The relationship between thyroid peroxidase antibody and differentiated thyroid cancer: a systematic review and meta-analysis.

Thyroglobulin antibody (TgAb) has been found to be associated with the occurrence and development of differentiated thyroid cancer (DTC) for several years, but there is still controversy over whether thyroid peroxidase antibody (TPOAb) is related to differentiated thyroid cancer. We scrutinized relevant studies published up to July 2023 across four major databases including PubMed, Embase, Cochrane Library, and Web of Science, to examine the association between TPOAb and DTC. Clinical outcome measures include the incidence of DTC, tumor size, extrathyroidal invasion, lymph node metastasis, multifocality, recurrence and bilaterality. 12 original studies were included, involving a total of 20,330 subjects. Our analysis of the included studies revealed that TPOAb+ individuals exhibited a higher risk of developing DTC (OR=1.57 [95% CI: 1.00-2.45], p=0.049) than TPOAb- individuals. Furthermore, TPOAb+ DTC patients were more prone to present with bilateral (OR=1.40 [95% CI: 1.21-1.62], p<0.00001) and multifocal (OR=1.40 [95% CI: 1.23-1.60], p<0.00001) tumors than TPOAb- patients. Sensitivity analysis indicated a high sensitivity for these three findings. No significant differences in the risk of extrathyroidal extension and lymph node metastasis, recurrence rate, tumor size, were observed between TPOAb+ and TPOAb- DTC patients. The presence of TPOAb is correlated with an increase prevalence of DTC. Development and Validation of a Clinical Risk Score to Predict Immune-mediated Liver Injury Caused by Sintilimab: Assessed for Causality Using Updated RUCAM.

Immune-mediated liver injury is a fatal side effect of sintilimab. This study aimed to shed light on the associated risk factors and characteristics of this adverse event. The clinical records of 772 patients treated with sintilimab were retrospectively reviewed to investigate risk factors associated with sintilimab immune-related hepatotoxicity, as well as its incidence and outcome. The Roussel Uclaf Causality Assessment Method was used to identify cases of sintilimab-induced hepatotoxicity. Furthermore, logistic regressions were performed to compare the clinical and bloodwork characteristics of patients with and without immune-mediated liver injury caused by checkpoint inhibitors. Of the 585 patients included in the study, 71 (12.1%) developed liver injury during sintilimab use. The median RUCAM score with interquartile range was 7 (6, 8). Hypoproteinemia, dyslipidemia, and the presence of thyroid peroxidase antibodies were risk factors for sintilimab-related hepatotoxicity. A nomogram model was constructed for sintilimab-induced immune-mediated liver injury based on these risk factors, which had a C-index value of 0.713 and a good calibration curve. When applied to patients with grade ≥3 and ≥4 sintilimab-induced immune-mediated liver injury, it achieved C-index values of 0.752 and 0.811, respectively. The nomogram model also showed a good prediction potential in patients ≥65 years and males. Six of the patients with sintilimab-related hepatotoxicity showed improved liver function upon treatment with steroids. This study demonstrated that hypoproteinemia, dyslipidemia, and the presence of thyroid peroxidase antibodies were clinically feasible prognostic biomarkers to predict liver injury in patients treated with sintilimab.

Potential Determinants of Thyroid Peroxidase Antibodies and Mortality Risk: Results From the ELSA-Brasil Study.

