Implication of system xc− in neuroinflammation during the onset and maintenance of neuropathic pain

BackgroundDespite the high prevalence of neuropathic pain, treating this neurological disease remains challenging, given the limited efficacy and numerous side effects associated with current therapies. The complexity in patient management is largely attributed to an incomplete understanding of the underlying pathological mechanisms. Central sensitization, that refers to the adaptation of the central nervous system to persistent inflammation and heightened excitatory transmission within pain pathways, stands as a significant contributor to persistent pain. Considering the role of the cystine/glutamate exchanger (also designated as system xc−) in modulating glutamate transmission and in supporting neuroinflammatory responses, we investigated the contribution of this exchanger in the development of neuropathic pain.MethodsWe examined the implication of system xc− by evaluating changes in the expression/activity of this exchanger in the dorsal spinal cord of mice after unilateral partial sciatic nerve ligation. In this surgical model of neuropathic pain, we also examined the consequence of the genetic suppression of system xc− (using mice lacking the system xc− specific subunit xCT) or its pharmacological manipulation (using the pharmacological inhibitor sulfasalazine) on the pain-associated behavioral responses. Finally, we assessed the glial activation and the inflammatory response in the spinal cord by measuring mRNA and protein levels of GFAP and selected M1 and M2 microglial markers.ResultsThe sciatic nerve lesion was found to upregulate system xc− at the spinal level. The genetic deletion of xCT attenuated both the amplitude and the duration of the pain sensitization after nerve surgery, as evidenced by reduced responses to mechanical and thermal stimuli, and this was accompanied by reduced glial activation. Consistently, pharmacological inhibition of system xc− had an analgesic effect in lesioned mice.ConclusionTogether, these observations provide evidence for a role of system xc− in the biochemical processes underlying central sensitization. We propose that the reduced hypersensitivity observed in the transgenic mice lacking xCT or in sulfasalazine-treated mice is mediated by a reduced gliosis in the lumbar spinal cord and/or a shift in microglial M1/M2 polarization towards an anti-inflammatory phenotype in the absence of system xc−. These findings suggest that drugs targeting system xc− could contribute to prevent or reduce neuropathic pain.

Neuropathic and Nociplastic Pain Profiles are Common in Adult Chronic Nonbacterial Osteitis (CNO).

Chronic nonbacterial osteitis (CNO) is a rare musculoskeletal disease causing chronic bone pain. It is known that chronic musculoskeletal pain may involve other mechanisms than nociceptive pain only. We investigate the prevalence of neuropathic and nociplastic pain in adult CNO and their association with clinical characteristics and treatment outcomes. Survey study among the Dutch adult CNO cohort (n = 84/195 participated), including PAIN-detect for neuropathic pain, and the Central Sensitization Inventory (CSI), Fibromyalgia Rapid Screening Tool (FiRST), and ACTTION-APS Pain Taxonomy (AAPT) for nociplastic pain. Clinical characteristics and CNO-related bone pain scores were compared between patients with exclusive nociceptive pain and those with nociceptive pain plus neuropathic and/or nociplastic pain (mixed pain). 31% (95% CI 21-41) of patients classified as likely having neuropathic pain according to PAIN-detect. 53% (41-64) of patients displayed central sensitization on CSI, 61% (50-72) screened positive for fibromyalgia on FiRST and 14% (7-23) of patients fulfilled the AAPT criteria, all indicative of nociplastic pain. Mixed pain was associated with longer diagnostic delay (mean difference 2.8years, 95% CI 0.4-5.2, p = 0.023), lower educational level (72% versus 20%, p < 0.001), and opioid use (37% versus 13%, p = 0.036). Despite comparable disease severity and extent, patients with mixed pain reported significantly higher CNO-related bone pain scores. This study demonstrates the high prevalence of mixed pain in adult CNO, in which neuropathic and nociplastic pain exist alongside nociceptive inflammatory bone pain. Disease burden in CNO may extend beyond inflammatory activity, highlighting the need for a multifaceted management approach.

