Household food insecurity and associations with hemoglobin A1c and acute diabetes-related complications in youth and young adults with type 1 diabetes: The SEARCH for Diabetes in Youth study

AimsTo examine, among youth and young adults (YYA) with type 1 diabetes (T1D), the association of household food insecurity (HFI) with: 1) HbA1c and 2) episodes of diabetic ketoacidosis (DKA) and severe hypoglycemia. MethodsHFI was assessed using the U.S. Household Food Security Survey Module in SEARCH for Diabetes in Youth participants with T1D between 2016 and 2019. Linear and logistic regression models adjusted for age, diabetes duration, sex, race, ethnicity, clinic site, parent/participant education, household income, health insurance, and diabetes technology use. ResultsOf 1830 participants (mean age 20.8 ± 5.0 years, 70.0 % non-Hispanic White), HbA1c was collected for 1060 individuals (mean HbA1c 9.2 % ± 2.0 %). The prevalence of HFI was 16.4 %. In the past 12 months, 18.2 % and 9.9 % reported an episode of DKA or severe hypoglycemia, respectively. Compared to participants who were food secure, HFI was associated with a 0.33 % (95 % CI 0.003, 0.657) higher HbA1c level. Those with HFI had 1.58 (95 % CI 1.13, 2.21) times the adjusted odds of an episode of DKA and 1.53 (95 % CI 0.99, 2.37) times the adjusted odds of an episode of severe hypoglycemia as those without HFI. ConclusionsHFI is associated with higher HbA1c levels and increased odds of DKA in YYA with T1D.

Indigenous Identity and Household Food Insecurity are Associated with Poor Health Outcomes in Canada.

Purpose: To examine whether Indigenous identity and food insecurity combined were associated with self-reported poor health.Methods: Data from the 2015-2016 Canadian Community Health Survey and multiple logistic regression were employed to evaluate the association between Indigenous identity, household food insecurity, and health outcomes, adjusted for individual and household covariates. The Alexander Research Committee in Alexander First Nation (Treaty 6) reviewed the manuscript and commented on the interpretation of study findings.Results: Data were from 59082 adults (3756 Indigenous). The prevalence of household food insecurity was 26.3% for Indigenous adults and 9.8% for non-Indigenous adults (weighted to the Canadian population). Food-secure Indigenous adults, food-insecure non-Indigenous adults, and food-insecure Indigenous adults had significantly (p < 0.001) greater odds of poor health outcomes than food-secure non-Indigenous adults (referent group). Food-insecure Indigenous adults had 1.96 [95% CI:1.53,2.52], 3.73 [95% CI: 2.95,4.72], 3.00 [95% CI:2.37,3.79], and 3.94 [95% CI:3.02,5.14] greater odds of a chronic health condition, a chronic mental health disorder, poor general health, and poor mental health, respectively, compared to food-secure non-Indigenous adults.Conclusions: Health policy decisions and programs should focus on food security initiatives for all Canadians, including addressing the unique challenges of Indigenous communities, irrespective of their food security status.

Food insecurity: Discrepancy within Australian couple households

AbstractFood security remains a global public health priority but there may be bias in the prevalence of household food insecurity, depending upon who answers the questions. Using a cross‐section from the Household, Income and Labour Dynamics in Australia (HILDA) Survey, we analysed components of food insecurity reported separately by both partners in 718 households, allowing examination of discrepancies among couples. We modelled discrepancy in reports of food insecurity on 8 items using binary logit models for each question and a binomial regression for the number of questions on which there was discrepancy. Among couples who differed on an item, (conditional) discrepancy rates ranged from 71.75% to 85.86%. Females were more likely than males to report food insecurity on most items. Key characteristics associated with discrepancy were female's and male's age, female's education, being born in Australia, renting and male's employment. Male's age and birthplace were associated with discrepancy on more items while male's higher education and being a homeowner were associated with discrepancy on fewer items. Among couples who differed in responses, females were more likely to report food insecurity if they were older or had a disability, if their male partner was younger or had no disability, or if either person had more education than high school. The prevalence of food insecurity among couple households may be underestimated by 7.4 percentage points due to discrepancies in reporting. Discrepancies can reduce the accuracy of measures of food insecurity, impeding the targeting of responses, and lead to a less efficient allocation of resources to combat food insecurity.

