Prevalence and correlates of mental health difficulties following the beirut port explosion: The roles of mentalizing and resilience.

Research has consistently highlighted an increased prevalence of mental health problems, such posttraumatic stress disorder (PTSD), depression, and anxiety, following both man-made and natural disasters. Mentalizing and resilience have been previously identified as potential protective factors against the onset of mental health difficulties following such events. This study first identified the prevalence of PTSD symptoms, depression, anxiety, and stress and subsequently assessed mentalizing abilities and resilience as predictors of PTSD symptomatology in a sample of 521 Lebanese participants following the Beirut Port explosion on August 4, 2020. Findings were consistent with existing literature highlighting elevated rates of PTSD, depression, and anxiety subsequent to man-made disasters, with higher rates of mental health symptoms observed among women, those with a preexisting diagnosis of psychiatric disorder (1.5 times more likely to meet the PTSD Checklist for DSM-5 [PCL-5] cutoff score), and those who had to move houses (over 2 times more likely to meet PCL-5 cutoff) as a consequence of the explosion. Higher mentalizing capacities were positively correlated with higher resilience scores and lower indices of mental health difficulties. Each unit increase in resilience scores was associated with a 3% reduction in meeting PCL-5 cutoff, and poorer mentalizing abilities was associated with a 2-fold increase in the risk of meeting PCL-5 cutoff. Presence of a previous psychiatric diagnosis, having to move houses, lower mentalizing capacities, and lower resilience scores were found to predict elevated PCL-5 scores. TMAO (trimethylamine n-oxide) as a potential biomarker of individual severe stress perception in posttraumatic stress disorder (PTSD)-vulnerable patients after acute myocardial infarction

Abstract Background Acute myocardial infarction is not only a somatic disease but potentially triggers psychological effects, too. Post-traumatic stress disorder (PTSD) is a common stress-related disorder. It is characterized by numerous symptoms, such as flashbacks, intrusions, nightmares and severe anxiety, as well as uncontrollable, intense and disturbing thoughts and feelings related to the traumatic experience. However, with regard to the development of PTSD, individual stress perception might be crucial since not every serious traumatic experience leads to PTSD. To date, almost no biological correlates of an individual's perception of stress have been identified as being associated with the long-term development of PTSD. Objective The aim of the study was to determine whether blood levels of TMAO vary immediately after AMI (1) in patients with or without depression, and (2) in patients with AMI induced PTSD symptomatology (subsyndromal PTSD and full PTSD). Furthermore, we investigated whether TMAO is a potential biomarker that might be useful in the prediction of PTSD symptomatology in the long term. Method A total of 114 AMI patients were assessed with standardized clinical psychiatric interviews based on the Hamilton Depression Scale (HAMD-17) after admission to the hospital and 6 months later. In addition, the CAPS-5 was used to explore PTSD symptoms (subsyndromal PTSD and full PTSD) 6 months after AMI. To assess patients' TMAO status, serum samples were collected at hospitalization and 6 months after AMI. Results Study participants with post-myocardial infarction PTSD symptomatology (subsyndromal PTSD and full PTSD) had significantly higher TMAO levels immediately after AMI than patients without PTSD symptoms (ANCOVA: TMAO (PTSD x time), F = 4.544, df = 1, p=0.035). In contrast, depressive symptomatology 6 months after AMI had no influence on TMAO levels (TMAO (depression x time), F = 0.083, df = 1, p=0.774). With the inclusion of additional clinical predictors in a hierarchical logistic regression model, TMAO becomes a significant predictor of PTSD symptomatology. Conclusions An elevated TMAO level immediately after AMI might reflect severe stress in PTSD-vulnerable patients, which might also lead to a short-term increased gut permeability to trimethylamine (TMA), the precursor of TMAO. Thus, elevated TMAO might be a biological correlate for stress that is associated with vulnerability to PTSD and might help to identify patients at increased risk. Cancer-and-treatment-specific distress and its impact on posttraumatic stress in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT).