The Frequency of Neuropathic Pain and Associated Factors in Patients with Rheumatoid Arthritis

Abstract Objective The aim of this study was to evaluate the presence and incidence of neuropathic pain (NP) in patients with rheumatoid arthritis (RA) and to determine the relationship between neuropathic pain scores, quality of life and functional status. Methods 119 patients with RA and 45 control subjects with acute non-inflammatory pain were recruited for the study. Disease activity with DAS28 (Disease Activity Score-28), pain visual analog scale, functional status and quality of life were recorded. Quality of life was evaluated with the Rheumatoid Arthritis Quality of Life Scale (RAQoL), and functional status was assessed with the Health Assessment Questionnaire (HAQ). The presence of NP was evaluated with the LANSS (Leeds Assessment of Neuropathic Symptoms and Signs), Douleur Neuropathique 4 (DN4) and painDETECT questionnaires (PDQ). Results The prevalence of NP according to LANSS, DN4 and PDQ was 63.9%, 58.8% and 51.3% in the RA group and 6.7%, 4.4% and 4.4% in the control group, respectively. NP scores were significantly higher in the RA group (p&lt;0.001). LANSS, DN4 and PDQ scores correlated with VAS, HAQ and RAQoL scores. Conclusion The NP evaluated with different pain questionnaires is not rare in patients with RA and is related to functional status and quality of life.

Neuropathic Pain With and Without Diabetic Peripheral Neuropathy in Type 1 Diabetes.

Diabetic peripheral neuropathy (DPN) is common; however, the features and burden of neuropathic pain (NP) in type 1 diabetes (T1D) are poorly understood. We evaluated the incidence of first occurrence, annual prevalence, remission, and risk factors for NP during long-term follow-up of participants with T1D. The Michigan Neuropathy Screening Instrument (MNSI) was administered annually (1994-2020) for 1,324 participants in the Epidemiology of Diabetes Interventions and Complications (EDIC) study. NP with clinical signs of DPN (NP DPN+) was defined according to self-reported NP plus an examination score >2, while NP without clinical signs of DPN (NP DPN-) was defined according to self-reported NP and an examination score ≤2. At EDIC year 1, median age for participants was 36 years (interquartile range 30, 41), diabetes duration 13 years (10, 18), and HbA1c 7.9% (7.2, 8.9). At year 26 (median diabetes duration 39 years), cumulative incidence of NP was 57%, regardless of concomitant clinical signs of DPN (36% NP DPN+ vs. 46% NP DPN-). NP prevalence was 20% at 26 years (11% NP DPN+ and 9% NP DPN-), suggesting frequent remission. Annualized remission rates were similar regardless of pain medication use. In addition to HbA1c, female sex was associated with NP DPN-. NP incidence in T1D was high and frequently occurred in the absence of clinical signs of neuropathy, as assessed with the MNSI. Pain remission was not explained by pain medication use. Effective clinical strategies for identification and management are needed.

Shedding light on neuropathic pain: Current and emerging tools for diagnosis, screening, and quantification.

Ensuring the proper management of neuropathic pain is a contemporary challenge for professionals who care for patients with this type of pain. The estimated prevalence of neuropathic pain in Europe is 7%-8%. The objective of this study was to perform a non-systematic review on the diagnosis, screening, and quantification of neuropathic pain. For this purpose, a search was conducted of the PubMed/Medline, ScienceDirect, OVID, and SciELO databases for available evidence. The findings highlight the common occurrence of chronic neuropathic pain in clinical practice. However, diagnosing and managing this type of pain pose challenges due to its complexity and the individualized nature of cases. Precise diagnosis is crucial for effective management, involving therapeutic approaches that go beyond traditional pain treatments. It is noteworthy that until recent times, general questionnaires were utilized to assess neuropathic pain, lacking the ability to distinguish it from nociceptive pain or evaluate its broader impact on well-being. Biomarker pain panels hold promise in identifying treatable pain causes and evaluating treatment effectiveness. In conclusion, this review describes the diagnostic methods and tools for screening and quantifying neuropathic pain.

