Bilateral primary fallopian tube carcinoma is a very rare case scenario. Approximately, its incidence is 0.02% of all the gynecological malignancy. A 40-year-old female patient visited the hospital with complaints of lower abdominal pain for 3 months. Ultrasound of the pelvis shows complex bilateral adnexal lesions with ORADS score 4 – intermediate risk. The serum CA-125 is elevated. The provisional diagnosis of bilateral pyosalpinx was made. The uterus with bilateral ovaries and fallopian tubes was removed. On histopathology, thus, the final diagnosis of primary bilateral papillary serous carcinoma of the fallopian tube was made. Bilateral primary fallopian tube carcinoma is a very rare case scenario. Intraoperative frozen study is to be done to diagnose this entity. The high degree of suspicion is required to diagnose a case of bilateral fallopian tube carcinoma as it is a very rare case scenario and usually confused for pyosalpinx.

Prolonged Exposition with Hyperthermic Intraperitoneal Chemotherapy (HIPEC) May Provide Survival Benefit after Cytoreductive Surgery (CRS) in Advanced Primary Epithelial Ovarian, Fallopian Tube, and Primary Peritoneal Cancer.

Simple SummaryThe mainstay of therapy for primary advanced ovarian, fallopian tube, and primary peritoneal carcinoma is cytoreductive surgery (CRS) and platinum-containing chemotherapy. Since the primary and recurrent disease is mainly confined to the peritoneal cavity, intraperitoneal chemotherapy with or without hyperthermia has been evolved in order to improve survival. However, the studies to date are mainly retrospective, nonrandomized, and heterogeneous in terms of patient selection. There are currently only a limited number of reliable data on the role of hyperthermic intraperitoneal chemotherapy (HIPEC), its ideal timing of application, and duration. The aim of this study was to report our single-center experience with CRS + HIPEC involving patients with primary FIGO stage ≥ III B epithelial carcinoma. Hereby, multimodal therapy was feasible with acceptable morbidity and mortality, with no difference in overall survival between interval and upfront surgery. However, prolongation of HIPEC was shown to provide survival benefits regardless of previous administration of chemotherapy.Background: The usage of cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC) for advanced gynecological cancers is increasing. Methods: Prospectively collected data of 85 advanced primary ovarian/fallopian tube cancer and peritoneal carcinoma patients of a single center were investigated. Results: A total 48, 37, 62, and 25 patients were enrolled into the HIPEC with/without neoadjuvant chemotherapy (upfront vs. interval) and into the 60 min and 90 min long HIPEC groups, respectively. Better overall survival (OS) was observed in the 90 min HIPEC group (p = 0.0330), compared to the 60 min HIPEC group. Neither OS (p = 0. 2410), disease-specific (p = 0. 3670), nor recurrence-free survival (p = 0.8240) differed between upfront and interval HIPEC. Higher peritoneal carcinomatosis index (PCI) values were associated with worse disease-specific survival (p = 0.0724). Age (p = 0.0416), body mass index (p = 0.0044), PCI (p < 0.0001), the type (p = 0.0016) and duration (p = 0.0012) of HIPEC, and increased perioperative morbidity (p < 0.0041) had the greatest impact on OS. Conclusions: Increasing data support the value of HIPEC in the treatment of advanced ovarian cancer. Ongoing prospective studies will definitively clarify the role and timing of this additional therapeutic approach.

HE4 is associated with clinical risk prognostic factors and survival outcome in primary fallopian tube carcinoma patients.

To explore whether HE4 was associated with clinical risk prognostic factors and survival outcome in primary fallopian tube carcinoma patients. Ninety-six primary fallopian tube carcinoma (PFTC) patients from March 2011 to June 2019 were enrolled in this study. Serum CA125 and HE4 concentrations were measured at four time points including primary diagnosis, postsurgery, pre-recurrence, and recurrence. The relations between clinical risk prognostic factors with HE4 concentrations were investigated, and multivariate survival analysis was used to calculate the hazard ratios between HE4 levels with recurrence-free survival and overall survival. HE4 were significantly elevated in poor performance status, advanced stage, high histological grade and residual tumor diameter >1 cm, and positive lymph node status, respectively, compared with those in well performance status, early stage, low histological grade, residual tumor diameter ≤1 cm, and negative lymph node status, respectively. Multivariate survival analysis indicated serum HE4 can predict outcome of recurrence-free survival and overall survival with hazard ratios of 9.92 (95% confidence interval [CI]: 2.95-33.32) and 3.12 (95% CI: 1.07-9.08), respectively. HE4 is associated with clinical risk prognostic factors in PFTC and contributes to predict survival outcome in PFTC cases.