The presence of thyroid peroxidase antibodies (TPOAbs) may be considered as an indicator of adverse health outcomes. We aimed to investigate the potential determinants of TPOAb levels and to analyze the association between TPOAb titers and the risk of all- and specific-cause mortality. Baseline and longitudinal data of 13 187 participants from the ELSA-Brasil Study were analyzed. We investigated the association of TPOAb, detectability, positivity, and persistent positivity with sociodemographic and lifestyle factors using logistic regressions. Cox proportional hazards and Fine-Gray subdistribution hazard regression analyses were used to verify the association of TPOAbs with mortality. The determinants of TPOAb detectability and positivity were younger age, higher body mass index, female sex, and former and current smoking status. Black, mixed, and other self-reported races, intermediate and higher education, and heavy drinking were determinants of detectable and positive TPOAb levels. Female sex, White race, and former smoking were determinants of persistent TPOAb positivity at 2 visits, although only the female sex maintained its association at 3 visits. Moreover, after multivariate adjustment, there were associations between higher levels of TPOAbs and higher risk of cancer-related mortality among men, and TPOAb detectability and mortality by other causes among women. Sociodemographic and lifestyle-related factors were determinants of multiple TPOAb categories. TPOAb levels were associated with mortality risk; however, the low mortality rate in this sample might have compromised this finding. We suggest further studies to explore the clinical importance of detectable TPOAb levels, not only its positivity, as a potential marker of inflammation.

Antithyroid antibody (TPO-Ab) Status in Type 2 Diabetic Patients

Background: Presence of thyroid peroxidase antibody (TPO-Ab) in type 2 diabetic patients is a risk factor for future development of thyroid dysfunction.Objective: To observe thyroid antibody status in type 2 diabetic patients.Materials &amp; Methods: This cross sectional study was carried out in the Department of Physiology, Sir Salimullah Medical College, Dhaka from July 2016 to June 2017. Total 60 subjects including male and female, age ranged from 40 to 60 years were included in this study, among them 30 were non-diabetic subjects and 30 were type 2 diabetic patients.Results: In this study, mean serum TPO-Ab level was significantly (&lt;0.01) higher in diabetic subjects in comparison to that of apparently healthy non-diabetic subjects and mean serum TPO-Ab level was higher in uncontrolled diabetic subjects in comparison to that of controlled diabetic subjects but the difference is not statistically significant. However, On the other hand, among the diabetic patients 6.67% had positive TPO-Ab whereas, among the uncontrolled diabetic patients 11.76% had positive TPO-Ab. There was no correlation between TPO-Ab and HbA1c.Conclusion: The present study reveals that TPO-Ab may present in type 2 diabetic patients. KYAMC Journal Vol. 14, No. 01, April 2023: 35-38

What Should be the Thyroid Stimulating Hormone Cut-off Level in the First-Trimester Screening? A prospective Cohort Study and Mini Review of The Guidelines

Aim: Thyroid Stimulating Hormone (TSH) and Human Chorionic Gonadotropin (hCG) are glycoproteins that the alpha subunit common. As a consequence of this, hCG has intrinsic thyrotropic activity. Because of high levels of hCG, maternal serum thyrotropin level is seen to decrease in 80% of pregnancies. Novel investigation and several commission opinions recommend that the TSH cut-off value should be 2.5 mU/L in the first-trimester thyroid function screening.. We aimed to evaluate the contributions of the maternal serum TSH cut-off value of 2.5 mU/L to the development of maternal and fetal complications. Materials and Methods: We constructed the study with pregnancies in their first 12 weeks and planned to do only observation prospectively. We excluded pregnant women with systemic disease and any history of thyroid surgery or thyroid pathology. According to the TSH level, a case group(TSH level &amp;gt;2.5 mU/L) and a control group (TSH level&amp;lt;2.5 mU/L) were created through the pregnants with normal Thyroxine (T4) levels. The cohort group were divided into four subgroups according to whether they were anti-thyroid peroxidase (anti-TPO) positive or not. We observed the fetomaternal outcomes like pregnancy loss, hyperemesis gravidarum, hypertensive disorders, gestational diabetes, prelabour rupture of membranes, placental abruption, with routine prenatal visits until delivery; also delivery style, birth weight, shoulder dystocia, newborn intensive care needs, and postpartum hemorrhage were recorded. Results: The incidence of miscarriage in the subgroup with TSH &amp;gt;2.5 mU/L and anti-TPO (+) was significantly higher than in those with TSH &amp;lt;2.5 mU/L and anti-TPO (+) (p&amp;lt;0.05). All groups had no significant difference in other maternal or fetal/neonatal complications. Conclusion: If only the population-based nomograms are created, we may advise maternal serum TSH level as &amp;lt;2.5 mU/L for first-trimester screening. Single or multiple pregnancy status, gestational age, and the presence of thyroid peroxidase antibodies should also be taken into account when creating these nomograms.