Obstructive Sleep Apnea Syndrome and Pain: A Cross-sectional Study

Aim: The present study aims to investigate the relationship between chronic widespread musculoskeletal&#x0D; system pain (CWP) and neuropathic pain (NP) among patients, who were diagnosed with obstructive&#x0D; sleep apnea syndrome (OSAS).&#x0D; Material and Methods: For this cross-sectional study, the study was carried out in sleep polyclinic of&#x0D; Uşak University’s Medical Faculty Hospital between 1 May 2017 and 1 February 2018. The patients,&#x0D; who applied to the sleep polyclinic for the complaint of sleep disorder and were diagnosed with OSAS by&#x0D; using polysomnography (PSG) but haven’t had a treatment for this purpose yet, were involved..&#x0D; Results: Examining the PSG results of participants, mild OSAS was detected in 16 cases, moderate&#x0D; OSAS in 15 cases, and severe OSAS in 26 cases. Pain was detected in minimum 1 region in 89% of&#x0D; the cases, whereas 38.5% of participants were found to have neuropathic pain (NP). NP was found to&#x0D; linearly increase with an increase in degree of OSAS (p=0.049). According to Pittsburgh Sleep Quality&#x0D; Index (PSQI) scores, 54% (n=31) of the participants had poor sleep quality, and according to Health&#x0D; Assessment Questionnaire (HAQ) scale scores, 36% (n=21) of the participants had low quality of&#x0D; life (QoL). When examining the differences between groups, due to the variables not being normally&#x0D; distributed, the Mann Whitney U and Kruskal Wallis-H Tests were utilized. to investigate the relationships&#x0D; among variables that do not follow a normal distribution, Spearman’s Correlation Coefficient was&#x0D; employed.&#x0D; Conclusion: CWP and NP were found to accompany each other among those with OSAS. The&#x0D; prevalence of NP increases with an increase in degree of OSAS. The QoL and quality of sleep among&#x0D; those with OSAS with pain were poorer than those having no pain. Women with OSAS were found to&#x0D; have higher pain and lower QoL when compared to men.

Analgesic consumption in a large sample of people in musculoskeletal rehabilitation: A descriptive study

BackgroundConsumption of opioids is increasing worldwide in people with chronic non-cancer pain, although their effectiveness is debated. ObjectivesThe aim of the current study was to evaluate analgesic consumption and its association with different variables (demographic variables, pain, anxiety/depression, catastrophism, and kinesiophobia), in the field of musculoskeletal rehabilitation, where no data are available. MethodsThis was a retrospective study over a period of 8 years on people hospitalised for rehabilitation after injury. Participants were classified into 3 categories: no analgesics (NA), non-opioid analgesics (NOA), and opioid analgesics (OPA). ANOVA or chi-squared tests were used to compare the 3 groups. ResultsA total of 4,350 people (84% men; mean [SD] age, 44 [11] years) were included. In total, 20% were taking OPA, 40% NOA and 40% NA. In the OPA group, tramadol was mainly used, and the morphine equivalent median dose was 8.3 mg/day. In the NOA group, paracetamol and ibuprofen were mostly used. Symptoms increased progressively across the 3 groups (NA/NOA/OPA), with increased levels of pain severity/interference, anxiety/depression and catastrophizing, and a higher prevalence of neuropathic pain in the OPA group versus the others. ConclusionsThese results are consistent with those found in groups of people with chronic pain taking larger doses of opioids and following opioid reduction or cessation programs. Opioid prescription did not increase over the 8 years, which was reassuring. These factors are important to emphasise because they can be modified in the rehabilitation setting with interdisciplinary management. RegistrationOur database was registered on Mendeley Data.

