Primary Renal Malignant Fibrous Histiocytoma in an End-Stage Renal Disease Patient

Besides renal cell carcinoma with increased risk reported in the dialysis population, other unusual types of renal tumors should also be considered. However, to the best of our knowledge, renal sarcomas have never been reported among end-stage renal disease patients undergoing dialysis in the literature. In this study, we present the first case of a primary renal malignant fibrous histiocytoma (MFH, also called undifferentiated pleomorphic sarcoma) in a 41-year-old woman with end-stage renal disease.

Malignant fibrous histiocytoma of bone: A survival analysis from the National Cancer Database.

Malignant fibrous histiocytoma (MFH) of bone, now known as undifferentiated pleomorphic sarcoma of bone, is a rare neoplasm that accounts for less than 2% of all primary malignant bone tumors. The objective of the current study was to evaluate prognosis and survival for MFH of bone. The 2004 to 2016 National Cancer Database was queried to identify patients with a primary MFH of bone. Kaplan-Meier survival and Cox regression analyses were used to analyze overall survival and risk factors associated with overall mortality. The overall 5-year and 10-year survival rates were 38.3% and 30.5%, respectively. Increasing stage and metastatic disease at presentation were associated with poor overall survival (P < .001). Patients aged 18 to 50 years (hazard ratio [HR], 0.51), 51 to 75 years (HR, 0.61), and those undergoing surgery (HR, 0.39) had improved survival. Having Medicare insurance (HR, 1.48), residing in a low educated area (HR, 2.56), and positive surgical margins (HR, 1.80) were associated with poor survival. The overall prognosis of MFH of bone is poor with a reported 5-year survival rate of 38.3%. Undergoing surgery and younger age were associated with a better prognosis. Older age, having Medicare insurance, and positive surgical margins were predictors of mortality.

A rare case of primary malignant fibrous histiocytoma: a sarcoma of the kidney

BackgroundPrimary malignant fibrous histiocytoma of the kidney (MFH) is an extremely rare tumor which is indistinguishable from its’ mimics in particular renal cell carcinoma (RCC) by clinical features and preoperative imaging evaluation. Due to its high predilection to local recurrence and distant metastases, early diagnosis has great value.Case presentationHerein, we describe a 62 year-old man with a 3-month history of abdominal discomfort and a palpable right loin mass whom computerized tomography (CT) showed a large heterogeneous solid mass in the right kidney. With the suspicious to RCC; the patient underwent radical nephrectomy. However, histopathological report revealed pleomorphic-storiform malignant fibrous histiocytoma. Immunohistochemistry study was also confirmed the diagnosis of MFH. Six month follow up showed no evidence of any recurrence.ConclusionsThe therapeutic options for MFH differ from other renal tumors, hence histopathology and immunohistochemistry studies are required to establish a definite diagnosis of the disease. Despite of progress made in clinical studies and advances in diagnostic modalities, early diagnosis of MFH has not achieved yet. Further studies and accumulated experience with renal MFH are required to determine the approach to prolong survival in selected cases along with management and prognostic factors of such tumors.

Primary Pulmonary Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma (MFH) cases are classified within the group of nonclassified sarcomas. The etiopathogenesis is unclear; however, MFH commonly develops in scar tissue and in areas exposed to radiation. MFH is the most common soft tissue sarcoma in adults and may be borne in the lungs, chest wall, mediastinum, or other tissues. Primary MFH of the lung constitutes less than 0.2% of all pulmonary neoplasms; thus, an optimal treatment strategy has not yet been elucidated. We aimed to report a case of MFH of the lung with subsequent treatment administration.

Malignant fibrous histiocytoma of the kidney

Primary renal malignant fibrous histiocytoma (MFH) is a rare tumor indistinguishable from renal cell carcinoma clinically and radiologically which can be differentiated only by histological and immunohistochemical studies. We report a case of primary renal MFH in a 40-year-old female who presented with left flank pain. Abdominal ultrasonography and computerized tomography revealed a well-defined renal mass. The contralateral kidney and renal functions were normal. Preoperative diagnosis was renal cell carcinoma and nephrectomy was performed. Histological examination and immunohistochemistry revealed MFH.