Influence of Thyroid Peroxidase Antibodies Serum Levels in Graves' Disease: A Retrospective Cohort Study.

Purpose Graves' disease (GD) is an autoimmune disorder caused by the presence of antibodies to the thyroid stimulating hormone (TSH) receptor (TRAbs), usually presenting with clinical signs of hyperthyroidism. Previous evidence suggests that higher serum levels of thyroid peroxidase antibodies (TPOAbs) may lead to more sustained remission of hyperthyroidism after treatment with antithyroid drugs (AT). However, doubts about the influence of TPOAbs in Graves' disease outcomes still remain. Methods A retrospective, unicenter cohort study was performed. All patients with GD (TRAbs > 1.58U/L), biochemical primary hyperthyroidism (TSH < 0.4µUI/mL), and TPOAbs measurement at diagnosis, treated with AT between January 2008 and January 2021, were included for analysis. Results One hundred and forty-two patients (113 women) with a mean age of 52 ± 15 years old were included. They were followed up for 65.4 ± 43.8months. TPOAbs positivity was present in 71.10% (n=101) of those patients. Patients were treated with AT for a median of 18 (IQR (12; 24)) months. Remission occurred in 47.2% of patients. Patients with remission presented with lower TRAbs and free thyroxine (FT4) levels at the diagnosis. (p-value <0.001, p-value 0.003, respectively). No association was found in the median TPOAbs serum levels of patients who remitted and those who maintained biochemical hyperthyroidism after the first course of AT. Relapse of hyperthyroidism occurred in 54 patients (57.4%). No difference was found in TPOAbs serum levelsregarding the patient's relapse. Moreover, a time-based analysis revealed no differences in the relapse rate after 18 months of AT therapy between patients with and without TPOAbs positivity at the diagnosis(p-value0.176). It was found a weak positive correlation (r=0.295; p-value <0.05) between TRAbs and TPOAbs titters at the moment of Graves' diagnosis. Conclusion In this study, a correlation between TRAbs measurements and TPOAbs titter was described, although no significant association was found between the presence of TPOAbs and the outcomes of patients with GD treated with AT. These results do not support the use of TPOAbs as a useful biomarker to predict remission or relapse of hyperthyroidism in GDpatients.

TPO gene rs2071400 polymorphisms as an independent risk factor for hypothyroidism in Iraqi patients

Introduction and Aim: Thyroid peroxidase (TPO) is a thyroid-specific antigen, and the presence of TPO antibodies is known to be associated with developing hypothyroidism. In this study we aimed to investigate the genotypes prevalent for the SNP rs2071400 of the TPO gene and its association to the development of autoimmune thyroid disease. Materials and Methods: This study involved 43 patients with hypothyroidism and 44 healthy controls. Genotyping of the TPO SNP rs2071400 (C/T) was undertaken using Quantitative Real-Time Polymerase Chain Reaction using the TaqMan® Assay. Data obtained was subjected to statistical analysis using the SPSS 25 software Results: TPO rs2071400 T carriers (CT + TT genotypes) were more frequent in patients with hypothyroidism compared with healthy control (P=0.0006), with an adjusted odds ratio of 8.0. Serum levels of Anti-TPOAb were also significantly higher in hypothyroid patients. There was no significant correlation between rs2071400 T mutation and the presence of high Anti-TPOAb. Conclusion: Study revealed TPO rs2071400 polymorphism to be an independent risk factor for developing hypothyroidism in those with or without high serum levels of TPOAb.

