Associations Between Auditory Working Memory, Self-Perceived Listening Effort, and Hearing Difficulty in Adults With Mild Traumatic Brain Injury.

Mild traumatic brain injury (TBI) can have persistent effects in the auditory domain (e.g., difficulty listening in noise), despite individuals having normal pure-tone auditory sensitivity. Individuals with a history of mild TBI often perceive hearing difficulty and greater listening effort in complex listening situations. The purpose of the present study was to examine self-perceived hearing difficulty, listening effort, and performance on an auditory processing test battery in adults with a history of mild TBI compared with a control group. Twenty adults ages 20 to 53 years old participated divided into a mild TBI (n = 10) and control group (n = 10). Perceived hearing difficulties were measured using the Adult Auditory Processing Scale and the Hearing Handicap Inventory for Adults. Listening effort was measured using the National Aeronautics and Space Administration-Task Load Index. Listening effort ratings were obtained at baseline, after each auditory processing test, and at the completion of the test battery. The auditory processing test battery included (1) dichotic word recognition, (2) the 500-Hz masking level difference, (3) the Listening in Spatialized Noise-Sentences test, and (4) the Word Auditory Recognition and Recall Measure (WARRM). Results indicated that individuals with a history of mild TBI perceived significantly greater degrees of hearing difficulty and listening effort than the control group. There were no significant group differences on two of the auditory processing tasks (dichotic word recognition or Listening in Spatialized Noise-Sentences). The mild TBI group exhibited significantly poorer performance on the 500-Hz MLD and the WARRM, a measure of auditory working memory, than the control group. Greater degrees of self-perceived hearing difficulty were significantly associated with greater listening effort and poorer auditory working memory. Greater listening effort was also significantly associated with poorer auditory working memory. Results demonstrate that adults with a history of mild TBI may experience subjective hearing difficulty and listening effort when listening in challenging acoustic environments. Poorer auditory working memory on the WARRM task was observed for the adults with mild TBI and was associated with greater hearing difficulty and listening effort. Taken together, the present study suggests that conventional clinical audiometric battery alone may not provide enough information about auditory processing deficits in individuals with a history of mild TBI. The results support the use of a multifaceted battery of auditory processing tasks and subjective measures when evaluating individuals with a history of mild TBI.

Emotional processing deficits in individuals with problematic pornography use: Unpleasant bias and pleasant blunting.

A growing body of evidence indicates a connection between emotional processes and the emergence and progression of addiction. However, scant research has examined the involvement of emotional processing within the framework of problematic pornography use (PPU). This study aimed to examine the electrophysiological and subjective differences in emotional processing between male individuals with PPU and healthy controls (HCs) following exposure to everyday affective images. Event-related potentials (ERPs) were recorded from 42 PPU participants (mean age = 20.14 years, SD = 1.35) and 45 HCs (mean age = 20.04 years, SD = 1.45) during an oddball task, in which unpleasant, pleasant, and neutral images were presented as deviant stimuli, while a neutral kettle image served as the standard stimulus. The Self-Assessment Manikin (SAM) was employed to assess participants' subjective experience on the dimensions of valence and arousal. Regarding subjective measures of emotion, individuals with PPU reported lower valence ratings for unpleasant images compared to HCs. In terms of electrophysiological measures of emotion, PPU participants reported larger P2 amplitudes for unpleasant pictures compared to both pleasant and neutral pictures. Moreover, HCs showed enhanced P3 amplitudes in response to pleasant images compared to neutral images, whereas this effect was not observed in PPU participants. These findings indicate that individuals with PPU may display deficits in emotional processing characterized by enhanced responsiveness to negative stimuli and attenuated responsiveness to positive stimuli. The heightened sensitivity to negative stimuli may contribute to the inclination of individuals with PPU to engage in pornography as a coping mechanism for stress regulation. Conversely, their diminished sensitivity to positive stimuli presents a challenge in seeking alternative natural rewards to counter potentially addictive behaviors.