Primary fallopian tube carcinoma is an uncommon malignancy that accounts for 0.14% to 1.8% of gynecological cancers. The clinical symptom is not specific and preoperative diagnosis is easy to miss or delay because of a lack of specific symptoms. We reported a case of bilateral adenocarcinoma of the Fallopian tubes, was occurred in a 51-year-old postmenopausal woman and diagnosed with bilateral salpingectomy for suspicious of suppurative salpingitis. The diagnosis is not always suspected preoperatively. They are assimilated to salpingitis in the early stage, or to ovarian tumors in the advanced stage. The diagnosis is made postoperatively, after an anatomopathological examination. Its etiology is still poorly understood.

Long-term follow-up of 46 cases of primary fallopian tube carcinoma: a single institute study.

Primary fallopian tube carcinoma (PFTC) is a rare malignancy. In recent years the incidence of PFTC has been rising. This study retrospectively analyzed 46 cases of PFTC to identify prognostic factors that may impact the survival of patients with PFTC and explored the clinical characteristics. The clinical data of patients who had undergone surgery and adjuvant chemotherapy in Ren Ji Hospital, School of Medicine, Shanghai Jiao Tong University from 1995 to 2015 were retrospectively analyzed. We analyzed clinical data. Cox proportional hazards model was used for univariate and multivariate survival analysis. The level of CA125 increased in almost all patients with advanced-stage (stage III-IV) carcinoma and about half the patients with early stage (stage I-II) carcinoma. On ultrasound examination, 41 cases had pelvic mass, and five cases had intrauterine space-occupying lesion. Nine patients underwent curettage (19.6%). By the International Federation of Gynecology and Obstetricians (FIGO) staging system, the distribution of patients was 18 at stage I, 16 at stage II, 10 at stage III, and 2 at stage IV. The mainstay of treatment was surgical cytoreduction and platinum-based chemotherapy. Four patients had residual tumors diameter ≤1 cm (R1), 10 had residual tumors diameter >1 cm, and 32 had no macroscopic residual tumor (R0). Forty patients received postoperative intravenous (IV) chemotherapy. The five-year overall survival (OS) was 94.7% in stage I, 80.0% in stage II, 44.4% in stage III, and 0% in stage IV. Univariate and multivariate analysis revealed that residual tumor was independent prognostic variable for OS. Univariate and multivariate analysis revealed that ascites tumor cells and residual tumor were independent prognostic variables for progression free survival (PFS). Any postmenopausal women with vaginal bleeding, vaginal discharge, or lower abdominal pain should be alert to PFTC. Complete tumor markers and imaging examination should be conducted as soon as possible to improve the early diagnosis rate of the disease. Regardless of whether the operation is a comprehensive staging operation or cytoreductive surgery (CRS), achieving satisfactory R0 can improve OS and PFS. It is important the ascitic fluid is tested for tumor markers in order to predict PFS.

Primary Fallopian Tube Carcinoma: Results of a Single-Institutional Retrospective Analysis of 17 Patients with Evaluation of Staging and Prognostic Factors

