Electrocortical activity correlated with locomotor adaptation during split-belt treadmill walking.

Locomotor adaptation is crucial for daily gait adjustments to changing environmental demands and obstacle avoidance. Mobile brain imaging with high-density electroencephalography (EEG) now permits quantification of electrocortical dynamics during human locomotion. To determine the brain areas involved in human locomotor adaptation, we recorded high-density EEG from healthy, young adults during split-belt treadmill walking. We incorporated a dual-electrode EEG system and neck electromyography to decrease motion and muscle artefacts. Voluntary movement preparation and execution have been linked to alpha (8-13Hz) and beta band (13-30Hz) desynchronizations in the sensorimotor and posterior parietal cortices, whereas theta band (4-7Hz) modulations in the anterior cingulate have been correlated with movement error monitoring. We hypothesized that relative to normal walking, split-belt walking would elicit: (1) decreases in alpha and beta band power in sensorimotor and posterior parietal cortices, reflecting enhanced motor flexibility; and (2) increases in theta band power in anterior cingulate cortex, reflecting instability and balance errors that will diminish with practice. We found electrocortical activity in multiple regions that was associated with stages of gait adaptation. Data indicated that sensorimotor and posterior parietal cortices had decreased alpha and beta band spectral power during early adaptation to split-belt treadmill walking that gradually returned to pre-adaptation levels by the end of the adaptation period. Our findings emphasize that multiple brain areas are involved in adjusting gait under changing environmental demands during human walking. Future studies could use these findings on healthy, young participants to identify dysfunctional supraspinal mechanisms that may be impairing gait adaptation. KEY POINTS: Identifying the location and time course of electrical changes in the brain correlating with gait adaptation increases our understanding of brain function and provides targets for brain stimulation interventions. Using high-density EEG in combination with 3D biomechanics, we found changes in neural oscillations localized near the sensorimotor, posterior parietal and cingulate cortices during split-belt treadmill adaptation. These findings suggest that multiple cortical mechanisms may be associated with locomotor adaptation, and their temporal dynamics can be quantified using mobile EEG. Results from this study can serve as a reference model to examine brain dynamics in individuals with movement disorders that cause gait asymmetry and reduced gait adaptation.

Characterization of Spinal Sensorimotor Network Using Transcutaneous Spinal Stimulation during Voluntary Movement Preparation and Performance

Transcutaneous electrical spinal stimulation (TSS) can be used to selectively activate motor pools based on their anatomical arrangements in the lumbosacral enlargement. These spatial patterns of spinal motor activation may have important clinical implications, especially when there is a need to target specific muscle groups. However, our understanding of the net effects and interplay between the motor pools projecting to agonist and antagonist muscles during the preparation and performance of voluntary movements is still limited. The present study was designed to systematically investigate and differentiate the multi-segmental convergence of supraspinal inputs on the lumbosacral neural network before and during the execution of voluntary leg movements in neurologically intact participants. During the experiments, participants (N = 13) performed isometric (1) knee flexion and (2) extension, as well as (3) plantarflexion and (4) dorsiflexion. TSS consisting of a pair pulse with 50 ms interstimulus interval was delivered over the T12-L1 vertebrae during the muscle contractions, as well as within 50 to 250 ms following the auditory or tactile stimuli, to characterize the temporal profiles of net spinal motor output during movement preparation. Facilitation of evoked motor potentials in the ipsilateral agonists and contralateral antagonists emerged as early as 50 ms following the cue and increased prior to movement onset. These results suggest that the descending drive modulates the activity of the inter-neuronal circuitry within spinal sensorimotor networks in specific, functionally relevant spatiotemporal patterns, which has a direct implication for the characterization of the state of those networks in individuals with neurological conditions.

Bereitschaftspotential in Multiple System Atrophy.