IUPHAR review- Preclinical models of neuropathic pain: Evaluating multifunctional properties of natural cannabinoid receptors ligands

Neuropathic pain remains prevalent and challenging to manage and is often comorbid with depression and anxiety. The new approach that simultaneously targets neuropathic pain and the associated comorbidities, such as depression and anxiety, is timely and critical, given the high prevalence and severity of neuropathic pain and the lack of effective analgesics. In this review, we focus on the animal models of neuropathic pain that researchers have used to investigate the analgesic effects of cannabidiol (CBD) and Beta-Caryophyllene (BCP) individually and in combination while addressing the impact of these compounds on the major comorbidity (e.g., depression, anxiety) associated with neuropathic pain. We also addressed the potential targets/mechanisms by which CBD and BCP produce analgesic effects in neuropathic pain models. The preclinical studies examined in this review support CBD and BCP individually and combined as potential alternative analgesics for neuropathic pain while showing beneficial effects on depression and anxiety.

Neuropathic Pain After Burn Injury: A Severe and Common Problem in Recovery.

The aim of this study was to address the limited understanding of neuropathic pain (NP) among burn survivors by comprehensively examining its prevalence and related factors on a national scale using the Burn Model System (BMS) National Database. NP is a common but underexplored complaint among burn survivors, greatly affecting their quality of life and functionality well beyond the initial injury. Existing data on NP and its consequences in burn survivors are limited to select single-institution studies, lacking a comprehensive national perspective. The BMS National Database was queried to identify burn patients responding to NP-related questions at enrollment, 6 months, 12 months, 2 years, and 5 years postinjury. Descriptive statistics and regression analyses were used to explore associations between demographic/clinical characteristics and self-reported NP at different time points. There were 915 patients included for analysis. At discharge, 66.5% of patients experienced NP in their burn scars. Those with NP had significantly higher Patient-Reported Outcomes Measurement Information System 29 (PROMIS-29) pain inference, itch, anxiety, depression, and sleep disturbance scores and were less able to partake in social roles. Multiple logistic regression revealed male sex, % total body surface area, and moderate-to-severe pain as predictors of NP at 6 months. At 12 months, % total body surface area and moderate-to-severe pain remained significant predictors, while ethnicity and employment status emerged as significant predictors at 24 months. This study highlights the significant prevalence of NP in burn patients and its adverse impacts on their physical, psychological, and social well-being. The findings underscore the necessity of a comprehensive approach to NP treatment, addressing both physical symptoms and psychosocial factors.

Prevalence and determinants of chronic pain and persistent opioid use after surgery: A review of systematic reviews.

Chronic post-surgical pain (CPSP) and persistent post-operative opioid use (PPOU) are inconsistently defined in published literature. This review comprehensively summarised their definitions, prevalence and determinants from existing systematic reviews or meta-analyses. Systematic reviews or meta-analyses evaluating the prevalence of CPSP and PPOU in adults after surgeries were retrieved from an electronic database search applying structured search strategies in PubMed, MEDLINE, Embase, CINAHL Plus and Cochrane Database of Systematic Reviews from inception to 31 December 2022. Two reviewers selected systematic reviews, extracted data regarding the definition, prevalence and risk factors of CPSP and PPOU and assessed the quality using the AMSTAR 2 tool. The study identified 6936 records related to chronic pain and persistent opioid use in patients after surgery, of which 24 articles were identified for full-text review. Eighteen systematic reviews were included in this umbrella review, of which five systematic reviews assessed chronic pain in patients who had undergone a surgical procedure, and 13 reviews assessed persistent opioid use after surgery. Despite considerable variations in patient characteristics (from age ≥18years), types of surgeries, follow-up duration and the definitions of measures, most reviews were of medium to good quality (fulfilled 9-11/16 AMSTAR 2 criteria). The prevalence of CPSP and PPOU, commonly synthesised at 3, 6 or 12months after surgeries, varied from 5%-58% and 2%-65%, respectively, despite various terminologies, definitions and timing of measures and associated determinants. The prevalence of neuropathic pain in CPSP was obtainable for four surgeries, with 9%-74%. To inform future practice and policy to optimise pain management and opioid safety, consensus on standardising measurements and further studies assessing risk factors associated with CPSP, PPOU and adverse outcomes are needed.

Utilization of PROMIS Neuropathic Pain Quality for Detection and Monitoring Neuropathic Pain in Heel Pain Patients.