Household food insecurity among child and adolescent athletics practitioners: A cross-sectional, descriptive, and exploratory study

ObjectiveThe aim of the study was to explore the prevalence of food insecurity among child and adolescent athletics practitioners and to investigate factors associated with exercise performance, dietary habits, body composition, sleep, and socioeconomic status based on food security status. MethodsThis was a cross-sectional, descriptive, and exploratory study conducted in Campinas, São Paulo, Brazil, between June and July 2023. The convenience sample included children and adolescents (7–17 y old) of both sexes. We evaluated exercise performance, household food insecurity (HFI), dietary-related parameters, and other body composition, lifestyle, and social-related variables. Exercise performance was assessed using the counter movement jump (CMJ), squat jump (SJ), horizontal long jump (HLJ), 50-m sprint test (50-m ST) and throwing strength test (TST). The assessment of HFI was conducted using the food insecurity experience scale (FIES). Comparisons and associations were investigated based on food security status. ResultsThe total sample size was comprised of 138 children (n = 42; 30.4%) and adolescents (n = 96; 69.6%). We found an association between food security status and sex (X2(138,1) = 4.42; P = 0.036). SJ was higher in the food security group than in the HFI group (t(117) = 2.112; P = 0.037; ES = 0.39). Sleep- and dietary-related factors did not differ between the groups. ConclusionsIn summary, the prevalence of HFI among child and adolescent athletics participants was approximately 40%. Regarding exercise performance, SJ was better in the food security group than in the HFI group. Concerning dietary-related data, the HFI group had a lower number of meals per day than the food security group, and other dietary data did not differ between the groups. Body composition and sleep-related parameters were similar between the groups.

Properties of the Household Food Security Survey Module Scale in Young Adults with Diabetes

BackgroundThe Household Food Security Survey Module (HFSSM) was not tailored to people with chronic diseases or young adults (YAs). ObjectivesWe aim to evaluate whether the 18-item HFSSM meets assumptions underlying the scale among YAs with diabetes. MethodsData from 1887 YAs with youth-onset type 1 diabetes or type 2 diabetes were used from the SEARCH for Diabetes in Youth Study, 2016–2019, and on 925 who returned for the SEARCH Food Security Cohort Study, 2018–2021, all of whom had completed the HFSSM. Guttman scaling properties (affirmation of preceding less severe items) and Rasch model properties (probability to answer an item based on difficulty level) were assessed. ResultsItems 3 (balanced meals) and 6 (eating less than one should) were affirmed more frequently than expected (nonmonotonic response pattern). At 1.2%–3.5%, item nonresponse was rare among type 1 diabetes but higher among type 2 diabetes (range: 3.1%–10.6%). Items 9 (not eating the whole day) and 3 did not meet the Guttman scaling properties. Rasch modeling revealed that item 3 had the smallest difficulty parameter. INFIT indices suggested that some responses to item 3 did not match the pattern in the rest of the sample. Classifying household food insecurity (HFI) based on items 1 and 2 compared with other 2-item combinations, including item 3, revealed a substantial undercount of HFI ranging from 5% to 8% points. ConclusionsUse of the HFSSM among YAs with diabetes could potentially result in biased HFI reporting and affect estimates of HFI prevalence in this population.

Household food insecurity and physical activity behaviour in Ecuadorian children and adolescents: findings from the Ecuador 2018 National Health and Nutrition Survey (ENSANUT-2018).