Objective: Multiple system atrophy (MSA) is a neurodegenerative disorder manifesting as parkinsonism, cerebellar ataxia, and autonomic dysfunction. It is categorized into MSA with predominant parkinsonism (MSA-P) and into MSA with predominant cerebellar ataxia (MSA-C). The pathophysiology of motor control circuitry involvement in MSA subtype is unclear. Bereitschaftspotential (BP) is a feasible clinical tool to measure electroencephalographic activity prior to volitional motions. We recorded BP in patients with MSA-P and MSA-C to investigate their motor cortical preparation and activation for volitional movement.Methods: We included eight patients with MSA-P, eight patients with MSA-C, and eight age-matched healthy controls. BP was recorded during self-paced rapid wrist extension movements. The electroencephalographic epochs were time-locked to the electromyography onset of the voluntary wrist movements. The three groups were compared with respect to the mean amplitudes of early (1,500–500 ms before movement onset) and late (500–0 ms before movement onset) BP.Results: Mean early BP amplitude was non-significantly different between the three groups. Mean late BP amplitude in the two patient groups was significantly reduced in the parietal area contralateral to the movement side compared with that in the healthy control group. In addition, the late BP of the MSA-C group but not the MSA-P group was significantly reduced at the central parietal area compared with that of the healthy control group.Conclusions: Our findings suggest that patients with MSA exhibit motor cortical dysfunction in voluntary movement preparation and activation. The dysfunction can be practicably evaluated using late BP, which represents the cerebello-dentato-thalamo-cortical pathway.

A peek into premonitory urges in Tourette syndrome: Temporal evolution of neurophysiological oscillatory signatures

BackgroundTics are the core symptom of patients with Gilles de la Tourette syndrome, yet the spatial-temporal dynamics of neural activity causing a tic remains to be determined. ObjectiveIdentification of cortical events preceding tic onset. MethodsIn twelve patients with Tourette syndrome we performed magnetoencephalography to trace the time course of beta oscillations (15–30 Hz) in motor cortical areas before tic onset. ResultsPatients showed a biphasic modulation of cortical beta activity during the second before tic onset. We observed an initial increase of beta power over the left-hemispheric channels overlying the motor cortex. This increase was subsequently replaced by a decrease in beta power. The beta decrease close to tic onset resembled the typical pattern accompanying preparation of voluntary movements. Only the initial increase in beta power positively correlated with the intensity of motor urges preceding tics. ConclusionsThe spatial-temporal dynamics of cortical activity suggests a voluntary component of tics that might be triggered by a failure of compensatory motor inhibitory mechanisms.

A Comparison of Cognitive Load and Emotional Arousal on Liar's Pupil Diameter Changes

Pupil size is an easy-to-measure, non-invasive method to index various cognitive processes. Although a growing number of studies have incorporated measures of pupil size into clinical investigation, there have only been limited studies in Parkinson's disease (PD). Convergent evidence has suggested PD patients exhibit cognitive impairment at or soon after diagnosis. Here, we used an interleaved pro- and anti-saccade paradigm while monitoring pupil size with saccadic eye movements to examine the relationship between executive function deficits and pupil size in PD patients. Subjects initially fixated a central cue, the color of which instructed them to either look at a peripheral stimulus automatically (pro-saccade) or suppress the automatic response and voluntarily look in the opposite direction of the stimulus (anti-saccade). We hypothesized that deficits of voluntary control should be revealed not only on saccadic but also on pupil responses because of the recently suggested link between the saccade and pupil control circuits. In elderly controls, pupil size was modulated by task preparation, showing larger dilation prior to stimulus appearance in preparation for correct anti-saccades, compared to correct pro-saccades, or erroneous pro-saccades made in the anti-saccade condition. Moreover, the size of pupil dilation correlated negatively with anti-saccade reaction times. However, this profile of pupil size modulation was significantly blunted in PD patients, reflecting dysfunctional circuits for anti-saccade preparation. Our results demonstrate disruptions of modulated pupil responses by voluntary movement preparation in PD patients, highlighting the potential of using low-cost pupil size measurement to examine executive function deficits in early PD.

T151. Visuospatial processing load enhance the brain activity associated with motor preparation