Clinical Gaps-in-Noise Measures in Blast-Exposed Veterans: Associations with Electrophysiological and Behavioral Responses.

It has been established that blast exposure and brain injury can result in self-reported and measured auditory processing deficits in individuals with normal or near-normal hearing sensitivity. However, the impaired sensory and/or cognitive mechanisms underlying these auditory difficulties are largely unknown. This work used a combination of behavioral and electrophysiological measures to explore how neural stimulus discrimination and processing speed contribute to impaired temporal processing in blast-exposed Veterans measured using the behavioral Gaps-in-Noise (GIN) Test. Results confirm previous findings that blast exposure can impact performance on the GIN and effect neural auditory discrimination, as measured using the P3 auditory event-related potential. Furthermore, analyses revealed correlations between GIN thresholds, P3 responses, and a measure of behavioral reaction time. Overall, this work illustrates that behavioral responses to the GIN are dependent on both auditory-specific bottom-up processing beginning with the neural activation of the cochlea and auditory brainstem as well as contributions from complex neural networks involved in processing speed and task-dependent target detection.

Large-conductance calcium-activated potassium channel haploinsufficiency leads to sensory deficits in the visual system: a case report

BackgroundMutations in the genes encoding the large-conductance calcium-activated potassium channel, especially KCNMA1 encoding its α-subunit, have been linked to several neurological features, including intellectual disability or autism. Associated with neurodevelopmental phenotypes, sensory function disturbances are considered to be important clinical features contributing to a variety of behavioral impairments. Large-conductance calcium-activated potassium channels are important in regulating neurotransmission in sensory circuits, including visual pathways. Deficits in visual function can contribute substantially to poor quality of life, while therapeutic approaches aimed at addressing such visual deficits represent opportunities to improve neurocognitive and neurobehavioral outcomes.Case presentationWe describe the case of a 25-year-old Caucasian male with autism spectrum disorder and severe intellectual disability presenting large-conductance calcium-activated potassium channel haploinsufficiency due to a de novo balanced translocation (46, XY, t [9; 10] [q23;q22]) disrupting the KCNMA1 gene. The visual processing pathway of the subject was evaluated using both electroretinography and visual contrast sensitivity, indicating that both retinal bipolar cell function and contrast discrimination performance were reduced by approximately 60% compared with normative control values. These findings imply a direct link between KCNMA1 gene disruption and visual dysfunction in humans. In addition, the subject reported photophobia but did not exhibit strabismus, nystagmus, or other visual findings on physical examination.ConclusionsThis case study of a subject with large-conductance calcium-activated potassium channel haploinsufficiency and photophobia revealed a visual pathway deficit at least at the retinal level, with diminished retinal light capture likely due to bipolar cell dysfunction and an associated loss of contrast sensitivity. The data suggest that large-conductance calcium-activated potassium channels play an important role in the normal functioning of the visual pathway in humans, and that their disruption may play a role in visual and other sensory system symptomatology in large-conductance calcium-activated potassium channelopathies or conditions where disruption of large-conductance calcium-activated potassium channel function is a relevant feature of the pathophysiology, such as fragile X syndrome. This work suggests that the combined use of physiological (electroretinography) and functional (contrast sensitivity) approaches may have utility as a biomarker strategy for identifying and characterizing visual processing deficits in individuals with large-conductance calcium-activated potassium channelopathy.Trial registration ID-RCB number 2019-A01015-52, registered 17/05/2019.