Diagnosis and management of neuropathic pain (NP) in foot and ankle patients remain challenging. We investigated the plausibility of using Patient-Reported Outcomes Measurement Information System (PROMIS) Neuropathic Pain Quality (PQ-Neuro) as an initial screening tool to detect NP and track the treatment effects. Patients with heel pain were prospectively recruited and grouped to no-NP, mild-NP, and severe-NP based on the initial PROMIS PQ-Neuro t scores. Pain Interference (PI), Physical Function (PF), and Self-Efficacy (SE) scores were evaluated at baseline, 30-day, and 90-day follow-up. Other factors such as age, smoking, body mass index (BMI), low back/neck pain, anxiety/depression, and medications were analyzed. Linear mixed modeling was used to assess the main effects of time and NP on PROMIS t scores, comparing minimal clinically important difference (MCID). Forty-eight patients with mean age of 52.4 years were recruited. Using the PROMIS PQ-Neuro as the assessment tool, 33 patients (69%) were detected to have NP at baseline-23 (48%) mild and 10 (21%) severe. BMI was the only independent factor associated with NP (P = .011). Higher baseline PQ-Neuro t score was significantly associated with higher follow-up PQ-Neuro (P < .001), PI (P = .005), and lower SE (P = .04) across time points. Patients with NP showed lower PF at baseline with significantly less improvement in PF (3 vs 9.9, P = .035) and did not meet MCID. Baseline PROMIS PQ-Neuro ≥46 was significantly associated with worse PI and SE across all time points, with less clinically significant improvements in PF. Prevalence of NP in heel pain patients was high. The PROMIS PQ-Neuro may serve as a valuable tool for detection of NP and guiding clinical treatment decision pathways for heel pain patients. Pain characteristics and sleep dysregulation in patients with rheumatoid arthritis and ankylosing spondylitis: relationships with pain intensity and disease activity

Background. Pain is a prominent manifestation in chronic immune-inflammatory rheumatic disorders, encompassing both nociceptive and non-nociceptive elements, the latter potentially resulting from central sensitization. The connection between inadequate sleep quality and the onset of central sensitization in individuals with chronic rheumatic diseases has been documented. Objectives. This study aimed to analyze the characteristics of pain, sleep quality, and their correlation with pain intensity and disease activity in adults with rheumatoid arthritis and ankylosing spondylitis. Methods. A prospective observational study was conducted, involving patients with RA, AS, and osteoarthritis. We evaluated pain intensity, pain characteristics, and sleep quality. Results. The study group included 152 patients. RA was primarily characterized by nociceptive pain, but AS exhibited a higher prevalence of neuropathic pain. Significant associations were found between PD-Q scores and disease activity in patients with RA, but no such correlations were identified in patients with AS. Individuals with AS showed a significant inverse correlation between PD-Q scores and pain intensity. The PSQI scores did not show any notable variations among the three groupings. The quality of sleep was found to be connected with disease activity in RA but not in AS. A substantial and noteworthy correlation was found between the overall PSQI score and the scores obtained from the PD-Q in both subgroups of RA and AS. Conclusion. Notable connections can be found between disease activity, pain characteristics, and sleep quality in chronic immune-inflammatory rheumatic diseases.

Relationship between Nontraumatic Shoulder Disorders and Neuropathic Pain: Retrospective Observational Analyses of Clinical Features and Background Factors.