Introduction A cortical region showing an event-related desynchronization (ERD) in the electroencephalographic (EEG) alpha power indicates that it is activated in preparation for voluntary movements. The aim of this study is to search for visual and spatial condition that degree and distribution of alpha band ERD by loading greater spatial attention with movement. Methods We measured movement-related ERD of scalp EEG over central to occipital areas. Just like Libet’s Clock paradigm, subjects were seated in front of a computer screen that displayed a clock rotating at 3 s per cycle. They were instructed to register in mind the onset time of the self-paced movement. After the movement, they reported the position of the clock when the movement took place. [Measurement 1] Two types of movement tasks were executed while the clock rotating direction is clockwise. In reaching trials, subjects reached the center of the screen with their right hand. In control trials, subjects performed a simple right wrist extension on the chair armrest. [Measurement 2] A wrist extension movement with right and left hand was performed in each of two kinds of rotating directions (clockwise and counterclockwise) of clock. Data analysis: We analyzed the effect of clock position, clock rotating direction, electrode position, side of hand, and type of movement to the EEG activity before the movement onset. The time-frequency representation of the EEG power was calculated using short-time Fourier transformation. Results [Measurement 1] In wrist extension, alpha band ERD was significantly greater in bilateral parieto-occipital area when clock position was in the left visual field (LVF) (while the clock is moving up) compared with the right visual field (RVF) (while the clock is moving down). Reaching trials showed the significant alpha band ERD in bilateral parieto-occipital area in both trials of LVF and RVF. [Measurement 2] There was a significant interaction between clock rotating direction and clock position that showed alpha band ERD in bilateral parieto-occipital area when the clock is moving up was significantly greater than that when the clock is moving down. This modulation is in common for the both left hand right hand movement. The common finding in measurement 1 and 2 is that alpha band ERD in the posterior area was greater in ipsilateral hemisphere to attended visual hemi-field compared with contralateral hemisphere. Conclusion We showed that alpha band power at motor preparation stage was significantly altered in posterior cortex by applying an appropriate spatial load in peripheral visual field without changing the movement itself.

P298 Can music enhance movement intention?

Objective Active participation of patients themselves in rehabilitation is a vital factor for the stroke recovery. Since music can create positive emotion and sometimes trigger spontaneous movements, such as dancing, combination of music and rehabilitation might be beneficial in improving patients’ mental and physical functions. To clarify the influence of music on human movement intention and to establish the significance of rehabilitation combined with music, we tested whether music may activate the cortical preparation of voluntary movement. Methods Fifteen healthy right-handed subjects were recruited. The task was to voluntarily press the button by using the right index finger at their own paces (every 5–10 s apart). Two conditions with different auditory stimulation were performed; music and non-musical sounds (reversed music). The movement-related cortical potentials (MRCP) were recorded for the central electrodes (C3, C1, Cz, C2, C4). The early and later parts (from −1000 to −500 ms and from −500 to 0 ms, respectively) of MRCP were statistically analyzed. Results As a result, the later MRCP amplitudes were significantly larger in the music condition than those in the non-music condition. In contrast, the earlier MRCP was not affected by the music. Discussion It is suggested that the combined use of music might activate the movement readiness condition, which can be achieved through the mental and motivational process of the brain. Conclusions Music may enhance the cortical preparatory process for movements, including mental effects. Significance Using music together with rehabilitation may be useful for long-term rehabilitation.

Movement-related potentials in Parkinson's disease.

To date, many different approaches have been used to study the impairment of motor function in Parkinson's disease (PD). Event-related potentials (ERPs) are averaged amplitude fluctuations of the ongoing EEG activity that are time locked to specific sensory, motor or cognitive events, and as such can be used to study different brain processes with an excellent temporal resolution. Movement-related potentials (MRPs) are ERPs associated with processes of voluntary movement preparation and execution in different paradigms. In this review we concentrate on MRPs in PD. We review studies recording the Bereitschaftspotential, the Contingent Negative Variation, and the lateralized readiness potential in PD to highlight the contributions they have made to further understanding motor deficits in PD. Possible directions for future research are also discussed.

Cortical facilitation of proprioceptive inputs related to gravitational balance constraints during step preparation

Several studies have shown that the transmission of afferent inputs from the periphery to the somatosensory cortex is attenuated during the preparation of voluntary movements. In the present study, we tested whether sensory attenuation is also observed during the preparation of a voluntary step. It would appear dysfunctional to suppress somatosensory information, which is considered to be of the utmost importance for gait preparation. In this context, we predict that the somatosensory information is facilitated during gait preparation. To test this prediction, we recorded cortical somatosensory potentials (SEPs) evoked by bilateral lower limb vibration (i.e., proprioceptive inputs) during the preparation phase of a voluntary right-foot stepping movement (i.e., stepping condition). The subjects were also asked to remain still during and after the vibration as a control condition (i.e., static condition). The amplitude and timing of the early arrival of afferent inflow to the somatosensory cortices (i.e., P1-N1) were not significantly different between the static and stepping conditions. However, a large sustained negativity (i.e., late SEP) developed after the P1-N1 component, which was larger when subjects were preparing a step compared with standing. To determine whether this facilitation of proprioceptive inputs was related to gravitational equilibrium constraints, we performed the same experiment in microgravity. In the absence of equilibrium constraints, both the P1-N1 and late SEPs did not significantly differ between the static and stepping conditions. These observations provide neurophysiological evidence that the brain exerts a dynamic control over the transmission of the afferent signal according to their current relevance during movement preparation.