Olfactory and Neuropsychological Functioning in Olfactory Reference Syndrome

Olfactory reference syndrome (ORS) is an underrecognized, understudied, and often severe psychiatric disorder characterized by a prominent and distressing or impairing preoccupation with a false belief of emitting an offensive body odor. As this condition has only recently been recognized in the International Classification of Diseases (the 11th Edition), no empirical evidence exists about the underlying features and etiology of the disorder. To examine the neuropsychological and olfactory functioning of individuals with ORS and address whether there is central nervous system or sensory dysfunction associated with the condition. In this preliminary investigation, 9 consecutive participants with ORS completed a structured clinical interview and neuropsychological and olfaction evaluations. A proportion of individuals with ORS displayed deficits in aspects of cognitive functioning (i.e., processing speed, executive functioning, recognition memory bias for ORS-related words), olfaction functioning (i.e., odor detection and discrimination), and emotional processing. Based on these preliminary findings of cognitive, olfaction, and emotional processing deficits in individuals with ORS, further neuropsychological and olfaction studies are needed that better characterize this understudied patient group and address this study's limitations.

Humour processing deficits in individuals with social anhedonia

Humour processing comprises the humour comprehension and the humour appreciation phases. Patients with schizophrenia have impaired humour processing. However, it is unclear whether such deficits affect subclinical populations such as individuals with social anhedonia. Our study recruited forty-eight individuals with high levels of social anhedonia (HSA, screened by the Revised Chapman Social Anhedonia Scale) and 50 individuals with low levels of social anhedonia (LSA). Participants completed behavioural tasks which tapped into humour comprehension and appreciation, and a set of questionnaires assessing their sense of humour, humour styles and subjective experiential pleasure. Using signal detection theory analysis, the d’ and β values were generated to measure the detection of humour signal in the comprehension phase and the inner criteria of the humour appreciation respectively. The results showed that the HSA and LSA groups did not differ in humour signal detection (d’) but the HSA group had significantly higher inner criteria of humour appreciation (β) than the LSA group. The β value was correlated with experiential anticipatory pleasure in all participants. The HSA group had significantly lower within-group coherence than the LSA group when processing humour. Our findings suggested that individuals with social anhedonia have impaired humour processing.

A Semantic Priming Event-related Potential (ERP) Task to Study Lexico-semantic and Visuo-semantic Processing in Autism Spectrum Disorder.

Individuals with autism spectrum disorder (ASD) have characteristic deficits in understanding the meaning of language, or semantic processing. However, some evidence indicates that semantic processing of non-linguistic stimuli is intact, suggesting that semantic deficits may be language-specific. To appropriately characterize semantic processing deficits in individuals with ASD, comparison of within-modality linguistic (e.g., written words) and non-linguistic (e.g., pictures) stimuli is required. This paper describes such a methodology that makes use of a semantic priming paradigm during concurrent recording of electroencephalographic (EEG) data. EEG provides a dynamic measure of brain activity that is well-suited to characterize subtle differences in semantic processing that may not be observable at the behavioral level. The semantic priming paradigm presents a prime picture or word (e.g., dog) followed by a target picture or word that is either related (e.g., cat) or unrelated (e.g., pencil) to the prime. This paradigm can thus be used to evaluate semantic processing across different modalities, and to compare lexico-semantic and visuo-semantic processing abilities in individuals with ASD and how they might differ from TD individuals. The specific steps involved in creating the stimuli, performing the EEG testing, and analyzing the EEG data are discussed. Representative results illustrate how the N400 component of the event-related potential (ERP) is reduced following semantically-related prime-target pairs compared to unrelated pairs. Comparisons of the N400 between conditions, modalities, and groups can provide estimates of the success of semantic processing, and can thereby be used to characterize semantic deficits in individuals with ASD or other clinical populations.