Ecuador has a high prevalence of household food insecurity (HFI) and is undergoing nutritional and epidemiologic transition. Evidence from high-income countries has reported negative or null associations between HFI and physical activity (PA) in children. It remains uncertain whether the same is true of those from low- and middle-income countries like Ecuador whose environmental and socio-demographic characteristics are distinct from those of high-income countries. We aimed to investigate the association of HFI with PA, sedentary behaviour (SB) and anthropometric indicators in children. Cross-sectional analysis of data from the nationally representative 2018 Ecuadorian National Health and Nutrition Survey. Data were collected on HFI, PA, SB, socio-demographic characteristics and measured height and weight. Unadjusted and adjusted linear, log-binomial and multinomial logistic regression analyses assessed the relationship of HFI with PA, SB, stunting and BMI-for-age. Ecuador. 23 621 children aged 5-17 years. Marginal and moderate-severe HFI was prevalent in 24 % and 20 % of the households, respectively. HFI was not associated with PA, SB, stunting nor underweight. Moderate-severe HFI was associated with a lower odds of overweight and obesity. However, adjustment for household assets attenuated this finding for overweight (adjusted OR:0·90, 95 % CI: 0·77, 1·05) and obesity (adjusted OR: 0·88, 95 % CI: 0·71, 1·08). HFI is a burden in Ecuadorian households, but is not associated with PA, SB nor anthropometric indicators in children aged 5-17 years. However, a concerning prevalence of insufficient PA was reported, emphasising the critical need for evidence-based interventions aimed at promoting PA and reducing SB.

Food Insecurity and its Association with Dietary Diversity: A Cross-Sectional Study in Rural Households in Jammu.

Food insecurity is a matter of public health concern as it is associated with adverse health outcomes particularly among vulnerable population. Accessibility and availability of nutritious and culturally appropriate food is paramount to achieve zero hunger. To assess the prevalence of household food insecurity, to estimate the household dietary diversity and its association with household food insecurity. Using multistage sampling 381 rural households were surveyed. Household Food Insecurity Access Scale and House hold Dietary Diversity Scale were used as study instruments. Chi square test was used to compare the two groups and P < 0.05 was considered to be statistically significant. Prevalence of household insecurity was 33.3% (127) among the surveyed households and 8.1% (31) had severe food insecurity. The mean HFIS score was 6.85 ± 4.82. The household Dietary diversity score was 8.14 ± 1.54 for food secure and 6.51 ± 1.38 for severely food insecure households. There was a statistically significant difference in intake of milk and milk products, fruits, eggs and meat/poultry among food secure and insecure households (P < 0.001 respectively). Food insecurity was high in rural households and was associated with lower dietary intake of foods from protein group, necessitating a need to reinforce the food security programmes in rural India with focus to enhance protein rich diet.

Prevalence of food insecurity and associated factors among households in Waliso town, Oromia, Ethiopia

BackgroundFood insecurity is the underlying cause of malnutrition. In addition, it is a condition where people lack consistent access to enough food for every person in a household to live an active and healthy life.Recently, urban food insecurity has been on the top agenda in Ethiopia, as households prone to food insecurity are rising because of various factors. Hence, we aimed to assess the prevalence of food insecurity and associated factors among households in Waliso town, Oromia, Ethiopia.MethodsA community-based cross-sectional study design was used from December 17, 2021, to January 16, 2022, among 397 randomly selected households in Waliso town. Study participants were selected using a simple random sampling technique. Data collection was performed using a structured questionnaire, and data were entered using Epi Info 7 and exported to SPSS version 20 software for analysis. To identify factors associated with household food insecurity, multivariable binary logistic regression analyses were done. An adjusted odds ratio along with a 95% confidence interval were estimated to measure the strength of the association. In this study, the level of statistical significance was declared at p < 0.05.ResultsThe prevalence of household food insecurity was 63.4%, 95% CI (58.60%, 68.20%). Factors associated with household food insecurity were family sizes 6 + [AOR = 3.06, 95% CI: (1.35, 6.94)], dependents 1–2 [AOR = 2.62, 95% CI: (1.35, 5.10)], dependents 3 + [AOR = 3.07, 95% CI: (1.38, 7.40)], unemployed [AOR = 6.86, 95% CI: (2.76, 17.04)], self-employed [AOR = 3.27, 95% CI: (1.50, 7.14)], wage laborers [AOR = 13.01, 95% CI: (4.93, 34.38)], low wealth index [AOR = 4.08, 95% CI: (2.18, 7.64)], and a medium wealth index [AOR = 2.08, 95% CI: (1.18, 3.69)].ConclusionNearly two in three households were food insecure in Waliso Town. Large family sizes, a high dependency ratio, unemployment, low wage employment, and a low wealth index were significantly associated with food insecurity. Hence, the implementation of actions to improve household income and livelihoods, minimize dependency rates, by strengthen the use of family planning and encourage urban agriculture is vital to alleviate household food insecurity problems in the town.