This sustained activity is reflected in the widely studied readiness potential (RP), which provides insight into the brain processes underlying the preparation of actions. Key contributing regions have been identified, including the middle cingulate cortex (MCC) and the supplementary motor area (SMA), but the relationship of activity within and between these regions to the RP is not understood. We sought to examine this relationship by integrating simultaneously acquired EEG and fMRI through computational modelling. Method We acquired simultaneous EEG and fMRI data from 20 healthy participants as they performed a simple task of voluntary, self-paced finger movements made every 10–15 seconds. We applied an EEG-informed fMRI analysis to examine trial-by-trial correlation between BOLD signals and neural responses reflected in the RP and global field power (GFP) activity preceding movements. We then used dynamic causal modelling (DCM) to examine interactions between the network of SMA and MCC during the preparation and performance of actions. DCM models were therefore informed by movement events as well as contextual modulators derived from EEG activity. The latter accounts for the correlation shown by the EEG-informed fMRI analysis. Bayesian model selection with a random-effects analysis was employed to select the most likely model of cortical interactions for our data. Results and Discussion The EEG-informed fMRI analysis revealed a positive correlation between GFP over the period from 750 ms to 400 ms prior to movement and BOLD responses in MCC. Specifically, trials in which neural activity in the GFP was greater over this premovement period also involved greater activation of the MCC. The best fitting model to infer the interactions between MCC and SMA comprised the GFP regulating the self-feedback connection of MCC to itself, and MCC activity regulating the self-feedback connection of SMA. This result suggests that the MCC modulates its own activity, and this modulation is strongest on trials with greater premovement GFP activity. Together, our study suggests that the internal modulation of MCC is essential to sustain premovement activity over time, and drives SMA activity in preparation for actions. The role of arousal in the preparation for voluntary movement

Planning and readiness for action are associated with pre-movement brain activity reflected in the readiness potential (RP). Previous research suggests that RP is affected by higher-order cognitive functions. The present study investigated the relationship between arousal and RP. Twenty participants performed a RP paradigm in which they executed self-paced movements approximately every 4–5 s. Participants’ arousal level was directly manipulated through interaction with the experimenter during the rest breaks preceding the movement task. Skin conductance level (SCL) differed between arousal conditions, indicating that the arousal manipulation was effective. RP was significantly higher under the low arousal than the high arousal condition. This arousal effect also changed depending on whether RP was measured at overall high or low levels of arousal. Our data indicate that arousal does not directly activate structures underlying action preparation. We suggest that the arousal effect may be mediated by the attentional resources allocated to the movement.

Motor timing and motor sequencing contribute differently to the preparation for voluntary movement

Two crucial processes preceding voluntary action are determining the time for movement initiation and planning of the specific sequence of motor output. In this study we aimed to differentiate the neural activity related to motor timing and motor sequencing and to examine over what time periods they contribute to premovement activity during the readiness for voluntary action. Eighteen participants performed self-initiated voluntary finger movements in a readiness potential paradigm, both during EEG measurement and during fMRI. The finger movement task involved three conditions: (1) simple repetitive sequences; (2) increased demand on the sequencing of movement order; and (3) increased demand on the timing of movement initiation. Functional MRI and 64 channels EEG were conducted in two separate sessions. Motor timing and motor sequencing were found to involve different neural processes occurring at different times prior to movement initiation. Motor timing involved greater activation in lateral prefrontal regions over the earliest part of premovement activity, from 1200 ms before movement onset. Motor sequencing involved greater activation of dorsal premotor and parietal areas and was reflected in central and parietal scalp regions only over the later part of premovement activity, within 600 ms of movement onset. We suggest that different neural processes contribute to different aspects of the intended action over different time periods during the preparation for movement, and it is the coordinated activity of these multiple regions that is represented in premovement activity during the readiness for voluntary action.

Preparing for a motor perturbation: Early implication of primary motor and somatosensory cortices

Although preparation of voluntary movement has been extensively studied, very few human neuroimaging studies have examined preparation of an intentional reaction to a motor perturbation. This latter type of preparation is fundamental for adaptive motor capabilities in everyday life because it allows a desired motor output to be maintained despite changes in external forces. Using fMRI, we studied how the sensorimotor cortical network is implicated in preparing to react to a mechanical motor perturbation. While maintaining a given wrist angle against a small force, subjects were instructed to prepare a reaction to a subsequent wrist angle displacement. This reaction consisted of, either resisting the imposed movement, or remaining passive. During the preparation of both reactions we found an early implication of M1 and S1 but no implication at all of the higher order motor area preSMA. This is clearly different from what has been found for voluntary movement preparation. These results show that the sensorimotor network activation during preparation of voluntary motor acts depends on whether one expects a motor perturbation to occur: when external forces can interfere with ongoing motor acts, the primary sensorimotor areas must be ready to react as quickly as possible to perturbations that could prevent the goal of the ongoing motor act from being achieved.