Investigating the Use of a video-based Social Information Processing Interview Schedule (SIPIS) for Individuals with ASD subtypes

Introduction: Although individuals with Autism Spectrum Disorders (ASD) and Attention Deficit/Hyperactivity Disorder (ADHD) share common social interaction deficits, there are specific differences in the quality of their social information processing. Objectives: The purpose of this study was two-fold: (i) what are the measurement properties of the SIPIS, in measuring the construct of social information processing from the three aspects of social encoding, social empathy and social reasoning, in regard to its degree of fitness to the Rasch model? (ii) Can the SIPIS discriminate SIP performances between neurotypical controls, individuals with ASD and their subtypes? Methods: Subjects with ASD and their subtypes were assessed through viewing social videos using the newlydeveloped Social Information Processing Interview Schedule (SIPIS). Rasch analysis was adopted to investigate the psychometric properties of the newly developed SIPIS including its discriminative validity. Results: Findings of the Rasch analyses supported that the SIPIS contains assessment items that tap into the social encoding, social empathy, and social reasoning aspects of social information processing. Findings confirmed that both individuals with ASD and with ADHD manifest more difficulties in social information processing compared to the control group and that the social information processing deficits in individuals with ASD-only and comorbid ASD/ ADHD tend to occur more at the cognitive level while in individuals with ADHD-only they to occur more at the emotional perception level. Conclusion: Our findings seem to support the parallel pathways of the bottom-up emotion perception-action coupling processes and the top-down meta-cognitive executive regulatory control. Future assessment and intervention can be further explored from the perspectives of social information processing.

Exploring (central) auditory processing deficits in individuals with Broca's aphasia: Based on case study

It is well known fact that persons with lesions involving either the right or left temporal lobe tended to show impaired performances for complex auditory task and showed auditory processing disorder symptoms. Present study was aimed to find out whether auditory processing deficit exists in individuals with Broca's aphasia due to stroke based on case studies outcomes through detailed behavioral and electrophysiological audiological assessment. There were two participants (case 'A' and case 'B') diagnosed as having Boca's aphasia taken for the study. Case 'A' and 'B' were 39 and 29 year old male participated in the study. They were assessed for routine audiological and detailed auditory processing disorders tests like pitch pattern test, gap detection test and dichotic digit test. Further, they were also assessed for electrophysiological tests like click evoked auditory brainstem response and auditory late latency response. The routine audiological tests reveal no peripheral hearing loss in both the cases. However, details behavioral (C)APD tests results revealed overall poorer performance for different tasks in both cases. In addition to that, electrophysiological tests results also revealed abnormal brainstem and cortical responses in both cases. Hence, preliminary report of the present study suggests that cases with lesion (strokes) should be evaluated in detail for auditory processing deficit.

Bayesian neural adjustment of inhibitory control predicts emergence of problem stimulant use.

Bayesian ideal observer models quantify individuals' context- and experience-dependent beliefs and expectations about their environment, which provides a powerful approach (i) to link basic behavioural mechanisms to neural processing; and (ii) to generate clinical predictors for patient populations. Here, we focus on (ii) and determine whether individual differences in the neural representation of the need to stop in an inhibitory task can predict the development of problem use (i.e. abuse or dependence) in individuals experimenting with stimulants. One hundred and fifty-seven non-dependent occasional stimulant users, aged 18-24, completed a stop-signal task while undergoing functional magnetic resonance imaging. These individuals were prospectively followed for 3 years and evaluated for stimulant use and abuse/dependence symptoms. At follow-up, 38 occasional stimulant users met criteria for a stimulant use disorder (problem stimulant users), while 50 had discontinued use (desisted stimulant users). We found that those individuals who showed greater neural responses associated with Bayesian prediction errors, i.e. the difference between actual and expected need to stop on a given trial, in right medial prefrontal cortex/anterior cingulate cortex, caudate, anterior insula, and thalamus were more likely to exhibit problem use 3 years later. Importantly, these computationally based neural predictors outperformed clinical measures and non-model based neural variables in predicting clinical status. In conclusion, young adults who show exaggerated brain processing underlying whether to 'stop' or to 'go' are more likely to develop stimulant abuse. Thus, Bayesian cognitive models provide both a computational explanation and potential predictive biomarkers of belief processing deficits in individuals at risk for stimulant addiction.

