Cell-Wall-Deficient Bacteria in Oral Biofilm: Association with Periodontitis

Cell-wall-deficient bacteria are those that lack cell walls and live in a pleomorphic state. The genus Mycoplasma and L-form bacteria are both members of this group. The aim of this study was to search cellwall-deficient bacteria in periodontal biofilm and link their presence to periodontal disease. Eighty-nine individuals were recruited and divided into three groups: periodontally healthy individuals, individuals with chronic periodontitis, and those with aggressive periodontitis. The presence of cell-wall-deficient bacteria was detected in freshly collected biofilm by light microscopy, transmission electron microscopy (TEM) with and without electron microscopy in situ hybridization, atomic force microscopy and DNA stain (Hoechst). A new dichotomic index of classification for prevalence and morphologic variants was developed to classify cell-wall-deficient bacteria in periodontal biofilm. Cell-wall-deficient bacteria were found in periodontal biofilm and classified into protoplastic, everted, filament and intracellular forms, the last one mainly associated with aggressive periodontitis. We also assessed the prevalence of periodontopathic bacteria by means of polymerase chain-reaction (PCR) and found no clear, statistically significant, correlation among periodontal pathogens tested (except T. denticola) that allowed individuals with chronic periodontitis to be distinguished from those with aggressive periodontitis. Association between cell-walldeficient bacteria and periodontal condition was: periodontally healthy, 3.3% (1/30); individuals with chronic periodontitis, 30.6% (11/36); and those with aggressive periodontitis, 100% (23/23). Cell-walldeficient bacteria were detected in periodontal biofilm and linked to aggressive periodontitis.

Comprehensive microbiological findings in peri‐implantitis and periodontitis

The microbial differences between peri-implantitis and periodontitis in the same subjects were examined using 16S rRNA gene clone library analysis and real-time polymerase chain reaction. Subgingival plaque samples were taken from the deepest pockets of peri-implantitis and periodontitis sites in six subjects. The prevalence of bacteria was analysed using a 16S rRNA gene clone library and real-time polymerase chain reaction. A total of 333 different taxa were identified from 799 sequenced clones; 231 (69%) were uncultivated phylotypes, of which 75 were novel. The numbers of bacterial taxa identified at the sites of peri-implantitis and periodontitis were 192 and 148 respectively. The microbial composition of peri-implantitis was more diverse when compared with that of periodontitis. Fusobacterium spp. and Streptococcus spp. were predominant in both peri-implantitis and periodontitis, while bacteria such as Parvimonas micra were only detected in peri-implantitis. The prevalence of periodontopathic bacteria was not high, while quantitative evaluation revealed that, in most cases, prevalence was higher at peri-implantitis sites than at periodontitis sites. The biofilm in peri-implantitis showed a more complex microbial composition when compared with periodontitis. Common periodontopathic bacteria showed low prevalence, and several bacteria were identified as candidate pathogens in peri-implantitis.

Colonization of periodontopathic bacteria is associated with increased risk of systemic diseases. However, few studies have investigated the relationships between oral status factors and health-related quality of life (HR-QOL) and the prevalence of such bacteria in elderly individuals. This study investigated the prevalence of Porphyromonas gingivalis, Prevotella intermedia, Treponema denticola and Tannerella forsythia in 165 community-dwelling functionally independent 85-year-old Japanese individuals (93 dentate, 72 edentulous) and the relationship to oral status, including oral malodour and HR-QOL. All four of the studied periodontopathic bacteria were found more frequently in tongue coating samples from dentate than edentulous subjects, and the prevalence of Porphyromonas gingivalis, Prevotella intermedia and Treponema denticola was significantly related to the number of teeth with a periodontal pocket depth ≥4 mm. These results suggest the existence of a stable circulation of periodontopathic bacteria between the gingival sulcus and tongue coating over time with teeth. In addition, the presence of teeth with a deep pocket and colonization of Treponema denticola were positively related to the level of CH(3)SH, whilst the number of present teeth contributed positively to HR-QOL, especially with regard to mental health. In conclusion, as the dentate state can retain colonization of periodontopathic pathogens in the oral cavity, both periodontal treatment and tongue care are important for maintaining a healthy oral status in the elderly, and possibly result in avoidance of risk for tooth loss and decline in HR-QOL, as well as protecting from systemic diseases.