Corticospinal control of wrist muscles during expectation of a motor perturbation: A transcranial magnetic stimulation study

Voluntary movement is often perturbed by the external forces in the environment. Because corticospinal (CS) control of wrist muscles during preparation of voluntary movement has been extensively studied without variation in the external forces, very little is known about the way CS control adapts when subjects expect motor perturbations. Here, we studied the CS control of wrist muscles during expectation of an imposed wrist extension. Subjects were instructed either to compensate (COMP) the perturbation (applied at variable delays) or not to intervene (NINT). In a quarter of all trials at random, in the time window when perturbation might occur, TMS was applied over contralateral M1. Motor evoked potentials (MEPs) were measured in the FCR (flexor carpi radialis) and ECR (extensor carpi radialis) muscles, as well as the silent period (SP) in the FCR. Following the perturbation, we found a larger long-latency stretch reflex in COMP than in NINT. During the expectation of the perturbation, MEP amplitudes did not differ across conditions in FCR. However, those evoked in ECR were greater in COMP than in NINT condition. Moreover in the FCR, the silent period lasted longer in NINT. Thus, we showed a selective effect of the prepared reaction on the anticipatory tuning of CS excitability and cortical inhibition in the agonist/antagonist muscles. This tuning clearly differed from the tuning during voluntary movement preparation without variation in the external forces. This shows that the tuning of the CS system during motor preparation depends on the dynamical context of movement production.

Inhibitory effect of voluntary movement preparation on cutaneous heat pain and laser‐evoked potentials

In our study, preparation of voluntary movement was used to physiologically activate the motor cortex areas and the effect of this activation on CO(2) laser-evoked potentials (LEPs) was explored. LEPs were recorded from 31 scalp electrodes in 10 healthy subjects after painful stimulation of the right C6-C7 skin dermatomes. LEP stimuli were delivered in the time interval between a visual warning stimulus followed after 1 s. by an imperative stimulus. The imperative stimulus triggered: (i) no task in the baseline condition (Pain); (ii) flexion-extension movements of the second finger of the right hand in the movement condition (Pain + Movement); (iii) cognitive task (mathematic computation) in the distraction condition (Pain + Cognition). The experimental conditions were also repeated during application of laser stimuli on the left C6-C7 skin dermatomes. Compared with the baseline condition (no task required), during preparation of right-hand voluntary movement there was a significant reduction in LEP amplitude and subjective pain rating after right- but not after left-hand stimulation, which suggests that the observed effect cannot be attributed to a nonspecific reduction in attention toward painful stimulus. During preparation of a cognitive task, LEP amplitude was reduced compared to baseline. Our results represent the first neurophysiological suggestion that physiological activation of the motor cortex, occurring during movement preparation, inhibits cortical pain processing by a centrifugal mechanism.

Involvement of human thalamus in the preparation of self-paced movement.

Cortical areas participating in the preparation of voluntary movements have been studied extensively. There is emerging evidence that subcortical structures, particularly the basal ganglia, also contribute to movement preparation. The thalamus is connected to both the basal ganglia and the cerebellar pathways, but its role in movement preparation has not been studied extensively in humans. We studied seven patients who underwent deep brain stimulation (DBS) electrode implantation in the thalamus for treatment of tremor (six patients) and myoclonus-dystonia (one patient). We recorded from the DBS contacts and scalp simultaneously, while patients performed self-paced wrist extension movements. Post-surgical MRI was used for precise localization of the DBS contacts in six patients. Back-averaging of the scalp recordings showed a slow negative movement-related potential (MRP) in all patients (onset 1846 +/- 189 ms prior to electromyography onset), whereas DBS electrode recordings showed pre-movement MRP in five out of seven patients. The thalamic MRP preceded both contralateral and ipsilateral wrist movements. There was no significant difference between the onset time of thalamic MRP (-2116 +/- 607 ms) and cortical MRP. Neither the scalp nor the thalamus showed pre-movement potentials with passive wrist extensions in two patients. In four patients with postoperative MRI who had thalamic MRP, the maximum amplitude or phase reversal occurred at contacts located in the ventral lateral nucleus. Frequency analysis was performed in the five patients with thalamic MRP. The medial frontocentral scalp contacts and the thalamic contacts with maximum MRP amplitude showed two discrete frequency bands in the alpha (mean peak 9 Hz) and beta (mean peak 17 Hz) range. Both frequency bands showed pre-movement event-related desynchronization (ERD). In the grand average, alpha and beta ERD in the scalp and beta ERD in the thalamus began 2.5-2.8 s prior to the onset of movement. However, the thalamic alpha ERD began considerably later, at 1.2 s before EMG onset. The beta band showed cortico-thalamic coherence from the beginning of the baseline period until approximately 0.5 s before the onset of movement. There was no cortico-thalamic coherence in the alpha band. Our findings suggest that the cerebellar thalamus is involved early in the process of movement preparation. Different cortico-subcortical circuits may mediate alpha and beta oscillations. During movement preparation, the motor thalamus and the supplementary motor area predominantly interact in the beta band.