Effects of clutter on information processing deficits in individuals with hoarding disorder

BackgroundCurrent cognitive behavioral models of hoarding view hoarding as a multifaceted problem stemming from various information processing deficits. However, there is also reason to suspect that the consequences of hoarding may in turn impact or modulate deficits in information processing. The current study sought to expand upon the existing literature by manipulating clutter to examine whether the presence of a cluttered environment affects information processing. MethodsParticipants included 34 individuals with hoarding disorder. Participants were randomized into a clutter or non-clutter condition and asked to complete various neuropsychological tasks of memory and attention. ResultsResults revealed that hoarding severity was associated with difficulties in sustained attention. However, individuals in the clutter condition relative to the non-clutter condition did not experience greater deficits in information processing. LimitationsLimitations include the cross-sectional design and small sample size. ConclusionsThe current findings add considerably to a growing body of literature on the relationships between information processing deficits and hoarding behaviors. Research of this type is integral to understanding the etiology and maintenance of hoarding.

Investigating graph comprehension in students with dyslexia: an eye tracking study.

The purpose of this study was to examine graph comprehension in college students with developmental dyslexia. We investigated how graph types (line, vertical bar, and horizontal bar graphs), graphic patterns (single and double graphic patterns), and question types (point locating and comparison questions) differentially affect graph comprehension of students with and without dyslexia. Groups were compared for (1) reaction times for answering comprehension questions based on graphed data and (2) eye gaze times for specific graph subregions (x-axis, y-axis, pattern, legend, question, and answer). Dyslexic readers were significantly slower in their graph comprehension than their peers with group differences becoming more robust with the increasing complexity of graphs and tasks. In addition, dyslexic readers' initial eye gaze viewing times for linguistic subregions (question and answer) and total viewing times for both linguistic (question and answer) and nonlinguistic (pattern) subregions were significantly longer than their control peers' times. In spite of using elementary-level paragraphs for comprehension and simple graph forms, young adults with dyslexia needed more time to process linguistic and nonlinguistic stimuli. These findings are discussed relative to theories proposed to address fundamental processing deficits in individuals with dyslexia.

Attention network test: Assessment of cognitive function in chronic fatigue syndrome

Information processing difficulties are common in patients with chronic fatigue syndrome (CFS). It has been shown that the time it takes to process a complex cognitive task, rather than error rate, may be the critical variable underlying CFS patients' cognitive complaints. The Attention Network Task (ANT) developed by Fan and colleagues may be of clinical utility to assess cognitive function in CFS, because it allows for simultaneous assessment of mental response speed, also called information processing speed, and error rate under three conditions challenging the attention system. Comparison of data from two groups of CFS patients (those with and without comorbid major depressive disorder; n = 19 and 22, respectively) to controls (n = 29) consistently showed that error rates did not differ among groups across conditions, but speed of information processing did. Processing time was prolonged in both CFS groups and most significantly affected in response to the most complex task conditions. For simpler tasks, processing time was only prolonged in CFS participants with depression. The data suggest that the ANT may be a task that could be used clinically to assess information processing deficits in individuals with CFS.

Basic Auditory Processing Deficits in Dyslexia

A review of research that uses behavioral, electroencephalographic, and/or magnetoencephalographic methods to investigate auditory processing deficits in individuals with dyslexia is presented. Findings show that measures of frequency, rise time, and duration discrimination as well as amplitude modulation and frequency modulation detection were most often impaired in individuals with dyslexia. Less consistent findings were found for intensity and gap perception. Additional factors that mediate auditory processing deficits in individuals with dyslexia and their implications are discussed.