Household Food Insecurity and Its Determinant Factors in Eastern Ethiopia in 2021

Background: Many households in urban areas suffer from severe food insecurity. In Ethiopia, current economic recession, pandemic, locust invasion, and conflict, disproportionately aggravated the problems. This study aims to assess food insecurity in household and its factors in the Eastern Ethiopia. Methods: A community-based, cross-sectional study was conducted to assess 616 households in urban areas of Eastern Ethiopia using simple random sampling. FANTA Indicator Guide (v.3) Questionnaires were used as data collection tool. Then, the information was entered into Epi-data version 3.02 and exported to SPSS version 21. To determine factors affecting household food insecurity, binary regression analysis was performed, and a p-value of &lt; 0.05 at a 95% confidence interval was considered statistically significant. Model fitness was checked using the Hosmer-Lemeshow test. Results: The prevalence of household food insecurity was 41.7% (95% CI: 37.4–45.6). The following factors were determined as food insecurity predictors: household head's sex [Adjusted Odds Ratio (AOR)=1.75, 95% CI: 1.08–2.84], household head's education (AOR= 0.52, 95% CI: 0.33–0.99), mother's occupation [AOR= 0.3, 95% CI: 0.19–0.57], households with the medium wealth index (AOR =1.72, 95% CI: 1.06–2.78) and households with the lowest wealth index (AOR=4.01, 95% CI: 2.35–6.86). Conclusions: The prevalence of household food insecurity in urban areas of Eastern Ethiopia is high. The food insecurity was significantly affected by residency, education, occupation, and wealth index. The government should focus on socio-economic policies targeting the vulnerable and improve their economic status.

Food Insecurity after Cash Transfer Program in Rural Areas of Tehran: A Mixed Method Study

Background: Cash transfer (CT) is one of the strategies used by many developing countries to improve food security and nutritional status of lower socio-economic groups. This study aims to assessing household food insecurity (HFI) and viewpoints of women after CT program in rural population of Tehran province by both quantitative and qualitative methods. Methods: After making arrangements with rural health centers of Shahid Beheshti University of Medical Sciences and Health Services, 250 households were selected from five rural of Iran with different socio-economic status (SES) by cluster sampling. Locally adapted Household Food Insecurity Access Scale (HFIAS), food frequency, and SES questionnaires were completed, and height, weight, and waist circumference of adult females were measured. .Data on families' coping strategies were collected through 7 focus-group discussions. Results: The frequency of mild, moderate, and severe HFI was 22.3, 22.3, and 16.0%, respectively. SES negatively affected HFI (P&lt;0.001), and HFI negatively influenced food-group consumption (P&lt;0.001). Most of the women reported strategies like borrowing from family and friends, skipping meals, and eating less food by mothers. Conclusions: It seems that in spite of CT program, the prevalence of HFI among Iranians in rural areas was relatively high and consumption of food groups in FI households was low.

Food insecurity and associated factors among households with under-5 children in slum communities in Ibadan, Nigeria

IntroductionFood insecurity is a leading cause of childhood morbidity and mortality. This study assessed the prevalence of household food insecurity and its associated factors among under-5 children in Ibadan, Nigeria.MethodsThis was a cross-sectional household survey of 1,027 under-5 children and their caregivers in urban and rural slums in Ibadan. We used an electronic interviewer-administered, semi-structured questionnaire adapted from the Nigeria Demographic Health Survey and Household Food Insecurity Access Scale was used to report sociodemo-economic characteristics, food insecurity, and anthropometric measurement. The household food insecurity scale consisted of nine questions graded from 0 (Never) to 3 (Often) computed to determine the presence of food insecurity. Nutrition indices were computed, and the results were classified according to World Health Organization 2006 cut-off points. Chi-square tests were used to assess associations between food insecurity and the independent variables. Binary logistic regression analyses were conducted to identify the predictors of food insecurity (α = 0.05).ResultsThe mean ages of the caregivers and under-5 children were 31.7 ± 7.47 years and 34.49 ± 15.8 months respectively. Overall, 530 (51.7%) children were females, and 765 (74.5%) had normal weight for height. In all, 195 (19.0%) households had food insecurity, while 832 (81.0%) households had food security (Chi-square = 103.364, p = < 0.001). Under-5 children living in urban slums were seven times more likely to experience household food insecurity compared to those in rural slums (AOR = 6.859, 95%CI = 4.524–10.509, p = < 0.001).DiscussionHousehold food insecurity was more prevalent in urban slums. Strengthening of the school health program would help identify children with nutritional deficits, and improve the overall health status of children living in slum communities.