The effects of probability and relative direction on human somatomotor electroencephalographic rhythms

Classical views on the preparation of voluntary movement involve separate motor and cognitive processes. Recent neurophysiological evidence challenges this dichotomy. Recordings from within the motor cortices have demonstrated that cognitive factors modulate neural activity. This issue was examined in event-related desynchronizations (ERDs) by using a goal-directed pointing paradigm. The ERD, which is typically recorded in response to self-paced finger movements, has been demonstrated to reflect specific movement parameters. Cognitive factors were introduced by manipulating probability and the relative direction of the responses in a choice reaction time task. The results demonstrated that probability influences the 20 Hz ERD.

The Preparation and Execution of Self-Initiated and Externally-Triggered Movement: A Study of Event-Related fMRI

Studies of functional brain imaging in humans and single cell recordings in monkeys have generally shown preferential involvement of the medially located supplementary motor area (SMA) in self-initiated movement and the lateral premotor cortex in externally cued movement. Studies of event-related cortical potentials recorded during movement preparation, however, generally show increased cortical activity prior to self-initiated movements but little activity at early stages prior to movements that are externally cued at unpredictable times. In this study, the spatial location and relative timing of activation for self-initiated and externally triggered movements were examined using rapid event-related functional MRI. Twelve healthy right-handed subjects were imaged while performing a brief finger sequence movement (three rapid alternating button presses: index–middle–index finger) made either in response to an unpredictably timed auditory cue (between 8 to 24 s after the previous movement) or at self-paced irregular intervals. Both movement conditions involved similar strong activation of medial motor areas including the pre-SMA, SMA proper, and rostral cingulate cortex, as well as activation within contralateral primary motor, superior parietal, and insula cortex. Activation within the basal ganglia was found for self-initiated movements only, while externally triggered movements involved additional bilateral activation of primary auditory cortex. Although the level of SMA and cingulate cortex activation did not differ significantly between movement conditions, the timing of the hemodynamic response within the pre-SMA was significantly earlier for self-initiated compared with externally triggered movements. This clearly reflects involvement of the pre-SMA in early processes associated with the preparation for voluntary movement.

The role of the supplementary motor area in externally timed movement: the influence of predictability of movement timing

A significant role in the planning and preparation for voluntary movement has been ascribed to secondary motor areas located on the medial wall of the cerebral hemispheres, and in particular to the supplementary motor area (SMA). Within the SMA, rostral and caudal subdivisions have been described, and differential roles have been attributed to these regions in relation to movement planning, preparation and execution. We have used functional magnetic resonance imaging (fMRI) to investigate the role of the SMA in the timing of movement execution, by recording the fMRI signal from mesial pre-motor areas and primary sensorimotor cortex (SM1) during the execution of a simple motor task externally cued at predictable (regular) and unpredictable (irregular) time intervals. The mean rate of movement was matched in both experiments. There was a greater activation of caudal than rostral SMA with both predictably and unpredictably cued movements, and a doubling of the signal when the timing of the motor response was unpredictable. In contrast, there was no difference in the activation of primary sensorimotor cortex with the two tasks. The data demonstrate that the caudal SMA has an important role in the execution of externally cued movements. The results also suggest a greater role for this region in the performance of unpredictably timed compared with predictably timed movements, however a model is proposed (based on electrophysiological data) which shows how the difference in functional signal in these two situations can be explained on the basis of a difference in the time course of neuronal activation in the SMA, rather than in the overall degree of activation.
