Pyloric Stenosis Syndrome Revealing Chilaiditi Syndrome

Chilaiditi syndrome is a radiological manifestation of a large bowel interposition between the liver and right hemidiaphragm that associated with gastrointestinal symptoms, We report a case of 50-year-old woman with no particular pathological history who presented to the emergency room with a presentation of pyloric stenosis, The presence of Chilaiditi signs can be caused by an abnormality of either liver, colon or right hemidiaphragm that leads to sub-diaphragmatic space enlargement or intestinal hypermobility. Computed tomography imaging is the best diagnostic modality. Conservative treatment is the first line in management.

Gastric outlet obstruction in a 12 year old male

Abstract Pyloric stenosis presenting beyond infancy is uncommon and poorly understood. Here, we present a case of primary acquired gastric outlet obstruction due to pyloric stenosis in a 12-year-old male child. The patient presented after a four-year history of persistent non-bilious, post-prandial emesis and failure to thrive. A Heineke-Mikulicz pyloroplasty was performed. Pyloric biopsy was without diagnostic abnormality. A gastrostomy tube was also placed at the time of pyloroplasty. The patient was asymptomatic after surgery and had gained 25 pounds after three and a half months. The gastrostomy tube was removed 4 months after the pyloroplasty. Literature review yielded 42 cases, including the current report, of patients ages 4 months to 17 years with a diagnosis of primary acquired gastric outlet obstruction. Useful studies for diagnosis of these cases include abdominal ultrasound, upper GI contrast study and esophagogastroduodenoscopy. The most common curative procedure reported in the literature was a pyloroplasty (31 cases). The remaining patients underwent Bilroth I gastrectomy (7 cases), balloon dilatation (3 cases) and pyloromyotomy (1 case).

Prematurity Affects Age of Presentation of Pyloric Stenosis.

Term infants with hypertrophic pyloric stenosis (HPS) typically present between 4 and 6 weeks. There is limited consensus, however, regarding age of presentation of premature infants. We aim to determine if there is an association between the degree of prematurity and chronological age of presentation of HPS. A total of 2988 infants who had undergone a pyloromyotomy for HPS were identified from the 2012 and 2013 NSQIP-P Participant Use Files. Two hundred seventeen infants (7.3%) were born prematurely. A greater degree of prematurity was associated with an older chronological age of presentation ( P < .0001). Prematurity was significantly associated with an increase in overall postoperative morbidity, reintubation, readmission, and postoperative length of stay. When clinicians evaluate an infant with nonbilious emesis with a history of prematurity, they should consider pyloric stenosis if the calculated postconceptional age is between 44 and 50 weeks. When counseling families of premature infants, surgeons should discuss the increased incidence of postpyloromyotomy morbidity.

Time to stop upper gastrointestinal studies for pyloric stenosis

We refer to the case series on pyloric stenosis.1 The authors selected 25 of IHPS patient prospectively with data regarding history, examination, operation and full follow up with time. In the study, 48% of patients had no signs of dehydration, 36 % had no electrolyte abnormalities, and none had urinary pH abnormality. With widespread availability of health care, late presentation of pyloric stenosis is less common. More babies present early with one or fewer days of vomiting with no evidence of dehydration, electrolyte abnormalities or urinary paradoxical aciduria. However, sensitivity of presence of palpable pyloric mass whose sensitivity was reported to be up to 85% has reduced to 15.5% probably due to early presentation, decreasing clinical skill and increased use of imaging.

Hypertrophic Pyloric Stenosis in an Adolescent

Hypertrophic pyloric stenosis presenting beyond infancy is an uncommon occurrence. The etiology of adult onset pyloric stenosis is unknown. In this case report, we describe a 14-year-old boy who presented with nausea, intermittent vomiting, and abdominal pain. He underwent upper gastrointestinal series, which demonstrated persistent narrowing of the pylorus. He was referred to Pediatric Surgery for evaluation and subsequently underwent laparoscopic pyloroplasty. Postoperatively, the patient did well, and at 20 months, he continues to be asymptomatic.

Pyloric stenosis: Unusual presentations and rare associations

Pyloric stenosis is a common paediatric surgical condition with a well-recognised pattern of clinical features. Diagnosis is usually straightforward and a Ramstedt pyloromyotomy is curative. We present three patients who developed pyloric stenosis incidentally during the management of other primary surgical conditions. Issues of diagnostic difficulty following presentation of pyloric stenosis in the post-operative period (all three patients), its rare syndromic associations (one patient) and its management in this unusual situation are discussed. An awareness of the unusual presentations of pyloric stenosis during treatment for other primary surgical conditions can help in earlier diagnosis and treatment.

Gastric fluid volume in infants for pyloromyotomy.

To quantify gastric fluid volumes in infants with pyloric stenosis presenting for pyloromyotomy and to demonstrate endoscopically the efficacy of blind aspiration for gastric fluid recovery. We hypothesized that previous diagnostic contrast studies, preoperative nasogastric suction, and fasting interval would not affect these volumes. Seventy-five infants scheduled for pyloromyotomy were given atropine before induction of anaesthesia. For those who had undergone preoperative nasogastric suction, the nasogastric tube was aspirated and removed. A 14 F multiorificed orogastric catheter was blindly passed to aspirate gastric fluid for measurement. Following tracheal intubation, 15/75 subjects underwent gastroscopy to measure residual gastric fluid. Gastric fluid volume removed by blind aspiration averaged 4.8 +/- 4.3 ml.kg-1 with 83% of patients having > 1.25 ml.kg-1. Although 14 of the 15 patients evaluated by endoscope had < or = 1 ml residual gastric fluid, one had 1.8 ml.kg-1. Recovery of total gastric fluid volume by blind aspiration averaged 96 +/- 7%. The large gastric fluid volumes were independent of a history of barium study, preoperative nasogastric suction, and fasting interval. Infants with pyloric stenosis have large gastric fluid volumes which are not substantially reduced by preoperative nasogastric suction. Blind aspiration of gastric contents prior to induction of anaesthesia provides a reliable estimate of total gastric fluid for most of these infants, although the occasional infant may retain a small amount of gastric fluid. The clinical importance of such a residual volume is uncertain.