Abstract P104: Association of Thyroid Peroxidase Antibodies and Mortality Risk in Participants of the ELSA-Brasil Cohort Study

Introduction: Thyroid peroxidase antibodies (TPOAb) has been considered a new indicator of low-grade inflammation. The presence of TPOAb was associated with a higher risk of cardiovascular diseases and mortality in previous studies. Hypothesis: We assessed the hypothesis that there is an association of TPOAb and risk of mortality in middle-aged participants from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). Methods: Baseline data of 6,115 men and 7,265 women (median age of 50 years-old) were analyzed. Fasting serum TPOAb levels were determined and categorized as detectable (&gt; 5.00 IU/mL) and positive (≥ 34.00 IU/mL). All-cause, cardiovascular, and cancer-related mortality data up to December 2018 were confirmed by death certificate. The association of TPOAb levels, detectability and positivity with mortality was determined by Cox regression, using hazard ratios (HR) and 95% confidence intervals (CI). The final model was adjusted for age, sex, research center, TSH, smoking status, natural log transformed (ln) BMI, alcohol intake, diabetes mellitus, (ln) serum CRP, (ln) serum LDL cholesterol, and hypertension. Results: After a median follow-up of 9 years, the number of events was 296, 86 and 112 for men, and 182, 50, and 85 for women, for all-cause, cardiovascular and cancer-related mortality, respectively. For men, higher levels of TPOAb were significantly associated with cancer-related mortality (HR: 1.25; CI: 1.00-1.57; p=0.049). There was no association of TPOAb levels with all-cause (HR: 1.07; CI: 0.92-1.25) and cardiovascular (HR: 0.73; CI: 0.50-1.08) mortality risk; neither of TPOAb positivity and detectability with any overall or specific mortality risk (p&gt;0.05). For women, the results showed no significant association of TPOAb levels with all-cause (HR: 1.10; CI: 0.97-1.25), cardiovascular (HR: 1.05; CI: 0.81-1.35), and cancer-related (HR: 1.08; CI: 0.89-1.31) mortality. No significant associations were seen between TPOAb detectability and positivity and mortality risk (p&gt;0.05). Conclusions: In conclusion, higher levels of TPOAb were associated with a trend for increased risk of cancer-related mortality among men. Despite this interesting association, a longer period of follow-up is necessary to confirm those results. ELSA-Brasil is a cohort with relatively younger participants and low rates of mortality.

Preconceptual thyroid peroxidase antibody positivity in women with recurrent pregnancy losses may contribute to an increased risk for another miscarriage.

To investigate preconceptual thyroid peroxidase antibody (TPO-ab) positivity and/or thyroid stimulating hormone (TSH) levels in the upper range of normal as risk factors for recurrent unexplained first-trimester miscarriage. A post-hoc study of a randomized trial, in which acetylsalicylic acid did not affect the risk of a new miscarriage. Women (n = 483) with at least three unexplained recurrent first-trimester miscarriages investigated at a Swedish secondary referral center. The levels of TPO-ab and TSH were determined before pregnancy. The occurrence of a new first-trimester miscarriage was analyzed by logistic regression with adjustments when applicable, for age, number of previous miscarriages, obesity and the investigated covariates levels of TPO-ab and TSH. Including all first trimester miscarriages, odds ratio (OR) according to presence of TPO-ab was 1.60 (95% confidence interval [CI]; 0.99-2.57), after adjustment 1.54 (95% CI; 0.94-2.53). Very early (biochemical) pregnancy losses occurred more often in women with than without preconceptual TPO-ab (6.8% vs. 2.0%), OR 3.51 (95% CI; 1.15-10.71), after adjustment 2.91 (95% CI; 0.91-9.29). There was no association between TSH in the upper range of normal and a new miscarriage, adjusted OR 0.76 (95% CI; 0.32-1.83). A prediction model for a new miscarriage included number of previous miscarriages, woman's age and presence of TPO-ab. In women with at least three recurrent unexplained pregnancy losses, the presence of TPO-ab may contribute to an increased risk of a first-trimester miscarriage, possibly more pronounced in very early pregnancy. TSH levels 2.5-4.0 mU/L do not seem to increase the miscarriage risk.