OBJECTIVE: Primary carcinoma of the fallopian tubes is one of the less common gynecological cancers. It constitutes (0.14-0.18%) of gynecological malignancies. Our study aimed to review the managing process of primary carcinoma of the fallopian tubes in the mono-center institute and to identify prognostic factors impacting survival. STUDY DESIGN: A retrospective cohort study regarding patients with fallopian tube carcinoma treated between July 1991 and December 2005 was identified at the Tunisian anticancer institute “Salah Azaiez”. During this period, we have identified 17 patients. Data such as age, gravidity and parity, menopausal condition, symptoms reported by the patient on presentation, adjuvant therapy, stage of illness, surgical intervention, pathological findings, tumor recurrence, and previous surgical procedures were obtained from the patients’ reports. RESULTS: The average age at the time of diagnosis was 58 years. Fourteen of the included patients were postmenopausal. Surgery was the initial therapy for 15 patients. Optimal cytoreductive surgery was achievable in seven patients with no residual tumors. Histologic examination revealed that serous adenocarcinoma type was the predominant type. Two were in stage I and, four were in stage II; seven were in stage III and four in stage IV. The median follow-up time was 24 months. At the time of the final analysis, 11 patients died of disease. 5-year OS, DFS was 21% and 37% respectively. In our study, only the residual tumor was a significant prognostic factor predicting survival. CONCLUSION: Complete optimal surgery with no residual tumor was the main goal of the surgeon to improved survival in primary fallopian tube carcinoma.

Developing and validating a novel nomogram used a competing-risks model for predicting the prognosis of primary fallopian tube carcinoma: a retrospective study based on the SEER database.

BackgroundThe current prognostic methods for primary fallopian tube carcinoma (PFTC) are inadequate. This study is the first to use a competing-risks model to perform an accurate analysis of the prognostic factors for PFTC cause-specific death (CSD). We used the model to established a nomogram for the 3-, 5-, and 8-year CSD rates based on the identified prognostic factors.MethodsThis study selected 1,924 patients from the SEER (Surveillance, Epidemiology, and End Results) database. The cumulative incidence function (CIF) was used in univariate analyses, and Gray’s test was used to determine the intergroup difference in the CIF. We then used the subdistribution proportional hazards model in a multivariate analysis. We finally used the prognostic factors identified in the analysis of the competing-risks model to construct a 3-, 5-, and 8-year CSD nomogram for PFTC patients. The concordance index (C-index) and calibration plots were used to evaluate the discrimination ability and consistency of the model.ResultsThe subdistribution proportional hazards model showed that age, histological type, FIGO stage, and the log of the ratio between the numbers of positive and negative lymph nodes (LODDS) were independent prognostic factors for CSD. The 3-, 5-, and 8-year C-indexes were 0.744, 0.744, and 0.733 in the training cohort, and 0.737, 0.748, and 0.721 in the validation cohort. In the calibration plots, the forecast lines were very close to the reference lines.ConclusionsThis study is the first to analyze the prognostic factors for PFTC based on a competing-risks model. This model indicates that age, histological type, FIGO stage, and LODDS are significant prognostic factors affecting CSD in PFTC patients. We have also constructed the first 3-, 5-, and 8-year CSD nomogram for PFTC patients. This nomogram exhibits good discrimination ability and accuracy and can help clinicians to provide individualized prognostic analysis for PFTC patients.

After examination, the patient prepared for laparotomy; salpingo-oophorectomy, omentectomy, mass resection from pelvic and rectum Result: The hystological examination result was high grade serous carcinoma from the fallopian tube metastasis to cavum douglas and omentum. This case is a rare case because the tubal carcinoma was metastasis to cavum douglas and omentum without infiltrate the ovarium.Discussion: Primary carcinoma of the fallopian tube is rare and accounts for about 0.14-1.8% of all gynecological malignancies. Clinically the diagnose of tubal carcinoma very similar to ovarian cancer, the diagnose can not distinguished pre operative. The new staging of FIGO may be reference for diagnostic and treatment of the desease. Thorough staging laparotomy is an important part of early management and systemic treatment. The RAD51D c.82G>A (p.Val28Met) variant disrupts normal splicing and is associated with hereditary ovarian cancer.

Mutations in RAD51D are associated with a predisposition to primary ovarian, fallopian tube, and peritoneal carcinoma. Our study aims to characterize a RAD51D missense variant in a hereditary ovarian cancer family. The effects of the RAD51D c.82G>A (p.Val28Met) variant on mRNA splicing were evaluated and characterized using RT-PCR, cloning and DNA sequencing. This variant completely disrupts normal splicing and results in the loss of 3'end of 5'UTR and the entire exon 1 (c.-86_c.82), which presumably leads to loss of the RAD51D protein. The RAD51D c.82G>A (p.Val28Met) variant is clinically significant and classified as likely pathogenic. Our results indicate that the RAD51D c.82G>A (p.Val28Met) variant contributes to cancer predisposition through disruption of normal mRNA splicing. The identification of this variant in an individual affected with high-grade serous fallopian tube cancer suggests that the RAD51D variant may contribute to predisposition to the ovarian cancer in this family.