In this prospective multicenter study, we investigated cancer-and-treatment-specific distress (CTXD) and its impact on symptoms of posttraumatic stress disorder (PTSD) in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). Patients were consulted before (T0, N=239), 3 (T1, N=150), and 12months (T2, N=102) after HSCT. Medical (eg, diagnosis and pretreatment) and demographic information, CTXD and PTSD (PCL-C) were assessed. Random intercept models revealed that the sum score of CTXD was highest pre-HSCT (T0), decreased by T1 (γ=-.18, 95% CI [-.26/-.09]), and by T2 (γ=-.10, 95% CI [-.20/-.00]). Uncertainty, family strain, and health burden were rated most distressing during HSCT. Uncertainty and family strain decreased from T0 to T1 (γ=-.30, 95% CI [-.42/-.17]; γ=-.10, 95% CI [-.20/-.00]) and health burden from T1 to T2 (γ=-.21, 95% CI [-.36/.05]). Women were more likely to report uncertainty (γ=.38, 95% CI [.19/.58]), family strain (γ=.38, 95% CI [.19/.58]), and concerns regarding appearance and sexuality (γ=.31, 95% CI [.14/.47]) than men. Uncertainty (γ=.18, 95% CI [.12/.24]), appearance and sexuality (γ=.09, 95% CI [.01/.16]), and health burden (γ=.21, 95% CI [.14/.27]) emerged as predictors of PTSD symptomatology across the 3 assessment points. Our data provide first evidence regarding the course of 6 dimensions of CTXD during HSCT and their impact on PTSD symptomatology. Specifically, results emphasize the major burden of uncertainty pre-HSCT and the impact of uncertainty and concerns regarding appearance and sexuality on PTSD symptomatology.

Exposure to Interpersonal Violence as a Predictor of PTSD Symptomatology in Domestic Violence Survivors

This study examines the interrelationships between childhood abuse, exposure to maternal domestic violence, and posttraumatic stress disorder (PTSD) symptomatology in a multiethnic sample of 111 adult female residents of a domestic violence (DV) shelter. Participants completed structured interviews about the DV and their prior violence exposure, as well as the Impact of Event Scale-Revised. As hypothesized, there was high co-occurrence between exposure to maternal DV and childhood physical and sexual abuse, and the frequency of lifetime violence exposure predicted PTSD symptomatology. A series of multiple regressions indicated a more complex pattern of relationships, in which specific forms of prior violence exposure predicted different PTSD symptom dimensions. A history of witnessing maternal DV predicted intrusion symptoms, and a history of childhood sexual abuse predicted hyperarousal symptoms. Ethnicity was not related to levels of violence exposure or to PTSD symptoms. Clinical implications of the findings are discussed.

Psychological Abuse and Posttraumatic Stress Disorder in Battered Women: Examining the Roles of Shame and Guilt

Psychological abuse among battered women has been relatively understudied. However, battered women's reports in the existing qualitative and quantitative research suggest that the effects of psychological abuse can be even more damaging than the effects of physical abuse. The current study attempted to clarify the relationship between psychological abuse and posttraumatic stress disorder (PTSD) within a sample of battered women by statistically controlling for the effects of physical abuse. This study also explored the affective experiences of shame and guilt as important variables in the development of PTSD in battered women. This investigation replicated previous work suggesting that battered women are very much at risk for a diagnosis of PTSD and suggests that clinicians and researchers may need to focus on psychological abuse as a predictor of PTSD symptomatology. The current findings encourage attention to shame reactions in battered women and suggest new directions in the study of PTSD for other traumatized populations.

Protective Factors and Posttraumatic Stress Disorder in Veterans With Spinal Cord Injury

This study examined the effects of putative protective factors, including current family structure (marital status and children), level of education, and perceived social support, upon posttraumatic stress disorder (PTSD) symptomatology in veterans with spinal cord injury (SCI). Participants were 124 veterans with traumatic spinal cord injuries, recruited from SCI services at Veterans Affairs Medical Centers in the Bronx, NY, Castle Point, NY, and East Orange, NJ. We found that the most consistent predictor of PTSD symptomatology was perceived social support from friends. In both univariate and multivariate analyses, it predicted current level of PTSD symptoms. Level of education was also a significant predictor of current PTSD symptoms in multivariate analyses. In contrast to the significant findings, perceived social support from family, marital status, and having children did not significantly correlate with the PTSD variables.

Sexual Harassment and Assault as Predictors of PTSD Symptomatology Among U.S. Female Persian Gulf War Military Personnel

Rates and sequelae of sexual harassment and assault among women in a wartime military sample were examined. A second goal was to explore the comparative impacts of these stressors and combat exposure on posttraumatic stress disorder (PTSD) symptomatology. Army women (n=160) were interviewed on return from the Persian Gulf War and again 18 to 24 months later. Rates of sexual assault (7.3%), physical sexual harassment (33.1%), and verbal sexual harassment (66.2%) were higher than those typically found in civilian and peacetime military samples. Sexual assault had a larger impact on PTSD symptomatology than combat exposure. Frequency of physical sexual harassment was significantly predictive of PTSD symptomatology. Furthermore, the number of postwar stressful life events mediated the relationship between physical sexual harassment and symptomatology but was not related to combat exposure. Sexual assault, sexual harassment, and combat exposure appear to be qualitatively different stressors for women, with different correlates and mechanisms of action.
