine sample was included in our dataset if it demonstrated pure growth of a single organism and accompanying antimicrobial susceptibility and subject demographic data were available.Result: UTI was more common in female (70.0%) than in male (33.0%). Half were in the age group 1-5 years. Fever was the most common presentation (64.0%) followed by abdominal pain (42.0%), dysuria /increased frequency (34%), decreased appetite (28%). Escherichia coli was the most common (64%) bacterial isolate followed by Klebsiella sp. (20.0%) and Proteus sp. (14.0%). E. coli was highly sensitive to ofloxacin, cefotaxime and amikacin (94.0%). Klebsiella was 100% sensitive to ciprofloxacin and amikacin. Greater degree of resistance was seen to ampicillin, cotrimoxazole and nalidixic acid.Conclusions: Though various microorganisms are responsible for UTI in children, E. coli is the most common causative agent. Antimicrobial resistance has already emerged against many antibiotics, making empiric treatment of these infections challenging.

Bacterial Profile of Urinary Tract Infections and Antimicrobial Susceptibility Patterns among Pregnant Women at the Benghazi Medical Centre, Benghazi/Libya

Urinary tract infection (UTI) is one of the most common diseases caused by bacteria during pregnancy, and it can create serious consequences for both the fetus and the mother, necessitating early detection and prevention of problems. The study aimed to isolate and identify urinary tract infections bacteria among pregnant women at the Benghazi Medical Centre. Initially, a sectional descriptive design was used on 200 pregnant women who attended the Obstetrics and Gynecology Clinic at the Benghazi Medical Centre from June 2018 to October 2018. A purposive sample was employed to collect data utilizing a structured interviewing questionnaire and laboratory test records. Bacterial infection during pregnancy was more frequent within the age range of 25 to 35 years with 25 out of 44 patients (56.8%). The second trimester of pregnancy was the time period most related to the infection with 23 patients (52.3%). The results also showed that housewives were more susceptible to the infection (28 patients; 63.6%). Several varieties of bacteria were related to UTIs during pregnancy including Escherichia coli (56.8%), which was the most frequently isolated pathogen followed by S. aureus (25%), Enterococcus (9.1%), Klebsiella (6.8%) and Staphylococcus saprophyticus (2.3%). Antibiotic sensitivity revealed that the response to antibiotics was variable; most of the isolated pathogens were sensitive to the tested antibiotics specifically to ciprofloxacin and levofloxacin. The isolated bacteria that were sensitive to all the used antibiotics included Escherichia coli and S. saprophyticus. Antibiotic resistance was increasingly seen in amoxicillin, azithromycin, and erythromycin. To avoid UTIs during pregnancy and the complications of infection, which could lead to fetal loss, all pregnant women should be routinely screened for UTIs with urine culture and treated with the appropriate antibiotics as needed as well as reducing the risk of developing a urinary tract infection by increasing the level of cultural awareness. Pathogen profile of urinary tract infections in Nephrology Unit

Abstract Introduction. Urinary tract infection (UTI) is one of the most common types of infection in both hospitalized and outpatient settings. The etiology is mostly bacterial, and the typical causative agent is uropathogenic Escherichia coli. There is a noticeable increase in drug resistance of pathogenic microorganisms. The aim of the study was retrospective analyses of etiological agents of UTI and their antibiotic resistance patterns in Nephrology Unit patients. Material and methods. An infection was diagnosed based on the patient’s symptoms and positive results of urine culture, carried out over 26 months. The clinical material was tested by using the VITEK system, the drug susceptibility of the emerged pathogens was identified. Results. The most common etiological agents of UTI were Gram-negative rods: Escherichia coli (51.23%), Klebsiella spp. (19.3%) and Proteus spp. (13.68%). The analysis of drug resistance profiles of these pathogens showed a high percentage of strains resistant to broad-spectrum penicillins and fluoroquinolones. At the same time, it seems that E. coli isolates presented the most favorable pattern of drug susceptibility in this comparison. Conclusions. The alarming tendency of increasing drug resistance among pathogens causing UTIs to antibiotics such as penicillins or fluoroquinolones prompts a careful choice of drugs in empirical therapies. The most appropriate practice in this regard seems to be meticulous control of nosocomial infections and making therapeutic decisions based on the knowledge of local microbiological data.

Clinicoetiological profile of urinary tract infection in pediatric population in a teaching hospital in south India

Background: Urinary tract infection (UTI) is one of the most common bacterial infections seen in children. A wide range of organisms with varying antibiotic sensitivity patterns have been known to cause UTI. The objective of the study was to analyse the clinical presentation of UTI in children between 1-month to15 years of age and to analyse the causative microorganism and their drug susceptibility in UTI in children between 1 month to 15 years of age.Methods: A retrospective study was conducted in a teaching hospital in Kerala, between July 2018 and June 2020 among children 1 month to 15 years of age who presented with symptoms of UTI. Patients who had culture positive UTI were included in the study. Clinical data was obtained from inpatient and outpatient records. Antimicrobial susceptibility was done for positive urine culture by Kirby-Bauer disk diffusion method. Statistical analysis was done using Statistical package for social sciences (SPSS) software version 16.Results: Of the 1057 urine samples analysed, 18.44% had significant bacteuria. 43.07% were children less than one year of age with male predominance. Fever and dysuria were the most common clinical presentation. E. coli was the most prevalent pathogen isolated followed by Klebsiella pneumoniae. Enterococcus faecalis was the only gram-positive bacilli isolated. Highest resistance was shown to ampicillin, third generation cephalosporins and co-trimoxazole. Least resistance was shown to nitrofurantoin, fluoroquinolones, aminoglycosides, piperacillin- tazobactam and carbapenems.Conclusions: Regular surveillance programme is necessary for implementation of guidelines for empiric treatment of UTI.