The prevalence of thyroid dysfunction in Jordan: a national population-based survey

BackgroundThe objectives of this study are to assess the prevalence of clinical and subclinical hypo- and hyperthyroidism and their associated factors among Jordanian adults.MethodsIn a cross-sectional population-based survey, a representative sample that included 3753 Jordanian adults was selected from the 12 governorates that represent the three regions of the country, in the year 2017. Sociodemographic and clinical data were obtained and blood samples were collected from all participants. Thyroid stimulating hormone (TSH), free tri-iodothyronine (FT3), free thyroxine (FT4), thyroglobulin antibody (TgAb) and thyroid peroxidase antibody (TPOAb) were measured to evaluate the thyroid function.ResultsThe overall prevalence of thyroid dysfunction was 11.9%. Around 76% of patients with thyroid dysfunction were previously undiagnosed. The prevalence of hypothyroidism and subclinical hypothyroidism was 3.1 and 5.3%, respectively. The prevalence of hyperthyroidism and subclinical hyperthyroidism was 1.0 and 2.5%, respectively. Female preponderance which was mainly related to hypothyroid disorders was evident. The prevalence of positive TPOAb and TgAb in the study population was 14.9 and 15.3%, respectively. The prevalence of detectable TPOAb and TgAb in the euthyroid participants was10.3 and 11.9%, respectively. Logistic regression analysis revealed that female sex, age ≥ 50 years and the presence of TgAb and TPOAb were strongly associated with hypothyroidism. Hyperthyroidism was significantly associated with the presence of TPOAb and age ≥ 50 years.ConclusionThe prevalence of unrecognized thyroid dysfunction is high among Jordanians. A public health policy of screening high risk groups particularly those ≥50 years of age is recommended.

Restoration of euthyroidism with levothyroxine: implications of etiology of hypothyroidism and the degree of residual endogenous thyroid function.

There are many thyroid-related factors that combine with non-thyroid-related factors in order to affect the patient response to treatment of hypothyroidism, in terms of their satisfaction with therapy. Some of the thyroid-derived factors include the etiology of the hypothyroidism and the amount of residual thyroid function that the patient retains. These two factors may be intertwined and affected by a third influence, the presence of thyroid peroxidase antibodies. The downstream consequences of the interactions between these three factors may influence both free thyroxine and free triiodothyronine levels, TSH concentrations, and various thyroid biomarkers. Evidence of the widespread importance of thyroid hormones can be inferred from the multiple genes that are regulated, with their regulation affecting multiple serum biomarkers. Thyroid biomarkers may extend from various well-known serum markers such as lipids and sex hormone-binding globulin to serum levels of thyroid hormone metabolites. Moreover, the interplay between thyroid hormones and biomarkers and their relative ratios may be different depending on the hypothyroidism etiology and degree of residual thyroid function. The ultimate significance of these relationships and their effect on determining patient-reported outcomes, quality of life, and patient satisfaction is, as yet, poorly understood. However, identification of better biomarkers of thyroid function would advance the field. These biomarkers could be studied and correlated with patient-reported outcomes in future prospective studies comparing the impact of various thyroid hormone therapies.

Association between vitamin D status and subclinical hypothyroidism

Abstract Aims and Objectives: The purpose of this study is to investigate the relation between Vitamin D levels and sub-clinical hypothyroidism and the effect of Vitamin D supplementation on treatment outcome. Design: A retrospective cohort file-based study. Method: In this study, the relation between Vitamin D level and subclinical hypothyroidism and the effect of Vitamin D supplementation on patients with subclinical hypothyroidism were evaluated where 30 patients were diagnosed with subclinical hypothyroidism followed up in Armed forces hospital southern region, KSA was included through electronic files revision. Results: The mean age was 41.6 +/- 12.4 years among the study group. Twenty-seven patients (90%) had their thyroid function test normalized after Vitamin D correction, while the rest had their TSH level improved from the baseline level. The presence of thyroid peroxidase antibodies (TPO) antibodies was found to be inversely related to TSH level after correction of Vitamin D level, and all patients who were negative for TPO 19 (100%) had their Thyroid Stimulating Hormone (TSH) level normalized after Vitamin D correction. In comparison, 3 (27.3%) of TPO-positive patients had subclinical hypothyroidism despite vitamin D correction. Conclusion: Vitamin D deficiency is inversely related to TSH, and TPO status and correction of Vitamin D deficiency in patients with subclinical hypothyroidism can normalize the thyroid function. Further studies are needed to establish the relationship between Vitamin D deficiency and subclinical hypothyroidism.