Primary fallopian tube carcinoma: A rare case report

Carcinoma of the fallopian tube accounts for 0.3% of all cancers of the female genital tract1. There has been a rapidly increasing body of evidence supporting the fallopian tube as the site of origin of high grade serous ovarian cancer. This case report describes 37 years old female with known case of hypothyroidism who presented with pain abdomen which was diffuse, progressive. On examination, there was hard irregular mass around umbilicus. She was then subjected to further investigations and was taken for staging laparotomy. During surgery, specimens were sent for histopathological examination at Rohilkhand Medical College and Hospital, Bareilly. Histopathological examination revealed the diagnosis of primary fallopian tube carcinoma (PFTC). This case is unique since we started it as a case of bilateral ovarian mass but histology revealed it to be a PFTC.

Primary Carcinoma of Fallopian Tube: Analysis of Two Cases Requiring Colectomy with Different Referral Symptoms

Primary fallopian tube cancer (PFTC) is a rare gynecological malignancy. PFTC can be diagnosed by surgery performed for another reasons. We report two cases of PFTC presenting with different referral symptoms. Both patients were postmenopausal. PFTC was detected after surgery for pelvic relaxation in one case, and after surgery for peritoneal carcinomotosis in the other. The distinction between PFTC and ovarian cancer (OC) is difficult because of the similarity of tumor presentations and histologies. As well as the histopathological similarities, treatment protocols of PFTC and OC are similar. Cytoreductive surgery is preferred in both diseases. Therefore in our cases colectomy besides the complete surgical staging were performed to achieve optimal debulking.

Clinical characteristics of primary Fallopian tube carcinoma: A single-institution retrospective study of 57 cases.

To analyze the clinical profile and prognosis of primary Fallopian tube cancer (PFTC) in order to improve earlier diagnosis. In this retrospective study, 57 women with PFTC were assessed from 2006 to 2016. Pathology, clinical index, recurrence, and survival were analyzed. Mean age was 57.35±9.01years, and 73% (19/26) of the patients with early-stage PFTC (I/II) were aged less than 60years. Of patients who presented with abnormal vaginal bleeding, 75% (9/12) were at an early stage and their condition was often misdiagnosed as endometrial carcinoma preoperatively. In patients with Stages I/II and Stages III/IV PFTC, 59.09% (13/22) and 96.43% (27/28), respectively, had adnexal masses on color Doppler ultrasonography. The 5-year overall survival (OS) and disease-free survival rates were 69.23% and 44.23%, respectively, and univariate analysis showed that tumor stage and residual tumor size significantly affected the two survival rates. Primary Fallopian tube cancer is more likely to be misdiagnosed in patients aged less than 60years or those presenting with vaginal bleeding at the premenopausal stage. Magnetic resonance imaging, cervical smear, and endometrial brush may be helpful for early PFTC diagnosis. Satisfactory cytoreductive surgery is critical because tumor stage and residual tumor size are significantly associated with the OS rate.

Primary Transitional Cell Carcinoma of Fallopian Tube: A Rare Entity with Review of Literature

Objective: Primary fallopian tube carcinoma (PFTC) is a rare entity constituting only 0.2-1.5% of all primary female genital-tract malignancies. Transitional cell carcinoma of fallopian tube is very rare constituting only around 10% of the PFTC and very less literature have been reported worldwide. Case report: A 59-year-old postmenopausal women presented with discharge per vaginum and abdominal distension, and was found to have right adnexal mass on clinical examination. Imaging findings were suggestive of a heterogenous adnexal mass and she underwent staging laparotomy. Post-operative histopathology examination confirmed a primary transitional cell carcinoma of right fallopian tube. FIGO stage was IA, and she received six cycles of adjuvant chemotherapy. The patientis nowalive, after 10-years of initial treatment.