Primary Sarcomas of Thyroid Gland-Series of Three Cases with Brief Review of Spindle Cell Lesions of Thyroid.

Primary Malignant Fibrous Histiocytoma (MFH-T) and Leiomyosarcoma (LMS-T) of the thyroid gland are extremely rare tumors. Very few cases have been reported in the literature. Both entities occur more commonly in women than men. The closest clinical and histological differential diagnosis is anaplastic carcinoma of thyroid. We present three cases of rare primary sarcomas of thyroid gland. Case-1 was a 63-year-old woman and Case-2 was a 52-year-old woman. Both of them presented with a rapidly increasing thyroid mass clinically mimicking anaplastic carcinoma (AC-T). Both the patients developed pulmonary metastasis and succumbed to the illness soon after the diagnosis of MFH-T was made. Case 3 was 65-year-old woman with neck swelling since six months diagnosed as LMS-T. The present communication adds three new cases to the literature on sarcomas of thyroid gland with an emphasis on differential diagnosis of spindle cell lesions of thyroid. MFH-T and LMS-T needs to be differentiated from AC-T, metastatic sarcomas, spindle cell variant of medullary carcinoma, synovial sarcoma, fibrosarcoma; final diagnosis rests on histopathology and immunohistochemistry.

Clinical Features and Prognostic Factors of Primary Intracranial Malignant Fibrous Histiocytoma: A Report of 8 Cases and a Literature Review

Objective Primary intracranial malignant fibrous histiocytoma (MFH) is rare. We describe the detailed clinical features of 8 cases and fully review the literature to evaluate several prognostic factors. Methods Eight patients with pathologically confirmed primary intracranial MFH were retrospectively reviewed. We searched PubMed for relevant articles with the term “intracranial malignant fibrous histiocytoma”. Results Of the 8 patients, 4 were men and 4 were women. Three patients had received previous radiotherapy. The age of the patients ranged from 19 to 69 years, with a median age of 48 years. Most tumors could be totally resected; and only 1 tumor was subtotally resected. Six patients received postoperative radiotherapy and 3 patients received postoperative chemotherapy. Most patients died within the first year after surgery; and only 1 patient was still alive on the date of the last follow-up. We reviewed the literature and included a total of 46 patients in the Kaplan-Meier survival analysis. Young patients (less than 30 years old) seemed to have a better prognosis and survival rate than older patients (more than 30 years old) (log-rank test, P = 0.008). However, sex (P = 0.675), extent of resection (P = 0.934), postoperative radiotherapy (P = 0.592), and postoperative chemotherapy (P = 0.424) did not affect patient prognosis. Conclusions The prognosis of MFH is usually poor, and most patients die within the first year after surgery. Younger MFH patients (less than 30 years old) seem to have a better prognosis and improved survival compared to older patients.

Primary malignant fibrous histiocytoma of long bones: long-term follow-up.

This study aims to evaluate patients diagnosed with malignant fibrous histiocytoma and investigate the possible prognostic factors associated with duration of survival. The study, which was conducted between May 1994 and September 2013, included 14 patients diagnosed as malignant fibrous histiocytoma (12 males, 2 females; median age 48 years; range 17 to 64 years). We evaluated patients' demographic features, location of the pathology, histological findings, surgical margins, and treatment modalities and investigated the effects of these parameters on survival. Femur was the most frequently involved bone, followed by tibia and humerus. The median follow-up duration of the patients was 129 months. We performed limb salvage surgeries in 13 patients and amputation in one patient. Surgical margins were marginal in three patients and postoperative radiotherapy was performed for local control of the disease. Although there was no local recurrence in these patients, distant metastasis developed in two patients, indicating the importance of surgical margin as a significant factor on survival. Five-year survival rate was 81.9% in patients with wide surgical margins and 33.3% in patients with marginal margins. Surgical excision with wide margins and adjuvant chemotherapy provided adequate control of the disease and longer survival. The only prognostic factor statistically significantly associated with duration of survival was surgical margins. Neoadjuvant chemotherapy may be used when there is a suspicion of not obtaining adequately wide surgical margin perioperatively due to close association with neurovascular structures.

