Association between long working hours and the onset of problematic alcohol use in young workers: A population-based longitudinal analysis in South Korea

BackgroundLong working hours can negatively impact mental health. We examined the association between long working hours and problematic alcohol use among young Korean workers. MethodsWe included a nationally representative sample of workers below the age of 40 from the Youth Panel between 2016 and 2020 (N = 7846; observations: 27,646). The exposure variable was working hours per week. Problematic alcohol use was assessed using the CAGE questionnaire, and generalized estimating equations were used to account for repeated measurements. ResultsObservations of the participants revealed that 5 % worked < 35 h/week, 51 % worked 35–40 h/week, 24 % worked 41–48 h/week, 11 % worked 49–54 h/week, and 9 % worked ≥55 h/week. The prevalence of problematic alcohol use among the workers was 2 % for <35 h/week, 2 % for 35–40 h/week, 3 % for 41–48 h/week, 4 % for 49–54 h/week, and 4 % for ≥55 h/week. The adjusted odds ratios (95 % confidence intervals) for the association between working hours per week and the onset of problematic alcohol use in the following year were 0.86 (0.45–1.66) for <35 h, 1.56 (1.22–1.99) for 41–48 h, 2.03 (1.49–2.75) for 49–54 h, and 1.68 (1.17–2.42) for ≥55 h, compared to 35–40 h. LimitationsThe observational design of the study prevented the establishment of a causal relationship between working hours and the onset of problematic alcohol use. ConclusionLong working hours were associated with the development of problematic alcohol use. Policy interventions should promote healthier drinking habits among workers with long work hours.

Prevalence of Depression, Problematic Drinking and their Correlates among University Students in Southwest Nigeria

Aim: To determine the prevalence and correlations of depression and problematic drinking among undergraduates in a Nigerian University.&#x0D; Study Design: The study design was a descriptive cross-sectional survey&#x0D; Methodology: The study was conducted among 412 full-time undergraduates of LAUTECH, 18 years and above and apparently well without previous history of mental illness. Alcohol Use Disorder Identification Test (AUDIT) and Patient Health Questionnaire-9 (PHQ-9) are instruments used for the study. Data analysis was done with the statistical package for social sciences version 21.&#x0D; Results: Approximately half (49.5%) of the respondents were depressed and 20.4% of them had major depression. The prevalence of problematic alcohol use in the study population was 10.4% mostly among males. There was no significant association between depression, alcohol use and problematic alcohol use (χ2 = 1.125, df = 1, P-value = 0.289) in the study sample.&#x0D; Conclusion: The increasing prevalence of depression and problematic drinking among university students is worrisome as this will not only adversely impact their academic performance but future productivity and the risk of other psychosocial and health problems.

Alcohol use patterns and associated variables among the Karipuna indigenous people in the extreme Northern Brazilian Amazon

The objective of this study was to identify the prevalence and patterns of alcohol consumption and associated factors in the Karipuna indigenous people. A cross-sectional population-based study was conducted with 230 Karipunan respondents aged 15 or over from 12 villages in the state of Amapá, in the extreme northern Brazilian Amazon. The participants completed the Alcohol Use Disorders Identification Test (AUDIT). The prevalence of alcohol use among the Karipuna was 70%. Of these, 59.6% had low-risk use, 38.3% had hazardous or harmful alcohol use, and 2.2% met criteria for probable alcohol dependence. Overall, 40.5% of the respondents had hazardous or harmful alcohol use; 66.6% were men, and 33.4% were women. In the regression analysis, age, sex, religion, not having an occupation, being a student, low educational attainment, suicidal ideation and having sexual intercourse after alcohol consumption were associated with hazardous or harmful alcohol use. Sex and Catholic religion were risk factors for this alcohol use pattern among the Karipuna. The prevalence of problematic alcohol use among the Karipuna is higher than that observed among the general Brazilian population, and preventive screenings should be widely implemented. Efforts to address and minimize the consequences of harmful and hazardous alcohol use among Brazilian Amazonian indigenous populations could be developed.