Accurate identification of neuropathic pain is necessary for appropriate treatment; however, the relationship between nontraumatic shoulder disorders and neuropathic pain remains unknown. Therefore, this retrospective observational study aimed to investigate the relationship, features, background factors, and prevalence of neuropathic pain among patients with nontraumatic shoulder disorders. We evaluated 198 patients who visited our outpatient clinic, which specializes in shoulder disorders, from April 2015 to March 2016. The patients' age, sex, affected side, diagnosis, and pain duration were recorded, and the results of physical examination, including passive range of motion, impingement sign, and muscular strength assessments, were analyzed. The presence of neuropathic pain was assessed using the painDETECT questionnaire. Participants were divided into two groups according to the presence of neuropathic pain. Pain intensity was assessed using a visual analog scale, and the patient's mental status was assessed using the short-form McGill Pain Questionnaire and Hospital Anxiety and Depression Scale. The scores were compared between the groups. Neuropathic pain was observed in 7.6% of patients. The visual analog scale score for pain, short-form McGill Pain Questionnaire score, and Hospital Anxiety and Depression Scale score were significantly associated with the presence of neuropathic pain in the univariate analysis. Patient background factors and physical function were not associated with the presence of neuropathic pain. The prevalence of neuropathic pain in patients with frozen shoulder was 33.3%, which was significantly higher than that in patients with other shoulder disorders. The occurrence of neuropathic pain may aggravate pain in patients with nontraumatic shoulder disorders. Neuropathic pain was not a rare condition in patients with nontraumatic shoulder disorders, particularly in those with frozen shoulder. The coexistence of neuropathic pain cannot be determined from background factors or physical function. Accurate diagnosis of neuropathic pain is essential in patients with nontraumatic shoulder disorders.

Factors correlated with neuropathic pain among industrial workers in Vietnam: a multi-site cross-sectional study.

Neuropathic pain is a debilitating condition resulting from various etiologies such as diabetes, multiple sclerosis, and infection, and is associated with decreased quality of life, poor health outcomes, and increased economic burden. However, epidemiological studies on neuropathic pain have been largely limited in Vietnam. A cross-sectional study was conducted on adult Vietnamese industrial workers across three manufacturing plants. Demographic, socioeconomic, occupational and health data were collected. Prevalence of neuropathic pain was assessed using the Douleur Neuropathique 4 (DN4) scale. Regression modeling was utilized to identify predictors of pain. Among 276 workers, 43.1 and 24.3% reported that they had suffered from spinal pain and osteoarthritis pain, respectively. In terms of work conditions, people maintaining constant posture when working from 30 to 60 min (OR = 3.15, 95% CI = 1.07; 9.29), or over 60 min (OR = 2.59; 95% CI = 1.12; 5.98) had a higher risk of suffering from spinal pain. People who worked in conditions lacking adequate lighting and with exposures to toxic chemicals were also likely to be suffering from osteoarthritis pain with OR = 4.26, 95% CI = 1.02; 17.74 and Coef. = 1.93; 95% CI = 1.49; 2.50, respectively. Regular health examinations and higher expenditure for healthcare were correlated with a lower prevalence of neuropathic pain. These results may inform the adoption of pain screening and other programs that increase health care access for this population, as well as more stringent occupational health and safety standards.

PREVALENCE AND PATTERNS OF NEUROPATHIC PAIN IN A COHORT OF PATIENTS WITH CHRONIC POST-SURGICAL PAIN AFTER TOTAL KNEE ARTHROPLASTY

AbstractIntroductionTotal knee replacement (TKR) is a successful operation for many patients, however 15–20% of patients experience chronic post-surgical pain (CPSP). Many will experience neuropathic characteristics.We describe the prevalence and patterns of neuropathic pain in a cohort of patients with CPSP three months after TKR.MethodologyBetween 2016–2019, 363 patients with troublesome pain, ≤14 on Oxford Knee score pain subscale, at three months after TKR from eight NHS hospitals were recruited into the Support and Treatment After Replacement (STAR) trial. Self-reported neuropathic pain was assessed at three, nine and fifteen months after surgery using painDETECT and Douleur Neuropathique 4 (DN4).ResultsAt three months post-operative, 53% reported neuropathic pain on painDETECT and 74% on DN4. Half (56%) remained in neuropathic pain over the twelve-month follow-up period, 26% reported improvement, and 9% reported new neuropathic symtpoms or fluctuated in and out of neuropathic pain (9%). Overall mean neuropathic pain scores improved between three and 15 months after TKR. When the painDETECT cut-off score of ≥13(ambiguous/possible) was used, DN4 and painDETECT measures showed similar prevalence rates at each timepoint.ConclusionNeuropathic pain is common among patients with CPSP at three months after TKR. Although symptoms improved over time, one quarter to one half of our cohort continued to report symptoms at fifteen months. We propose a painDETECT cutoff score of ≥13 be used to identify neuropathic features in the TKR population. Postoperative care should include identification, assessment, and treatment of neuropathic pain in patients with CPSP after TKR.