Relationship between the β-galactosidase activity in saliva and parameters associated with oral malodor

Volatile sulfur compounds (VSCs) are produced by enzymes capable of transforming S-amino acids to corresponding sulfides. Protein degradation by periodontopathogens plays an important role in this process, and the proteolysis of glycoproteins depends on the initial removal of the carbohydrate side chains. In the present report, we tested the relationship between the β-galactosidase activity in saliva and parameters that influence oral malodor, including daily habits and oral conditions. The prevalence of periodontopathic bacteria was also examined. Forty-nine saliva samples were collected from halitosis patients. Patients were examined for breath odor and other associated parameters. Their breath odor was assessed using an organoleptic test, a portable sulfide monitor and gas chromatography. The presence of periodontopathic bacteria in the saliva was also examined. β-galactosidase activity was measured with the chromogenic substrates 5-bromo-4-chloro-3-indoyl-β-d-galactopyranoside and isopropyl-β-d-thiogalactopyranoside. β-galactosidase activity was positively correlated with malodor strength (organoleptic score, portable sulfide monitor score and VSC concentrations). Enzyme activity was also correlated with the degree of observable tongue coating. However, it showed no relationship with periodontal condition, saliva flow, tooth decay, unfitted restorations or the color of any tongue coating. While there was no relationship with Porphyromonas gingivalis and Treponema denticola, there was a negative correlation with Prevotella intermedia. These results indicate that β-galactosidase activity plays an important role in malodor production. Interestingly, the activity of this enzyme was not related to the presence of periodontopathic bacteria, which are the main malodor-producing organisms. The results obtained here may have been associated with physiologic halitosis, which is not necessarily associated with oral problems or with periodontopathic bacteria.

Volatile sulphide compound levels in mouth air with plaque-related periodontal disease are associated with a change in the proportion of periodontopathic bacteria in saliva

This study investigated whether oral malodour with plaque-related periodontal disease was associated with the prevalence of periodontopathic bacteria in saliva and tongue debris. The subjects were 30 patients who visited the Breath Odour Clinic. An oral examination was performed at baseline and two months after periodontal treatment, and included measuring the volatile sulphur compounds (VSCs) in mouth air. The proportions of five periodontopathic bacteria versus all bacteria in saliva and tongue debris were determined using real-time polymerase chain reaction. The subjects were classified into four groups based on the existence of VSCs above the organoleptic threshold level (TVSC) and a periodontal pocket ⩾4 mm (PD4). At baseline, the group with TVSC/PD4 had higher proportions of periodontopathic bacteria in saliva compared to those with TVSC/no PD4 and no TVSC/no PD4, but there were no differences in the proportions in tongue debris among the four groups. At follow-up, the proportion of periodontopathic bacteria in the saliva of the group with TVSC/PD4 was significantly reduced, as compared to that at baseline, and the VSCs and periodontal health improved. The results suggested that the reduction in VSCs in mouth air of subjects with periodontal disease might be associated with the prevalence of periodontopathic bacteria in saliva.

Prevalence of periodontopathic bacteria on the tongue dorsum of elderly people

The aim of this study was to analyse the prevalence of oral bacteria on the dorsum of the tongue. In addition, the relationship between the number of teeth and the microflora present on the coating of the tongue in a population of 85-year-old people was assessed. Two hundred and five individuals (89 males, 116 females) from the same geographical area who were 85 years of age were examined. Five periodontopathic bacteria (Porphyromonas gingivalis, Tannerella forsythia, Actinobacillus actinomycetemcomitans, Fusobacterium nucleatum, and Treponema denticola) and one cariogenic bacterium (Streptococcus mutans) were analysed using a polymerase chain reaction assay of tongue samples from the population. Periodontal bacteria-positive individuals have more teeth than that of periodontal bacteria-negative people. Between the periodontal bacteria-positive and -negative individuals, there were significant differences in the mean number of teeth for P. gingivalis (p<0.0001), T. denticola (p<0.001), F. nucleatum (p=0.002), and T. forsythia (p=0.005), while there were no significant differences for A. actinomycetemcomitans (p=0.998) or S. mutans (p=0.147). A wide range of species, including anaerobes, was detected in 85-year-old subjects. It was found that the detection of periodontal bacteria on the tongue coating increased with the number of teeth. There was a positive relationship between the tooth number and periodontopathic bacteria, except for A. actinomycetemcomitans. These results suggest that tongue care is essential for preventing oral disease and needs to be part of any oral care programme in elderly people.