Clinical Epidemiology of Alcohol Use Disorders in Military Personnel versus the General Population in Canada: Épidémiologie clinique des troubles liés à la consommation d’alcool chez les militaires par opposition à la population générale du Canada

Objectives: Research suggests a high prevalence of problematic alcohol use among military personnel relative to civilians. Our primary objectives were to compare the prevalence, correlates, help-seeking behaviors, perceived need for care, and barriers to care for alcohol use disorders (AUDs) in the Canadian Armed Forces (CAF) and the Canadian general population (CGP). Methods: Data were from 2 nationally representative surveys collected by Statistics Canada: (1) the Canadian Community Health Survey on Mental Health collected in 2012 ( N = 25,113; response rate = 68.9%) and (2) the Canadian Forces Mental Health Survey collected in 2013 ( N = 8,161; response rate = 79.8%). Descriptive statistics and logistic regression were used to examine differences in outcomes of interest associated with AUDs in the CAF and CGP. Results: The prevalence of lifetime AUDs was significantly higher in the CAF (32.0%) than the CGP (20.3%; adjusted odds ratio [AOR] = 1.14, 95% confidence interval [CI, 1.02 to 1.27]) after adjustment for sociodemographic covariates. In contrast, the past-year prevalence of AUDs was significantly lower among CAF personnel (4.5%) than civilians (3.8%; AOR = 0.78, 95% CI [0.61 to 0.99]) after adjustment for sociodemographic covariates. Child abuse history and comorbid mental disorders were strongly associated with past-year AUDs in both populations. CAF personnel compared to the CGP were more likely to perceive a need for care (AOR = 4.15, 95% CI [2.56 to 6.72]) and engage in help-seeking behaviors (significant AORs ranged from 1.85 to 5.54). CAF personnel and civilians with past-year AUDs reported different barriers to care. Conclusions: Findings argue for the value of different approaches to address unmet need for AUD care in the CAF and CGP.

Prevalence of early morning alcohol consumption among commercial motorcyclists and its health consequences in Ado Ekiti, Nigeria

Alcohol is classified among legal substances, but its excess consumption can result in road traffic accidents via impairing psychomotor activity and concentration. This study investigated the prevalence of early morning alcohol consumption and its health consequences among commercial motorcycle (Okada) riders in Ekiti State, Nigeria. Systematic sampling technique was used to recruit subjects for this study. Cutting down alcohol; annoyed by comments on alcohol; guilt of alcohol use; early morning use of alcohol; eye opener (CAGE) questionnaire was used to estimate the prevalence of problematic alcohol use among Okada riders in Ado Ekiti. One hundred and seven Okada riders were assessed. Majority of them were young adults. Majority of the riders know another rider who have had a serious road traffic accident riding Okada in the past 12months and 29% of them know a rider who had died riding Okada in the past 12months. About 28.6% of them admit early morning alcohol consumption. Suppression of cold (45.5%), keeping awake (19.4%) and peer group effect (14.5%) were the major identified factors influencing them to use alcohol. The commonest types of injuries sustained were bruises and lacerations (51.1%) and fractures of upper and lower limbs (18.7%). The early morning alcohol consumption among Okada riders contributed to road traffic accidents in Ekiti state, Nigeria.

Problematic Alcohol Use in Pokhara, Nepal as Assessed by CAGE Questionnaire

Introduction: Nepal is known to be a hub for alcohol lovers. There is no restriction in sales despite efforts by the government of Nepal. Homemade alcohol is another major problem in the country. Although production and sale has legal policy it is far from being regulated. There has been no survey to assess the extent of problematic alcohol use in the Pokhara Municipality. This was conducted with the Objectives of i) assessing prevalence of problematic alcohol use in the community of a different Wards in Pokhara Metropolitan city and ii) To correlate this with various socio-demographic characteristics.&#x0D; Material And Method: The CAGE questionnaire was administered to all adult individuals above 20 years of age, in houses selected as convenience sampling in different wards of Pokhara Metropolitan city from Jan 2017 to Dec 2017. Nursing students in the community visit were trained and instructed to fill the preformed questionnaire as per the inclusion criteria after taking the consent. Individuals who were not currently undergoing treatment for alcohol use disorders were only included.&#x0D; Results: Among 136 adults who were included in this study, the prevalence of problematic alcohol use was found to be 64.7% (CAGE ≥2). The extent of current alcohol use (n. 124, 91.2 %,) and problematic alcohol use (n. 82, 66.12%) were significantly high (P-value: 0.0001).&#x0D; Conclusion: The prevalence of problematic alcohol use is too high in Pokhara, a tourist destination in Western Nepal. There is an urgent need to formulate a policy for alcohol use in the community taking into account the findings of this study. The distinction of traditional sanction of alcohol users and non users did not apply to this study. Problematic alcohol use was significantly high with no treatment initiate.

Alcohol and Drug Use Among Postoperative Bariatric Patients: A Systematic Review of the Emerging Research and Its Implications.

