The prevalence of endophthalmitis after minimally invasive glaucoma surgery (MIGS) in this large 5-year series was 0.13% [0.04% - 0.30%]. All cases were associated with implantable devices. Outcomes were variable.

Prevalence of Endophthalmitis after Aqueous Shunt Implants and related procedures in an eye service

AbstractPurposeTo investigate the rate of endophthalmitis, its microorganisms, risk factors and visual outcomes, related to aqueous shunt implant (tube) surgery in the Glaucoma service in 5 years of laboratory electronic database.MethodsThis study was approved by the Clinical Audit Office. All eyes with tube implant or related surgery were reviewed from 2009 to 2013. Development of endophthalmitis was identified from the electronic database and notes reviewed.ResultsA total of 1827 tubes, 893 revisions and 2391 removal of stent suture (RoS) were performed in that period. In total six eyes developed endophthalmitis related to tube surgery, all were following revision/RoS, none were after tube implant only. Time till onset was 8 ± 6.7 days. The rate of endophthalmitis was 0.67% after tube revision and 0.25% after RoS. No microorganisms were identified in any samples.ConclusionEndophthalmitis is a rare complication of tube surgery, its rate in this study was less than reported in literature and was only observed after revision/RoS surgeries.

Postcataract surgery endophthalmitis in Port Harcourt, Nigeria

Background: Endophthalmitis is a rare but dreaded complication of cataract surgery. It is, therefore, imperative for cataract surgeons to know about its prevalence and how to prevent it in their institutions. Aim: The aim of this study was to determine the prevalence of postcataract surgery endophthalmitis in a private health facility in Port Harcourt. Methods: A retrospective analysis of the case files and theater records of 92 eyes of all 83 patients who had cataract surgery at the DDS Eye Centre and Surgery, Port Harcourt, Nigeria, between August 2006 and November 2012 was done. This included pre-, intra-, and post-operative care and features of endophthalmitis. Statistical software package SPSS version 17 was used to analyze the data (P Results: A total of 2484 (1490 males and 994 females) patients were seen in the private eye hospital in the period under review and 92 eyes of 83 (3.34%) patients (59 males and 24 females) were included in the study. Sixty-seven (73%) eyes had extracapsular cataract extraction with intra-ocular lens (ECCE + IOL) whereas twenty-five (27%) had small incision cataract surgery with intra-ocular lens (SICS + IOL). Among those who had ECCE, one (1.49%) developed endophthalmitis while two (8%) developed endophthalmitis following SICS, and they were all males which was statistically significant for surgery type. The prevalence of endophthalmitis in this study was 3.23%. Conclusion: The high prevalence of endophthalmitis in this study may be due to inadequate management of comorbid conditions such as corneal ulcer and diabetes. The incisions were temporally placed and sutures were not used which may have compromised the integrity of the wound in SICS, causing a higher prevalence of endophthalmitis with this method.

Postoperative endophthalmitis associated with intraocular lens implantation is one of the most severe complications of cataract surgery. The prevalence of endophthalmitis following these procedures reported in several multicenter studies over the last decades is 0.05% to 0.32%. Currently, general consensus measures are available to prevent this serious complication. Abiotic biofilm formation with adhesion of pathogens on the surface of intraocular devices is a novel theory that is little understood, but widely accepted to explain the pathophysiologic aspects of intraocular infection. Bacterial adhesion on intraocular lenses has been observed in clinical practice and investigated with several in vitro approaches in various experimental conditions. Improvements in the surgical techniques used in cataract surgery and the advent of new antimicrobial agents with a wider spectrum of action and better penetration in the eye, have resulted in significant advances in the prevention and treatment of intraocular implant-related infection.

Postoperative endophthalmitis is still one of the most fearsome complications of cataract surgery. The present review's aims are to study the etiology and pathogenesis of endophthalmitis and the criteria for antibiotic prophylaxis. Endophthalmitis prevalence is 0.07%-0.32% in cataract surgery. The clinical presentation needs to be perfectly known, even if none of the signs is pathognomonic. Bacteria predominantly cultured in postoperative endophthalmitis are gram-positive, especially Staphylococcus epidermidis. Most of the bacteria come from the patient. Bacterial adhesion to intraocular lenses (IOLs) takes place during their implantation, a prominent etiological factor. Polypropylene was the first biomaterial that proved this relation of cause and effect between bacterial adhesion and endophthalmitis. The benefit of antibiotic prophylaxis during cataract surgery has yet not been proven, since the low prevalence of endophthalmitis makes controlled studies with a large cohort difficult. The criteria and the four possible administration approaches (topical, subconjunctival, irrigation liquid, systemic) are analyzed.

RISKS OF INTRAVITREOUS INJECTION: A COMPREHENSIVE REVIEW

To evaluate the prevalence of the most common serious adverse events associated with intravitreous (IVT) injection. A systematic search of the literature via PubMed from 1966 to March 1, 2004, was conducted to identify studies evaluating the safety of IVT injection. Data submitted in New Drug Applications to the U.S. Food and Drug Administration for drugs administered into the vitreous were included where available. Serious adverse events reported in each study were recorded, and risk per eye and risk per injection were calculated for the following serious adverse events: endophthalmitis, retinal detachment, iritis/uveitis, intraocular hemorrhage, ocular hypertension, cataract, and hypotony. Rare complications also were noted. Data from 14,866 IVT injections in 4,382 eyes were analyzed. There were 38 cases of endophthalmitis (including those reported as pseudoendophthalmitis) for a prevalence of 0.3% per injection and 0.9% per eye. Excluding cases reported specifically as pseudoendophthalmitis, the prevalence of endophthalmitis was 0.2% per injection and 0.5% per eye. Retinal detachment, iritis/uveitis, ocular hypertension, cataract, intraocular hemorrhage, and hypotony were generally associated with IVT injection of specific compounds and were infrequently attributed by the investigators to the injection procedure itself. Retinal vascular occlusions were described rarely in patients after IVT injection, and it was unclear in most cases whether these represented true injection-related complications or chance associations. The risk of serious adverse events reported after IVT injection is low. Nevertheless, careful attention to injection technique and appropriate postinjection monitoring are essential because uncommon injection-related complications may be associated with permanent vision loss.