Prevalence of periodontal pathogens in Brazilians with aggressive or chronic periodontitis

Previous studies suggest differences between geographically and racially distinct populations in the prevalence of periodontopathic bacteria as well as greater periodontal destruction associated with infection by highly leucotoxic Actinobacillus actinomycetemcomitans. The present study examined these hypotheses in Brazilians with aggressive or chronic periodontitis. Clinical, radiographical, and microbiological assessments were performed on 25 aggressive periodontitis and 178 chronic periodontitis patients including 71 males and 132 females, 15-69 years of age. The prevalence of Porphyromonas gingivalis was similar to that of other South American populations. The prevalence of A. actinomycetemcomitans and its highly leucotoxic subgroup was higher in Brazilians. Highly leucotoxic A. actinomycetemcomitans was more prevalent in aggressive periodontitis (chi2=27.83) and positively associated with deep pockets (>6 mm, chi2=18.26) and young age (<29 years, chi2=18.68). Greater mean attachment loss was found in subjects with highly leucotoxic A. actinomycetemcomitans than in subjects with minimally leucotoxic (p=0.0029) or subjects not infected (p=0.0001). These data support the hypothesis of differences between populations in the prevalence of periodontopathic bacteria and of greater attachment loss in sites infected with highly leucotoxic A. actinomycetemcomitans. Detection of highly leucotoxic A. actinomycetemcomitans in children and adolescents may be a useful marker for aggressive periodontitis.

Prevalence of Periodontopathic Bacteria in Aggressive Periodontitis Patients in a Chilean Population

Actinobacillus actinomycetemcomitans is considered a major etiologic agent of aggressive periodontitis (AgP). Other periodontopathic bacteria such as Porphyromonas gingivalis are also suspected of participating in aggressive periodontitis although the evidence to support this is controversial. The aim of the present study was to determine the prevalence of eight periodontopathic bacteria in Chilean patients with AgP. Subgingival plaque samples were collected from 36 aggressive, 30 localized, and six generalized periodontitis patients. Samples from 17 advanced chronic periodontitis (CP) patients were taken as controls. Samples collected from the four deepest periodontal pockets in each patient were pooled in prereduced transport fluid (RTF) and cultured. Periodontal bacteria were primarily identified by colony morphology under stereoscopic microscope and rapid biochemical tests. The identity of some bacterial isolates was confirmed by colony polymerase chain reaction (PCR). AgP showed a significatively higher prevalence of C. rectus than CP (P = 0.036). The only statistical difference found was for C. rectus. Patients with AgP showed a higher, but not statistically significant, prevalence of P. gingivalis, E. corrodens, P. micros, and Capnocytophaga sp. A similar prevalence in both groups of patients was observed for F. nucleatum and P. intermedia/nigrescens, and A. actinomycetemcomitans was less prevalent in AgP than CP patients. In localized AgP, P. intermedia/nigrescens, E. corrodens, F. nucleatum, and P. micros were the more prevalent pathogens in contrast to generalized AgP patients who harbored A. actinomycetemcomitans, P. gingivalis, and Capnocytophaga sp. as the most prevalent bacteria. C. rectus, P. gingivalis, E. corrodens, P. micros, and Capnocytophaga sp. were the most predominant periodontopathic bacteria of AgP in this Chilean population, but the only statistical difference found here between AgP and CP was for C. rectus, suggesting that the differences in clinical appearance may be caused by factors other than the microbiological composition of the subgingival plaque of these patients. In this study, the prevalence of A. actinomycetemcomitans was much lower than that of P. gingivalis.