Neuropathic pain in patients with primary knee osteoarthritis: A cross-sectional study.

Neuropathic pain (NP) is believed to be a distinct subtype of pain associated with knee osteoarthritis (KO). This study aimed to determine the prevalence of NP in patients with primary KO and examine its correlation with various factors. A cross-sectional study was conducted on 195 patients with primary KO who did not have the following conditions: knee surgery, infection, rheumatic diseases, peripheral and central neurological diseases, patients who received treatment for neuropathic pain in the past 6 months, diabetes, renal failure, and alcoholism. NP was evaluated using two standardized questionnaires, the Douleur Neuropathique 4 questions (DN4) and the painDETECT questionnaire. The majority of patients were female (87.5%), with a mean age of 59.15 ± 7.62 years. NP was detected in 55.9% of patients according to the DN4 questionnaire and in 50.7% of patients according to the painDETECT questionnaire. The DN4 score was positively correlated with age (p=0.041; rs=0.145), visual analogue scale (VAS)-pain at rest (p=0.009; rs=0.188), VAS-pain at movement (p=0.017; rs=0.173), Western Ontario and McMaster Osteoarthritis Index (WOMAC) pain score (p=0.030; rs=0.157), WOMAC stiffness score (p=0.000; rs=0.253), physical function WOMAC score (p=0.000; rs=0.271), total WOMAC score (p=0.000; rs=0.305), Lequesne score (p=0.002; rs=0.221), and negatively correlated with quadriceps circumference (p=0.008; rs=-0.210). Whereas, the painDETECT score was positively correlated with VAS-pain at movement (p=0.002; rs=0.220), WOMAC stiffness score (p=0.043; rs=0.147), physical function WOMAC score (p=0.017; rs=0.172), and total WOMAC score (p=0.012; rs=0.182). NP according to the DN4 was associated with quadriceps circumference (p=0.01), while the painDETECT score was associated with VAS- pain at movement scores (p=0.022) and total WOMAC score (P=0.001). This study found that NP is common among patients with primary KO and is associated with more painful pathology and functional impairment. These findings highlight the importance of recognizing and managing NP in this population to improve patient outcomes.

Analysis of Japanese nationwide health datasets: association between lifestyle habits and prevalence of neuropathic pain and fibromyalgia with reference to dementia-related diseases and Parkinson's disease.

Chronic pain is defined as pain that persists or recurs for more than 3months. This study focuses on neuropathic pain (NP) and fibromyalgia (FM) which are chronic pain states, and aims to identify lifestyle habits associated with their prevalence. Other neurological disorders are also analyzed as references. Association between the variable referring to disease prevalence (number of claims for reimbursement of marker drugs) and the variable for lifestyle habits/health examination results (collected from insured individuals aged 40-74 years) was determined by analyzing Japanese nationwide datasets, which were collected in 2018 and aggregated by prefecture. Pregabalin, donepezil, and levodopa were used as marker drugs for the chronic pain states, dementia-related diseases (Alzheimer's disease and Lewy body dementia) and Parkinson's disease (PD), respectively. Pearson's correlation analysis and multiple linear regression analysis were conducted. Variables showing correlation coefficient (|r|)>0.5 were put into the multiple linear regression. Exercise habits (ꞵ=-0.3182), smoking habits (0.3218), daily drinking (0.2683), and alanine aminotransferase>51 U/L (0.2309) were finally incorporated in the equation for pregabalin (R 2=0.7268). Walking speed (-0.4543) and daily drinking (0.5077) were incorporated in the equation for donepezil (R 2=0.5718). The prevalence of chronic pain states is associated with lifestyle habits, just like the dementia-related diseases. Exercise in daily life is negatively associated with the prevalence of the chronic pain states, although excessive alcohol drinking, smoking, and high serum ALT are positively associated with it. The prevalence of PD seems less associated with lifestyle habits.