Screening of Thyroid Function Test During First Trimester of Pregnancy

Introduction: During pregnancy maternal thyroid dysfunction has been associated with a number of adverseoutcomes, like preterm birth, placental abruption, fetal death and impaired neurological development inthe child. The presence of Thyroid Peroxidase Antibody (TPO-Ab) results miscarriage, preterm birth andmaternal post-partum thyroid disease. Hypothyroidism is closely associated with the presence of ThyroidPeroxidase Antibody. If a pregnant woman is positive for TPO-Ab in early pregnancy, her chances todeveloping thyroid abnormalities.Objective: To find out the level of TPO-Ab and thyroid status in first trimester of pregnancy. Method:Observational cross-sectional Study was designed in Department of Biochemistry, Medical college,Vadodara, Gujarat, India.Sample Size: 200 Normal pregnant women that randomly selected from the first trimesters of pregnancy.The study parameters were - thyroid peroxidase anti-body (TPO-Ab); serum thyroid stimulating hormone(TSH); serum T3 and serum T4.Result: Our study found that 30(15%) pregnant women of first trimester was hypothyroidism (TSH&gt;2.5mIU/l) Out of these 30 females, 9(4.5%) had overt hypothyroidism &amp; 21 (10.5%) had subclinicalhypothyroidism, among these 30 subjects, 26(13%) had found TPO-Ab positive (TPO-Ab &gt;35 IU//mL)there was a significant positive correlation between positive TPO-Ab and serum TSH level of study subjectsand there was a negative correlation between serum TSH and serum T4,T3 level in study subjects.Conclusion: In this study we found 15% prevalence of hypothyroidism (4.5 % overt &amp; 10.5 % subclinical)in first trimester of pregnancy. We found positive TPO-Ab in 13% of pregnant females and all these hadsome thyroid dysfunctions, subclinical or overt hypothyroidism, subclinical being much more common thanovert hypothyroidism.

Recent Issues Related to Thyroid Disease in Pregnancy

Maternal and fetal complications may occur because of pathologic or immunologic changes during pregnancy. The American Thyroid Association (ATA) suggests an optimal thyroid stimulating hormone (TSH) reference range of 0.50-4.00 mU/L in pregnant women. Based on Korean data, the same range may be applied to Korean pregnant women. According to the ATA guideline, levothyroxine therapy is recommended for thyroid peroxidase antibody (TPOAb)-positive women with a TSH greater than the pregnancy-specific reference range (approximately ＞4.0 mU/L in Korea) and TPOAb-negative women with a TSH ＞10.0 mU/L. The presence of TPOAb may be a sign of hypothyroidism due to damage to the thyroid. Because the titer of TPOAb decreases as gestation progresses, its measurement should be performed as early as possible during pregnancy. Although the mechanism is unknown, the association between thyroid autoimmunity and miscarriage/premature delivery is clear. Selenium may reduce the development of postpartum thyroiditis and permanent hypothyroidism; however, routine prescription of selenium is not recommended as it may increase the risk of type 2 diabetes. According to Korean nationwide data, birth defects in antithyroid drug (ATD)-exposed offspring in early pregnancy increased by 1.1 to 2.2% compared with non-exposed offspring. Avoidance of ATD in early pregnancy is the best option, otherwise, it is preferable to switch to propylthiouracil before pregnancy. When methimazole use is unavoidable in early pregnancy, it is recommended to use less than 5 mg per day.