Examining the effect of demographic and socioeconomic factors on household food insecurity in Lideta subcity, Addis Ababa, Ethiopia

Food security and vulnerability assessments in Ethiopia have traditionally focused on rural regions. In contemporary policy discussions, urban food security has garnered significant attention. Notably, there is a lack of empirical substantiation of urban food security and its impact on the livelihoods of city dwellers as they grapple with escalating food prices. The present study aims to determine the extent of household food insecurity and identify correlated factors among 692 households in three randomly selected Woredas (districts) within Lideta sub-city, Addis Ababa, Ethiopia. Six Ketenas (villages) were selected using a probability proportional-to-size technique from three Woredas between February and March of 2023. Data were collected through a validated survey administered by trained individuals, and household income and expenditure were used to measure food insecurity access. Chi-square (&amp;chi;2) and logistic regressions were used to identify factors associated with food insecurity in the region. The overall prevalence of household food insecurity in the study area was found to be 66.5%. Regression results indicate that seven of the hypothesized nine demographic and socioeconomic determinants of household food insecurity significantly influence the probability of experiencing food insecurity (p &lt; 0.05). Household food expenditure, household dependency ratio, age, sex, educational status of the household head, access to savings and credit, and the urban productive safety net program were identified as significant determinants of urban household food insecurity. This study underscores that food insecurity in Ethiopia is not limited to rural areas but also affects urban regions, mainly due to high urban poverty rates. Strategies to reduce household size, improve the household dependency ratio, and enhance socioeconomic factors can empower households to build resilience against food insecurity. Policymakers should adopt measures to stabilize the food insecurity improve living standards, and enhance the economic capabilities of urban households.

Gender, skin color, and household composition explain inequities in household food insecurity in Brazil.

It is well known that female-headed households (FHHs) are more likely to experience food insecurity (FI) than male-headed households (MHHs), however there is a dearth of evidence on how gender intersects with other social determinants of FI. Thus, this paper investigated changes in the prevalence of household FI in Brazil from 2004 to 2018 by the intersection of gender, race/skin color and marital status of the household reference person. Data from three cross-sectional nationally representative surveys that assessed the status of FI using the Brazilian Household Food Insecurity Measurement Scale were analyzed (N2004 = 107,731; N2013 = 115,108, N2018 = 57,204). Multinomial logistic regression models were used to examine the relationship between profiles of gender, race/skin color, marital status of the head of the household with household FI stratified by the presence of children <5 years of age. Over time, FHHs had a higher prevalence of mild and moderate/severe FI than did households headed by men. Food security prevalence increased from 2004 to 2013 and decreased between 2013 and 2018 for households headed by men and women. In 2018, households headed by black/brown single mothers with children < 5 years of age were at the highest FI risk. The probability of reporting moderate/severe FI in these households were 4.17 times higher (95% CI [2.96-5.90]) than for households headed by married white men. The presence of children in the household was associated with a higher probability of moderate/severe FI, especially for households headed by black/brown individuals regardless of the reference person's gender. The results suggest that gender inequities combined with darker skin color and the presence of children at home potentiate the risk of moderate/severe FI. Policy makers need to consider the principles of intersectionality when investing in codesigning, implementing, evaluating, and scaling up evidence-based programs to reduce FI.