EXPLORING THE PREVALENCE OF NEUROPATHIC HAND PAIN AMONG YOUNG ADULTS AND ITS IMPACT ON HAND FUNCTION

Background and Aims: To explore the prevalence of neuropathic hand pain in young adults and how it affects hand functionality. Methodology: A convenience sampling was used to conduct a cross-sectional study on 192 medical students aged 18 to 28 years from October 2022 to March 2023. The purpose of the study was to evaluate the prevalence of neuropathic hand pain in young adults and how it affects hand functionality. Both the Duruoz Hand Index and the Douleur Neuropathique Four Questionnaire were used. Results: This study included 192 participants, the majority of whom were females (84%) and aged 20-24 years. 16.1% of participants reported having had surgery or an injury to their upper limb in the past. Participants described neuropathic pain symptoms such as electric shocks (21.4%), burning pain (18.2%), and painful cold (15.6%). Limitations and Future Implications: A small sample size and a focus on a particular group of medical students may restrict the generalizability of the findings. To improve the generalizability of the findings, future research should replicate these findings using larger sample sizes and diverse populations. Insights into the development and chronicity of neuropathic hand pain in young adults may also come from longitudinal studies. Conclusion: Our study demonstrates the prevalence of neuropathic hand pain in young adults and how it affects hand functionality. Female participants were more likely to experience pain and functional hand impairments.

Age-related changes in patients with upper limb thalidomide embryopathy in the United Kingdom.

We report the long-term upper limb disability, health-related quality of life (HRQoL), functional impairment, self-perception of appearance and prevalence of neuropathic pain in patients with upper limb thalidomide embryopathy in the United Kingdom. One-hundred and twenty-seven patients responded to our electronic questionnaire. Mean Quick Version of the Disabilities of Arm, Shoulder, and Hand score was 54.3 (SD 22.6). Median EuroQoL 5-Dimension 5-Likert index, Work and Social Adjustment Scale, Derriford Appearance Scale 24 and Neuropathic Pain Scale were 0.6 (IQR 0.4 to 0.7), 15.5 (IQR 8.0 to 23.5), 35.5 (IQR 28.0 to 50.5), and -0.8 (IQR -1.4 to 0.8), respectively. Thirty-three patients (26%) reported neuropathic pain. Finger changes associated with radial longitudinal deficiency were an independent predictor of more severe upper limb disability. Eighty-nine patients (70%) reported deteriorating HRQoL with increasing age. Patients with upper limb thalidomide embryopathy experience age-related worsening of symptoms and function, highlighting the need for ongoing specialist care and support.Level of evidence: IV.

Neuropathic pain feature in cancer-induced bone pain: does it matter? a prospective observational study

Cancer-induced bone pain (CIBP) is considered to have both nociceptive and neuropathic components. However, the prevalence, risk factors, and impact of the neuropathic components are yet poorly understood. We estimate the prevalence of neuropathic pain (NP) features in patients with CIBP at a tertiary care pain clinic setting using the Douleur Neuropathique 4 questionnaire and evaluate their associated factors and their impact after 4 weeks of treatment using the Brief Pain Inventory questionnaire and the Edmonton Symptom Assessment System. A total of 133 patients were recruited. The estimated prevalence of NP was 30.8% (95% confidence interval: 23.6%-39.1%). Initially, the patients with NP had significantly higher average pain scores (6.00 vs. 5.05, P = 0.006), higher total interference scores (5.84 vs. 4.89, P = 0.033), and symptom distress scores (35.88 vs. 26.52, P = 0.002). After 4 weeks of treatment, patients in both groups reported significantly decreased pain intensity and improved quality of life. However, the patients with NP still reported significantly higher average pain (4.61 vs. 3.58, P = 0.048), trending toward higher total interference scores (3.52 vs. 2.99, P = 0.426), and symptom distress scores (23.30 vs. 20.77, P = 0.524). From multivariate analysis, the independent risk factors for NP were younger age, pain in the extremities, and higher average pain scores. NP are common in patients with CIBP. These conditions negatively affect pain intensity and the patient's quality of life before and after treatment.