The Presence of Thyroid Peroxidase Antibody Is Associated with Lower Placental Weight in Maternal Thyroid Dysfunction.

Thyroid dysfunction (TD) is caused by thyroid peroxidase (TPO) antibody, as seen in Hashimoto's disease. TD is a common problem of reproductive age and may impair fetal development. Here, we determined the effect of TPO antibody on perinatal outcomes in Japanese women with TD before conception. A retrospective study involving cases of maternal TD with term singleton birth was conducted. The subjects with TD were divided into two groups according to the presence (n = 22) or absence (n = 20) of TPO antibody. The control groups matched for age, parity, and gestational weeks were selected for TPO antibody-positive (n = 44) and -negative TD subjects (n = 40), respectively. Using the standard curve of Japanese placental weight, the frequency of placental weight less than the 50th percentile (small placenta) was examined. Placental weight was lower among TPO antibody-positive TD subjects, compared with TPO antibody-negative TD subjects (p < 0.01). However, other outcomes were similar between the groups. Importantly, compared with control mothers, placental weight was significantly lower (p < 0.01), birth weight tended to be lower (p = 0.07), and the incidence of gestational diabetes mellitus was higher (p = 0.02) among TPO antibody-positive subjects. There was no significant difference in placental weight between TPO antibody-negative subjects and controls. The frequency of small placenta was significantly higher in TPO antibody-positive subjects (odds ratio: 16.7) even when considering diabetes and pregnancy induced hypertension. Thus, the presence of TPO antibody is associated with lower placental weight among Japanese women having TD.

Association between diffuse lymphocytic infiltration and papillary thyroid cancer aggressiveness according to the presence of thyroid peroxidase antibody and BRAFV600E mutation.

Diffuse lymphocytic infiltration (DLI) is frequently found with papillary thyroid cancer (PTC), so there has been long interest in how it affects the characteristics of PTC. This purpose of this study was to define the association between DLI and PTC aggressiveness according to thyroperoxidase antibody (TPOAb) and B-type Raf (BRAF)V600E mutation positivity. There were 1879 patients with PTC who underwent surgery and were enrolled in this study. Clinicopathologic characteristics were compared between groups according to the presence of DLI and TPOAb. Multiple logistic regression analysis was conducted to assess odds ratio (OR) for each dependent variable (BRAFV600E mutation, tumor size >1.0 cm, multifocality, extrathyroidal extension, and lymph node metastasis) of each group according to the presence of DLI and TPOAb, with the group with neither DLI or TPOAb (DLI-negative TPOAb-negative PTC) as the reference. The DLI-positive PTC showed more frequent multifocality and less frequent BRAFV600E mutation than DLI-negative PTC. Among patients with DLI-positive PTC, extrathyroidal extension and BRAFV600E mutation was less frequent when serum TPOAb was positive. In multiple logistic regressions, DLI-positive TPOAb-positive PTC showed a high OR for multifocality (1.410; P = .017), but low ORs for BRAFV600E mutation (0.521; P < .001) and extrathyroidal extension (0.691; P = .008). The patients with DLI-positive TPOAb-positive PTCs showed a high OR for multifocality (1.588; P = .002), and high ORs for tumor size >1.0 cm (2.205; P = .019) and lymph node metastasis (2.005; P = .032) in subgroup analyses of PTC with wild-type BRAF. The DLI-negative TPOAb-positive group was not associated with any tumor aggressiveness-related variables. Although DLI was associated with multifocality regardless of TPOAb positivity, it was associated with an indolent feature when TPOAb was positive but with aggressive features in PTC with wild-type BRAF when TPOAb was negative. The TPOAb and BRAF status may help to define the clinical implication of lymphocytic infiltration found with PTC.