Household food insecurity, low maternal social support and maternal common mental disorders in East Mamprusi Municipality, Ghana

IntroductionBoth household food insecurity and maternal common mental disorders are public health concerns in Ghana but studies on them, and their interrelatedness are scarce. Social support is an independent determinant of mental health but can also moderate the link between risk factors and mental illness. Identifying the risk factors of mental illness may provide opportunities for intervention and help reduce disease burden and impact. This study examined the association between household food insecurity or low maternal social support and maternal common mental disorders in East Mamprusi Municipality, Ghana.MethodsThis was a community-based, cross-sectional study involving 400 mothers with children 6–23 months selected using multi-stage sampling. Summary scores for household food insecurity, maternal social support, and maternal common mental disorders were measured using Food Insecurity Experience Scale (FIES), Medical Outcome Study Social Support Scale (SSS), and WHO Self-Reporting Questionnaire 20 items (SRQ-20) respectively in personal interviews. Poisson regression models were fitted to determine the association of household food insecurity or low maternal social support with maternal common mental disorders, controlling for selected socio-demographic variables.ResultsThe mean age of the participants was 26.7 (± 6.68) years, and the mean FIES, SSS, and SRQ-20 scores were 5.62 [95% Confidence Interval (CI): 5.29–5.96] out of 8, 43.12 (95% CI: 41.34–44.90) out of 100, and 7.91 (95% CI: 7.38–8.45) out of 19 respectively. About two-thirds of the households (71.9%), and 72.7% and 49.5% of the women had food insecurity, low social support and probable common mental disorder respectively. In the adjusted analyses, a unit increase in FIES score was associated to a 4% increment in the predicted SRQ-20 score [Incident Risk Ratio (IRR) 1.04; 95% Confidence Interval (CI): 1.02, 1.06; p = 0.001], and the predicted SRQ-20 score of the women belonging to low social support category was 38% higher compared to that of women of high social support category (IRR 1.38; 95% CI: 1.14, 1.66; p = 0.001).ConclusionThe prevalence of household food insecurity and common mental disorders among mothers are high, and both household food insecurity and low social support are significantly related to common mental disorders in women. Interventions to reduce both household food insecurity, and common mental disorders in women are warranted, and should include social support for women.

Household food insecurity and children dietary diversity during the COVID-19 outbreak

The COVID-19 pandemic raises the likelihood of food insecurity and affects the nutritional status of children. This study aims to examine the relationship between household food poverty and children's dietary diversity. The cross-sectional study has done at Lhokseumawe City and North Aceh District. A sample of 289 children aged 12–59 months participated in this study. The Food Insecurity Experience Scale (FIES) was used to measure household food insecurity. Meanwhile, the Individual Dietary Diversity Score (IDDS) was done to assess children's dietary diversity. The Chi-square was employed to analyze the association between household food insecurity and children's dietary diversity. The prevalence of household food insecurity was 8,3%, consisting of 5,5% moderate food insecurity and 2,8% severe food insecurity. Most children had diverse food consumption (88,6%). However, 11,4% of children had low dietary diversity. Household food insecurity was significantly correlated with children's dietary diversity (p= 0,002). Conclusion, the COVID-19 pandemic indirectly impacts household food insecurity, which can worsen the condition of food consumption for toddlers, especially regarding diversity in food consumption. A strategy is required to assist food-insecure households to maintain and increase children's dietary diversity.

Household Food Insecurity and Associated Factors among Iranian Patients with Esophageal and Gastric Cancers.

Background: Household food insecurity (HFI) which has still been one of the major global public health issues is related to adverse health outcomes in individuals. Therefore, this study aimed to determine the prevalence of HFI and its associated factors in Iranian patients with esophageal and gastric cancers. Methods: The data of this cross-sectional study was obtained from 315 patients with esophageal and gastric cancers who were selected from a gastrointestinal cancer-based cohort study conducted in Firoozgar hospital, in Tehran. Food insecurity (FI) was measured using the Iranian version of the HFI questionnaire that was completed by a trained interviewer. The multivariable logistic regression model was used to determine the independent association of each factor with HFI. A P value lower than 0.05 was considered statistically significant. Results: The mean±SD of participants' age was 63.2±12.6 years and 65.4% were men. Most of the patients (75.8%) suffered from gastric cancer and 24.2% from esophageal cancer. The overall prevalence of FI among participants' households was 35.2%. There was an independent significant association between wealth index (WI) and HFI after the use of the multivariable logistic regression model, in such a way that the odds of FI in the poorest, poor, moderate, and rich patients' households were respectively, 6.41, 5.05, 2.74 and 2.04 times higher compared with the richest households. Conclusion: More than a third of participants' households struggled with FI, which was found to have a higher prevalence in loweconomic households. Therefore, health policymakers should intervene in food-insecure households by developing, establishing, and implementing strategies and control programs to improve affordable food access.