Primary malignant fibrous histiocytoma of the colon: A case report and review of the literature.

A 68-year-old woman was admitted to the Peking Union Medical College Hospital with a 6-week history of fever of undetermined origin. The computed tomography scan revealed a space-occupying lesion in the ascending colon and colonic endoscopy revealed a protrusive lesion in the initial segment of the ascending colon, occupying 3/5 of the lumen. Pathological examination of a biopsy specimen revealed malignant fibrous histiocytoma (MFH). The patient then underwent a laparotomy for right hemicolectomy and en bloc lymph node dissection. The histopathological examination confirmed the diagnosis of MFH and administration of adjuvant radiotherapy was decided. MFH is a high-grade sarcoma that rarely arises in the alimentary tract. To the best of our knowledge, there has only been one case of primary colorectal MFH treated by radiotherapy recorded to date, which makes our case the second trial. We herein present a case of MFH in the ascending colon and a review of previous case reports of patients presenting with this unusual type of MFH.

Combined Surgery, Chemotherapy and Radiotherapy of Primary Pulmonary Malignant Fibrous Histiocytoma: A Case Report

Primary pulmonary soft tissue sarcomas are a type of rare malignant tumor that could be misdiagnosed as intimal pulmonary carcinomas or metastasis of other carcinomas. Malignant fibrous histiocytoma (MFH) is one type of the soft tissue sarcoma. The MFH is often seen in deep soft tissue of the extremities, trunk and peritoneum, while primary MFH in lung is rarely seen. Here we report a case of a 46-year-old woman who first presented shoulder pain and a mass lesion occupying almost 1/2 of the left hemi thorax. A malignant pulmonary tumor was considered, and an exploratory thoracotomy was performed. Due to severe adhesion with pulmonary artery, the tumor was not completely removed. Subsequent pathological examination identified the lesion as primary pulmonary malignant fibrous histiocytoma. Combined chemo- and radiotherapy were performed after exploratory surgery and exhibited good therapeutic effects. The patient is on regular follow-up for six years and is currently recovering well with no evidence of relapse. The aim of this case report is to share our experience in treating rare primary pulmonary MFH with combined surgery, chemo- and radiotherapy.

A case of primary malignant fibrous histiocytoma of the pancreas with liver metastasis ⁎

Abstract Malignant fibrous histiocytoma (MFH) is one of most common types of soft-tissue sarcoma. However, it accounts for less than 1% of all human cancer types. In addition, primary MFH of the pancreas is very rare. Herein, we report of a 37-year-old man who presented with a pancreatic mass and did not show any specific symptoms. Pre-contrast computed tomography (CT) showed a heterogeneous, liquid-filled necrotic mass in the uncinate lobe of the pancreas. Contrast-enhanced CT showed a mild enhancement of solid components and the pancreaticoduodenal artery across the mass. The patient underwent a pancreaticoduodenectomy, ethanol ablation of the liver lesions, and chemotherapy with 4 cycles of ifosfamide and doxorubicin. The tumor recurred in the liver, but not in the pancreas, after 8 months.

Primary cardiac malignant fibrous histiocytoma with vulvar metastases: A case report.