Effect of Thyroid Peroxidase Antibody on Pregnancy Outcome among Hypothyroid Women

Introduction: Thyroid disorders are among the most common endocrine disorders affecting pregnant women. About 2 to 3% of pregnant women are affected by subclinical hypothyroidism and about 0.3%-0.5% by overt hypothyroidism. Thyroid autoantibodies are found in 5-15% of women in childbearing age and have been associated with adverse pregnancy outcomes. Aim: To compare the pregnancy outcomes in thyroid peroxidase antibody positive and thyroid peroxidase antibody negative hypothyroid women. Materials and Methods: This was a retrospective study done in Dr TMA Pai Hospital, Udupi, Karnataka, India from January 2015 to December 2016. All singleton pregnant women with hypothyroidism were included and multiple pregnancy, pre-existing hypertensive or diabetic pregnancies were excluded. Serum Thyroid Stimulating Hormone (TSH) levels were estimated during booking visit for all pregnant women and if >2.5μIU/ml, free T3, T4, and Thyroid Peroxidase Antibody (TPO-Ab) levels were done. Data were retrieved from the hospital records and divided into TPO-Ab negative and TPO-Ab positive groups. The analysis was done using SAS v9.3. Chi-square test and Fisher- Exact test were used for testing equality of proportions. The p-value <0.05 was taken as significant. Results: 2385 antenatal women screened with TSH during the booking visit in study period, 169 (7.08%) were hypothyroid. 100 were TPO-Ab negative and 69 were TPO-Ab positive with 40.8% prevalence of TPO-Ab positivity among the hypothyroid pregnant women. The initial TSH and the average Levothyroxine requirement was 37% (p-value 0.206) and 16% (p-value 0.388) higher in the TPO-Ab positive group than in TPO-Ab negative group. Though the odds for having miscarriage was 2.36 (0.94,5.92), gestational hypertension was 2.71 (0.76, 9.69) and gestational diabetes was 2.88 (0.92,9.03) times higher in the TPO-Ab positive group as compared to TPO-Ab negative group, the p-value for miscarriage was 0.061, gestational hypertension was 0.125 and that for gestational diabetes was 0.062 and hence, statistically not significant. However, there was no difference in the preterm delivery rate in our study. Conclusion: Presence of thyroid peroxidase antibodies may slightly increase the risk of miscarriage, gestational hypertension and gestational diabetes mellitus in pregnancy in the Indian population.

Thyroid peroxidase antibodies during early gestation and the subsequent risk of first-onset postpartum depression: A prospective cohort study

BackgroundDuring the postpartum period, women are at risk for the new onset of both auto-immune thyroid disorders and depression. The presence of thyroid peroxidase antibodies (TPO-ab) during early gestation is predictive for postpartum auto-immune thyroid dysfunction. The aim of this study was to investigate the association between TPO-ab status during early gestation and first-onset postpartum depression. MethodsProspective cohort study (n = 1075) with follow-up during pregnancy up to one year postpartum. Thyroid function and TPO-ab status were measured during early gestation. Depressive symptomatology was assessed during each trimester and at four time points postpartum with the Edinburgh Depression Scale (EDS). Women with antenatal depression were not eligible for inclusion. Self-reported postpartum depression was defined with an EDS cut-off of ≥ 13. ResultsThe cumulative incidence of self-reported first-onset depression in the first postpartum year was 6.3%. A positive TPO-ab status was associated with an increased risk for self-reported first-onset depression at four months postpartum (adjusted OR 3.8; 95% CI 1.3–11.6), but not at other postpartum time points. Prevalence rates of self-reported postpartum depression declined after four months postpartum in the TPO-ab positive group, but remained constant in the TPO-ab negative group. LimitationsDepression was defined with a self-rating questionnaire (EDS). ConclusionsWomen with an increased TPO-ab titer during early gestation are at increased risk for self-reported first-onset depression. The longitudinal pattern of self-reported postpartum depression in the TPO-ab positive group was similar to the typical course of postpartum TPO-ab titers changes. This suggests overlap in the etiology of first-onset postpartum depression and auto-immune thyroid dysfunction. Thyroid function should be evaluated in women with first-onset postpartum depression.