Emerging research suggests that some bariatric surgery patients are at a heightened risk for developing substance use problems, especially alcohol use problems. An exhaustive literature review was conducted in January 2015 to investigate all articles published that included data on postoperative alcohol use, alcohol use disorders, and illicit drug use among bariatric surgery patients. Twenty-three studies reported on alcohol and/or substance use among bariatric patients. Six studies longitudinally assessed alcohol use behaviors; 3 of these studies found an increase in alcohol use following surgery. Six studies were cross-sectional, and 2 studies assessed medical records. Five studies investigated the prevalence of admissions to substance abuse treatment, and 3 studies combined alcohol and drug use data in a single index. Six studies reported on illicit drug use and reported low-postoperative use. The studies' samples were primarily non-Hispanic white females in their upper 40s, and only 11 of the 23 studies utilized validated assessment instruments. Studies employing longitudinal designs and large sample sizes indicate that bariatric patients who had the gastric bypass procedure are at an elevated risk for alcohol use problems postoperatively. Research also indicates that bariatric surgery patients might be overrepresented in substance abuse treatment facilities. Risk factors for problematic postoperative alcohol use include regular or problematic alcohol use presurgery, male gender, younger age, tobacco use, and symptoms of attention deficient and hyperactivity disorder. As a whole, however, studies indicate bariatric surgery patients demonstrate a low prevalence of problematic alcohol use, and studies about gastric bypass patients are not entirely conclusive. Prospective, longitudinal studies are needed, utilizing standardized and validated alcohol assessment instruments that follow postoperative bariatric patients well beyond 2 years, and account for types of bariatric procedure. Finally, study samples with greater racial/ethnic diversity and wider age ranges are needed.

Substance and Alcohol use in Young Adults in Turkey as Indicated by the CAGE Questionnaire and Drinking Frequency.

The purpose of this study was to determine drinking problems and to analyze the socio-demographic factors associated with problematic alcohol use in young adults. The study included 262 students who were surveyed for substance use problems in a postgraduate program using the Cut down, Annoyed, Guilty, Eye opener (CAGE) Questionnaire. The relationships between socio-demographic variables and alcohol use were assessed using both univariate and multivariate analyses. Of the whole sample, 56.11% reported that they had tried drinking alcohol and 1.91% had tried cannabis. The prevalence of problematic alcohol use was 15.3% and 29.7% according to CAGE1+ and past-year drinking frequency, respectively. Alcohol use by mothers was an important differentiating factor for alcohol use by their daughters. Graduating from a university located in the Eastern/Southeastern Anatolia regions, graduating from a private high school, and having average academic performance levels were determinants of problematic alcohol use according to CAGE1+ and frequency of drinking. This study suggests there is need for early intervention to prevent exposure to the risk factors for problematic alcohol use in young adults, emphasizing that probable presence of an alcohol use disorder and high frequency of drinking are related to socio-demographic factors (high school type, geographical location of the university, and family structure).

The prevalence of substance use among patients at a dental school clinic in Michigan

Problematic alcohol use and illicit drug use are associated with a number of physical health consequences, including poor oral health. The authors evaluate the prevalence of problematic alcohol use and illicit drug use in adults who visited a dental school clinic. The authors recruited patients from the waiting area of the clinic. During recruitment, 85.9 percent of patients (n = 384) who the authors approached consented to participate in the study. Overall, 20.6 percent of the participants reported either recent problematic alcohol use or illicit drug use; 7.4 percent of reported problematic alcohol use and 18.6 percent reported illicit drug use. The most common illicit drugs participants reported that they used were marijuana (16.8 percent), amphetamines (2.6 percent) and cocaine (1.1 percent). Participants who reported recent problematic alcohol use or illicit drug use were more likely to identify as white, were younger and did not have a spouse or partner. There was a high prevalence of illicit drug use and problematic alcohol use among the patients seen at dental school clinic. These rates were higher than those in the general population. The findings suggest that dental clinics are appropriate settings in which to identify and provide interventions for adults at risk of experiencing problems due to alcohol or drug use.

Alcohol use and congestive heart failure: incidence, importance, and approaches to improved history taking

Alcohol use, abuse, and dependence have the potential to result in alcoholic cardiomyopathy (ACM). This distinct form of congestive heart failure (CHF) is responsible for 21-36% of all cases of nonischemic dilated cardiomyopathy in Western society. Without complete abstinence, the 4-year mortality for ACM approaches 50%. Therefore, accurate and detailed assessment of alcohol use in congestive heart failure is essential. The prevalence of problematic alcohol use is unrecognized by many clinicians. Clinical assessment of alcohol intake is often reduced to a simple question such as, "Do you drink?" Denial and minimization are hallmarks of alcohol abuse, with many individuals underreporting their use of alcohol. Clinicians can overcome these hurdles by implementing practical history taking measures to improve the accuracy of self-reported alcohol use. The data regarding the dangers of ongoing alcohol use in individuals with ACM make attempts to engage individuals in treatment to support abstinence essential. Suggestions for detailed and accurate assessment are discussed.