Malignant fibrous histiocytoma (MFH) occurring in the heart is rare. To the best of our knowledge, the current study was the first to report a case of left atrial MFH metastasizing to the vulva. A 37-year-old female presenting with a history of exertional dyspnea underwent echocardiography, which revealed a mass in the left atrium, with a short-wide stalk attached to the posterior wall, extending into the pericardium. Furthermore, the mass resulted in severe mitral stenosis during diastole. The tumor was excised together with a section of the left atrial wall. The initial diagnosis was a left atrial myxoma. However, post-operative frozen section histopathology results indicated a diagnosis of MFH. Following surgery, the patient received chemotherapy, however, vulvar metastases and left pleural effusions were identified after 1 month. Following surgical excision of the vulvar tumor and left thoracentesis, the patient began adjuvant chemotherapy. However, the patient succumbed due to local recurrence 6 months later.

Laparoscopic-Assisted Surgery for Malignant Fibrous Histiocytoma Originating from the Descending Colon: A Case Report and Literature Review

Primary malignant fibrous histiocytoma (MFH) localized in the large intestine is extremely rare. We report a first case of MFH in colon that underwent a laparoscopy-assisted colectomy. A 74-year-old female had a 3-month history of blood stool. Barium enema study showed protrusion in the descending colon. Colonoscopy revealed submucosal tumor-like lesion with central ulceration.

Primary cardiac malignant fibrous histiocytoma with abdominal wall metastasis

We present a rare case of cardiac malignant fibrous histiocytoma (MFH; undifferentiated pleomorphic sarcoma); to date, fewer than 100 cases of cardiac MFH have been reported. In this case, transthoracic echocardiography revealed cardiac tumors in the left atrium (LA) of a 53-year-old woman with a 3-month history of worsening dyspnea; the largest tumor was found to protrude through the mitral valve in diastole, causing stenosis. Three of the four tumors were resected during emergency surgery; however, the residual tumor extension into the left pulmonary vein could not be removed. Histological findings of the resected tumors, such as organized thrombus and myxomatous tissue changes, indicated that the tumors were benign. After 3 months, the patient underwent total resection for a small mass that developed on her right abdominal wall, which was revealed histologically to be MFH; additionally, the residual mass in the LA had enlarged progressively. After undergoing radiation therapy without further surgery, she died of cerebral bleeding 6 months after cardiac surgery. Postmortem examination revealed that the tumor in the LA was an MFH. Thus, cardiac MFH should be considered as a differential diagnosis for tumors on the posterior wall of the LA.<Learning objective: Primary cardiac malignant fibrous histiocytoma (MFH), which is easily mistaken for atrial myxoma, is a rare type of cardiac sarcoma. MFH occurs most commonly on the posterior wall of the left atrium (LA), and total resection is currently the only effective therapy; however, the prognosis is poor. Therefore, a high level of suspicion is required to facilitate early diagnosis. Cardiac MFH should be considered as a differential diagnosis for tumors on the posterior wall of the LA.>

Management of extremity malignant fibrous histiocytoma: A 10-year experience

Summary Background Malignant fibrous histiocytoma (MFH) is a common soft-tissue sarcoma in adults, which commonly affects the extremities, trunk, head, and neck. Of these regions, the extremities are more often affected. However, only a few studies have specifically investigated the clinical behavior and prognosis of this malignancy, especially in East Asian populations. Aims and objectives We collected data on MFH cases for a period of 10 years and present our experience here in treating patients with extremity MFH. Materials and methods This study was a retrospective analysis of patients with extremity MFH treated in the Department of Plastic Surgery at National Cheng Kung University Hospital between January 2004 and December 2013. Data on patient profiles, surgical resection, radiotherapy, and chemotherapy were all reviewed. The patients were followed up regarding their local recurrences and survivals. Results Thirteen patients (6 men and 7 women) were enrolled in this study. The mean age of the patients was 57.9 years (range, 33–82 years). The sizes of the clinical tumors ranged from 1.7 to 15 cm in a maximal linear dimension (mean, 7.3 cm), and 76.9% of the tumors developed in the lower extremities. The average follow-up time was 63.5 months. One patient received limb amputation, and 12 patients were treated with limb salvage including marginal or wide excisions. Adjuvant radiotherapy was performed in 10 patients whose tumor sizes exceeded 5 cm or who exhibited inadequate excision or recurrence. Chemotherapy was performed in six patients who exhibited a distant metastasis or vessel invasion. The local and distant recurrence rates were 53.8% and 30.8%, respectively. A total of 42.9% of patients with local recurrence developed distant metastases. The surgical margin was the only significant factor of local disease control, and distant metastasis was the only factor relevant to the overall survival (OS). Conclusion An adequate resection margin during treatment for the primary MFH tumor achieves optimal results for local disease control. It influences the OS rate, because local-recurrence-related distant metastasis is significantly associated with high mortality.