Household food insecurity and associated factors in South Ari district, Southern Ethiopia: A community-based cross-sectional study.

Household food insecurity is a major public health problem in Ethiopia despite the presence of various interventions implemented by the government. However, there is a dearth of evidence regarding the prevalence and responsible factors in Ethiopia, specifically in the South Ari district. This study, therefore, aimed to assess household food insecurity and associated factors in South Ari district, Southern Ethiopia. A community-based cross-sectional study was employed from March 11 to April 11, 2021, at South Ari district, Southern Ethiopia. A two-stage sampling technique was used to draw a sample of 717 households. Data were checked and entered into Epi-Data V3.2., and exported to SPSS V25.0 for data exploration and analysis. Variables with a p-value <0.25 in bivariable logistic regression were candidates for multivariable logistic regression. Multivariable logistic regression analysis was fitted to determine factors associated with household food insecurity. Hosmer-Lemeshow goodness-of-fit statistic was used to check model fitness and was satisfied. Adjusted odds ratio (AOR) with a 95% confidence interval (CI) was used to determine the strength of association. P-value <0.05 was used to declare statistical significance. The prevalence of household food insecurity was 44.8% (95% CI: 41.1%, 48.5%). Larger family size (8 and above) (AOR = 1.91, 95% CI: 1.10, 3.30), high dependency ratio (AOR = 2.71, 95% CI: 1.67, 4.40), medium dependency ratio (AOR = 1.72, 95% CI: 1.13, 2.62), poor wealth index (AOR = 2.30, 95% CI: 1.53, 3.46), not using agricultural extension service (AOR = 2.25, 95% CI: 1.57, 3.23), and non-beneficiary of productive safety net program (AOR = 1.71, 95% CI: 1.01, 2.87) were factors significantly associated with household food insecurity. The findings of this study showed a significant proportion of households were food insecure in South Ari District. Larger family size, high and medium dependency ratio, poor wealth index, not using agricultural extension service, and non-beneficiary of productive safety net program were significant risk factors associated with household food insecurity. Therefore, rigorous work is highly needed to enhance income-generating activities, strengthen agricultural productivity, expand the productive safety net program, and limit population pressure through improved family planning use.

Prevalence of household food insecurity in the marginal area of Birjand, Iran

IntroductionThe right to food is a human right. Food insecurity refers to the inability to prepare proper and healthy food. Food insecurity is associated with adverse outcomes among households. To determine the prevalence of food insecurity, its level, and the factors affecting it among households covered by health centers in marginalized areas of Birjand city.MethodsThis descriptive-analytical cross-sectional study was performed on 396 households aged 20-60 years covered by health centers in the marginal areas of Birjand in 2019. We used a 9-item Household Food Insecurity Access Scale (HFIAS) questionnaire. Chi-square, Fisher, and logistic regression analyses were performed using SPSS 22 software at a significance level of less than 0.05.ResultsThe mean age of the subjects was 34.04 ± 9.41 years. In total, 48% of the subjects had a level of food insecurity, of which 7.6% had severe, 12.6% moderate, and 27.5% mild insecurity. The prevalence of food insecurity in families with low members and higher levels of education is significantly lower (p &amp;lt; 001). Backward Logistic regression showed that illiteracy (OR: 8.26) and primary education (OR: 2.65), rental housing status (OR: 1.69), lower-income levels being covered by support organizations (OR: 3.13), employed members between 3-4 and more than five (OR: 2.73 and 2.93, respectively), and low weight (OR: 2.55) were associated with food insecurity. The prevalence of food insecurity in this study was high.ConclusionThe high prevalence of insecurity in studied households is an alarm for the authorities, which can be due to poverty and the low level of nutritional literacy of households' heads and mothers. Helping to improve the quality of life of families and food insecurity of households by improving physical and economic access, increasing literacy and nutritional culture, and reducing the burden of non-communicable diseases related to nutrition is a suggested solution.