Auditory processing deficits in individuals with primary open-angle glaucoma

Objective: The high energy demand of the auditory and visual pathways render these sensory systems prone to diseases that impair mitochondrial function. Primary open-angle glaucoma, a neurodegenerative disease of the optic nerve, has recently been associated with a spectrum of mitochondrial abnormalities. This study sought to investigate auditory processing in individuals with open-angle glaucoma. Design/Study sample: Twenty-seven subjects with open-angle glaucoma underwent electrophysiologic (auditory brainstem response), auditory temporal processing (amplitude modulation detection), and speech perception (monosyllabic words in quiet and background noise) assessment in each ear. A cohort of age, gender and hearing level matched control subjects was also tested. Results: While the majority of glaucoma subjects in this study demonstrated normal auditory function, there were a significant number (6/27 subjects, 22%) who showed abnormal auditory brainstem responses and impaired auditory perception in one or both ears. Conclusions: The finding that a significant proportion of subjects with open-angle glaucoma presented with auditory dysfunction provides evidence of systemic neuronal susceptibility. Affected individuals may suffer significant communication difficulties in everyday listening situations.

Startle modulation in autism: Positive affective stimuli enhance startle response

Behavioral evidence suggests that emotion processing deficits in individuals with autism spectrum disorders (ASD) may occur at the level of basic (early, rapid, automatic) affective processes. Consistently, neurological evidence indicates that key brain areas associated with basic affective processing are atypical in ASD. The current study sought to better specify these deficits by comparing different components of basic affective processing in 14 adolescents and adults with ASD and 14 typical controls matched for age and verbal ability. Participants viewed affective pictures, and their responses were assessed with (i) affective eyeblink startle modulation, an indicator of the brain's aversive motivational system; (ii) facial electromyography, an online indicator of implicit valence appraisal; and (iii) self-report, an indicator of overt valence appraisal. The results show that in contrast to the typical pattern, in which exposure to negative stimuli increases startle whereas exposure to positive stimuli decreases startle, individuals with ASD showed startle potentiation to both positive and negative stimuli. Atypical potentiation during positive stimuli occurred despite individuals with ASD demonstrating appropriate implicit valence appraisals, reflected in their facial EMG responses, and appropriate overt appraisals, reflected in their self-reported ratings of the stimuli. Potentiation of startle to both positive and negative stimuli suggests a disruption in basic affective processes in ASD at the level of the early motivational response. This atypical pattern of responses has implications for understanding social and emotion deficits in ASD and calls for further investigation of basic affective processes.

Psychopathy and verbal emotion processing in non-incarcerated males

Incarcerated males who score high on personality measures of psychopathy are reported to be less efficient than other incarcerated males in processing emotionally laden stimuli. In this study, we evaluated whether an emotion deficit is also present in non-incarcerated higher functioning males who score high on psychopathy personality measures. A sample of male university students was screened for psychopathic personality, and two subsamples, those who scored high or low on global psychopathy, participated in a series of language processing tasks designed to assess verbal emotion processing. Unlike previous studies of emotion processing in the verbal domain, stimuli also systematically varied in abstractness. The results showed that participants scoring high in psychopathy were less efficient than those scoring low in psychopathy in processing negatively valenced words, across all levels of word abstractness. The results extend previous accounts of emotional processing deficits in individuals with psychopathic personality to a relatively high-functioning non-incarcerated sample.

ReviewThe relationship between the mismatch negativity (MMN) and psycholinguistic models of spoken word processing

Background: The results from previous studies have indicated that a pre‐attentive component of the event‐related potential (ERP), the mismatch negativity (MMN), may be an objective measure of the automatic auditory processing of phonemes and words.Aims: This article reviews the relationship between the MMN data and psycholinguistic models of spoken word processing, in order to determine whether the MMN may be used to objectively pinpoint spoken word processing deficits in individuals with aphasia.Main Contribution: This article outlines the ways in which the MMN data support psycholinguistic models currently used in the clinical management of aphasic individuals. Furthermore, the cell assembly model of the neurophysiological mechanisms underlying spoken word processing is discussed in relation to the MMN and psycholinguistic models.Conclusions: The MMN data support current theoretical psycholinguistic and neurophysiological models of spoken word processing. Future MMN studies that include normal and aphasic populations will further elucidate the role that the MMN may play in the clinical management of aphasic individuals.