Primary pulmonary malignant fibrous histiocytoma.

Malignant fibrous histiocytoma (MFH) is one of the most common adult soft tissue sarcomas. MFH is very aggressive and is most often found in the extremities and the retroperitoneum, but it can manifest at other sites. Though the lungs are the most common sites of metastasis, they rarely present there as a primary tumor. Our report describes a rare case of a primary MFH tumor in the lung. Careful diagnostic procedure should be followed to ensure the tumor does not have extrapulmonary origins. Though MFH is highly invasive and deadly, surgical excision of the tumor has been shown to be successful.

Primary Osseous Inflammatory Malignant Fibrous Histiocytoma Masquerading as Chronic Osteomyelitis

Inflammatory malignant fibrous histiocytoma, in addition to a mass lesion, may present with fever and other constitutional symptoms, mimicking an infectious process. This article presents an extremely rare and unique case of primary osseous inflammatory malignant fibrous histiocytoma, highlighting retrospectively the subtle clinical, radiologic, and pathologic features that can suggest this diagnosis. A 63-year-old woman with a history of nephrectomy for right kidney tuberculosis 20 years ago presented with slowly increasing left hip pain for 6 months. The relatively benign-appearing radiograph and biopsy report of chronic inflammation resulted in curettage and nailing of the lesion. The pathologic diagnosis was chronic osteomyelitis, but culture findings for bacteria and tuberculosis were negative. The patient remained well until 4 months later, when left thigh pain returned with fever and leukocytosis (white blood cell count as high as 20.7×10(9)/L, 80% neutrophils). No source of infection was localized. The patient showed no response to broad-spectrum antibiotics and antituberculous drugs. Radiographs showed substantial enlargement of the femoral lesion and extraosseous extension; biopsy results and review of previous histopathologic findings led to a diagnosis of inflammatory malignant fibrous histiocytoma. The fever gradually resolved and the white blood cell count returned to normal within a few days after segmental resection of the proximal femur tumor and its soft tissue extension. However, deep venous thrombosis developed, resulting in left foot ischemia and toe gangrene, necessitating left below-the-knee amputation. Disseminated metastasis occurred 3 months after tumor resection. The patient declined further aggressive treatment and died 13 months after initial presentation.

Primary hepatic malignant fibrous histiocytoma mimicking hepatocellular carcinoma: A report of two cases.

Malignant fibrous histiocytoma (MFH) is a tumor that occurs throughout the body as a relatively uncommon entity. The current study presents two cases of primary malignant fibrous histiocytoma of the liver. The first case was of a 67-year-old male who exhibited no symptoms or abnormal physical signs, and in whom the lesion was found by ultrasound examination during a routine physical examination. The second case was of a 35-year-old male who presented with persistent malaise, weight loss and intermittent right upper quadrant pain. The presence of liver cirrhosis due to hepatitis B virus, which was identified 10 years previously, and the clinical appearance caused MFH to appear as hepatocellular carcinoma at the time of the initial diagnosis. Abdominal magnetic resonance imaging scans were the main tools of diagnosis, but the MFH mimicked hepatocellular carcinoma due to the similar morphological characteristics, the rare occurrence of MFH and the underlying diseases of the liver. The first patient underwent a complete resection and recovered well, while the second patient underwent palliative treatment due to the large size of the tumor and the obstructive emboli in the portal vein. The diagnoses of the tumors were confirmed as MFH by histopathology and immunohistochemistry.
